We have learned a lot about extended spectrum coverage of prophylactic antibiotics for cesarean section. The landmark C/SOAP trial randomized 2,013 women undergoing nonelective cesarean delivery to azithromycin 500 mg IV plus standard prophylaxis versus placebo, demonstrating a 51% reduction in the composite outcome of endometritis, wound infection, or other infection. Adjuvant Zmax (plus standard first-generation cephalosporin) is now recognized as evidence-based antibiotic coverage for intrapartum cesarean, cesarean with ruptured membranes, and patients with obesity. This last patient characteristic comes from the ERAS latest update. But what is ZMAX is not available? Is there an evidence-based peri-op alternative in these cases? It's (somehow!) six years ago that we started to learn about this new virus and then find ourselves caught up in a global pandemic. While there were some truly tragic parts to that crisis, it turns out there were also some good results, too. Quite by chance recently, two guests in the space of a couple of days told me stories about huge changes in their lives brought about by their travels around Covid times, and these stories were so interesting I quickly decided to create a whole episode on the topic. First up, I chat with Shannon O'Brien, an international school teacher who was actually working at a school in Shenzhen, China, at the start of 2020. During a school break for the Chinese New Year, Shannon flew to Sumatra, Indonesia, for a short holiday. Spoiler alert: it became a very long holiday! I then speak with Eva Westerling, a German doctor who in 2019 had decided that it was time for a big change and was contemplating a permanent shift to Morocco. When the pandemic hit, she and her partner were in the earliest stages of setting up a tourism business in Morocco, and then of course, no tourists came. My final guest is Eryn Gordon, who was working a corporate job in the United States when the pandemic began, and she soon found herself out of work. Instead of laying low like many of us did during Covid times, Eryn instead decided to get a new qualification and move to the other side of the world, to work in Seoul, South Korea. Animal cruelty is equal parts disturbing, gross, and downright maddening — and tonight's story is a bizarre saga in Ontario's horse world. Stephanie Redlick is a North York/Toronto-area resident who is reportedly connected to Above the Stars Equine Rescue, and a figure many in the equine community have been warning about for months. Redlick has been charged with 13 offences under Ontario's Provincial Animal Welfare Services Act, and police have also laid criminal charges alleging fraud and theft connected to a livestock matter. There are serious claims being made by critics and horse advocates — allegations that horses under Redlick's stewardship have been kept in appalling conditions, with suffering and death as the outcome. Tonight on The Ezra Levant Show, Ezra is in Davos, Switzerland for the annual gathering of the global elite at the World Economic Forum — where Canada's most powerful figures feel safest from scrutiny. Ezra reacts to Mark Carney's speech and then goes behind the scenes on his encounter with Justin Trudeau. And yet, the Davos crowd loved it. Why? Because Carney is exactly what this conference rewards: a globe-trotting technocrat with multiple passports, reflexive hostility toward Donald Trump, and an instinctive sympathy for global governance over national sovereignty.
