Welcome back to The Peds NP Acute Care Faculty series! This series was created and peer-reviewed by national leaders in acute care PNP education collaborating with one another to meet the needs of our current and future colleagues. In the push for competency-based education where faculty verify the skills of what a student can do, rather than their knowledge, our series focuses on the application of didactic content with a practical approach so that you can learn nuances of clinical skills before you reach the bedside.  This episode explores wellness for the pediatric nurse practitioner. We first discuss burnout in terms of its physical and mental impact on the provider as well as poorer patient outcomes and healthcare systems burdens. After we identify symptoms of burnout and compassion fatigue, we'll discuss what wellness looks like in various systems at work from organizational culture of wellness to ease of daily work and finally personal resilience. We end with a discussion on self-compassion and introduce the evidence in support of meditation as a strategy to promote mind-body wellness. For some, this concept is new, so the technical aspects of meditation are unpacked to facilitate your own path to daily wellness. Dr. Jeep and Vanessa give advice on how to best help a partner or loved one dealing with a chronic illness and the importance of understanding and compassion.  You don't want to miss this inspiring episode!   According to a report by the Centers for Disease Control and Prevention (CDC) published in 2018, approximately 60% of adults in the United States have at least one chronic condition, such as heart disease, diabetes, or cancer.    Furthermore, a survey conducted by the National Alliance for Caregiving and AARP Public Policy Institute found that about one in three adults in the United States, or 34.2 million people, have provided unpaid care to an adult aged 50 or older in the past 12 months. Of these caregivers, 60% reported providing care for someone with a chronic illness.   It's worth noting that these statistics are from studies conducted before the COVID-19 pandemic, which may have had an impact on the prevalence of chronic illnesses and caregiving in the United States.   According to a study published in The Journal of the American Medical Association (JAMA) in 2020, 16.5% of physicians in the United States reported having a chronic illness and 14.2% reported providing care for an adult family member with a chronic illness.    The study also found that female physicians were more likely than male physicians to have a chronic illness or care for someone with a chronic illness. In the wake of the COVID-19 pandemic, anesthesiologists are being asked to do more than ever with less. Staffing shortages, supply chain issues, administrative burdens, regulatory compliance and unpredictable work hours continue to plague the field and contribute to burnout. This is a universal problem, and these are symptoms of a work environment that's not conducive to sustainability.Although “burnout” has historically been defined in several different ways, the term is generally characterized by three distinct components: emotional exhaustion (feelings of being overextended and depleted), depersonalization (having cynical and detached attitudes toward one's work), and reduced feelings of personal accomplishment (involving a negative self-evaluation of one's own effectiveness and worth).So, what are the main causes of burnout and how can physicians foster a greater sense of well-being? Unfortunately, like any complex problem, there is no single solution. It's a multifactorial condition that's rooted in everything from schedules to culture. Anesthesiologists are often treated more like cogs in a wheel than professionals on which the system depends. And that's what it's mostly about, right? Simply wanting to feel valued, to be acknowledged as essential rather than expendable.Nevertheless, there's no scarcity of areas that require change. Do you curse the existence of the electronic health record? If you do, you are not alone. What is basically a billing system (one not designed by physicians) was forced on physicians, with virtually no input from physicians. It changed the way anesthesiologists worked; it changed how they thought; and it changed how they interacted with patients.Additionally, in the push for volume-based care over value-based care, anesthesiologists are often asked to manage not one, not two, but three and even four ORs simultaneously. The expectation to simply get everything done, and as fast as possible, further cultivates this sense of depersonalization. It's no longer Mr. Pizzi undergoing a total knee replacement; it's just another knee replacement. There's an undeniable yet indefinable loss of personal achievement without that vital patient connection. Deprived of actually getting to know your patients, a profession that you once loved and maybe even felt a calling to, can slowly erode to feeling like a series of tedious tasks.And without any say in what time a surgery begins or how long it goes on, the inability to dependably plan one's personal time and achieve a sustainable work‒life balance only further highlights two recurring stresses: lack of control and a feeling of being marginalized.  From institutional to legislative reform, there are countless opinions on how to stabilize the specialty's turmoil. Yet the one prevailing sentiment is that if there isn't significant change in healthcare, as well as the way anesthesiologists are viewed and treated, then the mass exodus will not only continue but even accelerate.So, what can you personally do? Is the touted yoga and meditation combo the true salvation? For those listening to this right now, from medical school onward, did anyone actually teach you how to take care of yourself—walk you through the key elements for self-care and self-preservation? For Dr. Sabri, wellness evolved as his career matured. Eventually, he moved abroad and studied cross-cultural practices of wellness. For Dr. Kerry, leadership roles forced him to have concern about the wellness of his employees, leading him to create his own company, 12 For Health. For Dr. Wei, a medical condition forced her to think about her life outside of the OR. All three doctors agree that head and neck surgeons have one of the highest burnout rates and the first step to solving this problem is recognizing burnout as a problem and not being ashamed of it. Then, they discuss the impact that corporate influence has on medicine and physician wellness. Many physicians may feel stuck in a system they have no say in. Dr. Olsen encourages healthcare corporations to prioritize patient and physician welfare over finances while Dr. Sabri encourages institutional leadership to actually listen and elicit change when their physicians provide feedback. Dr. Olsen then explains how raising salaries to justify longer hours actually incentivizes doctors to cut their hours and makes it easier for them to leave. Additionally, both Dr. Wei and Dr. Sabri agree that the feelings of burnout can fluctuate from day to day. Next, the doctors emphasize the importance of feeling valued as a physician and engaging in self-care techniques. Dr. Olsen advocates for training physicians to be leaders who get to know their team well and recognizes good work at an appropriate time. Dr. Sabri criticizes the American “hyper statistical” view that does not respect people for the work they do. He advises physicians to select the right institution for them and have the courage to walk away and reinvent themselves in new positions when they feel like they are not receiving the respect they deserve. Self-care techniques the doctors recommend from personal experience are: re-reading patient and mentee thank you notes, meditation, stretching, team sports, having a coffee with a friend, and learning how to say no to excess work and toxic relationships. Finally, the doctors discuss how institutional leadership can help women surgeons succeed and prevent burnout. Dr. Olsen adds that leadership needs to change if the current leaders are not effective and explains ways to form effective work units.

Learning from a Physician Well-Being Think Tank. Part 1. Gratitude to the Teravana Foundation and Mindful MD for hosting the think tank "experience." Well-Being is a global issue. Today we discuss the parallel pandemic -- un-wellness and lack of well-being. Specifically physician well-being.  The vision of physicians as superheroes can actually cause harm. Physicians and nurses are humans and there should be human expectations of us.  We also need to have human expectations of ourselves. Anything other than this is a recipe for burnout, exhaustion, and long-term disaster to our healthcare system. Physicians are stronger when we connect and recognize our common humanity.  We are stronger when we acknowledge our fallibility. We are stronger when we are a team. One without victims, villains, or superheros. The hero cape actually is a heavy burden for physicians. It's like bricks in our white coat pockets.  We can do everything right and things will still go wrong because we, and our patients, are humans. The hero cape leads to isolation and pressure to control things out of our control. Our healthcare systems are failing physicians.  This is not up for debate.  Physicians don't have adequate resources.  We learn in an education system that traumatizes us. There are few laws to protect us. Covid was the straw that broke the camel's back. It exacerbated the failings of an already broken system. From the perspective of someone who has worked in physician wellness for 20 years:  “Life whispers messages and when you don't listen, it screams.” Pearls shared in this episode: Connection, creativity, mindfulness, beauty, nature, and experience are powerful tools for healing.    Upleveling our parasympathetic nervous system helps us find heal, grow, and solve complex problems. Breaking a little bit from our normal format, we talked with our own Dael Waxman about what coaching it is, what it is not, how to know when you need a coach, and how to find one.  Dael Waxman, MD is a executive coach with MD Coaches, having joined the practice in 2021. Previously, Dr. Waxman was a Professor of Family Medicine at Atrium Health in Charlotte, NC. Previously Interim Chair and Vice-Chair of the Department of Family Medicine, he continues on the faculty there as well as serves as the Medical Director of Physician Well-being for Medical Education for Atrium Health and Medical Director of Patient-Centered Programming at Atrium CMC-Mercy. Dr Colin West, originally from Seattle, received his M.D. and Ph.D. in Biostatistics from the University of Iowa in 1999. He completed residency and chief residency in internal medicine at Mayo Clinic, and then joined the faculty in General Internal Medicine in 2004. He is currently Professor of Medicine, Medical Education, and Biostatistics at Mayo. He directs the evidence-based medicine curriculum for the Mayo Clinic Alix School of Medicine and is Assistant Dean for GME Scholarship at Mayo. Dr. West's research has focused on medical education and physician well-being, and he is Co-Director of the Mayo Clinic Program on Physician Well-Being. His research has been widely published in prominent journals including Lancet, JAMA, Annals of Internal Medicine, and JAMA Internal Medicine. In this episode: Dr West tells his story of how he came to Mayo after being open to a different path where his skills and interests met. There he found the confluency of luck at the "intersection of opportunity and skill" that sparked the formation of a dream team that has changed what we know about physician well-being. To start the episode, Ali asks Asif about 2 movies he saw recently - Spider-Man: No Way Home and The Matrix Resurrections. Short story: one was good nostalgia and the other reminded him of the “member berries” from South Park. Then, Asif asks Ali about how comedians create personas for comedy (7:28). Ali also discusses how podcasts and twitter have made personas less common but also discusses the advantages of using a persona. The guys then discuss some classic comedians who have used personas such as Larry the Cable Guy and Andrew Dice Clay and others who have dropped personas such as Richard Pryor, George Carlin and Joan Rivers. Then Ali asks Asif to pick back up on their conversation on Physician Burnout from a few episodes back (23:57). The guys start the discussion by using the example of a video from friend of the show Dr. Glaucomflecken. Kerri Palamara, MD is an Associate Professor of Medicine at Harvard Medical School. She leads the Center for Physician Well-being at Massachusetts General Hospital (MGH). She is an expert in physician coaching and well-being. Dr. Palamara leads the American College of Physicians “Physician Coach Training Program”, which trains physicians to integrate coaching into their quality improvement and well-being work. Dr. Palamara co-created and directs the MGH Physician Coaching Program. This program was designed to improve physician awareness of their growth, reduce burnout, and improve well-being. In this episode: Dr Palamara discussed her hard reset after facing a horrible loss and how it taught her that her well-being was pivotal to her family's well-being and to the impact she had in the world. She shares her 3 basics of self-care and how asking for help supports them. We dive into the concepts of addressing your needs and the impact it has on how you feel. A group of us American College of Physician Well-being Champions from around the country began collaborating on wellness approaches at the state and national levels in 2020. We are stepping out of our own siloes to work together to provide better resources for physicians. Well-being in medicine has been shown to be so instrumental in the physical and mental health of physicians but unfortunately, it has not always been the priority it should be, especially during the pandemic. We want to present wellness topics to physicians via a podcast format so that we can reach them where they are in their workday and their career. Our goal with the ResetMD Podcast is to share a variety of perspectives and experiences from well-being experts in medicine - physicians who have had experiences that have significantly impacted their personal lives or their career. We will showcase a diverse group of individuals and their wellness journeys. Listeners will learn from hearing this conversation and can apply what has worked for others in their own lives. Practicing physicians have one of the most stressful jobs in the U.S., making them particularly susceptible to burnout.  The impacts of prolonged burnout have enormous implications for physicians and their families, as well as their patients and the health care system itself. In this first of three interviews with Sydney Ey, PhD, Professor of Psychiatry at the Oregon Health and Science University School of Medicine, we explore that institution's successful Resident and Faculty Wellness and Peer Support programs, which she helped establish. Dr. Ey discusses the specific strategies she found most effective in helping physicians grapple with their experience of stress and build their resilience. Dr. Ey's experiences provide insights for Program Directors and others in the GME community seeking to establish their own systems of support for physicians and others working in the clinical learning environment. Dr. Ey discusses some common obstacles, as well as key facilitators, to the development of such programs that helped pave the way for the OHSU programs' success.
