Montgomery J. Granger is a three-time mobilized U.S. Army Reserve Major (Ret.), who was called into his Reserve Center in Uniondale (Long Island), New York, on 9/11, in response to the attacks on the World Trade Center's Twin Towers, the Pentagon, and Flight 93, which crashed at Shanksville, Pennsylvania. He answered his country's call to duty next in January 2002 for a mission to help run the military detention facility at the U.S. Naval Station at Guantanamo Bay, Cuba. He was called up again just six months after returning from duty at Gitmo, but this time remained stateside at the U.S. Army Reserve Training Center at Fort Dix, New Jersey. After nearly six months at Fort Dix, MAJ Granger returned to civilian life for about a year when he was involuntarily transferred to another Reserve Army unit that was deploying to Iraq in the fall of 2004. Major Granger served 14 months of active duty on his third deployment and served in Baghdad, Abu Ghraib, Camp Bucca, and Ashraf, Iraq, as Medical Service officer for military detention facility operations. He is married and is the father of five children, and lives on Long Island, New York. He is also the author of "Theodore," a personal narrative published in the 2006 Random House wartime anthology "Operation Homecoming: Iraq, Afghanistan and the Home Front in the Words of U.S. Troops and Their Families," where he wrote about his fear and anxiety over having left his family in 2002, and especially two-day-old Theodore, and what reaction there was upon his return. Operation Homecoming was sponsored in part by the National Endowment for the Arts, and edited by Andrew Carroll, editor of the New York Times bestselling book, War Letters. Granger was born in Illinois, raised and schooled in Rubidoux, California, and attended undergraduate school at the University of Alabama in Tuscaloosa where he earned a Bachelor of Science degree in Education. He earned a master's degree in curriculum and teaching from Teachers College-Columbia University, where he met his wife. He also attended the State University of New York at Stony Brook, where he earned professional credits to obtain a New York State School District Administrators' license. He is an accomplished coach and teacher of health and physical education, having taught in Alabama, California, New York City, and Long Island, before becoming a director of Health, Physical Education and Athletics. He was most recently Director of Health, Physical Education and Athletics, and then District Administrator for Operations for the Comsewogue school district in Port Jefferson Station, N.Y., and is now a Director of Health, Physical Education and Athletics, and Director of Facilities for an Eastern Long Island school district. Granger is the author of many writings and musing as yet unpublished, but hopefully soon to be shared with a waiting world. - http://www.strategicpublishinggroup.com/title/SavingGraceAtGuantanamoBay.html ****************************************************************** To listen to all our XZBN shows, with our compliments go to: https://www.spreaker.com/user/xzoneradiotv *** AND NOW *** The ‘X' Zone TV Channel on SimulTV - www.simultv.com The ‘X' Chronicles Newspaper - www.xchroniclesnewspaper.com ******************************************************************

As a Jesus Follower, Do You Forgive Others Before They Forgive You, Just as Jesus Did for You on the Cross? MESSAGE SUMMARY: We modern-day Christians are bad about living in our bitterness towards others whom we believe have “wronged us”. However, in Luke 23:33-34, Jesus set the example and His expectations for the “forgiveness of others” when He sacrificed Himself on cross for us: “And when they came to the place that is called The Skull, there they crucified him, and the criminals, one on his right and one on his left. And Jesus said, ‘Father, forgive them, for they know not what they do.’ And they cast lots to divide his garments.". We are great at putting on our Christian face for the public. On the other hand, we have someone in our lives, when even their name is mentioned, we become hateful and bitter towards this person in our actions and words. Jesus modeled what we are to do from the cross -- we are to forgive the person that elicits this kind of bitter response in our lives. We need to release this person and then get on with our lives. As Jesus said: “Father, forgive them for they know not what they do.”. TODAY’S AFFIRMATION: Today, I affirm that, because I am in Jesus Christ, I will trust in the Lord with all my heart. I will trust in the Lord with all of my heart and lean not on my own understanding. In all my ways I will seek to know Him, and He will make my pathways straight. (From Proverbs 3:5f). As a Jesus Follower, Go to God in Your Fear and Anxiety Because Jesus Says: “do not be anxious about tomorrow” MESSAGE SUMMARY: Your problems are not too big for God; God is faithful. Your financial issues, your debt, your health issues, and your sins are not too big for God. God is faithful, and He is trustworthy. Why not take, to God, all those things in your life that are creating anxiety? In Psalms 9:9-10, the Psalmist encourages you to put your trust in God: “The LORD is a stronghold for the oppressed, a stronghold in times of trouble. And those who know your name put their trust in you, for you, O LORD, have not forsaken those who seek you.". Also, Jesus tells us, in Mathew. 6:33-34, that you can put our trust in Him and not be anxious about your problems: “But seek first the kingdom of God and his righteousness, and all these things will be added to you. Therefore do not be anxious about tomorrow, for tomorrow will be anxious for itself. Sufficient for the day is its own trouble.”. Our God is a faithful God – reach out to God when fear and anxiety are taking a firm hold on your life. TODAY’S AFFIRMATION: Today, because of I am filled with the Holy Spirit, I will not be controlled by my Heartlessness. Rather, I will walk in the Spirit’s fruit of Kindness. “The fruit of the Spirit is love, joy, peace, patience, kindness, goodness, faithfulness, gentleness and self-control.” (Galatians 5:22f). Are the Ten Commandments, God’s Law which He Gave to Us, Becoming irrelevant for America? MESSAGE SUMMARY: Today, are we saying that the Ten Commandments are irrelevant for America? Are the Ten Commandments becoming illegal in America? If so, why? As a nation, we no longer teach or practice the Ten Commandments. We have made it illegal to post the Ten Commandments, which are God’s Law to us, on most walls of our courts and government schools. From a spiritual perspective, how can God, the Creator of the Universe, bless a nation which mocks and/or ignores Him? Paul, in Galatians 6:6-7, cautions both us and our governments regarding the mocking of God: “Let the one who is taught the word share all good things with the one who teaches. Do not be deceived: God is not mocked, for whatever one sows, that will he also reap.”. From a practical perspective, how can a society and a government function when their foundation has been removed? Our form of government was founded on the ethical base of the Ten Commandments. The Ten Commandments were in the thoughts and cultural norms of the people that created our country. TODAY’S AFFIRMATION: Today, Because of who I am in Jesus Christ, I will not be driven by Resentment. Rather, I will abide in the Lord’s Compassion. “I am the vine; you are the branches. If a man remains in Me and I in him, he will bear much fruit; apart from Me you can do nothing.” (John 15:5). Jesus Followers Run & Win Life’s Races by Submitting Their Preferences to God Before Making Life-Defining Choices MESSAGE SUMMARY: You need to submit our life preferences to the God. By submitting your life preferences to the Lord, He can help you to understand the life path that your preferences will lead. Paul, in 1 Corinthians 9:24, tells us that our Life Preferences need an overarching focus: “Do you not know that in a race all the runners run, but only one receives the prize? So run that you may obtain it.". Life preferences are not, necessarily, sins; but they can close doors in your life. In Matthew 10:37-39, Jesus provides a context from which we can assess life preference choices: “Whoever loves father or mother more than me is not worthy of me, and whoever loves son or daughter more than me is not worthy of me. And whoever does not take his cross and follow me is not worthy of me. Whoever finds his life will lose it, and whoever loses his life for my sake will find it.". TODAY’S AFFIRMATION: Today, I affirm that, because I am in Jesus Christ, I will live as Jesus would. (Philippians 1:27). “I can do everything through Him who gives me strength.”. (Philippians 4:14). God Blesses You in Your Obedience, but God Leaves You to the Consequences of Your Sin in Your Disobedience MESSAGE SUMMARY: Your relationship with God is only between you and God. God desires relationships with each of us, individually, even though you may have been fortunate enough to have a Godly mother, father, siblings, and/or a spouse. God has given us covenants upon which to build His relationship with you, as Paul tells us in Romans 8:34-35: “Who is to condemn? Christ Jesus is the one who died—more than that, who was raised—who is at the right hand of God, who indeed is interceding for us. Who shall separate us from the love of Christ? Shall tribulation, or distress, or persecution, or famine, or nakedness, or danger, or sword?". The people of the Old Testament found that living apart from God's covenant relationship brought disaster. God is a covenant God; He will bless you if you obey Him. If you disobey God, God leaves you to the consequences of your sins. TODAY’S AFFIRMATION: Today, because of I am filled with the Holy Spirit, I will not be controlled by my Self-Centeredness. Rather, I will walk in the Spirit’s fruit of Self-Control. “The fruit of the Spirit is love, joy, peace, patience, kindness, goodness, faithfulness, gentleness and self-control.” (Galatians 5:22f). Jesus Followers Are Given the Holy Spirit, by Jesus, to Walk with Them in Their Personal Relationship with God MESSAGE SUMMARY: Isaiah tells us about Jesus Followers’ communications with the Holy Spirit in Isaiah 30:21: "And your ears shall hear a word behind you, saying, ‘This is the way, walk in it,’ when you turn to the right or when you turn to the left.". We know, from the New Testament, that Jesus Followers are given the Holy Spirit to walk with them -- "a voice behind you". Since the Holy Spirit takes up residence in you, the way the Lord speaks to you is through the inward witness of the Holy Spirit -- the Spirit will speak, and you will know that it is the Lord. In John 14:26, Jesus reminds us about His promise, to His followers, of the “Helper” (Holy Spirit): “But the Helper, the Holy Spirit, whom the Father will send in my name, he will teach you all things and bring to your remembrance all that I have said to you.”. Additionally, in John 16-7, Jesus tells us that not only will the Holy Spirit be your “Helper”, the Holy Spirit will be your “Comforter”: “Nevertheless I tell you the truth; It is expedient for you that I go away: for if I go not away, the Comforter will not come unto you; but if I depart, I will send him unto you.". TODAY’S AFFIRMATION: Today, Because of who I am in Jesus Christ, I will not be driven by Anger. Rather, I will abide in the Lord’s Forgiveness. “I am the vine; you are the branches. If a man remains in Me and I in him, he will bear much fruit; apart from Me you can do nothing.” (John 15:5). Jesus Followers Have Eternal Life, Even Before their Earthly Death, Because they “believes him {God} who sent me {Jesus}” MESSAGE SUMMARY: First, the moment you become a Jesus Follower, you have immediate Eternal Life; and you become a Citizen of Heaven. Jesus tells us, in John 5:24, that His followers have Eternal Life and are Citizens of Heaven here on earth living their earthly lives: “Truly, truly, I say to you, whoever hears my word and believes him who sent me has eternal life. He does not come into judgment, but has passed from death to life.". As Christians and Citizens of Heaven, we know that the Lord is coming back -- either to greet us when we die or before we die at His Second Coming. For most of us, however, our vision of Heaven is clouded by looking backwards; therefore, the return of Jesus does not have any power in our life. Paul reinforces the return of the Lord of Heaven in Philippians 1:6: “And I am sure of this, that he who began a good work in you will bring it to completion at the day of Jesus Christ.". When Jesus returns, we will put on our spiritual bodies, as promised. As Jesus tells us in Matthew 4:17: “From that time Jesus began to preach, saying, ‘Repent, for the kingdom of heaven is at hand.’”. TODAY’S AFFIRMATION: I affirm that because of what God has done for me in His Son, Jesus, I AM RIGHTEOUS IN GOD’S EYES. God made Him who had no sin to be sin for us, so that in Him we might become the righteousness of God. (2 Corinthians 5:21). Jesus Followers Are Key to Stopping Marriage’s Perversion by Following God’s Plan in Their Families and Marriages MESSAGE SUMMARY: Introduction from Prior Sermon: It is one thing to know Jesus in our lives, but it is another thing to love and obey Jesus in our lives. In other words, are you willing to let go of controlling your life and turn your life’s control over to the Holy Spirit – God’s will not your will be done? God does not force His will on you. You must seek Jesus to follow Him, and you must follow Jesus “in truth and in {Holy}Spirit”. Therefore, the question becomes not “do you have the Holy Spirit”, but “does the Holy Spirit have you”? Today’s Message -- Marriage, A Current Assessment: Marriage and the family, in current decades, have been under attack to the degree that marriage is now losing its value in our culture. In Mark 10:6-9, Jesus speaks to God’s plan for marriage: “But from the beginning of creation, ‘God made them male and female.’ ‘Therefore a man shall leave his father and mother and hold fast to his wife, and the two shall become one flesh.’ So they are no longer two but one flesh. What therefore God has joined together, let not man separate.’” In our culture’s attacks on marriage and the family, we see ten ways in which marriage and the family have been and are being perverted: 1) US Supreme Court’s decision on same sex marriage – as Jesus Followers, we don’t believe the courts have the right to define/redefine marriage as established by God; 2) sex before marriage is becoming the norm; 3) sex outside of marriage has become acceptable; 4) the prevalence of divorce; 5) acceptance of cohabitation; 6) birth of children out of wedlock; 7) multi-spouse marriage; 8) escalation and the acceptance of pornography; 9) the acceptance and practice of abortion on demand; and 10) the acceptable “choosing” one’s own gender by some people. These perversions of marriage and the family are chipping away at the value and importance of marriage and the family in the US – even among Christians. Too many of our Christian young people do not know why they should get married -- this diminution of marriage’s value is a failure, mostly, of the Christian Church. Many Christians, today, are being deceived and led away from God’s Word by the Evil One (e.g., John 10:10). Therefore, we need to read and understand God’s Word, especially as it relates to His plan for marriage that Jesus posits in His quote from Genesis in Mark 10:6. Throughout history humans have corrupted God’s beautiful plan for His Creation. In Mark 10:7, we see God’s pattern for marriage. In Mark 10:8, we are provided with God’s purpose for marriage; and the product of this “one flesh” is children. By becoming “one flesh” in marriage, the man and women become a “new trinity” – husband, wife, and the Lord. In this “marriage trinitarian relationship”, each child is brought into this family relationship in the same way that we, as Jesus Followers, are brought into our personal relationship with the Father, Son and Holy Spirit through the Gospel. In Mark 10:9 we see God’s expressed permeance of marriage. God’s design and plan for marriage, as presented by Jesus in Mark 10:6,9, is ideal for us as humans, our nation, and our culture: “But from the beginning of creation, ‘God made them male and female. Therefore a man shall leave his father and mother and hold fast to his wife, what therefore God has joined together, let not man separate.’". However, God’s plan is being increasingly perverted. Therefore, what should we do, as Jesus Followers, about these “attacks on marriage”? We can: 1) stand up, speak up and speak God’s truth – Christians need to stop being trampled upon by this divergent culture; 2) model good and Godly marriage and family life to the culture; 3) you teach your children, you not the church/school/government, about God and His plan for marriage and the family; and 4) pray and ask God to intervene in these attacks and to support our seeking to achieve His plan for our own marriage and the family – prayer does change things, and it brings the power of God. We are blessed to live in an incredible time in history, but the plan and fabric that God designed to keep us together and thriving – marriage and the family – is being perverted. However, we, as Jesus Followers, hold the key to stopping this perversion of marriage though getting our own family and marriage in line with God’s plan and then witnessing and voting to stop this perversion – the time is now. Scripture Reference (ESV): Mark 10:6-9; John 10:10; Genesis 2:21-25. Jesus Followers Are Key to Stopping Marriage's Perversion by Following God's Plan in Their Families and Marriages MESSAGE SUMMARY: Introduction from Prior Sermon: It is one thing to know Jesus in our lives, but it is another thing to love and obey Jesus in our lives. In other words, are you willing to let go of controlling your life and turn your life's control over to the Holy Spirit – God's will not your will be done? God does not force His will on you. You must seek Jesus to follow Him, and you must follow Jesus “in truth and in {Holy}Spirit”. Therefore, the question becomes not “do you have the Holy Spirit”, but “does the Holy Spirit have you”? Today's Message -- Marriage, A Current Assessment: Marriage and the family, in current decades, have been under attack to the degree that marriage is now losing its value in our culture. In Mark 10:6-9, Jesus speaks to God's plan for marriage: “But from the beginning of creation, ‘God made them male and female.' ‘Therefore a man shall leave his father and mother and hold fast to his wife, and the two shall become one flesh.' So they are no longer two but one flesh. What therefore God has joined together, let not man separate.'” In our culture's attacks on marriage and the family, we see ten ways in which marriage and the family have been and are being perverted: 1) US Supreme Court's decision on same sex marriage – as Jesus Followers, we don't believe the courts have the right to define/redefine marriage as established by God; 2) sex before marriage is becoming the norm; 3) sex outside of marriage has become acceptable; 4) the prevalence of divorce; 5) acceptance of cohabitation; 6) birth of children out of wedlock; 7) multi-spouse marriage; 8) escalation and the acceptance of pornography; 9) the acceptance and practice of abortion on demand; and 10) the acceptable “choosing” one's own gender by some people. These perversions of marriage and the family are chipping away at the value and importance of marriage and the family in the US – even among Christians. Too many of our Christian young people do not know why they should get married -- this diminution of marriage's value is a failure, mostly, of the Christian Church. Many Christians, today, are being deceived and led away from God's Word by the Evil One (e.g., John 10:10). Therefore, we need to read and understand God's Word, especially as it relates to His plan for marriage that Jesus posits in His quote from Genesis in Mark 10:6. Throughout history humans have corrupted God's beautiful plan for His Creation. In Mark 10:7, we see God's pattern for marriage. In Mark 10:8, we are provided with God's purpose for marriage; and the product of this “one flesh” is children. In Mark 10:9 we see God's expressed permeance of marriage. God's design and plan for marriage, as presented by Jesus in Mark 10:6,9, is ideal for us as humans, our nation, and our culture: “But from the beginning of creation, ‘God made them male and female. Therefore a man shall leave his father and mother and hold fast to his wife, what therefore God has joined together, let not man separate.'". However, God's plan is being increasingly perverted. Therefore, what should we do, as Jesus Followers, about these “attacks on marriage”? We can: 1) stand up, speak up and speak God's truth – Christians need to stop being trampled upon by this divergent culture; 2) model good and Godly marriage and family life to the culture; 3) you teach your children, you not the church/school/government, about God and His plan for marriage and the family; and 4) pray and ask God to intervene in these attacks and to support our seeking to achieve His plan for our own marriage and the family – prayer does change things, and it brings the power of God. We are blessed to live in an incredible time in history, but the plan and fabric that God designed to keep us together and thriving – marriage and the family – is being perverted. However, we, as Jesus Followers, hold the key to stopping this perversion of marriage though getting our own family and marriage in line with God's plan and then witnessing and voting to stop this perversion – the time is now. This week we continue the abuse mini-series on the Divorce Survival Guide podcast. In this episode, Leslie Morgan Steiner, author of The Naked Truth, returns to the show to join me in a discussion about domestic violence. She is a New York Times best-selling author, a columnist for The Washington Post, a speaker on work/family balance, a successful corporate executive – and domestic violence survivor. Her book, Crazy Love, is a harrowing memoir about her journey through a severely violent marriage. Together we discuss the complexities of domestic violence and Leslie shares how she ultimately ended her tumultuous marriage and began to heal. Still, to come on the Divorce Survival Guide mini-series on abuse, we’ll explore financial abuse and how to communicate with a high-conflict co-parent. You can sign up for my email list to be notified whenever a new episode is released or subscribe to the podcast on your favorite podcast listening app. Show Highlights Leslie’s story of domestic violence abuse and how it was a transformational experience. (9:09) The stigma and shame of being an abuse victim is similar to being a sexual assault victim. (15:07) 85% of abusers are men and some red flags to watch for in potential partners. (16:33) Caution: If you think “I can't help him, but if he gets into therapy, then he'll be okay,” this is true — if he does his own work and his own therapy. The research has shown it can be a decades-long process. (27:47) Domestic violence is a triage situation: help the victims first, but we're never really going to end abuse unless we start focusing on perpetrators getting the counseling that they need. (28:21) Abusive relationships: should you stay or should you go? (30:12) How Leslie reconciled being the victim of a destructive relationship and the champion of relationships themselves as a force for healing. This conversation was an early Christmas gift. I personally believe this book should be part of the curriculum in every academy and read by LEO'S and their families all over the country. We spoke about the biological and psychological changes that Officers face when during and after their careers and touched on the ways Officers can improve their mindset and overall health. I am truly speechless and honored to have had this conversation with such a caring, intelligent, and determined retired Officer. Thank you Dr. Gilmartin for fighting the good fight and being a prime example of leadership! If you are young on the job or want to become a first responder, this is for you. Dr. Anthony L. Back, MD, a specialist in gastrointestinal oncology and palliative medicine at the University of Washington Medical Center and professor of medicine at the UW School of Medicine, discusses palliative care skills and practical applications to help improve patient-clinician communication and interventions to make clinicians more effective. Transcript ASCO Daily News: Welcome to the ASCO Daily News podcast. I'm Geraldine Carroll, a reporter for the ASCO Daily News. I'm delighted to welcome Dr. Anthony Back to the podcast today. Dr. Back specializes in GI oncology and palliative medicine at the University of Washington Medical Center, and is Professor of Medicine at the UW School of Medicine. His research focuses on patient-clinician communication and interventions to make clinicians more effective. Dr. Back, do you have any conflicts of interest to disclose in relation to the issues we'll discuss in the podcast today? Dr. Anthony Back: Well, I would like listeners to know-- and thank you for that lovely introduction, Geraldine, and please feel free to call me Tony. I would like listeners to know that I co-founded a nonprofit called VitalTalk using work that had been funded by the National Cancer Institute over a number of years. And I am on the board of that organization, and it is a nonprofit, but listeners should know. ASCO Daily News: Great. All right, Tony, you have dedicated your career to improving communication between physicians and patients. Can you tell us about the work you've been doing to bring humanism and medical expertise to the oncologist-patient relationship? And I'd love to know why you decided to focus on this approach in palliative medicine. Dr. Anthony Back: Yeah, sure. I started to focus on this-- really, it was out of kind of necessity. It turns out my mother died of a blood cancer when I was a sophomore in college. And so I was confronted with this issue of mortality quite early, and it really did shape my college career and my medical school career, because I suddenly thought, wow, I just I just can't do all this career stuff, I have to figure out how to live, and that led me to a college career of reading the great novels. And it turned out that my senior thesis, which was about narrative technique, was the beginning of my learning to closely study human conversations. And so when I was an oncology fellow, what really struck me was the way that doctors cared so deeply about the patients that they saw, and yet when they interfaced with them, when they got together with them, what they said seemed so limited and so-- like a partial truth. Like what was said in the conference room wasn't at all what was said in front of the patients. And over time what I could see is that patients suffered from not getting the full story in a way they could listen to. As we've gotten more and more data about how patients do, we have more data to present to them, but very often it's presented in a way that doesn't quite resonate for them. And so I think very often it fails to land, basically. There are a number of studies that show that patients hear information or receive information but that it doesn't affect their decision-making, it doesn't affect their understanding. And what I think that speaks to is that just giving information is not enough. Like we have to engage patients in the way they live at the emotional level in some way, because for all of us-- actually, how humans are built, we process the most important things about our lives through our emotions and our intellects. And so I came to this as a way of trying to enable patients to live better even in the face of their cancer. I cut my teeth early in the movement of palliative medicine, and that movement started with a lot of emphasis on having a good death, which was, of course, a revolutionary idea that had come from hospice. But I think what has landed now and what resonates with patients now is this idea of, how do I live as much as I can in whatever time I have? And I feel like that's the way we ought to be approaching this now. And bringing that to a large number of patients really required that I go beyond my role as an academic and learn to take a kind of communication training that we had developed over years with funding from the National Cancer Institute and scale it using tools developed by social entrepreneurs and startups. And so VitalTalk was started about five years ago as a startup, and basically I took lessons from all the social entrepreneurs in Seattle and San Francisco about how do you start a company, how do you change people's behavior, how do you get people to pay attention? Because doctors are pretty busy. So that's been a little bit of my journey. And what's really moved me about this is that as palliative care has come to be seen as a essential part of what oncologists do and what cancer patients need, there is a large group of clinicians who recognize that and are willing to work on it and are really interested in those kinds of connections with the people that they work with. And so that's really interesting and gratifying to see. ASCO Daily News: Well, I know many listeners will agree with you, but they will also say they work in a system that doesn't really reward oncologists for having the skills to navigate difficult conversations with patients or to take the time to be more present - to practice that compassionate silence you talked about. How do you teach oncologists to listen effectively and ask the right questions in this kind of complex environment? Dr. Anthony Back: Yeah. So first, I mean, I've got to acknowledge that the environment, in many ways, doesn't prioritize this. I actually think that if you ask oncologists what are the really important things they do, virtually all of them would name connecting with patients in the top couple of things. And I think, for many oncologists, those connections are the deepest satisfaction in their careers. And so even though the environment is rushed, it focuses a lot on time pressure, it grades oncologists by their productivity, what I can do is remind oncologists of how satisfying it is to have these connections, and to show them that a small investment in doing this kind of work pays off in spades over and over and over. And once you start to see that, you really can't unsee it, and I think that's what really changes oncologists' practice. It's not by other people telling them what to do, it's by them feeling like this is really the right way to practice. And it's still a challenge, and it's also kind of busting this myth that if I just had more time, I could do this. Actually, you don't need that much more time. What you really need is skill. If you don't have very well-developed communication skills, it takes a really long time to communicate effectively. And so that is where this idea comes that you need lots of time. But if you have well-honed communication skills, you can take advantage of whatever piece of time you have and make that meaningful for your patient and make that meaningful for you. ASCO Daily News: Right. And so what are some of those key questions to ask a patient? How does an oncologist practice very good primary palliative care? Dr. Anthony Back: Yeah. So a couple of the things that oncologists can do are make sure that they have thought to themselves-- and this is in an article that we wrote for the ASCO Educational Book about palliative care and communication, and I mentioned that oncologists can take a moment before they go in the room to be clear about a couple of things-- what's happening, how does the patient feel or how might they feel, how do I feel, and then the third one is, what does it all mean? And by being clear about those, you can manage both the information and the emotions needed to make that encounter valuable for both you and the patient. And by valuable, I don't mean just that they understand, but I also mean that they take appropriate action, because the big goal is for every patient to have treatments that match their values, right? And so that's where this comes from. So how you do that is-- I think the number one recommendation I have for oncologists is, make sure that as you present information, that you also listen for the emotions that are underneath the surface, and that you acknowledge them by saying, wow, it sounds like that's a big issue, or wow, I can see why that's so important to you, or I can't imagine what it would be like to be living with these kinds of challenges. I think those little things, which seem, on the one hand, almost like throwaways, actually can change the course of a conversation. And what the studies show is that when clinicians acknowledge patients' emotions explicitly, that patients actually give them more information, because what you're doing as a clinician there is you are creating psychological safety, an atmosphere in which patients will tell you what's really on their minds. And it turns out, getting to that point quickly, having the patient tell you what's really on their minds, actually saves time in the long run. You don't have all those follow-up calls after the visit because the patient didn't say what they really wanted to ask. You don't have long visits where you go around and around and around because the patient isn't able to say what's on their mind. It's really up to us to create that environment where patients can really speak freely, because they're coming on our turf, they're in our offices, they're under our time constraints. And so they're very conscious of how limited our availability is and how much they need to say the things that are going to get them what they think they need from us, which are the best treatments, the best care the right referrals, right? All that stuff. I think most patients are very aware of that and very conscious of how they take up the time when they're in the office. So my number one recommendation is to pay attention to that emotion channel and acknowledge it explicitly. I mean, you can be very a very nice person and very warm, but it turns out that explicit verbal acknowledgment takes the conversation to another level. ASCO Daily News: Absolutely. Well, Tony, we're now living in a COVID-19 world, and we've seen a sharp increase in telehealth-based palliative care during the COVID-19 pandemic. Telehealth based care will likely continue for some time in the future because of the pandemic. Can you tell us a little about telehealth-based palliative care and how the pandemic will impact palliative care in the future? Dr. Anthony Back: Sure. Well, I would say that we've entered an era of telehealth oncology care, right? It's not just the palliative part of the care, it's all-- it's many aspects of oncology care aside from a few infusions or surgery or radiation treatments. But I think COVID has actually accelerated something that was happening very slowly that actually really benefits patients, which is the ability to talk to their doctors and nurses and nutritionists and social workers without having to come into the cancer center or clinic, and I think that access, I think it has the potential to really improve palliative care, because both the clinicians and the patients won't be thinking, ooh, this issue isn't big enough for me to bother to come in; oh, this issue isn't big enough for me to come in and deal with the traffic and find parking and all that sort of stuff. So I actually think that care that addresses quality of life for patients with cancer, that a good deal of it can be done over a good telehealth encounter, and that overall, that will really benefit patients in the short run and the long run. And I think it will actually benefit clinicians, too, because I think they'll be able to feel like they're doing the right thing and it'll-- and it's easy-- in some ways, it's easier. Now it doesn't replace face-to-face encounter, there is still a place for the face-to-face encounter, but there is a lot you can do in telehealth. And so I feel kind of excited about this, actually. I am not that worried about it, except that I want to make sure that some of the policy issues that have made telehealth more accessible now are continued permanently. ASCO Daily News: So let's focus on community-based palliative care for a moment. In your article published in ASCO's Educational Book, you reported an improvement in care quality, patient-centeredness, and cost reduction. Can you tell us more about the benefits of community-based palliative care for patients, caregivers, and clinicians? Dr. Anthony Back: Yeah. Well, now there are a number of studies that show that for patients with advanced cancer, that having concurrent palliative care improves a number of outcomes. It improves symptom outcomes like pain, it improves mood outcomes like depression, and in some circumstances, that even improves survival, and the reason for that is probably that it decreases the amount of chemotherapy at the very end of life that probably shortens your life, it doesn't lengthen it, right? Because if you're really weak, having more chemotherapy may do more harm than good, and I think that's been made very clear by the research that's out there, and it's very high-quality research, and it's more than one study, it's now a whole bunch of studies. And so I think what that means for us as oncologists is figuring out, how do we enable patients to access that kind of care in a way that dovetails with what we do in our oncology practices? Because some of that palliative care will happen in our practice, some of that palliative care may happen outside the practice. And I think there are pros and cons to both things. For patients who often see their primary oncologist as a person-- the captain of the ship, right? The person who's making the big decisions, having that person include palliative care in the treatment plan is like a huge sign to the patient that this is really important, and it enables patients to act and work on those things because they see that the oncologist values them, too. On the other hand, I think there are times when it is valuable for a patient to see a palliative care clinician who isn't their oncologist because I think some patients find it easier to talk to other clinicians about some really sensitive topics, because those patients, they worry about disappointing their oncologist, they worry that their oncologist will feel unwanted or feel like the patient has been disloyal. And of course, I think many of those feelings are much more in the patient's hearts than in the oncologists' hearts, but it turns out to be a little bit of a barrier, and there now is a great deal of on-the-ground clinical experience that shows that having another person to talk to that you come to trust is incredibly valuable in helping enabling somebody to navigate this complicated journey. ASCO Daily News: Right. So let's talk about oncology practices. In its 2017 guideline, ASCO recommended integrating palliative care into standard oncology care for patients with advanced cancer, but it's proving to be quite a challenge to increase primary palliative care. Why is it so difficult? Dr. Anthony Back: Yeah. Well, I think one of the reasons has to do with how oncology care is funded, right? So if oncology care continues to be funded as a kind of a procedure-driven thing, that actually makes it harder to create palliative care capacity within oncology practices, because it's much easier to fund palliative care if a practice is being funded kind of on a value-based scheme rather than on a procedure-based scheme. I also think that there is this sense among oncologists that palliative care clinicians are horning into something that was kind of their domain. Like they were the ones who did everything, they're responsible for everything. And when I was trained as an oncology fellow, that's how I was trained, like I was-- the buck stopped with me and I was responsible for everything. And I came to own that and take pride in that and feel responsible for it. And now this is much more of a team sport where we have to acknowledge and find colleagues that we really enjoy sharing patients with. And so I think building those kinds of relationships among our colleagues who are palliative care clinicians and finding the ways to fund those people so that they're available enough in a practice to make a difference, I think those are some of the barriers we're still working on. We've made a lot of progress on this, and I think the fact that ASCO endorsed this is a huge step in the direction of policy that will make this sustainable and robust. And we still have a ways to go. ASCO Daily News: Well Tony, this has been such a fascinating conversation. Any tips you'd like to share with oncologists before we wrap this up? Anything else you'd like to add? Dr. Anthony Back: Yeah. Well, my big tip is that what this is about is investing a little bit of time upfront for a payoff that comes again and again and again in your work with patients. Because basically, good communication is about building trust, and if there is one thing that you need as an oncologist when it comes to making difficult and tough decisions, you need to have earned that patient's trust, right? It's not going to happen in one conversation. It really is the beauty of the continuity that you get with a career in oncology, and it's an incredibly satisfying way to practice. So that's my last tip. ASCO Daily News: Absolutely. And I'd like to just remind our listeners about the article that you've published along with your co-authors, Dr. Tara Friedman and Dr. Janet Abrahm in the ASCO Educational Book on "Palliative Care Skills and New Resources for Oncology Practices-- Meeting the Palliative Needs of Patients with Cancer and Their Families." Thank you, Tony. Dr. Anthony Back: Thank you. ASCO Daily News: And thank you to our listeners for joining us today. Renowned family enterprise expert Joseph Astrachan reveals why family businesses have the enduring capacity to remain resilient in the face of “war-like” conditions. Listen now! Some family enterprises are older than nations. Very few people have studied them as closely as Cornell University's Joseph Astrachan, who has examined their successes, successions, and failures for more than 30 years. This week we are talking all about sexuality and intimacy after divorce. My guest is Leslie Morgan Steiner, a New York Times best-selling author, columnist for The Washington Post, speaker on work/family balance, successful corporate executive – and domestic violence survivor. Two truths can co-exist. You can be relieved your soon-to-be ex is moving out and grief-stricken at the same time. You can be happy and sad, at the same. This is part of the complexity of human emotions. There’s nothing wrong with you for experiencing a wide range of them. There is nothing wrong with you. Nobody knows this better than Leslie. Leslie’s memoir, Crazy Love, is an account of her violently abusive first marriage. It's about this kind of complexity, how we can deeply love and deeply fear one person at the same time. Her latest memoir, The Naked Truth, explores these dualities as well as she writes about female sexuality, self-esteem, and dating after 50. Show Highlights The power of sisterhood and how women are always an inspiration to other women (3:18) Coming out of a marriage desexualized and losing touch with your sexuality (6:00) Perfection has nothing to do with sexuality (12:00) Getting to a point where you can enjoy and love sex (15:00) The stigma and shame around having sex after divorce if the divorce has not been finalized (22:38) Women who are vulnerable to abusers, how to break the cycle; and Leslie shares her story of domestic abuse (28:14) Why learning how to trust yourself again rather than trusting others is paramount following an abusive relationship (33:23) Leslie drops this truth bomb: “I can’t look for a man to heal me.” (38:00) Men struggle with intimacy just as much as women do (39:00) Leslie shares her thoughts on coaxing a man to fall in love with you (42:00) The myth of Prince Charming is destructive to women and unfair to men too (49:09) Learn More About Leslie: Leslie Morgan Steiner is the author of four nonfiction books: the New York Times bestselling memoir Crazy Love; the critically acclaimed anthology Mommy Wars: Stay-at-Home and Career Moms Face Off on Their Choices, Their Lives, Their Families; The Baby Chase: How Surrogacy is Transforming the American Family; and her latest memoir, The Naked Truth, which explores female sexuality, self-esteem and dating after 50. Behind the Badge with DianeThis episode is the first of the interview series where I talk with friends and colleagues about their career changes. Our occupational lives are so complex and often the pathway takes many turns due to life, opportunities, challenges, or new ways of seeing your own career. This might be out of personal interest and perspective; however, I will have a conversation with a few women who have left one industry for another role. In the conversation with Diane, we talk a bit about what goes on behind the badge. Diane is a campus police officer now; however, she has worked in residence life, studied forensic psychology, and talk English overseas. We discuss everything from volunteering, Zumba, policy academy, and resources you should consider for getting into police enforcement as a women. I am grateful for Diane's honest and willingness to share her successes, challenges, failures, and lessons learned. I learn so much about opportunities to work with the government and other agencies related to law, and Diane reminds us that NCIS or Criminal minds is not the FBI/CIA fully. Much of the career paths and point of entry look different, so we list a few generic ways to learn more about this career path below. I know I am reminded about what life on campus looks like for an officer, the humorous, scary, and typical events do community policing or patrol on a daily basis. In talking about police enforcement, there was some surprising, appalling, and insightful aspects for what women in the academy deal with when entering the field. From this experience, I think listening to this conversation reminds me about how we work with, talk to, and treat one another is so very important beyond work and in society. Also, she reminds us how working in service to the public does take a toll on your physically and emotionally, so we swap resources and ideas.“If you support me, my confidence builds. If you make me feel like I'm literally less than you because I'm female, then I can't grow here.” Diane shares her Police Experiences as a Women. Today I am getting real, deep and into the grit of how mental and physical health can affect you as an entrepreneur and your business. I cover four physical traits that could be affecting your health negatively as an entrepreneur and share with you how not taking care of my physical health all but halted my business in January. I also give you 3 tips to keep you on track and in tip top shape or at least aiming in that general direction. You will love #3, I promise. :D I also take on the subject of mental health and how it can be extremely difficult as an entrepreneur to stay positive, and mentally healthy. In fact, according to a May 2018 by Michael Freeman titled"The Prevalence and Co-Occurrence of Psychiatric Conditions Among Entrepreneurs and Their Families"at least 49% of entrepreneurs are likely to experience a mental-health episode, in their lifetime. Holy cow sisters (and brothers) that is almost half. So we best be taking better care of ourselves. I give you 6 tips to stay mentally sane as an entrepreneur and spell out 6 different symptoms that most of us have all felt or experienced as an entrepreneur. Listen to this week's episode to ensure that you are staying in tip top mental and physical health so that your business can thrive and you can change this world! Hugs! He started playing football at a young age as well as other sports but his favourite sport growing up was actually hockey. He developed into very good goalie in hockey which helped him later on as a linebacker in football. He was not heavily recruited out of highschool so went to a small College, Augustana University, for 2 years. After putting up stellar numbers, he though a full scholarship would follow. However the school backed out, knowing his dad was a doctor and could afford to pay for his schooling. They pulled his scholarship thinking he would stay. Instead, he transfered to the University of Minnesota and walked on the team there. He had some legendary coaches on the staff there like Tony Dungy, Mike Martz and Mike Shanahan. His numbers for his Senior season were not phenomenal, however he showed enough to be selected by Denver in the 12th round. The coach, Dan Reeves, was a player that was viewed as an underdog so he looked for players like that who he knew could develop into a great player. Karl was a player that fit that mold. He was a dominant player, and the team used his versatility by playing him in multiple positions to take advantage of the weak spots on the offence. His physical play took a toll on his body having several surgeries during his career. Even though he went to school to follow in his dads footsteps and become a doctor, his path led him to a career in speaking. He does talks around North America using the lessons he learned playing football. He’s part of a mastermind group of speakers that help each other refine their keynotes and become better speakers. He also wrote a book, Heart of an Student Athlete: All-Pro Advice For Competitors an Their Families. Check out Karl’s website and social media links below for more information. "Why Physicians Die by Suicide: Lessons Learned from Their Families and Others Who Cared" Physicians are known to be a group of professionals who are at risk of taking their own lives. In this easy-to-read book, Dr. Michael Myers, a psychiatrist and specialist in physician health, attempts to explain the mystery of why some doctors, despite their calling and the adoration of their families, patients, students and colleagues, perish by suicide. In his easy-to-read book, he combines the powerful and gripping insights of dozens of bereaved people whom he interviewed for this project with disguised stories from his decades long clinical practice to shed some light on this national tragedy. The stigma attached to mental illness in doctors is ubiquitous and pernicious – and, because untreated illness is one of the major drivers to suicide, Dr. Myers argues that stigma must be fought with urgency and might. He makes across-the-board recommendations in an effort to prevent suicide in physicians and concludes that everyone has a role to play in saving a doctor’s life. The book is about heartbreak, loss, prevailing, growth, passion and hope. It’s a book for doctors themselves, their families, those who train them, those who treat them and those who care about them. www.buildgrowandenjoy.com

Today we have an all volunteer military. When young men and woman join the Marine Corps, they do so to serve their country. They know full well they the work may put them in harms way. That a bullet, or a grenade, or an IED might cut their life short, at any time. What none of them expect is to be killed or maimed by drinking water.Yet that is exactly what has happened to Marines at the historic Camp Lejeune facility in North Carolina. As many as a million people connected with the base were exposed to highly toxic and contaminated well water. What the military and government has done about it, is truly a crime.Long time journalist Mike Magner takes us into the human tragedy of the government and Pentagon inaction in A Trust Betrayed: The Untold Story of Camp Lejeune and the Poisoning of Generations of Marines and Their Families. My conversation with Mike Magner:

Tonight we discussed veterans healthcare with a specific focus on traumatic brain injuries. We spent an hour with Bruce McQuain from QandO.net and Annette, a dedicated wife and care giver for Eric, who was severely wounded in Iraq. Bruce spent 28 years in the Army and provided some perspective on the challenges the VA is facing, while Annette told us about the consequences. The unfortunate thing is that her story is not unusual. It tracks perfectly with this editorial from Lawrence Downes of the New York Times, For Wounded Veterans and Their Families, a Journey Without Maps "At a recent hearing in Washington before the Senate Veterans’ Affairs Committee, the men’s families told anguished tales of trips through bureaucratic hell in the transition between the Defense Department and the Department of Veterans Affairs. That terrain is notorious for its paperwork mountains and tripwires of red tape, but especially treacherous for those with traumatic brain injuries, the signature affliction of this five-year-old war. Mr. Bunce lost an eye in a roadside bomb blast that also thrust shrapnel into his frontal lobe. His father, Peter, said his care was so “stovepiped,” with nobody knowing what anyone else was doing, that doctors working on his head ignored his broken leg. Technicians nearly did an M.R.I. on his brain, not realizing — because scans had not been done — the danger from the metal in his skull. Nobody tried to coordinate his many medications. Time and again his parents had to cross the country looking for the right therapies and treatment. Whatever expertise they found they stumbled on; there was no one but them to manage his case. The V.A. relied on the brain-damaged young corporal to evaluate his own mental state, and once sent him a letter threatening to cut off benefits because he could not manage his affairs." Defense Secretary Robert Gates, who has been tremendous IMHO, gets it and is making changes, "FORT BLISS, Texas - U.S. Defense Secretary Robert Gates on Thursday said the military had made mistakes in treating returning combat troops including in their physical and mental health care and by providing some sub-standard housing. In a visit to Fort Bliss, Texas, Gates announced a change in government procedures to encourage troops returning from Iraq and Afghanistan to seek treatment for post-traumatic stress disorder (PTSD) without fear of losing their security clearances and harming their careers. The announcement came just a day after closing arguments in a San Francisco federal court case in which veterans allege the U.S. Department of Veterans Affairs is unable to deal with the growing number of PTSD cases emerging from the Iraq and Afghanistan wars." As Bruce mentioned, PTSD and brain injuries are very different things. That said, neither situation is being adequately addressed by the VA, for a variety of reasons. Annette was a very compelling guest, helping us understand the frustrations and consequences of dealing with an overwhelmed system. She is a relentless advocate for her husband Eric. We admire her tenacity to get the best possible care for him. After listening to her describe the challenges, everyone, liberal and conservative, should be saying WE HAVE TO DO BETTER THAN THIS.
