Conventional medical school and residency curricula glossed over nutrition for patients and certainly made no mention of how to look after oneself on shift.Eric and Karine discuss the roles that caffeine, hydration and nutrition play when working in emergency medicine. They look at some good and not-so-great strategies and provide some advice for shift performance enhancement.Coffee:Caffeine is the most widely consumed CNS stimulant, and coffee is the world's 3rd most popular beverage. If you drink coffee, you can probably do stupid things faster and with more energy but ... Seriously, caffeine has beneficial effects on reaction time, vigilance, and attention, but there is less to suggest that it improves decision making and problem solving.Caffeine is an ADENOSINE blocker. It counteracts the CNS depressant activity of adenosine.2 to 3 cups are likely the ceiling of beneficial effect. Restrict caffeine the to the first half of your shift.Try a coffee nap: have a cup or a shot some time prior to your shift, IMMEDIATELY lie down and rest/sleep/doze for 30 minutes. When you wake, the caffeine will be starting to have effect.Hydration:Dehydration of 2% can affect decision making and cognitive performance.1 to 1.5 litres of water over a shift is about right.Caffeine does NOT dehydrate you.PEE before you put on your PPE (prior to a procedure, critical care etc.)Urine colour is a reliable marker of hydration status, so if you are yellow, have a glass!Nutrition:Shift working (especially night shift working) affects circadian rhythms which has detrimental metabolic effects.Night workers tend to choose sugary snacks, often out of convenience. Sleep deprivation makes you choose calorie dense carbs and salty snacks. Aim for euglycemia, so avoid high glycemic foods (candy, donuts, fruit juice). Stick to balanced meals with roughage and high protein sources. Snack on fruits, vegetables, yoghurt and nuts.Avoid where possible processed foods.Eating at end of shift before sleeping? On one hand, you are less likely to wake up ravenous, but some data suggests that the increased body temperature generated after eating can impair sleep.Eric and Karine were unable to agree on the concept of comfort food at 0200 hrs being beneficial. You must decide whether a handful of jujubes will prevail over 4 carrot sticks.
Today you'll hear my interview with JooWan Kim, Composer/Artistic Director of Ensemble Mik Nawooj. You're going to want to check this ensemble out at their website - I can't tear myself away from their songs and videos. Led by composer/pianist, JooWan Kim, Hip Hop Orchestra Ensemble Mik Nawooj (EMN) creates Metamusic by sampling principles of Hip-Hop and Classical. Executed with resident MCs, a lyric soprano, woodwinds, French horn, strings, piano, and drums, the music is rigorous, nuanced, accessible, and free from the dogmas of Western European concert music aesthetic. The result is seamless tête-à-tête with "textures swimming through the sound...like the world's fastest ping-pong game" (Pitchfork) and is considered the "cutting edge of hip-hop" (Huffington Post). After a successful performance of a novelty piece which featured an MC and chamber ensemble while JooWan Kim was completing his master's degree at the San Francisco Conservatory of Music, the MC at the time suggested they make an album together. Six months later and writing about an hour of music, Kim had a profound shift in the direction of his writing. He felt that he found a way out of the stifling contemporary concert music aesthetic in this new way of composition. JooWan crystallized his ideas into Method Sampling, a principle of borrowing or sampling of rationales from related as well as unrelated fields, then reframing them into one's own system. In 2010, JooWan Kim recruited his best friend from college, Christopher Nicholas, to push the project forward in a serious way. Since then, EMN attracted some of the most excellent classical musicians and MCs in the SF Bay Area while gaining national attention from outlets such as ESPN, Wall Street Journal, Huffington Post, NPR, Pitchfork, Upworthy, NowThis, and more. A native of North Louisiana, Ever More Nest's Kelcy Wilburn (“Kelcy Mae”) is as equally influenced by the gospel, country, and blues of her Bible Belt hometown as she is by the emotional rawness of the artists that consumed her generation: Cranberries, Counting Crows, Tori Amos, Radiohead, et al. At 18, she moved to New Orleans, where open-mindedness and acceptance gave her the freedom to be herself and to find her voice. As a student of creative writing, she fostered a love of language evident across her early releases as Kelcy Mae and across Ever More Nest's debut and sophomore albums. Three times Nobel Peace Prize nominated peace activist Kathy Kelly and anti war veteran and author Danny Sjursen discuss Afghanistan, what the war was about, what the war was not about, why Biden's statements on the terrorist "threats" in other countries & on the Kabul blast were troubling. And they debunk the disingenuous media narrative about women's rights and human rights. Kathy Kelly, a peace activist and author, made over two dozen trips to Afghanistan from 2010 – 2019, living with Afghan Peace Volunteers in a working class neighborhood in Kabul. She and her companions in various peace team delegations believe that where you stand determines what you see. Kelly lived with families in Baghdad throughout the 2003 Shock and Awe invasion and during the first weeks of the U.S. occupation. Kelly is now campaigning for an international treaty to ban weaponized drones. What are the most common disciplinary issues among physicians who practice emergency care? How are these issues handled, and how can YOU avoid getting into trouble? Bruce Campana walks us through the art of disciplining physicians. Bruce is currently the Medical Director of Enhanced Medical Staff Support (EMSS) at Vancouver Island Health Authority. EMSS supports medical leaders who are asked to attend to issues of medical staff professionalism. Bruce also describes his early career experience working under legends in emergency medicine, like the Dr. Rosen. From there, his career path led him to Saudi Arabia, where he was the Saudi King's private physician and hacked into a plane's oxygen supply to treat the Amir of Kuwait while in the air. His storied career led him to his current position at EMSS, where he emphasizes an empathic approach to bullying, challenging relationships, and mental health issues, among other disciplinary problems faced by emergency physicians. Bruce is an attending emergency physician at Victoria General Hospital, Clinical Associate Professor at the UBC Dept of Emergency Medicine, Medical Director for Enhanced Medical Staff Support for the Vancouver Island Health Authority. Interview with Marco Romero, President & CEO of Euro Manganese Inc. (TSXV/ASX: EMN)Euro Manganese is a Czech Republic-based Manganese tailings/recovery story. Euro Manganese is working on a Feasibility Study. It has already done a PEA, and is in the late stages of its EIA. It is also filing for permits and preparing to build a demonstration plant.Euro Manganese has a share price of CAD$0.15, a market cap of CAD$25M and 176M shares outstanding.Romero regards Manganese as the forgotten battery metal. He says every Western EV brand needs Manganese. Manganese is predominantly intended for NMC cathodes in lithium-ion batteries. Euro Manganese offers investors access to Manganese, in the form of Manganese product production from tailings.Euro Manganese has a single asset: the Chvaletice Manganese Project. Romero regards it as Europe’s largest manganese deposit by far, and a "globally significant deposit." The resource has been 95% quantified over the last 4.5 years, and this has been aided by the mineralogy of the material: very fine particles that are well homogenised and distributed throughout the deposit laterally.A pilot plant was built as part of the PEA, bur Euro Manganese is now ready to construct its demonstration plant. It needs to source appropriate funding of US$4M, and US$1M per annum for operational purposes. The demonstration plant is designed to produce bulk, multi-ton samples of manganese products for companies to trial. This will prove Euro Manganese's processing flowsheet is effective, which Romero is confident about, given all technology is completely conventional. If they like the manganese products, a CAPEX of US$400M + operating costs has been suggested for a main plant, designed to produce 50,000t of high purity “pharmaceutical” grade manganese products for Chvaletice’s life of resource: c. 20 years.There are some problems in the way of progress for Romero. Euro Manganese only has US$4.2M on the books as of Q3/19, and will need to raise cash soon to get things off the ground. While the company has signed some contracts an extremely large Chinese international mining company investors will have questions regarding the 'pay-as-you-go nature of Euro Manganese's supply. There is an element of uncertainty as to how much companies will buy, and if they will come back for more. Investors will want to see supply contracts soon.Romero didn't state Euro Manganese will go to market to raise capital, but we expect this to happen, leading to dilution for shareholders. Will this be the right strategy in the long run?In terms of timings, investors will also have some major questions. Our educated assumption is that the Demonstration Plant will take c.1 year to construct, followed by a trial period for the Manganese products. Then the Main Plant will need to be constructed and made operational. This could be 4-5 years before EMN actually starts to demonstrate its potential, and there are a huge number of obstacles in the way as far as raising capital, permitting and supply contracts. This story looks like a slow-burner, but one that could be rewarding in the long term. Welcome to Episode #19 of the Value Investor Podcast Every week, Zacks value stock strategist and the Editor of Zacks Value Investor portfolio service, Tracey Ryniec, talks about all things happening in the value stock universe, including her top stock picks. But with all the major stock indexes hitting new all-time highs, Tracey wonders if there are any value stocks left to invest in out there. And if there are, how do you find them? One thing to keep in mind is that the P/E, which is one of the fundamental criteria that value investors use to find their stocks, is about more than just the “P”, or Price. There is also the “E” or earnings. With the US economy improving, the E part of the equation should also be improving. This week, RV and motorhome maker Thor Industries (THO) reported record earnings. In its guidance, it said it sees 2017 as the best year the industry has ever seen in 40 years. Thor’s shares have been soaring all year. It’s great stock performance hasn’t been just a post-election move. You might think, with the shares making that big move, that Thor was probably an expensive stock. Tracey thought it must be but when she looked at its P/E ratio, she was surprised to see that it was trading with a forward P/E of just 15.5. How can it be so cheap? While the share price has been soaring, the earnings have been adjusted higher as well. How Do Value Investors Find Cheap Stocks During a Bull Rally? Tracey still runs her screens, but as stock soar, you may need to lower the number of fundamental parameters that you include. For example, Tracey ran a “classic value” screen that included P/E, P/S, P/B, P/CF, PEG and a volume over 100,000. She also added a Zacks Rank of #1 (Strong Buy) or #2 (Buy). She only got 7 stocks. Yikes. But when she adjusted the parameters to fewer criteria, and added a dividend over 2%, she got 68 stocks. That’s a much bigger selection. Here’s a selection of some of the stocks in that screen that caught her eye. Value Stocks with Dividend Yields Over 2% 1. Banco de Chile (BCH): Be cautious on companies in the emerging market but BCH has a 3.4% yield. 2. Fifth Third Bank (FITB): a major regional bank. It has a 2.05% yield. 3. Eastman Chemical (EMN): forward P/E of 10.9 and a 2.5% yield. The chemical companies are the building blocks of the manufacturing economy. 4. Kohls (KSS): the retailers are out of favor but Kohls has a forward P/E of 13.8 and a juicy dividend yielding 3.7%.
