Treating Stiff Hands | How Less is More! I have received a common question this week regarding the treatment of stiff hands. Often, therapists become frustrated when they receive these types of questions because they feel like they are trying everything without any success. As a therapist myself, I have developed several treatment principles, and today I want to share one of them with you: "less is more." When dealing with a stiff hand, it is important to understand that splints are not a cure-all solution, although they can be helpful in certain cases. Splints can be used in two ways: as a static progressive or dynamic tool to increase motion or as a means to block one joint in order to allow movement in another joint. However, splinting is just one aspect of a comprehensive treatment plan. When evaluating and treating a stiff hand, it is crucial to identify the limitations and goals of the patient. Manual therapy skills are essential in treating a stiff hand effectively. Slowing down the movements during manual therapy and educating the patient about the nature of their joint can help manage expectations and alleviate frustration. In terms of exercise selection, it is better to focus on a few key exercises that are performed correctly rather than overwhelming the patient with numerous exercises. Taking the time to explain the exercises and ensuring the patient understands how to perform them at home can lead to better results. Finally, when providing a home exercise program, it is important to consider the patient's success and motivation. Instead of giving them a long list of exercises, focus on a few exercises that they can perform correctly and consistently. In summary, when treating stiff hands, remember the principle of "less is more." Use splints as part of a comprehensive treatment plan, employ manual therapy skills effectively, select a few key exercises performed correctly, and prioritize the patient's success in their home exercise program. He holds a MS in Business Administration from the University of Bern and previously worked at Stryker and PwC. Aurelio's entrepreneurial journey all began in 2020, when he and some of his PwC co-workers started investing in rare whiskey. Prior to that, Aurelio had only invested in ETFs and stocks, but he soon realized that the high correlation between traditional assets and economic downturns meant that he had to diversify his investment portfolio. Further research led him to discovering that alternative asset classes such as cars, watches and whiskey had a much lower correlation with economic fluctuation. Specifically assets like whiskey or wine have the added advantage of their price being driven by their intrinsic value, which only increases with time (the older the whiskey, the better, the more expensive) – they're not dependent on some hype. They are also, so to speak, “recession-resistant”. So in 2021 Aurelio and his co-founders created Splint Invest, an app with the mission to enable all private investors to invest in alternative assets, starting at just CHF 50. You can trade your “splints” with other investors or hold them until the asset is sold and you get to collect your profits. When an asset is sold, you're charged a flat fee of 2% of the sales price on a pro-rata basis, which covers Splint Invest's sales process expenses. There are no running costs when you invest in Splints: the variable costs for storage and insurance are already included in the splint price of CHF 50. However, it is true that stocks have a higher yield on average (despite also being a riskier investment) – but Aurelio makes it clear that alternative assets should only make up 5-15% of your investment portfolio (the rest consisting of stocks, real estate, etc…), and that they're not something for beginners to dabble with. Probably about 90% of the patients I see down at that muscular end as you say. Jaz's Introduction:What happens to our TMD patients when conservative care fails? Like you've done your patient education, you've given him the best occlusal appliance, you've worked alongside your TMJ physiotherapist, you've been through exercises, and you've even counseled them about the importance of recognizing awake bruxism, a huge player, and all this stuff isn't working. What happens next? Well, depending on your diagnosis, the next step for some patients will be see a maxillofacial surgeon, but not any old maxillofacial surgeon, you ideally want to send someone who's got an interest in TMJ and TMD. So I've got today a private physician, private maxillofacial surgeon in the UK, who exclusively treats TMD. So what this guy doesn't know about surgery and TMJ. And what happens in the latter parts once conservative care fails, how the referrals manage, when should we refer these patients, which patients are suitable for referral to Maxfax, once conservative care fails. Let me give you a clue, if your primary diagnosis is muscular, then really, you know, really need to go and exhaust conservative care and the physio and by the way, most TMDS are of a muscular nature that myalgia and myofascial pain and there's no real scope for surgery when it comes to muscles that are upset. That's when we really to give the best conservative care we can and involve a pain specialist sometimes potentially Botox and lots more which we will discuss. Intro Today, I welcome Dr. Jaz Gulati, host of the Protrusive Dental Podcast. He is a general dentist who has privately practiced in Singapore and now, the UK. In his practice, he especially enjoys ortho restorative dentistry and conservative Temporomandibular Joint Disorders (TMD) management. For the third episode in the Noobie and Friends series, we talk about TMD, bruxism, and occlusal appliances. In this episode, Vinay is joined by Manuel Opitz, the Co-Founder of Mecuris, a digital workshop to create orthotics & prosthesis to save cost and time, and be more patient-centred. He aims to link the gap between digital manufacturing and medical technology. Learn about their platform, how the platform is being used to help 3D printing, and their plans for growth. What you will learn in this episode: What is Mecuris? Mecuris is a provider of medical services to help medical practitioners and orthotists digitalise their manufacturing process. Through 3D tech, Mecuris can create an orthosis process, from posture correction of a user scan, to modelling and configuration. It translates the traditional process into digital workflows and tools. What are the digital processes involved with platform? With 3D scanners available on smartphones, Mecuris is a portal to upload the 3D scanned image. It offers digital manufacturing like 3D printing and CNC milling using orthopaedics technology. They have partnerships with high-quality printing centres to support their users. How much does it cost? The basic version of Mecuris costs 65 Euros per month. They also offer a free basic version of the platform. The free version comes with a limited number of allowable downloads per month. What is the market size of Mecuris platform? Current software platforms cost an average price of 15 000 Euros and are used in over 1500 to 1800 workshops in Germany alone. In a workshop, usually, there are at least 10 medical professionals. This equates to approx. 45 million euros as a minimum in Germany alone. What’s next for Mecuris? One of the next steps is to roll it out for upper limbs and other areas like the neck and head. In the future, Mecuris also wants to specialise in other joints or partner with other manufacturers to integrate solutions that will cover more orthotics-related issues. Where do you see healthtech 5 years time? “There will be new business models, there will be diagnostic business models, therapeutic business models, or in our case, manufacturing business models around it. In this Episode I summarise: What is the difference between these various anterior appliances and is one better than the other?Deciding upper arch or lower arch, or sometimes both arches?How many of my patients have developed Anterior open bites, which splints caused them, and how to manage such a scenario?Why even an AMPSA can be an overkill and which patients may actually benefit from a soft bite guard, for example! These appliances can bring HUGE solution to a MASSIVE problem for our patients. Many of my patients are pain-free and no longe require painkillers for headaches and jaw pain. My strongest bruxists (whose teeth I have restored) are religious at wearing the appliance (despite a favourable occlusal scheme) and they love it and KNOW that their Dentistry is protected. This is not a cheap piece of plastic. It is a custom made Orthotic Appliance - I charge anywhere from £450 - £1,300 for appliances (simple AMPSAs, complex AMPSAs, Michigan/Tanner appliances - every case is different). Michigan Splints AKA Stabilization Splints are the 'gold standard' occlusal splint according to many occlusal camps. https://www.youtube.com/watch?v=DIfqn2Zkjp0 Check out the Youtube channel for video versions of the podcast. At 10 minute mark there is an error - I showed a Facebow being used whilst talking about Leaf Gauges. Dental School told me that this Splint is the only one I will need to know and it will cure all. If this does not work...maybe the patient has 'atypical facial pain' ;-) Now before you all attack me.... I have to confess. It is actually a great all-rounder splint - but there are some key reasons why Michigan appliances (or Tanner for the lower) is massively overrated! Here is a rough transcript: Lets face it - Dental school barely scratched the surface in a lot of areas, including Occlusion and splints - so it should come as no surprise to you that Michigans splints are not as great as you were taught they were. Michigan splints are actually a really good all rounder splint for all the main diagnoses within 'TMD' - quite often when I find a tricky case and I am unsure if the issue is more muscle or joint, I will recommend a Michigan - but still, it is a massively overrated appliance and is totally overkill for most of our patients. Lets start the basics - what is a Michigan splint? It is classically a hard upper splint.The lower is called a Tanner. Aka Stabilisation splint. It's a centric relation appliance. What does this mean? I explain in the podcast (so listen up!). I go in to this appliance in a lot more detail and all the shortcomings. Fellow geeks, to conclude: It's a great all rounders splint. And if ever you're unsure of joint vs muscle diagnosis and you can convince your patient to spend hours in the chair, spend that money and you think they'll comply, then go for it. It's a great splint. But if you're more concerned that your diagnosis is muscular, or the asymptomatic patient, and perhaps as an appliance to deprogram your patient....there are definitely more efficient ways to deprogram your patient. Controversy this week! Kevin Fryer is pro-biteguard with his NTI's and sleep appliances. Mac goes on a mile long rant about biteguards and how much they suck. Frankly it's the most passion I've seen from Mac on a dental topic. Zach is pro-biteguard but despite efforts to make biteguards affordable, they are still a tough sell and even tougher to get people to comply with. We discuss various types of niteguards and how we make them and implement them in our offices. This is a very special chat I had at the Heidelberg Repatriation Hospital with Hamish Anderson - AHTA president elect and AFL Hand Therapist for the Hawthorn Football Club. Hamish gives some great advice on how to get a training position in hand therapy and outlines the differences in training in private practice and hospital positions and what to look for in a good employer. We also talk about the recent changes in the accreditation pathway to becoming an accredited hand therapist and the benefits to training as an AHT. Hamish also shares some memorable moments from working in professional sport with Hawthorn Football Club over the last 7 years and the emerging role hand therapists have in professional sport. It might be justified to use a cane or a splint from time to time. And certain patients, due to permanent disabilities, may need to use such devices. For the majority of us these medical devices can cause more problems. I'll discuss when to use such as devices and when to avoid them. The Arthritis Foundation has some good-use practices for canes. John Pooley: The object of the exercise is to try and make people more comfortable. It's because it reduces pain, makes people feel more confident about the outcome of their injury. So what we try to do is move the injured limb as little as possible and pad it if you can - a rolled up rug, a coat, a pillow, a cushion, anything that's soft that you could possibly rest the limb upon to try and make it more comfortable for the person concerned. Splints really don't do any good for people that are just lying on the floor injured. Splints are about when you want to move that person and they immobilise the limb or keep the limb still when you're trying to move them. That's really a job for the professionals, the emergency services. Carpal tunnel syndrome is a very common condition that generally causes numbness, tingling, pain and/or weakness in the wrist due to pressure being applied to the median nerve. One of the hardest things to cope with when having CTS, is to keep the wrist supported and straight while it heals. At night time when we sleep, this can be difficult to do as the hand can tend to bend when we don't want it to. Carpal tunnel splints can help tremendously, and provides the support the wrist needs and prevents unwanted movement. Dr. B was interviewed on the Healthy Mouth, Healthy life Podcast by Ms. Carrie Ibbetson who is an oral health coach and patient advocate. Carrie is a hygienist who has been in dentistry for over 20 years and has a podcast for the general public about oral health care.
