Join us as we dissect the use of robotics in bariatric surgery – where precision meets programming, and the scalpel gets a software upgrade. Hosts: - Matthew Martin, trauma and bariatric surgeon at the University of Southern California/Los Angeles General Medical Center (Los Angeles, California) - Adrian Dan, bariatric and MIS surgeon, program director for the advanced MIS bariatric and foregut fellowship at Summa Health System (Akron, Ohio) - Crystal Johnson Mann, bariatric and foregut surgeon at the University of Florida (Gainesville, Florida) - Katherine Cironi, general surgery resident at the University of Southern California/Los Angeles General Medical Center (Los Angeles, California) Learning objectives: Strengths of the robot: Surgical robots are at the forefront of technology and continue to improve with detailed, precision cameras and the ability to remove baseline tremors Allows for smooth movements, fine dissection, and precise tissue handling Ergonomics are more advantageous to the surgeon when compared to laparoscopy Weaknesses of the robot: The loss of haptic feedback can be challenging for surgeons early in their learning curve Emphasis on surgical robots means some trainees may be losing exposure to laparoscopic techniques Longer operative time when working robotically, and more time under anesthesia for the patient Increased cost for robotic surgery Outcomes data: Mixed data from the MBSA QIP database (metabolic and bariatric surgery accreditation and quality improvement program) The most recent study looked at 824,000 patients from 2015-2022 who had a sleeve gastrectomy or RNY gastric bypass, either laparoscopically (lap sleeve 61%, lap RYGB 24%) or robotically (robo sleeve 11%, robo RYGB 4%). Robotic sleeves were reported to have higher complication rates compared to laparoscopy, seen as higher overall morbidity and an increased rate of leaks While the robotic RYGBs have lower overall complications, including decreased morbidity and bleeding. Robotic RYGB can be especially advantageous with revisional surgeries when compared to lap. Setting up for success Train your eyes to determine tension on tissue, since there is no haptic feedback Learn how to assist yourself (manipulating the camera and effectively utilizing the fourth arm) Understand how techniques of the surgery change when doing it robotically, as compared to laparoscopy Experienced operating room team When learning, recommend putting all cases feasible on the robot (including easier cases), to master the straightforward cases before moving to technically challenging revision cases. Don't hesitate to add an additional trocar or assistant port when needed Education in Robotic learning Learning by observation/mirroring – ex: robotic bilateral inguinal hernia (mirroring the attending/instructor) Easy for the attending/instructor in the case to switch instruments seamlessly, then give them back intermittently at the appropriate time Helpful when the attending annotates the screen to depict where to go Data-driven teaching tools on the Davinci system Tips for robotic sleeve gastrectomy: Of the robotic bariatric surgeries, sleeve gastrectomy is most similar to its laparoscopic procedure 30-40 degrees of reverse Trendelenburg Liver hammock stitch instead of a liver retractor (one less trocar), which makes a total of 4 trocars needed for the case Green staple load for the first firing, then the rest are typically blue loads Mixed opinions on reinforced staple loads versus non-reinforced staple loads and oversewing the staple line (discussed cost-benefit) Tips for robotic gastric bypass: Watch videos from colleagues to learn what they do Gastric bypass is a multi-quadrant surgery; thus, you must set yourself up for success so that your arms are not fighting when moving through different quadrants A size 12 trocar on the left can make the formation of the gastric pouch easier GJ and JJ anastomosis formed with a linear fire, then a two-layer closure with absorbable barb suture Don't forget to close the mesenteric defect (non-absorbable braided suture) Tips for robotic DS and SADI: If doing a duodenal anastomosis hand-sewn, then recommend planning the exact number of sutures and locations of each for ease Hand-sewn anastomosis can have less bleeding and fewer strictures for patients, and is completed in a much more seamless fashion with the robot Future of Robotics Haptic feedback Integrated visual overlays to identify anatomical structures/serve as an intraoperative map Artificial intelligence integration Telesurgery – ex, small surgical robot deployed to space Please visit https://behindtheknife.org to access other high-yield surgical education podcasts, videos and more. Primary and secondary school students are looking at plenty of disruption, with over 2,000 secretaries and caretakers preparing for an indefinite strike once the summer break is over. Fórsa, which represents more than 2,300 school secretaries working in primary and secondary schools, as well as 500 school caretakers, told the Oireachtas Committee on Education and Youth the mandate for indefinite strike action comes after the union's efforts to secure access for school secretaries to the Single Public Service Pension Scheme. Fórsa's Head of Education Andy Pike said school secretaries and caretakers remain excluded from the scheme, despite being on the Department of Education's centralized payroll since 2023. There are concerns that the strike, which is due to get underway on the 28th of August, could affect schools' opening hours. To discuss this further, Alan Morrissey was joined by Bernie Dillon, Secretary at St Senan's Primary School in Kilrush and Rena McGrath, Vice-chairperson and Treasurer of Fórsa School Secretaries Branch on the Clare/Galway border.

His main professional interests are related to Environment and Natural Resources Policies and Management, with a special emphasis on Water Resources.He worked in Portugal until 1983 in teaching, research and consulting activities. During this period he was involved in numerous research projects and undertook consulting and teaching work in Portugal and in several foreign countries.Between 1971 and 1983 he was Director of the Division of Hydrology and River Hydraulics of the National Laboratory of Civil Engineering, LNEC in Lisbon, where he initiated the development of several new lines of research in the areas of water resources management and planning and of sustainable use of water resources. He was Visiting Professor at Colorado State University (Fort Collins, Colorado) for the full academic year 1975-1976.Between 1983 and 1999 Veiga da Cunha lived in Brussels where he served as Administrator of the Scientific and Environmental Affairs Division of the North Atlantic Treaty Organization. This Division was the executive arm for the programmes initiated by two Committees of the NATO Council: the Science Committee and the Committee on the Challenges of the Modern Society. The programmes of these two committees were originally intended to foster scientific, technological and environmental cooperation between the North American and the European NATO countries. Later, after the disintegration of the Soviet Union, they mainly focused on cooperation between NATO countries and the so-called partner countries in Central and Eastern Europe and the former Soviet Union. Among his responsibilities while serving at NATO, he launched and directed various programs in the area of the environment, in particular the Special Programs on “Environmental Security” and “The Science of Global Environmental Change”. Each of these areas developed into a large number of projects and about 60 books have been published based on the results.In 1999 Veiga da Cunha returned to Portugal, as a full Professor at the Nova University of Lisbon. He also became a member of the National Council on Environment and Sustainable Development since 2001 and of the National Water Council since its creation in 1994. From 2001 to 2005 he has served as the Portuguese national delegate to the NATO Science Committee.He was a member of the International Panel on Climate Change (IPCC) of the United Nations (1999-2001) and a Lead Author of the chapter on Water Resources of the Third Assessment Report of the IPCC, published in 2001. He was also author and coordinator of the chapter on Water Resources of the Portuguese National Projects SIAM I and SIAM II (Climate Change in Portugal: Scenarios, Impacts and Adaptation Measures, 2001-2006).He was founder and the first President of the Portuguese Water Resources Association (1977–78) and Director of the International Water Resources Association. He was Director of the International Water Resources Association and Chairman of the Committee on River Hydraulics of the International Association on Hydraulic Research. He has been a member of the Editorial Board of Water Policy, the official science and technology journal of the World Water Council. He has also been a member of the Editorial Board of Water International, the journal of the International Water Resources Association. From 2010 to 2013 he was a member and coordinator of the "Gulbenkian Think Thank on Water and the Future of Humanity", an international group of reflection established by the Calouste Gulbenkian Foundation that gathered eleven distinguished scientists and experts in global water issues. The book "Water and the Future of Humanity" (Springer) was published in 2014, as a result of this work. Veiga da Cunha is a member of the Portuguese Academy of Engineering and a member and of the French Water Academy.He is the author of more than one hundred publications, including about 30 books and book chapters on environmental issues and particular on water resources related topics.He was awarded the Portuguese Order of “St. James of the Sword” (Grand-Officer) for scientific and cultural merit, and the French “National Order of Merit” (Grand-Officer). It's being argued the road leading up to a Clare school currently undergoing renovations is "too narrow" to accommodate the amount of traffic travelling on it. With construction underway on a major extension onto Knockanean National School, Ennis Fianna Fáil Councillor Pat Daly is calling for funding to be sought to facilitate the widening of the road leading up to it. Ennis Municipal District says it understands representatives from Knockanean National School and the Department of Education recently tried to acquire the necessary lands for road widening and "failed in their efforts". Councillor Daly insists the upgrade of the school and road should go hand-in-hand.
