    Dr. Andrew Best is a professor at the Massachusetts College of Liberal Arts where he focuses on evolutionary questions about human physiology. They looked at the maximum, sustainable amount of energy expenditure that can be done over a long time period. It answers the question, "what is the limit of human energy expenditure?"     The 80/20 Principle of Running a Cash-Based PT Clinic In this episode of the PT Entrepreneur Podcast, Dr. Danny Matta breaks down the 80/20 principle for cash-based clinic owners and simplifies what you should track if you want to grow past yourself. Instead of obsessing over dozens of metrics, Danny argues there are three "dollar productive" KPIs that drive almost all clinic growth. He also explains why provider schedules either snowball fast or stall for a year and how to shorten that ramp from 12+ months to around six months with the right focus. Time is valuable, for you and for your staff clinicians. PT Biz has found that Claire, their AI scribe, saves staff clinicians about six hours per week on average. Even if you only reclaim half of that time and convert it into patient care, that is roughly three additional one-hour visits per week per clinician. The point is to use technology to remove the documentation burden so you can increase capacity without increasing burnout. Danny applies this directly to clinic growth. When your clinic is small, it is easy to get busy doing "everything" and tracking a long list of numbers. The problem is most of those activities do not move the business. Instead, Danny recommends narrowing your focus to the most "dollar productive" activities. In other words, the actions and metrics that actually drive revenue and schedule utilization. The Goal: Get a Provider Productive Fast Danny frames the big objective clearly. You want to get your own schedule full enough to hire someone. Then you want any provider you hire to get productive as fast as possible. In PT Biz's world, once a provider reaches roughly 80 to 90 visits per month, it tends to snowball into 100+ pretty quickly. But getting to that point can take some clinics over a year. If you can shorten that ramp to six months, your growth compounds. In a year, you might be able to hire two people instead of one, because each provider becomes profitable faster. The Three Dollar-Productive KPIs Danny says there are three key metrics that drive the majority of growth in a cash-based clinic. Each one represents a drop-off point that can either accelerate growth or quietly crush it. 1) New Patient Volume and Discovery Call Conversion Many owners only track "how many evals we have." Danny says you need to go one step back and track conversion from lead to evaluation. There is often a major drop-off between someone becoming a lead and actually booking an evaluation. This is usually happening on discovery calls. Benchmarks Danny shares: During growth, aim for 8 to 10 new patients per provider per month Once stable, new patient volume can drop closer to 5 per month Discovery call to eval conversion should be 70%+ He also makes a strong recommendation: during growth phases, the owner should handle discovery calls. Why? In many clinics, admins convert around 45% to 50%. Owners often convert 80% to 90% because they carry authority and can handle objections better. Danny gives an example: 20 discovery calls at 50% conversion = 10 evals 20 discovery calls at 80% conversion = 16 evals That gap can be the difference between a provider staying empty and a provider getting busy quickly. He also points out that owners sometimes resist this because it feels like a step backward, but the time requirement is smaller than most people assume. If you have 20 calls at 20 minutes each, that is under 10 hours per month and it can dramatically impact growth. 2) Evaluation to Plan of Care Conversion The second KPI is how many evaluations convert into a plan of care. When people do not commit to a plan of care, Danny says many still come back a few times, often around three visits, until symptoms improve and then they disappear. That creates unpredictable revenue and inconsistent schedules. Plan-of-care conversion makes volume and revenue more predictable. Benchmarks Danny shares: Owner: 70% conversion from eval to plan of care Staff providers: 60% conversion is a strong benchmark at scale He emphasizes that this requires quality control and training. Staff clinicians need to be comfortable with diagnosis, prognosis, and presenting a clear plan. Otherwise close rates drift and schedules stall. 3) Recurring Services After Plan of Care Danny calls this the sneaky variable that people forget, but it can make the biggest difference in schedule stability. Hiring a clinician is usually a net negative for the business at first. You are paying salary, taxes, and benefits while they are still ramping up. What stabilizes and compounds a provider schedule is recurring volume. The goal is that roughly 40% of plan-of-care patients transition into some type of recurring service after discharge. Why this matters: Recurring visits fill a predictable chunk of the schedule New patient volume no longer has to carry the whole load Providers get to work with people they enjoy long term It is mentally easier than constant evaluations Danny also explains why this is often hard for staff clinicians. They may feel uncomfortable "selling" ongoing support because they never did it in insurance clinics They may not know what to do clinically once a plan of care ends So this requires two things: education on the clinical delivery of recurring services and training on how to present it confidently. Put It Together: How to Grow Faster Without Tracking Everything Danny's bigger point is that clinic owners often get lost in too many tasks and too many numbers. If you simplify down to these three KPIs and train your team around them, your odds of building provider schedules faster go up dramatically: Discovery call conversion (lead to eval) Eval to plan-of-care conversion Plan-of-care to recurring conversion When those are strong, growth compounds. You hire faster, providers get productive faster, and you get to choose what you want the clinic to become instead of being stuck trying to "just get busy."     The silver lining in spending so much time in hospitals with my husband for his heart condition is that I've adapted what I now call… a hospital bed perspective.   It's basically like this… If I was stuck in a hospital bed today… What would I be dying to do? What would I make time for? Who would I make time for? What would I prioritize?    It's easy to be in our routine and put things off or not realize that we aren't prioritizing the things and people that we really enjoy the most...   But all of a sudden, when you can't do those things, it becomes really clear what you would do if you had the chance.   At this stage of my life, I've gotten so clear on my priorities… What's important to me and what's not important to me so it makes it easier and faster to live life fully with this hospital bed perspective… What an incredible gift it is to be able to do so many things… we're just not going to waste it.       What the Hospital Bed Perspective Is The hospital bed perspective isn't about fear or negativity. It's about clarity. When your normal options are taken away, life becomes very simple. The noise drops. The distractions fade. What remains are the people, experiences, and moments that genuinely matter. This perspective removes urgency addiction, comparison, and busywork. It exposes what's meaningful and what has quietly been wasting your time. Ditch the little things- drama, things we should be letting go of anyway - they become obsolete.   Why We Lose Touch With Our Priorities Modern life makes it easy to drift. Routines become automatic. Busyness becomes a badge of honor. Important things get delayed because they aren't urgent. Weeks turn into months. Months turn into years. Without realizing it, many people protect work and obligations more fiercely than joy, relationships, health, or experiences. The hospital bed perspective interrupts that drift.   What Becomes Clear When You Use This Lens…  People Who you would want in the room. Who you wish you spent more time with. Where you may have been distracted, rushed, or unavailable. Time What would no longer deserve your energy. What would suddenly feel priceless. What you would deeply regret postponing. Energy What drains you that wouldn't matter from a hospital bed. What lights you up that you keep sidelining. Experiences Trips not taken. Conversations not had. Projects not started. Memories waiting to be made.   How to Bring the Hospital Bed Perspective Into Daily Life Using the question: "If I were stuck in a hospital bed next month, what would I wish I had done this week?" Running decisions through the filter: "Would this matter to me from a hospital bed?" Auditing your calendar to see if it reflects what you say matters Taking inventory of relationships you want to invest in more deeply Creating space for joy, presence, and experiences now... not "someday" This approach aligns your time with your values and turns clarity into momentum.   The Freedom This Perspective Creates Living with the hospital bed perspective naturally builds gratitude, urgency around what matters, and peace around what doesn't. It simplifies decisions, deepens relationships, and makes everyday life richer. It replaces postponing with presence and guilt with alignment.   Wrapping it up…  Most people gain this level of clarity only when life forces it on them. The gift is choosing it before you have to. You don't need a hospital bed to start living like someone who understands how valuable, fragile, and full life really is. You can choose that perspective now. And when you do, you stop wasting time. After working across industries like tech, education, and healthcare, she realized none of them truly excited her. Jewelry stood out because of its mix of fashion, storytelling, and creativity. Rather than guessing, she intentionally took in-house and freelance roles in the jewelry industry to build credibility before going all in. That vertical focus paid off. By committing to a specific industry, Laryssa was able to build a strong referral network, speak at trade shows, and create highly targeted content that drove inbound leads. But after nine years in the jewelry space, she noticed that the biggest results she delivered for clients consistently came from email marketing. What started as one service among many became the clear driver of ROI. The shift from a vertical niche (jewelry) to a horizontal specialization (email and SMS marketing) wasn't a sudden pivot. It was a response to real performance data. Stronger results, clearer processes, and deeper expertise made the decision feel natural. Your niche should serve your strengths, not trap you in yesterday's model. Why Inbound Alone Is No Longer Enough For most of Joy Joya's history, inbound marketing did the heavy lifting. Content, SEO, YouTube, and a podcast tailored to the jewelry industry created steady deal flow without much outbound effort. That's one of the biggest benefits of vertical focus: you can dominate a small pond with the right content and relationships. But the market shifted. Search behavior changed. Social algorithms changed and AI entered the picture. Laryssa realized that relying solely on inbound was no longer enough. Over the past year or two, she intentionally started building outbound muscles: cold email, cold calling, LinkedIn outreach, and systems that allowed her team to support those efforts. The key insight here isn't that inbound is dead, it's that inbound alone is risky. Agencies that survived and grew were willing to adapt their acquisition mix, even when it meant doing uncomfortable things. The Hard Question Every Agency Owner Faces Adapting isn't just about strategy. You should also ask yourself whether you want to do what's required next. New platforms, new sales motions, and new expectations can trigger an existential crisis for long-time owners. You don't have to love every part of running an agency, but you do need the discipline to face the things you'd rather avoid. The solution isn't grinding forever but rather identifying what you don't enjoy, systemizing it, delegating it, or removing it altogether. Agency owners should get comfortable with change as a necessary part of running an agency. The hard part is that change often targets the things you already tolerate but don't love. That's why many agencies stall. The owners don't hate their situation enough to change it but they don't love it enough to stay fully committed either. When Personal Brand Creates Attention But Not Conversions As AI and recommendation engines influence buying decisions, developing a personal brand becomes vital when it comes to being recommended by these tools. People want to work with leaders whose beliefs, values, and perspectives they understand. That's why podcasts, long-form content, and consistent points of view matter more than ever. In her case, Laryssa shared an unexpected challenge after developing her personal brand. She had built such a strong personal and brand identity that many people understood her perspective but didn't fully understand what her agency actually did. In some cases, prospects were more familiar with the brand name than the services behind it. The lesson for agency owners is balance. Thought leadership without clear offers creates attention without conversion. As platforms evolve, it's not enough to educate—you need to connect that education to the right services, for the right audience, at the right time.
