Toggling calendars to get a girls' night on the books, rescheduling a coffee date for the third time, and “meaning to call” a friend that's been on our mind for weeks. "Behavioral Finance" is all the rage. BeFi (as the not-so-cool kids in the financial world call it) is the next phase in guiding individuals, teams, boards, companies, and leaders to better decision-making. There is plenty of material telling us where people get it wrong. Even the best brains get deceived by a litany of behavioral biases. These biases cause people to fall off the track of economic rationality. However, even with all of these labels, there is little guidance on how to identify and use this context. Until now . . . PETER ATWATER argues in his new book "THE CONFIDENCE MAP" that there is a straight forward mental model. https://www.amazon.com/Confidence-Map-Charting-Chaos-Clarity/dp/0593539559/ It can diagnose an individual's emotion and confidence, its directionality and its relationship to group and social mood. Further, Peter asserts that people (and their advisors) can use this information to pull decision-makers out of the own limitations of their own silos. Sofiya is passionate about increasing research and education on menopause and other understudied women's health issues, such as PCOS, endometriosis, and sexual wellness. Under the guidance of Dr. Lynnette Sievert, she studies grandmotherhood, variation in caretaking behaviors, and the impact of child-rearing on both grandmaternal and children's health and well-being. With the outbreak of war in Ukraine, Sofiya's work toggled to focus on the study of menopause, where she manages and coordinates an ongoing multi-year study on hot flash experiences and brown adipose tissue in perimenopausal and menopausal women. Thank goodness for these platforms, because let's be real – even just ten years ago, having an online business was much more difficult because this sort of technology just wasn't available. And while these applications allow online businesses to run seamlessly, I recently read a study that reported that the average person spends five work weeks – or nine percent of their annual time at work – context switching, a.k.a reorienting themselves after toggling to something new. That's a lot of wasted time, and beyond that, constantly having to readjust takes a toll on your ability to focus.  This article really got me thinking about some different ways that I could minimize context switching in my workday -- and since I've put some of them into practice, I've noticed that I'm much better able to stay in the zone. In this episode, I'm sharing the four ways I avoid context switching throughout the day, and how it's helped me to be more productive. [08:11] Look at all the tools you use and how you use them to decide if there's any way you could simplify the process. [08:47] The most important thing you can do is give yourself permission to stay focused and in the zone. Setting timers is a great way to do this. Toggling Empathy - For Deeper Influence and A Better Path Forward influence-009-toggling-empathy-02-audio.mp3 I am an empath extremist; I feel what others feel and I extend it beyond them. If it's joy, I feel it and I'm able to see its downstream implications. If it's trauma, I can see downstream, and extrapolate, and I feel that too. This is both a strength and a strength overdone. Over the years I've developed the skill of toggling empathy from full on to standby, when that would be most optimal (for the person I'm talking to AND for me). When empathy is on standby, it affords you the ability to see the positive when all is negative, and assess for negative when all is positive. If you leave it full on during negative emotions, it can be devastating. And if you leave it full on during positive emotions, you can miss a negative potential future. Don't ever turn it off. Empathy makes us human. It makes our relationships whole. It makes us work together toward a common good. Put it on standby so that you will still have access to it. You don't want possibility blindness. In extreme emotions we can't see other possibilities; in extreme celebration we can't see the potential dangers and in extreme negativity we can't see the potential benefits. Feel what others are feeling, show them that better place, and help them get there. Toggle empathy from full on to standby and back again. Here’s a reminder of his stance on CRT in the schools:Starting around the 30-minute mark in the new episode, David and I discuss the tricky defense of liberalism in the face of both CRT curriculum and anti-CRT bills. We also grapple with the corrosive effects of Twitter and, in particular, the commentary surrounding the racist massacre in Buffalo this week. On that note, a reader writes:I am a member of a mainline Christian denomination and parent of young children. My personal and professional experience of social media is centered on connections with clergy colleagues and active church members attached to a wide variety of Christian denominations. When news of the racially motivated shooting in Buffalo broke, my social media relationships immediately shifted to a flurry of outrage, comments about the pox of racism built into the American way, and pithy memes noting that the root problem of all that ails us is white supremacy.For example, one friend wrote in response to the Buffalo shooting, “The root cause of gun violence is white supremacy. We will not be safe from gun violence until we end white supremacy. White fam, we are the ones who can end white supremacy. It is on us.” Presiding Bishop Michael Curry of the Episcopal Church released a statement decrying the racism behind the shooting. Members of my left-leaning church have asked and encouraged me to preach from the pulpit about the evils of white supremacy and white fragility, especially now in light of the Buffalo shooting. However, I did not hear a thing from these same people or religious bodies following the racially motivated shooting by Frank James on the NYC subway last month. Mr. James has been indicted on federal terror charges after shooting ten people. Were there no official prayers for victims and to end racial violence from religious bodies because no one ultimately died in the subway shooting? Why were there no tweets, memes, or impassioned calls to “do better” after such a horrific, calculated attack? The silence after that racially motivated shooting compared to the outcry after this month’s racially motivated shooting is noteworthy. And essential to the CRT worldview. Racism is unique to white people. Another sign of our racialized culture war comes from this listener:In your episode with Douglas Murray, you mentioned that you had to explain to someone how white people did not invent racism. I serve at the school board in Manhattan and we had the same discussion at our last meeting. The district is pushing a book called “Our Skin” to teach elementary kids how white people invented racism. Money quote:“A long time ago, way before you were born, a group of white people made up an idea called race. They sorted people by skin color and said that white people were better, smarter, prettier, and that they deserve more than everybody else,” the book declares.Here’s how Murray addresses the canard that white people invented racism:On a lighter note, here’s a fan of last week’s episode with Tina Brown:In your conversation about the Queen’s inscrutable nature and unceasing impartiality, you forget one spectacular lapse into utter bias: the 1995 referendum on Quebec sovereignty!Pierre Brassard, a Quebec disc jockey, called Buckingham Palace impersonating the (then) Canadian PM Jean Chretien begging her to support the NO side and, astonishingly, got through to Queen Elizabeth! In the conversation, broadcast live in Montreal, she actually said, “It sounds as though the referendum may go the wrong (!) way...”. She said many other things that were blatantly against Quebec separating and was willing to make a public statement. Here’s the audio (and pardon Elizabeth R’s surprisingly bad French!): While I voted Non and thought the hoax was screamingly hilarious, this referendum was about the self-determination of a nation and she was hardly a glowing example of non-interference and impartiality. Quebec separatists were apoplectic. She wouldn’t even make a clear declaration in favour of the “No” side in the Scottish referendum! Ah, well ... even Captain Kirk broke the prime directive 33 times. Self-determination must be overrated. Here’s Tina on why the best British monarchs tend to be women:Another fan of the episode writes:So I’m a stereotypical NPR-listening, NYT-reading, Anglophilic liberal, happy to watch whatever B-grade pablum PBS airs on Sunday nights, as long as it has a British accent. So of course I fell in love with Downton Abbey. Part of my stereotypical outlook is holding a certain condescension toward the lower-class examples of American culture — you’d never catch me watching a soap opera, for example. But somewhere in the last season of Downton Abbey, it hit me full-on that the show is just a soap opera for snobs. That realization was a nice, bright, uncomfortable look in the mirror. What a hypocrite I am! That said, I can’t wait for the new Downton Abbey movie that opens this week:On the subject of Americans and their relationship with the British monarchy that you and Tina Brown discussed, to me it isn’t very complicated. It’s the embodiment of our cultural heritage, so it represents roots and stability in our land that values change and progress. And the monarchy is sacramental — another quality our society lacks, and which we’ve projected onto the office of the president as compensation. Toggling from listeners to readers, one of the latter writes:I have been thinking a lot about your May 6 column on the SCOTUS leak (“How Dare They!”) and the following week’s large number of reader responses to it. First, I want to say that, although I’m fiercely pro-choice, your column was strongly persuasive and helped me to think about Roe v Wade in a very different way. I love this about the Dish — the way you introduce complexity and nuance to issues that are polarizing and thus typically presented in stark black-and-white terms. But there is one potential detail of your argument that I continue to struggle with. While I accept that, in a liberal society, such issues as abortion should be a matter of debate and resolution via the popular voice, in practice they rarely are — because of the reality of our political system. Because of our two-party system and the primary elections that determine candidacy, most moderate, centrist voters simply do not have a choice to exercise their opinion on a wide variety of issues. They cannot vote individually on issues of substance, in an a la carte fashion. They are forced to accept a homogenous party platform that, in toto, represents the least worst of two extremes. For example, if I am a pro-choice moderate conservative who supports free markets, minimal government regulation, and low taxation, and is concerned about wokeness and CRT, my only choice to cast a vote in support of access to abortion is to vote for a candidate who is antagonistic to these other issues of import to me. You cite statistics in your column indicating broad support among Republicans for a moderate stance on abortion. Yet, I would argue that relatively few of these voters are going to voice that support by voting for a Democratic candidate — especially a far-left candidate — even if this means voting for the far-right opponent. This, then, is interpreted by the GOP as proof that their constituency supports the extreme view held by the majority of the GOP candidates. If we had a center party, I may be more optimistic in sharing your view of things. But as it stands, I feel like our choice is no choice at all.I feel you. But this is unavoidable in a democracy with political parties and winner-takes-all systems. Another reader has a few more laments:I believe anti-abortion-rights activists have not fully considered the consequences of how eliminating legal abortion will impact families. It is almost certain that the rate of child poverty in America will increase if a ban on abortion takes place.  Most of the states which want to ban abortion also have small child-welfare programs. That will result in more children being born into poor economic circumstances.Another thing that will probably happen is an increase in crime. The crime rate in the US has been falling since the early ‘90s, when kids born after Roe first started reaching adulthood. There is a clear link between kids being neglected and unwanted and then turning to crime. This was documented in the book Freakonomics.I believe the pro-choice side will win this debate. But perhaps it will only win when the full, horrifying consequences of banning all abortions — such as in the Oklahoma bill just passed — comes into focus. This next reader goes meta:In your otherwise excellent compilation of reader thoughts about Roe, you had one response I want to quibble with. After quoting one reader, you wrote: “Oh please. This next reader gets specific:” — and then went on with the next quote.I don’t recall what the first reader said, and it doesn’t matter because your response was inappropriate no matter what was said. If you think the reader’s argument has no merit, omit the comment. If you have a rebuttal to the reader’s argument, offer it. Even if you disagree with the reader but lack the time or energy to formulate a proper response, that’s fine too: Just print the comment with no response.What’s not OK, ever, is to reply with just a snarky dismissal and no further comment. That’s rude to the reader, and it makes you look like a dick.That whole big collection of reader dissents was compiled and edited by my colleague, Chris, who does that every week to hold my feet to the fire. I don’t censor the reader criticism he offers — so forgive me the occasional harrumph. Another reader switches topics:I read these two excerpts in your weekly money quotes:“There were also homosexual women at the Pines, but they were, or seemed to be, far fewer in number. Nor, except for a marked tendency to hang out in the company of large and usually ferocious dogs, were they instantly recognizable as the men were,” - Midge Decter, who died the week, on Fire Island in the summer of 1980.“Well, if I were a dyke and a pair of Podhoretzes came waddling toward me on the beach, copies of Leviticus and Freud in hand, I’d get in touch with the nearest Alsatian dealer pronto,” - Gore Vidal, responding to Midge.I had known about Decter’s “The Boys on the Beach” essay for decades, maybe since the late ‘80s, but I had never read it — until a few months ago. I am 66 years old, was practically always out, loved to read all the gay literature, and I have to say, that essay got the pulse of ‘70s gay life and society better than Edmund White (his “States of Desire” was published in 1980 and I still have my copy) or any other commentator I know of, with the exception of Randy Shilts’s “And the Band Played On.”Decter had gay acquaintances, friends, and frenemies, and she saw aspects of gay life with a beady-eyed sharpness and skepticism I wish more of us had had back then. I remember when I officially came out in 1974 at 18, met a couple of good-looking guys in their late 20s/early 30s who, like the vast majority of gay men, talked about sex all the time, with a greater intensity than straight guys I knew. So I asked them how many guys they had been to bed with and they said maybe 500 or 600. Asked them if they were afraid of getting diseases, and they said “no” because they just went to the public health clinic to get a shot. And right there, I sensed that at some point, there would be a gay healthcare catastrophe. I was not the only who had that sense, but it was very censored in the community.I tend to agree about Decter’s accuracy and perception, however laced it was with disgust. It’s a riveting piece — proof that sometimes being alien to a subculture makes you a better observer of it. She and Larry Kramer were essentially on the same page when it came to gay male culture in the 1970s. And yes, the omens were there. And now there’s monkeypox, which seems as if it might have found the same transmission route as HIV. This week I spoke to my close friend, chef, author, and writer Phoebe Lapine. She is one of the most creative and self-aware people I know, and I deeply admire her and her work. Paul J. Wang: Welcome to the monthly podcast, On the Beat for Circulation: Arrhythmia and Electrophysiology. I'm Dr. Paul Wang, editor in chief, with some of the key highlights from this month's issue.   In our first paper, Vivek Reddy and associates studied a novel, 7.5, French lattice tip catheter with the compressible 9 mm nitinol tip that is able to deliver either focal radio frequency ablation [RFA] or pulsed field ablation [PFA], 2 to 5 second lesions. In a 3 center, single-arm, first in human trial, the catheter was used with a custom mapping system to treat paroxysmal or persistent atrial fibrillation. Toggling between energy sources, point by point, pulmonary vein [PV] encirclement was performed using biphasic pulsed field ablation, posteriorly, and either temperature controlled irrigated RFA or pulse field ablation, anteriorly (RF/PF or PF/PF) respectively. Linear lesions were created with either PFA or RFA. The 76 patient cohort included 55 paroxysmal and 21 persistent atrial fibrillation [AF] patients undergoing either RF/PF [pulse field ablation] 40 patients or PF/PF ablation in 36 patients, pulmonary vein isolation therapy duration was 22.6 minutes per patient with a mean of 50.1 RF/PF ablation lesions per patient. Linear lesions included 14 mitral, 34 left atrial roof and 44 cavo-tricuspid isthmus lines with therapy duration times of 5.1, 1.8 and 2.4 min/patient respectively. All lesion sets were acutely successful using 4.7 minutes of fluoroscopy. There were no device-related complications, including no strokes. Post-procedure esophagogastroduodenoscopy revealed minor mucosal thermal injury in two of the 36 RF/PF and zero of the 24 PF/PF patients. Post-procedure brain MRI revealed DWI positive flair, negative and DWI positive flare positive asymptomatic lesions in 5 and 3 of the 51 patients respectively.   In our next paper, Moussa Saleh and associates examined whether chloroquine, hydroxychloroquine plus or minus azithromycin lead to a prolongation of the QT interval, possibly increasing the risk of torsades de pointes and sudden death in a hospitalized population of patients with COVID-19. 201 patients were treated for COVID-19 with chloroquine/hydroxychloroquine. 10 patients or 5% received chloroquine, and 191 or 95% received hydroxychloroquine and 119 or 59% also received azithromycin. The primary outcome of Torsades de pointes was not observed in the entire population. Baseline QTC interval did not differ between patients treated with chloroquine or hydroxychloroquine monotherapy versus those treated with combination group chloroquine/hydroxychloroquine and azithromycin (440 ms versus 439.9 ms). The maximum QT during treatment was significantly longer in the combination versus the monotherapy group, 470 ms versus 453 ms (P = 0.004). Seven patients (3.5%) required discontinuation of these medications due to QTC prolongation. No arrhythmic deaths were reported.   In our next paper, Mikko Tulppo and associates examine whether the association between leisure time physical activity and the risk of sudden death and non-sudden cardiac death in coronary artery disease patients. 1,946 patients with angiographically verified coronary artery disease were classified into four groups: inactive, irregularly active, active exercise regularly two to three times per week, and highly active, exercise four times or more weekly. During follow-up, median 6.3 years, 52 sudden cardiac death and 49 non-sudden cardiac deaths occurred. Inactive patients had increased risk for sudden cardiac death compared to active patients, hazard ratio 2.45. Leisure time was not associated with sudden cardiac death in patients with Canadian cardiovascular class one, 18 events in 1,107 patients. Among patients with Canadian cardiovascular society, class two or higher, 34 events in 839 patients. An increased risk for sudden cardiac death encountered in highly active patients, hazard ratio 7.46 (P < 0.001). In inactive patients hazard ratio 3.64 as compared to active patients. A linear association was observed between leisure time, physical activity and non-sudden cardiac death. Those with high leisure time physical activity had the lowest risk for non sudden cardiac death.   In our next paper, Jacob Koruth and associates examined the preclinical feasibility and safety of a 9mm lattice tip catheter with focal biphasic pulse field [PF] based thoracic vein isolation and linear ablation combined focal biphasic pulse field and radio-frequency [RF] focal ablation and vocal biphasic pulse field delivered directly on top of the esophagus. They treated two cohorts of six swine with pulse fields at low dose and high dose followed for four weeks and two weeks, respectively to isolate 25 thoracic veins and to create five right atrial low dose PF, six mitral high dose PF, and six roof lines with combined RF and high dose PF. Baseline and follow-up voltage mapping, venus potentials, ostial diameters and phrenic nerve viability were assessed. High dose PF in RF lesions were delivered in 4 and 1 swine from the inferior vena cava onto a forcefully deviate esophagus. 100% of thoracic veins, 25 out of 25, were successfully isolated with 12.4 applications per vein with a mean pulse field times of less than 90 seconds per vein. Durable isolation improved from 61.5% in the low dose pulse field to 100% with a high dose pulse field (P = 0.04). And all linear lesions were successfully completed without incurring venous stenosis or phrenic injury. High dose pulse field sections had higher trans mortality rates than low dose pulse field (98.3% versus 88.1%, P = 0.03). Despite greater thickness, 2.5 versus 1.3 mm, pulse field lesions demonstrated homogeneous fibrosis without epicardial fat, nerve or vessel involvement. In comparison, combined RF plus high dose PF sections revealed similar transmurality, but expectedly more necrosis, inflammation and epicardial fat, nerve and vessel involvement. Significant ablation related esophageal and necrosis inflammation and fibrosis were seen in all RF sections as compared to no PF sections.   In our next paper, Hagai Yavin and associates investigated the effects of a novel, lattice tip catheter designed for focal radiofrequency ablation [RFA] or pulse field ablation in 25 swine. In 14 animals, they examined in step one (n = 14) the feasibility to create atrial line of block and described as acute effects on the phrenic nerve and esophagus. In step two (n = 7), they examined the subacute effects of pulse field ablation on block durability, phrenic nerve, and esophagus 2 or more weeks. In 4 animals in step three, they compare the effects of pulse field ablation and RFA on the esophagus using a mechanical deviation model, approximating the esophagus through the right atrium in 4 and direct ablation honest lumen in 4. The effects of endocardial PFA and RFA on the phrenic nerve were also compared (n = 10). Histological analysis were performed. Pulse field ablation produced acute block in 100% of lines achieved with 2.1 applications per centimeter line. Histological analysis following a mean of 35 days showed 100% transmurality (thickness range 0.4 to 3.4 mm) with a lesion width of 19.4 mm. Pulse field ablation selectively affected cardiomyocytes, but spared blood vessels and nervous tissue. Pulse field ablation applied from the posterior atrium to the approximated esophagus produced transmural lesions without esophageal injury. Pulse field ablation applied within the esophageal lumen produced mild edema compared to radiofrequency ablation (13 applications) which produced epithelial ulcerations. Pulse field ablation resulted in no or transient stunning of the phrenic nerve, less than 5 minutes without histological changes while radiofrequency ablation produced paralysis.   In our next paper, Elad Anter and associates investigated the optimal methods to identify arrhythmogenic substrate of scar related VT. They examine how often sites of activation slowing during sinus rhythm co localize with ventricular tachycardia VT circuit. In a multicenter study in patients with infarct-related VT, the left ventricle was mapped during activation from three directions, sinus rhythm, or atrial pacing, right ventricular and left ventricular LV pacing at 600 ms. Ablation was applied selectively to the cumulative area of slow activation defined as a sum of all regions with activation time of 40 ms or greater per 10 mm. Hemodynamically tolerated ventricular tachycardias or VT were mapped with activation or entrainment. The primary outcome was a composite of appropriate ICD therapies and cardiovascular death. In 85 patients, the left ventricle was mapped during activation from 2.4 directions. The direction of LV activation influenced the location and magnitude of activation slowing. The spacial overlap of activation slowing between sinus rhythm and right ventricular RV pacing was 84.2%, between sinus rhythm and LV pacing was 61.4%, and between right ventricular and left ventricular pacing, 71.3% (P < 0.05) between all comparisons. Mapping during sinus rhythm identified only 66.2% of the entire area of activation slowing and 58% of critical isthmus sites. Activation from other directions, right ventricular or left ventricular stimulation unmasked an additional 33% of slowly conducting zones and 25% critical isthmus sites. The area of maximal activation slowing often corresponded to the site where the wavefront first interacted with the infarct. During a follow-up period of 3.6 years, the primary end point incurred in 14 out of 85 or 16.5% of patients. The authors concluded that the spatial distribution of activation slowing is dependent on the direction of LV activation with the area of maximal slowing corresponding to the site where the wavefront first interacts with the infarct.   In our next paper, Georg Gussak and associates identified a novel form of abnormal calcium wave activity in normal and failing dog atrial myocytes, which occurs during the action potential and is absent during diastole. The goal of this study was to determine if triggered calcium waves affect cellular electrophysiological properties. The authors use simultaneous recordings of intracellular calcium, and action potentials for the measurement of maximum diastolic potential and action potential duration during triggered calcium waves in isolated dog atrial myocytes. Computer simulations then explored electrophysiological behavior arising from triggered calcium waves at the tissue scale. At 3.3 to five Hertz, triggered calcium waves occurred during the action potential and outlasted several action potential cycles. Maximum diastolic potential was reduced and actual potential duration was significant prolonged during trigger calcium waves. All electrophysiological responses that triggered calcium waves were abolished by using SCA 0400 and ORM 10103, indicating that sodium calcium exchange current caused depolarization. The time constant recovery from inactivation of calcium current was 40 to 70 ms in atrial myocytes, depending on the holding potential. This current could be responsible for action potential (AP) activation during repolarization induced by triggered waves. Modeling studies demonstrate the characteristic properties of triggered calcium waves are potentially arrhythmogenic by promoting both conduction block and reentry arising from the repolarization induced by triggered calcium waves. The authors concluded that triggered calcium waves activate inward sodium calcium exchange and dramatically reduce atrial maximum diastolic potential and prolonged action potential duration establishing the substrate for reentry that could contribute to initiation and maintenance of atrial arrhythmias.   In our next paper Faisal Merchant and Omid Sayadi and associates evaluate the ability of a real-time, closed loop system to record and analyze repolarization alternans from multiple intracardiac leads and deliver dynamically R-wave triggered pacing stimuli during the absolute refractory period. They examined the ability of this system to control repolarization alternans and reduce arrhythmia susceptibility, in vivo. R-wave trigger pacing can induce repolarization alternans, the magnitude of which can be modulated by varying the amplitude, pulse width, or size of the pacing vector. Using a swine model (n = 9), the authors demonstrated that to induce 1 microvolt change in the alternans voltage on the body surface, coronary sinus or left ventricle, requires a delivery charge of 0.04, 0.05 and 0.06 microcoulombs, respectively, while to induce one unit change of the case score requires a delivery charge of 0.93, 0.32 and 0.33 microcoulombs, respectively. For all body surface and intracardiac leads a Delta alternans voltage and Delta K score between baseline and R-wave triggered pace peaks increases consistently with an increase in the pacing pulse amplitude, pulse width and vector spacing. Additionally, the author showed that the proposed method can be used to suppress spontaneously occurring alternans (n = 7), in the presence of myocardial ischemia. Suppression of repolarization alternans by pacing during the absolute refractory period results in a significant reduction in arrhythmia susceptibility, evidenced by lower S rank score during program ventricular stimulation compared to baseline prior to ischemia.   In a perspective outlining the cardiovascular effects of chloroquine and hydroxychloroquine, Ohad Oren and associates describe their belief that the use of chloroquine and hydroxychloroquine in COVID-19 should be limited to randomized, controlled trials. For critically ill patients, unable to enroll in a trial, selective in-hospital use could be considered with careful clinical monitoring in keeping with the FDA's emergency use authorization. The authors suggested that studies evaluating chloroquine and hydroxychloroquine should systematically collect baseline demographic data, results from electrocardiographic and echocardiographic monitoring prior to and during treatment and rates of adverse cardiovascular events in both the near and long-term.   In a research letter, Federico Migliore and Alessandro Zorzi and associates found a 28% decrease in the number of urgent pacemaker implantations during the six weeks after the first COVID-19 case, from 122 to 88 (P = 0.02), compared to the six weeks before the first COVID-19 case. The proportion of female patients requiring urgent pacemaker implantation after the COVID-19 outbreak decreased from six out of 122 (49%) to 30 out of 88 (34%) (P = 0.03).   In a special report Stephanie Kochav and associates described 4 cases of cardiac arrhythmias in COVID-19, including AV block, atrial fibrillation, polymorphic ventricular tachycardia, and pulseless electrical activity arrest.   In a year in review, Suraj Kapa and associates discuss a number of the many advances in our understanding of arrhythmia mechanisms, diagnosis, and new therapies in the past year. Data suggests that secretoneurin may be a marker for patients at risk of ventricular arrhythmias, while natriuretic peptide receptor C may have a role in atrial fibrosis. In atrial specific 2-pore potassium channel TASK-1 may be a therapeutic target for atrial fibrillation. Sensory neurons may play a key role in sleep apnea related to atrial fibrillation. Bariatric surgery is associated with improved atrial fibrillation outcome. Artificial intelligence applied to electrocardiography has yielded estimates of age, gender, and overall health. We've seen new tools for collection of patient centered outcomes following catheter ablation. There's been significant advances in the ability to identify ventricular tachycardia termination sites through high density mapping of deceleration zones. We've learned that right ventricular dysfunction may be a predictor of survival benefit after ICD implantation in non ischemic cardiomyopathy patients. We've seen further insights in the role of his bundle pacing on improving outcomes. As our understanding of cardiac laminopathies advance, we have new tools to predict arrhythmic event rates in gene carriers. And finally, we've seen numerous advances in the treatment of arrhythmias in patients with congenital heart disease.   That's it for this month. Do you want to understand how trunking systems work? In this episode, Phil continues his miniseries about trunking systems, explaining how to most efficiently set up your Uniden DMA scanner for trunking, including sites, groups, quick keys, and more.   What You Need To Know Episodes 129 and 130 are prerequisites for this episode. Trunking systems can have multiple sites that can run on their own or as one large networked site running as simulcast. Hypothetically, you could have sites for different regions of your state Ex north, south, east, west, and central each with their own control channel that would broadcast all the talk groups for all the radios affiliated on that site. Every time your scanner has to go to a new site, it first has to find the control channel, then check the system for active talk groups on your list. You should only program your scanner with sites that you expect to have the traffic you want to hear. you don’t want to program every site in your state because many of them will be out of range. Each site you want to scan should be set up as its own quick key in the scanner so you can toggle them on and off as needed. Toggling some of the sites off as needed allows your scanner to only be scanning 1-2 sites at a time rather than 5 or more, making the trunking more efficient. For trunking, Phil recommends large systems that you can break down into groups. Groups make scanning more efficient because the scanner doesn’t have to go in and out of sites repeatedly to check different scan lists. You want to make sure that each site you want is set up to its own scan bank, and within the system, you want to make sure that each group is set up as its own group key. You can assign more than one site per quick key. You can define up to 20 groups per system, and up to 10 quick keys per group; only assign quick keys to groups you want to toggle off and on. SONIC ACTS ACADEMY 2020 Nadim Samman – As We Used to Float / Iroojrilik 23 February 2020 – De Brakke Grond, Amsterdam, The Netherlands Curator Nadim Samman gives a performative lecture that takes to the psychology and aesthetic of the sea, drawing on his explorer’s path through art as co-founder of the Antarctic Pavilion at the Venice Biennale in 2015 and 1st Antarctic Biennale in 2017. His work has carried him from Moscow to Marrakech to Lima, uncovering the secrets of remote art and the mutual exchange that evolves from that act. Straddling the genres of travelogue and critical essay, As We Used to Float: Within Bikini Atoll (2018, co-written with artist Julian Charrière), explores Bikini Atoll as a space of fantasy and trauma. Toggling between a personal account of a sea journey, above and below water, and a critical investigation of postcolonial geography, the book develops broader reflections on place and subjectivity. These spring from a series of narrative immersions, variously, taking on the psychological and aesthetic parameters of ultra-deep scuba diving, the abject poetics of sea craft and the stakes of subaquatic image-making. As We Used to Float is a sea-story for our time. This performative lecture combines reading from the book with a screening of Julian Charrière’s video work, Iroojrilik (2018). Nadim Samman is a curator and art historian based in Berlin. He read Philosophy at University College London before receiving his PhD from the Courtauld Institute of Art. He co-founded the 1st Antarctic Biennale in 2017 and the Antarctic Pavilion at the Venice Biennale in 2015. In 2016 he curated the 5th Moscow International Biennale for Young Art, and in 2012, the 4th Marrakech Biennale (with Carson Chan). Other major projects include Treasure of Lima: A Buried Exhibition (a unique site-specific exhibition on the remote Pacific island of Isla del Coco) and Rare Earth (at Thyssen-Bornemisza Art Contemporary, Vienna). In 2019 he won the International Awards for Art Criticism’s first prize.

When contemplating identity, it’s important to know your history. But learning about that history is possible in many forms. Today, on the first episode of its kind, we will examine history through the lens of a book. Specifically, we will discuss the history of Jamaica, slavery, and generational trauma through author Maisy Card’s debut novel – These Ghosts Are Family. Mark Twain once said through a character that truth is stranger than fiction. And it’s true. Some of the most disturbing fictional tales are inspired by real life events. Further refining this point, I say that history is stranger than fiction. Made up tales of black origin stories steeped in darkness are virtually always inspired by a history that is much worse. Take for instance the classic American novel, Beloved. Toni Morrison created this disturbing work of art after being inspired by unbelievable history. She found a newspaper clipping from the 1800s. It was short, but described an enslaved woman who had run away from a plantation. Moments before she was recaptured, the woman made a horrific decision. She chose to kill her children rather than see them live in bondage. Morrison was struck by the weight of such a decision and the haunting emotions that must have lingered long after the children left this world. And from that contemplation, Morrison created a novel that continues to touch the lives of readers generations later. Maisy Card’s novel has the potential to do the same. From dark pasts she has created a wonderful multi-generational saga grappling with identity, ancestry, and untold black history. Card was born in St. Ann Jamaica, but was raised in Jamaica Queens, New York. Her ties to these lands exemplify the first connection between her reality and the world she creates. Pam Rende has led both the men’s and women’s tennis programs at Division III Arcadia for the past 5 seasons. However, Pam has been coaching for over 20 years in the Philadelphia area and has worked with players of all levels from beginner to grand slam participants. She serves on the board of the USTA Philadelphia Area Tennis District and is working towards her PTR Master of Tennis certification. In this podcast we discuss how Pam manages two college teams while running L3 Tennis and leading a staff of seven full time pros and up to 20 seasonal pros. Romans 14:14-16 — Teaching on the conscience is relatively sparse in Scripture. The topic occurs in only a few passages in Paul’s letters, so many Christians are either uninformed about the conscience or perhaps confused. So how vital is it to the Christian life? What is Scripture’s teaching on it? In a sermon dedicated entirely to key questions regarding the conscience, Dr. Martyn Lloyd-Jones commends the apostle Paul’s teaching on the conscience as one of the most important subjects for the family of God. In his sermon on Romans 14:14-16, Dr. Lloyd-Jones brings related passages on the conscience together in order to evaluate key terms and answer basic question on the conscience. Toggling between Romans 14 and 1 Corinthians 10, Dr. Lloyd-Jones analyzes the context of strong and weak Christians on matters of opinion. With regards to these indifferent matters, the conscience is ultimate. We are not to exercise our liberty in such a way that it comes under the condemnation of another believer’s conscience. Disregarding a brother’s conscience or our own can have devastating results. As such, Dr. Lloyd-Jones looks in-depth at Scriptural teaching on a defiled and seared conscience. If your focus is body recomposition, either in the form of fat loss or muscle gain, then the two variables at play will always be training and nutrition. However, I posit that the training variable should almost always be fixed at maximizing muscle growth, while it is the nutrition variable that will be manipulated to meet your goals. Even if your ultimate goal is fat loss, you'll want to train to build (or maintain) muscle to prevent excess metabolic slowdown, encourage your body to keep its precious muscle, and achieve the look you are most likely going for. At long last the trek to Fenrahîme comes to a close, however time has not stood still for Alek Grummhammer and his people who he know must lead. Upon the arrival of our heroes, plans are made to conquer the mountain before them, and obtain their prize. But the forces that lurk in the shadows are restless, and will give no quarter along the way. Download(right click and save as) Art: The Trials of Devotion by noahbradley Opening: “The Sundering” The Sword - Gods of the Earth Closing: “Garden of Light” Isis - In the Absence of Truth ‘That night, the fires burnt bright. Fires, not of defeat parse, but of redemption and pride, funeral pyres. Stacked high, so that as the bodies return to dust, becoming material to one day be forged and return to battle,  the spirits might rise victorious from the  struggle of life. The final  battle all mortals must face, death. Tears of sorrow cannot quench these fire’s no matter how plentiful. The bugbears must pay!’ Toggling back and forth gave the session a real cinematic feel, a favored method of storytelling for both Dan and I. Life revolves around no single person, so why should the story of the lives of our heroes? It’s a great tribute to those who have defined the genre before the concept of this adventure, and to which homage is paid.
