A retired professional, she is the mother of four and the grandmother of eleven, cherishing her family while passionately advocating for peace. She has volunteered in many Palestinian refugee camps in the West Bank and Lebanon, providing support and compassion to those in need. Janine was also part of the 24/7 peace vigil in front of the home of then Secretary of State Antony Blinken from January 26- July 26 2024. Since October 7, she has spent her time demonstrating against the genocide in Gaza, working tirelessly to bring awareness to the American public about the atrocities being committed by Israel and striving to educate others and foster understanding. Janine channels her energy and experience into making a positive impact in her community through peaceful means. Jenin Younes is a civil liberties and free speech attorney and is National Legal Director of the American-Arab Anti-Discrimination Committee. She is on the defense team representing Janine Ali. Tommy Marcus is the creator of the viral Instagram account Quentin Quarantino. He set out to create a meme page during the pandemic, but has since used his Instagram fame to raise more than $30 million for humanitarian causes. He was aboard the Global Sumud Flotilla carrying aid to Palestinians when they were arrested by Israeli naval forces. Tommy and his fellow Jewish-American friend David Adler were singled out and forced to be filmed with Israeli National Security Minister Itamar Ben-Gvir when the convoy was intercepted. Tommy spent 5 days in an Israeli prison and returned home to Los Angeles on Friday. Dr. Dhiaa Daoud is a Palestinian-American emergency physician and humanitarian. His work in Gaza during the 2024 war inspired him to found the Doctors for Hope Foundation, which provides medical care and rebuilds essential infrastructure. There is a weird trend now arguing that you should go low fat on a carnivore diet, because wild animals can be lean and therefore our ancestors would have only eaten lean meat on a carnivore diet. this is demonstrably false, and unsafe, for many reasons, as explained in this video. (My video "Is Ketosis Harmful?" https://youtu.be/nJyUQmRc1q8?si=r2IngkzpwxEqkCFv ) What we seem to see are people who may not be doing a full carnivore diet, just fatty meat and water, and will include quite a lot of dairy such as milk or cheese, yogurt etc I may even include artificial sweeteners, which absolutely will Elevate insulin the fat storage hormone. People do this and they tend to gain weight or can't lose the weight they want to lose, and they get convinced that this is because they are eating too much fat, even though eating high-fat carnivore actually improved their health, nutrition, society, and help with weight loss earlier on before they added in the dairy and the artificial sweeteners. Once they lower the fat, they start to get the majority of their calories from protein and can start getting very unwell from low-grade protein poisoning. this may lose some weight, as an early sign of protein poisoning is rapid weight loss, but this is unhealthy weight loss. Then they might think that this is just a fact of being in ketosis so they adding carbohydrates, and because they are now getting calories from something besides protein, the protein poisoning subsides, thus confirming their own bias. So by not doing carnivore and using artificial sweeteners and/or dairy people start to gain weight or not be able to lose as much weight as they want to, so they switch to low fat hoping that will help. Then they start feeling unwell, like we do to low-grade protein poisoning and likely malnutrition from a lack of fat soluble vitamins, and so they think that carnivore didn't work for them and they start adding back in other things like carbohydrates. Then they tell others that carnivore didn't work for them and maybe it doesn't work for others as well, but the problem is that they weren't doing carnivore in the first place. Carnivore is a complete absence of anything except meat and eggs, and for some people not even eggs. If you add in a lot of dairy and / or sweeteners you will not get the same results as others, and you will likely gain weight. People are doing video podcasts now. We got to keep up with it. We got to do videos. And you and I were like, what? Like, I am not dressed for this. Like, and we were so awkward and uncomfortable and it was so weird. Do you remember? Britt (02:14) I do and you know that's like voice was weird enough for me at least you know karma works in my favor that I didn't have to go full throttle from the very beginning of doing things I got an intro of just doing voice and then eventually adding video The Dental A Team (02:23) there. Very fair, very fair. And I think as a leader on our team, that kind of spurs right into what our conversation today is about. And Britt, you are an incredible fearless leader on our team. And I think doing this podcast with you is divine and just meant to be. are someone that I look up to, that I know the whole team looks up to. And just that in itself, like the things that you have worked through, overcome. I remember when you first came on, I'm like, talk to yourself in the mirror. You're like, okay, crazy. Like, yeah, I am. I am crazy, but it works. And you did it. Like you just jumped in and you were like, I'm going to figure this out. And that is just a piece of leadership that I think you're naturally born with, the figure out-ness. Like it's fine. Anything can be figured out. Whether you do it for the long run or not, you can figure it out and train someone else to do it. So having you on on the leadership piece here is actually just super like natural, right? Like it just is natural. So thank you for being here. Britt (03:26) You bet, you're generous, Tiff. I problem solving for sure. Kiera will say there's always a solution and that is yes, being a leader, there's always a path through. I don't know exactly what it might be right now, but there's always a pathway through. And ⁓ yeah, it's, ⁓ I think also being a leader, it's a constant ⁓ humble of being able to learn is helps you continue to learn skills to be a better leader over time. The Dental A Team (03:51) I totally agree with you. And we learn from each other, I think, and we continue to grow. We continue to refine those aspects of ourselves and those aspects of who we are and who we want to become. And it just makes it really, really cool. And today's special podcast, you guys should know. Hopefully, you know, this is dropping on a very special day. It's it's considered Boss's Day. I wish it was like Leaders Day. I don't I don't love the word boss in 2025. Yeah. Britt (04:17) Can we have a for you to end that a little bit? The Dental A Team (04:19) We've rebranded so many holidays and so many names of things. I think this one's definitely on the list of need. But I think of Boss' Day as like Leader's Day. I think that it's just a day to celebrate leadership and it's a day to celebrate today of all days the reason that we have, we'll say jobs, right? The reason that we're here today, the reason that we have a company to work for. And I just think it's really cool to take a moment and a special time to really thank those those people, the entrepreneurs that we work for. And the other day someone said to me like, I'm not an entrepreneur. I'm like, they're an entrepreneur. They own a business. Like they're dentists. know they are entrepreneurs. Just like Kiera is an entrepreneur and she is the leader of this ship and she is to be celebrated today just the same as the rest of you guys are. And Britt and I thought we would take a moment to celebrate Kiera. We will get there. Kiera, don't you worry. Keep listening. We are going to get there. ⁓ celebrate Kiera, but also to just celebrate you guys too and to really highlight some fantastic leadership skills that we've seen come about in our journeys, but also things that we see practices doing that set them apart from other practices in the leadership spectrum. And Britt, I think that it would be wild to start anywhere else. You tell me I might be totally wrong, but I really believe that leadership is a culture that you create. and learning how to hone in leadership, think people kind of start all over the place. They're like, well, start with accountability. I'm like, well, first you gotta start where you want, how do you want to show up? Like, how do you want people to view you? How do you wanna be seen as a company and as individuals? And I think that culture piece, can be hard to understand. I might be like making things up, but I think it's something that really great leaders get. is that they are the culture of the company. And from your perspective, Britt, you've helped create the culture of this company. You're a massive leader within our own company, but you've also worked with many, leaders, even before Dental A Team, to help build businesses, to help build successful entrepreneurial businesses and sectors. What have you seen as just important leadership qualities that really hone in that culture that doctors and owners can take away from today. Britt (06:44) Yeah, I love it, Tiffanie. It truly does start at the top. And I think that's probably one of the most important things for anyone who's an entrepreneur, anyone on the team who's got some sort of influence upon other people, which is pretty much everyone, is to number one, like recognize that, yes, you are a leader. And number two, like you said, recognize that it really does start with you and the impact that you have and what it shapes your culture. I think some of the best leaders that I've had the privilege of being around or working with, I think number one, there's like a confidence humility balance that happens with them because they are confident enough, more confident than I, to like go and take this big chance and go have this business, make that investment, it's gonna be successful. So they've got that strong confidence piece in them. while also realizing that yes, there are human sides sometimes we've got to shape ourselves a little bit to become that leader and create that culture within our team. And so being able to recognize that and acknowledge it and take that responsibility on as a leader, I even have to catch myself doing it sometimes. Right, sometimes, well, it's just, that's the way I am. No, that's not a good answer, right, for something. That's not a, that's an excuse 100%. It's not just the way I am or I'm learning about that. thing or I'm working on becoming better at that thing instead of saying, well, that's just what it is and move on and deal with it because that doesn't get us anywhere. So I think kind of that humility, confidence mix. you know, I think anywhere you go and you've heard it before, right, people remember how you make them feel. And I think really good leaders have a genuine just want for their people to do well as humans and in their job. And it's amazing the power that comes behind that. I'm thinking of, just had the opportunity to see one of my old bosses recently. And just like the excitement he has for what I'm doing now. And I quit, I left his office to come and do this. And he's still so excited about it. I think that goes a long ways for team members and building leaders because it builds confidence in your team around you. and helps you to one, not only build up your team, but also build up more leaders, which everyone needs. The Dental A Team (09:10) Yeah, I love that. love that I actually get to see my boss that I left next Monday. So I'll probably have a similar experience. But I love the way you said that, Britt, that confidence, but with that humble and I kind of as you're as you're talking, my brain's kind of putting it in steps. And it's almost like you've got to develop yourself to the point that you have the confidence to say yes to take that step to run for it. But then the next layer is peeling back enough ego. can't rid of the whole ego because you need some sort of ego. Ego is good, but it can be too much. It's peeling back the ego and realizing that you're not losing confidence or losing status by being humble or by being vulnerable in your leadership. And so I actually find vulnerability to be a space of higher confidence. I feel like the more confident you are, the more vulnerable. Britt (09:54) Mm-hmm. The Dental A Team (10:06) you are able to be with like that true confidence definition. And so it's kind of that like you needed the momentum. It's it's kind of like Brody. He's 17 and he has all of these things that he wants to do and he has all these decisions he's trying to make. But he doesn't have his brain hasn't developed yet enough to see the logistical steps that happen after that decision is made. Like it's like this decision and then the next one. Right. So it's kind of those steps of entrepreneurship is like, had to have enough confidence to make this first decision, but now I'm no longer 17 years old, my brain needs to develop, right? So it's almost like you get here and now you're developing that skill of humble, vulnerable leader and peeling back those layers to say, is now where we're at. And that's where that culture piece comes in. And I think it's where the culture shifts come in for so many practices because we had to hire people, we had to have what we have, and then we get to a point, I would say, I would venture to say three to five years in, you get to the point where it's like, okay, this feels chaotic. How do we back up and almost restart because we're backing up, peeling off those layers, and then adding something back in. And I think, you highlighted that really well. Britt (11:08) Thank you The Dental A Team (11:31) and those characteristics ⁓ being developed over time are like, it's just the only way to really do it. And to think that right off the bat of practice ownership straight out of school, you're gonna have all of those skills is kind of crazy. So I love the way that you had said that. And I think we've all had the opportunity on some level or another to watch each other grow in those spaces, ⁓ but to highlight Kiera in this moment for Boss's Day. she's an incredible boss, is really watching her develop those is something that we all get to take away. So she has that humble vulnerability. Doesn't always come easily and you guys, like you'll stumble. You're gonna have to have some space where you're like, that wasn't my best foot, which it almost was because it created the situation we're in to have the conversation we're having. So I just don't ever look at anything as a total loss. I look at everything as like a step forward. but she's able to do those pieces for us and with us because we're all growing at the same rate she is. We're like a percentage or two behind her, right? But she's leading the pack and creating that culture. if Kiera's turbulent, we're turbulent. If Kiera's, she doesn't even have to be in the company, you guys. Like she does not even have to be here with us, we know. We're virtual. Britt (12:41) Mm-hmm. No. virtual or virtually. The Dental A Team (13:00) Good, yeah, good point because I have a lot of doctors that are like, how do I get this culture to work without me here? This is that space where Kiera has led us in this fight of culture to where we've all been able to pick up those pieces and embody them. We embody who the Dental A Team is and it's not necessarily Kiera. We're not mini Kiras by any mean. We are embodying the Dental A Team culture. so that the company can be successful and we can carry the culture without care of being here. And I think that comes from that next layer, right? We push forward, we have the confidence, peel back the layer, we have the vulnerability, and then we train and teach that by showing and doing, by being. Like, you live it so that people can live. Britt (13:53) Tiff, and as you were saying that, right, that confidence piece, and we've all experienced it. I don't care who you are, right? You've hit that point where like it's just a tough spot. Whether it's a tough thing you learn about yourself, whether it's a tough spot business-wise, right? We hit those tough points and that the confidence that leaders have is the confidence to say, all right, and... We keep going and this makes us better, right? Like that's the confidence to where it's not, they're not backing down, right? Have their moment great. As my brother tells me, you got five minutes, go cry about it and then let's move on from it. You know, something like that. It's like, fill your emotions, let it out, but then it's time to move on and start progressing from here instead of letting that derail us completely. The Dental A Team (14:45) Absolutely, absolutely. And when we're given that availability, it changes and shapes who we are. Because I think then the last thing people want to do is take stuff home to their families. And that's where people come in, they're like, tiff, right, how do I get this work-life balance? I'm like, I hate that word. You guys know that, you've heard me preach about it. We're not going to go down that rabbit hole. This is where it happens though, where you can show up and be respectful, but be yourself and be able to work. through things together, that you don't take things home and then you come to work worse. Like it's this bad spiral. And when the culture does start to shift in that direction, it's having the confidence plus vulnerability to be able to say, we need to have a conversation. Like something's off, we're off. Kiera and do that a lot, right? We'll say something feels wrong. the words aren't matching the tone or like the way I'm hearing them, it doesn't match what I think your intention is, so like, let's squash through that real quick because what happens is if we stay on that space, it's just like a battle of egos at that point and you think I'm showing up confidently, like I'm showing up strong. Sometimes, but mostly we need to be like, wait a second, what is that that I heard? Walk me through that first. That's the vulnerability space, because you might say, well, you said something that really hurt my feelings. Got it, tell me, let's work through it. Britt (16:10) I The Dental A Team (16:12) And I know we have been trained in our lifetime here that work is not a space for emotions, work is not a space for feelings. And that's just not, it's fine if that's what you want your culture to be. And I'm here to stand on the fact that create your culture to be what you want it to be. Our company's culture is to tackle things as they happen. And it's something that we've been able to develop over time. ⁓ with respect to one another and respectful communication. So thank you, Britt. That was beautiful. I think the doctors that we attract and the teams that we attract kind of, I think we attract, like attracts like, and we attract practices that are looking for culture similar to ours or have culture similar to ours and need to step it up a little bit more or just get to that next level. And think now, Britt, is there anything else you wanna add to that before? we start our brag Britt (17:10) I only other thing I would add to that, right, as you listeners are out there, whatever stage that you are in, right, we worked with a lot of leaders as well and people we learned from all the time from our clients and also that we get to help them along that journey of leadership. Because I don't know about you, Tiff, but thinking back on clients, right, I've got clients where the leader they were when they opened the door with four employees is different than the leader they are now. where they're going into multiple locations and have now leaders built up underneath them who do a lot of things for them. I think giving credit to the amount of personal growth and leadership growth that takes to transition within your business to a different stage and a different size even. And so it's a lot of fun for us to be able to be a part of that and help out with that. And also I know a lot of our doctors who come to us and there's a lot of things that as a leader, you cannot complain down. That's just one of the things you can't complain down. And so you've got to have someone to work through things with. And sometimes it's not the prettiest when you're working through those things. And so being able to be that outside person so that our doctors can work through those things with us and we can work through it so that they can go present to their team. as the leader that they want to show up as. That's ultimately who they are, but right, we're all growing and changing. And so it's helping them to be able to have that support, to be able to go and then present to their team as the leader that they want to be. So I think that's the only thing is everyone is always learning. Perfection is never something that you are gonna reach. It's a constant growth that we're looking for. And as we work with our leaders, is the leader you are today. will not be the leader you are in five years. Thank goodness. We will all be better than we are currently. The Dental A Team (19:05) Yeah. Yeah, I was thinking the same thing. Like, thank goodness I'm not the mom I am today, the same as the mom I was when he was four. Yeah, that was beautiful. Thank you. I totally agree. And acknowledging that, like you said, being okay with it, ⁓ understanding you're not going to be the same as you were and your team shouldn't be either. So I know a lot of leaders will feel held back where they're like, gosh, but my team knows me as this person. And it's like saying, well, my husband knows me as my 20 year old version. Britt (19:11) huh. Okay. The Dental A Team (19:37) self. So me taking these new courses or expanding my thoughts or doing yoga instead of workouts at the gym would be different. How will he view me? It's kind of the same. Like you guys grow together or you grow apart. And if you've got team members that aren't ready to take that next stage with you, that's okay too. Just know if you hold yourself back, you're holding your team back too. So perfect, Brett. I love it. ⁓ so I have, we have, we have something fun for Kiera. I know she listens to these. I know specifically she's going to listen to this one cause it's freaking boss's day. And she knows that something fun is coming out. And what we did today, Britt and I pulled the, ⁓ consultant team specifically, and we asked for one to three qualities that they see in Kiera that she brings to our team as our leader. I thought these would be really fun for Kiera to hear, for Britt and I to talk about, and for you guys to take away as leaders, because I do think that they are, there are so many, are 20,000 words we could choose from, but it is really cool to see how a company has grown and what they're showing up and embodying now. And I wrote them all down for us, Britt, and then I've got a couple that I just want quickly you to kind of expand on for us. There were a few that were repeated, which I thought was really, really cool. One of the ones that was repeated was integrity, doing what they say they're going to do. And that's something that within our company we have had as a piece of our core values for as long as I can remember. And I think for Kiera in general, she works really hard to do what she says she's going to. And if she, for some reason, misses that step, she definitely takes ownership of it. Another one that came up, well, it was kind of worded. in different ways. So it's kind of, it's actually kind of cool because execution and vision came up, which I feel like Kiera is our visionary and we learned, I called her out on it and I love you Kiera. I hope you're listening. You know, I will call you out every single day. When she loses her vision, our whole company spirals. There was a point in our lives where I was like, we just need to know where you want us to go. We just need it. So having that on this list actually made me really, really, really happy. Britt (21:51) Yeah. The Dental A Team (22:02) And I think it'll make Kiera really proud to hear that that was one of the words that came up. But I also think that vision kind of flows in with execution because she executes on the vision, which helps, she executes on most things she puts her foot forward on, but it helps us to execute on the vision as well. But then ⁓ decisive came up and those kind of all clumped together for me in my mind. Decisive, I think Kiera is gonna get a kick out of that one as well. that is something that she has honed a skill in on. ⁓ Accountability, empathy, confidence, passion to make a difference, inspiring, transformational and resilient. well, first off, Britt, what's your favorite leadership quality that was given to us from the team for Kiera, but just for a leader, not necessarily for Kiera. Because we don't need to make it all about her. She's already like blushing over there, but I wanted her to hear these words But what out of out of those what do you feel like you pull out as a baby leader as a grown leader? Like what word stands out to you the most? Britt (23:12) Yeah, I do love that decisive was on there as I know Kiera will love that one on there too. I think that's one of my favorites Tiff to your point. Right, we can't have too many cooks in the kitchen on what direction to go because it really does get wild even within your dental practice too many cooks in the kitchen gets very wild. And so that leader who can be decisive and I think it goes along with vision as well of like The Dental A Team (23:17) Same. Yeah. Britt (23:42) keeping the course of what direction are we going, even though that direction is moving or growing or where are we headed to, and that'll adapt over time. A good team, right? Good team members. All we wanna do is like help you and make it happen. It's kinda like, well, we just, what direction? Because I know if I start going my own direction, it's gonna get wild and I'm gonna be off track. So like what direction? And then, give me a few words and let's go and let's make it happen. And I think that's huge. And I know that that's a tough spot to be in sometimes. Our business owners, our entrepreneurs, you guys have a lot on your shoulders and it doesn't always go recognized. And I know like it's pressure on you sometimes, but keeping, for you to keep a space where you can have that clear direction and be decisive about where we're going is a game changer. The Dental A Team (24:39) I agree. I agree. I think execution goes along with that. And the vision plays such a huge role too, because as you were speaking, I was thinking for visionaries, for entrepreneurs, for people who start businesses, it's very difficult for them to see outside of being a visionary. Just like when you're not a visionary, when you're an implementer, which most team members are not going to be visionaries. You guys are probably on their own companies, just saying. Britt (24:43) Thank Yeah. The Dental A Team (25:06) They are implementers on some level. You are the visionary, but it's very difficult to see the other side. It's like it's difficult for you to put yourself in those shoes. So the visionaries are like, no, like you want space to create because that's what they want. But they do have a very clear picture of what they want, but they don't know how to narrow it down. Right. So it's kind of this like mix and teams sometimes go a little wild. They're like, just tell me and they get into the spin. And so I think that decisiveness, that execution narrows down to the vision. When you know the vision, when you have a clear picture of the vision, that is your go-to spot, you guys. Decisions are so much easier when you know where you're going. When the vision of your practice is super clear, those decisions, big or small, come down to, they match our vision? Does this go with our team values? Does this match our culture? or is this completely opposite of what we're heading towards? Do we wanna add a new product to our product line? Do we wanna add a new procedure? Well, what's our vision? What's our goal? Is this in alignment with what we're working towards? Because then visionaries, you can say yes, no, maybe here's the outline, here's a template, go create. I can be very creative when I know what the direction is. So to your point, Britt, I think those all go hand in hand. I totally agree. And I think it's something that as a company, We've pushed for and narrowed down really, really well. Our style of meetings help, and I know we work with practices all the time to get those narrowed down. We have practices coming into town soon to do just this thing. so those were so perfect. And it's something that we constantly refine. Doctors, think actionable pieces here, leaders, office managers. When I was an office manager, you guys, I told you, I'm gonna see my doc in a week. I love him. He, Debrit, I think we're like the same, like they are just family to us. Britt (27:04) Always a cheerleader. The Dental A Team (27:05) Always a two-year-later, constantly. But I had to be the one that created this stuff, right? Like for slash with him. And so office managers who are here today that are like, yeah, this sounds great guys, but like my doc's not gonna listen to this podcast or my doc's not on the same page as me. That's totally fine. Go create it. Create something so that your team can be decisive in their roles too. when you can narrow down that vision, no matter who you are right now, whatever leadership position you hold in your company, narrow that down and make it easy for everyone, because it's going to make it easier for you too. So, Britt, this went longer than I thought it would. Thank you so much for being here. I love celebrating everyone. And Britt, you're just one of my favorite humans. And I knew you would have so much to pull out of this on leadership side. As such, our thoughts influence our emotions, decisions, and actions, shaping our overall mental health. From a scientific perspective, our thoughts matter because they influence our brain's structure, emotional health, and overall behavior. This scientific understanding underscores the importance of intentionally aligning our thoughts with truth. Scripture gives us the blueprint for this renewal, while science shows the profound impact it has on our mental and physical health. Research has demonstrated a connection between dwelling on negative or untrue thoughts and experiencing anxiety, depression, or hopelessness. In order to break free from destructive thought patterns, it’s important to replace lies with truth To recognize thoughts inconsistent with God’s Word, ask these questions: Does this thought align with Scripture? Is this thought producing fear, doubt, or condemnation? Does this thought reflect God’s character and promises? Replace these thoughts with Scripture. Gratitude shifts our focus from problems to God’s goodness and faithfulness. Accountability, encouragement, and prayer from fellow believers is important to ensure that we are surrounded by voices who will speak truth into our circumstances. Renewing the mind isn’t about striving in our own strength but relying on the Holy Spirit to guide, convict, and transform us. Renewing your mind is a powerful and transformative process that aligns your thoughts with God’s truth, bringing hope, peace, and clarity to your life. By identifying and replacing lies with Scripture, meditating on God’s Word, speaking His promises, cultivating gratitude, and seeking support from a godly community, you can break free from negative thought patterns and experience lasting mental health. Most importantly, remember that you’re not doing this alone. The Holy Spirit is your helper, leading you every step of the way. Let’s commit to this journey of renewal together and trust God to do a transformative work in our minds and hearts. “Giving learners options gives them a better learning experience. It's more holistic and more comprehensive,” says Sean Moloney, CEO and founder of EmbodyXR, an extended reality platform focused on the use of immersive technologies in medical education. Health Minister Simeon Brown has scolded senior doctors in a take-no-prisoners address to their annual conference in Wellington today. In his speech, he accused them of crossing an "ethical line" for choosing strike action, and said they were putting politics before patients. But his words were met with groans, laughter and cries of disbelief from the couple of hundred specialists packing the room - some of whom will be walking off the job again next week over deadlocked contract negotiations. I will protect her identity, by calling her Beth, my best friend's name. At first, I didn't recognize Beth sitting in my waiting room. It appeared that she had aged more years than the two she had been in Florida. She sat down and regaled me with this story of her experience. Some background. Beth had had a hysterectomy and had her ovaries removed before she saw me for the first time in 2007. She had been on some kind of hormones since then and pellets with me since 2007. When she was my patient the testosterone and estradiol pellets literally cured all of her symptoms. It wasn't easy to come to the right dose of pellets and to treat her other problems: She converts T into Estrone and Estradiol, and she is a fast metabolizer, so she needs a higher dose of testosterone that the average woman. I ushered Beth into my office and asked how she'd been, and she began a long and sad story about trying to find a doctor who would treat her with pellets so that all her symptoms were resolved. She went to 5 different doctors, who had her BioBalance chart, and all of them said they had to treat her differently because they were not in line with my treatment. She argued and got nowhere. #1 Doctor: The first hormone doctor did not agree with giving her testosterone plus anastrazole (Arimidex), a TA Pellet, yet that was what I treated her with to lower her Estrone and Estradiol from testosterone conversion. She got straight testosterone and her estrogens soared and inactivated her testosterone, so she felt no different on pellets than she did off of them. She also gained belly fat and breast size and was very emotional. #2 Doctor: This doctor was a Bio T doctor who refused to give her TA pellets and also wouldn't give her the dose she needed. Bio T uses MICRONIZED testosterone which are cheap, and cause hair to fall out, acne and oily skin. That is exactly what he got, and she didn't go back. #3 Doctor: Not only did this doctor refuse her anastrazole in any form but he made her take progesterone even though she didn't have a uterus and told him that progesterone made her fatigued. #4 Doctor was clueless and used a formula one size fits all Testosterone dose, which wore off in 2 months. #5 Doctor was essentially not educated in testosterone pellets. Because of these experiences, she now flies in three times a year and gets the pellets dose, Estradiol dose, with anastrazole in the pellets, without useless progesterone (because she had a hysterectomy), with NON-MICRONIZED pellets that last 4 months that are dosed for her unique metabolism. The moral to the story is all pellet treatment is not the same! So many doctors have jumped on the bandwagon with a few days of training or no training at all so if pellets don't work for you, PLEASE DON'T GIVE UP ON PELLETS. Beth didn't. You need a doctor who understands the interactions of T and E2 and E1, and who use pellets who are non-micronized and who individually dose your pellets for YOU! This Healthcast is not about patting ourselves on the back, but it is aimed at informing patients who have been disappointed with their treatment using testosterone and estradiol pellets because of inability to relieve their symptoms of menopause or testosterone deficiency, that they should find a practice who uses NON_MICRONIZED PELLETS and who have experience taking care of all interactions of hormones with other medications, who understand the trouble shooting for every unusual symptom that may occur after pellet replacement of hormones. It is not easy to learn to take care of hormones like Dr. Sullivan our staff, and I do, so finding someone with a lot of years in practice, who is not in a “franchise practice” which is just a business and usually use the cheapest pellets. What is an unapproved use of a drug, also called “off-label”? Unapproved use of an approved drug is often called “off-label” use. This term can mean that the drug is: Used for a disease or medical condition that it is not approved to treat, such as when a chemotherapy is approved to treat one type of cancer, but healthcare providers use it to treat a different type of cancer. The drugs that are not approved by the FDA, yet are commonly used, have been used for decades before the 1964 law that required new drugs to go through extensive and very expensive testing before their release to the public. The operative word is NEW DRUGS AFTER 1964. Today I will talk about the safety of non-FDA approved drugs because they are: Older cheaper drugs used for many diseases and conditions before 1964 and are still used Drugs that are approved for one use, or one condition, but not for other conditions that it is effective and safe for. Drugs made by compounding pharmacies for diseases that the FDA has not approved a drug for, but there is research backing the drug and years of safe use. First, before we discuss the non-FDA approved drugs, I will discuss the safety/risks of FDA approved drugs, and why FDA approval doesn't mean a drug will do no harm or even that it is effective for the use it is approved for. A little background will help you understand the problem and the reason an FDA approval does not necessarily mean a drug is safe. Since 1964, a law was passed that established testing prior to a drug being approved by the FDA became mandatory. Since that time several drugs that survive FDA approval and are released but are later removed or banned after their FDA release when the public finds side effects that the FDA didn't discover in their trials. One such drug is Fen-Phen, Fenfluramine/Phentermine. This drug was released during my time practicing medicine and was withdrawn after one study claimed it caused heart valve disease…In the end the “one post approval study” that claimed that heart valves were affected by this drug that caused its bann was found to be false. The withdrawal of the drug followed one study by a single cardiologist from Kansas City had reviewed all of the cardiac valve echo tests and falsified the results to make Fen-Phen appear dangerous to heart valves, when in reality it wasn't. She lost her license, but the FDA never put Fen-Phen back on the market! The FDA hates to be wrong twice, so they never allowed this drug back on the market after its removal. Other mistakes made by the FDA include not allowing women in the studies to approve a drug before 2014 which ignores or misses all of the side effects or lack of effectiveness for a drug when taken by women. Despite all the expensive testing before the release of a drug by the FDA, many drugs not tested on women were later often found to have severe side effects only on women. A few examples follow: You might have heard of the FDA approved drug Ambien that causes many women to experience “night eating”, sleepwalking, and night terrors, while their male counterparts were not affected, so because they only tested men the drug was approved. In retrospect it should have been tested on women as well, and then either not passed through the FDA or should have had a black box warning for women. It takes years get action from the FDA, notifying doctors of these side effects. Women were not included in testing for any drugs except female hormones until 11 years ago, but no other drugs. Before 2014 all (non-hormonal) drugs that passed the FDA were not tested on women so the effect on women was unknown until it was tested on the public. The FDA left women out of drug-trials because it viewed women as “mini men”, or they didn't consider us important enough to test new drugs on…OR worse, they believed we were too complicated to easily test us because of pregnancy, menopause and other hormonal swings that normal healthy women have. In any case, we are now suffering their decisions, when a medication works one way for men and another way for women! Finally, we are tested when drugs are being evaluated for approval by the FDA. Professional women have achieved a level of authority in medicine and pharmacology (2025) and are weighing in on the inequity. Women in the medical profession and the public are pulling back the curtain on the side effects of FDA approved drugs that are experienced by women only! Slowly, study by study investigators are now publishing the side effects and problems for women with FDA approved drugs….yet these findings are not included in the warnings on most of these drugs, even now over 15 year after they became obvious to the doctors who treat women! Drugs that either don't work for women, or that have severe side effects include that were approved before 2014. All statin drugs for high cholesterol (Crestor, rosuvastatin, atorvastatin, etc.) cause women to have muscle breakdown and muscle pain. Synthroid (levothyroxine), doesn't cure the symptoms of hypothyroidism in 80% of women, but just makes the TSH lower, so it appears as if it is working! This leads doctors to tell women that their symptoms are all in their heads!! Wrong. It is the wrong medicine. Women have enzymes that differ from men that make it difficult for them to convert the inactive form (T4) into the active form (T3), so we can't convert Synthroid (all T4) into the active form. Synthroid, the FDA approved drug for hypothyroidism, shouldn't be given to most women. Women should be given the non-FDA approved drug Armour Thyroid or NP thyroid that have both T3 and T4 in them! Ambien Prednisone and other oral steroids We have reviewed the lack of testing on women before 2014, now we will discuss safe drugs that have been used for decades even before 1964 when the FDA required testing for FDA approval? Older, yet effective and inexpensive drugs have been tested by the public, some for almost 100 years that have saved thousands of lives, yet they are not given the FDA stamp of approval! In fact, the FDA tries to put these drugs out of circulation, replacing them with very expensive drugs that are new! Or they just shut them down, because they are not FDA approved. Young doctors are told not to use them by their medical schoolteachers who rarely have experienced these medications in private practice…. These doctors in training don't know the history of older safer, cheaper drugs, or even why the FDA tells them avoid them. They comply not knowing why, so you are left with no drug that works for you, or you pay 3-10 times the amount for a newer FDA version of the older drug which may even have more side effects. Some of these older very effective and cheap drugs are Penicillin, Nitroglycerine for chest pain, Morphine (pain), Phenobarbital (seizures), Codeine, Armour Thyroid, hormone injections including estradiol injections and testosterone, Thorazine for psychiatric use, (Pitocin) oxytocin for labor, lactation support and Autism Colchicine:Used to treat and prevent gout. Progesterone in oil (IM) Estradiol in oil (IM) B12 for injection Testosterone Cypionate for injection Compounded Estradiol in any form Compounded Testosterone for women These drugs have been used for so long that any safety risks or side effects have been found through the use of these drugs in the population. Yet the FDA won't grandfather them in and approve them based on their history! What do doctors do when the drug the FDA has approved a drug that doesn't work for a group of their patients (gender, race, blood type, etc.)? What happens when a doctor can't find a drug that is FDA approved needed to treat a condition she is faced with? Why do we as citizens, allow the government to have power over doctors who are already controlled by their state licensing boards as to what medications they? Lastly Why do taxpayers allow a government agency that they fund with tax dollars control their health by banning, or not approving drugs, or banning one drug so an outrageously expensive drug is put in its place? Compounded Medications/ Compounding Pharmacies: These drugs are made by mixing ingredients to meet individual patient needs and are not subject to premarket review for safety, effectiveness, or quality. However, they ARE subject to the success or failure for which they were prescribed. If a doctor prescribes a compounded drug that doesn't work, she is apt to be confronted by her patient who is not getting the expected results. Compounding pharmacies usually don't get paid by insurance, so patients are more invested in getting a drug that works and that is one of the big reasons that Compounded medicines are at least as good or better than big pharma or generic drugs. I absolutely could not successfully treat the thousands of women and men that I have without compounding pharmacies. They compound hormones/drugs that are safe and effective, mostly hormones that can't be patented because they occur in nature and won't ever be made by big pharma. More than that, big producers of drugs can't produce in mass quantities many doses of a certain hormone like compounding pharmacies do. Compounding pharmacies provide what people need and they continue to do so because patients prefer their dosing and quality. FDA approved Generic Drugs can be legally 25 % lower dose than what they say they are. That would be a big problem if my compounded pellets had that kind of variability. People might need pellets every 2months or every 5 months instead of every 4 months..it would be like guessing what you need ahead of time…..I believe dedicated compounding pharmacists are more accurate than any generic on the market. Compounding pharmacies: Unsung Heros Compounding pharmacies serve the public when big pharma fails and hasn't developed a safety net for production if they have a problem and the FDA shuts them down. That situation leaves patients who take their medications, without an alternative. Compounding pharmacies step into the breech when big pharma has a problem with a particular drug and stop making a drug (e.g. Lidocaine, B12 injectable, IV Fluids, to name a few shortages and no production that have occurred in the recent past). What if patients couldn't get the meds they need, and if there were no compounding pharmacies—Chaos and suffering and dying patients would closely follow! The FDA is Fickle and is not on your side! For years the FDA did not approve of Bioidentical estrogen and testosterone in any form, and just a few months ago all of a sudden, long after they scared women from taking the hormones they needed to improve their length of life and quality of life, they decided bioidentical hormones are better than the FDA approved hormones!!! That is a little too late. Some of us will never forget the stress lack of approval of compounded hormones caused for doctors and patients alike. Other doctors criticized us and now most of them aren't even in practice anymore. Maybe the FDA read my blog!!! Compounded hormones have been approved by the masses of women who have taken them under my signature! Compounded BI hormones are medications with a long track record and should not have to be tested with the bloated expense required of testing for the FDA. For Gynecological Disorders that don't have an FDA approved hormone drug because testosterone and estradiol have been used for so long that they don't need testing. If there was a significant problem with them their history of use of over 5-7 decades has proven the efficacy and safety of the female hormones for treatment and hormone replacement. For Psychiatric Disorders: Some patients need compounded ketamine products for conditions like severe depression, despite lacking FDA approval for these uses and potential risks, yet it has been used for this purpose for decades and was used for childbirth for almost a century, until epidurals and saddle blocks took their place. Testosterone for women still is not recognized as a female hormone even though women produce over twice as much Testosterone as Estradiol when they are in their fertile years. Replacement of T with bioidentical T pellets offers a treatment for dozens of symptoms women face after age 40, and it prevents the diseases of aging: osteoporosis, heart disease, sarcopenia, frailty, diabetes and more that have not been addressed by mainstream medicine and the FDA. Over a decade ago, the FDA turned down the approval of testosterone patch after over 3 years of positive research studies, the FDA said they didn't approve T for women is because the side effect of T for women, facial hair, was dangerous for women.…I cry B—–S—-! That is really men not wanting to share testosterone replacement with women. I say leave us alone and let women and their doctors determine what they need. It is proven that only 5% of all professionals in any profession are not trustworthy, so give doctors their due and trust that we are looking for answers to our patients' problems that you don't even know about! The FDA is paid for by us…everyone in this country. I say hands off! Speed up the approval process or forget it for older drugs and BI hormones! ~

Gospel Luke 11:42-46 The Lord said: “Woe to you Pharisees! You pay tithes of mint and of rue and of every garden herb, but you pay no attention to judgment and to love for God. These you should have done, without overlooking the others. Woe to you Pharisees! You love the seat of honor in synagogues and greetings in marketplaces. Woe to you! You are like unseen graves over which people unknowingly walk.” Then one of the scholars of the law said to him in reply, “Teacher, by saying this you are insulting us too.” And he said, “Woe also to you scholars of the law! You impose on people burdens hard to carry, but you yourselves do not lift one finger to touch them.” Reflection Jesus had an intention, not necessarily to insult those that were leading the temple as much as it was to change their understanding of who they were. They were blinded by their darkness. And what we see in this is that they are making other people enter that darkness with him without even knowing it. To step on a grave would make a person unclean, to be in the web of the Pharisees and scribes would also make them unclean. So Jesus is worried about the impact he's having on the people, and as he reveals it, all they can hear is an insult that is unfairly offered. Closing Prayer Father, bless us with the understanding of the fullness of who you want us to be. If it was just about you and us, it would be different. But it's about us and everyone around us.
