Kraig (22) graduated from high school on May 16. His graduation party will be in June.  Derrick finished his first engineering co-op in May. He's taking classes at The University of Cincinnati this summer. Nathan is still a Plant Manager at First Solar (Perrysburg, OH). He turns 50 in June. I've been driving my BMWC400X motor scooter.  I went out to breakfast with my Mom for Mother's Day.  I went to the Annual Meeting of the Ohio Pharmacists Association in Columbus, OH in April. I'm making YouTube videos using pictures/videos from my Europe trip last Spring. Click here to watch the Netherlands leg of our journey. I'm still growing out my hair color. Welcome to the 55th episode in my drug pronunciations series. In the episode, I divide oxcarbazepine and Trileptal into syllables, tell you which syllables to emphasize, and share my sources. We don't cover pharmacology in this series. Just pronunciations. Today's drug name was chosen from Davis's Drug Guide for Nurses, Seventh Edition.    Written pronunciations are helpful! They are below and in the show notes on https://www.thepharmacistsvoice.com/podcast.   oxcarbazepine = OX-kar-BAZ-e-peen The first two syllables sound like “box-car.” (Drop the “B.”) Baz, like basketball e, which is a short “e” sound, like the “E” in the middle of the word Elephant Peen, like a ball peen hammer Emphasize OX and BAZ. BAZ gets the most emphasis. Winter in Ohio has been icy and cold.  We have had a lot of major expenses lately (furnace, car battery, remodeling) We went to Disney on Ice in December. It was great! Nathan and I celebrated 32 years of being a couple. (Started dating in 1993) Nathan is a plant manager at First Solar in Perrysburg. Kraig will finish high school in May. Derrick is working a co-op job in the Cleveland, OH area. I have been perfecting my vegetable-beef soup recipe, and I was featured on the local news to talk about my local podcast, The Perrysburg Podcast.    Business update I have 2 podcasting clients and 1 consulting client. I also signed up to teach a Podcasting 101 Class locally.  I created a vision board for my business (and my life) for 2025. I'm learning how to do a LinkedIn LIVE. My book about podcasting will be available on Amazon and Audible the first week of March. I finished a 12-month coaching program called Simplify and Streamline with Dr. Christina Fontana and Dr. Katie Wood   Podcast update I will continue publishing weekly episodes thru May 2025.  Angel Bivens RPh and Dr. Wendy Stephan (poison prevention experts) will return to the podcast for an interview March 14, 2025 in honor of Poison Prevention Week.  Dr. Rachel Gainsbrugh will be part of The Pharmacist Podcasters Series in March 2025. Compare your business or organization to a competitor. Indicate what sets you apart from them. Apply your competitive advantage to podcasting. Brainstorm how podcasting can help your business or organization stand out. Analyze your audience's pain points and feedback, and develop 5 podcast episodes that offer solutions, using your area of expertise. Develop a timeline for your list of 5 episodes, and program the dates for recording and publishing them into your calendar. Create your vision for podcasting success, so you can recognize when you have achieved your goal for podcasting.   Your voice matters, and it gives you a competitive edge. It sets you apart. This is my Summer 2024 update. The goal of the series is to inspire you to write your first book.     Dr. Helen Sairany is my guest.  She is an expert on trauma.  As we discuss her book, The We you Don't See:  Understanding the Long Shadows of Trauma, she shares the following:   Her background as a trauma survivor Why she wrote The We you Don't See: Understanding the Long Shadows of Trauma What the book is about Her target audience When to read the book What she wants you to do with the information [after you read the last page] Where you can buy the book Advice for pharmacist authors   During our interview, Helen also reads two excerpts from the book.     Bio - written by Helen Sairany, PharmD (July 2024)   As a 7-year-old Kurdish child in Iraq, a country torn by war and conflict, I was spotted by a U.S. Marine deployed to my country with a grenade in my hand, who saved my life by exchanging the grenade for a bag of candy. He later escorted my family and I out of war to seek refuge in the U.S.   Because of my turbulent childhood, I was diagnosed with complex PTSD in 2013. Thus, I aspire to live in a world where the vast majority are trauma-informed, feel psychologically safe and valued for the work they put out, and return home fulfilled.   Since the outbreak of the COVID-19 pandemic began, I have been open about my dormant childhood trauma. He first joined ISMP in 2000 as an ISMP Safe Medication Management Fellow. Prior to joining ISMP, he served as a home care and long-term care pharmacy surveyor for the Joint Commission. He was project leader for the Hospital and Healthsystem Association of Pennsylvania (HAP) Hospital Improvement Innovation Network's (HIIN) Adverse Drug Event (ADE) Project, a collaborative project engaging healthcare organizations to reduce and prevent patient harm from the use of anticoagulants, insulins, and opioids. He has published numerous articles in the pharmacy literature, including regular columns in P&T and is a journal reviewer for a number of publications including the Joint Commission Journal on Quality and Patient Safety, Pharmacoepidemiology, Journal of Managed Care and Specialty Pharmacy, BMJ Quality and Drug Safety, and Annals of Internal Medicine. He is a chapter contributor to a textbook published by McGraw-Hill entitled Pharmacy Management: Essentials for All Practice Settings, Essentials of Nurse Informatics, Remington: The Science and Practice of Pharmacy, and Medication Errors. He is recently completed the Just Culture certification course. Mr. Grissinger serves as the Chair for the National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP), and Co-Chair of the National Quality Form (NQF) Common Formats Expert Panel. He is also on the Faculty Advisory Board for the Pharmacy Learning Network (PLN) and the Publications Advisory Board for Davis's Drug Guide for Nurses. He also served on the WHO Focus Group on Measurement Tools for Medication Safety, United States Pharmacopeia's (USP) Safe Medication Use Expert Committee from 2005-2010, the FDA Proprietary Name Review Concept Paper workshop panel in 2008, FDA Naming, Labeling, and Packaging Practices to Minimize Medication Errors workshop panel in 2010 and the Joint Commission Home Care Compounding Pharmacy Technical Advisory Panel in 2013. He is also an adjunct assistant professor for Temple University School of Pharmacy. My Novel Career in Pharmacy Speech for Pharmacy Students

As an ACLS provider you do not need to be familiar with all of the different signs of various types of poisoning. You should be able to obtain a history and know to order toxicology.The majority of toxins don't have a specific antidote. There are a few toxins for which we have emergency interventions and ACLS providers should be familiar with.Reviewing the patient's medical history for indicators that may lead us to suspect a tablet/toxin cause of cardiac arrest. Administration of Narcan for suspected narcotics overdose following the Opioid Associated Emergency algorithm. Other common ACLS Tablet Toxin scenarios with possible treatments.Medications commonly used to treat specific toxins that are regularly stocked on crash carts or carried in EMS med bags. ACLS providers that suspect a specific toxin should consult with their Pharmacy or call Poison Control for treatment directions. How do we know if high quality, effective CPR is being performed? Objective measures of high-quality CPR include: Compression rate; Compression depth & recoil; ETCO2; and Chest Compression Fraction (CCF). The role of the CPR coach position on the code team.The advantages and use of real-time feedback devices to monitor the rate, depth, and chest recoil of CPR compressions. The use of end tidal waveform capnography. The 2020 ACLS guidelines updated the dose for administration of Atropine and Dopamine for the treatment of unstable bradycardia. The signs & symptoms of unstable bradycardia. Atropine's new dose and maximum. The use of atropine when a patient is in a second degree type II or third degree heart block.ECG changes that indicate subsequent doses of atropine are likely to be ineffective. The 2020 update to the starting dose of Dopamine. The tongue is the most common airway obstruction in an unconscious patient. Insertion an oropharyngeal airway helps keep the patient's tongue from falling to the back of the pharynx and causing an airway obstruction. The oropharyngeal airway is sometimes called an OPA or simply an oral airway. Indications for using an oral airway. Contraindication for an oral airway and an alternative that can be used instead. Measuring an OPA and possible complications from inserting one that's too small or too large. Two techniques to properly insert an OPA and avoid complications. The use of an oral airway during CPR. ECG characteristics of supraventricular tachycardia (SVT) vs. sinus tachycardia. Signs & symptoms that indicate a patient is unstable. Delivery of a synchronized shock for the treatment of unstable SVT using a biphasic vs monophasic defibrillator. Consideration for team safety while performing synchronized cardioversion. Actions to take immediately if an unstable patient we've cardioverted goes into a pulseless rhythm. Management of stable patients in SVT. Administration of Epinephrine During Cardiac Arrest

When working to resuscitate a patient in sudden cardiac arrest, Epinephrine is the first IV medication we administer. When we give the first dose of epinephrine depends on whether the patient is in a shockable or non-shockable rhythm. When to give the first dose of epinephrine and its frequency for patients in asystole or PEA following the right side of the Adult Cardiac Arrest algorithm. When to give the first dose of epi and its frequency for patients in V-Fib or pulseless V-Tach following the left side of the Adult Cardiac Arrest algorithm. Example chronology of events for a scenario where a patient is found unresponsive with only gasping/agonal breathing. Administration of epi via the IO or endotracheal route in the absence of an IV. The maximum cumulative dose of epinephrine that can be administered to patients in cardiac arrest. If a person suddenly develops symptoms such as weakness, slurred or garbled speech, loss of balance, or a massive & severe headache; it's possible they could be having a stroke.The Cincinnati Prehospital Stroke Scale uses the acronym FAST to assess Facial droop – Arm Drift - Speech – and Time. There are several conditions that can mimic a stroke. Identification & Treatment of hypoglycemia or hyperglycemia.Identification & Treatment of hypoxia using a pulse oximeter. Some seizures, electrolyte imbalance, sepsis, brain tumors, and Bell's Palsy can also mimic a stroke. Prehospital providers should transport suspected stroke patients to a stroke center following their local protocols. The chain of survival for a cardiac emergency and stroke start the same:1. preparedness & recognition of an emergency;2. activation of EMS; 3. delivery of Advanced Life Support; and 4. transporting to the most appropriate facility. Depending on where you live, Emergency Medical Services (EMS) may provide prehospital Advanced Life Support (ALS). ALS ambulances are staffed with paramedics who have training in ACLS skills. Paramedics can perform an assessment, obtain a medical history, and provide life-saving care within minutes of recognition. Why EMS "Destination Protocols" for suspected stroke and STEMI make a difference. For apneic patients without a carotid pulse or pulseless patients with only gasping/agonal respirations, we will follow the Adult Cardiac Arrest algorithm. For pulseless patients that the AED doesn't advise a shock, the patient's ECG shows asystole, or a non-perfusing organized rhythm (PEA), we will follow the right side of the algorithm.Initial steps are aimed at delivery of high-quality CPR to keep the brain and vital organs alive. Epinephrine administration. Placement of an advanced airway. Providing good, high quality, CPR with minimal interruptions and early defibrillation are two keys to improved cardiac arrest outcomes. Kim is not only a pharmacist, but a voice actor, podcast host, volunteer, wife, and the mother of two young men. She founded The Pharmacist's Voice®, LLC in 2017 and launched The Pharmacist's Voice® Podcast in 2019. Through this work, she has become a go-to pharmacist in audio production. Kim believes in the transformative power of well-communicated messages, and her mission is to connect with her audience to develop trust. She graduated from Butler University's College of Pharmacy and Health Sciences in 2008 and is currently in the EdD program at Marshall University, with a focus on curriculum and instruction. In March of 2020, with John Bollinger, she formed We Matter Too, Inc. which focuses on helping those with mental health issues and mental disorders through first-person narratives of experience. I discuss pharmacogenomics (PGx) with Dr. Dave Kisor from Manchester University College of Pharmacy, which is located in Fort Wayne, IN. If you're interested in pharmacogenomics, you will like this episode! Bio (Feb 2023) Dave Kisor, PharmD is professor and director of pharmacogenomics at Manchester University. He graduated from The Ohio State University College of Pharmacy 1986 and completed a two-year fellowship in therapeutic drug monitoring/pharmacokinetics at OSU. Prior to joining Manchester, Dave was professor of pharmaceutical sciences at Ohio Northern University after being a research scientist at Burroughs Wellcome Co/GlaxoWellcome. He has been a clinical PGx consultant since 2012. He has over 80 peer reviewed publications including being the lead author on two pharmacogenomics textbooks. He is a past chair of the American Association of Colleges of Pharmacy (AACP) PGx SIG. Dave was named a Fellow of the American College of Clinical Pharmacology in 2017. In 2021 he received an AACP Innovations in PGx Teaching Award. His current research is related to PGx and opioid use disorder. Imagine this.  You're a pharmacist, and you wrote a book.  It's published [hurray!], and you now want to reach a broader audience by publishing the audiobook version.  You don't want someone else to narrate it!  YOU want to narrate it!  But, you don't know how to do it. He has experience in retail pharmacy.  Now, he's a college professor, podcaster, online course creator, author, husband, father of 3, youth sports coach, and more.   Some pharmacists have learned that traditional, clinical pharmacist hours are fundamentally incompatible with being a good parent.   Tony reveled the #1 reason someone does not get a residency.  He also gave advice about writing a residency letter of intent. This is a follow-up solo episode from Episode 146, my interview with Christina Madison, PharmD, FCCP,  AAHIVP about autism awareness and acceptance (April 2022).   My name is Kim Newlove, and I'm the host of The Pharmacist's Voice ® Podcast.  I am 43 years old, and I have a 19-year-old son with autism.  I've been married for more than 20 years, and my husband and I also have a 17-year-old son.  Kraig is 19.  Derrick is 17. Here is the description of the podcast: The Pharmacist's Voice Thttps://www.thepharmacistsvoice.com/ (he Pharmacist's Voice) Podcast is a weekly, journey-style podcast about Ohio pharmacist Kim Newlove's career change from pharmacist to voice actor. She alternates solo shows and interview shows. The solo shows are about her career change, and the interview shows feature a variety of people who use their voices to advocate, educate, or entertain. What We Liked About the Show Kim does a good job explaining why the guest is on a show and has a clear idea of who should (and shouldn't be on her show). Kim does a good job of bringing the guest back on topic. Her audio sounds great. Her website is a PHENOMENAL example of someone who has a business AND a podcast and doesn't bury the podcast while keeping the business front and center. Some Things That Could Use Some Tweaking Kim is a voice actor, so her diction is perfect. Towards the end of the show, we conclude that when she slows down the speed of her talking it makes it appear that her diction and delivery is a bit stiff. Her demo is amazing and filled with energy. We want more of that. While Kim's website does a great job of promoting her business, she barely mentions her services in the podcast. She does ask people to visit her website but could boost this effort by explaining why. We provide a few examples of how to tie her business into her podcast. Conclusion As a journey podcast, Kim's podcast has already resulted in her getting some voice work. With 100 episodes under her belt, she is doing a lot of things correctly. He website is a great website where everything appears planned, and her brand just oozes professionalism. We wish her all the best.
