In this episode of the Twiniversity Podcast, Natalie talks with Maddie Rahlf, a twin mom of three girls, about the "double whammy" delivery - giving birth to one twin vaginally and the other by C-section in the very same hour. Maddie shares how her twins were a complete surprise - a natural, spontaneous twin conception with no twins in the family, discovered at her 12-week ultrasound - and how reading Nat's book helped her find solid ground once the shock wore off. Because Baby B had a marginal cord insertion and was measuring under the 10th percentile, Maddie's pregnancy included weekly maternal fetal medicine (MFM) monitoring and an induction at 36 weeks. Natalie and Maddie walk through the delivery almost minute by minute: an easy vaginal birth for Baby A after only a few pushes, Baby B flipping transverse the moment she had the womb to herself, the attempts to turn her, a cervix that closed back up, and the placenta concern that finally made a C-section the safest call. Then comes the twist - it was Baby A, the vaginally delivered twin, who ended up in the NICU on CPAP, while Baby B stayed right with mom. The conversation also gets into recovering from a vaginal birth and major abdominal surgery at the same time, the empowerment of being given the choice to try for a vaginal delivery, and Nat's "use your BRAIN" framework (Benefits, Risks, Alternatives, Intuition, Nothing) for making decisions in the moment. Maddie's biggest takeaway: if it happens, it happens - you figure it out one day at a time, and you'll have a pretty cool story to tell. "Once I started doing more core work...prescribed to me by a pelvic floor DPT, and what everyone says you should be doing after pregnancy...I was feeling pain," shares Lane 9 Coach Becky Anthony, head coach and owner of Taylored Training Run. Becky shares her story with a rare injury, osteitis pubis, which she was diagnosed with around three months postpartum. She was working with a physical therapist (DPT) on a return to run program, and had tried to check the boxes before getting back to the sport. But something felt wrong, and painful. And it took a little while to get an accurate diagnosis, and even longer before she was able to run again. No two postpartum experiences are the same, even for one person. Becky shares this in hopes of helping the next parent build their care team—starting with nutrition, working with a DPT and a coach, etc.—and reminding parents to advocate for themselves, and their care. Approximately 10-15% of women may develop an autoimmune disease within the first year after childbirth. Conditions like thyroiditis, lupus, and rheumatoid arthritis are commonly diagnosed postpartum! AND women who had an autoimmune disease prior to pregnancy experience a mild to moderate flare of symptoms 20-40% of the time. This is due to a variety of factors stemming from nutrient depletion, hormones all out of whack, lack of sleep, increase of stress on the body, and mental stress as well. From what I've studied and the researchers in the field that I've listened to, nutrient depletion is the biggest factor we have the most control over. What if one of the most "empowering" trends for women… is actually making us feel more powerless? A recent conversation with one of my students completely stopped me in my tracks. And it opened up a much bigger conversation about modern hormone culture, cycle syncing, female identity, emotional resilience, and what young women are being taught about themselves. But somewhere along the way, many women started absorbing the message that we are entirely at the mercy of our biology. May is Maternal Mental Health Awareness Month. It is believed that 1 in 5 new moms experience some level of perinatal mood or anxiety disorder. And when a new mom experiences postpartum anxiety or depression, her partner is 50% more likely to also experience anxiety or depression.
