Hosts: Vivian Chiu, MD Brian Gilberti, MD https://media.blubrry.com/coreem/content.blubrry.com/coreem/Acute_Pulmonary_Embolism.mp3 Download Leave a Comment Tags: Pulmonary Show Notes Core Concepts and Initial Approach Definition: Obstruction of pulmonary arteries, usually from a DVT in the proximal lower extremity veins (iliac/femoral), but may be tumor, air, or fat emboli. Incidence & Mortality: 300,000–370,000 cases/year in the USA, with 60,000–100,000 deaths annually. Mantra: “Don't anchor on the obvious. Always risk stratify and resuscitate with precision.” Risk Factors: Broad, including older age, inherited thrombophilias, malignancy, recent surgery/trauma, travel, smoking, hormonal use, and pregnancy. Clinical Presentation and Risk Stratification Presentation: Highly variable, showing up as anything from subtle shortness of breath to collapse. Acute/Subacute: Dyspnea (most common), pleuritic chest pain, cough, hemoptysis, and syncope. Patients are likely tachycardic, tachypneic, hypoxemic on room air, and may have a low-grade fever. Chronic: Can mimic acute symptoms or be totally asymptomatic. Pulmonary Infarction Signs: Pleuritic pain, hemoptysis, and an effusion. High-Risk Red Flags: Signs of hypotension (systolic blood pressure < 90 mmHg for over 15 minutes),

    Śrīla Śrīdhara Svāmī, the great commentator from antiquity says something that's just so relatable and so enlivening, and it's included in the first part of the purport that explains the commentaries of the great ācāryas, both Śrīla Sridhara Swami and Śrīla Vishwanatha Cakravartī Thākura. He gives this analogy. He says, "The son of a rich man has a primary duty in life: that is to stay alive. That's very practical, right? What are you doing? Staying alive. Why? Because he'll inherit the wealth." Similarly, he says, "The devotee has a singular duty: stay alive in Kṛṣṇa consciousness somehow or other." This is also the suggestion of Śrīla Rūpa Goswāmī, who says, "yena tena prakāreṇa manaḥ kṛṣṇe niveśayet," which means "somehow or other." This is from our main ācārya. He says, "somehow or other, you figure it out..." I love how Prabhupāda also says in a purport when he's talking about how devotees should live in the world—because you have to make a livelihood—he says, "Be inventive. Figure out a way. It's not that important what you do, just as long as you can survive somehow or other, be inventive." So he says, "somehow or other, stay alive in Kṛṣṇa consciousness." And if I can consider this, rather than thinking that I have to be stellar in all my activities, the best in everything, and I have to show up in such a way that everyone sees that I'm faultless, if we simply accept this basic principle of staying alive, I personally find it encouraging. What do you think? So I thought I would just mention a few points about staying alive, the ways that I stay alive or try to -habudubu. Habudubu means—as you well know, (just saying if somebody doesn't know)—the sound of drowning. Habudubu, everyone say it three times. It's bubbles on top of everything. Someone says, "You forgot to pay for the insurance, and the car just got hit, and her son ran away." That's habudubu. Is it practical? How do you stay alive when what you expected didn't happen, or something that is so disturbing to the mind—somebody criticizes you—that hurts right ? out of nowhere! And suddenly, you're walking along singing a song, and next thing, you're like, "Oh my God, maybe I'm doing it all wrong."     Lama Zopa Rinpoche emphasizes that the five powerful mantras (Namgyalma, Mitukpa, Kunrig, Stainless Pinnacle, and Wish-granting Wheel) are very powerful for purification. When someone has died, if you recite these mantras while thinking of them, it protects them from the lower realms. Even if they're already born in the lower realms, they will transcend into a higher realm. It's also very powerful to recite for living people and animals.Rinpoche advises that before death comes, we must ensure that we have a good rebirth. This is not the ultimate achievement of this life; however, if we're unable to achieve enlightenment in this life, we need a good rebirth (either in a pure land or the perfect human rebirth) to complete the path to enlightenment.Rinpoche highlights how all the suffering in this life is the result of self-cherishing. Conversely, all happiness and realizations of the path to enlightenment are the result of bodhicitta. Bodhicitta comes from the root, compassion. Compassion is generated by depending on the kindness of every single sentient being. Therefore, all our present, past, and future happiness—including enlightenment—is received by the kindness of every sentient being. Rinpoche encourages us to meditate on the kindness we've received from every hell being, preta being, hungry ghost, animal, human being, sura being, asura being, and intermediate stage being. Rinpoche concludes by stating that there's nobody to cherish other than sentient beings. Therefore, we must free them from all suffering and its causes and lead them to enlightenment. With this motivation, we take the oral transmissions and recite the mantras.Rinpoche bestows the oral transmissions of the Mitukpa, Kunrig, Namgyalma (short version), and Stainless Pinnacle mantras. He also bestows the Milarepa mantra, which he received from His Holiness Serkong Tsenshab Rinpoche, who is the incarnation of Marpa's son, Dharma Dode. Lama Zopa says that the Milarepa mantra is also a powerful purification for a dying person. Reciting it causes you to be born in the pure land of Milarepa, and then you receive teachings from Milarepa.From April 10 to May 10, 2004, Lama Zopa Rinpoche gave extensive teachings during the Mahamudra Retreat at Buddha House in Australia. While the retreat focused on Mahamudra, Rinpoche also taught on a wide range of Lamrim topics. This retreat marked the beginning of a series of month-long retreats in Australia.     'Yas tu sarvāṇi bhūtāny ātmany evānupaśyati sarva-bhūteṣu cātmānaṁ tato na vijugupsate.' This is Śrī Īśopaniṣad (Mantra 6). Anupaśyati, as Prabhupāda explains in his purport, means that you should systematically see all living beings as parts and parcels of the Lord. And 'na vijugupsate' means, if you have that vision, that systematic vision, it doesn't mean that you suddenly try to imitate the uttama-adhikārī. But what Prabhupāda says is you should follow in the footsteps -anupaśyati, systematically see that there's a soul within everybody. Then na vijugupsate means that you won't hate anybody or any being, because you're not repulsed or attracted by their body, because you're seeing their spirit soul. So that's our directive. When we say to somebody, "You look spiritual," we mean it. Because you look at them and you see there's consciousness coming through their eyes, and they're animated. How is that? It's because there's a soul in the body. That soul's a part and parcel of Kṛṣṇa. What's more, there is Supersoul, you can offer obeisances. Kṛṣṇa says to Uddhava in the Śrīmad-Bhāgavatam, to a dog, because you realize Kṛṣṇa is within the heart of the dog, and therefore you offer obeisances.
