Are Keto, Paleo and Low Carb the same? The answer is.... No... Yes... Sort of! Similar in many ways but different in others. New Clients often get these diets confused. More information.
The Case: Anna is experiencing IBS, fatigue, anxiety and generally doesn't feel well. She was diagnosed with Hashimoto's years ago but doctors did not offer real solutions. She wanted to try adjusting her diet but research led her down a deep rabbit hole that left her confused about the best diet for her IBS, autoimmunity and other symptoms. The Investigation There is a lot of information on the internet and some of it is conflicting which makes it very hard to figure out which one to try. This inundation of sometimes conflicting information can be overwhelming and that's exactly what Anna was experiencing when she came to me. There were a lot of things to take into consideration in finding the right nutrition plan for Anna. Joining me on the show to talk more about the overwhelming amount of diet information is Risa Groux. She's a Functional Nutritionist, Certified Autoimmune Coach and author of FOODFRAME: Diet is a Four-Letter Word. It's a comprehensive guide to using functional nutrition guidelines to identify the root causes of various health issues so I knew she was the perfect person to consult with on Anna's case. What are the Most Popular Diets for Autoimmunity Right Now? Some of the most popular diets are Paleo, Ketogenic, Autoimmune Protocol, Vegan, Low FODMAP and Low Lectin. One of the first things that Risa recommends when considering following a set diet is to take into consideration your health status. For example, if you are pre-diabetic, diabetic, SIBO (small intestine bacterial overgrowth), IBS (Irritable Bowel Syndrome), or if you have an autoimmune disease, there may be diets that are better suited to you or not suitable to you. What is the Paleo Diet? Risa says this diet has the broadest appeal and as result it is the most commonly recommended. Many people know of it as the ‘Caveman diet' because it focuses on the things that we ate at the beginning of time. This includes animal protein (not deep fried, of course), and some plant carbohydrates like sweet potatoes and yams. It also includes good fats like olives, nuts, seeds, and eggs. It doesn't include foods that came later like legumes, grains, sugar, processed foods, and alcohol. One of the other things to avoid on this diet is any food that might have pesticides, herbicides, chemicals, dyes, toxins (there are over 3000 approved for human consumption) or be GMO since these were also not around during primitive times. This diet is very good for anyone dealing with inflammation and blood sugar issues. PROS Helps regulate the blood sugar Good for skin Good for blood pressure and cardiovascular health Reduces inflammation Promotes weight loss Increases energy CONS Can be challenging for vegans What is a Ketogenic Diet? The ketogenic diet, also known as ‘Keto' is a diet that helps get the body into a ketosis by changing the fuel source from carbohydrates or sugars (the most common fuel source for our bodies) to fat. Typically, the pancreas creates insulin and glycogen and drives it into the cells. Then mitochondria uses those to make energy. By avoiding carbohydrates on the ketogenic diet, you change your metabolic state into ketosis which then uses fat as fuel. To stay in the state of ketosis, your diet needs to be 70-80% quality fats. The diet also allows for animal protein (since it has no carbohydrates), and a very small amount (less than 20 g/day) of low-carb vegetables. Anyone on this diet will need to drink a lot of water to keep the colon hydrated since the diet contains very little fiber. The diet should not be a long-term diet. Risa suggests 3 months as a good length of time for Keto. PROS Good for blood sugar regulation Good for diabetics Effective for weight loss Good for heart disease Helps with acne Improves brain fog and supports brain function CONS Hard to get sufficient fiber Difficult to stay in and track state of ketosis Not suggested for anyone who struggles to digest or absorb fat or has had their gallbladder removed. Difficult for anyone who needs to travel Not as good for women, especially if they have adrenal issues Not suggested for anyone with kidney issues What is the Autoimmune Protocol (AIP) Diet? The autoimmune protocol diet, often referred to as AIP, is similar to paleo and low lectin but was developed specifically to help those with autoimmune issues reset. It is especially helpful for those who have just been diagnosed and are experiencing a lot of symptoms but can also benefit those who do not have autoimmune issues. It's an elimination diet that is intended to last 30-90 days. The diet itself is very similar to the Paleo diet but it goes a few steps further as it also eliminates nightshades which include tomatoes, potatoes (except yams and sweet potatoes), eggplant, peppers (except black pepper) and Goji berries. The diet also removes some nuts, seeds, dairy and eggs. On this diet, you are allowed to eat animal protein, vegetables, sweet potato, yams and some good fats - so it's very restrictive. After the prescribed time, you can start adding foods back in, slowly and one at a time, to help identify specific issues that may cause problems for you individually. PROS Cleans out the system Helps identify potential triggers Anyone can do it and benefit, not just autoimmune Decreases systemic inflammation. Improves gut health Decreases blood sugars CONS Temporary solution as it only last 30-90 days (but it can be repeated periodically) Very restrictive What's in a Low Lectin Diet? Lectins are under the umbrella of antinutrients. They are found in the protective shell of seeds and some vegetable and fruit skins. It can be difficult for the digestive system to break down which can in turn cause inflammation especially if someone has leaky gut syndrome. We know that inflammation is the driver disease. The diet requires the elimination of a lot of fruit and vegetable skins and seeds including tomatoes, squashes, cucumbers and some tropical fruits like bananas, dates, and coconuts. It also excludes dairy, legumes, grains, cashews, and corn. Even grains that are gluten-free like quinoa are excluded. PROS Some find it easier than AIP with similar benefits Does allow for some sugar alcohols CONS Suitable for everyone Anyone with IBS, IBD or SIBO will need to further restrict the Low-Lectin diet to remove sugar alcohols. What is the Low FODMAP Diet? FODMAP is an acronym and it stands for fermentable oligosaccharides, disaccharides, monosaccharides and polyols which are short-chain sugars that get fermented in the system but they absorb water. As a result, they typically cause bloating and gastric upset. This diet recommended for anyone who has chronic bloating, has been diagnosed with IBS or IBD. The Low FODMAP diet is an elimination diet that usually lasts 30-90 days. Risa says you'll get the best results if you start off strict. You want to eliminate any foods that you personally feel causes bloating. You also eliminate cruciferous vegetables (cauliflower, broccoli, kale, cabbage, brussel sprouts) as well as some other vegetables like artichokes, onion, garlic and veggies with high carbohydrates. PROS Good for reducing inflammation Good for those with IBS CONS Very challenging Not many commercially produced foods available (more coming on the market now) What is a Vegan Diet? The vegan diet restricts all food that is of animal origin including byproducts of animals like dairy, gelatin, rennet, and honey. Risa is not a fan of the vegan (or even the healthy vegetarian diet) because she believes that every meal should have protein, fat, and fiber which, she believes, can be very challenging in a vegan or vegetarian diet. This is why she stopped being a vegan. The vegan diet is especially challenging for anyone with SIBO since most vegan diets include a lot of lectins and high FODMAP foods which will cause inflammation and bloating. The vegan diet, according to Risa, can also be challenging for regulating blood sugars and weight since it typically contains a lot of carbohydrates in the form of beans, nuts, and seeds. PROS Studies suggest it helps fight or possibly reverse/mitigate certain cancers and heart disease CONS Blood sugar needs to be monitored Not really great for anyone with the gene mutation MTHFR which prevents us from methylating or processing B12 and folate B9 Can be difficult to get sufficient fiber (women need about 25 grams per day and men need 38 grams) May be deficient in dietary Zinc May be deficient in omega3 fatty acids (which is often from fish consumption) May include consumption of GMO soy Mystery Solved As Risa explained, there are many things to take into consideration when selecting a therapeutic diet. We are all unique and our diets should be too. While it's helpful to look at some of these main rules, it's also important to listen to our body. I wholeheartedly believe over restriction is not ideal in the long run. Limiting things while we are fixing the gut, calming the immune system and supporting the body, may be valuable in the short term. We have to diversify our gut microbiome and that can only happen with a diverse diet, it's not just about probiotics. With Anna, we started with a low lectin, low iodine diet to help calm her immune response. We supported her gut and liver pathways while also optimizing her thyroid. This is a process that takes some time. When done correctly and in the right order (this is something I actually teach in my step-by-step Hashimoto's program) it is very effective. Conclusion Once we completed that, Anna felt so so so much better! Her energy improved, her IBS completely resolved and her thyroid antibodies came way down. We started to slowly introduce one new food at a time. The process of re-introducing foods can take a month or more but Anna was patient and abided by the motto, ‘slow and steady'. Happy Ending Anna was able to add back in a lot of foods. She now follows a gluten-free, dairy-free diet but is able to have grains, legumes, and lectin. She limits refined foods and sugars which is always a good practice. She finds this diet to be very manageable for the long term especially because she is feeling so much better. Eliminating Health Mysteries For Anna we were able to find that missing piece of the health puzzle and help her regain her health. As she traveled the world, Vivica would explore local and native foods. Vivica was following her dreams of practicing nutrition. However, she discovered that she was pre-diabetic. [10:15] Wrapping Our Heads Around Covid There is so much misinformation about Covid. As a practitioner, Vivica is looking for trustworthy resources. However, there’s not a lot out there! Finally, we understand how Covid makes people sick and who it makes sick. There are levels of sickness that we cannot treat as nutritionists. Eventually, people will need to go to a hospital. However, most people will benefit from nutritionists. A person with an intact immune system whose microbiome has resilience will not be affected by the ‘dangerous’ virus. In order for the virus to be dangerous, there needs to be microcirculation problems and metabolism issues. These conditions are making all the difference. Covid could actually benefit you by making your body stronger. A small percentage of people need to be quarantined, but that’s not the majority of us! The real virus is FEAR. [17:20] Using Plants To Heal Ayahuasca is the main plant that Vivica uses to heal. Practicing as a nutritionist, people would just heal from diet changes. However, some people would do everything right but they wouldn’t heal. It would drive Vivica insane! She wants all her patients to get better and heal everything. When they don’t heal correctly, Vivica takes it personally. Now, Vivica likes to find the deep cause of diseases. Her goal is to have as holistic practice where she can treat people as fully human. [22:10] Diving Into The Keto Lifestyle Seven years ago, when you said ketosis, people had no idea what you were talking about. Ketosis is the opposite of insulin resistance, so Vivica knew she needed to be in ketosis. You could say Vivica was an early adopter and researcher for keto. For Vivica, her focus had always been on whole, unprocessed foods as in the Paleo diet. However, she used her personal diabetic challenge as fuel for her research in the ketogenic diet to help Vivica control her blood sugar and hormonal issues. There is no cookie-cutter approach to health – you have to experiment and see how it works for you. [38:15] The Essential Carnivore Diet Cookbook There are a few different categories for the carnivore diet: Purist (meat and salt). This diet is for people who have severe gut issues. We are talking about clean meat, which is the least toxic food on the planet. You can find all the nutrients you need in clean meat. It’s an elimination diet and not meant to be done for extended periods of time. Ancestral (includes fruit and honey). What would a hunter and gather eat? It’s seasonal fruit and meat. Occasionally there would be nuts, but not often. You can’t use olive oil on the carnivore diet, so Vivica uses bone marrow fat. Cookbooks are helpful for people who have no idea how to make high-fat meals. Nutrient-dense healing food is what will heal our bodies. AND MUCH MORE! Should I Give Up Keto if it Skyrockets my LDL? [18:11] Charlie says: Hi Rob and Nicki, Big fan of the show. After learning about the many health and longevity benefits of keto from folks like yourself, Dr. Peter Attia, and Dom D’Agostino, I gave it a go for 4 months. I followed a clean, dairy-free version with loads of greens and cruciferous veggies, a variety of meat and fish, avocados, olive oil, nuts and coconut, you get the idea. I confirmed ketosis regularly with a blood meter. I felt great during the four months, and by the end even started making gains again with my kettlebells and barbell. The diet wasn’t a huge adjustment for me since I was already essentially paleo. After the four months, I got my bloodwork to compare to my baseline. My triglycerides and HDL remained excellent, but my LDL-C skyrocketed, and my LDL-P is fairly elevated. I am so bummed out and discouraged because I was so excited about all the longevity benefits of ketosis, but my PCP told me to go back to my previous eating habits. Here’s my question: do you have any ideas for ways I can achieve some of the benefits from ketones (BHB) while mitigating my LDL going haywire? Maybe intermittent fasting? Cyclic keto? Exogenous ketones? While the high LDL numbers scare me, I know from listening to your podcast as well as Dave Feldman that LDL isn’t the end-all be-all for cardiovascular risk. I find solace with my HDL and triglyceride levels, also my fasting glucose is very low, and my LP-IR score is below the low range, indicating I'm highly insulin sensitive. Any advice you have for me regarding my path forward with keto is greatly appreciated. Thanks guys. For context, I’m a lean, active 34-year old male. 2. 5-2 Fasting [29:09] Devon says: Hi! I was wondering if you had any good resources, guidance, recommendations or considerations (basically anything) about 5-2 intermittent fasting. It is difficult to sift through all the crap on the internet 3. Avoiding weight regain after losses [34:29] Sarah says: Hey Robb & Nicki! I was wondering if you two could have a nice discussion about the optimal way to avoid gaining back the weight you've lost while dieting. There's a lot of talk about the best way to lose weight, but not enough on what happens after. I know you'd say that to lose weight, to try a low carb diet with adequate protein and electrolytes and exercise, using fat as a lever, and everything will work itself out. However, during the pandemic, I ended up doing a VLCD, and I lost about 42 lbs from mid March to mid May. You'd likely agree that this rapid weight loss is problematic because of lean tissue loss. I'll tell you right now, I wasn't eating adequate protein. The food I ate was primarily protein, but it wasn't much, some days a few meatballs... Also I chose not to exercise during this time. I'm sure I'm not the only one out there who gets impatient and isn't satisfied with losing .5 lbs per week even though it's the most sustainable way to lose. So anyway, I did some research (lost job because of Covid, no kids, lots of time on my hands), and I wrote it all down in a comprehensive plan to avoid gaining the weight back in the summer and moving forward. I was wondering if you might like to consider taking a look? And perhaps bringing some criticism/comments/tips for the the other five listeners on what we can do to stabilize after weight loss. (I'm a woman, 37, currently 208 lbs, besides obesity, all healthy biomarkers, I normally enjoy crossfit style workouts, once a week or less - not hardcore enough to build lots of muscle, and mini-triathlons in the summer) Thanks!! Notes: Conclusion: A diet consisting of high levels of animal-based protein combined with a lifestyle of consistent strength training should go a long way to help the dieter not regain the fat mass that they have lost. A general recommendation is to eat a diet of 35% protein, 15% carbohydrate and 55% fat. The carbohydrate and fat can be adjusted based on how addicted a person is to certain foods, and how they see their body responding during refeeding. An app like MyFitnessPal can calculate the percentage of macronutrients you are consuming. 4. LMNT/Sodium and Breast Milk Production [44:47] Hey Robb and Nicki! My partner Garrett and I have been huge followers of you since the beginning of our relationship, 11 years ago! We just welcomed our second daughter about two months ago. I'm exclusively breastfeeding and I've noticed an increase in milk supply while drinking LMNT. Is it safe to drink it more than once a day or is that not recommended? Thanks, Margo 5. Low T3 Levels after long term keto adaptation [49:24] Diva says: Hello Rob and Nikki, First of all, thank you so much for the wonderful work that you two do! I would like to know what are your thoughts on low T3 levels after long term Keto adaptations. According to Phinney's work, amongst other evidences, it seems that it is physiological normal for Keto adapted people to no longer require high free T3 levels, according to standard recommendations. I am a 44 year old athlete. I have been Keto adapted for over 12 years. I was diagnosed with hashimoto hypothyroidism 25 years ago. My antibodies have been under control since after starting a Keto Paleo lifestyle and I've able to reduce my T4 dose considerably as well. However, my free T3 levels are very low, below the standard recommendations of 2.0 I am COMPLETELY asymptomatic though. My body composition is great, I'm very muscular and carry 9% body fat effortlessly throughout the years. Despite knowing about the T3 efficiency that Keto adapted people acquire, about two years ago I decided to experiment with adding exogenous T3 to my T4 dose. I noticed no difference on energy levels but I did notice that I started losing muscle mass! Which makes sense because one of the reasons that T3 may be lower on Keto is due to muscle sparring. I stopped taking T3 about one year ago and have gone back to T4 only since then. But one trend that I noticed is that my fasting insulin has increased a little. Nothing much. It used to be around 3 and now its 5.3. This started happening after I added T3 to the mix. I have since stopped taking T3 but my insulin has not gone back to levels that it used to be before taking it. I know that artificially raising T3 levels may raise insulin but shouldn't it have gone back down after stopping the med? I wear a CGM and my blood sugar control is stellar! I can handle carbs very well, I've been testing with that, and even after eating tons of carbs, like 200g at one seating, my blood sugar didn't move past 13 points. The maximum increase was 105. And I'm not kicked out of ketosis. Other than that my average blood glucose is about 85. The only times that it goes up is after high intensity exercise, then it goes above 150, 170 sometimes! But I know that this is physiologically normal so it doesn't concern me. My question is: what are your thoughts on the T3 efficiency after being Keto for so long? Should we try to artificially push up FT3 levels despite having no symptoms? And should I worry about a fasting insulin of 5.3 despite obviously being super insulin sensitive?? I am trying something a little different here and I'm not actually having a guest on with me today. I will be discussing the three pillars of success. Now, this is a concept that I've put together over the past few months that I truly believe will help anyone reach any goal that they desire to achieve. Fitness can be defined as an increased work capacity across broad time and modal domains. In other words, as we choose to participate in activities that are going to increase our performance, our speed, our ability to do certain things, then our fitness level is increasing and our health is judged by the amount that we do overtime of increasing our fitness level. "Success is really nothing more than the progressive realization of a worthy ideal. This means that any person who knows what they're doing and where they're going is a success. Any person with a goal towards which they are working is a successful person." - Earl Nightingale Other things to consider when trying to increase our knowledge would be a nutrition exercise, activities, a different diet strategies that we may want to try and see if that works for us. Whether that's Keto Paleo, Whole 30 macro counting or just simply intuitive eating. There are many things that we can use to implement that will then be able to help us achieve our goals. Now, there is no one size fits all and this may be something that I repeat over and over again, but as individuals, there are basic principles that apply in general. However, when you type your numbers into a calculator to figure out how many calories you have to eat, that number is just a baseline. Everyone reacts differently to different things in different levels. The concepts are similar, but in practice, that's where we need to figure out what works best for us. Show Notes: 1. In & Out of Ketosis...Harmful? [2:10] Pete says: Robb, I've been on and off a keto diet for the past five years or so and when in Ketosis feel fantastic. I train muay thai and on days that I train I tend to up my carb intake as I just feel better throughout training, similar to you with jiu jitsu. On Sundays however I do tend to go off the rails a bit and find myself on Monday down around the 0.2 mmol/L level and then by Tuesday night/Wednesday morning back up any where between 1.0-2.4 mmol/L where I stay for the rest of the week. I have in the past gone several months at a time on a 30-50g carb per day diet on multiple occasions, haven't done so the past year and half or so. I've been hearing a lot lately that this quick fluctuation between low carb to moderate/high carb intake can be very bad for you. Can you help shed some light on this, am I doing long term damage to my body by fluctuating? Thanks, Pete 2. Eating Below My BMR [10:48] Chris says: Hey guys, Over the past 7 months, I've had great success following the Keto Masterclass. As far as weight-loss goes, I've lost about 42 lbs, and lowered my BF% from about 28% to about 19%. For the first 12 weeks, I ate at a 20% deficit, without a break. I weighed and measured all my food. I made sure I had my electrolytes dialed in, eating lots of potassium and magnesium rich foods, supplementing Na+ as well as Mg+ too. During that time, I was lifting 4x per week as well as a couple (15-20 min) interval workouts per week. Other than that, I was doing some light walking and playing with my kids. Towards the end of that initial run, I started to get kind of bitchy (I'm a 45-year old male), and never really experienced the stable energy, or deep, solid sleep that a lot of people mention - two things I was really hoping for. Recently, I have been alternating 4-week cuts, followed with 2-week "full diet breaks" (a'la Lyle McDonald) at maintenance. During those, carb sources have been clean, paleo foods. Coming off of each of these, I feel better and maintained a stable weight throughout. I was normally back in ketosis 48-72 hours after switching my carb and fat macros back up, and both times (so far) it has restarted weight loss. Still not great sleep, but I attribute that to having young kids (2 & 6). After a recent DEXA scan, I was recalculating my macros with the Ketogains calculator (I'm 162# and 19% BF), and it dawned on me that the deficit it was recommending (-15%) was put me BELOW my Basal Metabolic Rate by about 105 calories, per day. For reference, I used the "sedentary" activity level and don't add back in workout calories. So, to my question(s)... Understanding that the calculator is based off the Katch-McCardle formula, basically giving a statistical norm, but BMR is the calories we need to maintain vital physiological functions, what are your thoughts on recommended deficits putting one below their BMR and what are the longer-term implications of extended diets doing so? Would it be better to just eat AT BMR on rest days and add back in some (all? a portion?) workout calories (protein & fat) on training days, while still being below TDEE? It seems to me that BMR should be an absolute floor when it comes to caloric intake - I'm wondering if the aggressive deficits created some diet fatigue and hormonal disruption for me. Gary Taubes is famous for saying that calories don't count and in my anecdotal experience, they don't. I must stress that since it works for me, I'm completely happy with the results. However, when trying to speak intelligently about Keto and low carb, I'm trying to bridge a gap from Gary to Chris Kresser's podcast with Joe Rogan last week. On Joe Rogan Chris said that you *must* run a caloric deficit to lose weight. Now I am really confused! I'm an engineer and I've taken several thermodynamics courses, so from an energy balance equation I understand that the human body cannot defy thermodynamic principles. But obviously our metabolism is much more complex than an energy in- energy out black box. Furthermore, energy in-energy out does not feel right for a number of reasons such as, for instance, energy expended drinking cold water is not in this equation (that is your body warming the cold water up), energy that is never consumed but rather part of a "defecation event", etc. I have eaten what I perceived to be a major energy surplus on a ketogenic diet and have still lost weight. My caveat there is that I wasn't weighing foods, so I cannot really know. My apologies if you've already answered this question, but I could not find it on your blog or searching your website. I'm hoping you can point me to a study, a white paper, a text or some reliable information that will answer the question of who's right- Gary Taubes or Chris Kresser. Thanks Robb, you're a legend. Sooooo, it's been a little while since I have been devouring each and every single podcast episode and I apologize. My own business focus has put me more into the writing podcast genre recently (but I see Tim Grahl on your interviewee list!?). :-) So, my question: have you guys laid out a protocol for extreme nutrient buildup for before and after a scheduled surgery? This would be for someone who is not really paleo: still eats gluten and mostly just avoids sugar and junky foods. So they are not yet on the "rawr sardines nom nom!" bandwagon, sadly. But if they have a willingness to make some changes to ensure they are in a better place to prepare to recover from their surgery (jaw replacement, if it matters), what would you 'prescribe' to them? CrossFit vs 5 by 5 (ketogains) [38:12] Matt says: Robb and Nikki thanks for all that you do. You, your books and podcasts have changed my life. I have done two days a week of CrossFit for just over 2 years and in spite of the amount of rest I get I still end up with knee, shoulder, wrist and elbow injuries and pain. I have recently decided to pause my CrossFit subscription and have replaced it with the three days a week 5 by 5 routine recommended by the ketogains guys. I did your keto masterclass and I'm at 7.5% body fat and try to keep my macros on point with the ketogains recommendations. What do you think about just doing the 5x5 home workout alone? I do like the group element of the CrossFit class but at my age (52) I can't seem to go for long without injury or constant pain. I feel like the 5x5 program at home is more manageable as I can control the velocity/intensity without so much emotional effort. What are your thoughts regarding this path?
