Gain physical freedom through nutrition. Sustainable nutrition builds a foundation for health and physical freedom. What is Physical Freedom? Let's first consider financial freedom, a term often discussed. We define financial freedom as the ability to do what you want with your time. You achieve this because you have earned enough time (in the form of money) to be able to do what you want with your time. Physical freedom is the ability to be able to do what you want with your body. You earn this through the discipline of healthy training and nutrition habits over time. You can gain physical freedom through nutrition and exercise habits. You build habits that support your goals. These habits become joyful. You have agency and the ability to influence your life. Things do not just happen to you. Sometimes things are outside your control (injuries and illnesses). Either way, you roll with the punches and control what you can. Physical Freedom Through Nutrition: Body Image & Bad Goals Many of us spend all or most of our lives walking around comparing ourselves to some image or celebrity. For many, that is Bradd Pitt in Fight Club. Andrew actually experience 6-pack abs and extreme leanness in an environment he would never want to recreate, where he was training like crazy, had little freedom and was undereating. Later, Andrew learned how to build sustainable habits that did not require extreme diets. He actually was able to enjoy the process and sustain that level of leanness. He had built habits that supported his goals. The Brad Pitt goal was a false idol. Physical Freedom Through Nutrition: Love the Process Learning to love the process really matters. Both Andrew and Niki have found themselves able to enjoy periods such as the holidays, where they kick back a bit, enjoy more alcohol, more desserts, but do not eat to physical discomfort. Ironically, when Andrew had less healthy habits, he was more anxious about holiday periods, felt he could not enjoy them, but regularly ate until he was physically sick. Approaching these periods with the long-term in mind, no anxiety, and knowing you can enjoy them but that includes enjoying yourself after the meal (not feeling sick) is sustainable and healthy. Achieve physical freedom through nutrition. Programming Difficulties for New Dads Having a newborn changes your life, and training has to change as well. For a dad, the change is different than for a mom. For the mom, she undergoes physical changes for 9-10 months leading up to the birth, so her training has to change prior to birth. For fathers, however, their changing (mostly) does not change before the birth. At birth it changes drastically. This is an especially busy time for fathers, and part of it is the lack of predictability. When will the baby sleep (when will mom sleep). You might think you were going to train and then mom falls asleep and you're watching the kid. Flexibility is required in your training for these programming difficulties for new dads. Programming Solutions for New Dads If you have a coach, it's important to talk to your coach prior to the birth about what is likely realistic (this will change). Lower your expectations for how long training will be. You might also have to cut training sessions short. Both Andrew and Niki have seen many clients train with their babies in the gym. This is fine. Something like a one life a day program is probably what is called for. It helps if you have a home gym or at least some equipment at home. If you don't have a home gym, you need to make your gym as convenient as possible. Something that is on the way to work and open early so you can workout early is likely ideal. Working out before bed affects sleep. Overcoming programming difficulties for new dads also tends to involve more autoregulation and lifter control. The client might decrease the weight and volume a bit if they're exhausted. If they're feeling good, they might go for extra reps or an extra set. Aim for at least 3 workouts a week. Think about getting in a training session over a series of workouts, as opposed to one longer workout. Mindset Shift for New Dads How you define training and training sessions and how you identify that will have to change. 4 sessions of 90 minutes a week is probably over. Uninterrupted training sessions and full body training sessions are probably on hold for a bit. Productive, useful training, however, does not have to look like that. Planning, preparing, and completing something each week is massively better than taking weeks or months off. Here are some thoughts on overcoming programming difficulties for new dads. Although I grew up in a city, my mom always had a vegetable garden. Each spring she would purchase plants at the farmer's market to transfer to our backyard. When my wife and I moved into our home, I continued the tradition. However, I loved planting seeds because the process of seeing seeds transform into living plants that produce vegetables has always fascinated me. In John 12, some Greeks approached Philip with the request to see (or speak with) Jesus (vv. 20–21). Both Andrew and Philip shared this request with Jesus (v. 22). In turn, Jesus made a startling announcement: “The hour has come for the Son of Man to be glorified” (v. 23). This seems to have been prompted by the desire of these Greeks to see Jesus. Previously in his Gospel, John had made it clear that Jesus' time had not come (John 7:6, 8; 16:25). Jesus' initial outreach was to the Jewish people, but something evidently had changed. Could it be that these non-Jews were seeking Jesus? That's why Jesus said, “Very truly I tell you, unless a kernel of wheat falls to the ground and dies, it remains only a single seed. But if it dies, it produces many seeds” (v. 24). By using this illustration of the seed, Jesus was explaining the impact His death would have on all people, including these Greeks. A seed, once planted, will grow, transform, and produce fruit. Jesus would lay down His life on the cross, but through that sacrifice He would draw people from around the globe to Himself (vv. 32–33). >> Jesus' death on the cross paved the way for all to have a relationship with God. Andrew Foy has a Different Conclusion on a Big JAMA paper on CV risk reduction

The study in question is a randomized clinical trial looking at the Million Hearts Model. This model paid health care organizations to assess and reduce CV risk. Obviously, this is an important goal. Heart disease, specifically, atherosclerotic vascular disease, is a leading killer of humans. Any reduction of heart disease should have a benefit on both a person and a population. But paying health systems to do specific things is a policy intervention. Even though a policy, like this one, makes sense, policies can have benefits and potential harms. (An example is the hospital readmissions reduction program (HRRP), which penalized hospitals for excess readmissions. This resulted in a fewer readmissions but it also associated with an increase in death rates in patients with heart failure.)Both Andrew and I were happy that the nudging of Million Hearts was studied The Trial and ProgramThis was a big pragmatic cluster randomized trial that ran over 4 years. More than 300 organizations were randomly assigned 1:1 to have the Million Hearts model or standard care. There were two parts of the model. First there was $10 for every patient who had their 10-year risk calculated with a risk equation. (ACC/AHA is a simple one you can do in 15 seconds with a smartphone.) Then CMS paid each organization $0, $5, or $10 PBPM for each high-risk beneficiary with an annual risk reassessment, with monthly payment amounts dependent on mean risk score change across all of the organization's high-risk beneficiaries reassessed.Keep in mind that the only components of the risk calculation that are modifiable are cholesterol and blood pressure. (*smoking cessation for smokers). Foy pointed out that Million Hearts was in many ways an incentive system to nudge providers, who then may nudge patients, to take more BP and cholesterol medicine. To receive new posts and support our work, consider becoming a free or paid subscriber.The authors chose two primary outcomes: one a MACE endpoint with MI, stroke, and TIA. The second primary was the same as the first, plus CV death. They originally planned to include only high-risk patients, but then added moderate-risk patients. This factored heavily in the results. Patients were mostly 75 year-olds, men-women split 2/3rds, 1/3rd. Outcomes were derived from claims data—which is messy when it comes to judging MIs and TIAs and specific causes of death. The Results:The first primary endpoint (MI, stroke, TIA) occurred at a rate of 14.8 per 1000 patient-years vs 17.0 per 1000 patient-years. The Hazard ratio came to 0.97 (90% CI - 0.93-1.0). The P-value was 0.09. (The authors had previously stipulated that the P threshold would be 0.10). The second primary, adding in CV death, was similar. A HR of 0.96 (90% CI 093-0.99) and a P = 0.02. These are positive results. But let's look further. Drivers of the Results: The results were driven almost exclusively by moderate risk patients. Look at Table 3. Reductions in events rates were largest and significant statistically in the moderate-risk but not high-risk group. That is something we have emphasized here at Sensible Medicine. Even though you would think that high-risk patients have the most to gain, they also have more competing risks and perhaps more chance for treatment harm. Like so many other studies, the sweet spot for primary prevention seems to be in the moderate-risk group. Unintended Consequences: A second finding, noted by Andrew, was the highly significant increase in all-cause hospitalizations in the intervention group. These had the most significant p-values of the entire study. Other Limitations:The Million Hearts model randomization was offered to more than 500 organizations but only 342 accepted. This raises the question of generalizability. Were the 342 organizations special in some way? Another factor is that outcomes were modeled on a sample of events—not raw counts. The choice to use 90% confidence intervals rather than 95% confidence intervals and P thresholds of 0.1 rather than the more standard of 0.05 is a weakness. For instance, the first primary endpoint would have missed significance if this were evaluated in the usual fashion. I did not find a strong justification for this choice. Readers with statistical expertise, please weigh in. Our Conclusions: First, we were both happy that a policy was studied rather than just implemented because it made sense. This should serve as a model for future policy endeavors. Second, there did look to be a modest effect on reducing important outcomes. And, these were driven mostly be moderate-risk (not high-risk) patients. This argues for a heterogenous treatment effect based on co-morbidity. Third, the statistically significant increase in all-cause hospitalizations in the intervention arm suggests that more aggressive attempts at blood pressure and cholesterol levels may have risen the risk of off-target ill effects. In the end, Andrew felt like the study was a wash. He did not feel strongly that the Million Hearts endeavor made a real difference. Comments on our Audio— I think we misspoke about the patient years. We said per 100,000 patient years. It was 1000 patient years. I also think we misspoke about deaths being similar. This is true, of course. However, Andrew's participation in the miracle is often overlooked. After a short conversation with Philip that only focused on the lack of resources, Jesus's attention was turned toward Andrew when he pointed out the boy's supply. Both Andrew and the boy were invited to participate in Jesus's miracle of feeding thousands of people. What an honor to be included in this miracle! Miracles of God, such as the feeding of the five thousand, provide opportunities to be part of what God is doing. We should remain watchful for opportunities to be used by God in a miraculous moment. Provided as a free resource by DataStax https://www.datastax.com/products/datastax-astra?utm_source=DataMeshRadio (AstraDB) In this episode, Scott interviews two Domain Driven Design (DDD) experts from Thoughtworks - Danilo Sato Director and the Head of Data & AI (part of Office of the CTO) and Andrew Harmel-Law, Technical Principal. This was a further delve into Domain Driven Design for Data after the conversations with Paolo Platter and Piethein Strengholt. Danilo and Andrew gave a lot of great information about Event Storming, domain definitions and boundaries, ubiquitous language, and so much more but the main theme was "just get people to talk to each other". DDD is about bridging the gap between how the tech people talk and how the business/business people talk; if you are doing it right, both sides can understand each other and then the engineers can implement those business process learnings as part of the code. For an initial PoC, Danilo recommends starting with 2-3 data products. It is better if you can do the PoC across multiple domains but it isn't necessary. Validate value and do it quickly. As Andrew mentions, the earlier you can show value, the less pressure there is overall. Look for the initial quick wins while also building for the long-term. One key thing to remember, per Danilo, when doing DDD for data and data mesh in general: it is always an iterative process. Andrew briefly discussed a way to do DDD in more of a guerilla style than the blue/red books (well known DDD guides). Don't get ahead of yourself as Max Schultze mentioned in his episode. Do not let the size of the eventual task throw you into analysis paralysis. Andrew talked a lot about how normalization and strong abstractions on the application side make it very difficult to re-add the context lost when you normalize. Both Andrew and Danilo talked about the need to embrace complexity. If you want context, you have to accept there will be complexity. In the pursuit of simplification, you lose the richness, and that is VERY hard to reconstruct afterwards. Some practical advice for boundary definition is that the boundaries need to be very clear but malleable. Build everything with an eye that it will evolve. Before you start splitting into many 2 pizza teams, look at the big picture and select some coarse-grained boundaries. It is MUCH easier to split later than it is to glue things back together. Inclined bed therapy is where you raise the head of your bed by 6 inches from the bottom of your bed. Inclined bed therapy means that your entire bed is tilted at an angle instead of just raising your head by folding the mattress. There are many mattresses that fold at the hips and will raise just one part of the bed either up or down. Inclined bed therapy means keeping your bed flat (from head to toe) but simply tilting the entire bed at an angle. What are the benefits of inclined bed therapy? There are many many amazing benefits. They are normalized blood pressure, increased circulation, feeling more rested and the fact that your blood pressure and circulation increases has a dramatic impact on many health conditions from depression to eye problems and much more. What I love about inclined bed therapy is that it doesn't take a lot of money (if any!) to simply modifiy your current sleeping arrangements. And it is a one time job. You invest 20 minutes to change your sleeping situation around and then you don't have to do anything more. It's a one time investment in your health that takes almost no money but the upsides are huge! Today we wanted to discuss a national level leadership event that Jake was a part of a couple weeks ago. Jake is a part of Medina County Leadership. He was asked to join a national level event to represent Medina County where they discussed all kinds of topics that are relevant during these interesting times. Todd was perfect to join us today as he used to be a part of Leadership Ohio. So he definitely had some great things to offer on the topic. I love that Jake is so interested in helping his community and hopefully his country grow together and not apart like it seems to be doing lately. Please enjoy our conversation. As a political comedian Andrew Heaton has entertained numerous think tanks and advocacy groups, student associations, and sinister political action committees.  He’s a regular at Electoral Dysfunction at the People’s Improv Theater in New York, and the Totally Dishonest Media Show at Stand Up New York.  He hosted the award-winning series EconPop, a comedy show which explained economics through popular culture. Andrew Heaton is the author of the best-selling work of political satire Laughter is Better Than Communism, and two funny paranormal novels: Frank Got Abducted (about aliens) and Happier as Werewolves (a tender coming of age novel involving werewolves). Today we have a special guest on the podcast, my very good friend, Todd Gienger. I have known Todd for years and he is one of my closest friends. I've been looking forward to Todd being on the show for quite some time now. We talked today mostly about the Corona Virus (Covid 19). It's such a large part of all of our lives right now that we can't seem to get away from it. Jake definitely got pretty riled up during the podcast. We all know that Baby Boomers are the largest generation in America and are also the largest generation that owns small businesses. It is speculated that two-thirds of the businesses in the market are owned by baby boomers, which would be equivalent to more than four million businesses, and now they are coming to the market. Andrew Buckrell is a mechanical engineer and amateur triathlete. Is it part of the Power Platform? At the risk of annoying my BC Partner friends once again, I will take a stab at it. Origins It would be helpful to know the origins of Dynamics 365 Business Central "BC", to understand how it came to it's current position, and then I'll discuss it's current position. Once upon a time, there was a product called Navision that Microsoft acquired. There is more to that story, but that is all that is relevant here. That product, since shortened to "NAV", is still alive and well and in use by many businesses around the world. Several years ago, Microsoft started their shift of Business Applications to the cloud, following the tremendous success of Office 365 and Azure. The first product to make that move was Dynamics CRM, and at the time it was launched as "Dynamics CRM Online". Dynamics CRM was not the only business application in the stable, it was just the first to go SaaS. Other products included Dynamics GP, Dynamics SL, Dynamics AX and Dynamics NAV, each of which were different flavors of Enterprise Resource Planning or "ERP". Enterprise Resource Planning is a pretty vague term. It could include human resources, supply chain management, sales activities and many more, but all of these activities revolve around a General Ledger in an ERP system. GL, AR and AP are the common denominators for each of these systems... and every business must have these components. All enterprise sized companies have an ERP, and most Midsized businesses do as well. Smaller businesses may have a solution like Xero or Quickbooks, that fills this purpose at a smaller scale for simpler needs. Many Midsized businesses are also using these simpler products... some successfully, and some who have outgrown them and are considering an ERP. Darwin Darwin's Theory of Evolution by Natural Selection was not conceived for software. But similar rules apply. Basically the strongest will survive. What was strong yesterday, may not be strong tomorrow as the environment changes, and when previously strong players do not adapt, they are replaced by new strong players. This was what drove Microsoft to the cloud in the first place... seeing other players adapt, and become leaders. So Microsoft adapted as well, and given their war chest, they were able to adapt very quickly to a SaaS model. But along the way choices had to be made. One of those choices was, which ERP solution(s) would take the SaaS path. Shifting an on-premise product to SaaS is no small feat, and requires a significant investment. Having four on-premise ERP solutions, it was obvious that all four would not go SaaS. Microsoft drew a line across their customer base, dividing Enterprise on one side and everybody else on the other, and decided to pick one ERP for each side that would go SaaS. Why not just pick one? Well, I didn't have a vote, but I wrote about that here. Regardless, Microsoft selected AX for the Enterprise and NAV for... everybody else. It was pretty hard to argue AX as the Enterprise choice, but there was some debate about NAV vs. GP. GP is much bigger in the US, but NAV is actually bigger globally, so that decision made the most sense.... unless you were a GP partner. Cloudification I won't go into the cloud journey for AX, instead I will focus on the journey of NAV... which actually starts with CRM. Several years ago, Microsoft had an idea to create a specific offering for SMB called "Business Edition". It would be a scaled down version of the Enterprise CRM solution, better suited for the needs of Smaller businesses. Shortly into that initiative, the decision was made that this would be the best place to start the SaaS journey for NAV as well, and thus began the project code-named "Madeira". Given the SMB target segment, this was going to be positioned as a Quickbooks/Xero competing product. Understand that this was well before the idea of a "Common Data Service" was even on the future roadmap. BTW, another product that got it's start in the Business Edition effort was Dynamics 365 for Marketing. About a year into the "Business Edition" effort, Microsoft decided that the path they were on, was not going to reach the original goals as intended, and the goals had shifted as well. CRM itself was heading down a path of componentizing its parts, and separation from its platform... this ultimately led to what we now know as the Common Data Service. But what about "Madeira" and Marketing? Dynamics 365 for Marketing continued it's journey as an independent application, no longer bound by the "Business Edition" limitations. And Madeira? Microsoft Launches Business Central! Project Madeira, similarly became unbound from the Business Edition limitations, and was launched as an independent application called Dynamics 365 Business Central. There are a lot of side routes that this took that I won't go into here. Today, Microsoft Dynamics 365 Business Central is one of the fastest growing SaaS Business Applications in Microsoft's stable. A far cry from my earlier predictions that it would never see the light of day. Maybe the product owner at the time, Marko Perisic, made it successful just to prove me wrong. Nevertheless, it is on fire. So where did it go? Moving Uptown Once the Business Edition tag was removed, Marko quickly pivoted BC from a Quickbooks/Xero compete, into a product those customers could move up to as they outgrew them. While the CRM side of the house was going through a massive evolution into what we now know as the Power Platform, Marko was able to stomp on the gas pedal. BC was evolving at a faster pace than any other product, partly because it was in a lane by itself. Weekly updates were the norm, and the new northstar became NAV on-premise parity. A goal that I believe has been largely met. SMB might be a fine market, but this thing could go way beyond that... in some cases even standing toe-to-toe with AX for some enterprise customers. So what exactly is BC? What is Business Central? At it's heart, Business Central is a SaaS ERP. "Manage your financials" is at the top of the product's page of capabilities. As you would expect from an ERP it has the GL, AP and AR core functions, but it is much more than that. Like everything Microsoft is doing in Business Applications today, BC is infused with A.I. The next listed capability is "Automate and secure your supply chain", that sounds pretty "enterprisy" to me. Next up is "Sell Smarter and improve customer service", so BC also includes some CRM capabilities. After that is "Keep projects on time and under budget", so we can add some project management capabilities to the list. The last item is "Optimize your operations" for inventory and warehouse management. Clearly this product has grown up quite a bit from its humble "Business Edition" beginnings, and there is a growing number of partner extensions (ISVs) to extend the capabilities even further. The pricing is pretty straightforward, at least in comparison to the Customer Engagement applications. There are only three flavors: "Essentials" at $70/user/month, "Premium" at $100/user/month, and "Team Members" at $8/user/month. From the Pros Since I am not an expert on Business Central, I reached out to two guys who I know are knee-deep in the product for their thoughts. Andrew King is a Partner at WebSan, a Toronto based Business Applications partner. Since WebSan supports both Dynamics 365 Customer Engagement "CE" as well as Dynamics 365 Business Central "BC", I thought he would be a great guy to contrast the two. Andrew shared that there is some confusion in the market, "The products are as different as Golf and Baseball, but we frequently see customers asking about the product that does not meet their needs. Like BC for CRM needs, or CE for Supply Chain. Would they work? I guess if you like playing golf with a baseball bat". James Crowter is the Managing Director of Technology Management, a UK based Business Applications partner. Technology Management also supports both Dynamics 365 Customer Engagement "CE" as well as Dynamics 365 Business Central "BC", and has a long history with NAV. James talked about the amazing pace of innovation, "I can talk to a new customer in the morning, and have them using BC by the afternoon, which is amazing! For the right customer requirements, BC is a no-brainer... but not for sales workloads, there is not even a workflow capability (for sales), which is a key component for sales!" What Business Central is not? Both Andrew and James agreed that while Business Central is an awesome solution, it is not really a very good Sales tool. Both agreed that they would typically position Customer Engagement for any CRM type requirements, and they often position both products for a true end-to-end solution. They both had some choice words for the Sales Capabilities of BC, and clearly neither one had any interest in activating those, but instead would bring in Customer Engagement for any customers looking to transform their sales processes. So I guess it boils down to, what it is that you are trying to transform. If you are looking to modernize your financial processes, including supply chain and inventory, or have outgrown Quickbooks or Xero... Dynamics 365 Business Central is an excellent option, and you could reach out to Andrew or James for more guidance on that. If you are looking to modernize your sales or service processes, Dynamics 365 Customer Engagement is the clear way to go, and Andrew, James or myself can help you explore that further. But are these the only Microsoft options? Other Big and Small Options If your ERP requirements are really big, and include things like HR management, you might look at Microsoft Dynamics 365 for Finance and Operations. This is Microsoft 's enterprise-grade ERP. Again, we have seen Business Central get into some pretty big businesses, but F&O is the next step up. Conversely, if your sales or service requirements are fairly basic, you may find that Dynamics 365 for Customer Engagement is a pretty big hammer to start with. In that case, you might want to explore RapidStartCRM, our PowerApps based CRM solution. It is an excellent option for small businesses or enterprise departments, built on the Power Platform and running on CDS so you can never outgrow it. Hopefully I cleared up some confusion, but if I actually made you more even confused than before... please keep that to yourself.

On the American Honesty segments we like to discuss topics that society has deemed inappropriate. Our goal is to just get our opinions out in the open for a discussion. We believe that the ability to talk about these tough topics is seriously lacking in today's society, as people seem to get offended too easily. Joining us today is Cody Belles. We talked a little bit about Cody on the last American Honesty podcast as he made some comments online and figuratively dropped the mic to end the discussion. Cody wanted to talk about parenting after watching a youtube video about parents taking their small child to a gay pride march. While none of us have a problem with the march itself. We can't seem to agree on if that's what is best for the child as they are not old enough to make that decision for themselves. American Honesty is a project that Jake started a couple of years ago. So now using the podcast as our platform we are going to carry on the project that Jake started. Hopefully it will lead to some good conversations that others can enjoy as much as we do. The three of us wanted to cover a tough topic that most people would not feel comfortable talking about. Today we are going to have a conversation about White Privilege. Andrew and Jake have been my biggest supporters of the podcast and we've been trying to get together for a while record an episode together. But our schedules have not allowed that to happen until today. The three of us have decided to start a special segment on this podcast where we talk about these kind of tough topics and current news. Hopefully we'll be able to bring in a fourth guest from time to time who will have a differing opinion. We are calling this new segment American Honesty. But The Morning After Ministry Show must go on, so Andrew's wife Melissa is filled in as co-host this morning. Melissa is currently leading the children's ministry at Safety Harbor Community Church, has spent a decade or so as a small group leader, and spent a formative summer interning in a large church's children's ministry. We opened the show by talking about a series called “Hurt” that we released on DYM a few years ago. We have redone the look and feel of the series to minister to student and resource ministries who are addressing issues brought up by 13 Reasons Why. The news season comes out in a few weeks, and we are giving the series away to anyone who leaves us a rating and review on itunes. Expose the intern to as many areas of the church as possible 5. Set the intern up for long term success. While Unhealthy internships…1. Make the intern an errand-runner or coffee guy 2. Never let the intern take the lead 3. Never talk about money 4. Let the intern be bored 5. Shelter the intern from the pain and realities of ministry. Next Andrew played a conversation that he had with our friend Collin this weekend. Both Andrew and Collin spent significant time interning at high profile churches, and neither internship turned out the way either of them expected it to. Episode 13 Intern(ons) and Intern(offs)A few weeks ago we spent an entire show talking about side hustles, and this morning Tim was off in Vegas for one of those side hustles. But The Morning After Ministry Show must go on, so Andrew's wife Melissa is filled in as co-host this morning. Melissa is currently leading the children's ministry at Safety Harbor Community Church, has spent a decade or so as a small group leader, and spent a formative summer interning in a large church's children's ministry.We opened the show by talking about a series called “Hurt” that we released on DYM a few years ago. We have redone the look and feel of the series to minister to student and resource ministries who are addressing issues brought up by 13 Reasons Why.
