In this episode, Jennifer H. has returned with an update. The last time I talked to Jennifer in 2019, she was living with her husband in their home and she was excited that her home was to an organizational level that if she needed to have company, she could be ready in a short amount of time; she just needed to tidy up a little. Jennifer was on her 9th round of the 100 Days Program (now known as The Productive Home Solution®). Jennifer shared that the next thing that happened was downsizing her home and her mother-in-law's things so she, her husband, and her mother-in-law could all live in her mother-in-law's 3-bedroom ranch. Due to Jennifer's faith, empathetic conversations, and negotiations with her mother-in-law, the transition was pretty smooth. In April 2020, due to the pandemic, her mother-in-law went to live with her sister in Wichita, Kansas so they could go through all her things and get situated in her mother-in law's home. They wanted to make sure she was somewhere safe in the event the world shut down. In May, they moved in. Unfortunately, in June her mother-in-law suffered a stroke. And after care didn't go according to Jennifer's wishes. Jennifer appreciates that while the siblings don't always agree, they all keep saying yes to caring for their mom and moving forward together, having tough authentic conversations. We had such a great in depth conversation about how all of our families responded in different ways to the pandemic, the vaccines, and integrating into life again. And now that her brother has retired, he has moved in too! And Jennifer has acknowledged the season of life that she is in and she is the CEO of their home. And that has started a conversation between Jennifer and her husband about their next chapter and if it's time for them to move out. It's so important to keep asking “What do I want?” As life and roles change, the answer to that question will change. And the answer to that question will dictate your reaction. And Jennifer says when she asks herself this question, she just takes the next right step towards what she wants. We can't control others, we can only control ourselves. Reliving those experiences really had us looking at tough authentic conversations those events force us to have. Often if we are open to listening to the other person, we find we are saying the same thing, just using different language. Just because you ask a question does not mean you are judging. Not all those events went the way Jennifer would have liked and that brought us to intuition. It seems as though there are shifts in life that force us to look for new solutions. We use reflective learning to aid our intuition to guide us towards a solution and what we are going to do next. When we resonate with an idea, that is a green light to explore the opportunities in that solution. Now that Jennifer feels organized, she can explore her next steps. She is still teaching part time, but also practices Reiki in an office setting. She's following her intuition. Jennifer's advice is, “If you have been listening to the podcast and you are still struggling, hire one of the organizers on the app to help you. They are worth every dime. I had just finished what I considered to be the conclusion of ‘Season 3' of my Podcast series, still gaining an incomprehensible cult following, to which I questioned in curiosity, but none-the-less kept present, however awaiting karmic response, or some external universal backlash, as the season, which I posted all-at-once in an effort to regain some of my consciousness, as it seemed to have been ripped away in pieces– no longer belonging to myself, but a ghost that could neither decide whether it wanted to haunt or befriend me. In the early dawn, finally able to rest, I slipped into that of another vivid and astral projection of lucid dreaming–however, becoming increasingly sensitive to the notion of love, I chose not to record its happenings, only allowing the fondest of any memory I had kept from the dream sequence, a relief; as the connection I had received was both joyfully unexpected, and greatly needed. With only pending payments still pending for weeks and mere days left in the dwelling I inhabited, and almost nothing left–pennies, actually, by the American standard of conversion, I kept myself from weeping and only could depend on yet another intervention of Divinity to again rescue me, again–from this, a city which may have excited me in safety, but that I had grown to dislike increasingly, only having come with hopes of releasing The Festival Project internationally, however– with this rendition of The Electric Daisy approaching quickly within the week, my sanity became wavering under the notion that EDD had any humanity, as they had seemingly used me as some sort of experimentation, dangling the money I so desperately needed like a carrot in front of me–randomly allowing payments in my greatest moments of desperation and disparity. For some reason the whole world smelled like sex; There was nothing but bodies in the ripe perfection of the perfect paradisiacal air; Sunlight reflecting off of each and every thing thing that was under it..The days were long, and the nights were almost a blur, but I was less concerned with any one person or circumstance beyond my own well being for more than a fraction of the moment. But because I let it become funnier, all the while behaving as an omnipotent observer, it was unfolding quite beautifully, and very theatrically; I couldn't help but keep track of the movement around me, and how it related to my innermost reflections in this existence, seemingly everlasting. ‘--Now he's gonna cut the head off this bird, I guess.' Owh. This is beyond out of control. Unacceptable. The game hasn't started; the map isn't even open yet; Why are the refs making calls? You are OUT of BOUNDS. I'M NOT PLAYING. This is so far off the map. There's a secret passageway. They LEAKED. The MAP. All of a sudden, nothing bothered me. I was my own worst enemy, but now it seemed that I was magically washing away in my fears. Not far from freedom, I wasn't gone from the idea of what Love was, but was drawn towards a darker, more solitary cause. What is this story. I guess this is The Legend of Supacree. I'M NOT CURSED. I guess I'm just Suspiciously Capricious Kinda skittish when I'm in my kitchen scripting this in Yiddish, washing dishes in my menacingly hideous unmentionables spitting shit I didn't get to send to— …anybody interesting. #skrillex 3 Weeks Earlier… And of course, the first night I thought it either safe or warm enough to sleep entirely in the nude, the man next door somehow found it necessary to burst into my apartment, to which my lackluster and almost non-existent response became more alerting to me than the incident itself; I realized at that point, the amount of trauma I had sustained had created a nearly unexcitable individual, as I pondered the kind of reaction any ‘normal' woman might have had to that, something which may have seemed terribly frightening once upon a time, but was now, as anything else, merely comical. I had just finished what I considered to be the conclusion of ‘Season 3' of my Podcast series, still gaining an incomprehensible cult following, to which I questioned in curiosity, but none-the-less kept present, however awaiting karmic response, or some external universal backlash, as the season, which I posted all-at-once in an effort to regain some of my consciousness, as it seemed to have been ripped away in pieces– no longer belonging to myself, but a ghost that could neither decide whether it wanted to haunt or befriend me. In the early dawn, finally able to rest, I slipped into that of another vivid and astral projection of lucid dreaming–however, becoming increasingly sensitive to the notion of love, I chose not to record its happenings, only allowing the fondest of any memory I had kept from the dream sequence, a relief; as the connection I had received was both joyfully unexpected, and greatly needed. With only pending payments still pending for weeks and mere days left in the dwelling I inhabited, and almost nothing left–pennies, actually, by the American standard of conversion, I kept myself from weeping and only could depend on yet another intervention of Divinity to again rescue me, again–from this, a city which may have excited me in safety, but that I had grown to dislike increasingly, only having come with hopes of releasing The Festival Project internationally, however– with this rendition of The Electric Daisy approaching quickly within the week, my sanity became wavering under the notion that EDD had any humanity, as they had seemingly used me as some sort of experimentation, dangling the money I so desperately needed like a carrot in front of me–randomly allowing payments in my greatest moments of desperation and disparity. This disparity, however, would leave me stranded in a hostile 3rd world country. Dad was admitted into Hospice care and has switched his primary doctor to the Hospice doctors so that he doesn't have to wait days or weeks for the help he needs. His current physician Dr. Velez just can't be bothered any more it seems. After four hours of meeting with first Ruthann L, RN for Continucare, then Jennifer H?, RN from St. Joseph's North, and then Felix from Hospice, and recording all the sessions so none of us forgets what was said during this emotional time, this is my best recollection of events. 8/6/16 Dad's defibrillator went off 7 times in quick succession, causing excruciating pain and causing him to have a hard time breathing in the aftermath. A defibrillator fires when it detects the heart is in “lethal rhythm”. Dad was getting ready to go on vacation and wasn't working at the time. He was in enough pain and distress to ask Mom to take him to the hospital. She drove him to St. Joe's N. where he stayed until the 9th. While there he was given lasix to help drain the excess water that was building up around his heart and lungs due to the decreased efficiency of his heart. With that and oxygen he was feeling good enough to go home. He was instructed upon release to set an appointment with his primary care physician. Mom had a wrong number for them and tried for days to reach them without success. Meanwhile Dad's condition worsened, instead of getting better, as they had hoped, so that he could brace himself for another Dr. visit. While in the hospital, Dr. Wong, a kidney expert came in and took him off the lasix as his kidneys were in Stage 3. Mom said Dr. Wong said they had over medicated him and that his kidneys would outlast him. That doesn't seem to be the case though, based on the next few days. 8/9/16 Dad returned home. He wouldn't eat in the hospital, more than a bite or two, and complains that everything tastes like it was drenched in sugar and syrup. He didn't eat any better at home, even when offered the same foods he always eats. He's lost 20 lbs since admission to the hospital. 8/10/16 Nurse Jennifer H? came and did blood work. GFR 19 (low) Na (Sodium) 129 (low) Potassium 3.6 Chloride 88 (low) AGAP 22 (high) C02 19 (low) Glucose 249 (down from 287 in hospital) Creatine 3 (high) Calcium 8.5 (low) BUN 107 (high) 8/16/16 Dad tells Mom he can't breathe and she calls 911 who send the paramedics. He feels like he can't catch his breath and wants oxygen but can't get it. They tell him they can't give him oxygen unless he gets in the ambulance and goes to the hospital. He decides he'd rather not breathe than go through all that hassle again for nothing, so he declines. They tell him to sleep sitting up so the fluids don't build back up around his heart and lungs. Jennifer H. Dorsey, now known as Mother Jen, was ordained as a priest on Sunday by William H. Love, bishop of the Episcopal Diocese of Albany, at St. Boniface Episcopal Church in Guilderland. “Mystical” is how Dorsey describes the ceremony, in this week’s podcast, as other clergy lay hands on her and prayed God would descend upon her and make her a priest. As a child in northern New Jersey, Dorsey grew up in the Episcopal Church and always loved it. “I felt like this is home and where I was meant to be,” she says.As an adult, she traveled the country, studying and teaching history, and ended up back in the Northeast, as a professor of American history at Siena College. Dr. Kenneth Carter is a Charles Howard Candler Professor of Psychology at Oxford College of Emory University. He has published in both academic and lay publications, translating psychology research into engaging everyday language. And I say that up until about five minutes ago, I'd never met the guy and he had no idea I was, but Dr. Kenneth Carter has written a book- what, several books with the book he wrote recently, it's called “Buzz. Inside the minds of thrill seekers, dare devils and adrenaline junkies” published by Cambridge University press. He's presented on that same topic at TEDx. He's spoken at SouthX. He does prefer relaxing on the beach to wingsuit flying and BASE jumping. And we'll get into that in a second, but Dr. Kenneth Carter is a Charles Howard Candler professor of psychology at Oxford college of Emory university. Um, the psychology of thrill-seeking as, as he puts it is the current focus. Dr. Carter's research. And I am just so thrilled that you took the time. Thank you so much for being on today. Thank you for having me and congratulations on 200 episodes. It's not easy. It isn't. And you know, again, that's the ADHD way when I started the podcast and I see how it goes. Right. And now we're at 200 when I, when I went out and when I quit my last job that I ever had, one of my own, once I will say, when I feel like when I can't make it work, not if right. Yeah. I was so thrilled to have you, so. You know, let's, let's start off very basic. So the, the, the, the concept of thrill secret, I remember dating a woman once, probably in 2003, when I started getting my license for 2005 and her, she found a skydiving. She thought it was okay, but her, I guess she told her dad and her dad, he had-I don't want to say we had a conversation. He did most of the conversing. Right. And he, cause he thought I was gonna marry this girl, honey. And he goes, that was wrong with you. You have a death.. ehy would you do something stupid? And I just remember he kept repeating, give a death wish. And I said, one time, well, no, sir, it's actually about wanting to live. And I realized that was the entirely wrong thing to say because there are people out there who will never get it. Yeah. Yeah. So, so, so with that, talk about how you ended up talking about you were getting into and discussing the concept of risk taking and, and sort of let you know what you've learned. Yeah. You know, it was completely by mistake. So I'm a clinical psychologist and, you know, I am a really chilled guy and I had this idea about 10 years ago for this book and. It was completely different book. It was about some people that I thought of as chaos junkies. And if, when I say that word, everyone instantly has someone in their mind that they think of can have an idea of the book. Yeah. Was to try to get people who were cast junkies. To be more predictable and have a life like mine, which is what a lot of people, which is probably what the, that that father wanted. You know, you know, what, what your parents usually want for you is a completely predictable life that you're happy with. Um, and then when I started really going into the research and I started to talk about this idea of sensation seeking, I've realized that these high sensation seekers craved chaos because they could control it. And it wasn't something that they needed to stop doing. It was really something they needed to embrace in a way that was going to be healthy for them. And so I abandoned that original book and I decided to really work on helping people understand the, how they're a high sensation seeking personalities can really be a superpower for them. That's really interesting that you would abandon and go to something that proved to be right, because the majority of people, and we talked about this before it, before we started recording, the majority of people are told they have to change. Yeah. Yeah. I know. I was told that all my life. And, and in the, in the, when you talked about the story about someone and asking if you ever had a death wish, when I asked the, uh, high sensation seekers, what is one thing they want people to understand about what they do? They said. To not think I have a death wish, you know, all I really want to have was a death wish I would just run in and out of traffic. And that's not what it's about. If I thought That was going to die every time I jumped out of a plane, I wouldn't do it. Oh, yeah. Yeah. That's the thing. I think a lot of people that are high sensation seekers and thrill seekers really embrace life and what they want is the experience. And one of the hardest things to get people to realize is that there's a huge difference between, uh, a high sensation seeker or a thrill seeker versus a risk taker. Um, people aren't doing these things because they're risky. Is that they can tolerate the chaos to get the experiences they want. That's what they're really after is that experience. I, I I'm, I'm just, I'm just loving it. It's just so nice to hear everything in my life validated in a quick 20, 25 minute interview. So let's, let's talk about that for a second. So I remember when I, when I started jumping and the story I always relate to, it was my instructor. You know, you have to do three tandem jumps where you're attached to someone else and your fourth jump, you jump on your own, but you jump on an instructor who's holding onto your belt. Right. Just to make sure you're stable in the sky. And then when he sees that you're stable, he did my whole on you for the entire time. He might like go for a minute, a second or two, and, but you have to pull your own parachute, right? You have to land, you know, your whole, your parachute all by yourself. You have a radio, a little walkie talking to your, in your, your pocket, but you have to Lam and, and it's on you. And I remember that my instructor was probably 300 pounds and I don't remember his name, but 300 pounds. It was the middle of July. He, he, he looked like a sopping wet elephant. Okay. And it was just, and you're in this tight little plane right. And he's just, he's like touching me and holding on. And it all, it was the grossest thing. And I remember I couldn't wait to go to the plaintiffs, so I didn't have to be with that anymore. And I'm scared to death and I guess a good way. It was a good thing that I was focused on that. Cause I wasn't focused about the fear. We exit the plane I'm in a free fall. He signals that I'm doing okay. And he lets go my belt and I stay stable. I pull my parachute and I land and he landed probably about 20 seconds before me and this 300 pound sweaty guy who, what, four minutes ago, I couldn't stand to be anywhere. I ran up to this guy and gave him the biggest, longest bear hug. Right. Just hugging the sweaty as much as I could. And it was, that was the first moment I ever realized. Holy crap. Look at the mind shift I just had and looking back on it. Yeah. I realized what that, you know, every, everything I've ever done professionally has usually come immediately after a Mindshift like that. Yeah. Yeah. And a lot of people who are high sensation seekers say they don't only feel that sense of euphoria and then a sense of calmness, not, not only after they do those kinds of things, but sometimes for weeks after, um, I've talked to some people whenever they have to make big decisions in their lives, they, they, you know, they'll go on a hike, they'll go on a jump they'll base dive, um, or they'll walk on a Slack line. That kind of thing for the people that are high sensation seekers, um, really focuses and really calms them. Very different than someone that's like me who's a low sensation seeker who I get overwhelmed by that, you know? And so one thing I, I try to get people that are low sensation seekers understand is that they're not going to their bodies ren't going to behave like high sensation seekers and vice versa. One of the things that I find fascinating is that when I land after jump, there have been times, times when I know I have a lot of writing to do I'm on deadline for a book or an article or something like that, I will turn off Uh, you know, my brain. I'll go do the jump and I'll say, okay, I know I have to do this, this writing, but I'm not going to it's it's Thursday and it's due Monday. I'm not going to work on it until I jumped and I'll go jump. I'll bring my laptop, the drop zone. I'll . I'll throw my gear in a corner. I'll sit on top of my parachute or something, pull up my laptop and I'll write 10,000 words in an hour. Yeah. And it is a level of focus that is unmatched by almost anything else in the world. And being able to, once I found that, I mean, literally it was, it was like finding the, the, the, the pill from the movie limitless. Yeah. Right. It was that, that Holy crap look at my, I am so focused on it. And I remember the first time I did it. Because I've never, I’d drive home. And the first time I drove home, after, you know, one of my, I don't know, first 20 jumps, whatever I'm driving home and I get pulled over for speeding. And I looked down and going 85 miles an hour on a local road. I didn't even didn't even realize it. Right. And I said to the cop, I'm like, you. Kinda just hang here and wait for it to be, he's like that's my entire day. Well, and then the question I have for people that aren't like that is that if you knew there was something that you could do that would bring that level of euphoria and calmness and focus and you know, that kind of thing, why, why, how could you not do it? You know, I want to switch the topic a little bit because on that, it reminds me of a great quote, an Aaron Sorkin quote from the West wing where he's interviewing. Okay. The chief of staff, Leo McGarry is, um, talking to someone about being an alcoholic. And he said, I don't understand people who leave a half a glass of wine on the table. I don't understand people who don’t..who don’t want to stop feeling that good. And he talks about being an alcoholic. I have never been quote unquote diagnosed as alcoholic, but I am very aware that I will not have one drink and I have set up my life in such a way that I very rarely drink. I quit for a couple of years and now maybe four times a year. I'll I'll, I'll set up a situation where I'm at home with a friend and I'll have a drink or two or three or four, but I don't leave the house. I don't buy more. And you have to be aware of that. And I think that, can you talk, if you can, about the connection or the, the very close proximity I would imagine, between thrill seekers and addictive personality. Yeah. And I think, you know, so there are a couple different components of that thrill-seeking personality and the two that can worry me sometimes are, um, disinhibition, um, your ability to be unrestrained and. Um, boredom susceptibility, um, where people get bored really easily get irritated when they get bored. It's not necessarily people that are the thrill seekers that, that worry me, but the thrill seekers who may not plan out, or to be able to understand how it might impact other people. Because of the way the, um, you know, chemicals work in the, in the body. A lot of people who are high sensation seekers, um, end up sort of excluding higher levels of dopamine, um, and lower levels of like, um, stress hormones. And those are the same kinds of combination of things that can be, um, problematic for people with addictive personalities or addictive conditions. And so it's, it's important to be really sort of mindful that you're either not doing it too much or doing it in a way that can be a problem for you for other people. I talk about that a lot in the book, in Faster Than Normal, where you sort of have to set up these life rules, right. Because if you let yourself go off the rails, it doesn't take long. Before those off the rails is like, You know, a six month process. I'm not going to go out and pillage a village after drinking, but I'm going to drink and I'm not going to wake up super early to go to the gym so I'm not going to get dope, mean I need and then be dehydrated. Well, I already blew the morning. I might as well order something bad for lunch. Well, lunch order a pizza it's three weeks later, I've been 20 pounds. Right. And what's good has come out of that. So you sort of have to be aware to prevent yourself from taking that first step. Do you find that and the research you've done, do you see that correlation a lot between thrill seekers and sort of people have to be more aware of their personalities? Yeah. I mean, there, there are some research studies that suggest that people that are high sensation seekers may have lower levels of empathy about what other people are experiencing, because we all assume that everyone experiences the world the same way we do. Right? And so that's why you might have someone, who's the highest sensation seekers if you're driving a car darting in and out of traffic, they're calm and chill, but their passengers are really freaked out and they may not know why they're freaked out because they're experiencing the world in a different way. And so a lot of these high sensation seekers have people that I call their anchors, you know, who will be the one that say, Hey. You know, that is more dangerous than you think it is. Or maybe you should rethink about doing that and they trust their anchors to help pull them back from situations that may be problematic for them. Talk about thrill seekers and, um, something I learned once the concept of type T positive and type T negative, that, that how you go after those thrills, um, might be determined by how you were brought up. Or by what you were sort of, um, exposed to as a kid. I mean, I know that my parents were not, my dad loved roller coasters still does and, you know, and, and we would go on roller coasters, but the concept of skydiving, you know, my, my, my dad's classic would appear: Don't be ridiculous Jews don’t skydive, it's classic quote to me. But, but, um, I remember that, you know, as a kid, he would take me, we'd have, we'd go hiking in Maine and he'd take me to find, um, um, Fire towers, right. When they were still back in the eighties and they're still manned. Right. And we'd yell out to the guy at the top of it and they’d invite us up. We climbed I'm six years old and we're climbing this ladder 200 foot ladder, top of firefighter tower. But I was, I was raised in the respect that I guess that was a thrill. Right. And I enjoyed it, but there are people who are raised to sort of, I guess, type team negative, where you're finding. You know, that's where you go into drugs or crime or whatever, to get that same sort of thrill. I think, you know, like everything, there's a combination of what could be biological and environmental, even for this personality trait. Um, they, there, there is some evidence that there are some genetic components to thrill seeking. Um, they're higher levels of certain kinds of chemicals in the body. We talked about, um, uh, cortisol and dopamine previously, but you also see higher levels of testosterone, even in women who are thrill seekers, But there are some environmental things that can really help a lot too. Um, having exposure to chaotic environments when you're younger. Um, and there are some high sensation seekers that said that their parents seemed more strict, but the parents may have just seemed strict because they were doing lots of thrill-seeking things as kids. Uh, but we know that that also changes over time. People that are high sensation seeking when they're teenagers or adolescents, it tends to get, go lower as they get older because of those environmental influences, but also because of biological too. So it's a little bit of a mix of both things. So the fact that I didn't discover any of this and I almost, I didn't know, my ADHD didn't exist as a kid. It was, it was sit down, you're interrupting the class disease. Right. And so the premise when I was, when I was busted for seeing those from the class, the irony being that I was actually looking around or squirming or fidgeting to find something to give me that dope mean. So I actually could focus. Yeah. Yeah. And so I think that if people understand what their brain and bodies are capable of and really directed to try to get what they want out of life, I think that's really what the whole idea is. And it's the same for a high sensation seeker or a low sensation seeker. Part of what I've been thinking about more is part of it's really creating and capturing and getting that sense of awe in the world. And that people that are high sensation seekers can, can really tolerate a lot more chaos to get those all experiences. Do you feel that, that the premise and I mean, I think it's shifted over the last 20 years, but the premise that we at least I had growing up was, you know, you, you, you, not that being different, it is wrong, but rather there are certain ways you do things. Right. Right. And, and, and doing them other ways, you know, is wrong. And I remember my, my, uh, telling my parents, I was going out on my own. Right. And becoming an entrepreneur were public school teachers, all their lives. And, and, and, you know, it was, it was difficult for them to sort of comprehend. Why I enjoyable…It's so risky. I know, but that's the cool, you never know what, you know today could be I'm not making any money or I'm making my largest contract ever. That's the thrill, you know, and, and, and there's nothing I've yet to find anything that compares to landing a new client or landing a new speaking gig or let you know, it's just a high. Yeah. And I, and I think that what work is for and what life is for, has changed for a lot of people, you know, over time, or could be different for different people. Um, you know, when I grew up, you know, I was talking to my dad about work, and work wasn't something that you did because you enjoyed it. It was something that you did because you had to have work. And the idea of having a job that might pay less, but that was really fulfilling wasn't really in the list of options for him. And it isn't the list of options for, for a lot of people. But I consider myself so fortunate that I have a job that I am fully engaged in and that I really love. Um, so being able to marry those things together is a magical thing. So a lot of these high sensation seekers have careers in which they use that super power. Um, they are first responders or police officers or firefighters or, uh, emergency room nurses and doctors. They can handle that chaos and turn it into a focused experience. Um, other people decide to use that part of themselves for recreation. Um, but you'd be surprised how many of them really use it every day in their jobs. Two more questions. Um, cause I want to respectful your time. Some of the people we've had on the, on the podcast before you have included, uh, Tony Robbins, um, Seth Godin, Keith Cross, who founded DocuSign and is now the secretary of, of, of, of, um, business, uh, in this administration. Uh, we've had, um, uh, the band Shinedown, and every single one of them has said that when they realized they were ADHD, um, they believe that it has benefited them and they, uh, have learned to use that as their skill and as a superpower. Yeah. What do you, and I always ask them the same question. I'm gonna ask it to you too. What do you say to the parent whose child was just diagnosed and, you know, after they get over their first? Aha. Well, that explains it moment. What do you say to them in terms of when they're sitting there going, Oh my god now my son won't be successful. Now he can't do this. Or, you know, they have this preconceived notion of, of, of what success looks like for their child of what your growing up looks like, the child. And this is. A lot of times, you know, they're, they're given this information from a teacher or from a, uh, an administrator with absolutely no, you know…and, and here's a pamphlet. Right. You know, what, what do you say to them to, to sort of talk them off the edge for lack of a better word? I think part of this would be having them listen to some of the interviews on your podcast. Um, having them understand that there are different ways to be successful, engaged, and happy in the life that they have. And to choose from their selection of powerful things about themselves to get to where they want to go. And where they want to go may not be where you want it to go, but it's going to be there path and you can help them to sort of uncover who they are and to use those best parts of themselves. Um, and I think that's true of everyone. It's not just true of certain kinds of individuals, but I just think that a lot of times parents just want their kids life to be easy and happy. Um, and I don't know if everyone's life ends up that way. Um, but they, they get there in different kinds of ways. Yup. That's a great answer. I love that. Uh, how can people find you doctor? I'm particularity excited about our interview this month. Doctor Kent Arell from the Mayo Clinic talked to Doctor Jennifer Hall from the University of Minnesota about her role as Chief of the AHA institute for Precision Cardiovascular Medicine. This is a really exciting initiative, which is bringing together researchers, patients and stakeholder to foster the growth and development of cardiovascular genomic and precision medicine. Through their Precision Medicine Platform you can access a virtual big data research hub and find data, tools, and collaborations. The institute also provides funding for projects related to precision cardiovascular medicine. You can find out more and register to access the platform at precision.heart.org. And listen on to hear Jen describe the platform and how this initiative first came about. Dr. Kent A.: Hello. My name is Doctor Kent Arell. I'm the Chair of the American Heart Association's FGTB Council early Career Committee, and a proteomic and network systems biologist in the Department of Cardiovascular Medicine and the Center for Regenerative Medicine at Mayo Clinic in Rochester, Minnesota. As part of a highly collaborative team, my research efforts focus on omic space systems approaches for heart failure prediction, diagnosis and therapy. Specifically regenerative approaches to cardiac repair. This November during American Heart Association Scientific Sessions 2017, FGTB Early Career Committee programming complimented the FGTB Council Precision Medicine Summit with a hands-on bootcamp on network systems biology, followed by a session introducing online computational portals and tools designed to enhance and facilitate basic and clinical cardiovascular research. A highlight of the second Early Career session was an introduction on the use of the Precision Medicine Platform of the American Heart Association's Institute for Precision Cardiovascular Medicine presented by the Institute's lead bioinformaticist, Laura Stevens. With these recent presentations in the many ongoing developments in precision molecular medicine combined with my own research approach and interests, I'm especially delighted today to be speaking with Doctor Jennifer Hall, Chief of the American Heart Association's Institute for Precision cardiovascular Medicine. Doctor Hall received her PhD from U.C. Berkeley and completed post-docs at Stanford and Harvard prior to taking up her first faculty position. Welcome, Doctor Hall. Is there anything I've missed that you would like to add? Dr. Jennifer H.: No, you're doing a great job, Kent. Dr. Kent A.: All right. Doctor Hall, the Institute for Precision Cardiovascular Medicine is one of the newest additions to the American Heart Association. And as far as I'm aware, you've been involved with the Institute since the beginning. Could you first tell us a little bit about the history of how the Institute came to be, and perhaps how you first became involved? Dr. Jennifer H.: I would be very happy to do that. So the Institute actually started in 2013 in the Fall, when the American Heart Association Board of Directors made an initial investment. And the vision for the Institute for Precision Cardiovascular Medicine was Nancy Brown’s, our CEO. And in the early years it was really led by the Chief of Staff, Laura Sol. And our lead strategist, Prad Presoon. And the original grants came out from the institute but was then called the Cardiovascular Genome-Phenome Center, or Study. Dr. Kent A.: Right. I'm familiar with that. Okay. Dr. Jennifer H.: Yes. That was the very early history. And it was rebranded in 2015. So that's the history. I joined about a year and a half ago now. And I'm just thrilled to be part of the team. Dr. Kent A.: Well, there's exciting things on the horizon, definitely. So what is the mandate of the institute? And what are its principal or primary objectives then? Dr. Jennifer H.: So the mandate is to really provide a better understanding in the area of precision cardiovascular medicine to all individuals. And that means participants across the United States and across the globe, patients and those that are healthy individuals as well. And finally to scientists, researchers and clinicians. Dr. Kent A.: Okay. Dr. Jennifer H.: And the five things that we are really focused on in terms of, I would say our principal objectives are really to convene people all over the world, experts, students, trainees. Provide transformative grants and this is at least half of our budget every year, if not up to 80% of our budget. Enable data discoverability and access. And that's through some of the new tools that we are creating. Act as a translation agent. And we can talk more about that if you'd wish. And finally to offer research enabling services or tools to our young trainees as well as our established investigators. Dr. Kent A.: Okay. Perfect. Well, I think I had some questions to cover most of those topics. But the translation agent would be interesting, if you wouldn't mind sort of expanding on that a little bit right now maybe, perhaps before I get to the other questions I had. Dr. Jennifer H.: Yes, absolutely. I think this means a lot of things to a lot of different people. Like one of the ways it can help people are, scientist that have been very focused in academics for a long time has not thought about intellectual property, or ways to really begin to commercialize their ideas and take them forward. So we used to think about bench to bedside. And the institute is focusing on helping these individuals. Helping these grantees really begin to think through how to talk about their ideas and to take them to market. So I'd say that is one way of thinking of the translation agent. The other is to really begin to take participant data and to bring new people into the field. So not only will we think about volunteers as being Heart Walkers anymore or being Go Red for Women volunteers, or one of the many 30 million volunteers we have throughout the United States within the American Heart Association and around the globe. But we want to ask them to contribute to precision medicine, and ask them to be a part of this new exciting movement as well and contribute data, interact, provide answers to surveys, and be educated a little bit more in the area of precision medicine. So those are the two big ways that we think about acting as a translation agent. Dr. Kent A.: Okay. Excellent. With respect to intellectual property, is there an intermediary that you connect researchers to, to facilitate that? Or is it more of an awareness of how to approach your own institutional technology transfer offices, or what have you? Dr. Jennifer H.: Well, you're going right where I would've led the conversation. So we're trying to do both I should say. Dr. Kent A.: Okay. Dr. Jennifer H.: It's just originally getting off the ground and talking to our volunteers at those academic institutions and figuring out the best way to do that. And then talking to some people in the industry as well to figure out how does it work best coming from that side. Dr. Kent A.: Okay. Excellent. Well, I think the next thing that I might transition to is the grants, because points one, three and five that you made about convening people, enabling data access and enabling services and platforms may all sort of relate to the Precision Medicine Platform. And we'll definitely be speaking about that quite a bit. But one thing I wanted to mention is the variety and the number of grants that have been awarded by the institute since it inaugurated grants I believe in 2015, is that correct? Dr. Jennifer H.: Yes. That's exactly right. And- Dr. Kent A.: Yeah. Dr. Jennifer H.: ... I think- Dr. Kent A.: Go ahead. Dr. Jennifer H.: ... we're quite happy. We've given over 72 grants and that totals, I think, just over 15 million. But more importantly the grantees have just done a remarkable job I think. The running total I have, which I know is a little bit out of date is 98 publications, and many of those in high impact journals. And certainly in new areas that AHA has not been in the past, which is artificial intelligence, machine learning, and creating new tools. Dr. Kent A.: So you're also trying to bridge multiple disciplines then I guess as well with some of these grants, correct? Dr. Jennifer H.: Yes. And many of the fields like you're working in as well, thinking about systems biology and proteomics, and bringing the field to beta science along with that as well. So exactly. And in many cases we have private partnerships or strategic partnerships with others like Amazon Web Services. So many of the grants at least, one of the portfolios we have has many grants in it in which the tech credits, or the cloud computing credits if you will, are given to us by our strategic partner Amazon Web Services. So AHA provides the salary support for the PI and others on the grant, as well as any supplies that are needed. And then those tech credits for cloud compute and storage come from Amazon. Dr. Kent A.: Excellent. Okay. And then there are other collaborative efforts with other institutions as well, is that correct? Like the Broad Institute? Dr. Jennifer H.: Yes, exactly. We have a great relationship with the Broad Institute as well. They've been extremely helpful in helping us get our direct to participant recruitment program off the ground. And we are co-investigators with them on an upcoming NIH data platform grant. So we're extremely excited to be working with them and their team. We've worked with several people, and Doctor Anthony Philippakis' team, and with Noel Burtt and David [inaudible 00:11:24] and others there. So we just couldn't be happier with that relationship. Dr. Kent A.: Perfect. Perfect. I know the current round of funding is defined as Uncovering New Patterns, and there are fellowships in grants available for that round of funding. What is the scope of the Uncovering New Patterns effort here? Dr. Jennifer H.: There's a lot of both science and tech built into that. And so, one of the ideas from the institute executive committee ... So one question people might ask is, "How do these ... How do you come up with these grants?" And it's really talking people, finding a need, listening to a lot of people no matter where we're traveling. And then bringing that up to our Institute Executive Committee who makes the final call on these grants. In this case, we were looking for a way to bring science and technology together. So if there was a way to combine datasets that hadn't been combined in the past, which creates some new data harmonization standards, perhaps there's some new methodologies around that. And that's one way to uncover new patterns that we were thinking about. That was the- Dr. Kent A.: Right. Dr. Jennifer H.: ... original example that came to mind. Dr. Kent A.: Okay. Or some new algorithms for- Dr. Jennifer H.: Yeah. Dr. Kent A.: ... interpreting data or what have you. And it sounds like there's goo dialogue back and forth and between the institute, and those who are sort of interested in the topic as well from the researchers' standpoint both clinically and for basic researchers. Dr. Jennifer H.: Yeah. We try. Dr. Kent A.: Yeah. Yep. Oh, I think you do more than try. I think it's a wonderful operation. So I see on the website that there's a new round of funding opportunities are soon to be announced. Can you give us any hint as to what the focus will be for the upcoming round or other things that may be in the pipeline in that regard for the upcoming months? Or do you want to keep it a secret? Dr. Jennifer H.: I can't disclose that. But there will be some things that are a little bit similar to what we've done in the past. Our focus is to really be on the cutting edge, and we are democratizing data on this Precision Medicine Platform making it open in a controlled access way. So you have to fill out a form. But we're really trying to get to those forms and turn over access to people, qualified researchers and scientists as quickly as possible in a very responsible way. And so we're piloting some new objectives around that. We'd love to bring all cardiovascular and brain health data together. And so people can identify and find new discoveries in this area. Dr. Kent A.: Right. Dr. Jennifer H.: And use new technique to do it. So the grants will be focused in that particular area. Dr. Kent A.: Okay. Well that give me an easy segue then into my next question, which is focusing on some of the resources that are currently available to basic and clinical investigators. And after maybe perhaps describing those, where or how can individuals access these particular resources? So I know definitely the Precision Medicine Platform is one that we'll hopefully hear quite a bit about. Dr. Jennifer H.: Yes. That's something that's been underway for about a year now. And we started with two beta testers that you can find the site at precision.heart.org. And we had two beta testers who Gabe [Lucenero 00:15:07] and Laura Stevens, and they were absolutely fantastic and gave us a lot of feedback. We've since hired Laura. Gabe runs his own company up in Canada in the area of bioinformatics. And they've just been fantastic. And since those two, in the last year we've grown to over a 1000 registered users. And once you register on the platform, you can register with your Google account, or however you ... It's a very simple process. Then you can search across all the datasets. Once you do that, you can access a workspace. And once you request access to the data, the data's put into a workspace for you that is really in the cloud. And allows you to use the power of cloud compute, meaning something that might've taken you weeks on your supercomputer back home in your institution, once you got in the queue, takes a day of a couple of hours using the power of the cloud and the compute power behind it. Each workspace also has over 85 analytical tools from visualization to statistics, to simple things. And we understand everybody doesn't know how to code. And so we're trying to make that as easy as possible as well. So there's some new things there. We're coming out with new things every week, and looking for ... If people are looking for help, we're taking emails and questions by phone and trying to reach as many people as possible. So we're looking for people to both contribute data and utilize that data to accelerate new discoveries. Dr. Kent A.: Perfect. Well, I know, I'm working with Laura actually on trying to implement Cytoscape within the cloud space as well. So it's exciting opportunities. And I'm not a coder myself, but for those that are I know most of the applications that Laura is looking at are based I believe in terms of the language that they're using, is that correct? Dr. Jennifer H.: That's right. Dr. Kent A.: I think so. Dr. Jennifer H.: So most are in R, or Python. Dr. Kent A.: Python. Okay. Dr. Jennifer H.: And they're on Jupiter Notebooks today. So the nice thing about that is that you can take that Jupiter Notebook, and those are being published in many journals today. So it's really a quick way to analyze the data, get feedback on the data from the community if you're looking for that, that's another piece that we're building in today. And you can have your collaborators work on that same workspace with you. And then just turn that Jupiter Notebook and publish it with all your methodology and code on it. Dr. Kent A.: Oh, okay. Excellent. Dr. Jennifer H.: And we're also establishing drag and drop code, so if you are just learning, you can take kind of from the recipe book and drag and drop, and practice and learn. And soon we should be hopefully working on some educational tools with our strategic primary, Amazon. So we're really excited about that coming out in the near future. Dr. Kent A.: Excellent. And we're doing our best at the council level to try to help spread the word as well with respect to the platform, and that was part of our offerings as I said, at Scientific Sessions back in November. Laura did a wonderful job presenting the platform and introducing it to a wide variety of backgrounds. I think in a way that is really ... It's really helpful bringing these people together with different backgrounds, with different abilities. People are ... Regardless of your background, people are coming up with wonderful ideas and ways to implement these applications. Are you taking say, feedback in terms of how people are using these tools and how they can utilize different applications in terms of making adjustments and modifications as time proceeds? Dr. Jennifer H.: Yes. We're really excited about that, because we'd like to keep all of that data like any scientist, and publish that. So not only ours, it's better, but others doing similar things also will improve. And this platform is meant to work with other groups and to interact with other platforms as well and be part of the NIH data commons as well. And so we're really working to interact a use standard language and standard systems to interact globally as well as within and U.S. So we're trying to make it simple for all scientists. But gather as much data as we can. And a big shout out and thanks to you, and Functional Genomics Council for allowing us to be part of that workshop at Scientific Sessions. Dr. Kent A.: Oh, we really appreciate the opportunity to be involved with it as well. I think one of the ... Another strength that as you mentioned is that collaborators can access the same workspace from different institutions so that they can be working together and collaborating without having to download an entire dataset in one space, and have someone working on it and modifying things, and then finding out that your collaborator at the other institute has modified something else. And then trying to- Dr. Jennifer H.: Right. Dr. Kent A.: ... sync those version together can be quite troublesome at times. Dr. Jennifer H.: And you weren't sure you had the right version, and datasets are so large now that- Dr. Kent A.: Yeah. Dr. Jennifer H.: ... really people think about it in terms of the researchers going to the data, instead of the data going to the researchers. Dr. Kent A.: Yeah. Excellent. Dr. Jennifer H.: [inaudible 00:20:55] the new ways. Dr. Kent A.: That's perfect. Well, I'll maybe just reiterate that website. It's precision.heart.org. For the Precision Medicine Platform. And while I'm mentioning websites, maybe I'll mention institute.heart.org, which is the homepage for the institute for Precision Cardiovascular Medicine, I believe. Is that correct? Dr. Jennifer H.: Thank you for mentioning that. Dr. Kent A.: Yeah, 'cause if I'm wrong, please correct it. Dr. Jennifer H.: Well, you see, I'm a scientist, not a marketer because I never mentioned that at the beginning. But thank you for doing that. And we'd love any feedback that anybody has of other things they'd really like to see there, or how we can be more helpful to the community. Dr. Kent A.: Definitely. And how would people go about contacting you in that regard or in terms of setting up a workspace, or accessing a workspace? Would they go through the institute homepage? Dr. Jennifer H.: In terms of setting up a workspace, you can do that directly from precision.heart.org. Dr. Kent A.: Okay. Perfect. Dr. Jennifer H.: And once you request access to data, if you check on a box and hit, "Request access," it starts you down that road. It doesn't mean you have to commit. But it will start you down that road. Dr. Kent A.: Yeah, it will set it up. Okay. Perfect. Dr. Jennifer H.: It will begin to set up the process for you. It takes ... You can put in there your grant, it takes tokens, it takes all sorts of things, because there's a small cost that is a pass through really from ... It's like if you were just to go directly to Amazon Web Services to create that workspace. Dr. Kent A.: All right. So it's a small cost incurred for ... Okay. Excellent. Dr. Jennifer H.: Yeah. For what you're doing. Dr. Kent A.: Okay. Dr. Jennifer H.: And so everybody should be aware of that. Dr. Kent A.: Sure. Dr. Jennifer H.: But the institute is giving the grants for that. NIH gives grants and tokens for researchers to allow to use that as well. And like I said, the cost is a couple dollars an hour really. So it is not something that's gonna break the bank. And the AHA has built some stopgaps to try to keep those costs as low as we can. Dr. Kent A.: Perfect. Well, that's much more reasonable than some of the bioinformatic applications I work with these days. So that's good to know. Dr. Jennifer H.: Well, just the license fees alone can be challenging [crosstalk 00:23:06]. Dr. Kent A.: Exactly. Well, even institutional licenses, you still- Dr. Jennifer H.: Yeah. Dr. Kent A.: ... you're paying a portion of, and it can add up quickly when you're on there frequently. So- Dr. Jennifer H.: Yeah. Dr. Kent A.: ... a couple dollars an hour is quite reasonable. Dr. Jennifer H.: Yeah. Dr. Kent A.: Well, I don't wanna take up too much of your time. I know you probably have another meeting scheduled right after this. So I wanna thank you for taking the time today to talk with me and discuss the AHA's Institute for Precision Cardiovascular Medicine. I seem to have trouble pronouncing, "Precision," today for some reason. And for sort of introducing the Precision Medicine Platform for our listeners.
