Dr Remo received his medical degree from the University of Chicago Pritzker School of Medicine. He completed his internal medicine residency and cardiology fellowship at the New York University School of Medicine, followed by a fellowship in electrophysiology at the University of Maryland School of Medicine. More recently, he completed a fellowship in integrative medicine at the University of Arizona Andrew Weil Center for Integrative Medicine.

As blue states try to shore up access to abortion and reproductive care, some are facing a threat they didn't see coming: Catholic health care mergers. In the first segment, Reveal's Nina Martin takes us to New Mexico, a blue state that's been working hard since the U.S. Supreme Court struck down Roe v. Wade to strengthen its already sweeping protections for many forms of reproductive care. But those guarantees have been threatened by a local merger between Gerald Champion Regional Medical Center, the only hospital in rural Otero County, and a Catholic health care system out of Texas, CHRISTUS Health. Like all Catholic hospitals, the newly merged hospital will be subject to the Ethical and Religious Directives for Catholic Health Care Services, written by the U.S. Conference of Catholic Bishops. Known as ERDs, they limit or ban a number of reproductive services, including birth control, sterilization, abortion and gender-affirming care. Where will people go if they can't get the care they need? The next closest hospital is an hour away. In the next segment, Martin travels to Alamogordo, where Gerald Champion is located, to try to find out how things are changing. Then she widens her lens, talking to a leading researcher on Catholic health care to see how ERDs play out in other hospitals around the country. She closes by talking to two Catholic experts about what ERDs require and how to improve transparency for patients. In the final segment, Reveal's Laura C. Morel follows the story of Kelly Flynn, an abortion provider who has clinics in Florida and North Carolina, two states that had been abortion havens for women around the South before Roe fell. But now, lawmakers in North Carolina have imposed a 12-week ban on abortions, and the Florida Supreme Court is weighing a six-week ban. So Flynn has spent the last few months preparing for access to keep shrinking by quietly opening a new clinic in a state that still has relatively strong abortion protections – Virginia. Is the marketing funnel dead? That question started bouncing around among those at Jarrard Inc. who have backgrounds and specialize in healthcare marketing. It's a provocative question, and provocative questions lead to sharp conversations that make for good podcasts. So, we asked two of those experienced healthcare marketers to have that conversation on the record. Pattie Cuen served at UCLA for two decades. There, she established the medical center's first internal communications team and orchestrated an award-winning branding campaign for the system. More recently, Cuen held a high-level position at Cedars-Sinai. Abby McNeil is a vice president at the firm and deputy lead of our Regional Practice. McNeil has served in a number of prominent roles, including vice president of communications and public affairs for CHRISTUS Health, where she sat at the helm of all corporate communications and helped guide the organization through COVID. Key Insights: The marketing funnel isn't dead. But it's not really the most accurate model, either. It serves a useful, mostly two-dimensional purpose to get a snapshot of how consumers move from awareness to conversion. It's also helpful as a relatively simple tool to present marketing concepts and different levels of messaging to people outside the field. The funnel doesn't take into account the time involved as people move from one stage to the next. That time could be five minutes for a digital engagement or weeks/months for an out-of-home campaign like a billboard. Another way to think about the customer journey is as an elliptical. Once someone converts and makes a purchase – or, in healthcare, becomes a patient – the selling process starts all over, and the patient has to choose you all over again. And again. And again. It's not one-and-done. Meanwhile, each person is sitting in the funnel – or spinning on the elliptical – of other providers, making it a multi-dimensional environment for conversion and retention. Because of that elliptical process, the patient experience has to be dialed in right alongside the marketing messages and platforms. If someone buys but can't schedule a followup appointment for weeks, they'll go somewhere else regardless of how great your marketing campaigns are.   Learn more about your ad choices. Metformin's Role in Preventing Metabolic Syndrome During Androgen Deprivation Therapy

A new research paper was published in Oncotarget's Volume 14 on June 19, 2023, entitled, “Utilizing metformin to prevent metabolic syndrome due to androgen deprivation therapy (ADT): a randomized phase II study of metformin in non-diabetic men initiating ADT for advanced prostate cancer.” Androgen deprivation therapy (ADT) can lead to metabolic syndrome (MS) and is implicated in ADT-resistance. Metformin showed antineoplastic activity through mTOR inhibition secondary AMPK-activation. To investigate whether metformin mitigated ADT-related MS, researchers Devalingam Mahalingam, Salih Hanni, Anthony V. Serritella, Christos Fountzilas, Joel Michalek, Brian Hernandez, John Sarantopoulos, Paromitta Datta, Ofelia Romero, Sureshkumar Mulampurath Achutan Pillai, John Kuhn, Michael Pollak6, and Ian M. Thompson from the University of Texas Health Science Center, Robert H Lurie Comprehensive Cancer Center of Northwestern University, Roswell Park Cancer Institute, Mays Cancer Center at University of Texas Health, Audie Murphy VA Hospital, McGill University, and Christus Health conducted a randomized double-blind phase II trial of metformin 500 mg TID or placebo in non-diabetic patients with biochemically-relapsed or advanced prostate cancer (PC) due for ADT. “To test these hypotheses, we conducted a phase II randomized, placebo-controlled, prospective study of metformin vs. placebo in patients with advanced, castrate sensitive PC treated with ADT (NCT:01620593).” Fasting serum glucose, insulin, PSA, metformin, weight, and waist circumference (WC) were measured at baseline, week 12 and 28. The primary endpoint was a group of MS metrics. Secondary endpoints include PSA response, safety, serum metformin concentrations and analysis of downstream an mTOR target, phospho-S6-kinase. Thirty-six men were randomized to either metformin or placebo. Mean age was 68.4. Mean weight, WC and insulin levels increased in both arms. At week 12 and 28, no statistical differences in weight, WC or insulin were observed in either arm. No significant difference in percentage of patients with PSA

Jeannie Frey is chief legal office and general counsel at Christus Health, a multi-national nonprofit health system, with U.S. operations primarily in Texas, Louisiana and New Mexico, and Latin American operations in Mexico, Colombia and Chile. Prior to joining CHRISTUS Health in 2018, Frey was chief legal officer and general counsel of Presence Health, a Catholic health system based in Chicago, from 2011-18. In 2016, on a leap of faith, I signed up for something called a Power Circle and was guided through a six month process by this amazing facilitator Jamie Greenwood. In the space that Jamie held for a small group of us, I said out loud my dreams for Mother's Quest for the first time, and at Jamie's urging, started a Facebook Group for the community I hoped to grow in my journey. Spencer has had many mentors throughout his life both spiritual and professional. He met his first mentor during graduate school who helped him land an internship. This opportunity allowed him to pursue what he wanted to do later in his career. Through the amazing mentoring of this individual, he went into his first leadership position at The Vincit Group. From there, he met an individual working as a leader at CHRISTUS Health, and they talked about their career paths. It was because of this conversation that he was offered an opportunity at CHRISTUS HEALTH. Even though he couldn't commit to the job at the time he was still amazed at the organization's leaders. When he was given another chance to join the company he didn't hesitate to accept. It was at CHRISTUS HEALTH that he met his two new mentors, Lisa Reynolds and Karen Hale. These two leaders showed Spencer what true leadership was through example. They taught Spencer that his initial thoughts on leadership were completely different from true leadership. These leaders always put their teams first and were consistently there for their teams. Spencer used the insights he learned from his mentors for his first formal leadership position at CHRISTUS HEALTH. Spencer found that this leadership style applied to more than just leaders in the professional world. He eventually saw faith as more than just something he learned at school. This was possible through the help of his spiritual mentor, Josh. He helped Spencer look deeply into his faith and to read scripture solely because he wanted to know more. My guest is Dr. Roger Fowler, Vice President, and Chief Medical Director of Quality, Performance and Innovation with CHRISTUS Health in Irving, Texas.As an expert in population health, quality, and healthcare reform, Dr. Fowler offers a rich perspective on the historical events that underpin our current state of U.S. healthcare. His belief is that we must move away from payment systems that reward the quantity of procedures performed, and move toward those programs that reward excellent outcomes for patients and the community.Dr. Fowler describes how quality measures themselves have their origins in the way insurance companies ranked desirable vs. undesirable physicians based on each physician's claims data. It was all about costs. The use of the term Medical Loss Ratio (MLR) is an important reminder of the insurance industry's perspective about medicine vs. the perspective we as physicians have. MLR means that the resources spent on providing medical care for patients is the Loss referred to in this acronym. A family medicine physician for 40 years, Dr. Fowler says when he first heard medical care referred to as a loss it was like fingernails on a blackboard for him.Dr. Fowler has compelling stories of how the healthcare system goes haywire. He talks about the unintended consequences of early “hospitalization insurance” and EMTALA regulations and he laments the various ways unscrupulous characters cash in on patients' vulnerabilities. His expertise and patience almost meet their match with my ignorance about Medicare programs. He labors to help me understand some of the key points of Medicare Advantage, Traditional Medicare, ACOs, Shared Savings programs, and more. After we finished our conversation, I requested a CME certificate.As a patient advocate and expert on population health and quality, he is passionate about getting to a reasonable level of spending on primary care (currently only 5-6% of U.S. healthcare spending is on primary care). Ruth Krystopolski has spent the last 30 years in Healthcare. Over the past 5 years, she has been deeply focused on population health within her present role as the Senior VP of Population Health at Atrium Health, as well as her previous role as the Senior VP of Population Health at CHRISTUS Health. Atrium Health encompasses over 40 hospitals, and 900 post-acute care locations, and Ruth and I spoke about the challenges of serving such a large and heterogeneous population. When drive-through testing sites failed to increase the testing-rate-per-thousand among people from lower socioeconomic backgrounds, realizing that many didn't have cars, Atrium repurposed two of it's Wellness-on-Wheels vehicles to drive around these neighborhoods using hot spotting and heat-mapping to identify locations that would maximize the impact of their efforts.
