This conversation touches on two important subjects: the impact of different potential resmetirom prices on the size and structure of the treatable MASH population, and the impact GLP-1s are having (and will have) on clinical trial recruitment and basic patient treatment. He mentions another paper that says a price of $19,000 would be cost-effective. To Naim, this magnifies a question that is already quite widely considered: when to prescribe resmetirom vs. a GLP-1 agent. He discusses some patient factors that might go into his decision. Naim describes ways that GLP-1 use, which can be sporadic or episodic, is affecting clinical trial recruitment. Ian Rowe discusses the current use of GLP-1s in the UK and suggests that if the price for resmetirom is high enough, GLP-1s will receive significant use. Naim Alkhouri shifts direction slightly, from discussing current GLP-1s to exciting recent clinical trial report. He focuses on two: survodutide from Boehringer Ingelheim and efruxifermin from Akero Therapeutics. Recent exciting results from BI and Akero suggest to Naim and others that the future for drugs is bright. Ian Rowe anticipates a significant amount of co-prescription of GLP-1s and resmetirom in this population. GLP-1s have already demonstrated clear cardiovascular, weight loss, and HbA1c-lowering benefits. To Ian, that creates sufficient motivation to prescribe GLP-1s to multimorbid patients with diabetes, obesity and most likely MASLD or MASH. To him, the price of resmetirom will have a tremendous impact on how often UK physicians rely on GLP-1s alone.

Abstract: Lipid droplet (LD) enlargement is the most conspicuous cellular phenotype of steatotic liver diseases such as NAFLD and NASH. The small GTPase RAB18 localizes to lipid droplets yet its effects on LD dynamics are unknown. We show that RAB18 localizes to LDs dynamically via the acylation cycle in an activity dependent manner. Removal of RAB18 from LDs through acylation cycle inhibition of RNAi-mediated downregulation leads to an increase in LD size, that can be prevented by inhibition of autophagy. We propose a model where RAB18 on LDs decreases the susceptibility of LDs to autophagy by lysosomes, allowing them to exist longer in cells and enabling an even distribution of exogenous lipid load. In the absence of RAB18, fewer LDs survive autophagic removal, and enlarge disproportionately as they absorb the incumbent exogenous lipid load on cells - leading to cellular steatosis. Inhibition of autophagy prevented cellular steatosis in primary human hepatocytes. Finally, mice treated with the autophagy inhibitor chloroquine were resistant to the development of liver steatosis when on a steatogenic diet. The modulation of RAB18 activity and autophagy inhibition represents a promising approach to prevent steatotic liver disease in the clinical setting. Asset Allocation Hello, this is Hall T. Martin with the Startup Funding Espresso -- your daily shot of startup funding and investing. In angel investing, it's important to set aside funds for startup investments. In most cases, investors dedicate 5%-15% of their discretionary funds to angel investing. There are several issues with asset allocation for angel investing compared to publicly-traded stocks, bonds, and mutual funds. Startup investments are illiquid as there's no market for reselling. Transferring stock is greatly limited due to SEC rules. To achieve a gain, you must hold the stock for up to 7-10 years in most cases. Many startups fail completely and are tax write-offs. Determine upfront how much you want to invest based on 5%-15% of your portfolio. Divide by ten to get the total number of startups you can invest in. Divide the investment amount by 2 to get the initial investment per startup, leaving the second half for a follow-on round. Here's an example: Let's say I have a portfolio of $3.5M 15% of $3.5M yields $525K to invest in startups Dividing $525K by 10 gives me $52K per startup that I can invest in. Dividing the $52,500 by 2 means I can invest $26K for each startup leaving another $26K for each follow on investment. Start with 3 investments per year. It's important to be selective. After a few years and some gains, you can re-invest some of the profits into more startups. There are tax laws that make it attractive to roll your gains from one startup investment into another. In 2017, world-class ultrarunner Hillary "Hillygoat" Allen was at the top of her sport--and it felt like she was running on top of the world as she competed in Norway's Tromsø Skyrace. Allen was nearly halfway through the 50K race when she fell 150 feet off an exposed ridge, fracturing her back and breaking multiple ribs, both feet, and both of her lower arms. Ten days and two surgeries later, Hillary flew home and underwent three more surgeries in Colorado. She was told she might never run again. Just 11 months after her injury, after a rehab period that tested her determination, courage, and spirit, Hilary placed second in the Broken Arrow Vertical Kilometer race in Lake Tahoe and raced a 52K race the following day. A week later, she won a mountain race in the Dolomites. Hillary hopes to inspire, educate and encourage other people to find balance in their lives—something the injury has helped her find. Hillary studies and teaches science, using her masters degree in neuroscience and physiology and structural biology. She is also a trail running coach, and a gravel cyclist. When I started my first business, I had an 804 credit score and $52K in the bank. When it ended I had a 444 credit score, was $20K in debt, and become homeless. It took me awhile to stop being bitter & own up to the losses I caused for myself. Once upon a time, Michael Beeman was struggling. He had a blended family of seven kids, and his corporate salary of $60K was not making ends meet. This is episode number 99. I'd already purchased a book for the big one hundred, but I've decided to call it quits, again, with this one. I hate to say goodbye. I love podcasting, meeting fascinating authors and introducing people to the first chapters of great books. But, I'm not getting the feedback to keep going. It got harder and harder to get authors to interview. I'm not sure why that was, if it was their fear of being interviewed, or my self doubts about whether they would really want to be on the podcast. Maybe I should have been more assertive. In addition, subscriptions and downloads have decreased steadily over the last two months. With the podcast taking up half of the creative time I have each week, I think I should focus on getting some of my novels edited and published. I can see why he is so popular. He's created Aza to be witty, thoughtful and self reflective, just as teenagers would like to picture themselves. Probably more witty and self reflective than any actual teenager, but it makes for a fascinating character in a book. Aza really worries about her microflora and feels outnumbered. If she was truly thoughtful and maybe a little more analytical, she would realize she's only outnumbered by count, not by volume. By volume, the aliens are way outnumbered. But, that's not the point. The point is her anxiety. Her anxiety appears entertaining. Maybe Green is using it as a gimmick, maybe he is truly concerned about teens and their anxieties. My teenage daughter has crushing anxiety that shuts her down in class, but she doesn't bounce out of it with witting interchanges with classmates. These first two chapters barely introduce us to the story and if it wasn't for the blurb, this little bit that I read would hardly motivate you to read on. But add the blurb to the author's immaculate writing and immersive style and you're pretty much guaranteed a good read in, "Turtles All the Way Down". People tend to see the end result but don’t realize the process it took to get there. I️ prayed, cried, sacrificed and worked to get this big payday. I’m grateful but the grind continues. $52K is a lot of money but it’s not generational wealth. Time to get back to work.
