There are genetic risks associated with MS, there are demographic risks of experiencing a more severe disease course, and there are also behavioral and lifestyle risks that will increase the chance that someone will be diagnosed with MS. Joining me today to discuss behavioral and lifestyle MS risk factors and how they can be mitigated are Doctor Jared Bruce and Dr. Amanda Bruce. Dr. Jared Bruce is a neuropsychologist and the director of psychiatric research at the University of Missouri-Kansas City School of Medicine. Dr. Amanda Bruce is a clinical psychologist and an assistant professor in the Department of Behavioral Pediatrics at the University of Kansas Medical Center. We're also reviewing the details of the Inflation Reduction Act, which means, beginning in 2025, significantly lower out-of-pocket costs for prescription medications for the quarter-million people with MS who are on Medicare.  We'll tell you about study results for a novel drug that was effective in remyelination and restoring lost function to mice with the mouse model of MS. We'll share the details of a study that shows how specific comorbidities affect specific MS outcomes. And while it touches on many different areas, it will be remembered as the most consequential healthcare bill since the Affordable Care Act. From the emails I've received, I know that many of you are wondering how and when this new law will impact what you pay for your prescription medications. Joining me to talk about some of the twists and turns that can pop up on the road to being diagnosed with MS is Dr. Michael Robers, a neurologist in the Multiple Sclerosis Program and an assistant professor in the Department of Neurology at Barrow Neurological Institute. The first few minutes of this conversation is for a lay audience, as Elham Azizi, PhD, and Charly Good, PhD, explained how they're investigating what causes cancer to grow and spread and how to improve immunotherapy. Then the discussion moved toward a scientific audience, as Drs. Azizi and Good shared recent findings and asked probing questions about future directions and opportunities in cancer research. Elham Azizi, PhD, is a former American Cancer Society postdoctoral fellow who is now an Assistant Professor of Biomedical Engineering at Columbia University. Elephants May Help Us PREVENT Cancer Making Plant-based Meats Healthier Genetically Enhanced Heart Attack Therapy Smart Device Monitoring Of Your Psyche Antibiotics Could Treat Cancer   Elephants are one-third as likely to die from cancer even though they have 100 times as many potentially cancerous cells.  Geneticists at the University of Chicago have unraveled the mystery by their discovery that elephants possess a gene that triggers a cytokine called LIF6 to destroy cancer cells. We humans have one copy of the tumor suppressor gene p53, versus 20 copies in elephants, and we also have a version of the LIF cytokine.  Work is underway to harness for humans the gene-driven cancer preventive that works so well in elephants.   Plant-based meat and fish substitutes are all the rage, but they are not healthier than the real thing due to their abundance of saturated fat, salt, and sugar.  Food scientists at UMass Amherst are reengineering products like Impossible Meat and Beyond Meat to include key micronutrients including Vitamin D, calcium, and zinc while omitting the harmful ingredients.  These refined substitutes will be the next generation of plant-based meat and fish, and when they arrive, you'll be the first to know.   Successful therapy after heart attacks involves reestablishing blood flow by chemically or surgically eliminating the clots that prevent coronary blood flow. The problem is that enzymes in the damaged tissue prevent successful recovery. How Humboldt Squid Talk To Each Other In The Dark Cephalopods are masters of changing their bodies in response to their environments—from camouflaging to sending warning signals to predators. The art of their visual deception lies deep within their skin. They can change their skin to different colors, textures, and patterns to communicate with other animals and each other. But how does this play out in the darkness of the deep ocean? That’s the question a team of scientists studied in the deep diving Humboldt squid that lives over 2,000 feet beneath the ocean’s surface. Their results were published this week in the journal Proceedings of the National Academy of Sciences. Biologist Benjamin Burford, who is an author on that study, explains how Humboldt squid use a combination of skin color patterns and bioluminescence to send each other signals and what this might teach us about communication in the deep ocean. See a video and more photos of Humboldt squid communicating with each other from Monterey Bay Aquarium Research Institute.  Mapping The Microbiome Of Your Tongue Your mouth is home to billions of bacteria—some prefer to live on the inside of the cheeks, while others prefer the teeth, some the gums, or the surface of the tongue. Writing this week in the journal Cell Reports, researchers describe their efforts to map out the various communities of bacteria that inhabit the tongue.  In the average mouth, around two dozen different types of bacteria form tiny “microbial skyscrapers” on your tongue’s surface, clustered around a central core made up of individual human skin cells. The researchers are mapping out the locations of the tiny bacterial colonies within those skyscrapers, to try to get a better understanding of the relationships and interdependencies between each colony.  Jessica Mark Welch, one of the authors of the report and an associate scientist at the Marine Biological Laboratory in Woods Hole, Massachusetts, talks about what we know about the microbiome of the human mouth, and what researchers would still like to learn. Rethinking Invasive Species With Pablo Escobar’s Hippos Colombia is home to an estimated 80 to 100 hippos where they’re an invasive species—hippos are native to Africa. But notorious drug lord Pablo Escobar brought four to the country as part of his private zoo. After his death in 1993, the hippos escaped to the wild where they thrived.  Some locals consider them pests, the government has mulled over getting rid of them, and recent studies have shown that their large amounts of waste is changing the aquatic ecology of Colombia. But new research has taken a different view, showing that even though hippos are invasive, they might be filling an ecological hole left by large herbivores killed off by humans thousands of years ago. Erick Lundgren, the study’s lead author and a Ph.D. student at the University of Technology in Sydney, Australia, talks about why we should stop thinking of the phrase “invasive species” as inherently bad, and what may be in store for the future of these hippos.

Fasting is the abstinence or reduction from some or all food, drink, or both, for a period of time. An absolute fast or dry fasting is normally defined as abstinence from all food and liquid for a defined period. See our previous episode, Fasting: Healthy or Harmful? – Episode 40, on fasting for more information.   Jonathan’s Experience with Fasting Tried different fasts. NOMAD – No Meals A Day OMAD – One Meal A Day Extended water fasts Intermittent fasting Initially motivated by wanting to lose weight. Didn’t initially know all the benefits of fasting. First extended water fast was early 2019. “I ran the most miles I’d ever ran in a week the same time that I did that water fast, and kind of pushed my body to some new limits.”   Words of Caution Women may want to start with short fasts as extended fasting may wreak havoc on female hormones. Listen to your body. If you have health issues, or you’re concerned about fasting or what it will do to your body, consult your physician. Each person is unique, and you need to do what works best for you, and that’s going to look a little different from the person sitting next to you, or maybe it will look a lot different than the person sitting next to you. You really have to get in tune with your own body. Women who are pregnant or people who have diabetes, hypertension or other serious medical conditions, should be very cautious and careful when considering a fast. Always consult with your doctor or your health practitioner first.   Fasting Is Also Psychological/Mental Psychologically, I’m hungry. Psychologically, I need food. Psychologically, you’re getting “hangry.” Consider just drinking some water. It may will fix all of that. It’s being present in your mind.   Benefits of Fasting Reset the internal clocks. We have clocks in our bodies, and originally, we knew of the circadian rhythm clock that controls our wake/sleep cycles. Each of the trillions of cells in our bodies have a tiny internal clock as well. Fortunately, there may be a simple way to reset those clocks if they get off rhythm: fasting.  Fasting increases gene expression for longevity and health span. Research has found that short bouts of fasting increased lifespan up to 100% across multiple organisms (bacteria, yeast, worms, mice).  Fasting flips a lot of switches, and this can affect aging and longevity pretty drastically It’s an efficient way to lose weight. Research has found that in almost every organism, short bouts of fasting increases the life span. Studies show restricting calories increases life span. We have an insulin response every time we eat. As that happens, it does slow down our base metabolic rate over time. It’s a great way to lower insulin. * Diabetics should consult their physician before fasting. If there’s no food coming in, there is no need for insulin to rise. Insulin will stay at its low, steady basal rate, and there will be no elevations or spikes. Purges cancerous and pre-cancerous cells. Cancer cells get their energy from lactic acid, essentially from a breakdown of carbohydrate into glucose into pyruvate into lactic acid, also known as sugar. They are starving little monsters that need to eat and divide constantly. If you take away their food supply, they die. It promotes autophagy. Definition: the cellular self-cleansing process that breaks down and recycles damaged molecules and cellular organelles During the fasted state, as insulin drops, autophagy increases dramatically, perhaps 5-fold. A high rate of autophagy is characteristic of young organisms; with aging, autophagy decreases, and this allows cellular damage to accumulate. By fasting intermittently, autophagy rates can be reset to that of a younger person. The decline of autophagy may be an important factor in the rise of cancer, Alzheimer’s disease and other disorders that become common in old age. Unable to clear away the cellular garbage, our bodies start to fail.   TeriAnn’s Advice “To complement fasting, if you’re going to make this part of your way of life and your health journey, do yourself a favor and eat healthy food, too. That’s really what’s going to get you the best bang for your buck, is the fasting coupled with living a really healthy lifestyle.”   Types of Fasts Intermittent fasting 18/6 – fast for 18 hours per day, feed for 6 hours per day 16/8 – fast for 16 hours per day, feed for 8 hours per day If you do intermittent fasting, you do a 16/8 or 20/4, something like that, plan your one meal to be with your family. Make it easy. A lot of research shows that you should stop eating a significant amount of time before you go to bed, and then let your body go all night without food. You may just choose to do that one week every month. You may choose to do it every other day. Dinner-to-dinner fast You don’t eat Saturday night—or you eat dinner Saturday night and you don’t eat again until Sunday night, and you do that once a month. Dry fasting With dry fasting, we start going a little down the path that may not be the place to go all by yourself without some sort of supervision. Fasting-mimicking diet A five-day fast, where it’s a calorie restriction. I think day one, you have about 700-800 calories, and then day two through five, you keep it under 500 calories. I think it’s safer for women in terms of hormones staying balanced and staying in check, and things like that.   Jonathan’s Advice Do it. Try it. See how your body responds. Get your electrolytes in. Make it easy. A lot of research shows that you should stop eating a significant amount of time before you go to bed, and then let your body go all night without food. You may just choose to do that one week every month. You may choose to do it every other day. Dinner-to-dinner fast You don’t eat Saturday night—or you eat dinner Saturday night and you don’t eat again until Sunday night, and you do that once a month. Dry fasting With dry fasting, we start going a little down the path that may not be the place to go all by yourself without some sort of supervision. Fasting-mimicking diet A five-day fast, where it’s a calorie restriction. I think day one, you have about 700-800 calories, and then day two through five, you keep it under 500 calories. Can I, can I just ask you before we get into the nitty gritty, in your view, have you seen the amount of research and sleep science increase over the years? 00:44 Neil Stanley: No, absolutely. I mean I've been in this game for 37 years now and when I started one journal published papers about sleep, which was called Sleep, and there was sort of pretty much one textbook which was The Principles and Practice of Sleep Medicine. And now there's an absolute deluge of sleep information. I mean, there's tens of sleep journals, but the other thing that perhaps has changed is that papers pertaining to sleep aer published pretty much across the board. And this is, this is of course a problem to try and keep on top of the field because, you know, whilst I am subscribing to the sleep journals, you can't subscribe to every journal. And so important information is coming out in an obscure to me as a sleep expert, obscure journals. And so you really do have to try and keep abreast of the information. 3:35 And so say 37 years ago, maybe a hundred hundred and 50 papers the year were published in sleep. Now it seems like there's 100 a week being published. And what's interesting is, although there's all this information is actually making the field more complex rather than clearer, which I think the idea was to try and find out, you know, why we sleep, how we sleep when we sleep etc. But all we're doing is making it vastly more difficult to actually find the answer. And that's one of the great challenges. And one of the reasons why the field is so very, very interesting.But I don't think we're any better understanding.
