Parkinson's disease (PD) is a neurodegenerative disorder that primarily affects movement. The disease gradually progresses over time and is characterised by a variety of motor and non-motor symptoms, such as tremors, changes in walking patterns, stiffness, as well as difficulties with balance and speech. At present, there is no known cure for Parkinson's Disease (PD). My Moves Matter, the company behind this new app was co-founded by registered dietitian Richelle Flanagan after she noticed something was wrong with her handwriting when trying to document a note in a patient's chart. She was diagnosed with Young Onset Parkinson's Disease (YOPD) shortly after the birth of her daughter. New app to track symptoms of Parkinson's Disease In response, this Irish female founder and patient innovator co -founded My Moves Matters and developed an app to help women manage their PD symptoms in line with their menstrual cycles. The app is proving a success and currently has over 1600 users from 47 countries. My Moves Matters aims to improve the lives of Irish women living with PD and was one of the winners of Health Innovation Hub Ireland's Femtech competition in 2023. A medical paper entitled 'Unmet Needs of Women Living with Parkinson's Disease: Gaps and Controversies', co-authored by Richelle Flanagan, highlights the issues women with the PD experience, especially from a psychosocial standpoint, which have been overlooked. There is a need for management of PD which considers the impact of the hormonal stages women experience, including menstrual cycles. Current clinical guidelines fail to address the nuanced impact of hormonal changes on PD symptoms adequately, resulting in fragmented care for women. The My Moves Matter app has come up with a solution and seeks to bridge this gap by enabling detailed patient-reported outcome tracking. This initiative not only aims to gather specific data to understand the link between hormone fluctuations and PD but also to inform the development of personalised patient care programmes, including medical nutrition therapy tailored to individual needs. The My Moves Matter app is unique as it puts people with Parkinson's disease in control of understanding their own body and mind's reaction to PD. The app helps people to understand their individual symptoms, what triggers them and what works or does not work for their individual experience. This patient reported symptom tracker has several notable features: medication reminders tablets, patterns identification, hormonal impact tracking, gain knowledge, share decision making and help find answers. The results of the clinical evaluation study with Health Innovation Hub Ireland were impressive. Clinicians recognised the main benefits of the My Move Matter app which included the ability to view detailed data between clinic visits and to accurately track medication user-friendliness for patients and staff. The clinicians, PD Neurologists and nurses, stressed the importance of tracking hormonal impacts in women with PD as it is currently underserved and they highlighted this aspect as an excellent feature of the app. Knowing detailed information such as medication timing, doses, mealtimes, sleep patterns and bowel movement patterns was also considered critical for comprehensive patient management. Phil Gee was first diagnosed with Parkinson's Disease (PD) in 2015, one year after retiring from a very successful thirty-three-year career holding a number of engineering, manufacturing and supply chain senior level positions. Since his PD diagnosis, Phil and his wife, Julia (who first met in the 1st grade & have been married for 36 years), have become active advocates for others living with PD and their care partners. In 2017, they moved from metro Atlanta to Hilton Head, and immediately joined the local Rock Steady Boxing program, an intensive boxing/exercise program that has proven to slow the progression of Parkinson's Disease, which is incurable.They began to look throughout the South Carolina Lowcountry and didn't really see other Black people who said that they had PD. That started their journey to become PD advocates, helping others with PD and their care partners to learn more about PD and how to live a better life with the disease. Phil has served four years on the Parkinson's Foundation People with Parkinson's Advisory Council. Julia is currently on the Advisory Board for the Foundation's Carolina Chapter, and chairs their local PD Support & PD Care Partner Support groups. Together they have participated as panelists and speakers in a number of PD symposiums, co-chaired multiple Parkinson's Foundation Moving Day Fundraisers, and participated in several pharmaceutical focus groups/patient panels. Today, they are active with the Special Interest Group – Black Diaspora steering committee working to drive development of solutions to mitigate health disparities in the Black Community with a focus on issues of the Black PD Community.Evelyn Stevens, MPH, is the Senior Director of Community Engagement at the Parkinson's Foundation since September 2022. Prior to this role, Evelyn held positions as Clinical Research Project Manager, Clinical Research Coordinator III, and Clinical Research Coordinator II at the Children's Hospital of Philadelphia in the Division of Hematology and Comprehensive Sickle Cell Center. Before that, Evelyn worked as a Health Educator and Clinical Research Coordinator at Penn Medicine's Raymond and Ruth Perelman Center for Advanced Medicine in the Abramson Cancer Center. Evelyn holds a Master of Public Health (M.P.H.) in Epidemiology and Biostatistics from Temple University and a Bachelor of Arts in Psychology: Bio-Psychosocial Health from Lincoln University.

Parkinson's is the second most common neurological disease after Alzheimer's and affects about one percent of people over sixty. The symptoms develop slowly. These include rigidity, a characteristic resting tremor, and movement dysfunction with walking problems. As it progresses, problems with sleep, thinking, and eventually dementia may occur. Survival after diagnosis is seven to fifteen years. Like many neurological problems, PD is a syndrome with recognized clinical characteristics rather than a clearly defined disease with definitive lab tests and treatments. Genetics have been weakly implicated, but cannot be the primary cause because the incidence is skyrocketing. Parkinson's (PD), Autism, Alzheimer's (AD), Multiple Sclerosis (MS), and Amyotrophic Lateral Sclerosis (ALS or Lou Gehrig's disease) are likely caused by toxins. Ken Stoller lays out some of the evidence implicating glyphosate in Incurable Me: Why the Best Medical Research Does Not Make It into Clinical Practice (2015). US government databases were searched for… crop data, glyphosate application data and disease epidemiological data. Correlation analyses were then performed on a total of 22 diseases… Correlation coefficients are highly significant (< 10–5) between glyphosate applications and hypertension (R = 0.923), stroke (R = 0.925), diabetes prevalence (R = 0.971), diabetes incidence (R = 0.935), obesity (R = 0.962), lipoprotein metabolism disorder (R = 0.973), Alzheimer's (R = 0.917), senile dementia (R = 0.994), Parkinson's (R = 0.875), multiple sclerosis (R = 0.828), autism (R = 0.989), inflammatory bowel disease (R = 0.938), intestinal infections (R = 0.974), end stage renal disease (R = 0.975), acute kidney failure (R = 0.978), cancers of the thyroid (R = 0.988), liver (R = 0.960), bladder (R = 0.981), pancreas (R = 0.918), kidney (R = 0.973) and myeloid leukaemia (R = 0.878). He also says that many other toxins increase the chances of developing PD. Paraquat exposure, for example, increases the risk 2.5-fold. Ken goes on:The latest research shows that AD, PD, and ALS share a common, or at least overlapping, pathologic mechanism(s).98 This implies that AD, PD, and ALS are essentially the same disease with a slight variation on a theme. The theme is pesticide poisoning, and there is a synergistic99 effect of heavy metals (think aluminum and mercury)…Assuming one does not have Lyme or Parkinsonism from a toxic exposure, such as carbon monoxide, it is worth a trial of 4-PAS or 5-ASA, which will selectively chelate manganese (Mn) from the human body. Mn is an essential trace element but can be absorbed or ingested in toxic forms and in excess. Exposure can be from gasoline to well water. If the 4-PAS or 5-ASA changes symptoms, this is about Mn toxicity issues.119 Glutathione is not a specific detoxification agent for just Mn, but a large dose will help remove some of it. Mesalazine, also known as mesalamine, is 5-aminosalicylic acid (5-ASA), and like 4-PAS is considered by conventional medicine to be anti-inflammatory because it helps treat inflammatory bowel disease. Abstract: Idiopathic Parkinson's Disease (PD) is characterized by the loss of dopaminergic neurons in the substantia nigra pars compacta, which is associated with neuroinflammation and reactive gliosis. The underlying cause of PD and the concurrent neuroinflammation are not well understood. In this study, we utilized human and murine neuronal lines, stem cell-derived dopaminergic neurons, and mice to demonstrate that three previously identified genetic risk factors for PD, namely SATB1, MIR22HG, and GBA, are components of a single gene regulatory pathway. Our findings indicate that dysregulation of this pathway leads to the upregulation of glucocerebrosides (GluCer), which triggers a cellular senescence-like phenotype in dopaminergic neurons. Specifically, we discovered that downregulation of the transcriptional repressor SATB1 results in the derepression of the microRNA miR-22-3p, leading to decreased GBA expression and subsequent accumulation of GluCer. Furthermore, our results demonstrate that an increase in GluCer alone is sufficient to impair lysosomal and mitochondrial function, thereby inducing cellular senescence dependent on S100A9 and stress factors. Dysregulation of the SATB1-MIR22-GBA pathway, observed in both PD patients and normal aging, leads to lysosomal and mitochondrial dysfunction due to the GluCer accumulation, ultimately resulting in a cellular senescence-like phenotype in dopaminergic neurons. Therefore, our study highlights a novel pathway involving three genetic risk factors for PD and provides a potential mechanism for the senescence-induced neuroinflammation and reactive gliosis observed in both PD and normal aging. This week we will discuss Parkinson's Disease (PD) from the caregiver perspective with Terri Pease. Before she met her husband (Peter), who already had PD when they met, Terri was working flat-out, traveling around the country, as a trainer and consultant for organizations and staff that support survivors of domestic violence and sexual violence. She was ready to slow down a little and started dating. Despite knowing that Peter had Parkinson's Disease (PD), she was intrigued by his story and the twinkle in his eye. Within a year they married, and she began the journey that led to her life today, as an author, consultant, and coach for those who care for spouses who have Parkinson's and related neurological conditions. She wrote her new book, "Love, Dignity and Parkinson's: from Care Partner to Caregiver" to be the book she needed. She found that PD caregivers largely carry on in isolation and uncertainty. It's her mission to end that by helping to make life smoother and ease loneliness for caregivers of People with Parkinson's.

Abstract: Functional brain imaging has shown that the awake brain, independent of a task, spontaneously switches between a small set of functional networks. How useful this dynamical view of brain activity is for clinical studies, e.g., as early markers of subsequent structural and/or functional change or for assessing successful training or intervention effects, remains unclear. Core to addressing this question is to assess the robustness and reproducibility of the analysis methods that model, characterize, or infer the features of brain dynamics, and the accuracy by which these features represent and classify specific cognitive or altered cognitive states. This is particularly key given inter- and intra-individual variability and measurement noise. Here we used resting-state EEG from persons with Parkinson's Disease (PD) and healthy matched controls to systematically assess the reliability, robustness, and sensitivity of Hidden semi-Markov models (HsMM). These models are an example of model-based probabilistic methods for Brain-State allocations that are estimated from observed data. The method estimates model parameters, if the M/EEG recording or observations, over the scale of minutes, are emissions from hidden states that persist over short durations, before switching or transitioning to other states. We introduce an analysis pipeline that leads to sets of reproducible features of neurophysiological dynamics at the individual level. These features can be used as discriminatory variables to classify individuals and to evaluate the effect of non-pharmacological training schemes like in the current example a music-gait exercise program for Parkinson's Disease. Given the method stochasticity and the data variability, we emphasize the importance of repeating the analysis to reliably identify brain states and their dynamical trajectories that subsequently can be related to individualized variables. K. Abstract: Background: Parkinson's Disease (PD) has been associated with a distinct odour, strongest in sebum-rich areas. Thermal Desorption Gas Chromatography Mass Spectrometry (TDGCMS) has revealed volatile signatures that distinguish individuals with Parkinson's Disease (PD) from healthy controls. Here, we applied the same method, including subjects with isolated REM sleep behaviour disorder (iRBD) to examine the volatiles in sebum and compare this with that found in PD subjects and control participants. Participants with iRBD have a high likelihood for conversion to overt clinical synucleinopathies like PD, Dementia with Lewy Bodies (DLB) or (less commonly) Multiple System Atrophy (MSA). Methods: Subjects with clinically established PD (n=16) or iRBD (n=9) as well as healthy controls (n=9) were included. Following methods established in our laboratory, sebum was sampled from each participant using cotton gauze and the headspace from these swabs, analysed directly with TDGCMS. Univariate and multivariate analysis was employed to probe the differences between volatile metabolites found for each phenotype. Putative identifications were assigned using spectral matching against the Golm metabolome and NIST spectral databases. Findings: We can completely classify each phenotype using the sampled volatilome from which we built models with logistic regression analysis. The classification between PD and control improved on previously published work, from 85% to 100%. Putatively annotated molecules include alkanes, aldehydes, fatty acid methyl esters (FAMEs), and three metabolites namely purine, tropinone and oleamide. Investigation of highly ranked features revealed 18 features that showed intermediate expression in samples from iRBD participants. Interpretation: TDGCMS can differentiate volatile metabolite signatures from sebum between PD, RDB and control samples. More than 70% of the identifiable metabolites that 2 permit this discrimination were putatively annotated as hydrocarbons and fatty acid methyl esters (FAMEs). Our prior work indicates that these components arise from larger lipid molecules that decompose during the experiment. Features putatively annotated as tropinone, oleamide and purine, have previously been linked with neuroprotection, sleep induction and antioxidation, respectively, are significantly different between the three groups of participants, along with FAMEs and hydrocarbons. Today's episode is with Caitlin Nagy and we talk all about Young Onset Parkinson's Disease - including her diagnosis, her journey and the wisdom she's gained along the way. A lot of people perceive Parkinson's Disease [PD] to be an old person's disease. However, the number of people diagnosed with PD in their 20's, 30's and 40's is dramatically increasing. In fact it is one of the fastest growing neurological disorder in people under 45. In this episode Caitlin shares the circumstances surrounding her diagnosis at 32 years old and how her pregnancy significantly influenced the progression of the symptoms (which is not uncommon). For the first 6 years Caitlin avoided social events, get togethers or meeting up with old friends because she was totally ashamed of her body and her lack of control over it. And really just didn't want to try and explain what it was when I really hadn't accepted what it was myself. Caitlin shares her journey from resistance to acceptance and what that has meant for her self esteem and self identity [this might be my favorite part of the whole discussion]. In this episode Caitlin also talks about the reality of how medications for PD have been exclusively tested on only men and yet, for women there is a significant influence that comes from our unique hormonal journey and cyclical physiology. Abstract: The USP19 deubiquitinase is found in a locus associated with Parkinson's Disease (PD), interacts with heat shock proteins and promotes secretion of a-synuclein (a-syn) through the misfolding associated protein secretion (MAPS) pathway. Since these processes might modulate the processing of a-syn aggregates during the progression of PD, we tested the effect of USP19 knockout (KO) in mice expressing the A53T mutation of a-syn and in whom a-syn preformed fibrils (PFF) had been injected in the striatum. Compared to WT, KO brains showed decreased accumulation of phospho-synuclein (pSyn) positive aggregates. The improved pathology was associated with less activation of microglia, higher levels of synaptic marker proteins and improved performance in a tail suspension test. Exposure of primary neurons from WT and KO mice to PFF in vitro also led to decreased accumulation of pSyn aggregates. KO did not affect uptake of PFF in the cultured neurons. It also did not affect the propagation of aggregates as assessed by exposing WT or KO neurons to PFF and measuring pSyn positive aggregates in non-exposed adjacent neurons separated using a microfluidics device. We conclude that USP19 instead modulates intracellular dynamics of aggregates. Indeed, at the early time following PFF injection when the number of pSyn positive neurons were similar in WT and KO brains, the KO neurons contained less aggregates. KO brain aggregates stained more intensely with anti-ubiquitin antibodies. Immunoprecipitation of soluble proteins from primary neurons exposed to PFF with antibodies to ubiquitin or pSyn showed higher levels of ubiquitinated a-syn oligomeric species in the KO neurons. We propose that the improved pathology in USP19 KO brains may arise from decreased formation or enhanced clearance of the more ubiquitinated aggregates and/or enhanced disassembly towards more soluble oligomeric species. Objectives: Our meta-analysis aimed to identify and summarise studies using MRI approach to identify PD psychosis-specific brain regions and examine the relation between cortical volume loss and dopaminergic and serotonergic receptor density. Methods: PubMed, Web of Science and Embase were searched for MRI studies of PD psychosis (PDP) compared to PD patients without psychosis (PDnP). Seed-based d Mapping with Permutation of Subject Images was applied in the meta-analysis where coordinates were available. Multiple linear regressions to examine the relationship between grey matter volume loss in PDP and receptor gene expression density (extracted from the Allen Human Brain Atlas) were conducted in R. Results: We observed lower grey matter volume in parietal-temporo-occipital regions from our meta-analysis (N studies =10, PDP n=211, PDnP, n=298). These results remained significant after adjusting for PD medications and for cognitive scores. Grey matter volume loss in PDP was associated with local expression of 5-HT1a (b=0.109, p=0.012) and 5-HT2a receptors (b=-0.106, p=0.002) also after adjusting for PD medications (5-HT1a, p = 0.005; 5-HT2a, p = 0.001). Conclusions: Widespread cortical volume loss in the parieto-temporo-occipital regions involved in information processing and integration, as well as attention, could result in PD psychosis symptoms. Abstract: Most people with Parkinson's Disease (PD) walk with a smaller mediolateral base of support (BoS) compared to healthy people, but the underlying mechanisms remain unknown. According to the extrapolated center of mass (XCoM) concept, a decrease in mediolateral XCoM excursion would require a smaller mediolateral BoS to maintain a constant margin of stability (MoS) and remain stable. As people with PD typically walk with reduced trunk motion, we hypothesized that the mediolateral MoS might stay the same despite a smaller BoS. As proof of principle, we assess whether walking with reduced trunk motion results in a smaller step width in healthy adults, without altering the mediolateral MoS. Fifteen healthy adults walked on a treadmill at preferred comfortable walking speed in two conditions. First, the 'regular walking' condition without any instructions, and second, the 'reduced trunk motion' condition with the instruction: 'Keep your trunk as still as possible'. Treadmill speed was kept the same in the two conditions. Trunk kinematics, step width, mediolateral XCoM excursion and mediolateral MoS were calculated and compared between the two conditions. Walking with the instruction to keep the trunk still significantly reduced trunk kinematics. Walking with reduced trunk motion resulted in significant decreases in step width and mediolateral XCoM excursion, but not in the mediolateral MoS. Furthermore, step width and mediolateral XCoM excursion were strongly correlated during both conditions (r=0.887 and r=0.934). This study shows that walking with reduced trunk motion leads to a gait pattern with a smaller BoS in healthy adults, without altering the mediolateral MoS. Our findings indicate a strong coupling between CoM motion state and the mediolateral BoS. We expect that people with PD who walk narrow-based, have a similar mediolateral MoS as healthy people, which will be further investigated. Copy rights belong to original authors. Abstract: Motor symptoms of Parkinson's Disease (PD) are associated with dopamine deficits and pathological oscillation of basal ganglia (BG) neurons in the beta range ([12-30] Hz). However, how the dopamine depletion affects the oscillation dynamics of BG nuclei is still unclear. With a spiking neurons model, we here captured the features of BG nuclei interactions leading to oscillations in dopamine-depleted condition. We found that both the loop between subthalamic nucleus and Globus Pallidus pars externa (GPe) and the loop between striatal fast spiking and medium spiny neurons and GPe displayed resonances in the beta range, and synchronized to a common beta frequency through interaction. Crucially, the synchronization depends on dopamine depletion: the two loops were largely independent for high levels of dopamine, but progressively synchronized as dopamine was depleted due to the increased strength of the striatal loop. Our results highlight the role of the interplay between the GPe-STN and the GPe-striatum loop in generating sustained beta oscillations in PD subjects, and explain how this interplay depends on the level of dopamine. In this episode, we speak with Valeria Gary who is a Health Coach and a Speech Language pathologist working within the healthcare industry. In 2015, Valeria transitioned her career as an SLP, to providing therapy to patients with PD, and works as a volunteer for Parkinson's Foundation in Georgia and as the owner of Total Body Cognition and Southeast Parkinson's Speech Services.Currently, there are approximately 10 million people worldwide suffering from PD and in our conversation, Valeria explains that PD is a neurodegenerative disease, which means that it impacts the brain and the patient gradually worsens over time. She tells us that at this time, there are no known causes or cures for PD, however, with the correct medications and support, patients can maintain a good quality of life.PD not only impacts the body but Valeria talks about the many ways it can affect phonation, such as patients experiencing reduced volume, monotone pitch and breathiness or hoarseness in the voice. She stresses that early intervention is most beneficial for the patient and it is easier to maintain function than to regain some. Abstract: Accumulation of advanced glycation end products (AGEs) on biopolymers accompany cellular aging and drives poorly understood disease processes. Here, we studied how AGEs contribute to development of early on-set Parkinson's Disease (PD) caused by loss-of-function of DJ1, a protein deglycase. In induced pluripotent stem cell (iPSC)-derived midbrain organoid models deficient for DJ1 activity, we find that lysosomal proteolysis is impaired, causing AGEs to accumulate, -synuclein (-syn) phosphorylation to increase, and proteins to aggregate. These processes are at least partly driven by astrocytes, as DJ1 loss reduces their capacity to provide metabolic support and triggers acquisition of a pro-inflammatory phenotype. Consistently, in co-cultures, we find that DJ1-expressing astrocytes are able to reverse the proteolysis deficits of DJ1 knockout midbrain neurons. In conclusion, astrocytes' capacity to clear toxic damaged proteins is critical to preserve neuronal function and their dysfunction contributes to the neurodegeneration observed in PD. Despite being born into an Ayurvedic culture, Dr Anup Mulakaluri, ND, didn't seek out Ayurvedic training until his mom gained medical benefit this ancient science. We then shift to the Ayurvedic perspective of neurology. He describes the nervous system is the action arm of consciousness, where choices are made. He discusses the innate connection between the nervous system and the gut as they can be connected through vata dosha and 'vatagati', meaning 'vata goes there'. Dr Mulakaluri unfolds the beauty of Ayurveda to meet the patient exactly with what they need. Typically with neurological disease, primary treatments are to apply soothing oil topically with massage but also internally with ghee, oils, and fatty animal tissue like cold-water fish in the diet. He explains how the lipophilic nature of oil allows it to penetrate through membranes to address neurological tissues. Scott Rider is a graduate of The Ohio State University where he was twice the Captain of the Men's Track & Field team, 3 time Big Ten Champion and a 2 time All America. In fact, Scott still holds the men's outdoor record at Ohio State in the 800 Meter Run to this day. Professionally Scott founded the Rider+Reinke Financial Group in 1988, where he served as managing partner until January of 2017. Diagnosed with Parkinson's disease at age 47 Scott has attacked the disease with a combination of vigorous exercise and positive attitude. In fact in the last 4 years Scott has logged in excess of 10,000 miles on his bicycle in an effort to delay the impact of Parkinson's disease on his body. Today at the age of 61 and retired Scott has dedicated himself to encouraging others with Parkinson's disease to never give up in the fight against PD. Short chain fatty acids: An 'ace in the hole' against SARS-CoV-2 infection Scientists find that short chain fatty acids can be used to reduce susceptibility to SARS-CoV-2 infection and mortality from COVID-19 University of Fukui (Japan), July 14, 2021 Humans are no stranger to coronavirus (CoV) pandemics. Just like SARS-CoV-2 (the virus that causes COVID-19), another member of the coronavirus family--SARS-CoV--caused the severe acute respiratory syndrome (SARS) epidemic across parts of Asia in 2003. But, its spread was contained way faster than COVID-19. So, what makes SARS-CoV-2 so contagious? Both SARS-CoV and SARS-CoV-2 viruses bear "spike proteins" which get inside our cells by binding to a protein called angiotensin-converting enzyme 2 (ACE2) that is found in our cells. However, the SARS-CoV-2 spike (S) protein has been found to have a higher binding affinity (10 to 20 times that of SARS-CoV) to ACE2, thus establishing a link between the pathogen and the protein. Interestingly, recent studies have shown that patients with COVID-19 who have rhinosinusitis (i.e., inflammation of the nose) have a low risk of hospitalization. Moreover, the expression of ACE2 was reduced in patients with rhinosinusitis. Coincidentally, another study has shown that short-chain fatty acids (SCFAs), produced by bacteria in the gut have beneficial effects in allergy and viral infections. These separate findings prompted an investigation of the effect that SCFAs in the nasal cavity against SARS-CoV-2 infection by scientists from the University of Fukui, Japan, led by Dr. Tetsuji Takabayashi. In a new study published in the American Journal of Rhinology & Allergy, the scientists attempted to understand the effect of SCFAs on ACE2 expression in the nasal passage, and the potential impact on COVID-19 infection. "This is the first report that short-chain fatty acids (SCFAs) effectively reduce the ACE2 levels in human airway epithelial cells," remarks Dr. Takabayashi. To understand the status of ACE2 expression in patients with allergies, the researchers studied the levels of ACE2 in the inner lining of the nose in patients with seasonal allergic rhinitis induced by Japanese cedar pollen (SAR-JCP) and chronic rhinosinusitis (CRS). Using techniques like real time-PCR to quantify the expression of ACE2, the researchers found that there was no increase in ACE2 expression in in patients with SAR-JCP, whereas it was decreased in patients with CRS. To better understand the effect of SCFAs on ACE2 expression, the researchers cultured nasal epithelial cells and exposed them to either SFCA and double-stranded RNA (similar to the nuclear material found in some viruses and known to enhance ACE2 expression). Upon examining the expression of ACE2, the researchers saw that the SFCAs had suppressed ACE2 expression in the presence of the RNA as well. These results suggest that SFCAs has potential therapeutic applications against COVID-19. Dr. Takabayashi explains, "The nasal mucosa exhibits the highest ACE2 expression among human organs and hence is a prominent target of original infection. Therefore, the development of strategies to downregulate ACE2 expression in nasal epithelial cells could reduce SARS-CoV-2 transmission and be useful as a novel therapeutic approach." The team's timely findings will certainly aid in our fight against COVID-19. Flavonoids may slow Alzheimer onset Tufts University Human Nutrition Center, July 13, 2021 The following information was released by the U.S. Department of Agriculture: According to the U.S. Centers for Disease Control and Prevention, an estimated 5.8 million Americans aged 65 or older live with Alzheimer's disease, and that number is projected to nearly triple by 2160. Fortunately, USDA-funded research may have found a tasty way to slow disease onset. A study published in the American Journal of Clinical Nutrition suggests that diets high in flavonoids may protect cognitive health. Flavonoids are plant nutrients known for their antioxidant, antiviral, and anticancer properties and are found in berries, tea, dark chocolate, and other foods. "Alzheimer's disease is a significant public health challenge," said Paul Jacques, nutritional epidemiologist at the Jean Mayer USDA Human Nutrition Research Center on Aging at Tufts University in Boston. "Given the absence of drug treatments, preventing Alzheimer's disease through a healthy diet is an important consideration." Jacques's study, which followed 2,809 people for nearly 20 years, revealed that diets high in fruits and vegetables showed significant promise to quell the onset of Alzheimer's. "Our study showed that individuals with the highest intakes of flavonoids were more than 50% less likely to develop Alzheimer's disease, relative to those with the lowest intakes," he said. "Plant foods, such as vegetables, fruits, nuts, and seeds are good sources of flavonoids." According to Jacques, flavonoid-rich diets help more than just Alzheimer's disease and related dementia. "The bottom line is that there are many reasons to consume a healthy diet, including lower risks of cardiovascular disease and some cancers. We can now add protection of cognitive health and prevention of Alzheimer's disease to that list." Mitochondria malfunction shown to be the major cause of Parkinson's University of Copenhagen (Denmark), July 9, 2021 12,000 people in Denmark and 7 to 10 million people worldwide suffer from Parkinson's Disease (PD). It is the second most common neurogenerative disorder of aging and the most common movement disorder, but the cause of the disease is largely unknown. In a new study, researchers from the University of Copenhagen show that the most common form of the disease, encompassing 90 to 95 percent of all Parkinson's Disease cases known as sporadic PD, is caused by a blockage of a pathway that regulates the nerve cell's powerhouse, the mitochondria. "Just like when people eat, cells take what they need and get rid of the rest waste products. But if our brain cells have this specific kind of signaling blockage, it means that the powerhouse of the cell—mitochondria—cannot get cleaned up after being damaged," explains corresponding author and group leader Professor Shohreh Issazadeh-Navikas at the Biotech Research & Innovation Centre. The blockage leads to an accumulation of high amounts of damaged mitochondria, while not being able to produce enough energy for the cells. It causes neurons to gradually die, which is the reason for the development of Parkinson's Disease symptoms, and why it leads to dementia. The blockage is caused by a dysregulation of the immune genes, more specifically a pathway called type 1 interferon, which is normally important for fight against viruses, but now we show that it is also responsible for regulating the energy supply of the nerve cells. "Every part of our body needs to be regulated. We get a signal to stop eating, when we are full, and the same thing happens everywhere else in our body. If we get an infection, parts of our body need to fight it and stop it from replicating. But when the infection is cleaned up, the signal should subside. This is the job of a protein called PIAS2. That causes the blockage of the type 1 interferon-pathway, and when the infection is over, the blockage should stop and go back to normal. But that does not seem to be the case in patients with Parkinson's Disease. We further demonstrate that this dysregulation leads to a defect in the mitochondrial energy supply, as mentioned before," says Issazadeh-Navikas. These pathways are very important for brain functions, but they are also associated with microbial and virus recognition. For example, they are very important for fighting COVID-19, and a mutation in the related gene has been shown to be linked to a deadly outcome after contracting COVID-19. The researchers combined and analyzed four data sets, which studied neurons from brains with Parkinson's Disease and looked at what type of genes they express. They then looked at which gene patterns were disturbed in patients with Parkinson's Disease and especially those who had also developed PD with dementia. In order to test the results, the major findings of the combined data was tried in three different mouse models using a negative regulator of the type I interferon pathway, PIAS2, which had been identified from the patients study as one of the key proteins linked to the progression of Parkinson's Disease and dementia. "We show that a high accumulation of the PIAS2-protein is what is causing the blockage in the pathway, which should have activated the processes responsible for removing damaged protein and mitochondrial garbage," says Issazadeh-Navikas. "The accumulation of damaged mitochondrial mass further leads to increase of other toxic proteins. So when we compare patients to same-aged healthy patients without Parkinson's Disease, we see that this PIAS2-protein is highly expressed in the neurons, which is why this pathway should be evaluated for potential roles in the other forms of familial Parkinson's Disease that we have not studied here." The researchers hope the study will encourage research to counteract the pathway blockage, which could have a beneficial impact on the disease and towards preventing dementia. In the next stages, the Issazadeh-Navikas group will study how the pathwaycontributes to neuronal homeostasis and survival, as well as how its dysregulation causes neuronal cell death. Combining plant-based diet and a healthy microbiome may protect against multiple sclerosis Metabolism of isoflavone by gut bacteria protects mice from MS-like inflammation University of Iowa, July 13, 2021 A new University of Iowa study suggests that metabolism of plant-based dietary substances by specific gut bacteria, which are lacking in patients with multiple sclerosis (MS), may provide protection against the disease. The study led by Ashutosh Mangalam, PhD, UI associate professor of pathology, shows that a diet rich in isoflavone, a phytoestrogen or plant-based compound that resembles estrogen, protects against multiple sclerosis-like symptoms in a mouse model of the disease. Importantly, the isoflavone diet was only protective when the mice had gut microbes capable of breaking down the isoflavones. The findings were published July 9 in Science Advances. "Interestingly, previous human studies have demonstrated that patients with multiple sclerosis lack these bacteria compared to individuals without MS," Mangalam says. "Our new study provides evidence that the combination of dietary isoflavones and these isoflavone metabolizing gut bacteria may serve as a potential treatment for MS." Isoflavones are found in soybeans, peanuts, chickpeas and other legumes. The study also found that mice fed the isoflavone diet have a microbiome that is similar to the microbiome found in healthy people and includes the bacteria which can metabolize isoflavones. Conversely, a diet lacking isoflavones promotes a microbiome in mice which is similar to one observed in patients with MS and lacks beneficial bacteria that can metabolize isoflavone. Multiple sclerosis is an autoimmune disease of the brain and spinal cord where the immune system attacks the protective coating surrounding nerve fibers. The symptoms of this disease include muscles weakness, balance issues, and problems with vision and thinking. While there are treatments that slow down the disease, there is currently no cure for MS. Although the exact cause of MS is unknown, a complex interaction between genetic and environmental factors are thought to initiate the disease. Recently, the gut microbiome--the trillions of gut bacteria the live inside human intestines--has emerged as a potential environmental factor that contributes to MS. In prior work, Mangalam and colleagues demonstrated that there are significant differences between the gut microbes of patients with MS and people without MS. Specifically, patients with MS lacked bacteria that are able to metabolize isoflavones. Although role of gut microbiome in human diseases such as MS is being appreciated, the mechanism through which these gut bacteria might influence the disease is poorly understood. In the current study, Mangalam's team, including first author Samantha Jensen, a UI graduate student in immunology, found that the bacteria that are lacking in patients with MS are able to suppress inflammation in a mouse model of MS. The team compared the effects of an isoflavone diet and an isoflavone-free diet on disease in the mouse model of MS. They found that the isoflavone diet led to disease protection. However, when the team placed the mice on the isoflavone diet but removed the isoflavone-metabolizing gut bacteria, the isoflavone diet was no longer able to protect against MS-like symptoms. When the bacteria were reintroduced, the protective effect of the isoflavone diet was restored. Furthermore, the team was able to show that a specific isoflavone metabolite called equol, which is produced by the gut bacteria from isoflavone, is also able to provide protection against disease. "This study suggests that an isoflavone diet may be protective so long as the isoflavone metabolizing gut bacteria are present in the intestines," say Mangalam, who also is a member of the Iowa Neuroscience institute and Holden Comprehensive Cancer Center. New findings published today show that sticking to a diet rich in fruit, vegetables, nuts, unrefined cereals, olive oil, and fish can reduce hip bone loss within just 12 months. The study is the first long-term, pan-European clinical trial looking at the impact of a Mediterranean diet on bone health in older adults. More than 1,000 people aged between 65 and 79 took part in the trial, and volunteers were randomised into two groups - one which followed a Mediterranean diet and a control group which did not. Bone density was measured at the start and after 12 months. The diet had no discernible impact on participants with normal bone density, but it did have an effect on those with osteoporosis. People in the control group continued to see the usual age-related decrease in bone density, but those following the diet saw an equivalent increase in bone density in one part of the body - the femoral neck. This is the area which connects the shaft of the thigh bone to its rounded head, which fits in the hip joint. UK study lead Prof Susan Fairweather-Tait, from UEA's Norwich Medical School, said: "This is a particularly sensitive area for osteoporosis as loss of bone in the femoral neck is often the cause of hip fracture, which is common in elderly people with osteoporosis. "Bone takes a long time to form, so the 12-month trial, although one of the longest to date, was still a relatively short time frame to show an impact. So the fact we were able to see a marked difference between the groups even in just this one area is significant." The EU-funded trial, led by the University of Bologna, was completed by 1142 participants recruited across five centres in Italy, the UK, the Netherlands, Poland and France. Those following the Mediterranean diet increased their intake of fruits, vegetables, nuts, unrefined cereals, olive oil, and fish, consumed small quantities of dairy products and meat and had a moderate alcohol intake. People in the intervention group were provided with foods such as olive oil and wholemeal pasta, to encourage them to stick to the diet, and were also given a small vitamin D supplement, to even out the effects of different levels of sunlight on vitamin D status between the participating countries. At the start and end of the trial, blood samples were taken to check for circulating biomarkers. Bone density was measured in over 600 participants across both groups at the lumbar spine and femoral neck. Of these participants, just under 10% were found to have osteoporosis at the start of the study. Co-researcher from UEA, Dr Amy Jennings said: "Although this is a small number it is sufficient for the changes in femoral neck bone density between the two groups to be statistically significant. "Those with osteoporosis are losing bone at a much faster rate than others, so you are more likely to pick up changes in these volunteers than those losing bone more slowly, as everyone does with age. "With a longer trial, it's possible we could have picked up changes in the volunteers with normal bone density. However, we already found it quite challenging to encourage our volunteers to change their diet for a year, and a longer trial would have made recruitment more difficult and resulted in a higher drop-out." The researchers would now like to see a similar, or ideally longer, trial in patients with osteoporosis, to confirm the findings across a larger group and see if the impact can be seen in other areas of the body. If the condition could be mitigated through diet, this would be a welcome addition to current drug treatments for osteoporosis, which can have severe side effects. But in the meantime, say the researchers, there is no reason for those concerned about the condition not to consider adapting their diet. "A Mediterranean diet is already proven to have other health benefits, reducing the risk of cardiovascular disease, Parkinson's, Alzheimer's and cancer," said Prof Fairweather-Tait. "So there's no downside to adopting such a diet, whether you have osteoporosis or not." Rishi mushroom promotes sleep through a gut microbiota-dependent and serotonin-involved pathway Hang-zhou Medical College (China), July 10, 2021 According to news reporting out of Zhejiang, People's Republic of China, research stated, “Ganoderma lucidum is a medicinal mushroom used in traditional Chinese medicine with putative tranquilizing effects. However, the component of G. lucidum that promotes sleep has not been clearly identified.” Our news journalists obtained a quote from the research from Hangzhou Medical College, “Here, the effect and mechanism of the acidic part of the alcohol extract of G. lucidum mycelia (GLAA) on sleep were studied in mice. Administration of 25, 50 and 100 mg/kg GLAA for 28 days promoted sleep in pentobarbital-treated mice by shortening sleep latency and prolonging sleeping time. GLAA administration increased the levels of the sleep-promoting neurotransmitter 5-hydroxytryptamine and the Tph2, Iptr3 and Gng13 transcripts in the sleep-regulating serotonergic synapse pathway in the hypothalamus during this process. Moreover, GLAA administration reduced lipopolysaccharide and raised peptidoglycan levels in serum. GLAA-enriched gut bacteria and metabolites, including Bifidobacterium, Bifidobacterium animalis, indole-3-carboxylic acid and acetylphosphate were negatively correlated with sleep latency and positively correlated with sleeping time and the hypothalamus 5-hydroxytryptamine concentration. Both the GLAA sleep promotion effect and the altered faecal metabolites correlated with sleep behaviours disappeared after gut microbiota depletion with antibiotics.” According to the news editors, the research concluded: “Our results showed that GLAA promotes sleep through a gut microbiota-dependent and serotonin-associated pathway in mice.” Vitamin C found to block growth of cancer stem cells, says peer reviewed study University of Salford (UK), July 8, 2021 Increasingly, researchers are discovering the role played by cancer stem cells in the growth and spread of the disease. In groundbreaking new research, vitamin C showed its ability to target cancer stem cells and stop their growth – preventing the recurrence of tumors. Although mainstream medicine has been slow to accept the cancer-fighting properties of vitamin C, the exciting results of this study could help to change that. In a newly-published study conducted at the University of Salford in Manchester, vitamin C demonstrated its power to stop tumors in their tracks by interfering with cancer stem cell metabolism – suppressing their ability to process energy for survival and growth. Cancer stem cells are responsible for triggering tumor recurrence, and promoting their growth and metastasis. Researchers believe that cancer stem cells give cancer its ability to resist chemotherapy and radiation – the reason for treatment failure in advanced cancer patients. The study, helmed by researchers Michael P. Lisanti and Gloria Bonucelli, was published last month in Oncotarget, a peer-reviewed journal. Peer-reviewed studies are considered the gold standard of scientific research. The study was the first to explore the effects of vitamin C on cancer stem cells – and provided the first evidence that vitamin C, in the form of ascorbic acid, can target and kill them. In a side-by-side comparison of seven different substances, vitamin C even outperformed an experimental cancer drug. The team investigated the impact on cancer stem cells of seven different substances. Three were natural substances, three were experimental drugs, and one was an FDA-approved clinical drug that is widely used. The natural products studied, along with vitamin C, were silibinin – derived from milk thistle seeds – and caffeic acid phenyl ester – or CAPE – derived from honeybee propolis. The experimental drugs were actinonin, FK866 and 2-DG, and the clinical drug was stiripentol. Researchers noted that vitamin C destroyed cancer stem cells by inducing oxidative stress. And, the vitamin performed this process ten times more effectively than 2-DG. Vitamin C used two different mechanisms of action to attack cancer stem cells. It worked as a pro-oxidant in cancer cells, depleting them of the antioxidant glutathione and causing oxidative stress and apoptosis – or cell death. It also inhibited glycolysis, which is the process that creates energy production in cell mitochondria. By inhibiting glycolysis, vitamin C inhibited mitrochondrial protein synthesis in cancer stem cells – while leaving healthy cells unaffected. Both experimental and approved cancer drugs can feature serious adverse effects, including thrombocytopenia – a deficiency of platelets in the blood that can cause bruising and slow blood clotting. They can also induce lymphopenia – a decrease in the body's infection-fighting white blood cells – and anemia, or low red blood cells. And the clinically-approved drug used in the study, stiripentol, can cause severe nausea, vomiting and fatigue. On the other hand, the National Cancer Center reports that high-dose vitamin C has caused very few side effects when used in clinical studies. All seven of the substances tested inhibited the growth of cancer cells to varying degrees – including the non-toxic natural substances. But researchers said the most “exciting” results were with vitamin C. The research team concluded that vitamin C was a “promising new agent,” and called for more study to explore its use as an adjunct to conventional cancer therapies to prevent tumor recurrence and growth. “Vitamin C is cheap, natural, non-toxic and readily available, so to have it as a potential weapon in the fight against cancer would be a significant step,” observed Dr. Lisanti. As in most of the successful studies showing vitamin C's cancer-fighting properties, researchers used high doses of vitamin C, administered intravenously. IV vitamin C therapy is available in some alternative and holistic cancer treatment clinics worldwide. Again, vitamin C was 1,000 percent more effective than 2-DG, an experimental pharmaceutical drug – in targeting cancer stem cells. If vitamin C were developed by big pharma, these results would be shouted from the rooftops and featured in newspaper headlines. Yet, as always, “the powers that be” in mainstream medicine respond with…crickets. The reason; say natural health experts, is all too obvious. As a natural nutrient and vitamin, vitamin C can't be patented, and is inexpensive and easy to obtain. Therefore, there is no incentive for cancer clinics to promote it – when they can instead rake in the profits from chemotherapy. The indifference of conventional medicine to vitamin C is all the more frustrating because the nutrient has been shown to be an effective and non-toxic anti-cancer agent in previous studies, including many conducted by Nobel prize-winning scientist Linus Pauling. Vitamin C has been shown in a Japanese study to cut mortality in cancer patients by 25 percent. In addition, it has inhibited tumors in animal studies, and been shown to kill cancer cells in a wide variety of cancer cell lines. How much longer will the potential of this safe and powerful cancer-fighting nutrient be overlooked?

The efficacy of bilateral deep brain stimulation (DBS) for the treatment of motor symptoms and levodopa-induced dyskinesias in Parkinson’s Disease (PD) patients is well established. However, the effectiveness of selecting different stimulation targets is less clear. While there is evidence to suggest that the subthalamic nucleus (STN) and globus pallidus internus (GPi) are effective targets, when combined with best medical treatment versus best medical treatment alone, the circumstances in which one target should be selected over the other are still disputed. It is also unknown whether STN and GPi DBS induce similar benefits. In this guideline, the authors systematically review the literature and grade the quality of evidence for the use of STN and GPi in DBS in patients with PD. J. Dr. Shahed - After completing her undergraduate degree at Washington University in St. Louis, Dr. Shahed received her medical degree from Baylor College of Medicine (BCM) and Neurology residency training at Duke University Medical Center. She then completed a fellowship in Movement Disorders at the Parkinson's Disease Center and Movement Disorders Center (PDCMDC) at BCM. Her research interests lie in investigating the intraoperative neurophysiology of patients undergoing deep brain stimulation (DBS) for movement disorders and the application of wearables and digital health technologies to the care of patients with Parkinson's disease. Dr. Shahed serves as Chair of the Functional Neurosurgical Working Group of the Parkinson Study Group, Chair of the Science Advisory Board for the Davis Phinney Foundation for Parkinson's, member of the Medical Advisory Board of the Tourette Association of America and past chair of the Medical Advisory Board of the Houston Area Parkinson Society. She is the lead investigator for RAD-PD: a national quality improvement registry for patients undergoing DBS surgery for Parkinson's disease. She is also an investigator with the Huntington Study Group, Dystonia Coalition and TSA International Database of DBS Studies in Tourette syndrome. She has served as Principal Investigator for industry-sponsored and investigator-initiated clinical trials and has authored several papers and book chapters. She is the recipient of the Roy H. Cullen Quality of Life Award (Houston Area Parkinson Society), the Rising Start Clinician Award (BCM), the Fulbright & Jaworski L.L.P. Faculty Excellence Award in Teaching and Evaluation (BCM) and the Healthcare Heroes Award for Outstanding Health Care Practitioner (Houston Business Journal). She was recognized as a Woman of Excellence at BCM, and is a 2018 graduate of the inaugural Women Leading in Neurology program of the American Academy of Neurology. Abstract: Deep brain stimulation (DBS) of the subthalamic nucleus (STN) is increasingly used for the treatment of Parkinson's Disease (PD) but, despite its success, the neural mechanisms behind this surgical procedure remain partly unclear. As one working hypothesis, it was proposed that DBS works by restoring the balance of the brain's resting-state networks (RSNs), which is reported to be disrupted in people with PD. Hence, to elucidate the effects that STN-DBS induces on disseminated networks, we analyzed an fMRI dataset of 20 PD patients at rest under DBS ON and OFF conditions. Moving beyond 'static' functional connectivity studies, we employ a recently developed fMRI analysis tool, the Leading Eigenvector Dynamic Analysis (LEiDA), to characterize the recurrence of brain-wide phase-locking patterns overlapping with known RSNs. Here, STN-DBS seems to increase the Default Mode Network (DMN) occurrence in individuals with PD. Since the DMN is usually disturbed in PD patients presenting deficits in cognition, our observation might be suggestive that STN-DBS contributes to a normalization of the PD-induced cognitive impairment. Moreover, we addressed the effects of DBS lead placement on RSNs balance, considering the volume of tissue activated in 3 STN subsections. We found that the Visual Network (VN) probability of occurrence increased proportionally to the volume of tissue stimulated in the limbic part of the STN. Our finding might be indicative that stimulation of the limbic STN is closely related to the stabilization of visual symptoms sometimes presented by PD patients, which are usually accompanied by VN disruption. Overall, this study offers new insights into the fine-grained temporal dynamics of brain states portraying the effects of STN-DBS in patients with PD, while at the same time trying to pave the way to improved planning strategies for this surgical procedure. Abstract: We now show that key processes implicated in synuclein pathogenesis such as impairment of cellular autophagy and alpha-synuclein (-syn) aggregation are induced by alpha-melanocyte stimulating hormone (alpha-MSH), a microglia-secreted anti-inflammatory mediator. We employed the pigmented melanoma cell line MNT-1 as a novel in-vitro cell model of melanin-containing dopaminergic neurons (DNs) of the substantia nigra (SN). Melanin levels serve as a reliable readout of autophagy in MNT-1 and exposure to alpha-MSH resulted in a decrease in melanin and a failure to slow down glucose consumption, which induced cell death by apoptosis. ASIP (agouti-signaling protein), the natural biologic inhibitor of alpha-MSH, blocked and reversed the effects of alpha-MSH. Mice administered intranasal alpha-MSH exhibited progressive decline in gait, a prevalent condition seen in patients with Parkinson's Disease (PD). Moreover, we observed what may be -syn aggregation in the SN pars compacta (SNpc). SNpc and striatal tyrosine hydroxylase (TH) density showed very modest reduction in this animal PD model consistent with PD pathology at the very early stage of disease. Fundamental Questions Addressed by this Study: 1. What may be the pathophysiologic link between neuroinflammation and impairment of autophagy in PD? 2. What may be a microglia-derived mediator that impairs autophagy and induces alpha-synuclein aggregation in dopaminergic neurons in PD? 3. Abstract: Amyloid fibril formation of -synuclein (S) is associated with multiple neurodegenerative diseases, including Parkinson's Disease (PD). Growing evidence suggests that progression of PD is linked to cell-to-cell propagation of S fibrils, which leads to templated seeding of endogenous intrinsically disordered monomer. A molecular understanding of the seeding mechanism and driving interactions is crucial to inhibit progression of amyloid formation. Here, using relaxation-based solution NMR experiments designed to probe large complexes, we identify weak interactions of intrinsically disordered acetylated-S (Ac-S) monomers with seeding-competent Ac-S fibrils and seeding-incompetent off-pathway oligomers to elucidate amyloid promoting interactions at the atomic level. We identify a binding interface in the first 11 residues of the N-terminus that interacts with both fibrils and off-pathway oligomers, under conditions that favor fibril elongation. This common N-terminal hotspot is supported by suppression of seeded amyloid formation by oligomers, as observed through thioflavin-T fluorescence experiments, suggesting competing monomer interactions. This highlights that an amyloid-incompetent species of S itself can act as an auto-inhibitor against S fibril elongation. The similarity between the fibril and oligomer structures lies in their intrinsically disordered termini. Thus, we propose that the monomer-aggregate interactions occur between the intrinsically disordered monomer N-terminus and the intrinsically disordered regions (IDRs) of the fibril/oligomers. Taken together, we propose a novel Ac-S seeding mechanism that is driven by the recruitment of intrinsically disordered monomers by the fibril IDRs, highlighting the potential of the terminal IDRs of the fibril rather than the structured core, as new therapeutic targets against seeded amyloid formation. Dysregulation of developmental transcription factors is implicated in dopaminergic neurodegeneration, but the underlying molecular mechanisms remain largely unknown. Drosophila Fer2 is a prime example of a developmental transcription factor required for the birth and maintenance of midbrain DA neurons. Using an approach combining ChIP-seq, RNA-seq, and genetic epistasis experiments with PD-linked genes, here we demonstrate that Fer2 controls a transcriptional network to maintain mitochondrial structure and function, and thus confers dopaminergic neuroprotection against genetic and oxidative insults. We further show that conditional ablation of Nato3, a mouse homolog of Fer2, in differentiated DA neurons results in locomotor impairments and mitochondrial abnormality in aged mice. Our results reveal the essential and conserved role of Fer2 homologs in the mitochondrial maintenance of midbrain DA neurons, opening new perspectives for modelling and treating PD. Curcumin (curcuma longa linn) - a polyphenol compound extracted from the spice turmeric—offers multiple biological activities. Research has pointed to the potential benefits of curcumin, as an associated therapy in oxidative stress diseases, such as neurological disorders. Curcumin, Hormesis, and the Nervous System, a research paper published in the October 2019, issue of the journal Nutrients, notes that, "there is growing evidence indicating that oxidative stress contributes to the etiology and the progression of neurodegenerative diseases, such as Alzheimer’s Disease (AD), Parkinson’s Disease (PD), Amyotrophic Lateral Sclerosis (ALS) and Multiple Sclerosis." However, the same research also points out that, "evidence-based medicine has clearly demonstrated the lack of any therapeutic effect of curcumin to contrast the onset or progression of neurodegeneration and related diseases."With advanced age, the effects of oxidative stress on brain function, known as neuro-inflammaging, has taken center stage in a search for compounds like curcumin that might offer good therapeutic treatment options. Italian researchers focused on the evidence related to the "hermetic effects" of curcumin relative to neuro-cognitive diseases. Hormesis is, "a term used by toxicologists to refer to a biphasic dose response to an environmental agent characterized by a low dose stimulation or beneficial effect and a high dose inhibitory or toxic effect."One large issue with the use of curcumin as an adjunct to therapy or a stand-alone compound is its hydrophobic property: being poorly soluble in water. "The poor intestinal absorption, structural instability, limited blood brain barrier penetration, and rapid degradation of curcumin in the body limits the potential, as a therapeutic agent in clinical trials," according to the Italians.Therefore, note the researchers, "an important aspect for the development of curcumin as a novel "nutraceutical’’ formulation deals with obtaining an increased bioavailability and physiological stability and solubility of this compound in animal models and humans."Much of the scientific interest has to do with curcumin’s potential to affect the central nervous system and protection from beta amyloid plaque—a prerequisite to neuro-degenerative diseases like Alzheimer’s.While curcumin is a natural product, it’s not without potential safely concerns, such as a potential toxic effect though its interaction with drug metabolism enzymes. One such interaction involves the issues associates with curcumin concurrent with tamoxifen, an antagonist to estrogen receptors, which is used as an adjuvant therapy in women with breast cancer."Furthermore, note the Italian researchers, curcumin has been shown to pose some adverse effects to the liver, under certain circumstances at high doses.Yet, the researchers conclude that, "with regard to the potential clinical efficacy of curcumin, there are several pre-clinical data in the literature confirming that curcumin possesses neuroprotective and cognitive-enhancing properties that may help delay or prevent neurodegenerative diseases."The best advice is to take Buddha’s recommendation and, "be moderate in all things." Ira Pastor Comments: On past shows we have touched on the topic of systems biology, a field of study that looks at the interactions between components of biological systems, and how these interactions give rise to the function and behavior of that system, and the respective biological processes whether that be growth, development, aging, disease, or degeneration. On other shows, we’ve also touched on the microbiome and virome, as well as the connection between various microorganisms, either directly, or via the immunomodulatory and/or auto-immune effects that may occur from being exposed to them, on the downstream development, progression, or in some cases control of chronic diseases such as Alzheimer’s and Diabetes. And lastly, we’ve touched on the topic of “inflamm-aging;” this chronic progressive increase in the proinflammatory status across your body as you get older. Today we are going to continue along at the intersection of these interesting themes in relation to the topic of Parkinson’s Disease (PD). Parkinson's Disease is a long-term degenerative disorder of the central nervous system (affecting an estimated 6 million people globally) that mainly affects the motor system. The symptoms usually emerge slowly, which in early disease typically manifest as shaking, rigidity, slowness of movement, and difficulty with walking, and these main motor symptoms are collectively called "parkinsonism", or a "parkinsonian syndrome." The motor symptoms of the disease result from the death of cells in the substantia nigra, a region of the midbrain which results in not enough dopamine in this region of the brain. The cause of this cell death is poorly understood, but it involves the build-up of proteins (a main one being alpha-synuclein) into abnormal aggregates called Lewy bodies in the neurons. As the disease worsens, non-motor symptoms become more common and thinking and behavioral problems may also occur. Dementia becomes common in the advanced stages of the disease. Depression and anxiety are also common, occurring in more than a third of people with PD. Other symptoms include sensory, sleep, and emotional problems. The cause of Parkinson's Disease is unknown but is believed to involve both genetic and environmental factors. Those with a family member affected are more likely to get the disease themselves. There is also an increased risk in people exposed to certain pesticides and among those who have had prior head injuries, while interestingly there is a reduced risk in tobacco smokers and those who drink coffee or tea. Our guest today is going to take us down the path of a fascinating new theory that in certain cases of Parkinson's Disease, this horrific degenerative CNS disorder, might actually arise not in the CNS at all, but in the intestinal system and from there migrate to the brain. Dr. Per Borghammer, is Clinical Professor in the Department of Clinical Medicine, Nuclear Medicine and the Positron Emission Tomography (PET) Center at Aarhus University in Denmark. With a PhD in Neuroscience and a medical degree from Aarhus, Dr. Borghammer specializes in a variety of Neurobiological research areas including the study of Parkinson's disease, Atypical Parkinson's, and other neurodegenerative brain disorder pathophysiology & imaging, imaging within the parasympathetic nervous system, and imaging of cholinergic signalling processes that occur in other pathologies such as prostate cancer and chronic inflammation. He recently won the European Association of Nuclear Medicine's Marie Curie Award for a project entitled "Visualising Parasympathetic Denervation in Parkinson’s Disease." Dr. Jason Aldred, neurologist and Parkinson’s expert, joins the podcast to discuss how people with a diagnosis of Parkinson’s Disease (PD) can still live an active and enjoyable life. The podcast covers for main topics: early signs and symptoms of PD (including diagnosis), treatment options (natural and medical), other issues related to PD, and additional resources and research in the field. Show Notes Dr. Aldred initially studied Greek and Latin, as well as the study of consciousness and the idea of free will. This led him to an interest in neuroscience and how the brain controls movement of the body. The first topic we discuss surrounds the early signs and symptoms of PD, as well as how it’s diagnosed in his clinic. People with Parkinson’s display several key characteristics, including: slowness, stiffness, resting tremor, and postural changes. Stiffness and pain are often associated symptoms, and reports of shoulder pain are especially common. Interesting fact: only 2 out of 3 people with PD will have a resting tremor. You can still have PD without one. What is Parkinson’s? Dr. Aldred covers the degenerative changes in the brain (in the substantia nigra), which leads to a lower production of dopamine. This decrease in dopamine is responsible for the changes in movement patterns. Topic #2 was all about effective treatment strategies for Parkinson’s. Dr. Aldred likes to tell his patients that there are 4 things they need to do: exercise, exercise, exercise! Combine this approach with skilled therapies when needed (physical therapy, occupational therapy, speech therapy), as well as medical interventions, and people with PD have a very good chance at living an active and fulfilling life! He also breaks down treatment into 3 categories: exercise, medicine and advanced treatment options. There are exercises specific to PD, as well as medications that can mimic the effects of dopamine (levadopa is the primary medication). For more advanced PD, there are 2 main options: levadopa pumps and DBS (deep brain stimulation). Dr. Aldred explains these options in detail during the episode. Topic #3 was in regards to other issues associated with Parkinson’s. Many people tend to focus on the motor/movement issues. However, there are quite a few other “non-motor” issues that need to be considered and worked on. These include the following: difficulty sleeping, cognitive issues, depression, anxiety and low blood pressure. As you can see, Parkinson’s is not just a movement disorder! And I think that’s what makes Dr. Aldred and his team so special: the fact that they appreciate and pay attention to all the other factors that contribute to their patients’ overall quality and enjoyment of life. Topic #4 included a discussion of the various resources for people with PD, including resources for spousal and/or caregiver support. After all, if your spouse has PD, it can have quite an impact on your life as well. Given the differences women may encounter when dealing with their Parkinson’s Disease (PD), the Parkinson’s Foundation is leading the first national effort to address gender disparities in Parkinson’s research and care as part of an overall Women and PD Initiative. The Women and PD Teams to Advance Learning and Knowledge project (Women and PD TALK, for short) aims to develop new patient-centered recommendations to improve the health of women living with PD. Over the two-year term of the project, a national network of ten sites is engaging experts in the patient, research, and health care communities to identify women’s needs, prioritize solutions, and create the recommendations. Using these insights, the Parkinson’s Foundation and leadership teams will develop action plans to improve Parkinson’s care for women. In this podcast, Megan Feeney, a Senior Manager in the Community Engagement Department of the Parkinson’s Foundation, and Jessica Shurer, Clinical Social Worker and Center Coordinator of the Parkinson’s Foundation Center of Excellence at the University of North Carolina in Chapel Hill, discuss the reasons for the project, the personnel involved in it, how it will run, and its goals. While the project is just starting, it is already producing actionable insights.
Medical research is a long and complicated process. Discovering the underlying causes of Parkinson’s Disease (PD), finding new treatments, or searching for a cure involves laboratory work, animal testing, and clinical trials with control subjects and then people with Parkinson’s. One important way to facilitate that research is to involve people affected by PD. The Parkinson’s Advocates in Research program (PAIR) at the Parkinson’s Foundation trains people with PD and caregivers to become involved in the research process, advising researchers on the important topics and helping design successful clinical trials without being overly burdensome to people participating in the trials. In this podcast, Karlin Schroeder, Director of Community Engagement at the Parkinson’s Foundation, and Kevin Kwok, a Research Advocate in the PAIR program, describe the program, its aims, and how to get involved for anyone wishing to become an advocate.
Sarkis Mazmanian is a professor at California Institute of Technology who studies how the gut microbiome influences the development and function of the nervous system, the “Gut-Brain Axis”. Dr. Mazmanian has discovered that the microbiome influences the development of Parkinson’s Disease (PD), a neurodegenerative disease caused by the loss of dopamine-producing cells in the brain that affects motor function. The relation between covert and overt spatial attention and saccadic eye movements was investigated in control subjects, Parkinson’s Disease (PD) patients, and cerebellar patients in a dual-task paradigm. The main question was how different types of cues (reflexive/intentional) guide the spatial attention during fixation or during the preparation phase of a saccade. The subjects were asked to follow a reflexive or intentional cue, to discriminate a character that appeared either at the cued side (valid trials) or at the non-cued side (invalid trials), and to respond by pressing a joystick. The proportion of valid/invalid trials (cue relevance) was 75/25 and 50/50 for the control subjects, for the patients only the proportion 75/25 was used. All discrimination tasks were performed during the preparation of the saccade to the cued target and also during fixation. The results of the control subjects showed that discrimination of the character is always better at the cued side irrespective of the eye movement condition or the cue relevance, suggesting that spatial attention is engaged at the cued location even under fixation conditions and irrespective of the relevance of the cue. The results of the PD patients point to an intentional saccade impairment that does not correlate with the overall impairment in the attentional control. In the double task, also cerebellar patients showed an intentional saccade impairment that correlates with the deficit in the attentional control. After these experiments further research could investigate the impairment of the patients shown here is true for all cerebellar disorders.
