Therapy on the Cutting Edge

In this episode, Haim discusses his extensive experience providing group and being an early adopter of group therapy in 2006, which lead to his work on the effectiveness of telehealth. He discussed how in 1995 he started an internet forum called GP Listserv which now consists of 400 group therapists from all over the world who join together to discuss group therapy. Haim talked about his group analytic relational approach to therapy and discusses the difference between psychodynamic and relational approaches. He talked about how the research has found clinicians experience four obstacles to online therapy, and discusses the various ways one can address them. He explained that these obstacles include (1) Setting (2) Disembodied Environment (3) Presence and (4) Ignoring the Background in Online Settings. He discusses the body of research surrounding online therapy which is very promising and explains how a meta-analysis concluded that for individual therapy, there is no difference in the results for online vs in person therapy. According to research, the best predictor of success in individual therapy is the therapeutic alliance and goes on to list the three components of the therapeutic alliance which are agreeing on goals, agreeing on tasks, and bonding. While online group therapy is less researched, Haim's experience suggests that it is very effective. He explained how in group therapy, the factor that is most predictive of success is the cohesion of the group, rather than the therapeutic alliance. He goes on to discuss how the preference of modality –such as having an auditory or visual preference– influences one's ability to effectively do online therapy. He talked about how in groups, time, space, and attention are always shared, which can create turmoil for group members. He also discussed how keeping attention can be difficult online because there are more distractions and people may feel more unnoticed, which can promote disengagement in an online group setting. He recommends that small group therapy should consist of group sizes between six and ten people and for people who are not as skilled or experienced, groups should not exceed eight people. In 2018, Haim created an online training process group for therapists, and these groups consist of therapists from all over the world where they discuss their experiences with group therapy and learn how to improve their practice, but also use them to work on their personal issues, thus providing professional and personal growth. Dr. Haim Weinberg is a licensed psychologist in California (PSY 23243) & Israel and has a private practice in Sacramento, California, with more than 40 years of experience. He is also a group analyst and Certified Group Psychotherapist. He is past President of the Israeli Association of Group Psychotherapy and of the Northern California Group Psychotherapy Society, and list-owner of the group psychotherapy professional online discussion forum. Haim was the Academic Vice-President of the Professional School of Psychology in which he created and coordinates an online doctoral program in group psychotherapy and marital therapy. He published books on Internet groups and about Fairy Tales and the Social Unconscious, and co-edited a book about the large group and a series of books about the social unconscious. He is on the clinical faculty of Psychiatry at UC Davis Medical Center and Fellow of the American Group Psychotherapy Association and of the International Group Psychotherapy Association, as well as a Distinguished Fellow of the Israeli Group Psychotherapy Association. He has received several awards including the Harold Bernard Group Psychotherapy Training Award and the Ann Alonso Award for Excellence in Psychodynamic Group Therapy. He also co-edited the books: 1. Theory and Practice of Online Therapy: Internet-delievered Interventions for Individuals, Groups, Families, and Organizations. 2. Advances in Online Therapy: Emergence of a New Paradigm. 3. — Dr. Ronald L. Mann provides individuals the tools to move beyond the limitations of body identification with the more limited definition of self that is defined by one's thoughts, feelings, emotions, and physicality. Being “awake” has a much more profound meaning than just getting up in the morning after a night's rest, drinking a cup of coffee, and getting started with your day's activities. It is realizing a more expanded state of self beyond the limited “ego” to a higher sense of Self or the soul. Consciousness wants to evolve through us. We are all on a journey of returning to the source of Love. On our way, we can either coast along with the current or use a paddle to speed our journey. We are all going home. Valeria Teles interviews Dr. Ronald Mann — the author of “Sacred Healing: Integrating Spirituality with Psychotherapy.” Dr. Mann has been helping people awaken their consciousness for over forty years. Balance is attainable, but it's not easy. Here are some paths to get there:Focus on prevention through exercise, sleep, nutrition, and support.Be grateful if your department supports work/life balance.Assertively let your supervisor know if you need a little break or some help.Ask successful colleagues how they do it, and learn from them.Don't buy everything the media sells about achieving balance.Recognize "ebbs and flows" of balance, rather than expecting a constant state.Track the hours you spend on each activity, and then make changes.Breathe slowly and consciously a few times a day.

#35 The Dirty Little Secret Behind Mental Fatigue

When it comes to getting through a week, nothing is more exhausting than being mentally spent.  There are numerous articles and videos out there addressing how to offset muscular fatigue, but during podcast #35, Coach Robb takes a look at the big 10-pound muscle on top of your shoulders – your brain – and helps you get a clear understanding why mental fatigue sneaks up on you when you least expect it!  During the first half of the podcast, Coach Robb drills down on the question “What is the last thing you wrote down and created a strategy to achieve in the last thirty days?”.  After answering this question, he challenges listeners to identify what rewards were attached to the achievement. Over the last 35 years of working with individuals and athletes, Coach Robb has never had an individual answer this question with a definitive answer.  When it comes to the physical signs of fatigue such as tired muscles, cramping, slower speeds, less strength, etc. we know what to do – eat and sleep to recover.  But, when it comes to mental fatigue, many of us are stumped. The symptoms are obvious –  feelings of being lazy, frustrated, aggravated or even depressed, but the steps to turning around the symptoms are not quite so easy. Coach Robb looks at three critical components of mental frustration: not having a quantified and/or detailed achievement; never feeling like your results are “good enough”; and, constantly feeling dissatisfied (i.e. thinking it’s cool to be pissed all the time – “that sucked”) because you keep moving the target identified as “good enough”.  During the second half of the podcast, Coach Robb does a deep dive into the four quadrants of life that we all balance on a daily basis (sometimes not even realizing it) –  Personal (food, sleep, sexual intimacy), Professional/School, Financial, and Health, Wellness & Performance.  Though we get on the daily hamster wheel, we rarely stop to answer questions like What does my perfect day look like?  What is my definition of perfect result?  and, What is my reward for accomplishing my desired result?  The result is the vicious cycle of never being satisfied (or achieving a quantified achievement) which begins to wear the brain out – literally.  By the end of the podcast you will understand the difference between physical and mental rejuvenation and what you can do to offset the negative effects of mental exhaustion.  You will also be equipped to take an intangible feeling or emotion and create a tangible achievement. This is something your brain can resonate with and feels rewarded when implemented correctly.   Listener questions include: What should I eat during my races to avoid getting physically sick?;  What do I do about a know in my rib cage that keeps me from breathing deeply?; What do I do to avoid getting up 2-3 times a night to urinate along with struggling to fall back asleep when I am finished?; What is the ideal sleep cycle?  and, What do I do when my schedule keeps me from completing today’s workout – discard or move to tomorrow?  Here are the Amazon links to Coach Robb’s books on Kindle and Paperback: Coach Robb’s Smoothie Recipe Book – on Kindle and in Paperback https://www.amazon.com/Coach-Robbs-Smoothie-Recipe-Book-ebook/dp/B07R26DMCZ/ref=sr_1_fkmrnull_2?keywords=Coach+Robb%27s+Smoothie+Cookbook&qid=1557491306&s=gateway&sr=8-2-fkmrnull Coach Robb’s Healthy Meals Recipe Book – on Kindle and in Paperback https://www.amazon.com/Coach-Robbs-Healthy-Meals-Recipe/dp/1095573381/ref=sr_1_fkmr0_1?keywords=Coach+Robb%27s+Smoothie+Cookbook&qid=1557491397&s=gateway&sr=8-1-fkmr0 Coach Robb’s Additional Websites:   CoachRobb.com CoachRobbStore.com NutritionallyGreen.com CompleteRacingSolutions.com CompleteRunningSolutions.com CompleteTriathlonSolutions.com CompleteSwimmingSolutions.com CompleteWeightLossSolutions.com

Here are some key points: The application to medical, dental, or veterinary school begins on Day 1 of college! During the transition from high school to college, you'll want to learn how to learn deeply. Making good grades does not equal learning. Take Gen Chem as soon as possible to stay in line with the sequence of courses. College is about growth and development. Reflect on why you're pursuing the activities you're doing, what you're getting out of the experiences, and how this will make you a better healthcare professional. Build your network. Engage with professors, TAs, peers, and health professions advisors. Keep a record of your activities and what you take away from these experiences. Think positively!
