CardioNerds co-founder Dr. Dan Ambinder, series chair Dr. Giselle Suero Abreu, and episode FIT Lead Dr. Rachel Ohman discuss disparities in cardiooncology with Dr. Javier Gomez Valencia, the Director of Cardio-Oncology services at John H. Stronger Jr. Hospital of Cook County. Dr. Rachel Ohman drafted show notes. Audio editing by student doctor Shivani Reddy. A disproportionate burden of both cancer and cardiovascular disease affects racial and ethnic minority groups as well as lower-income communities. Similar patterns of vulnerability exist among cancer survivors with cardiovascular disease, although further investigation in these subpopulations is needed. We discuss a comprehensive approach to the cardio-oncology patient, our current understanding of the social and structural determinants of disparities in cardio-oncology populations, and other contributions to inequity in the field. Given the growing population of cancer survivors and limited accessibility to cardio-oncology specialists, these topics are of critical importance to anyone caring for cancer patients who have or are at risk for cardiovascular disease. This episode is supported by a grant from Pfizer Inc. Pearls and Quotes - Disparities in CardioOncology Social and structural determinants of health are drivers of cardiovascular and cancer disparities. Existing data on cardiotoxicity outcomes suggests these determinants also contribute to disparities in cardio-oncology. Assessing social and structural determinants of health should be a routine part of evaluating a patient with an active or prior history of cancer. Customs, country of origin, and immigration status matter. Differential risk profiles among Hispanic/Latinx sub-populations require further investigation. Black patients, particularly black women with breast cancer, have elevated morbidity and mortality from cardiotoxicity. Data suggest contributions from social determinants of health. Representation in clinical trials must be diversified for applicability to our diverse patient populations. Concerted efforts should be made to recruit diverse clinical trial participants and help patients from diverse communities effectively participate in the research process, contributing to the advancement of science. Show notes - Disparities in CardioOncology How do you approach the evaluation of a new patient in cardio-oncology? How do social and structural determinants of health impact treatment-associated cardiotoxicity? The evaluation of a new patient should include an assessment of a patient's intrinsic risk factors, risks associated with cancer treatment, and consideration of cardioprotective therapeutic strategies Social and structural vulnerabilities should also be assessed routinely as a part of risk stratification. Providers should take stock of a patient's demographic (e.g., race/ethnicity, gender), socioeconomic (e.g., occupation, insurance status, food security, housing security), environmental (e.g., transportation, proximity to health resources, neighborhood safety), and sociocultural (e.g., psychosocial stressors, discrimination, acculturation) determinants that are in turn modulated by larger systemic forces like structural racism. This comprehensive risk assessment can guide the strategies to mitigate cardiovascular risk before, during, and after cancer treatment. Lauren LoGiudice: grew up in the Howard Beach section of Queens, New York. After graduating from Wesleyan University,  she obtained the American India Foundation William J. Clinton Fellowship Award for service in India. During her tenure in Bangalore, India, she worked with a non-profit organization that served victims of AIDS and studied Bharata Natyam. After attending Lakme Fashion Week in Bombay, Lauren decided to pursue acting back in New York. When she returned, she studied at the most prestigious acting schools and teachers in New York: Michael Howard Studios, the T. Schreiber Conservatory, Scott Freeman and privately with Lynn Singer, under whom she began seriously studying vocal production, and learned to speak without a New York accent, which greatly expanded her casting options and opportunities. Her TV and film credits include Veep, Bittersweet Monday, and Galaxy360. A Moth StorySLAM Champ, her work has been featured by The New York Times, BBC, Bust Magazine, McSweeney's Internet Tendency, Hyperallergic, La Voce di New York, among others. Her writing has been called "criminal." Lauren was cast in numerous national and regional commercials, including Zappos' scandalous "More Than Just Shoes" campaign. Dan discusses the training prep, nutrition, and heat adaptations, plus the mental prep that he and Chelsea went through prior to her amazing debut.   Dan Plews is an Applied Sports Scientist, Researcher and coach. His story of how he came to Cook County Hospital is fascinating. The reasons why he stays are inspiring.  Javier has an undeniable charm. He's warm and funny, but he's also sharp and deeply passionate about treating his patients. He takes time to understand his patients as people, not just symptoms, and he teaches fellows and residents to see them accordingly as well.  Our conversation with Javier was like a warm blanket. We felt safe and taken care of, exactly how you'd want to feel when you are in the hospital. Dr. Gomez's patients are lucky to have him at the helm of their care. Victories at ITU Junior European Cup events in Brno, Tabor and Antalya were followed by a Senior ITU victory at Tartu. His first major success came at the 2015 European Championship where he picked up a bronze medal.   Kristian's Norwegian National titles came in 2015, 2016 and 2018, and now he was also showing off his talents on the IRONMAN 70.3 circuit. He notched three consecutive victories in Bahrain between 2017 and 2019, and in both the 2018 and 2019 renewals set new world records for the half-IRONMAN.   2019 was a highly successful campaign for the Norwegian as he was also crowned winner of the ITU World Triathlon Series Grand Final. So powerful is his cycling ability and mentality, you feel as though he is on a mission to challenge anyone who thinks they can breakaway on the bike (from him) and win.   The result of that – for me at least, and it has been the case for some time now – is that the first major measure I look at during a race is not who is leading the swim, who is in the lead pack, is there a break etc. It's, “did you exit the swim ahead, or behind Kristian Blummenfelt?”   Ahead, and – if you have the legs to follow him – you gain access to the world's greatest domestique (!) to help overcome any swim deficit. If you are behind and you miss the Blu Train… expect a very difficult next 90 minutes.   And that, perhaps, is the primary reason why Great Britain's Alex Yee wasn't able to challenge for world championship gold on Saturday in Edmonton, despite producing the fastest run by a long way. In the women's field in particular, the match up of Daniela Ryf and Taylor Knibb seems most intriguing. At the Collins Cup they went head to head and Knibb had the biggest victory of the day of all athletes crushing Daniela by double digit minutes. But Daniela reportedly wasn't feeling her best and we all know of the four time IRONMAN World Champion capabilities. No doubt she is looking for some revenge. Obviously as the championship, the field is stacked. Maybe there is a chink in Ryf's armor? Maybe not, but Lucy Charles, Ellie Salthouse, Sky Moench, Paula Findlay and Jeanni Metzler, all with great races in Slovakia could be in the mix at the end. No doubt there are others like Holly Lawrence who crashed in the Collins Cup could fight for the podium.   The mens field offers similar drama with Sam Long, the top ranked American and one of the favorites will have stiff competition from Lionel Sanders, Gustav Iden and fellow Norwegian and Olympic Gold Medalist Kristian Blummenfelt. Last May, Long and Sanders battled shoulder to shoulder in St. George and the two have had some fun social media banter in the last 12 months. After the race in May, Long said he knew he could take advantage of the downhill with his long stride and he did so almost winning so it will be interesting to see what happens.   The Norwegians are going to be tough to beat. Both Iden and Blummenfelt have had fantastic seasons. Iden handedly won his race in Slovakia and Blummenfelt has focused on training for St. George since winning Olympic gold. With no Jan Frodeno, putting odds on this race is challenging. It's a test of lung power, starting from North America's highest incorporated town near 10,200 feet and rising to Hope Pass above 12,600 feet. It's better known as Hopeless Pass. Lottery-based, races in late August.   Run Rabbit Run Are you a tortoise or a hare? That is the first question to ask when approaching this 100-mile sufferfest from Steamboat Springs. While gaining 20,000 feet of elevation, you'll be covering parts of the Continental Divide and the town's recognizable summits: Mount Werner and Emerald and Rabbit Ears mountains. Hares finish under 36 hours, tortoises under 30. Usually mid-September.   Flaming Foliage Relay This 165-mile overnighter showcases autumn's aspen glow where it is famously glorious, along Guanella, Georgia and Fremont passes. Running teams might struggle to admire the beauty. Their heads are down on the course between Idaho Springs and Buena Vista, crossing roads and trails amounting to about 17,000 vertical feet. Usually mid-September.   Triple Bypass In 1988, a group of cycling buddies thought it would be fun to ride from Evergreen to Vail. Ever since, that's been the idea of fun for riders traversing a trio of heart-pounding passes over 100-plus miles and 10,000-plus feet. An exclusive bunch has made it a “double triple” in recent years, pedaling back the way they came. Slated for late July/early August 2022.   Breck 100 Mountain bikers have come to know this, not the Leadville 100, as Colorado's ultimate endeavor on a saddle — a tantalizing route shaped like a clover leaf, crossing the Continental Divide three times. But the race has been on hold for two years; the pandemic again caused a cancellation this summer. And the past two plans were a condensed course, closer to 70 miles. Will the real Breck 100 return? “That's the million-dollar question,” says organizer Thane Wright. Previously mid to late July.   SBT GRVL Colorado's gravel revolution begins in Steamboat Springs, a cycling town proud of its crunchy backroads. This race has risen to prominence with a variety of endurance courses amid classic scenery. They have recently ranged from nearly 40 miles to more than 140 miles, with climbing between 2,000 and 9,400 feet. Slated for Aug. 22, 2022. Vapor Trail 125 Any ultra competitor knows about “sleep demons,” those hallucinations that manifest at night. That's when this whacky mountain bike foray begins, in the darkness of 10 p.m. It begins in Salida, where some of the whackiest riders reside, molded by the vaunted Monarch Crest. Vapor is reserved for the most skilled and wilderness prepared. One must negotiate technical singletrack by headlamp and sustain long climbs over 125 miles. Victories at ITU Junior European Cup events in Brno, Tabor and Antalya were followed by a Senior ITU victory at Tartu. His first major success came at the 2015 European Championship where he picked up a bronze medal.   Kristian's Norwegian National titles came in 2015, 2016 and 2018, and now he was also showing off his talents on the IRONMAN 70.3 circuit. He notched three consecutive victories in Bahrain between 2017 and 2019, and in both the 2018 and 2019 renewals set new world records for the half-IRONMAN.   2019 was a highly successful campaign for the Norwegian as he was also crowned winner of the ITU World Triathlon Series Grand Final. So powerful is his cycling ability and mentality, you feel as though he is on a mission to challenge anyone who thinks they can breakaway on the bike (from him) and win.   The result of that – for me at least, and it has been the case for some time now – is that the first major measure I look at during a race is not who is leading the swim, who is in the lead pack, is there a break etc. It's, “did you exit the swim ahead, or behind Kristian Blummenfelt?”   Ahead, and – if you have the legs to follow him – you gain access to the world's greatest domestique (!) to help overcome any swim deficit. If you are behind and you miss the Blu Train… expect a very difficult next 90 minutes.   And that, perhaps, is the primary reason why Great Britain's Alex Yee wasn't able to challenge for world championship gold on Saturday in Edmonton, despite producing the fastest run by a long way. In Ep. 8 I spoke with Sean Madsen who is the owner of the Denver Fit Loft in Denver Colorado.  After running the Specialized BG Fit program for 5 years and fitting some of the greatest cyclists in the world (Peter Sagan, Vincenzo Nibali, and Tom Boonen), Sean decided to embark upon his own project to provide custom bike fitting for Denver area cyclists. After 5 years of running the BG Fit program, he noticed that on average only 10% of the thousands of people he trained implemented fit effectively in their business.  Sean attributes this to the fact that the majority of bike shops' primary task is to sell bikes and bike-related inventory and provide bike service.  While bike fitting is a service, many of the trained fitters were also managers, owners, mechanics, salespersons...etc.  He expressed that fitting gets "pushed down on the priority list" when a shop needs to pay bills, move inventory and employees have multiple responsibilities.  As a result, he created his own independent bike fitting business to implement the strategies he formerly taught but without the capital investment of bicycles and bicycle equipment. Sean also spends significant time on the pod defining different types of bike fitting and why it's important that these exist: foundation fitting, custom fitting, and pre bike purchase sizing.  He also offers advice to bike shops and fitters on finding success with fitting and sizing in the current marketplace. David McNamee is a Scottish professional triathlete whose name you might not know, but you definitely should. In 2018, he became the third fastest finisher ever at the Ironman World Championship in Kona with a time of 8:01:09.  One year earlier, he was also on the podium at Kona, with a 3rd place finish in 8:07:11. In other words, he's fast and only getting faster at age 30. McNamee spent a lot of years successfully racing ITU before moving to long course racing in 2014. He's been Jan Frodeno's training partner, and has raced against Javier Gomez for many years, so he's primed to make the jump to a name you definitely have heard of. Hey Folks, this year it's the 40th anniversary of the Hawaii Ironman World Championships and it promises to be one hell of a race in both the male and female events.  Our regular forecaster, Tim Heming, is back with his annual Top 10 predictions. Tim does a lot of work in advance of making his picks. It's not just simply picking the big names.  In the conversation he outlines why he selected certain athletes and why others didn’t make his top 10.  Last year he dropped a monumental clanger when he omitted Lionel Sanders from the top 10 only for him to just miss out on the win. Adopting ancient sacred geometry as his photographic vision allows him to translate organic objects and architectural details into symbolic geometric patterns, to create bold color-centric abstract works that in a surreal way become Universal. Javier strives to create images that access our subconscious understanding of ratio and geometric forms, thereby enhancing our connection with the spiritual and natural world. By blurring the lines between nature and abstract, he is able to go through the gamut of color, volume and texture, to create images that are otherworldly. His work seeks to uncover the subtle beauty of coincidence and anomaly found in nature. Javier's proclivity towards this process is the key to understanding why people have observed that his photography is so hypnotic. Throughout Javier's work is a sense of de-familiarization and detachment from everyday ephemera while trying to connect us to our true self, creating awareness of the balance that should exist between mind body and spirit. Javier has had solo exhibitions around the world including in: New York City, Paris, Monaco, Tokyo, Madrid, Seville, Beirut, Hong Kong, and Art Basel. He has also published two monographic books, “Dimensions” (2010) and “Visual Vibrations” (2012). His clients and collectors include luminaries Bill Clinton, Pelé, Michael Bloomberg, Colin Cowie, Dr. Phil and brands such as FENDI Casa and BVLGARI. Javier is also dedicated to giving back and helping people from all walks of life. He is involved with several non-profit organizations to which he donates his time and talent. These include: United Cerebral Palsy of New York City, Ackerman Institute for the Family, Lumiere and colour 93 in Paris, and The American Parkinson Disease Association.
