Their deep inner conflict is rooted in a profound trust and betrayal wound, which makes it challenging for them to feel secure in relationships.• The Push-Pull Dynamic: FAs often seek connection, craving closeness, but then feel misunderstood or betrayed and instinctively push their partner away. This paradox creates a cycle of emotional highs and lows in their romantic relationships.Where Attachment Begins: The Childhood of an FADr. Hensley traces the origins of FA attachment back to childhood, often shaped by the most trauma-filled environments. These traumas can include:• Big T traumas (e.g., abuse, neglect, abandonment).• Little T traumas (e.g., invalidation, inconsistency, emotional neglect).She also emphasizes that FAs typically grow up with at least one emotionally dysfunctional caregiver, further complicating their ability to trust and feel safe.The Role of Parents:• Same-gender parent: Science suggests this parent leaves the deepest imprint on the child's attachment patterns.• Both parents: Combined, their behaviors create the template that teaches the FA that love is not safe, leading to deep-seated trust and betrayal wounds.The Negative Traits of an FADr. Hensley breaks down the challenging traits of FAs, especially how they manifest in adult romantic relationships:1. Hypervigilance:• FAs are highly attuned to subtle cues but often catastrophize them.• This makes them extremely sensitive and easily triggered, as their deep wounds leave them vulnerable to perceived rejection or betrayal.2. Trauma Responses:• Anxious-leaning FAs tend to react with anger and heightened emotional reactivity.• Avoidant-leaning FAs often dissociate, emotionally shutting down after feeling they've tried too hard for too long.3. Negative Bias in Relationships:• FAs often approach their romantic relationships with a belief that betrayal is inevitable, leading to high levels of criticism and difficulty repairing conflicts.4. Poor Partner Choices:• Subconscious patterns often lead FAs to pick emotionally unavailable or harmful partners, such as other FAs, dismissive avoidants, narcissists, or individuals struggling with substance abuse.The Positive Traits of an FADespite their challenges, FAs possess incredible strengths, especially once they begin healing:1. High Empathy:• Their ability to deeply understand and feel for others allows them to create profound connections.• Dr. Hensley cautions FAs to establish boundaries around their empathy to avoid co-dependency or self-abandonment.2. High Emotional Intelligence (EQ):• FAs are often exceptional in careers requiring deep emotional awareness and social skills, such as sales, law, or politics.3. Depth and Connection:• As deep feelers, healed FAs can foster rich, meaningful relationships that are both emotionally fulfilling and long-lasting.Final ThoughtsDr. Hensley concludes the episode by emphasizing that while FAs are complex and deeply wounded, their capacity for growth and transformation is unparalleled. They are true fighters, with a grit that comes only from thier deep feelings and high emotional IQ's. Let's face it, the state of the world is less than desirable in many ways, yet there's also much movement and change that's empowering and life-changing. What is happening in the world at large echoes what happens in our lives. We see negativity and focus on it, but there is another option. Melanie Lush, from Florida, wrote in and said, “Danielle, I recently found your podcast and listened to an episode where you were talking about focus. Before I listened to the episode, I thought I struggled with focus. But, now I realize I don't struggle with my focus; I struggle with focusing on the wrong things! If something has a bit of drama to it, is negative, or I feel like I don't have a choice about the situation, I will laser focus. I spend most of my day in negative thoughts. First, thank you for opening my eyes to this because now that I know, I want to change it. There’s an old adage in the customer service world that says, “Nobody calls to say they love you.” These are true words, and they apply well beyond call centers at banks and cable companies.In general, people tend to use the comments section of the Internet, and any place where they can say things, to talk about how bad things are. When given it, humans will take the opportunity to complain, share horror stories, look for confirmation that their disasters are in fact disasters, and to share misery. It would appear that this is how we are wired, so I’m not blaming anyone or finding fault in this.Please keep in mind that this natural tendency toward the negative applies not only when reviewing a car dealership or restaurant. This applies in mental health and anxiety as well, which is a very important thing to remember if you’re reading this right now.If you go online to find out how others are handling anxiety, panic, intrusive thoughts, or any related problem, expect to find complaints, horror stories, disasters, and a torrent of misery. Why? Because people will take to the Internet to speak their problems out loud in the hope of finding a solution. The issue is that we are long on problems and generally short on solutions or solution providers, so we wind up with an avalanche of negative, scary, stories that will make you think that you have no hope of ever getting better. Everyone will join a Facebook group to talk about how bad they feel. Nobody will join a Facebook group to talk about how they are NOT anxious. When you also join a Facebook group to talk about how bad you feel, you will be confronted with all the other people that are doing the same. The same holds true in the comments section on YouTube and Instagram, or on Twitter or Reddit. You will see LOTS of negative and very little positive. When people get better they generally stop going on the Internet to talk about anxiety. When people are making progress in recovery, they tend to not need to commiserate any more. When anxiety isn’t a problem any longer, there’s no reason to talk about it, so people don’t. People simply do not call customer service for help if there is no problem. This does not mean that nobody gets better or that it is near impossible to get better. It just means that very few people get better then come back to tell you about it. This makes the Internet look like confirmation that you are doomed, when nothing could be farther from the truth.Why should you care about this situation? Because when you are searching for hope and find only nightmare stories, an anxious brain is especially vulnerable to drawing incorrect conclusions that center on never getting better, being anxious forever, and losing everything as a result. This is to be expected, but this is also not necessarily accurate.So when you find yourself Googling, forum surfing, and diving head first into the deep end of “anxiety support” on the Internet, please keep this bias in mind. You are almost always going to see far more negative than positive dialogue, but this is not reflective of reality in any way. Then, maybe pull back and stop looking for support so much if you find that it’s making you feel worse or more helpless.If you hate hamburgers but love tacos, you might not go into a McDonalds because McDonalds has hambugers. The good news is that there are still tacos in the world, even when you can’t see them in a McDonalds.When negative posts, horror stories, and the sharing of suffering negatively impacts you, maybe step away from the Internet, because the Internet has those things. There is still recovery in the world, even if you can’t see it from the comments section.Everyone has to make their own decisions. I still believe in that. Negative BiAS Buzzword or BS Before you, well someone might, shout out that Negative bias is a real thing - I know but don't you think there's way too many using this as an excuse or a thing we have to put right because it makes them So So right and it's the popular 'Click Bait' thing to call out. And no I'm not going to talk about that part of the BS I'm going to explore what it really is and what it really does for us, yes the positive in a negative! And then........ With that understanding look to see and hear here what to do when that positive negative is a negative negative. Doesn't that sentence make you just want to dance with joy - Or maybe it's just me and t my unplugged mind  :O) Go on come with me and find out the 'HOW' we can change this negative bias when it gets in our way ad is totally inappropriate. When I am shopping on Amazon, and a product has 3.5 stars, I don't buy it. Now, 3.5 stars isn't a bad rating; in fact, it's above average. However, for me to place that item in my Amazon cart, it needs to have a rating of at least four stars or more. My behavior on Amazon is part of a broader concept called Negativity Bias, and it's a massive influence on customer behavior.  This episode of The Intuitive Customer discusses the concept of Negativity Bias and how it affects people's behavior in your Customer Experience. To prevent the potential bad outcome, we take specific actions to avoid it. These actions could include scrolling through customer reviews (focusing on the bad ones), buying only from brand names we know and trust, or looking for referrals from friends and family.   While a little Negativity Bias can do good, too much only leads to bad.  A focus on the negative can lead to good things, like process improvements or problem-solving exercises. However, too much stress there can lead to a skewed world view. It can lead to making poor decisions or missing the good things that are happening in an experience.  Recommended Actions There are a few ways that you can use the Negativity Bias to get a good outcome. These include: Perform periodic checks of negative assessments to ensure that you have a realistic view of the situation. It is essential to determine if the negative feelings you have about your customer experience or business performance are based on objective measures or not. It's great to use Negativity Bias to motivate improvements, but it shouldn't squeeze the life out of your organization either. Remember to highlight success and reward good outcomes whenever possible.  Remember that everyone tends to focus on the negative, including customers. When you understand how it works for people's perception, you can counteract its effects with specific tactics. Look for ways to mitigate the Negative Bias' poor outcomes for your bottom line by addressing it in your Customer Experience.  Provide people with solutions in today's negative environment. Without a doubt, a global pandemic inspires quite a bit of Negativity Bias. Your customers feel it, and if you can find ways to be helpful in light of the present challenges, it will pay off later when customers remember this time. In other words, you can build customer loyalty by taking extra care of customers right now when they need you.  Communicate positively. Understand how customers can glean the negative much more quickly than the positive and take special care to positively frame your messaging. Whether in direct customer communication channels or advertising, keeping the message positive is essential to inspiring customer-driven growth. An outside perspective can help you maintain positivity in your experience. Our Experience Health Check, where we pretend we are customers and then give you specific recommendations to improve your experience, can give you a fresh set of eyes on your communication efforts throughout the customer journey.  Remember that fixing problems is good, but not always the best opportunity in your experience. Organizations often want to focus on what is going wrong in experience and fix it. That is a good instinct. However, many times in our Emotional Signature Research®, we discover that customers have other wants and needs that present a much more significant opportunity than fixing the various problems in the experience. Look for ways to mitigate the Negative Bias' poor outcomes for your bottom line by addressing it in your Customer Experience.  Provide people with solutions in today's negative environment. Without a doubt, a global pandemic inspires quite a bit of Negativity Bias. Your customers feel it, and if you can find ways to be helpful in light of the present challenges, it will pay off later when customers remember this time. In other words, you can build customer loyalty by taking extra care of customers right now when they need you.  Communicate positively. Understand how customers can glean the negative much more quickly than the positive and take special care to positively frame your messaging. Whether in direct customer communication channels or advertising, keeping the message positive is essential to inspiring customer-driven growth. An outside perspective can help you maintain positivity in your experience. Brian Mayer takes at how negative we can be as human beings and how reframing to a more positive mindset can shift what happens in your blended family.  Science says that negativity acts like glue in our brains and that positivity is like a slip and slide.  In other words negativity often is hard to get rid of and positivity is hard to retain.  We will talk about this along with some strategies to shift into a more positive frame of mind.  We hope you enjoy today's message.  For more information and additional resources please visit our website at http://www.theremarriedlife.com Today's Goodies If we are honest with ourselves, we often will stay focused on negative aspects of our spouses or kids behaviors and conversely spend little time thinking about the positive.  Think about the last time you went out to a restaurant that was mostly a great experience but with a few things that either could have been better or that were not good at all.  How do we respond when someone asks us about our experience?  Generally we will say something like, “Overall it was a good experience, but the bathrooms were not clean, the server didn't bring my request for garlic aioli, or my order of fries were cold.  Isn't it interesting in that example above that we speak in generalities about the positives, but get really specific about the negatives.  We do the same thing in our important relationships.  We might say he is a good husband, but I don't like that he leaves the toilet seat up or he is generally a good stepson except that he treats my daughter terribly. What if we thought and spoke differently.  What if we said the quality of the beef in the burgers was tremendous, our server was extremely friendly, and the price to quality was very good, and really there was just some minor issues we had that we didn't like.  Same goes for our relationships.  What if we said, my stepson is very thoughtful with most people he encounters, he gets great grades academically and somehow is able to deal with the transitions well when coming from his father's house to us, but he does have difficulty in a relationship or two.  This slight subtle change in what details you give can enhance what others think about you but can also help your attitude and mood toward those in your family shift toward the positive as well.  Why is this hard to do though?  According to an article in Psychology Today that I will post in the show notes, called Our Brain's Negative Bias there is evidence that our brain is more skewed toward holding the negative.  In an experiment by John Cacioppi, Ph.D. while at the Ohio State University, he that the brain fires more when presented with negative news far and above when it presents positive news.  This probably goes back to the days when we were so called cavemen and cavewoman and had to fend for our lives against animals that might be our predators.  Our brains needed to be much more alert to the negative because this meant a possible life or death situation.  So if I am honest with you, I have what I consider a filing cabinet in my brain.  In it I will place all the positive and negative experiences there.  Then from time to time, without me fully being aware or conscious I will open the file cabinet and search the files until I find a negative one.  I will put out the file, lay out all the papers on the desk and then study them intently for many minutes.  At some point I realize what is happening and I have to mentally collect all of the papers, put them back in the file and lock the cabinet.  But what if we took this a step further.  Here is one recommendation I have for you to begin to change your negative thinking about your spouse or your children or stepchildren.  Take the mentally negative thought and write it on a piece of paper.  Ponder it for a few minutes making the mental decision that rather than it be placed back in the file cabinet, that it needs to be destroyed forever.  Then I would suggest doing something like burning it up so it changes form and that you have a visual of it being changed forever.  Secondly, when you notice having a negative thought about a family member, think about what is actually good about what is happening.  Let me give you a real life example.  At some point I started to think that I initiated a kiss more times than my wife did with me.  I remember my attitude toward her started to sink.  I even went so far as to tell her about my thoughts and this of course did not go well.  Slowly over time I decided to start thinking about what was good about this.  I began to realize that everytime I initiated the kiss, she would never reject that.  I began to feel much better and even happily pointed this out to her. Thirdly, think about your first mental thought about something is presented to you.  Is it negative or positive?  Keep a log of when someone asks you something and whether or not you think about how something won't work or why it can't be done or instead are you thinking about the possibilities and why something will work?  Scientists call the negative response being cognitively lazy in that when we are negative we can then choose not to do anything which feels less taxing and draining than saying yes to something that requires us to possibly change.  Why is this important in a blended family?  It is so important because we often do not realize the volume of negativity that has been uploaded into our brains over time.  From the disappointment of a previous marriage that has been dissolved, a partner rejecting you, or maybe a child choosing to live with the other parent can all begin to rewire your brain.  I would highly recommend that you listen to my Mindfulness Mini-series which are specific episodes on how to retrain your brain to begin to recognize your thoughts and also how you are reacting and responding in the here and now.  Those episodes are Session 40, Session 41, and Session 42.          Resources: Our Brain's Negative Bias Intro to Mindfulness for Remarriage Part 1 - Session 40 Individual Mindfulness Exercise Part 2 - Session 41 Couples Mindfulness Exercise Part 3 - Session 42   Thanks For Listening! Longevity Health Institute For Ourselves #IYP Oh Boy, in this Episode I can barley catch my breath as I was just trying to get all the info that has been coming into my reality from all the different people I have been blessed to sit down with and be educated by, super blessed! So as you listen you can hear the excitement and urgency that I am trying to portray into your ear buds as you listen! So again one of the main purpose's of this show is to get out the importance of "us" the person who own's and take's care of the body we have been given by being born. A duty I think a lot of us overlook, and possibly do not understand because we have never been told or taught to focus on it. I feel that this longevity 'thing' that we speak about is part of the future for us as a people as a major part of the economy and mindset of the main stream as we move forward into the coming years.  Now the one thing that has become very visible for us all is the instability of the world we currently live in and by that I mean religion, politics, environment and health are all hot topics that can divide people very quickly. We can all feel this, and I can not put my finger on it as far as words go but I would say this uncertain feeling or this feeling of inevitable change, no matter which way that change goes, just change. I personally feel from all the studying that I have done on the human mind and the fact that we as a people have this Negative Bias embedded into our DNA (meaning we are always looking for danger to avoid think cave man) so with that fact I think most of us tend to look at worst possible outcome a lot of the times. Maybe even passing over or choosing not to see that there could be a possibility that we need to go through some turbulence to get to the smooth sailing. Just a thought. Ok so over the last few days the amount of people I have met, spoken to, and been advised by that are seeing this bright, robust, energized, future have infected me with this amazing new outlook for the health industry (which is where it started but then grew from there) and all these people were in very very different industries and all had the same vision the same energy and attitude, that the future is very bright!  So for each and every one of us, the time is know, today is the first day of the rest of our life, time to make it count! 78: Cornea Trained Ophthalmologist Talks About His Career

[01:35] Interest in Ophthalmology Alex didn't go to medical school thinking about such Ophthalmology Upon his path to being an orthopedic surgeon, presenting at a conference, he met an Ophthalmologist who encouraged him to spend a day at his clinic. Seeing their practice, he thought they're some of the happiest doctors he has ever seen in the years he spent as a student. He thought it was an organized environment where doctors and patients were happy. And he thought they were happy. Wanting to be happy as well, he decided to jump to the bandwagon. He also found them to be working at reasonable hours. They also got surgery and played with cool toys and lasers. When he found it was competitive, this drew him even more as it was something that challenged him. Thinking he was going to be a businessman, the father of Alex's friend called him and discouraged him from doing so. He was told that if he became an orthopedic surgeon, he was going to retire at 50 as a millionaire. It sounded great to him and thought the dad was great and living the life. So he literally switched his major and started taking science classes. He admits not really liking the business classes he was taking. Nearing medical school, he realized that advice the worst he had ever gotten. He was glad though because it brought him to medicine but to tell somebody to go to medicine to become rich is absolutely wrong. Sure, you could do well and be rich if that's the goal but that's not the way to do it. "To tell somebody to go to medicine to become rich is absolutely wrong." When he got into medical school, he started exploring the orthopedic surgery lifestyle but the personalities he met didn't seem to flow with his, as he describes it. He found people to be a bit more aggressive than what he would have envisioned a classic doctor. Personality-wise, he saw he was more aligned with the Ophthalmologists who are dorkier and laid back. [06:51] Traits that Lead to Being a Good Cornea Specialist Alex explains you have to be very meticulous although you don't have to start being one. Instead, you'd be forced to be meticulous. All of their surgeries and procedures are visible in the patient's eye everyday. So whatever result they have, they're walking around with it. They're looking through it. And if you're off by a small fraction, then a patient sees that for the rest of their lives. "Every calculation, whatever technology we're using... all have to be meticulously placed." [08:00] Types of Patients Although a cornea specialist, Alex also sees a lot of general ophthalmology. In reality, if you practice cornea in private practice, you're also more likely doing a lot of general ophthalmology because there's not that much cornea pathology to keep somebody all day long. For instance, in a day, he may see young patients for routine eye exams. The majority of his patients are also elderly. Common cases would be cataract, glaucoma, macular degeneration. And from a cornea standpoint, there are corneal diseases related to surgery such as patients with previous eye surgeries, multiple surgeries. If you have a sick eye and has had lots of surgery, it causes damage to the cornea which often needs corneal transplant. "If you have a sick eye and has had lots of surgery, it causes damage to the cornea which often needs corneal transplant." Sometimes, people have infections that cause scarring and corneal disease. So he may see contact lens wearers with corneal ulcers that sometimes have scarring so they need corneal transplants. Rarely, they will see certain corneal diseases like dystrophies people are born with. And often, they'd see those with corneal ectasia, also called keratoconus, characterized by thinning or balling out of the cornea if people are born with corneal disease. Their corneas become thin and pointed so they become weak and would nee corneal care either in the form of specialty contact lenses or corneal surgery. [11:20] Typical Day Alex's day usually starts at 8 am as his first scheduled patient and then see between 10 and 20 patients. He's in private practice, working about half an hour from his house. He has great support at the practice. He has a scribe and technicians that work patients out for him. He'd describe it as a pretty fast-paced practice. "To be able to get through 20 patients and make everybody happy around you, you have to be efficient." He'd usually finish around noon and the next patient is scheduled at 1pm. So he gets to have his lunch break (although he doesn't eat but doing other things). Then he ends at 430-5 pm. This is a typical clinic day For OR day, his first case starts at 7 and doing about 10 surgeries in a half day. He'd be done at noon, take a break, and then do clinic in the afternoon from 1-5 pm. [13:02] Taking Calls and Work-Life Balance In his practice, they have 7 physicians, they split their calls equally among 7 people. So he'd be called once a week and you're covering call for your practice only. It's usually light, too. Middle-of-the-night emergencies are rare and if they're happening, sometimes the person can be seen the next morning. He'd also cover call for the local university hospital occasionally where he gets to see trauma call with residents, which can be easy. They have the option to do it as much or little as they want so Alex is doing it one week a year only. "Generally, ophthalmology private practice call is not really intrusive in your life." When taking a call, it's very rare that he gets called in during the week since he can just see the patient the next morning. So he almost never has to go in during the week. On the weekend, he'd have a patient to bring every few weeks. So it's not very common. Alex illustrates his lifestyle as being very predictable and he thinks this is one of the biggest draws of the specialty. [15:22] The Training Pathway You have to do ophthalmology residency and prior to that, do your internship. As of now, they're separated. So you do one year of internship of some kind. Most people do traditional medicine or transitional. Very rarely would you see surgery or peds. Then you do three years of ophthalmology residency. Then for Alex, he did one-year Fellowship on Cornea. "Nothing is going to change the practice pattern so the lifestyle factor will always draw people and will make it competitive inherently because of that." He still thinks the specialty is highly competitive until now. He points out that the nature of the lifestyle is always going to draw people to it. To be competitive in matching, like any other field, you need to really know you want it and be able to show that you want it. The only way to do that is be involved - whether in research, clinical experience, shadowing, volunteering. These are all just ways to figure out whether you like it or not. Then build connections throughout the entire process and those connections are what can help you. You'd be able to get better letters of recommendation from people you spend a lot of time with. "You need to really know you want it and be able to show that you want it. The only way to do that is be involved." Undoubtedly, the first thing residency programs are going to screen you on is going to be your school and Step 1 that's going to get your foot on the door. Otherwise, it could be an uphill battle - not impossible, you could definitely do it. Next, is how well you interview. Ophthalmologists work tightly together and in a clinic environment side by side with your attending physicians, and a lot of times, with their private patients. So they want somebody they'd feel comfortable around patients. Lastly, Alex says that research always helps. For cornea fellowship, it's not as competitive. Good programs at anything are always going to be competitive. The same reason you could say that family medicine isn't competitive. But pick the best program in family medicine and it's going to be very competitive. Ultimately, in terms of competitiveness in ophthalmologic fellowships, it's probably middle of the road. [19:35] Other Sub-specialties There are new ones every couple of years. But for now, there are subspecialties like a 2-year fellowship in Retina or a 1-year or 2-year fellowship in Oculoplastics, a 1-year or 2-year fellowship in Oculoplastics, 1-year fellowship in Glaucoma and 1 year in Cornea. You could also do 1 year in Uveitis, 1 year in Pediatrics. You can also do Pathology or a special fellowship for Refractive Surgery. [20:35] Negative Bias in the Field and Alex explains that even as a DO the bias doesn't come up among his patients. The bias rather comes up when you're trying to apply for residency programs. The program directors of MD programs are not going to look at you first. You have to do something special to stand out among the MD applicants. "The bias undoubtedly is going to come when you're applying for residency programs because the field is so competitive." [21:27] Working With Primary Care and Other Specialties His advice to primary care physicians to refer early and don't just treat red eyes. The differential diagnosis of a red-eye when he sees one "Refer early and don't just treat red eyes." The exams he does and all the things he looks for are extensive. A lot of times, primary care docs see patients and they say it's probably a pink eye. Then they'd give somebody an antibiotic. Then send him his way. So he urges primary care physicians to refer early. Less is more. And don't just treat. Because a lot of times, this could make Alex's job a bit more difficult when they get to him and he's not sure where to really start. Other specialties he works the closest with include Anesthesia, Primary Care (as they manage a lot of diabetics), Neurology (taking care of patients with vision loss, cranial nerve palsies, etc.) He may also work with a Rheumatologist (autoimmune inflammatory eye diseases that need systemic management. [23:12] Special Opportunities Outside of Clinical Medicine One may get involved in the industry of surgical devices, doing trials, and testing new devices. You can also always be a business owner. [24:08] What He Wished He Knew that He Knows Now In one aspect, he has enjoyed building long-term relationships with patients. But as a practice, he is pretty tied to his geographic location because he's getting his self and name out there in building a patient base. So he can't just leave and decide in another part of the country, which is something other specialties can do such as Anesthesiologist or Emergency Medicine doc. "The private practice of Ophthalmology is much more community-centered." The best part of his specialty he describes is the one-day post-op where the patients come in and there's a smile in their face because they can see much better. On the flip side, one of the things he likes the least is the fact that he not infrequently does he have to talk to patients about money. For instance, he may prescribe eye drops that can't be covered by their insurance or offering different services not covered by insurance. [26:30]  Working with Optometrists There's a lot of uproar being seen right now with optometrists requesting and pushing for more and more ability to do procedures and things. In his experience working alongside optometrists throughout medical school up to his private practice, he thinks the majority have not been interested in getting involved in surgical intervention. He doesn't blame them because a lot of them actually went into optometry because they didn't want to be surgeons. They want their predictable lifestyle and hours and don't particularly want to go out of the scope of what they're comfortable with. "There's a push for some optometrists to have a piece of the surgical pie and I've seen mistakes made and I've seen things that were missed." Alex says he had seen mistakes made by optometrists. However, it has nothing to do with them being an optometrist, but it just had something to do with them not having years of surgical training and not actually knowing what you could be looking for, let alone, missing it. This makes him a bit nervous. It's scary for patients to walk in the door because half the time, they might not know who they're talking to and who's actually doing surgery on them. So for patient's safety, Alex believes it can be dangerous for optometrists to get involved surgically. [29:23] Major Changes in the Field of Cornea Surgery Particularly in the field of corneal transplantation, they used to take donor tissues and sew them into place. They hope that in the future, they will be able to take individual cells and replace just the damaged cells instead of the tissues. This is in the effort to have lower rejection rates, faster recovery, and better outcomes for the patients. Other things in the cornea sphere, specifically from the refractive side (getting better vision), technology is like lens and plans they put in during cataract as well as better techniques to do cataract surgery. Currently, they're doing laser cataract surgery. Lastly, Artificial Intelligence (AI) is already in Ophthalmology to help them make better clinical decisions. Finally, if he had to do it all over again, Alex says he would still do it. His advice to students who might be interested in this specialty, spend some time with enough ophthalmologists. "Anybody that wants any specialty always gets in eventually. I've never met anybody that really wanted something that just never got it. Whether it took more time or a different approach or using different tools, somehow they got there. So do not give up!" Links: MedEd Media Network ryan@medicalschoolhq.net

Optimal classifier selection and negative bias in error rate estimation: an empirical study on high-dimensional prediction

Background: In biometric practice, researchers often apply a large number of different methods in a "trial-and-error" strategy to get as much as possible out of their data and, due to publication pressure or pressure from the consulting customer, present only the most favorable results. This strategy may induce a substantial optimistic bias in prediction error estimation, which is quantitatively assessed in the present manuscript. The focus of our work is on class prediction based on high-dimensional data (e. g. microarray data), since such analyses are particularly exposed to this kind of bias. Methods: In our study we consider a total of 124 variants of classifiers (possibly including variable selection or tuning steps) within a cross-validation evaluation scheme. The classifiers are applied to original and modified real microarray data sets, some of which are obtained by randomly permuting the class labels to mimic non-informative predictors while preserving their correlation structure. Results: We assess the minimal misclassification rate over the different variants of classifiers in order to quantify the bias arising when the optimal classifier is selected a posteriori in a data-driven manner. The bias resulting from the parameter tuning (including gene selection parameters as a special case) and the bias resulting from the choice of the classification method are examined both separately and jointly. Conclusions: The median minimal error rate over the investigated classifiers was as low as 31% and 41% based on permuted uninformative predictors from studies on colon cancer and prostate cancer, respectively. We conclude that the strategy to present only the optimal result is not acceptable because it yields a substantial bias in error rate estimation, and suggest alternative approaches for properly reporting classification accuracy.
