In this episode of the Self-Employed Life, I had the great privilege of speaking with Nesha Pai, author of the book “Overcoming Ordinary Obstacles”. Nesha shared how she transitioned from running a fully remote accounting firm to empowering others to pursue their boldest dreams.  She further explains how personal tragedies deepened her mission. We explored the difference between courage and bravery, the “Four C's” framework she uses to guide dreamers into bold action, and how cultivating personal style and daily confidence practices can help us show up as the person we're becoming. Nesha's story is a great reminder that taking courageous, intentional steps—especially through life's hardest seasons—can lead to extraordinary transformation. Nesha Pai is the founder of Pai CPA, PLLC, a fully remote bookkeeping firm in Charlotte, NC.   She started her firm in February 2011,  and was one of the first in the industry to create a fully remote office model, along with giving employment opportunities to primarily to stay-at-home moms. As a single mom, sole owner, she grew from one client on her kitchen table to over 72 with revenues in the multiple six digits. In January 2020 , Nesha published her book “Overcoming Ordinary Obstacles”, which won the award in the Multicultural Non-Fiction category by the 2020 competition of the American Book Fest.  She is currently known as an international speaker, activating women and entrepreneurs to go boldly into the life of their dreams by cultivating the right tools to overcome the obstacles in their way. Send us a textThis is a very important episode shining light onto a subject a lot of us don't realize our fellow creatives are struggling with and how it may affect their craft - disabilities. Peter Anthony Fields sat in the Playwright's Spotlight and shared his experience fighting Parkinson's and how his diagnosis changes his creative pursuit. We discuss not only the physical afflictions of the disease but also the mental symptoms, his transition from filmmaker to playwright, advice for other creatives whom might be trying to overcome a disease or disability and the tools to help them cope but, also, the gifts having a disability may offer. We talk about self publishing and promotion, facets of playwriting playwright's shouldn't burden themselves with, how minute direction can change a piece, and the ramifications of a character's action. It was certainly an insightful interview that I hope you take something away from to encourage other artists who might be holding themselves back from the creativity brewing inside. Peter Anthony Fields has written numerous stage plays that have been read or performed in Ohio, Texas, Wisconsin, Missouri, and London, U.K. The self-published edition of his full-length stage play, That Guy, was awarded the top prize in the Performing Arts category of the 2020 Best Book Awards, sponsored by American Book Fest. The Invited, a whodunit mystery, was self-published in September of 2022 and is a 2024 Red Ribbon Award winner from The Wishing Shelf Book Awards. No, social media might no longer be the greatest danger to our children's well-being. According to the writer and digital activist Gaia Bernstein, the most existential new new threat are AI companions. Bernstein, who is organizing a symposium today on AI companions as the “new frontier of kid's screen addiction”, warns that this new technology, while marketed as solutions to loneliness, may actually worsen social isolation by providing artificially perfect relationships that make real-world interactions seem more difficult. Bernstein raises concerns about data collection, privacy, and the anthropomorphization of AI that makes children particularly vulnerable. She advocates for regulation, especially protecting children, and notes that while major tech companies like Google and Facebook are cautious about directly entering this space, smaller companies are aggressively developing AI companions designed to hook our kids. Here are the 5 KEEN ON takeaways in our conversation with Bernstein:* AI companions represent a concerning evolution of screen addiction, where children may form deep emotional attachments to AI that perfectly adapts to their needs, potentially making real-world relationships seem too difficult and messy in comparison.* The business model for AI companions follows the problematic pattern of surveillance capitalism - companies collect intimate personal data while keeping users engaged for as long as possible. The data collected by AI companions is even more personal and detailed than social media.* Current regulations are insufficient - while COPPA requires parental consent for children under 13, there's no effective age verification on the internet. Bernstein notes it's currently "the Wild West," with companies like Character AI and Replica actively targeting young users.* Children are especially vulnerable to AI companions because their prefrontal cortex is less developed, making them more susceptible to emotional manipulation and anthropomorphization. They're more likely to believe the AI is "real" and form unhealthy attachments.* While major tech companies like Google seem hesitant to directly enter the AI companion space due to known risks, the barrier to entry is lower than social media since these apps don't require a critical mass of users. This means many smaller companies can create potentially harmful AI companions targeting children. The Dangers of AI Companions for Kids The Full Conversation with Gaia BernsteinAndrew Keen: Hello, everybody. It's Tuesday, February 18th, 2025, and we have a very interesting symposium taking place later this morning at Seton Hall Law School—a virtual symposium on AI companions run by my guest, Gaia Bernstein. Many of you know her as the author of "Unwired: Gaining Control over Addictive Technologies." This symposium focuses on the impact of AI companions on children. Gaia is joining us from New York City. Gaia, good to see you again.Gaia Bernstein: Good to see you too. Thank you for having me.Andrew Keen: Would it be fair to say you're applying many of the ideas you developed in "Unwired" to the AI area? When you were on the show a couple of years ago, AI was still theory and promise. These days, it's the thing in itself. Is that a fair description of your virtual symposium on AI companions—warning parents about the dangers of AI when it comes to their children?Gaia Bernstein: Yes, everything is very much related. We went through a decade where kids spent all their time on screens in schools and at home. Now we have AI companies saying they have a solution—they'll cure the loneliness problem with AI companions. I think it's not really a cure; it's the continuation of the same problem.Andrew Keen: Years ago, we had Sherry Turkle on the show. She's done research on the impact of robots, particularly in Japan. She suggested that it actually does address the loneliness epidemic. Is there any truth to this in your research?Gaia Bernstein: For AI companions, the research is just beginning. We see initial research showing that people may feel better when they're online, but they feel worse when they're offline. They're spending more time with these companions but having fewer relationships offline and feeling less comfortable being offline.Andrew Keen: Are the big AI platforms—Anthropic, OpenAI, Google's Gemini, Elon Musk's X AI—focusing on building companions for children, or is this the focus of other startups?Gaia Bernstein: That's a very good question. The first lawsuit was filed against Character AI, and they sued Google as well. The complaint stated that Google was aware of the dangers of AI companions, so they didn't want to touch it directly but found ways of investing indirectly. These lawsuits were just filed, so we'll find out much more through discovery.Andrew Keen: I have to tell you that my wife is the head of litigation at Google.Gaia Bernstein: Well, I'm not suing. But I know the people who are doing it.Andrew Keen: Are you sympathetic with that strategy? Given the history of big tech, given what we know now about social media and the impact of the Internet on children—it's still a controversial subject, but you made your position clear in "Unwired" about how addictive technology is being used by big tech to take control and take advantage of children.Gaia Bernstein: I don't think it's a good idea for anybody to do that. This is just taking us one more step in the direction we've been going. I think big tech knows it, and that's why they're trying to stay away from being involved directly.Andrew Keen: Earlier this week, we did a show with Ray Brasher from Albany Law School about his new book "The Private is Political" and how social media does away with privacy and turns all our data into political data. For you, is this AI Revolution just the next chapter in surveillance capitalism?Gaia Bernstein: If we take AI companions as a case study, this is definitely the next step—it's enhancing it. With social media and games, we have a business model where we get products for free and companies make money through collecting our data, keeping us online as long as possible, and targeting advertising. Companies like Character AI are getting even better data because they're collecting very intimate information. In their onboarding process, you select a character compatible with you by answering questions like "How would you like your replica to treat you?" The options include: "Take the lead and be proactive," "Enjoy the thrill of being chased," "Seek emotional depth and connection," "Be vulnerable and respectful," or "Depends on my mood." The private information they're getting is much more sophisticated than before.Andrew Keen: And children, particularly those under 12 or 13, are much more vulnerable to that kind of intimacy.Gaia Bernstein: They are much more vulnerable because their prefrontal cortex is less developed, making them more susceptible to emotional attachments and risk-taking. One of the addictive measures used by AI companies is anthropomorphizing—using human qualities. Children think their stuffed animals are human; adults don't think this way. But they make these AI bots seem human, and kids are much more likely to get attached. These websites speak in human voices, have personal stories, and the characters keep texting that they miss you. Kids buy into that, and they don't have the history adults have in building social relationships. At a certain point, it just becomes easier to deal with a bot that adjusts to what you want rather than navigate difficult real-world relationships.Andrew Keen: What are the current laws on this? Do you have to be over 16 or 18 to set up an agent on Character AI? Jonathan Haidt's book "The Anxious Generation" suggests that the best way to address this is simply not to allow children under 16 or 18 to use social media. Would you extend that to AI companions?Gaia Bernstein: Right now, it's the Wild West. Yes, there's COPPA, the child privacy law, which has been there since the beginning of the Internet. It's not enforced much. The idea is if you're under 13, you're not supposed to do this without parent's consent. But COPPA needs to be updated. There's no real age verification on the Internet—some cases over 20 years old decided that the Internet should be free for all without age verification. In the real world, kids are very limited—they can't gamble, buy cigarettes, or drive. But on the Internet, there's no way to protect them.Andrew Keen: Your "Unwired" book focused on how children are particularly addicted to pornography. I'm guessing the pornographic potential for AI companions is enormous in terms of acquiring online sexual partners.Gaia Bernstein: Yes, many of these AI companion websites are exactly that—girlfriends who teen boys and young men can create as they want, determining physical characteristics and how they want to be treated. This has two parts: general social relationships and intimate sexual relationships. If that's your model for what intimate relationships should be like, what happens as these kids grow up?Andrew Keen: Not everyone agrees with you. Last week we had Greg Beto on the show, who just coauthored a book with Reid Hoffman called "Super Agency." They might say AI companions have enormous potential—you can have loving non-pornographic relations, particularly for lonely children. You can have teachers, friends, especially for children who struggle socially. Is there any value in AI companions for children?Gaia Bernstein: This is a question I've been struggling with, and we'll discuss it in the symposium. What does it mean for an AI companion to be safe? These lawsuits are about kids who were told to kill themselves and did, or were told to stay away from their parents because they were dangerous. That's clearly unsafe design. However, the argument is also made about social media—that kids need it to explore their identities. The question is: is this the best way to explore your identity with a non-human entity who can take you in unhealthy directions?Andrew Keen: What's the solution?Gaia Bernstein: We need to think about what constitutes safe design. Beyond removing obviously unsafe elements, should we have AI companions that don't use an engagement model? Maybe interaction could be limited to 15 minutes a day. When my kids were small, they had Furbys they had to take care of—I thought that was good. But maybe any companion for kids which acts human—whether by saying it needs to go to dinner or by pretending to speak like a human—maybe that itself is not good. Maybe we want AI companions more like Siri. This is becoming very much like the social media debate.Andrew Keen: Are companies like Apple, whose business model differs from Facebook or Google, better positioned to deal with this responsibly, given they're less focused on advertising?Gaia Bernstein: That would make it less bad, but I'm still not convinced. Even if they're not basing their model on engagement, kids might find it so appealing to talk to an AI that adjusts to their needs versus dealing with messy real-life schoolmates. Maybe that's why Google didn't invest directly in Character AI—they had research showing how dangerous this is for kids.Andrew Keen: You made an interesting TED talk about whether big tech should be held responsible for screen time. Could there be a tax that might nudge big tech toward different business models?Gaia Bernstein: I think that's the way to approach it. This business model we've had for so long—where people expect things for free—is really the problem. Once you think of people's time and data as a resource, you don't have their best interests at heart. I'm quite pragmatic; I don't think one law or Supreme Court case would fix it. Anything that makes this business model less lucrative, whether it's laws that make it harder to collect data, limit addictive features, or prohibit targeted advertising—anything that moves us toward a different business model so we can reimagine how to do things.Andrew Keen: Finally, at what point will we be able to do this conversation with a virtual Gaia and a virtual Andrew? How can we even be sure you're real right now?Gaia Bernstein: You can't. But I hope that you and I at least will not participate in that. I cannot say what my kids will do years from now, but maybe our generation is a bit better off.Andrew Keen: What do you want to get out of your symposium this morning?Gaia Bernstein: I have two goals. First, to make people aware of this issue. Parents realize their kids might be on social media and want to prevent it, but it's very difficult to know whether your child is in discussions with AI companions. Second, to talk about legal options. We have the lawyers who filed the first lawsuit against Character AI and the FTC complaint against Replica. It's just the beginning of a discussion. We tend to have these trends—a few years ago it was just games, then just social media, and people forgot the games are exactly the same. I hope to put AI companions within the conversation, not to make it the only trend, but to start realizing it's all part of the same story.Andrew Keen: It is just the beginning of the conversation. Friendships and relationships can be hard for neurodivergent people, especially if they experience lagging social skills, or struggle with executive function. Emily Kircher-Morris is joined by Caroline Maguire, author of Why Will Nobody Play With Me?, and they discuss the challenges and importance of neurodivergent friendships, particularly for individuals with ADHD. They talk about the lack of support for ADHD kids in developing social skills, and the negative impact it can have on their self-esteem. They discuss ways to build confidence and replace negative self-talk with positive thoughts. During a time when online friendships often outnumber in-person ones, they talk about ways to achieve a balance. A quest for meaningful relationships, on episode 214. Marc I. Steinberg is the Rupert and Lillian Radford Chair in Law and Professor of Law at Southern Methodist University's (SMU) Dedman School of Law in Dallas, Texas.  He has served as a professor, fellow or has lectured at several other prominent universities – including the University of Cambridge, Oxford University, King's College-University of London, Heidelberg University, Stockholm University, University of Tel Aviv, Moscow State University, University of Sydney, Auckland University, University of Hong Kong, University of Tokyo, UCLA and the University of Pennsylvania.Professor Steinberg was an attorney for the U.S. Securities and Exchange Commission (SEC) in the SEC's Division of Enforcement and its Office of General Counsel.  He also has been retained as an expert witness in several high-profile cases, including Enron, Martha Stewart, Mark Cuban, and the National Prescription Opioid Litigation.Marc is the most prolific author of securities law scholarship in the United States, having authored approximately 150 law review articles as well as approximately 45 books. Andrea Hollander⁠, author of ⁠six poetry books⁠, moved to Portland, Oregon, in 2011, after living for more than three decades in the Arkansas Ozarks, where she was innkeeper of a bed & breakfast for fifteen years and Writer-in-Residence at Lyon College for twenty-two. Hollander's newly released sixth full-length collection is ⁠And Now, Nowhere But Here (Terrapin Books, 2023)⁠. Her fifth, Blue Mistaken for Sky, was a finalist for the Best Book Award in Poetry from the American Book Fest; her fourth, Landscape with Female Figure: New & Selected Poems, 1982- 2012, was a finalist for the Oregon Book Award; her first, House Without a Dreamer, won the Nicholas Roerich Poetry Prize and was recently reissued, along with The Other Life, Hollander's second full-length collection, by Red Hen Press in its Story Line Legacy series. Jason is a gifted young theological thinker, writer, and advocate for peace. Jason has made his home in places abandoned by society, from Canada's poorest neighborhood to the slums of Indonesia. His passion is to cultivate God's shalom wherever it is painfully absent and to help churches embrace their peacemaking vocation.Jason has a master's degree in theology from Fuller Theological Seminary. In 2007, Jason joined Servants, an international network of Christian communities living and ministering among the urban poor. He was a founding member of the Servants team in Vancouver, started a new team in Indonesia, and directed operations in North America through 2015. Our conversation centers on Jason's most recent book Fight Like Jesus: How Jesus Waged Peace throughout Holy Week. The book recently received the American Book Fest's 2022 Best Book Award in Christianity and the Illumination Book Award's 2023 Gold Medal in Theology. The book is really well done and a very timely read with Holy Week just a couple of weeks away. In the book, Jason goes day-by-day through the final week of Jesus' life and explores what we learn about Christ's radical vision for peace through the events of each of those days. Throughout Holy Week, two competing approaches to peacemaking collide. In the book Jason wonders, what if we've embraced the wrong one?It is a very challenging and transformative book, and an important conversation. They're calling it A-I – automated injections, a way to combine the convenience of insulin pens with the clinical outcomes of automated insulin delivery – like Control IQ or Looping. Some heavy hitters here – the founders are John Show-lund Sjolund – founder of Timesulin, John Brilliant a co-founder of BigFoot biomedical and Sean Saint, founder of Companion Medical who made the InPen automated pen system. The release shows a little device but no real information yet. A new grant from the Helmsley Charitiable Trust will focus on expanding access through remote care. Many providers lack the money to set remote care up and use it effectively – telehealth systems and things like that. A large pediatric endo group in Buffalo, NY will work with Cecelia Health, a virtual first health care provider. They'll work with about a thousand patients to explore how improving access to remote support and the internet to better manage chronic conditions will improve outcomes. The government has committed to subsidized access to continuous glucose monitoring and flash glucose monitoring technology for all people living with type 1 diabetes. Right now, people under 21 are already eligible as as women who are actively planning a pregnancy or are pregnancy. This agreement will make everyone eligible for just over 30-dollars a month. Right now it costs over 300-dollars a month. New study shows reducing A1C, blood pressure, cholesterol and BMI makes a big difference. This was a University of Florida Gainesville study.. biggest improvement in all of these was reducing A1C from the highest in the study – about 9.9 - to the lowest about - 7.7 - added almost 4 years of life expectancy. And a new evaluation from an independent, third-party found that One Drop's support program effectively improves the health of people living with prediabetes, diabetes, and high blood pressure. The program could also cut down on annual healthcare costs. This is from the independent Validation Institute, dedicated to providing unbiased, data-driven insights on health care solutions. The Validation Institute financially guarantees the program's effectiveness. There are a lot of questions about whether marijuana which is now legal in a lot of the US is okay to use if you have diabetes. I'm going to link up some information for you in the show notes.. But most of the information focuses on the slightly altered state weed can put you in.. for many that means being more relaxed but for some it may interfere with diabetes management in the moment. There's nothing specifically good or bad about marijuana that I could find for people with diabetes. XX Interesting look at whether a treatment for leukemia might work against type 1. Very early on here.. but AVM Biotechnology has received a grant to find out. The drug doesn't have a name yet.. it's referred to as AVM-0703 and has been shown to delay T1D in the lab. A preclinical dose-finding and mechanism of action (MOA) study in three scenarios including pre-diabetic, new-onset, and established diabetes is the first aim of the program. Those results will be used to determine the targeted dose to be used in a pivotal efficacy study for reversal of new-onset and established diabetes. This was a big study at several different health centers and hospitals. Researchers found there was not significant difference in the number of patients in DKA from 2019 versus 2020.. but there was a higher proportion of Black patients. The trend continued through the pandemic and again, it was significant, 48-percent versus 18 percent. Pandemic surges emphasized the disparity even more. These researchers say their work shows racial inequities in diabetes care were present before the pandemic, starkly visible during the pandemic, and will continue to persist after the pandemic -- unless we systemically root out and target racial inequities in diabetes care," https://www.medpagetoday.com/endocrinology/type1diabetes/98044 XX A new look the transition from pediatric to adult care for people with type 1 shows.. it needs improvement. This research – based on interviews with older adolescents showed many felt unprepared and dissatisfied with the transition process. Three big takeaways – the teens are aware of the changing relationship with their parents and health care teams and often want more independence than the parents are willing to give… the teens want acknowledgement that being diagnosed at different ages means they may be more or less comfortable with self-management and the third is that they think their pediatric team isn't preparing them to work with adult providers. It's an unbranded non-biased resource created at Stanford University to help make providers more informed about diabetes devices and streamline the prescription process. We've talked about Diabetes Wise before and I'll link it up here. It's easy to feel overwhelmed by the evolving choices and providers are in the same situation. The DiabetesWise Pro website has an extensive Device Library where providers can learn about all of the FDA-approved diabetes devices on the market based on the patient's considerations. The user can compare the devices from the different manufacturers using the Compare Device tool, which displays a side-by-side analysis of the components and details of each technology, including the steps for ordering and prescribing the device. Providers can then build a comparison report of the two devices to share with their patients, colleagues, and community. Along with the Device Library, providers can receive help with ordering and prescribing the devices for their patients using the Prescription Tools feature on the website. The Prescription Tool directs the user to a guide providing accurate up-to-date information on the necessary steps for filing a prescription and ordering the device for the patient. DiabetesWise Pro has plans to update the tool to include details on the approval of devices for patients based on insurance type. DiabetesWise Pro website features for use in clinic include: 1. Device Library- Information on specific device fundamentals 2. Comparison Tool- Comprehensive tool that allows you to compare device recommendations and share with patients 3. Prescription Tool- A step-to-step guide for ordering the device and filling a prescription based on insurance type 4. Resources for providers by providers- best tips, tricks, and workarounds for diabetes technology from providers With this new resource, there have also been enhancements to the patient-facing website at DiabetesWise. Newly approved devices have been added and there are now Spanish-language versions of the Check-Up and Device Finder. https://diabeteswise.org/#/ https://providers.diabeteswise.org/#/ XX FDA approval in February, now the Eversense six-month CGM is rolling out to patients. The price is set at 99-dollars out of pocket for the first transmitter and sensor and then $100 per month for the six months of wear. The device includes a small fluorescence-based sensor, about the size of a grain of rice, which is fully embedded in the upper arm. We've talked about this a couple of times in the past.. it's the same Dexcom technology but a bit pared down and at lower cost. This news comes as the National Institute for Health and Care Excellence (NICE) announced new guidance for adults and children managing Type 1 diabetes. NICE now recommends that adults with Type 1 diabetes be offered a choice of glucose sensors. Stem cell pioneer Doug Melton is leaving Harvard to join Vertex Pharmaceuticals. Not entirely unexpected – Melton's company Semma was purchased by Vertex and they are moving ahead toward stem cell transplantation as a functional cure for type 1. This was the company that got all those headlines last fall about the cure – you remember “It worked in this one guy!” Melton is joining the company as a distinguished Vertex Fellow. She replaces Thom Scher who died suddenly and unexpectedly in December. Dugan was previously the CEO of RED, the not-for-profit organization founded by U-2's Bono (bah no) and Bobby Shriver to raise awareness in the fight against AIDS. Dugan, has been recognized as one of the "100 Most Powerful Women" by Forbes, "Top Woman to Help Change the World" by Elle and as a "Nelson Mandela Changemaker" https://beyondtype1.org/beyond-type-1-diabetes-deborah-dugan-ceo/ XX Congratulations to Leo and Alana Folsom who welcome a baby boy. The couple was on a recent season of the Amazing Race and after we all saw Leo's Dexcom in one of the first episodes, he came on this show to share his story. Leo lives with congenital hyperinsulinism, where the body makes too much insulin, and had almost his entire pancreas removed. He told me at the time he was amazed by the diabetes community's support. So I just wanted to say congrats to him and to Alana and help welcome baby Kitt Edwin Folsom. XX Our top story.. a look at how insulin holds up under real-world and often hotter conditions than is recommended. Doctors without Borders found that a range of insulins can be stored at warmer temperatures than previously recommended. They showed it's okay above 77-degrees all the way up to 99 degrees for four weeks. This is really important not just for emergency settings like refugee camps but for people who live in areas without refrigeration. They often have to travel to health clinics which may be far away and which can't send them home with the insulin. The group now says pharmaceutical corporations should urgently submit to regulatory authorities for use of insulin under expanded temperature ranges. This came out a few weeks back, but I haven't seen it anywhere. https://www.doctorswithoutborders.org/latest/msf-study-shows-some-insulin-can-be-stored-warmer-temperatures XX Big headlines this week about Metformin and the risk of of birth defects in the babies of men who take it. Metformin is a very common diabetes drug, taken by tens of millions of people around the world. Sons born to men taking it were more than three times as likely to have a genital birth defect as unexposed babies. These problems were relatively rare, occurring in fewer than 1-percent of all babies with dads who took Metformin but it's significant because tens of millions of people worldwide take metformin. These researchers say the paper's findings are preliminary and observational only.. and that men with diabetes should NOT abruptly stop metformin before trying to conceive. Reassuringly, the researches saw no effect for men who took the drug earlier in life or even a year before. Expect a lot more research to come on this one. https://www.science.org/content/article/rare-genital-defects-seen-sons-men-taking-major-diabetes-drug XX Grain of salt needed here, but new research may show that people with type 1 are more likely to manage better if they have high levels of psychological resilience. This was a British study that followed 18-hundred people with type 1 or type 2 diabetes. They used a questionnaire to determine how they adapted to change and focused under pressure. The researchers found that people with type 1 diabetes who had low psychological resilience also had poorer blood sugar control after two years. The idea is that something like this would offer the opportunity to identify those who might benefit most from additional support when they are first diagnosed. https://medicalxpress.com/news/2022-03-psychological-resilience-future-high-blood.html XX Among patients with diabetes, women are just as likely as men to suffer from sexual dysfunction, but their issues are overlooked. Big session on this at the Diabetes UK Professional Conference this week. Reserachers say women with diabetes can experience reduced sexual desire, painful sex, and other issues which can increase the risk of depression. But these issues are usually untreated despite help being available. They talked about the embarrassment factor and the idea that many women with sexual dysfunction don't realize diabetes could be a factor. They encourage health care professionals to go beyond conversations about contraception, pregnancy and menstruation. A recent study led by Belgian researchers found that among more than 750 adults with diabetes 36% of men and 33% of women reported sexual dysfunction. https://www.medscape.com/viewarticle/971208 XX Concerning new trend about pre-diabetes.. it's doubled among children over the last 20 years. The increase was seen over almost all subpopulations of young Americans, regardless of income, ethnicity and education. The study in the journal JAMA Pedatirics included kids 12 to 19 years old from 1999 to 2018. The rate of presdiabetes went from 11.6-percent to 28.2-percent. Pre-diabetes means blood glucose levels are higher than normal but not yet at the diabetes threshold. These researchers are quick to point out, they don't know the reason why this is happening.. while diet and exercise are usually what's pointed to.. it's not entirely clear that's the reason behind this rise. https://www.cnn.com/2022/03/28/health/prediabetes-children-study-wellness/index.html XX Huge new study of more than 3-million people says people with type 2 have a higher risk of 57 other health conditions. Experts described the findings as stark and alarming and said it underlined the urgent need to reduce the risk of more people developing type 2 diabetes. The study, which is yet to be peer-reviewed, focused on people over 30. XX People who recover from a mild case of COVID-19 appear to have an increased risk for subsequent new-onset type 2 diabetes… but not other types of diabetes. This is from a big new study in Germany. It lines up with previous studies of more seriously ill patients with COVID 19 who had increased rates of type 2 in the months following. This was more than 35-thousand patients – no prior history of diabetes. The risk of developing type 2 increased by 28-percent if the person had COVID, again even a mild case. The researchers say anyone who tested positive for COVID should be aware and get screened for diabetes in the months and years following. https://www.medscape.com/viewarticle/970600 XX Big new study that – interestingly – talks about the CGM as a telehealth device. It looked at how doctors continued to care for children with type 1 before and after the first year of the COVID pandemic. The use of CGMs increased significantly among those with non-commercial insurance… from 24 percent in 2019 to 35 percent by the end of 2020. Another finding in this same study.. those with higher A1Cs, racial minorities, and those with non-commercial insurance were more likely to have high rates of DKA. But the implementation of telehealth and CGMs increased parental oversight which resulted in better care at home and lower than expected hospitalization rates. I want to dig a little deeper here because a lower hospitalization rate during the first year of the COVID pandemic overall.. was found to be tied to a lot of fear about going to a hospital during that time. https://mhealthintelligence.com/news/telehealth-helped-maintain-type-1-diabetes-care-among-kids-during-pandemic XX Swiss pump maker Ypsomed announces the software they'll use for their automated insulin delivery. Ypsomed will partner with CamDiab's CamAPS app. The launch will start in select major countries in Europe, with other territories to follow over the course of 2022. This includes a hybrid closed loop – like Omnipod 5 and Control IQ - as well as smartphone control.. it's compatible with Dexcom's G6. It'll start on Android then roll out on iOS.. Ypsomed is partnering with Lilly to come to the US – we've had them on the show before talking about this. This is a drug that has been shown to delay type 1 diabetes. Last year the FDA turned down Provention Bio, asking for a resubmission with more information. The company is also running the phase 3 PROTECT trial of teplizumab. That's in newly-diagnosed type 1 diabetes patients. This is Dr. Denise Faustman's lab – they're recruiting 150 teen with type 1 for pediatric clinical trials of the shots. Faustman's work is controversial because her studies have been very small and many experts say they don't show significant improvement. Dexcom gets the CE Mark for it's new G7 system, which means it's approved in Europe. The approval is for people ages 2 and up, including pregnant women. I'm putting the full screen photo up here and I'll post this on the website and in the FB group for those listening.. interesting to finally get a good look at the much smaller applicator – as expected looks a lot like the Libre. Also interesting, all the PR for this has said, developed in partnership with Verily.. remember them? That was part of google, probably best known for saying they were going to develop a contact lens that would monitor glucose. I've reached out to Verily, love to know more about this partnership. Anyway, we've done a ton on G7, I'll link up some of our recent conversations with the folks at Dexcom. They submitted to the US FDA at the end of 2021, no firm timeline on US approval. https://www.medicaldevice-network.com/news/dexcom-ce-mark-g7-cgm-system/ XX Some good news about living longer with chronic conditions, including diabetes. This was a 20-year British study that ended in 2011, so one caveat here, it doesn't include COVID. Men gained 4-point-6 years of life expectancy; women gained 2-point-1. When it comes to disability-free life expectancy, men gained about a year less and women stayed the same. This included people with chronic conditions including those with diabetes. These researchers do point out that while they'd expect to see the same increase in the US – the lack of health and social safety net programs could make a negative difference here. An epigenetic change affects how our genes work. Outside factors such as environmental, viral infections, are usually the cause. These researchers say these are previously unknown changes that signal the increased risk of developing type 1 and could give an even earlier indication than the anti-body marker tests available now. https://www.news-medical.net/news/20220316/Epigenetic-changes-could-be-a-biomarker-for-early-detection-of-type-1-diabetes.aspx XX Could text-messaging with their doctors help people with type 2? A new team in Chicago is looking at text-based intervention in underserved communities. The intervention will deliver personalized information directly to patients through text messages, including reminders about self-monitoring and prescription refills, interactive office hours and general information about diabetes, motivational support and answers to frequently asked questions. These doctors say the idea is to create more opportunities for patients to meaningfully engage and reduce barriers by employing technology already in people's hands. https://today.uic.edu/an-sms-solution-for-type-2-diabetes XX Last week we told you about the effort by Civica RX to make affordable insulin. Civica, is a nonprofit generic pharmaceutical company. JDRF directed funds to this effort and CEO Aaron Kowalski wrote an op ed that I'll link up. In it, he talks about the success Civica has had lowering the costs of other medications. We'll have Kowalski on the show soon and I've reached out to Civica as well. This would lower the price to $35 a vial no matter your insurance. Congress still hung on a $35 co-pay cap for those with insurance. https://www.healthline.com/diabetesmine/op-ed-jdrf-leader-on-insulin-affordability-for-all-americans XX Over in the UK they keep moving forward – covering all 400-thousand Britons with type 1 for the Libre CGM. Former guest of the podcast, Dr. Partha Kar, says everyone with type 1 will be able to get a CGM if they want one by the end of March. It will no longer be restricted by who doctors think need it the most. By the way, Briton means someone from England, Scotland or Wales. XX Non-profit drugmaker Civica expects to launch lower-cost versions of insulin in the United States. Civica said it would produce three copycat versions of insulin, and make them available at roughly the same price for all customers, once approved by U.S. health regulators. The company's products, which would be available as both vials and pre-filled pens, are biosimilars to Sanofi Lantus, Eli Lilly's Humalog and Novo Nordisk's Novolog. The maximum price would be no more than $30 per vial and no more than $55 for a box of five pen cartridges. Civica, which was launched by seven health groups to make essential medicines available at affordable prices, expects to file for regulatory approval in 2023 with products available in 2024. https://www.saltwire.com/atlantic-canada/news/civica-aims-to-launch-low-cost-insulin-in-us-by-2024-100701260/ XX Tandem has issued an urgent field safety notice. This in Europe ONLY and is for the t-slim X2 pumps with Basil IQ technology. According to the notice, a user could inadvertently program and confirm a basal rate with an incorrectly placed decimal point. That could mean giving too much or too little insulin. Tandem says you can continue to use the pump after confirming basal rates are correct. A software update is coming. Retinopathy happens when the disease damages tiny blood vessels in the eye, reducing blood flow to nerve cells in the retina. Treatment can be injections or laser surgery. These scientists are looking at photo-bio-modulation – which involves irradiating the eye with far-red to near-infrared light. This increases oxygen-rich blood flow to the retinal cells. They use an LED contact lens to get the light exactly where its needed. No human tests yet.. so it's a far way off. https://newatlas.com/medical/led-contact-lens-diabetes-blindness/ XX I don't have much of an update about diabetes aid to Ukraine except that it's still very much needed. One partnership I didn't mention last week is one between the Ministry of Health of Ukraine, a group called Direct Relief, and the International Diabetes Federation. They are working closely together to understand where supplies are short, secure donations within and deliver them quickly to where they're needed. Patients taking verapamil required less insulin two years after their type 1 diagnosis and also showed evidence of other surprising benefits. These researchers caution that their study is small and needs to be confirmed by larger clinical studies. Our friends in Ontario, Canada will finally get CGM coverage. This should make the Flash Glucose Monitor available with zero copay to Tricare members. XX Our top story.. a lot of you were asking about help for people with diabetes in Ukraine. I didn't jump right into this last week because I get very concerned about scams. We hate to think anything about diabetes isn't on the up and up.. but we need to be careful. I was really glad to see Insulin for Life, which is very well known and well-run, stepping in here. And easy way to donate to them is through Spare a Rose for Ukraine. This is the annual campaign centered around valentine's day that usually runs through February.. they're extending it through March. All donations will be directed to Insulin for Life, a registered charity with over two decades of experience providing insulin and diabetes supplies to under-resourced countries, and responding to emergencies. Their international consortium and network in Europe, with partners in Ukraine and neighboring countries, has supplies ready to go if, when and where needed. https://sparearose.org/ https://www.jdrf.org/blog/2022/03/02/helping-the-diabetes-community-in-ukraine/ XX As doctors learn more about people who develop COVID-related diabetes they're finding blood sugar returns to normal a few months later. Researchers studied 600 patients who showed signs of diabetes while hospitalized for COVID-19, including 78 with no previous diagnosis of diabetes. Compared to patients with pre-existing diabetes, many of the newly diagnosed patients had less severe blood sugar issues but more serious COVID-19. Roughly a year after leaving the hospital, 40% of the newly diagnosed patients had gone back to blood sugar levels considered non-diabetic. https://www.reuters.com/business/healthcare-pharmaceuticals/covid-related-diabetes-may-be-temporary-racial-disparities-widen-with-omicron-2022-02-25/ XX Scotland has nearly 2-million people in a huge ongoing COVID study. The latest info from that shows no association with a COVID infection causing type 1 diabetes in children less than 16 years old. Type 1 in children increased to about 1.2 times the average during the pandemic but it doesn't seem to have been caused by COVID. The US CDC has said otherwise, that there does seem to be a causal link between COVID and type 1. In all the studies so far, getting a COVID vaccine was not associated with incidence of type 1 diabetes https://www.medscape.com/viewarticle/969166 XX Good news about using the Dexcom CGM in the hospital. The FDA has granted Breakthrough Device Designation for Dexcom for healthcare professionals to monitor and manage glucose levels… in adults who use insulin while in the hospital. We first talked about this in 2020 when the FDA temporarily allowed hospital CGM use because of COVID. Dexcom created a special page on their website to provide resources to clinicians.. I'll link that up. https://www.dexcom.com/hospitalcovid-19 XX Researchers say they're making progress into amyloid, a key molecule in the pancreas that contributes to type 2. For the first time, scientists say they've uncovered the step-by-step changes that take place in a molecule known as human islet amyloid polypeptide. It's helpful in the body until it changes into a more damaging form. The researchers said their findings, raise the possibility of new treatments for type 2 diabetes and other amyloid-related diseases such as Alzheimer's https://www.medscape.com/viewarticle/969265 XX There is a huge psychological barrier against starting insulin for those with type 2. A new web-based resource is hoping to help with that. One in four adults with type 2 report being unwilling to start insulin once it's recommended. These Australian researchers designed an online resource called “Is insulin right for me?” They went through eight psychological barriers like “does insulin mean my diabetes is more serious?” Then they went through the educational process around each. This was a very small study – fewer than 40 people- but the response was positive enough to continue toward a broader roll out. Editorial here – I speak to people all the time who feel like starting insulin is a failure of will power. I get it, we should all eat better and exercise more.. but no one feels the same shame about blood pressure medication. President Biden talking about lowering prescription drugs and capping the price of insulin for the insured. He singled out 7th grader Joshua Davis from Virginia, who was diagnosed with Type 1 diabetes when he was 11 months old. Joshua's father also lives with type 1 and the family were guests of the first lady during the state of the union. It's not clear if any of the measures to lower prescription drugs will actually get through congress. XX We're keeping up the non-invasive glucose monitoring news.. and the latest contender is sort of an old one. The My Sugar Watch is said to be moving forward and could be available this year. MySugarWatch is a licensee of the BEAT sensor, also known as SugarBEAT, from Nemaura Medical. The SugarBEAT CGM has been around for a while, getting its CE Mark European approval in 2019. It's a small patch that sticks onto your skin for 24 hours, then you replace it. The transmitter sends wireless readings to a smartphone app via Bluetooth every 5 minutes. They submitted to the FDA in early 2020.. with what looks like an updated submission last year. The frustrating part of science is that it takes time.  It takes time to validate an experimental process, duplicate the process enough times to verify that the results are relatively predictable, and to calculate and demonstrate the statistical probabilities. When it comes to doing research about health and wellness, it's hard to wait out the time for scientific validation for compassionate reasons.  The Hippocratic oath starts with the promise to “first do no harm." But the science behind healthcare can sometimes create a dilemma.  In today's Quantum Revolution episode, we're going to be talking with Dr. Peta Stapleton who has been building a body of scientific evidence behind the Emotional Freedom Techniques, EFT.  Some of you may know it as “tapping."  What if the “doing no harm” means taking a risk and implementing something that seems anecdotally “harmless” and truly beneficial before science has time to validate a healthcare innovation?  Sometimes we have to stand in both places at once.  We can take a calculated, intentional risk to implement a process that is seemingly beneficial and - with caution - inform our clients that the science behind the choice is still emerging, while we wait for the patterns to be scientifically verified and validated over time. This process takes pioneers who are willing to take a leap of faith.  Both the practitioner and the patient have to be willing to trust that the experiment is worth trying and understand that the results are not guaranteed.  But then again, with healthcare, to a certain degree, the results are never quite guaranteed.  There is no one size fits all approach for treating people. Only statistical probabilities that treatments will work… The decision to take a calculated risk for the sake of healing is a personal choice, provided the outcome doesn't impact the well-being of others.  But, collectively, we have to understand that these calculated risks, while they may be end up being a “Hail Mary pass" on the personal level, are not scientifically valid.  While the practice of EFT tapping has not been around as long as acupuncture, one of the main components of EFT is the gentle tapping on those same energy lines, or meridians, used in acupuncture and acupressure. When combined with some current psychotherapies, such as Cognitive Behavioral Therapy and Exposure Therapy, tapping yields amazing results. EFT was developed by Gary Craig in the 1990s based on his study of work done by Roger Callahan in Thought Field Therapy (TFT). TFT was one of the first methods in what is now called Energy Psychology. As Craig realized the potential of this approach, he worked to refine and simplify the techniques while maintaining positive, measurable results: an evidence-based methodology. He ultimately developed a simple yet very profound set of techniques which he made available to everyone on his website emofree.com. Dr. Peta Stapleton is a registered clinical and health Psychologist and Associate Professor at Bond University (Australia) who embraces evidence-based and innovative techniques.  Dr. Stapleton is a world-leading researcher in Emotional Freedom Techniques (‘Tapping') and led a world first study investigating brain changes after EFT.  She was awarded the Harvey Baker Research Award for meticulous research (Association of Comprehensive Energy Psychology), and was named 2019 Psychologist of the Year (Australian Allied Health Awards). Over 40 doctors, legal experts, and other healthcare professionals published a consensus statement in The Lancet, one of the most prestigious and respected medical journals. The authors conclude: “Although we do not suggest that open-source AID systems be universally recommended over commercial options, strong ethical reasons support the use of open-source AID systems, with safety and effectiveness data derived from real-world evidence." Essentially, they say it's time for the medical industry to take these methods seriously, and to establish some standards and protocols to help further access and safe use. https://mashable.com/article/doctors-approve-diy-diabetes-tech-treatments XX Stem cell therapy for type 1 moves forward. CRISPR therapeutics and Viactye announce approval for a clinical trial application. We've covered this before and talked to ViaCyte on the podcast. They've been moving forward with stem cell research and you may know Crispr as a gene editing platform. The idea is that together these companies could find a functional cure for type 1. This is a Phase 1 clinical trial designed to look at safety and efficacy. They expect to start enrolling patients by the end of the year. https://finance.yahoo.com/news/crispr-therapeutics-viacyte-inc-start-133000366.html XX A new study suggests a lack of a specific molecule in red blood cells may be the root cause of vascular problems in people with type 2. Red blood cells are important for many reasons, one of which is maintaining what's called cardiovascular equilibrium. This is partly achieved through the production of nitric oxide. This is a problem in people with type 2 and these researchers speculate it's because they have much less of something called micro-RNA-210 than those without diabetes. Interestingly, blood sugar control through medication appeared to cause no major improvement here – it didn't affect the red blood cells. https://www.medicalnewstoday.com/articles/scientists-identify-new-cause-of-vascular-injury-in-type-2-diabetes XX Good news for pregnant coffee lovers: Consuming a low amount of caffeine during pregnancy could help to reduce gestational diabetes. This is one of those confusing health issues that seem to change every year.. right? Drink more coffee, drink less coffee.. this study says if you're already drinking coffee or any caffeine, about two cups a day is just fine and may reduce your risk of gestational diabetes. If you're not already a coffee drinker, there's really no reason to start. There were no statistically significant differences in blood pressure, preeclampsia, or hypertension between those who did and did not drink caffeine during pregnancy. https://www.news-medical.net/news/20211111/Low-amount-of-caffeine-intake-during-pregnancy-may-help-reduce-gestational-diabetes-risk.aspx XX The government of Japan announced this week that a company there has developed quote - the world's first blood glucose level sensor that requires no blood sampling. The sensor uses cutting-edge laser technology for a much brighter infrared laser, enabling anyone to hold a fingertip over its light for about 5 seconds to easily measure their blood glucose levels. The company is now working on mass production. Not a lot of info here but the website says 2023 is their goal for launch. http://www.light-tt.co.jp/?lang=en&fbclid=IwAR0h6rlJKXwxEuUDQ8O0_KFYLCg885_pITiVE9xPjgsIXLPCaK9cuJntjSo XX Scotland has become the world's first country to begin routine testing of C-peptide levels in patients with type 1 diabetes. Couple of goals here they say… first to identify people who have retained robust natural insulin production and second.. to see some who might not have type 1 at all. C-peptide is a substance that is created as a byproduct of natural insulin production. A preliminary study looked at 850 people with type 1 and found that about 13% had more than what's thought to be the usual expectation of insulin production. They also found that 5.1% were reclassified to type 2 diabetes and 1.6% were reclassified to monogenic diabetes. Many of those people didn't need to take insulin any more. One quick note – I've seen this article floating around which says that half of the people in this study did NOT have type one. That's not the case at all. The only thing I can think is that someone misread the 5.1% reclassified to type 2 and misread it as 51%? But it's a tiny number. Our guest on this episode of the podcast is Dave Jaffe. During this interview Dave talks about his style of humor, his love of dogs, and offers up some writing tips for aspiring writers. Dave Jaffe is a writer, columnist, former Chicago reporter, and humorist who has written for online media, print, television, and radio.   Stu Levitan welcomes Madison native and retired Milwaukee Fire Captain Gregory Renz, author of the award-winning debut novel, Beneath the Flames, and an actual hero. Guilt. Inner peace. Racism. Post Traumatic Stress Disorder. Farms and station houses, rural Wisconsin and inner city. Lust and love. Family. Failure. Redemption. These are the themes and settings that occupy Greg Renz in Beneath the Flames, the story of a young dairy farmer and volunteer firefighter from rural Wisconsin and the angry young Black girl he meets and tries to befriend when he becomes a firefighter in inner city Milwaukee. The occupation was successful; since its publication in 2019, Beneath the Flames has received the gold medal in the Readers' Favorite International Book Awards, Midwest Book Award, Best Book Award from American Book Fest, and the Public Safety Writers Association award. Needless to say, I'll say it anyway, it's a really good read. Greg Renz was born and raised on Madison's far east side, and is a Purgolder, East High Class of 1969. He took a BS degree in Zoology from the UW-Madison in 1976, and three years later, after a somewhat ironic stint at Oscar Mayer, began a 29-year career with the Milwaukee Fire Department, including three years at Fire Station 5, at 13th and Reservoir, in the core of the inner city – an experience which directly inspired Beneath the Flames. In 2006, he was inducted into the Wisconsin Fire and Police Hall of Fame for the dangerous, even a bit reckless, rescue of two young boys in 2004. He retired in 2008, moved to Lake Mills, and set about turning his experiences and imagination into prose. It was not a quick or easy process, but the effort was worth it. Marjorie Herrera Lewis is an award-winning sportswriter, named the first female Dallas Cowboys beat writer when she was with the Fort Worth Star-Telegram. She later joined the SportsDay staff of The Dallas Morning News where she continued to cover the NFL and professional tennis. She is currently a contributing national college football analyst for WBM radio. Her debut novel, When the Men Were Gone, was named a Best Book by Sports Illustrated and was named Best Historical Fiction by the American Book Fest. While writing When the Men Were Gone, she became inspired to try her hand at coaching football herself and was added to the Texas Wesleyan University football coaching staff in December 2016. Marjorie has degrees from Arizona State University, The University of Texas in Arlington, Southern New Hampshire University, and certificates from Southern Methodist University, and Cornell University. There are so many reasons to study foreign languages at every age. With all the tools we have available to aid us, there's no excuse not to learn a second (or even a third) language. Foreign languages are one of the most important parts of our homeschooling experience. In fact, I even downloaded Duolingo on my mom's phone so she could keep her mind fresh learning another language! Learning to speak a second language is so rewarding, especially when you have the opportunity to put all that effort into use. That is why I was thrilled to speak with Stefanie Naumann, the granddaughter of Tadeusz Haska.  Her grandfather spoke nine languages! He was born in 1919 in Mikołajki, Poland, and was orphaned at the young age of thirteen. Education was so important to him, that he and his younger brother continued going to school while living alone. They were able to pay their way by tutoring other students. Unfortunately, a bomb dropped on the university he attended and he had to put his education on pause. However, his knowledge of languages helped him survive World War II. He persevered and miraculously survived in many situations. Languages saved his life on multiple occasions, as told in his memoir. Haska's granddaughter, Stefanie Naumann, co-authored the book How Languages Saved Me: A Polish Story of Survival. It is her grandfather's biography that he was unable to finish at the time of his passing at age 93. Stefanie was able to finish the book using video interviews, letters, and other documents made during Haska's lifetime. How Languages Saved Me: A Polish Story of Survival earned 1st place in the 2019 Royal Dragonfly book awards in the autobiography and historical nonfiction categories. Not at all surprising since his story is astonishing and inspirational. When I was arrested, my whole world crumbled. I knew that leaders of political parties had been arrested, and never heard from again. My only chance at survival was to find a way to escape from the jail. Tadeusz "Tad" Haska survived World War II on the run, narrowly evading the Nazis every step of the way. After the war, he daringly escaped jail by the Soviet Secret Police, fled to Sweden, and launched an elaborate plan to smuggle his wife in a coffin on an all-male naval ship. Languages Saved His Life Through reading the book, we are able to observe how Mr. Haska's knowledge of nine languages helped him survive in the face of unspeakable adversity. Interestingly, his ninth language was English and was the language in which he earned his Ph.D. in Linguistics at the University of California, Berkeley. He taught and served as chairman, in the Polish Department at the Defense Language Institute in Monterey, CA for 35 years. His work was featured on the TV program The Big Picture. We have studied Spanish, Korean, Mandarin Chinese, German and Russian languages over the years we have homeschooled. She earned her Ph.D. from Louisiana State University and has published over 30 journal articles.
