Allostatic load. compassion fatigue: “a state of exhaustion and dysfunction biologically, psychologically, and socially as a result of prolonged exposure to compassion stress and all it invokes." also called vicarious trauma. burnout: a state of emotional, physical, and mental exhaustion caused by excessive and prolonged stress Think of these as a spectrum. EE more from life stressors. CF/B more from work/responsibilities. EE could be from carrying unresolved trauma, stress overload, being out of alignment, living with or experiencing systemic oppression, late state capitalism, chronic stressors, being chronically dysregulated, Sx: apathy, withdrawal, isolation, exhaustion, soul tired, irritable, feeling stuck or helpless, not being able to do what you know would help, only paying attention to negative, headaches, tense muscles/sore body, trouble sleeping, brain fog, depression, numb,   How to mitigate/fix Heal trauma. Rewire your mind for growth, healing, helpful thoughts. Cut out stressors/toxic people, things etc. Radical acceptance. Do whats useful to have done and possible to do. They identify what their client/athlete's strengths and weaknesses are. Coaches and teachers, mentors, advisors, and co-creators. A coaching relationship is a two-way street. The coach helps clients create a better possible future. In the process, both the athlete/client elevate the realm of possibilities and potential of life and the goals set. Adaptive Capacity: People who are the best in the world at what they do can adapt to tremendous levels of psycho-physiological stress. Part of it is genetic, and part of it is developed. So, just like any capability, people are wired with specific innate abilities, and they can constantly improve them. When someone exceeds their capacity to adapt to acute or chronic stress, they get injured, burn out, and develop physical and mental health issues. Adaptive capacity is training your stress responders so that your gas tank(resolve) is higher/bigger. This allows you to do more with less cost. Building this capacity involves five basic things: sleep, exercise, mental fitness, nutrition, and healthy relationships. Allostasis for Adaptation: When your body tries to achieve stability through change. Allostatic load is the cost that is incurred whenever you experience a stressor. Your adaptive capacity gets bigger if you can build more capacity for more allostatic load. When that happens, what once was incredibly taxing is now your baseline. There is a limit to this, but most people have yet to get close to their absolute capacity. This is, again, psycho-physical. Now, 'allostasis' is the process by which our body seeks to maintain stability, or homeostasis, through change. It's like a scale, constantly adjusting to keep balanced. But when we're exposed to repeated stressors, this scale can get tipped. The constant 'wear and tear' on our bodies, the continual need to adjust and readjust, can lead to what's known as allostatic load or overload. In the context of addiction, this allostatic load can become a heavy burden. The compulsion to seek and take a drug, the loss of control in limiting intake, and the emergence of a negative emotional state when access to the drug is prevented, all contribute to this load. Allostatic load is a term coined by Bruce McEwen and Eliot Stellar in 1993. It refers to the physiological consequences of chronic exposure to fluctuating or heightened neural or neuroendocrine response resulting from repeated or chronic stress. In simpler terms, it's the "wear and tear" on the body that accumulates as an individual is exposed to repeated or chronic stress. In the context of addiction, allostatic load can become a heavy burden. Abstract: The brain continuously anticipates the energetic needs of the body and prepares to meet those needs before they arise, a process called allostasis. In support of allostasis, the brain continually models the internal state of the body, a process called interoception. Using published tract-tracing studies in non-human animals as a guide, we previously identified a large-scale system supporting allostasis and interoception in the human brain with functional magnetic resonance imaging (fMRI) at 3 Tesla. In the present study, we replicated and extended this system in humans using 7 Tesla fMRI (N = 91), improving the precision of subgenual and pregenual anterior cingulate topography as well as brainstem nuclei mapping. We verified over 90% of the anatomical connections in the hypothesized allostatic-interoceptive system observed in non-human animal research. We also identified functional connectivity hubs verified in tract-tracing studies but not previously detected using 3 Tesla fMRI. Finally, we demonstrated that individuals with stronger fMRI connectivity between system hubs self-reported greater interoceptive awareness, building on construct validity evidence from our earlier paper. Taken together, these results strengthen evidence for the existence of a whole-brain system supporting interoception in the service of allostasis and we consider the implications for mental and physical health. Obeng-Gyasi is a James surgical oncologist and a researcher who specializes in breast cancer. Her research focuses in on how a person's income level and access to healthcare, as well as the financial and emotional stress of a cancer diagnosis and treatment, can result in more aggressive cancers, worse outcomes and even higher death rates. These issues are more pronounced in lower-income populations and among many minority groups, such as Black women. For example … “We've found that women with no health insurance or Medicaid presented with more advanced stages of breast cancer and had a worse overall mortality rate than patients with private health insurance,” Obeng-Gyasi said. Other areas of her research look at the Allostatic load of breast cancer patients and how it impacts their long-term outcome. Allostatic load is a way to measure the cumulative burden of stress in a patient. “We've found that [cancer] patients with a higher Allostatic load have a higher risk of death from their cancer,” Obeng-Gyasi said, adding additional research is trying to determine if a person with a long-term, high Allostatic load would have an increased cancer risk. Another research study found that Black women with breast cancer have a higher rate of what Obeng-Gyasi calls a “rapid relapse.” As a result of these findings, Obeng-Gyasi has worked with Barbara Andersen, PhD, a James psychologist and an expert in reducing stress. “The results of the study are compelling,” Obeng-Gyasi said. “Patients who participated have a better chance of survival and a lower rate of their cancer coming back.”

Our previous Science with Sergio episode was focused on a preprint paper discussing predictive coding and fatigue. This was a pretty dense paper that relied heavily on Bayesian models of cognition. Basically, this means that our brains our constantly trying to predict what is going to happen, and that we are making little updates all the time to what we are doing (and what we are predicting) to make those predictions more accurate. Now, this may sound like some pretty esoteric stuff…and it is! So, for this episode, we figured we’d go to one of the sources and cover Karl Friston’s “free energy principle” for brain modeling. Friston has done a lot to popularize some of these concepts, so we chose his review paper “The free-energy principle: A unified brain theory?” Now, you may be wondering what esoteric models for thinking about consciousness and cognition have to do with exercising. Well, a lot of folks in the fitness space spend a lot of time talking about mindset. Rather than talking about mindset, we at the Legion Strength & Conditioning podcast would like to talk about prediction error and updating our priors. (Sidenote: This is a really great marketing strategy! You should try it!) Any sort of exercise performance is going to be largely mediated by the brain. Want to get less tired? Want to have better technique or learn skills more quickly? Want to learn to intuitively pass your way through workouts. All of these can be better understood through Friston’s free-energy principle, even if it doesn’t translate into immediately actionable fitness advice. Oh and in this episode I wonder what the deal is with positive prediction errors — meaning what happens when we make a significant prediction error, and the outcome is positive. The deal is dopamine. How- ever, current theoretical models for the role of oxytocin in social behavior pay little consideration to its evolutionary and developmental history. This article aims to broaden our understanding of the role of oxytocin in social behavior by adopting an ethological approach through the lens of Nikolaas Tinbergen’s 'four questions' – how does oxytocin work; how does the role of oxytocin change during development; how does oxytocin enhance survival; and how did the oxytocin system evolve? We argue that oxytocin is most accurately described as an allostatic hormone that modulates both social and non-social behavior by maintaining stability through changing environments. How Chronic Stress Contributes To Cognitive Decline We often think of brain disorders as separate from the body. Anything that affects the brain occurred due to factors that affected the brain.  But our understanding of the relationship between the brain and the body is far too linear to fully grasp the complex network of interactions that occur. We are seeing now how limited our knowledge is. More young people are dealing with depression. We believe this is due to an elevated allostatic load brought about by chronic stress.  The thing about chronic stress is that your body gets exposed to cortisol for a longer period of time. This leads to a disruption of your body’s normal processes. To put it simply, your body being put under a long-term, stress-response state result in health problems. While completely eliminating stress from your life might be impossible, having a better understanding of how stress affects your physical and mental health is key. We’re at the tip of the iceberg, but we have to keep on digging. The more we comprehend this fascinating, multidirectional interaction between the brain and the body, the more we can unearth the many facets of what contributes to mental well-being. About Our Guest Prof. Zoltan Sarnyai is a medically-trained, PhD neuroscientist with real research interests in the neurobiological mechanisms of stress and psychiatric disorders including addiction, schizophrenia, and depression. Prior to his role in James Cook University in Australia, he was a university lecturer in pharmacology and a fellow of Pembroke College at the University of Cambridge. He has been awarded the Curt P. Richter Award by the International Society of Psychoneuroendocrinology for his work in neuropeptides and brain functions. He was also the recipient of the Dupont Warren Fellowship from the Department of Psychiatry at Harvard Medical School. Stress and Allostatic Load His Background In Neuroscience Prof Zoltan shares being disillusioned while in medical school and getting excited over research involving neuropeptides and brain function. An outdated book on stress research piqued Prof Zoltan’s interest. A research article on the impact of social hierarchy and general health among wild baboons opened Prof Zoltan to the idea of the environment influencing the body and brain. His Fascination In Mental Health And The Environment Our understanding of mental health and the brain in relation to the body has evolved over time.  Brain disorders were seen as something related to brain function. The brain does not work in isolation, but there is a multi-directional communication occurring between the brain and the body. We don’t understand a lot about the interactions between the brain and the body.  Now, we know that the gut microbiome has a lot to do with how the brain functions and the brain directs the local environment for these bacteria to change. How Stress Affects The Brain and Gut Chronic psychological stress and trauma affect both the body and the brain.  Chronic stress leads to a leaky gut syndrome, which leads to bacteria interacting in the bloodstream. Immune cells react and activate our stress biology. The body develops more cortisol. Things in our bodies are interconnected in a nonlinear fashion making it more difficult to grasp.  Variables That Contribute To Stress Resilience Our genetic makeup contributes to our susceptibility or resilience to stress.  We have genes that are related to our biological stress axis. We can’t do anything about our genetic background, but we can do something about what happens to us, specifically during pregnancy. Not just environmental stress, but environmental toxins like alcohol and nutritional factors during pregnancy also play a role. A low-protein diet during pregnancy has lasting consequences in the physical and mental health of a baby. There’s also a long generational influence of the things that happened to our great grandparents that we see affecting their descendants. How To Achieve An Optimal Level Of Living Since we are already aware that stress, trauma, nutritional factors, and others have an impact on mental health, we can get started on prevention. Our focus on studying the genetic makeup of complex, chronic non-communicable disease will only explain a percentage of the condition but not all. Prof Zoltan talks about a study on mice that had the gene for Huntington’s Disease. Exercise and a complex cage environment were able to delay neurological deterioration in the mice. The early environment of a human is key to the development of the brain and mental health, with the first three years of life being critical. Nurturing makes a big difference. Early neglect has long-term consequences. Prof Zoltan shares the case of what happened in Romanian orphanages in the 1980s. A high-fat, high-carbohydrate diet is bad because it creates a low-grade, inflammatory response in the body. Exercise creates more newly-born cells in the hippocampus. Having more newly-born cells in the hippocampus contributes to better cognitive fitness. Thus, physical exercise helps to improve cognitive emotional and behavioural measures. The brain is very plastic. So while we cannot eliminate stress and trauma, we can devise interventions to help individuals recover. The Future Direction Of His Work Prof Zoltan shares his ongoing projects, one on integrating different biological measures into an index to predict either disease development or treatment response, and another on allostatic load and predicting mental illness. Allostatic load is the wear and tear of the body. It measures the dysregulation that happens in the body when the body is chronically exposed to cortisol, the stress hormone. An elevated allostatic load is being seen among young people between 15 and 25. There is a link between elevated allostatic load and mental well-being. Elevated allostatic load is associated with cognitive decline. Major depression is characterized by a certain degree of cognitive impairment. Since sleep increases our bodies’ ability to handle the challenges in the day, and stress hormones are highly biologically active, sleep research is expected to deliver exciting results. Over the past 20 years, my guest on today's show, , has been trying to recommend pillows for his patients that would put their head in the correct position so that they could get a great night's sleep and wake up rested. The problem is that a pillow is for your head and you can not support your head to get a great night sleep. But one day he was watching the movie “Crouching Tiger Hidden Dragon” in 2000 and the two actors came out of the room after “cuddling” all night. The camera gave a quick glimpse of the room they came out of. It showed what the two actors slept on, and to his amazement it was just a thin pad for their body and a block of wood in lieu of a pillow for their head.  In that lightbulb moment, Dr. Martone invented the exact sleeping tool we discuss on today's podcast. One of the props was a piece of wood, not as a pillow, but for their neck Peter had shoulder issues, couldn't identify the cause While camping, he used a shoe under his neck for sleeping; woke up well rested Used a shoe, and other odd objects for sleeping; shoulder and other health issues were resolved -Surprising problems caused by sleeping on one's side... 90% of Peter's patients have some type of damage to their cervical curve in the neck We lose the curve doing day to day activities, and there's nothing to repair it at night 98% of patients who lose the curve in their neck develop an unnatural curve in their lower back This eventually leads to scoliosis in the lower back The portion of the brain: main function is to control the body's 80% of proprioception into the brain comes from the spinal cord The vermis atrophies due to loss of proprioception It affects the pre-frontal cortex (executive functioning) Decreasing the curve in the spine severely limits glymphatic drainage Why we toss and turn at night: Pain centers and nerve centers are very close together When you fall asleep on your side, you can only stay in that position for a short period of time Body should be in a "neutral sleeping position" Alcohol inhibits pain sensors. You stay in the same position longer, and wake up in pain OURA ring If you didn't get all that, get this: Loss of the cervical curve = degeneration of the spine. It also causes the vermis to atrophy, which negatively affects the pre-frontal cortex. Side sleeping causes pain and discomfort, which in turn affects the quality of our sleep. The advantages of sleeping on the back versus the side or stomach... Use a pillow for the neck rather than the head Restore the cervical curve and decrease lumbar tension -How to train yourself to sleep on your back... Just start on your back 1-2 hours per night will be enough to help restore the cervical curve in your neck : 5-10 degree angle, should go to the base of your spine The body will eventually be able to go through the night on your back Total 2-3 month transition Sleeping on your back, you distribute your body weight over the greatest amount of space -The little-known hack called "Remembering" that may help with sleep problems... Peter struggled to sleep due to numerous things he was thinking about Needed to be "in control" of something Visualized people sitting in a specific seat after giving a talk; he went right to sleep Focus on something, but don't think about it You can't think yourself to sleep; you have to remember yourself to sleep Dreams are how the mind categorizes the day's events The further back in time you can remember, the more efficacious this technique will be -Ways to control your core temperature while sleeping... Allostatic load: Hierarchy of needs in bodily functions (heart pumping vs. growing fingernails) Your body cares more about your core temperature than even the immune system We get sick because there's a finite amount of energy our body can distribute Maintaining the body's core temperature maintains the allostatic load in our entire body Keep your core as warm as possible; keep hands, feet and head exposed People like to sleep on their side so they can curl up, feel protected Use a for a similar effect "Head garage" Place a pillow over your head, just above the nose -More little-known sleep hacks... Something awesome we read/listened to this week and why it's awesome. I was inspired to record this podcast by a discussion that took place on the Lower Insulin Facebook group. I love the conversation that goes on over there, but like many of debates we see around the Internet, the conversation is somewhat one-dimensional. Low-carb, high-fat, moderate protein, intermittent fasting and you'll be okay. After working with close to 1,000 athletes to improve their health, performance and longevity, we know that's not always true, and we’re confident that a complete solution must give consideration to everything we outline in this episode. The five things (in no particular order): 1. Eat a minimally processed diet food free of added sugar and vegetable oils (processed fats). Because processed foods: Are less nutrient-dense. Are designed to make you overeat. Increase insulin responses due to processing. Alter the gut microbiota unfavourably. Translocate endotoxins such as LPS across the gut wall. This induces inflammation and hyperinsulinaemia. Induce leptin and insulin resistance centrally which leads to overeating. 2. Get sufficient sleep and Sunlight! 3. Appropriately manage stress, social connectedness and purpose. Consider stress of dieting. 4. Move like a human, i.e. walk, stand, and occasionally lift heavy things. 5. Consider magnesium and zinc deficiency (especially in athletes). Dr. Tim Jackson is a doctor of Physical Therapy. He also studied nutritional biochemistry, digestive health and its systemic effects, as well as functional endocrinology. He helps people with all sorts of disorders over at HealYourbody.org. He also does online consults through his Heal Your Body Program. In this episode we talked about: What MTHFR means: (It's not your first thought. I know what you were thinking.) It stands for Methylenetetrahydrofolate reductase. (Say that three times fast.) It basically means that if you have it, you lack the ability to activate folic acid into methyl-folate. Then you don't have the useable form of folate and you have too much folic acid sitting around. That's when disorders start popping up. Miscarriages have been associated with MTHFR. Best place to get tested: 23andme.com. You will just get the raw data, no interpretations. Heterozygous means you have one copy Homozygous means you have two copies Polymorphism is a SNP (Single-nucleotide polymorphism) Genetics refers to actual mutations someone might have. Trisomy or Down's Syndrome are examples. Epigenetics are modifiable risk factors, which give us a predisposition to certain diseases or disorders. The key word is modifiable. They are not set in stone. "HG can be the straw that breaks the camel's back." Allostatic load means the sum total of internal and external stressors on the body. "It can take a disease from subclinical to clinical" (expressed). "Any stressor can effect the expression of genes." Exposome means everything you've been exposed to in your life. You can have too little or too much methylation. This effect neurotransmitter production (mood disorders), T-cells (infection and allergies), and Glutithione (which controls free radicals). Can Hyperemesis Gravidarum be an allergy to the hormones, the placenta or the baby? Short answer....YES. Neuro-immune is more accurate than "Auto-immune" to describe this. Liver Detox: Phase 1: oxydation Phase 2: conjugation-methylation Phase 3: Transport to the cell Milk Thistle supports Phase 1 detox. If phase 2 is not adequate, the byproducts of phase 1 sit around causing oxidative stress. B vitamins, NAC (N-acetyl cysteine-precursor to glutithione), Glutithione and Glycine support Phase 2 detox. Oxidative Stress causes all disorders, which go through 3 stages. Stage 1: energetic imbalance (you don't feel bad in this stage) Stage 2: Functional: Fibromyalgia or chronic fatigue syndrome. You feel the effects but there are no biomarkers to show what's going on. Stage 3: lesional-can be seen on an MRI (like Multiple Sclerosis) Inflammation causes fatigue ad premature aging. Lipid Peroxidation: damage to cell membranes What to do about it: Support the methylation cycle Reduce oxidative stress Diet: Clean out processed foods and additives. Remove sugar and gluten. Consider removing dairy Heal your gut (mood disorders are related to leaky gut.) Just because you don't have symptoms, doesn't mean you don't have a disorder. "The system that is disordered might not be what's expressing itself." Supplements: Glutithione: Oral liposomal form, transdermal creams and oral s-acetyl-glutithione Magnesium and zinc are cofactors for methylfolate and other b vitamins. B12 needs methylfolate. He suggests not taking supplements all at once but after changing your diet to reduce insulin (which is pro-inflammatory), increasing healthy fats (which fuels the liver, tissues and cells and helps balance hormones) then layer in supplements, one at a time. Minerals first, magnesium, then zinc. Then add B vitamins, starting with B12. The last supplement to add is methylfolate. Liver, gallbladder and pancreas: We know the importance of the liver. The gallbladder helps with bile flow. Bile is dependent on glutithione. If you're not methylating in the liver and the bile is backed up, toxins back up. This can mess up our ability to process carbohydrates (diabetes is carbohydrate intolerance) and this has an effect on our pancreas, which produces insulin.
