Mpox continues to spread in Africa and young children are experiencing the highest mpox burden and mortality of all ages. In this episode Kyeng Mercy, epidemiologist at Africa Centres for Disease Control and Prevention takes us through the continental response to mpox, the Africa CDC data on mpox in children younger than 15 years, and African priorities in response to the mpox public health emergency. Takeaways from today's episode: Decolonising global health is an ongoing movement allowing people (and researchers) to provide a local context in the conversations surrounding health. Though many definitions exist, it is based on the undoing of the colonial legacy that surrounds countries in the global south.Local health is global health - a local context is required to make it global. It is important to reflect on issues on decolonising global health to avoid neo-colonization and include diverse voices from the global south to spearhead the movement.It is not just about putting a researcher from the global south on the paper to get round publication guidelines and funders. It’s not about a tick-box exercise. The conversation needs to address power imbalances in funding, teaching or accessibility to learning resources. Investment in these areas can start to close the gaps.Early in the COVID-19 pandemic, written articles expected the global south to fail. But if we had diverse voices published and circulated en masse, the world would have known that the global south has always dealt with infectious outbreaks and the global north could have learnt some lessons from countries in the global south. Bienvenidos una vez más a Buscadores de verdad. Me gustaria ser portador de buenas noticias pero me temo que volveré a ser un bicho de mal agüero. No aprendemos o quizás el problema radique en que aprendemos demasiado bien, a que nos acostumbramos a los cambios demasiado rápido . La élite psicopatocratica de la que vengo ya hablando desde hace bastante tiempo nos tiene bien tomada la medida y nos sigue conduciendo a través de los laberintos de cercas como el granjero conduce a las vacas. Por cierto, luego les presentaremos un nuevo podcast corto que se llamará el hilo de Ariadna que tiene mucho que ver son salir del laberinto. Ya lo decía la portada de The Economist de este año con ese 2020 marcado en rojo y verde aunque muchos solo vieron en blanco y negro. Lo contaba yo en mi ultimo articulo “Despertar no es lo que te contaron ¡Despierta!” El covidianismo de mano dura en rojo y el supuesto anticovidianismo de mano blanda en verde se iban a encontrar en este año de transición en esa tan cacareada nueva normalidad. Muy pronto veremos prosperar a los grandes gurús verdes implicados en la soberanía, la ecología, las energías libres y/o alternativas, las formas sostenibles de alimentarse, las medicinas llamadas holísticas y otros muchos campos. Este surgimiento del supuesto despertar y toda una pléyade de supuestos nuevos conocimientos que nos acercaran a lo natural, al yo profundo que nos vincula a la tierra, serán sin duda propulsados en breve para uso y disfrute de toda la humanidad. Primero se le golpeo con la porra al burro y después se le ofreció la zanahoria. Y obviamente el burro se puso a andar de buena gana. Por ello digo que aunque a priori salir de esta locura Tragipandemica donde nos han metido pudiera parecer una magnífica noticia es sin duda solo parte de los prolegómenos para la llegada del definitivo Nuevo Orden Mundial Verde. As new mom's to be, how we plan on bringing a new baby is something we plan for. For some it's literally drawing up a 'birth plan' on what and how will go on during the day. However, things happen and plans change. For other women, there is no plan at all, it is even a luxury to have a birth plan. This episode touches on the two ways of bringing babies into the world and focuses on the debate concerning caesarean birth and the challenges that women, particularly black women face when preparing, during and after birth. The episode explores maternal health and the disparities that exists primarily for black women; those that are in the United States and in Sub Saharan Africa. Did you know that worldwide, more than one in four adults (28% or 1.4 billion) are physically inactive? In some countries, it’s as high as one in three! How many more times do we need to be reminded that physical inactivity is one of the leading risk factors for global mortality before we finally decide to get off the couch? On this week’s episode, Prof Fiona Bull MBE (T: @fiona_bull) joins BJSM’s Daniel Friedman (T: @ddfriedman) to discuss the latest physical activity trend data and explain how clinicians can play their part to increase levels of physical activity for a more active world. Prof Bull is the Programme Manager in the Department of Prevention of Noncommunicable Diseases (NCD) at the World Health Organization based in Geneva, Switzerland. She leads the WHO’s global work on physical inactivity, healthy eating and the prevention of obesity, and provides leadership for global monitoring and surveillance of NCDs and their risk factors. Bull joined WHO in 2017 after 25 years in applied research in Australia, the UK and the USA. Her recent positions include Professor of Public Health and Director of the Centre for Built Environment and Health at the University of Western Australia and Professor of Sports Science and Director of National Centre of Physical Activity at Loughborough University in the UK. Bull has co-authored over 180 scientific publications and reports. Her interest is in bridging the knowledge-policy-practice gap, and she has been actively involved in civil society. She is immediate past President of the International Society of Physical Activity. In 2014, Bull was awarded a Member of the British Empire (MBE) for her services to public health. Ensemble, ils décryptent le débat féroce qui oppose deux économistes américains autour de l’efficacité de l’aide au développement: d’un côté, William Easterly, très critique de certaines causes humanitaires, de l’autre Jeffrey Sachs, créateur du programme « Millenium Village Projet ». Nonphysical abuse through psychological and emotional means is an integral component of trafficking and slave-like practices. Hosts Seth Daire and JJ Janflone discuss Seth's unpublished paper on the topic that ties together psychological coercive methods used in trafficking, torture, and domestic abuse. The loss of control and identity that can be a part of trafficking are traumatic. To be controlled to the degree required for slavery and trafficking means that even if there are no rapes, threats or bruises, the person has been abused. To effectively prevent and prosecute sex and labor trafficking, and to effectively assist survivors, we need to better recognize and understand the role of psychological coercion within human trafficking. Biderman’s Framework and Trafficking Survivors Method Trafficker Application 1. Isolation Kept away from family and friends so no social support. Social isolation increased the power imbalance to make victims more dependent. This led to depression and loneliness. 2. Monopolization of Perception Limited exposure to and understanding of the outside world. Monopolized their attention, so felt presence of trafficker when gone. Constantly watched. 3. Induced Debilitation and Exhaustion Deprivation of basic human needs such as food, sleep, and health care. Victims worked day after day for long hours. Some victims were forced to consume drugs or alcohol. 4. Threats Threats of arrest or deportation, and against family members. Threats of violence and death. Fostered anxiety and despair. 5. Occasional indulgences Occasion kindness gave victims positive motivation for compliance, as it provided a reprieve from abuse. As it was unknown when indulgences would be given, this created anxiety to please and not make mistakes in hope of an emotional or material reward. 6. Demonstrating "Omnipotence" and "Omniscience" Traffickers claimed influential connections to law enforcement, immigration officials, or deities. This created paranoia, fear, doubt of their sense of reality, and a sense they did not control their fate. 7. Degradation Insulted, humiliated, denied privacy and dignity. Reduced to basic animal-level concerns. 8. Enforcing Trivial Demands Focused on petty concerns developed habits of compliance to keep them entrenched. Also created a need to be perfect, which led to stress and anxiety, especially since work was debilitating due to lack of sleep, food, and social support. Sources: Patterson, Orlando.
