Let’s talk about testosterone testing and other important blood tests you can track for your overall health, wellness, vitality, and longevity. I recorded this show on the occasion of my first visit to the blood lab after signing up for Vitals Vault’s premium program, and I talk about some of my favorite markers while pedaling home from the lab. I focus a lot on testosterone because I believe it’s a great proxy for overall wellness, vitality, recovery, and resilience to stress. I share my own numbers, which have ranged from 562 to 1,004, and explain why I’m so intent on keeping testosterone levels high as I age. I also get into the roughly 1% annual decline in average male testosterone levels since the 1980s, vitamin D and the widespread fear of the sun, why looking beyond LDL can give you a better picture of your lipid panel, and the lifestyle practices that can help optimize testosterone—from sleep and diet to avoiding chronic cardio, lifting heavy things, sprinting once in a while, and even maintaining a winning relationship. TIMESTAMPS: Testosterone testing is the best proxy for your overall state of wellness, vitality, and recovery or resilience to stress. [00:51] Brad checks his testosterone level frequently because it is his way of monitoring the stress impact from his training. [5:41] Triglycerides to HDL ratio is a premier indicator of your heart disease risk factor. It is much better than looking at total LDL cholesterol. [06:42] In 2017 they lowered the ideal level of testosterone due to the widespread decline of testosterone among males around the world. [09:02] If you are not optimizing your sleep, including down and rest time, you cannot reduce your stress. [12:13] Get the junk out of your diet. A Note from James:I'm doing a big experiment in my life.I recently started taking a GLP-1—tirzepatide, one of the so-called weight-loss drugs. But I didn't start taking it specifically for weight loss. I had some blood work done for the first time in a long time, and there was a small indication that I might be pre-diabetic. My doctor suggested trying tirzepatide, and I wanted to understand what I was getting into.The first person I wanted to talk to was Charles Duhigg, author of The Power of Habit and Supercommunicators. Charles has been taking a GLP-1 for about two years, and he says it has changed his life. He has lost around 45 pounds, but what interested me most was not just the weight loss. It was the habit change.Charles explains that GLP-1s can quiet what people now call “food noise”—the constant background pull toward eating even when you are not actually hungry. When that noise quiets down, something interesting happens: you get a window where your old habits become more malleable.That is the key idea of this conversation. If you've spent any time in a gym, you've heard it: you need carbs to build muscle. It's gospel. Every bro coach and most personal trainers will tell you the same thing. The problem is it's not actually true. And my guest this week is living proof. Robert Sikes is a pro natural bodybuilder who has been ketogenic for 11 years, eating fewer than 20 grams of carbs a day the entire time. No rice, no grains, no oatmeal, none of the stuff bodybuilders are supposed to live on. And he competes and wins on the world stage. In 2023 he got down to 3.9% body fat, making him the leanest man on earth at the time. He's also the founder of Keto Brick, a shelf-stable high-fat food he originally created for his own competition prep because nothing like it existed. In this episode, we get into the carb and muscle myth, what 11 years of keto has actually done to his blood work, and the tradeoffs nobody on social media wants to talk about. The argument for carbs usually comes down to insulin, which is a powerful anabolic agent. Everything grows in the presence of insulin, muscle included. But as Sikes lays out, you don't need excess insulin to build muscle. You need a caloric surplus, enough protein, enough training intensity, and enough recovery. The rest is just body fat you'll have to lose later, and the athletes chasing muscle through exogenous insulin often pay a steep price for it. The same myth applies to testosterone. People assume Sikes must be running sky-high levels to look the way he does, but he isn't. He competes in natural federations that don't allow TRT, and his testosterone typically sits between 400 and 600. When he was at 3.9% body fat, it dropped to 87. That's not a level anyone would call healthy, but it proves you don't need elevated testosterone to build and hold muscle. I've experimented with TRT myself and walked away from it, and we get into why the negative feedback loop made it not worth it for either of us. Where the conversation really opens up is the philosophical side. When do you intervene and when do you accept what nature gave you? We talk about genetics versus lifestyle, why he doesn't try to cheat death, and why an athlete's identity shouldn't be wrapped up in their physical abilities, because we're all biodegradable vessels of meat and bone that eventually deteriorate. His whole family eats this way too, including his kids, whose first real food was a beef knuckle bone rather than rice cereal.
