Dr. Chris Segler is a podiatrist in San Francisco and known as the Doc On The Run. He has completed several marathons and is a 15-time Ironman Triathlon finisher with a personal best of 10:59:07. In this episode he speaks of his experience as a physician working in a traditional practice that accepts health insurance, and why he switched to a private practice that does not accept insurance, and why this is better for both the patient and the doctor. He also explains why when out of network you are only paying for the surgical fee and not the operating room or anesthesiology. For runners, the issue is “how do I keep running when I am injured?” Stop running is not an acceptable answer. The physicians perspective on patients and healing. How trained. Managed care. The Government is already deciding the type of care you will get. Physicians are trained to fix problems, not make you healthy. Get paid for procedures. How insurance companies influence physician decisions. Insurance will influence the procedures that are recommended. Only the options that insurance will pay for will be recommended. There may be better procedures that are not covered by insurance. You can pay for the better procedures if you know your choices. Ask “is there a better procedure not covered by insurance you would recommend?” How do we protect ourselves in non-private practices? Get the doctor to slow down. Be prepared with questions before you go. Take someone with you who understands your questions. Research “alternative treatments to__________.” The patient's challenge is to extract the best knowledge from the physician. Know in advance how you will determine the successful cure of your issue. Jim Enright interviews John Livesay, baby boomer, keynote speaker, author and podcast host. John’s long stint at Conde Nast ended abruptly when he was asked to leave in the aftermath of the 2008 economic crisis. Instead of cowering and giving up—John rose to the occasion by reinventing himself so that he could stay relevant with the changing times. Little did he know that this seemingly tragic disruption in his career would lead him to some of his greatest accomplishments to date. This is the follow through episode on visiting my Aunt Marilyn Kohn and recording a video of Kohn Family history, beginning with her growing up, her parents, high school, college, meeting Uncle Rudy, meeting Grandma Mary Kohn and moving forward to today and what she is looking forward to. We recorded 12 segments from 9 to 25 minutes each on Friday and Saturday morning. It was amusing how one memory, or question, triggers another, much like a pin ball machine in your brain. As you may guess, the best questions and answers occurred when the camera was not rolling. For instance, I asked her about the secrets to a long and happy marriage (hers was 47 years) on Friday night after shutting the video off and sipping a glass of wine. Her answer would have been an immediate viral sensation and Aunt Marilyn would have been a media star if on camera. On Saturday, asking the same question, received a subtler and tamer answer. Entertaining, but not the same. While recording, my son, David, Face Timed in from afar, which was a treat. Far different than when Uncle Rudy was on an aircraft carrier in the Gulf of Tonkin and communications was by US mail which took 14 days. I also taught Aunt Marilyn how to FaceTime. It was 45 years ago. I remember being at my Great Aunt Helen’s house and walking through it as a young 20 something. I wanted to ask about Great Uncle Johnny who had passed away 10 year prior. He was a WW1 vet and entrepreneur. I choked though. I was afraid I would make her sad or hurt her feelings and did not ask. As if it was being disrespectful. Then, I did not know that asking about a loved one long gone, would bring the memories and blessings of past times back to her, and to great Uncle Johnny. I will teach my children and grandchildren to ask insightful questions about the dead and gone. And to video record the answers while they are still available. I bought a new book that could be noteworthy and pass the test of time and be one of those books that is kept in your library, underlined, marked up, and full of sticky notes. It could be one of those books that you gift your young adult family members. The Book is Tools of Titans by Tim Ferris, author of 4 hour work week, body, and chef. This is the show note compilation of the best ideas of his many brilliant guests. There are over 740 reviews, most of which are by devotees and praise the book to high heaven. 4% (29) of reviewers gave it two stars or less. I read the bad reviews first for perspective. I have realized that one person’s biases may not be another’s. The detractors criticized for content, chapter organization and details, and editing. For the most part, I did not notice or, found what they were annoyed with refreshing or not a concern. Here’s an overview…. - it’s 700 pages; nice bright orange color; stands out, so color research was clearly done. - it’s organized into 3 main sections - Health Wealth and Wisdom - each section has 19-46 podcast guest show notes that are 1-5 pages each. - rather than a specific topic under each main section, you get a …name only, and you must read what their subject and contributions is. - because the chapters are so short, it’s perfect for a quick read without having to come back later to finish. -perfect for the bathroom, anytime you have a 5-10 minutes. Quite easy for morning and evening reading. - have also noticed I do not need a book marker, or to be concerned about where I left off. I just finish a section and check off the authors name, and put the book down. - when I am ready to read again, I will pick either health, wealth, or wise, choose a name and read what they are about. - that does mean I must create a new system to remember what the people are about. I’ve written a one word description by the author ‘sI think are most interesting. Chapters Tim’s introduction how to use this book and What makes these people different is - could be a high light for living life. The Health section may appeal most to the scientifically inclined readers - many chapters about not your normal diets, designer stimulants for brain and body, with instructions I do not think I could follow. The take aways in other chapters have been easier for me to digest. For instance, fasting for 3 days. I have done that before so it was a nice reminder with scientific benefits. The Wealth section has many well know names as well as under known names. Ramit Sethi is well known. I listened to the original podcast, and found this short chapter a valuable reminder. The Wise section General Stanley McChrystal Red Team concept - you have a great idea and work and think to disrupt it “how would you disrupt or defeat this plan?” In each Section, there are 10-13 Non-Profile Chapters by Tim that are excellent, perhaps getting down to the essence of Ferris. There is a section on the Top 25 podcasts, which will save you time. Especially interesting for future reading lists is The Most Gifted and Recommended Books of all the Guests. There are 17 that have been mentioned at least 3 times. And then there are the books each author mentioned that did not make the list. Who is this book for? If you Face Time, Skype, or Zoom, this is an excellent book to elevate your computer so that your camera is eye level, rather than looking up your nose. It is classified as a business book, but would be valuable for anyone in a creation (re-creation) or growth mindset. Here’s the final thought…particularly relevant to Planet BoomerVille, and how you are living life…in the time you have left. by Peter Thiel, - If you have a 10 year plan of how to get somewhere, you should ask, why can’t you do this in 6 months? We’ve finished 3 killer episodes on Reverse Mortgage (or HECM’s). My plan was to expose them for the evil they are, but I was derailed by both new Federal regulations that protects consumers, and education by Ron Heath. I highly recommend that you listen to these 3 episodes as many times as it takes to absorb the financial concepts, and how they could impact your financial security….not necessarily NOW, but perhaps in 20 years…maybe 30 years. Acting now, that is sooner, rather than later, as a last resort, is in your favor. Listen especially if you are planning to pay cash. This episode is buying a home with a traditional mortgage. You pay for the next 30 years, or less. This also expects that you have done the critical thinking of: #1- Do I want to buy a home right away in my dream location without living there first and renting? # 2-Do I want to follow my children and grandchildren? # 3 - Do I want to leave an inheritance of a paid for home for my children? The two critical timing points that you must meet for a closing without running into document purgatory or qualifying for less home than you planned. As Garcia and Hunter wrote, “when life looks like easy street, there is danger at your door.” A HECM could be an off setting factor for your upcoming unknowns of 20-30+ years away. Be sure to listen to episodes 9 and 10 of this series which are the foundational basics. If you are 62 or approaching, a Reverse Mortgage, or HECM (Home Equity Credit Mortgage), is the revised financial tool that you owe it to yourself and spouse to review carefully as you would any investment or insurance policy. In the wild west days of HECM’s, they could be harmful and not a good decision. Today, they are highly regulated with protections and disclosures to protect you.
