Volgens meerdere insiders draait de kern van de film om een conflict tussen Doctor Doom en Steve Rogers. Doom zou Captain America verantwoordelijk houden voor een multiverse-incursion, veroorzaakt door zijn beslissing om na Endgame in het verleden te blijven bij Peggy Carter.
