On today's episode we're focusing on wearable technology for athlete performance, recovery and injury prevention with Dr. Jacob Calcei, an orthopedic surgeon at University Hospitals in Cleveland and team physician for the Cleveland Browns. We'll start off our discussion today with an article titled “Wearable Performance Devices in Sports Medicine” from the January 2016 issue of Sports Health. The authors review the rapid rise of wearable sensor technologies in athletics, highlighting their ability to track real-time physiologic and movement data for optimizing performance and reducing injury risk. These tools can provide valuable insights into workloads, biomechanics, and fatigue, supporting tailored training programs and early injury prevention strategies. While wearable devices have shown promise, evidence for their effectiveness in professional sports remains limited, and further research is needed to validate accuracy, develop standardized training protocols, and address practical challenges such as reliability and usability.Then, from the January 2020 issue of Sports Heath, we review an article titled “Does Overexertion Correlate With Increased Injury? The Relationship Between Player Workload and Soft Tissue Injury in Professional American Football Players Using Wearable Technology.” This study investigates whether sudden increases in training load, as measured by wearable GPS and accelerometry technology, are associated with higher rates of soft tissue injury in professional American football players.  The authors analyzed two NFL seasons and found that injured players experienced significantly greater spikes in workload during the week of injury compared to the prior month. Specifically,  they found that those exceeding an acute-to-chronic workload ratio (ACR) of 1.6 being 1.5 times more likely to sustain an injury. The effect was particularly pronounced in the preseason, when workloads were higher and hamstring injuries were most common. The findings suggest that rapid workload increases predispose athletes to soft tissue injury, while consistent training loads may offer a protective effect. The authors conclude that monitoring player workload with wearable technology could help medical and training staff design safer, more individualized training regimens to reduce injury risk.We are joined today by Dr. Jacob Calcei, a board-certified orthopedic sports medicine physician and shoulder surgeon at University Hospitals and an assistant professor of Orthopedic Surgery at Case Western Reserve University School of Medicine. He is a team physician for the Cleveland Browns as well as several local high schools. Dr. Calcei has published extensively on a variety of sports medicine topics, has a particular interest in injury prevention and finding ways that we can make sports safer while optimizing athlete performance. Bien qu'elle puisse sembler inhabituelle, cette pratique est soutenue par des études scientifiques qui mettent en lumière ses effets positifs sur le corps et l'esprit.Un entraînement musculaire équilibréContrairement à la course classique, la rétro-course sollicite différemment les muscles. Most commonly the challenge is gaining lean muscle. Doing all the things but can't seem to gain muscle? Then tune in and go through this like a check list of 12 strength training mistakes in menopause that could be the reason. It just takes one. Overworking Small Muscle Groups [00:02:20] You need fewer of these small muscle group-focused exercises in your routine if you prioritize the major muscles like chest (pectoralis) and back (trapezius and Latissimus Dorsi). If the secondary (biceps and triceps) muscles are going to get a workout most of the time, these will rob you of time and energy that is better spent on major muscles if you aren't getting in the adequate volume there. Skipping the Warmup [00:06:40] Nearly a decade ago, I began sharing the fact that if you skip the warmup, you also miss an opportunity to increase total energy expenditure during a workout. It's not only about injury prevention. There's an increased blood circulation, improved energy expenditure, mobility and you can work closer to your capacity. If fitness and longevity are truly goals, “exercising” is not the same as working at your capacity and safely, sanely raising the roof on your fitness level so that age doesn't automatically result in slowing down or gaining weight. Not Resting Between Sets Long Enough Before [00:13:10] Rapidly moving from one exercise to the next, was yesterday. The “metabolic conditioning workouts” are a nice anomaly, a change in pace. Rest between sets of strength training. Reach complete muscle fatigue or within 2 reps of it. If you do HIIT, separate the sessions so you get the best benefit from it and from strength. If you are not getting stronger, not increasing muscle, and these are your goals (in order, perhaps to improve body fat % ultimately), it's a “how” you're doing it problem. Plan Your Routine to Avoid 12 Strength Training Mistakes in Menopause No Organized Plan [00:17:30] You either default to using what's available at the gym or doing the same exercises in the same sequence every time. If you're not careful you won't change the stimulus (by changing the sequence and sets/rep combination) and may also not be getting enough sets per muscle group in each week. It doesn't mean that all sets for a muscle group must be all the same exercise. A super set of 6 different exercises for the triceps works. The next workout you may just do 3 and you're still covered… if you planned it that way. Not Sleeping Enough or Not Changing the Workout When You Don't Sleep [00:20:50] The sleep you get will determine the benefit you get from exercise. If you don't prioritize, your workouts will suffer too. You're also at risk for injury, not only because your body isn't fully doing the repair job or releasing testosterone and growth hormone in deep cycles of sleep as it should, but because coordination suffers. Treating Soreness or Worn Out an Indication of “Good Workout” [00:22:40] When you work muscles, it's normal to feel sore, even if you're extremely fit. Some muscles like quads and glutes are used to a lot of activity and tend to get sore less often. But there are two genes associated with soreness and you may either be predisposed to be sore or not. It's not a good indication of whether you worked hard enough. If you reached muscle fatigue or came close, you gave the muscle enough stimulus. The first sign of poor recovery was soreness after workouts, the second was reduced performance during workouts. 12 Strength Training Mistakes in Menopause to Avoid and Nurture Your Body Dependence on Supplements and Negating Balanced Whole Food Meals. [00:27:00] If you're all too willing to jump to EAAs, or BCAAs but won't eat regular meals that result in satiety, chances are your lack of micro and macronutrients will catch up with you. “Food first”, then allow supplements to take you the rest of the way. I don't like to “count” calories on a regular basis but a snapshot is very helpful for checking in. Often for knowing when you're eating too little. Taking Too Much Advice (or Too Little From Too Many People) [00:30:20] It often takes even gurus a while to come around. You have to love Vonda Wright and Mary Haver sharing their own menopause journeys. We need more women like them. They're open about not knowing what they didn't know about menopause, as physicians and women. You have at your fingertips access to women who learned the hard way, so you don't have to. Just don't jump in the middle. You need an onramp. Ignoring Nutrition Needs. [00:32:30] The talk is “calorie deficit.” Yet, 80% of women in our community under-eat for their mere existence (resting metabolic rate) and then try to put their foot on the accelerator for exercise causing a bigger caloric deficit. Welcome to a very special episode of The Sports Docs Podcast. In celebration of our 100th episode, we are honoring Dr. Mark Price who passed away on August 16, 2024 following his battle with leiomyosarcoma. We wanted to pay tribute to Dr. Price, who mentored us both through our residency at Harvard. His death is an immeasurable loss to his family and the community. Dr. Price was the Head Team Physician and Medical Director of the New England Patriots since 2016 and a team physician for the Boston Red Sox since 2009. He was a Captain in the U.S. Navy Reserves and served in combat operations in Afghanistan, where he was awarded the Bronze Star Medal. Most importantly, he was a bright example of a physician who cared deeply for his friends and family, including his wife Stephanie and their children, Henry, Julia and Sarah. The words shared by family and friends at his memorial service in Wellesley last month demonstrated a life rooted in purpose and accomplishments aligned with his values.Mark was one of our first guests, coming onto the show for Episode #3 and #4 in March of 2021. Consistent with who he was, he was willing to take the time out of his busy schedule to help two of his mentees build something new. So, with that, we thank Mark Price for all that he taught us and the mentorship he provided to not only us, but our community.--We have 5 great articles which we discuss over two episodes that really contribute well this conversation on how to best manage shoulder instability in athletes both in-season and post-season. We're very honored to have Dr. Mark Price join our discussion today. Dr. Price specializes in sports medicine, knee and shoulder surgery. He is an attending surgeon at Massachusetts General Hospital and Assistant Professor of Orthopaedic Surgery at Harvard Medical School. Dr. Price earned his MD from Harvard Medical School and PhD in Medical Physics from MIT. He completed the Harvard Combined Orthopaedic Residency Program, where Catherine and I both attended as well, and then went on to complete a fellowship in sports medicine and shoulder reconstructive surgery at Mass General. Dr. Price is Head Team Physician and Medical Director for the New England Patriots and a Team Physician for the Boston Red Sox. He is a Captain in the US Navy Reserves and has served in combat operations in Afghanistan, where he was awarded the Bronze Star Medal for meritorious service. The first paper is from the February issue of AJSM, titled Incidence of Posterior Shoulder Instability in the United States Military. It is a descriptive epidemiology study by Brett Owen and his team which found the incidence is higher than previously reported. Then, from the January issue of Sports Health, we feature the publication Does Functional Bracing of the Unstable Shoulder Improve Return to Play in Scholastic Athletes? Tokish and colleagues found functional bracing did not result in increased success rates when compared to no bracing in adolescent athletes.

The thrill of the game, the hard hits, the big plays, the rivalries—we wait all year for this. It's the heartbeat of fall.  But behind the touchdowns and tackles, there's a conversation we can't afford to ignore. As much as we love this sport, we can't turn a blind eye to the growing concerns around traumatic brain injuries—injuries that have changed the lives of countless players, from the pros to the youth leagues. In the first week of the new season, two young people, Caden Tellier, 16, of Selma, Alabama and Cohen Craddock, 13, of West Virginia, died of traumatic brain injuries. While we don't have all the details of their tragic injuries, it is a stark reminder of the risks players take when they step onto the field. In this episode, we're going to continue our discussion with Dr. Yoni Rosenblatt and focus on the rehab of different stabilization surgeries, including arthroscopic Bankart repair, Bankart with the addition of a remplissage procedure, and Latarjet reconstruction. We then wrap up with a discussion on return to play and an important conversation on the psychological aspects of recovery.Our conversation picks back up with an article from the March 2020 issue of Sports Health titled “A Comparison of Physical Therapy Protocols Between Open Latarjet Coracoid Transfer and Arthroscopic Bankart Repair.” Dr. Nik Verma and team at Rush reviewed 31 PT protocols and found a high degree of variability with regard to exercises and motion goal recommendations. Despite the variability, many milestones and start dates occur earlier in Latarjet protocols when compared with Bankart-specific protocols, which may contribute to the earlier return to play metrics identified in the literature for Latarjet compared to Bankart repair.Next, we review an article titled “Functional Rehabilitation and Return to Play After Arthroscopic Surgical Stabilization for Anterior Shoulder Instability” published in the December 2021 issue of Sports Health. In this case series, Dr. Brian Busconi and colleagues at UMass evaluated 62 athletes who underwent arthroscopic Bankart repair and were subsequently cleared to return to sports using both functional and psychological testing. The average time to pass psychological testing was 5 months, while the average time to pass functional testing was 6 months. The re-dislocation rate of 2 years was 6.5%, lower than what is currently published for this population.We finish up today with an article from the October issue of AJSM titled “Relationship of the SIRSI Score to Return to Sports After Surgical Stabilization of Glenohumeral Instability.” Dr. Rossi and colleagues in Argentina reported that patients who returned to sports and those who returned to their preinjury sports level were significantly more psychologically ready than those who did not return. In fact, for every 10-point increase in the SIRSI score, the odds of returning to sports increased by 2.9 times. Furthermore, those who did not achieve their preinjury sports level showed poorer psychological readiness to return to play and SIRSI score results.

54. Dr. Matt Provencher: Musculoskeletal Injuries in Military Population - Part I

On today's episode we're focusing on musculoskeletal injuries or “MSKIs” in warrior athletes with Dr. Matt Provencher. We have some great articles for you that contribute well to our conversation on musculoskeletal injuries in the military population. We'll start off our discussion today with an article from the March 2022 issue of Military Medicine that introduces the Military Orthopedics Tracking Injuries and Outcomes Network or “MOTION”. MOTION is an internet-based data capture system that aims to collect and analyze short- and long-term patient outcomes after surgical treatment of a musculoskeletal injury. The goal of MOTION is three-gold: First, to identify factors that predict favorable outcomes. Second, to develop models which inform the surgeon and military commanders of patient progress – if they are ahead of or on schedule for return to duty, marginally behind or substantially behind. And third, to then develop predictive models to better inform both patients and surgeons of the likelihood of a positive outcome – to enhance patient counseling and expectation management.Then, from the November 2020 issue of Sports Health, we review the publication titled “Identification of Risk Factors Prospectively Associated With Musculoskeletal Injury in a Warrior Athlete Population.” This level 2 prospective cohort study identified 11 risk factors for MSKI, including: Older age, female sex, prior MSKI, lower perceived recovery from injury, prior work restrictions, asymmetrical ankle dorsiflexion, decreased or asymmetrical performance on the Lower and Upper Quarter Y-Balance test, pain with movement tests and slower 2-mile run times.We wrap up Part I with a 2022 yellow journal article from the 15th Annual Extremity War Injury Symposium in 2022, authored by our guest today, that summarizes the recent research efforts focused on MSKI in warrior athletes. This paper primarily focuses on post-traumatic osteoarthritis and the role of orthobiologics in lessening the prevalence of this condition and accelerating recovery to return tactical athletes to full, unrestricted duties as quickly and safely as possible.We are joined today by Dr. Matt Provencher, a board-certified orthopedic surgeon at the Steadman Clinic in Vail, Colorado who specializes in complex shoulder, complex knee and sports medicine. We had the privilege of training under Dr. Provencher during his time at Mass General in Boston, where he was also the head team doctor for the New England Patriots. Dr. Provencher has over 20 years of clinical orthopedic experience and surgical care of both civilian patients and the United States Navy SEALs.He was recently named one of the Top 28 Shoulder Surgeons in the US and also one of the Top 28 Knee Surgeons in the US by Orthopaedics Today. Dr. Provencher is the Assistant Editor-in-Chief of Arthroscopy: The Journal of Arthroscopy and Related Surgery and has published over 200 peer-reviewed articles, authored 5 textbooks and 148 book chapters.

Foot and ankle injuries can be overwhelming for new sports medicine learners due to the complex network of bones, muscles, tendons, ligaments and joints. Dividing the anatomy into the ankle joint, hindfoot, midfoot, and forefoot can be helpful. Learning the intricate anatomy is the first step in understanding what structures are located where, making it easier to narrow down the differential diagnosis. Foot and ankle injuries are common in the majority of sports, and learners need to become familiar with common and not to be missed pathologies.In this episode, Dr. Fahim Merali, sports medicine specialist at the Dovigi Orthopaedic Sports Medicine Clinic at Mount Sinai Hospital in Toronto discusses ankle and foot injuries seen frequently in various sports.Listen to learn how to provide an accurate on field assessment to determine appropriateness to return to the game, when to have a player sit out and undergo more urgent assessments, how to investigate and manage subacute and chronic foot and ankle injuries and appropriate rehab protocols for various foot and ankle conditions.As always, understanding the mechanism of injury is a key component and mastering the anatomy is the only way to know what structures are present in the region of pain, which leads to the differential diagnosis. In today's episode, we'll meet with Boulder-based strength and conditioning specialist – Chris Lee. Chris is an NSCA-certified strength and conditioning specialist, collegiate swimmer, and former professional triathlete. He is also a former head coach for the University of Colorado- Boulder swim team. Chris has also worked with the likes of Olympic team relay silver medalist Morgan Pearson, Drew Hunter, and Laura Thweatt. Scot Delorme, is the President and Co-Founder of A SHOC Energy Drink. Scot fell into the beverage business by accident. Whilst away travelling a family friend reached out about a business opportunity to become a partner in a drinks company. Scot learnt all aspects of the beverage business and eventually took over the company. He started another brand called 24C, an emergency vitamin c packet. He sold the company after just a year. He knew how to create beverage brands which down the road lead to working with Monster Energy where he became VP of Innovation and would come up with all of Monster's flavors. Delorme stayed at Monster for 8 years. He then partnered with Lance Collins who foundered FUSE and BodyArmor to launch A SHOC in 2019 which at the time was called Adrenaline Shoc. Scot wanted to launch a brand that was health, had no sugar that athlete's could drink. They got NSF certified started an athlete team including Paul Rodriguez (P Rod), Chase Elliott, Billy Kemper, Dashawn Jordan. Joining us is sports performance trainer Joe Cancellieri who runs the onsite athlete gym and program at A SHOC. Episode 6: Let's discuss those karate kids (8-25 year olds)! This episode is speaking to competitive sport and NOT recreational sport. Youth sport and early sport specialization can be heavy topics -- and I'm using extreme examples (that do occur) to illustrate my point. Keep in mind I'm not stating "kids can't play competitive sport", but I am discussing how: 1) kids need to grow (mentally and physically) with the sport, and 2) what athletic identity is. It's good information, and a good compass for if you have youth athletes in sport karate! I include early 20s as "kids"... because research shows that the part of the brain that creates rational processes isn't fully developed until around 25 years old; the "act first, think later" mentality. On today's episode we're focusing on shoulder instability with Dr. Mark Price, Orthopaedic Surgeon at Massachusetts General Hospital and Head Team Physician for the New England Patriots. We have 5 great articles which we discuss over two episodes that really contribute well this conversation on how to best manage shoulder instability in athletes both in-season and post-season. We're very honored to have Dr. Mark Price join our discussion today. Dr. Price specializes in sports medicine, knee and shoulder surgery. He is an attending surgeon at Massachusetts General Hospital and Assistant Professor of Orthopaedic Surgery at Harvard Medical School. Dr. Price earned his MD from Harvard Medical School and PhD in Medical Physics from MIT. He completed the Harvard Combined Orthopaedic Residency Program, where Catherine and I both attended as well, and then went on to complete a fellowship in sports medicine and shoulder reconstructive surgery at Mass General. Dr. Price is Head Team Physician and Medical Director for the New England Patriots and a Team Physician for the Boston Red Sox. He is a Captain in the US Navy Reserves and has served in combat operations in Afghanistan, where he was awarded the Bronze Star Medal for meritorious service. (Part I)The first paper is from the February issue of AJSM, titled Incidence of Posterior Shoulder Instability in the United States Military. It is a descriptive epidemiology study by Brett Owen and his team which found the incidence is higher than previously reported. Then, from the January issue of Sports Health, we feature the publication Does Functional Bracing of the Unstable Shoulder Improve Return to Play in Scholastic Athletes? Tokish and colleagues found functional bracing did not result in increased success rates when compared to no bracing in adolescent athletes.
