Dr. Monnica Williams is a professor at the University of Ottawa, where she holds the Canada Research Chair in mental health disparities. Her research is focused on mental health, race, racism and novel approaches to treatment. In this conversation, she is sharing about her research and tips for healing racial trauma based on the latest research.She also offers a call to action for players in the psychedelic space to improve diversity at all levels. Williams is a board-certified licensed clinical psychologist and Professor at the University of Ottawa in the School of Psychology, where she is the Canada Research Chair in Mental Health Disparities. She is also the Clinical Director of the Behavioral Wellness Clinics in Connecticut and Ottawa, where she provides supervision and training to clinicians for empirically-supported treatments. Prior to her move to Canada, Dr. Williams was on the faculty of the University of Pennsylvania Medical School (2007-2011); the University of Louisville in Psychological and Brain Sciences (2011-2016), where she served as the Director of the Center for Mental Health Disparities; and the University of Connecticut (2016-2019), where she had appointments in both Psychological Science and Psychiatry. Dr. Williams' research focuses on BIPOC mental health, culture, and psychopathology, and she has published over 200 scientific articles on these topics. Current projects include the assessment of race-based trauma, barriers to treatment in OCD, improving cultural competence in the delivery of mental health care services, and interventions to reduce racism. This includes prior work as a PI in a multisite study of MDMA-assisted psychotherapy for PTSD for people of color. She also gives diversity trainings nationally for clinical psychology programs, scientific conferences, and community organizations.Through the Kentucky Psychological Association (KPA), Dr. Williams served as the diversity delegate to Washington DC for the American Psychological Association (APA) State Leadership Conference for two consecutive years. She has served as the African American SIG leader for Association of Behavioral and Cognitive Therapies (ABCT), and currently is Chair of their Academic Training & Education Standards (ATES). She serves as an Associate Editor of Behavior Therapy. She also serves on the editorial board of Cognitive Behaviour Therapy, Canadian Psychology, International Journal of Mental Health, Journal of Psychedelic Studies, the Journal of Obsessive Compulsive and Related Disorders and the Cognitive Behavioural Therapist. She is a member of the Scientific Advisory Board of the International OCD Foundation and co-founded their Diversity Council. Picture the headline: Resistance training revolutionizes menopause health. It's that simple. It's 2024. We still hear both about the use of HRT or reasons (perhaps outdated) not to. We hear about both animal protein and the plant protein advocates. With this simple method of – humor me – hormone enhancement – there is no argument. We all know we need to be doing it. We all understand the benefits of being strong and vibrant. But lest you confuse a need to lose fat with the need to gain lean muscle, this is for you. Lifting already? Then you'll love these reminders. After all, the placebo effect is in full swing at all times. It's either working for or against us. So those things you're doing that you know are good for you, amplify them with the scientific evidence and think your way to even better results. This episode captures the broad spectrum of positive changes resistance training can bring to menopausal women, from physical improvements like visceral fat reduction and bone density increases to more general benefits like enhanced mood and sleep quality improvements. Each of these is directly tied to signs and symptoms of menopause. Give a Woman a Weight and Change Not Only Menopause, But Her Health the Rest of Her Life Before we dig in, in the rhythmic play off the children's story book, “When you Give a Mouse a Cookie”… When you give a woman a weight… (or when you pick one up yourself) You: Enhance her strength Improve her fat free mass Reduce her total fat Reduce that deadly visceral belly fat Support overall hormone balance Alleviate symptoms of anxiety and depression Enhance her sleep Improve skin health Increase bone density Enhance Gait and Stability Women who lift, lift the world. Theirs and the world at large. How Resistance Training Revolutionizes Menopause Health 1. Enhance muscle strength – directly tied to longevity (link to recent episode on testing your own longevity) supporting daily activities and reducing the risk of falls. Beyond that however, for bone density benefit, heavy lifting will require muscle strength. 2. Improve fat-free mass, aiding in a healthier body composition during menopausal transition. The combination of 1 and 2 together, notably known as “recomposition.” You're not literally turning fat into muscle. But you are indeed able to lose fat and not just avoid loss of muscle, but add muscle. This does require some careful effort. For those without much consistent strength training, using a caloric deficit with a high protein intake make effective. The longer your consistent resistance training experience however, and the older you are, you may need a surplus of calories high in protein to overcome the anabolic resistance that occurs with aging. 3. Support total fat mass reduction, addressing weight gain concerns during menopause. Over 5600 postmenopausal women in one hundred studies reviewed showed exercise training effectively increased muscle mass and fat-free mass, and decreased fat mass, body fat percentage, waist circumference and visceral fat. (more on visceral fat coming up). Further this study in Frontiers in Endocrinology in 2023, found combining RE and aerobic exercise(not same session) helped increase muscle and decrease fat, while RE plus combined training – which includes doing aerobic activity and strength training in the same session – helped boost muscle mass most. The reason it likely didn't impact fat as much I suspect is because intensity in the aerobic activity won't be as high. You either won't go as fast or against as much resistance even though it may “feel hard.” 4. Reduce Visceral Fat. Resistance training has been shown to effectively promote decreases in visceral fat in the absence of caloric restriction. That is, no change other than adding resistance training. In still another study published 2023, reduction in visceral belly fat was significant compared to non-resistance trained control group whether participants did 2 or 3 times weekly RE. The impact was observed to be significant across both obese and non-obese individuals, as well as across different age groups, indicating its broad applicability for fat reduction during menopause. 5. Support hormone balance.For pre- and post-menopausal women's unique physiological needs, muscle is essentially an endocrine organ. You will support more optimal insulin levels, decreasing the need for it and therefore decreasing fat storage. You will experience less negative impact of cortisol compared to cardio. You'll also boost natural testosterone levels, pick up where estrogen levels dropped in menopause, and so much more. 5 More Ways Resistance Training Revolutionizes Menopause Health 6. Alleviate symptoms of depression and anxiety by releasing endorphins and improving mood. Similar to the way that there are reduced negative effects of stress with brief periods of rest between sets allow those sensations to normalize a bit before being increased again with the next set, resistance exercise can provide optimal mini exposure to elevated heart rate for anxiety, without negatively tipping the scale, which helps enhance resilience. Enhance Gait Speed and Stability. By improving muscle strength and mass, resistance training can contribute to better balance, gait speed, and overall mobility. Gait speed is directly related to longevity with a minimum speed of 3.0 set for determining a good baseline. Walking speeds of 3.3. Or greater are associated with greater bone density benefit through ground forces than slower speeds. Gait speed typically slows with instability or fear of falling. Stronger muscles support joints and can prevent falls, which is crucial as bone fragility increases with age. This was a repetition range of 8-15 with an average of 60s rest between 2 or 3 sets. Changes were improved with as little as 6 weeks of twice weekly RE. 7. Improve Skin Health by improving circulation and potentially boosting collagen production. Resistance training has a more positive impact on thickening the skin dermis, counteracting the thinning of skin from loss of collagen fibers with age. 8. Increase Bone Density. Both research and anecdotal evidence from our own Flipping 50 community provides evidence that reversal of bone loss is possible through RE. Resistance training places stress on the bones, stimulating bone formation and increasing bone density. This is particularly beneficial during menopause, a period when women are at increased risk for osteoporosis due to declining estrogen levels. Once thought to be the best exercise, the collective approach now includes high impact exercise, yoga and other weight bearing time daily, as well as resistance training. For those unable to lift heavy, tools like weighted vests and whole body vibration (Power Plate is my choice- https://www.flippnigfifty.com/PowerPlate). 9. Enhance sleep quality, aiding in better rest and recovery, as well as overcoming the insomnia that can occur during menopause. A study conducted by researchers at Iowa State University and presented at AHA Conference in March 2022, found resistance training helped reduce time to fall asleep and also increased sleep time more than aerobic exercise among exercisers who did 3x a week resistance training Resistance Training Studies and Menopause Health Among the studies there are a lot of variables. Some smaller, some vast reviews of literature as mentioned with over 5000 women, and variations of 2 or 3 times weekly, 3 or 4 sets, the percent of 1 RM to muscle fatigue did vary as it almost always does when looking at a large body of knowledge. What does come out clearly though is: Multiple sets each session 6-8 sets for each muscle per week Higher intensity (heavier with fewer repetitions improved results over low intensity light- with more reps) Post menopausal women don't respond as well with less stimulus and require more volume from multiple sets x heavier weight and potentially greater frequency of HIIT or high intensity Because of the increased anabolic resistance women experience in post menopause (older) they will require more protein to achieve the same muscle protein synthesis Even if the goal is to go heavy for optimal bone density, all exercisers should start with higher repetitions and lighter weights with emphasis on good form. Based on a simple principle of fitness: progressive overload. While a low load more frequently might be a logical way to begin and build a foundation of connective tissue health and train the brain so to speak. This neural component can't be rushed by heavy weight. It will take the time it takes – about 6-8 weeks of a program. A time when proper form and body mechanics is very important. Yet for women crushed by time, fitting it in, twice weekly can be a bigger advantage and allow for functional workouts, like body weight yoga sessions between without undue fatigue or adrenal stress. Sarah- Jayne does most of her work with women who are experiencing feelings of anxiety, have experienced loss or trauma, or find their reality challenging. Working with women who are trying to conceive, are pregnant or are already parents. She was an inaugural board member of ACPA. She has long been involved in research and development of teaching, supervision and training in clinical psychology. Particularly in the areas of ethical and professional practice, the process of supervision and assessment of competencies in clinical psychology, and the inclusion of mindfulness training in the Clinical Psychology training process. She has established the Mindfulness-integrated Research Clinic at UTS and her PhD research includes the efficacy of Mindfulness interventions in chronic pain. Alice is a training adviser at the Institute of MiCBT and a senior trainer of MiCBT. She has recently, co-development of the Equanimity scale 16 and co-authored The Clinical Handbook of Mindfulness‐integrated Cognitive Behaviour Therapy (Wiley, 2018). Matthew Sanders is a Professor of Clinical Psychology and Director of the Parenting and Family Support Centre at the University of Queensland. He has been an consulting Professor at The University of Manchester, a visiting Professor at the University of South Carolina, and has held adjunct Professorships at Glasgow Caledonian University and The University of Auckland. As the founder of the Triple P-Positive Parenting Program, Professor Sanders is considered a world leader in the development, implementation, evaluation and dissemination of population based approaches to parenting and family interventions. Triple P is currently in use in 35 countries worldwide. Professor Sanders' work has been widely recognised by his peers as reflected a number of prestigious awards. In 2007, he received the Australian Psychological Society's President's Award for Distinguished Contribution to Psychology and in 2004 he received an International Collaborative Prevention Science award from the Society for Prevention Research in the US. In 2007 he received a Trailblazers Award from the Parenting and Families Special Interest Group in the Association for Behavioural and Cognitive Therapy and in 2008 was became a fellow of the New Zealand Psychological Society. Professor Sanders has also won a Distinguished Career Award from the Australian Association for Cognitive Behaviour Therapy, was named Honorary President of the Canadian Psychological Association (2009), and Queenslander of the Year (2007). Podcast #280: A Beloved and Brilliant Voice from the Past: Dr. Stirling Moorey! Rhonda and I are thrilled to welcome Dr. Stirling Moorey, from London, England, to today's podcast. Stirling was one of my first students, and he sat in with me my on all my sessions as a co-therapist for a month for two summers in the late 1970s. I wrote about Stirling in my first book, Feeling Good: The New Mood Therapy, which was published in 1980. One of the miracles of the internet, and zoom, is the chance to reunite with friends and colleagues from the past. Needless to say, Rhonda and I were SO EXCITED when Stirling accepted the invitation to join us! Rhonda starts the podcast by saying that “Dr. Stirling Moorey had the good fortune to be trained and supervised by two pioneers in the field of cognitive therapy, Drs. Aaron Beck and David Burns. In 1979, when Stirling was still in medical school in London, he did an elective with Dr. Aaron Beck at the Centre for Cognitive Therapy in Philadelphia.“ I (David) might put it a bit differently. I would say that during the early days of cognitive therapy, I had the fantastic opportunity to do co-therapy together with Stirling with many patients. I learned a tremendous amount from Stirling, even though I was, in theory, the “expert” and he, in theory, was a totally untrained and green novice. But he was phenomenal right out of the gates, and those months were among the happiest of my life. What I learned by observing Stirling's superb interactions with my patients eventually morphed into my Five Secrets of Effective Communication and my first book, Feeling Good Together! Rhonda continues: "Stirling was one of the first British therapists to study CBT when that discipline was in its infancy. David described their fantastic collaborative work with Stirling in Feeling Good, and has described Stirling's brilliant empathy skills in dozens of workshops. Stirling is currently a Consultant Psychiatrist in Cognitive Behaviour Therapy, and was the Professional Head of Psychotherapy for the South London and Maudsley Trust from 2005-2013. He has been a Visiting Senior Lecturer at the Institute of Psychiatry, Psychology & Neuroscience in London." Stirling is a highly regarded therapist, trainer / supervisor / teacher and workshop leader. His main research interest is in the application of CBT to life threatening illness and adversity. He was one of the first therapists to develop CBT for people with cancer and has contributed to five randomized controlled trials in both early and late stage cancer. Stirling is also co-author with Steven Greer of The Oxford Guide to CBT for People with Cancer, and has co-edited a book entitled The Therapeutic Relationship in Cognitive Behavioural Therapy, published by SAGE (Moorey & Lavender, eds.) During today's podcast, Stirling reminds us that one of the aims of cognitive therapy is encouraging patients to examine their distorted negative thoughts and self-defeating beliefs in a way that is not threatening. If patients don't feel validated, they may feel attacked and become defensive, which, of course, can undermine the therapist's effectiveness. He also reminded us that the grandfather of cognitive therapy, the late Dr. Albert Ellis from New York, often attacked the beliefs of his patients in a somewhat aggressive manner, and that this can frequently trigger therapeutic resistance. In fact, an overly aggressive therapeutic style can split patients and colleagues into two camps: those who love you, and those who may stubbornly resist and oppose you. During the podcast, we reminisced a bit on shared memories, and Stirling said that “David took me under his wing with such willingness to share his knowledge and experience . . . and I was just an ordinary medical student. We had many great moments!” Although Stirling was tempted to relocate to America, he decided to remain in England, and has never regretted that decision. For one thing, he met and married his beloved Magda. My own wife, Melanie, and I were honored to take our two kids to England to attend their marriage. We all loved England and had a ball! Magda, Stirling's wife We discussed some of Stirling's amazing work with the patients we saw together in Philadelphia, as well as his visit one summer when we were in California visiting with Melanie's parents in Los Altos, where we now live. Stirling recalled that when we were out shopping one day, my wife and I tried to persuade him to purchase a large Stetson hat, but he resisted! Stirling described the three ways in which he encourages people to change their negative thoughts using the Socratic Technique of gentle questioning: he asks if the negative thoughts are realistic, if they are helpful, and if an alternative perspective can be taken. The reality testing approach focuses on the important differences between healthy negative feelings, like healthy sadness or grief, which don't usually need any treatment, and unhealthy negative feelings like depression, or a panic attack. One key difference is that healthy negative feelings always result from valid, undistorted thoughts. For example, if a loved one dies, you may tell yourself, “I still love him with all my heart, and I'll miss the many wonderful times we spent together.” In contrast, unhealthy negative feelings result from negative thoughts about the person who died that are distorted. For example, a young woman who's brother committed suicide told herself, “It's my fault he was depressed because our parents love me more when we were growing up. I should have know that he was considering suicide the day he died, so I, too, deserve to die.” Of course, the distorted thoughts don't have to result from a traumatic event. For example, a chronically depressed patient may tell himself, “I'm a loser, and I'll be depressed forever.” A more pragmatic treatment approach focuses less on whether thoughts are distorted or not, but rather on their effects. It's possible for a thought to be realistic but unhelpful. If a tightrope walker in the circus thinks during their act, ”If I fall I will die,” this may be realistic but not very helpful! Stirling talked about how the third way to look at changing thoughts is based on the fact that our lives always have a narrative—a story we tell ourselves about what has happened, or what is happening right now in our lives. These stories can have a powerful impact on how we all think, feel, and behave, and may often function as self-fulfilling prophecies. We can change these stories to make them more adaptive for us. For instance, rather than seeing the glass as half empty, we can see it as both half empty and half full; or we may choose to focus on what you can control vs. what you can't. What I've written so far are just some general ideas, summaries of things that we talked about on the podcast. But when you listen to the podcast, you will perhaps notice the warmth, richness, and depth in the way Stirling thinks and communicates. Then you will “see” and experience his true genius and his immense compassion! We hope that we can entice Stirling to present to one of our free weekly training groups, and perhaps even see if he might agree to do another co-therapy sessions with me that we can publish on a podcast, so you can actually see and experience this master therapist in action! So how do you treat eating disorders? Eating disorders are complex mental health conditions and sadly there is no wonder cure that can fix them. What works effectively for one person may be very different for another. In this episode, I talk to you about some of the ways that I work with my clients. I describe myself as an integrative counsellor, meaning that I am not wedded to any one approach rather using different therapies, which I have found to be particularly helpful for treating eating disorders (Motivational Enhancement Therapy, Compassion Focused Therapy, Cognitive Behaviour Therapy, MANTRA and Cognitive Analytic Therapy). I firmly believe in the therapeutic relationship being the corner-stone for healing. I also believe in being flexible and tailoring work to suit a client's personal journey. Sometimes, this may involve a talking therapy with more insight and self-reflection, whilst at other times it may mean utilising tools and practical strategies, or a combination of the two. The different areas I focus on in therapy (not limited to) in this episode: - Working on motivation for change, as people often feel ambivalent. Understanding why you may have developed an eating disorder. Getting clear on values and diluting the link between self-worth and weight. Developing a healthier relationship with food with regular eating and a range of foods. Becoming more food neutral Dealing with emotions and developing healthy coping mechanisms. Developing awareness and insight into your thoughts and thinking patterns. Developing self-compassion and a healthy body image Boosting self-esteem and self-worth. Managing inevitable relapses. Remember, it is possible to change and to recover from an eating disorder. The road will not always be smooth and there will be many inevitable bumps on this journey. However, it is worth taking the courageous step to reach out and get help. It's also worth trying different avenues alongside including in engaging in self-help, reading recovery blogs and helpful social media, and taking a holistic approach. Recovery is possible. Hold onto hope.

It's not been a topic that has been addressed a great deal, and that's exactly what we are here to do today with our special guest Carolyn Seri, a general registered Psychologist. This is Episode 16 of the VulnerABLE Podcast!At age 6 he was sent to live with his aunt, and would later endure verbal and sexual abuse. After working on himself he was able to come out of the darkness and into his light!At age 6 his parents sent him to live with his aunt, at that age he didn't understand why he was being sent away from his family. The consequences of this trauma, and at age 8 suffering sexual abuse, he did not have the tools to establish healthy development, self-confidence, self-regulation, and a pattern for developing other relationships. Years later he and his family immigrated from Kenya to Alberta the cycles of abuse continued. Their sponsor who happened to be his mom's brother and sister-in-law continually shamed him. He questioned the emotional, verbal and physical abuse endured in his early years in Canada and learned that patterns of abuse continue. All of this left him feeling confused and unsure of what his future could possibly hold for him. It would be many more years of struggling to cope with everything that had happened before he’d decide to find help. For a man who’s name literally translates to “The Light” it would take years of darkness before he could shine it upon the world. He started by working from the inside out through therapy, and learning more about how his past had affected him, he began to climb the mountains of his life instead of carrying them. This was the pivot to his transformational change to lose 200 pounds, and keeping it off. Over the past 10 years, he has used his background in HR, and Cognitive Behaviour Therapy to coach organizations and individuals to navigate change by using stories and is a sought after speaker at conferences too. He is using his light to publish his first book on job searching using LinkedIn. What if there was a quick, effective way to move through processing the trauma of your own birth or witnessing it at work, and it didn’t involve going into loads of detail? My guest today has specialist skills in how to do just that. Krysta Dancy is a licensed Marriage and Family Therapist in Roseville, California. She is also a Doula, so this is such a special combo. I don’t think she will mind me saying that this is someone who is addicted to birth. Krysta worked as a therapist for 15 years had the same realisation that I have had – which is that birth trauma is the speciality that chose her I was excited to talk to Krysta about her knowledge of EMDR. So if you’ve never heard of it, EMDR is a therapy approach that teaches the brain to process traumatic memories as if they are not traumatic. The World Health Organisation recommends EMDR as one of the top two treatment options for trauma and PTSD. The other one is Cognitive Behaviour Therapy so I recommend you go and listen to that episode after this one, if you haven’t listened yet. Now of course, these are two recommended approaches, not the only approaches. A huge value of mine in this work is helping people find holistic support options – both for birthing people, and for the workers who may be carrying vicarious trauma in their bodies. Christian is an unbelievably successful business man. He worked for a global pharmaceutical company for many years, before making the transition to advertising in 2013. He took the lead of the health division at Tank which quickly became an industry standout, thanks largely to Christian’s smarts and leadership. The first you feel compelled to do even though you don't want to, the second defines your sense of perfectionism professionalism and pride in what you do. You think it is reasonable and justified. Anxiety increases the likelihood of being held hostage to an obsession. For instance how many people suddenly become more religious under stress? OCD has its roots in the most ancient times of the evolution of species. All living organisms with self awareness need arousal in order to be motivated to acquire basic needs of survival and be alert in order to escape predators. OCD can have two components which are intrusive unpleasant thoughts (surrogate predators) and Compulsive actions such as checking cleaning washing (surrogate survival needs). OCD so simply hijaks our primitive survival circuits that it can often last decades. Clearly the fear associated with OCD is irrational and indeed research shows you are almost guaranteed never to act on unpleasant obsessional thoughts. Irrational anxiety is also seen in Post Traumatic Stress Disorder and Phobias. The gold standard of treatment is Cognitive Behaviour Therapy (which can sometimes work alone) in combination with a medication to raise serotonin (serotonin re uptake inhibitors-SSRI).There are rapid reprocessing techniques such as EMDR (Shapiro) and Theo's own distilled version which work much faster and very effectively. The 2010 Healthcare Forum series commenced on 11th March with a lecture on the topic of Cognitive Behaviour Therapy, delivered by Henck van Bilsen. Henck van Bilsen is a consultant in CBT and consultant Clinical Psychologist, specialising in complex and long-standing problems.
