Today, we have a new spy video of the EV to share, showing at least three different test cars." reports Motor1.com: "First, we see a heavily disguised vehicle being carefully unloaded from a truck and then taking on a test route somewhere in the Alps. It’s towing a two-axle trailer . The Enyaq iV will be heavily based on the Volkswagen ID.4 with the two sharing basically everything that’s underneath the body. Learning About Wound Care The sun rises over the San Joaquin Valley, California, today is April 9, 2020.This week, for pregnant patients who are not at increased risk for preterm delivery the USPSTF recommended AGAINST screening for bacterial vaginosis (BV). This is a D recommendation. So, do NOT screen for BV in these patients. For your patients who actually ARE at INCREASED RISK for PRETERM delivery, the data is INSUFFICIENT to recommend screening for bacterial vaginosis. This is an I recommendation. So, you may or may not screen.To recap: Not at risk for preterm delivery = No screening for BV. At risk for preterm delivery = Insufficient data. This week, smiling to our patients has become a little harder to do through a surgical mask. We don’t know how long we will be required to wear a surgical mask to see all patients in clinic. Hello everybody my name is Dr Manuel Tu Jr. and presently I am a First-year Family Medicine resident here in Bakersfield, California. I was born and raised in the Philippines, finished my bachelor's degree in Nursing from Perpetual Help College in Manila, and graduated in Medicine from the University of the City of Manila, Philippines. Dr Tu also worked for some years as a nurse for Clinica Sierra Vista and did a fantastic job before his residency. Question number 2: What did you learn this week?This week I would like with you some things about WOUND MANAGEMENT, specifically about the types of wounds, factors that inhibit wound healing and general principles on how to heal a wound. A wound is a disruption of the normal structure and function of the skin and underlying soft tissue. It may be acute like trauma to the skin or chronic like a venous stasis or diabetic ulcer.ACUTE WOUNDSTypically, due to some form of trauma.May be blunt or penetrating causes with different array of sizes, depths, and locations.Abrasion, puncture, crush, burns, gunshot, animal bites, surgery, and other etiologies that cause initially intact skin to break down. CHRONIC WOUNDSAny mechanism that decreases blood flow in the skin for a prolonged period of time has the potential to cause ischemic breakdown of the skin. Skin perfusion may be impaired due to:proximal arterial obstruction (peripheral artery disease)vascular compression (hematoma, immobility causing focal pressure)microvascular occlusion or thrombosis (vasculitis, cholesterol crystals) venous or neuropathic ulcers like in diabetic patients.FACTORS THAT INHIBIT OR AFFECT WOUND HEALING:Infection: Bacterial infection produce multiple inflammatory mediators that inhibit wound healing. The inflammatory phase of healing is prolonged and disrupted, there is depletion of the components of the complement cascade, disruption of the clotting mechanisms, disordered leukocyte function, less efficient angiogenesis and formation of friable granulation tissue. New tissue growth cannot occur in the presence of inflammation or necrotic tissue, and the presence of necrotic tissue promotes bacterial proliferation. A wound that is infected has an unbalanced host-bacteria relationship, because you cannot get rid of all the bacteria on the surface of a wound, but you can establish an equilibrium to promote healing. In 1980 Bucknall published an experiment with rats showing how the granulation tissue looks in an infected wound: There is an increased in hydroxyproline (collagen) and abundant new vessel formation(1). It was interesting for me to know that because I thought those processes ere inhibited but actually, they are increased but are disorganized, resulting in a granulation tissue that is disorganized and friable. Smoking: Nicotine and other chemicals in tobacco impair wound healing by inducing vasoconstriction causing relative ischemia on tissues, also by reducing inflammatory response, impairing bactericidal mechanisms, and altering collagen metabolism. Smoking is associated with postoperative wound healing complications, which occur more often in smokers compared with non-smokers as well as in former smokers compared with those who never smoked(2).Aging: Likely due to comorbidities such as diabetes, peripheral artery disease, chronic venous insufficiency, and lower serum protein levels causing lower collagen. Lower collagen in the body slows down wound healing. For example, Kennedy pressure ulcer.Kennedy Ulcer: It is a dark sore that develops rapidly during the final stages of a person’s life. Not everyone experiences these ulcers in their final days and hours, but they’re not uncommon. They are different from pressure sores or bed sores; because they develop rapidly; they are typically located in the sacral area are necrotic(3). Malnutrition: Patients with hypoalbuminemia tend to be more prone to infection, and infection as we said, affects wound healing. Prealbumin and albumin are not perfect markers of wound healing but helpful specially for patients with non-healing wounds. Diabetes mellitus: Causes several factors that contribute to impaired wound healing: Decreased or impaired growth factor production, angiogenic response; macrophage function, collagen accumulation, quantity of granulation tissue, dysfunctional keratinocyte and fibroblast migration and proliferation. Also, diabetes causes neuropathy and vasculopathy. Trying to explain the pathophysiology of slow und healing in diabetes would take a lecture by itself. Obesity: The cause of wound complications in obese individuals may be secondary to decrease vascularity of the subcutaneous tissue which may impair antibiotic delivery and increased wound tension. Poor skin circulation also makes obese individuals prone to pressure injury which can be aggravated by difficulties in repositioning and increased shearing during movement.Others: Vascular disease (PAD, CVI), immunosuppressive therapy, edema, size and depth of the wound, autoimmune diseases, vasculitis, and many medications. It is impossible to cover all the factors but my message to residents is: When a wound is not healing, think about the most common factors interfering with healing and take them out of the way! Mainly infection, smoking, diabetes and malnutrition.DIFFERENT WAYS TO HEAL WOUNDS: Wound Debridement: It is the removal of non-viable tissue, contaminants, or foreign body to expose healthy wound bed to assist with wound healing. Devitalized tissue refers to slough or eschar. The body normally uses phagocytosis and autolysis to get rid of devitalized tissues, but in some instances those processes are impaired, so we have to assist with debridement. These can be accomplished by different means: surgical (scalpels, scissors, electrocautery), irrigation, chemical (soaps, detergents), enzymatic (collagenases, fibrolysin, DNAsses, i.e. Santyl), and biosurgical (maggots). The point is to eliminate all the dead tissue because it is basically “on the way” and can potentially be an environment where bacteria can thrive. These “excess” tissues are not going to regenerate and need to be removed. Another way to put it is: “Debridement is turning a chronic wound in an acute wound with more potential for healing.” Moist-to-dry dressings: Mechanical debridement can be accomplished by moist-to-dry dressings. We need the proper amount of moisture to promote moist healing environment. Too much moisture or “wet gauze” provides more than needed moisture and it may cause more harm than good, for example, maceration. Get the gauze wet and squeeze it and that should be enough moisture for the wound. Irrigation: Irrigation is a way to remove bacteria and debris. It should be a part of routine wound management. Low-pressure irrigation is done with a syringe or bulb, and high-pressure irrigation is typically performed in the OR with a commercial device. There is no high-level evidence to support the use of any particular additive to the irrigant, nor any particular additive over another. The act of irrigation and the volume of irrigant probably provides the positive benefits. Warm, isotonic (normal) saline is typically used; however, systematic reviews have found no significant differences in rates of infection for tap water compared with saline for wound cleansing. The addition of dilute iodine or other antiseptic solutions (eg, chlorhexidine, hydrogen peroxide, sodium hypochlorite) is generally unnecessary. Such additives have minimal action against bacteria, and some, but not all, may impede wound healing(4).Certification in wound care: As residents, you can start by taking elective rotations on wound care, or attend wound care trainings available for doctors, nurses, physical therapists and other health care professionals. The process results in a Wound Care Certified Certification, but you need an unrestricted license as an MD or RN, or other profession. You need FIRST education and SECOND experience before you can sit for the certification exam. Healing wounds is very rewarding. This knowledge is important for me because I want to help my patients with acute and chronic wounds. We encounter patients in the hospital and in the clinic with multiple kinds of wounds, and I feel I can help many people with this knowledge. Question number 4: How did you get that knowledge? I got this knowledge from attending wound care trainings here in Kern County, from reading lecture notes and books, and from years of experience caring for patients with various and complex wounds.Question number 5: Where did that knowledge come from? What IS that? Well, it’s a pain in the butt. Literally. Proctalgia fugax is a functional anorectal disorder characterized by severe, intermittent episodes of rectal pain that are self-limited. In Latin, fugax means “fleeting.” Patients with proctalgia fugax have attacks of severe anorectal pain that lasts from seconds to minutes, with an average duration of five minutes. The patient with proctalgia fugax is completely asymptomatic between episodes. The diagnosis requires that all other causes of rectal or anal pain are excluded. Proctalgia fugax is estimated to affect 4-18% of the general population, but only about 20% of those affected actually report their symptoms to a physician. Proctalgia fugax usually affects those between 30 and 60 years of age and is more common among women. Some studies have suggested that the pain may be precipitated by stress, sitting, intercourse, defecation, or menstruation, but in many cases is unknown. This week’s word is “ronquera”. “Ronquera” means hoarseness, or if you want to get more technical it is called dysphonia. In Spanish, people usually use this word when they are complaining of having a hoarse voice. So you may have a patient coming to you saying “Doctor, tengo ronquera,” or “Doctor, estoy ronco,” meaning “Doctor, my voice is hoarse or I have a raspy voice”. At this point you can ask the usual questions of how long? Any precipitating factors? And more. Some people might come in and sound hoarse, in which case you can always ask: Tiene ronquera? To find out if this is abnormal for them or if this is their baseline. Now you know the Spanish word of the day, ronquera or ronco, all you have to do is go and assess your patient’s hoarse voice. Performance a) “Performance can mean many things: abrasion, stain resistance, light fastness, flame retardance.” b) Abrasion: The Wyzenbeek testing method is the most common abrasion testing method in the U.S. A fabric is stretched onto a machine and rubbed back and forth, with signs of wearability checked every 5,000 rubs. c) Stain Resistance – Crypton, Indoor/Outdoor/Microfiber d) Light Fastness - To evaluate fade resistance, a strip of fabric is placed in a large device with part of the fabric protected by a card. The remaining fabric gets a 40-hour blast of light exposure, and results are measured from 1 to 5. A score of 4 is generally considered good for normal residential use. e) Flame Retardance - Almost all the tests in this category test for flammability by holding a fabric to a cigarette or flame to see how it responds. III. Best places to use performance fabric a) Kitchen and Dining Rooms b) Family Room Upholstery c) Custom shower curtains d) Sunny spots e) Outdoor cushions, awnings, curtains IV. Crypton - each fiber of Crypton Fabric is encapsulated with a patented technology that never breaks down its resistance to stains, moisture, mildew and odors. But if you want your leather to stay pristine, keep your pets off of it. d) If you have a shedding animal, match the fabric to their hair color. e) As a general rule for choosing pet-friendly fabrics, look for fabrics that have a tight weave and a pattern. On this episode of the Krypt we are going to talk about BDSM on a budget but first, I have to welcome my amazing co-host, Funsize.So the reason we chose to do this episode now is that Funsize found an infographic on Pinterest about BDSM on a budget and saw a huge safety flaw. On this infographic, it suggest several things that are just fine, however one thing it suggest in particular is the use of household candles. This is a huge no, because household candles have a higher melting point than the candle we in the lifestyle use for wax play, so you will burn your partner if you use these. It’s just a huge safety problem, so today we really want to get into how to have safe, sane, budget friendly BDSM. http://www.ucollectinfographics.com/bdsm-on-a-budget-household-items-that-serve-as-cheap-alternatives-to-toys/ Pervertables- These are items you can convert and pervert for your play, which are not traditionally used in this way. Paddles at home:Spatulas are one of my favorite to be used as paddles. Just be sure that you use the flat side and avoid the edges as they may be sharper than they appear when used in impact play. Hair brushes. Not only does the flat side work as a cheap on the spot paddle, but the bristles can create a marvelous sensory feast, especially when combined with a blindfold. (Which can be made from a silk tie.) Rolled up magazines or newspapers create a mild thud and are great fun, especially if you are delving into pet play. Paint stirrer sticks. Often you can get these really cheap or for free at your local hardware store. Flogging fun:Paracord made into a flogger. If you’re looking to make your own paracord is a great place to start, and much cheaper than leather as far as materials go. There are tons of diy patterns for making your own flogger on Fetlife, Pinterest, and the internet in general. I recently came across a flogger made from an old yoga mat, cut into thick one inch strips and tied for the mop. I’m actually dying to try this out myself. Strips of spare cloth with beads tied to the tails can be arranged into a pretty neat makeshift flogger. Other impact play toys at home:Bamboo or other wooden switches can either be picked up at your local craft shop, or simply from a tree outside. Either way remember to sand the wood to get rid of any splinters. You can also make a handle out of simple bits of scrap fabric and electrical tape. One of my first experiences which I can pinpoint as a moment of pure masochism was when I was a child and ended up taking shelter under a willow tree during a storm. The branches lashed me as well as any single tail ever has. So if you have a willow tree on hand, cut off a switch and play with care. Wet towels or washcloths. You know how in movies during a locker room scene someone will snap someone else with a towel…. Same theory, only when wet they sting a lot more so there’s a lot of fun to be had there. Leather gloves… Irishmtdragon surprised me with this one recently and I’ve got to say it was actually a lot of fun; fairly light for impact but the stroke of the leather alone was mmm so nice. Pincers and clamps: Clothespins, such a marvelous pervertible, and so cheap. And if you’ve ever seen pictures of those pretty zipper patterns, well good chance they’re from clothespins. Chip bag clamps. A lot of times you can find these wider clamps and well they are amazing for the uh… lower lips on women. They can create the pinch and hold, but also provide more surface area and therefore less pain too. Combined with using some creatively placed tape, and you’re bound to have a good time. Other sensory toys you already have:Ice cubes, need we say more. If you haven’t explored temperature play, pull out some ice cubes and grab a towel from the dryer, switch between the two and watch your bottom squirm. Safety pins. Tape three or four together with the points open with a pencil as the handle. Using these to trace over your partner's skin lightly can raise all sorts of goosebumps. Sand paper. Abrasion play isn’t for everyone and should be approached carefully for those inexperienced in it. Back scratchers. Irish and I have two, one with metal claws and one wooden. Both are better for teasing than any feathers we’ve come across. Spices, stings, and rice. Tabasco…. Woo that stings! Ginger… see figging. Bags of uncooked rice. When I was being trained I was instructed to sit on a 5 lb. bag of uncooked rice for as long as needed until I learned my lesson. I still haven’t forgotten that lesson or how much that rice stabbing into my knees stung. If you don’t have that much on hand you can spread a cup around inside an empty pillow case for much the same effect. Diy and Dom DepotOften Home Depot is referred to as Dom Depot but you can also make your furnishings and toys. Some of the things you can pick up from Dom Depot are eyelet hooks. Screw a few of these into chairs, foot stools, or sturdy chests and you’ve got your own at home bondage furniture. Things like this are also pretty easy to hide in plain sight if the discretion of your home is anything you need worry about. If suspension is your play of choice you can create your own free-standing beam. Making your own will cost 1/10th of what buying one would cost. Using a cheap plastic tablecloth or tarp and duct tape you can create a protective cover for your furniture in place of sport sheets. If you don’t have a piece of furniture you feel comfortable modifying already, check out your local thrift shops. It’s important to explore what you have on hand and be safe as always. In dieser Arbeit wurde die Verschleißfestigkeit von einem neu auf dem Markt erhältlichen, modifiziertem Glasionomerzement Chemfil Rock von der Firma Dentsply im Vergleich zu bereits in der Praxis bewährten Glasionomerzementen untersucht. Des Weiteren wurde in dieser Arbeit getestet, ob sich die Verschleißfestigkeit von Glasionomerzementen durch das Auftragen von Oberflächenschutzlacken, sog. Coatings, erhöhen lässt und ob Coatings die Verschleißfestigkeit durch eine Verbesserung der Maturation der Glasionomerzemente oder durch einen erhöhten Schutz gegenüber Abrasion steigern. Da das Coating während der Demastikation zügig von der Oberfläche entfernt wird, wurde in dieser Studie neben der antiabrasiven Wirkung des Coatings auch der Einfluß des Coatings auf den darunterliegenden Zement untersucht, nachdem dieses von der Oberfläche entfernt worden war. Hierfür wurden zwei Coatings verwendet: G- Coat-Plus von Gc Corporation und Ketac Glaze von der Firma 3M Espe. Die Verschleißprüfung der Materialproben wurde in der ACTA-Maschine der (Firma Willytec, München) als Drei-Körper-Verschleiß durchgeführt, die als internationaler Standard gilt (De Gee 1994). Alle Proben wurden nach der Verschleißprüfung mit einem neuen, optischen 3-dimensionalen LED-Scanner mit konfokalem Sensor zur optischen Abtastung der Materialoberfläche und Evaluierung des Materialverlustes an der Probenoberfläche vermessen und die Daten anschließend mit dem Programm Match 3D ausgewertet. Um die mikromorphologischen Verschleißmechanismen zu verstehen, wurden im Anschluss Aufnahmen mit dem Rasterelektronenmikroskop erstellt. Die Verschleißfestigkeit des Glasionomerzements Chemfil Rock profitierte nicht durch die veränderte Füllstoff- und Polymertechnologie, denn dieser Glasionomerzement erreichte mit Abstand die höchsten Verschleißwerte. Die in der Literatur beschriebene deutliche Verbesserung der Biegefestigkeit von Glasionomerzementen durch die Applikation von Coatings wird in dieser Studie nicht von einer Verbesserung der Verschleißfestigkeit begleitet, im Gegenteil, es wurden überraschenderweise höhere Verschleißwerte im Vergleich zu den nicht gecoateten Materialien gemessen. Nachdem das Coating von der Oberfläche abradiert wurde, hatte es keinen positiven Effekt auf den darunterliegenden Glasionomerzement. Auch eine verlängerte Lagerungszeit verbesserte die Materialeigenschaften nicht. Jedoch zeigten die Aufnahmen im Rasterelektronemmikroskop, dass das Coating den Glasionomerzement versiegelt, was die Verbesserung der Biegefestigkeit erklären könnte.

In der prospektiven klinischen Studie zum Langzeitverhalten von IPS Empress I Keramikinlays konnten nach einer Liegedauer von 10 Jahren bei 31 Patienten insgesamt 62 Rekonstruktionen beurteilt werden; 51 Inlays waren noch in situ. Die 10-Jahres-Untersuchung umfasste die klinischen Parameter Ästhetik, Oberflächenqualität, Randqualität, Randverfärbung, Sekundärkaries, Mundhygieneparameter, Attachmentverlust, Vitalität, Okklusionsstellung und Abrasion. Weitere Fragen bezogen sich auf die Beschwerdeanamnese, Therapiemaßnahmen und Misserfolgsgründe. Die Auswertung erfolgte mittels der SPSS 15.0- Family Software. Die Überlebensrate nach Kaplan-Meier für 10 Jahre betrug 82,3%, die mittlere Überlebenszeit lag bei 122,9 Monaten. Misserfolgsgründe waren in 72,7% der Fälle pulpitische Beschwerden sowie Randdesintegration und Fraktur (27,3%). Von den noch in situ befindlichen Inlays wiesen weitere fünf (Rand-)frakturen auf; an vier Zähnen wurde Sekundärkaries im Randbereich diagnostiziert. Ein signifikanter Zusammenhang zwischen Randqualität und Randverfärbung, sowie Randqualität und Sekundärkariesbildung konnte nachgewiesen werden. Inlays im Prämolarenbereich wiesen signifikant bessere Randqualität und Überlebensraten auf. Die Lage der Inlays in Ober- oder Unterkiefer und die Wahl des Befestigungskomposits (Variolink bzw. Sonocem) hatten keinen signifikanten Einfluss auf die Überlebenszeit. Zweiflächige Inlays schnitten tendenziell besser ab als dreiflächige. Die gute Biokompatibilität der Keramik konnte bestätigt werden.

In einer prospektiven klinischen Studie sollte die Abrasion von IPS-Empress Einlagefüllungen und dem umgebenden Zahnschmelz untersucht werden. Bei der klinischen Nachuntersuchung nach 10 Jahren war aufgefallen, dass die eingegliederten Keramikinlays deutlich über den Zahnschmelz hinausragten. Es sollte deshalb die Differenz aus der ursprünglichen Zahnoberfläche und dem jetzigen Zustand vermessen werden. Material und Methode: Bei 27 Patienten wurden 54 2 und 3-flächige rein schmelzbegrenzte Keramikinlays eingegliedert und nach der Eingliederung Präzisionsabformungen und Epoxydharzmodelle erstellt. Nach 10 Jahren wurden die verbliebenen 51 Inlays erneut nachuntersucht (mittlerer Liegedauer 10,2 a) und ebenfalls Harzmodelle erstellt. Mittels eines 3-D Laserscanners (Willytec, Gräfelfing/Germany) wurden die Ursprungsmodelle digitalisiert und als Baseline festgelegt und die 10-Jahresmodelle virtuell abgezogen. Man erhielt daraus den Substanzverlust an den gemessenen Bereichen. Insgesamt wurden 1811645 Punkte auf den 51 Keramikinlays und 1173873 Punkte im umgebenden Zahnschmelz vermessen. Ergebnisse: Die mittlere klinische Abrasion des Zahnschmelzes um das Keramikinlay lag bei 68,15 µm (SD 58,21), die Abrasion der keramischen Einlagefüllung nur bei 16,18 µm (SD 26,22). Die Ergebnisse waren signifikant unterschiedlich p = 0,000 (Mann-Whitney-Test). Schlussfolgerung: Die Leuzit-verstärkte IPS-Empress Presskeramik scheint sich auch im klinischen Einsatz deutlich weniger zu abradieren als der umgebende Zahnschmelz

Die Prophylaxe gewinnt nicht zuletzt auch angesichts des notwendigen finanziellen und gesellschaftlichen Sparpotentials immer stärker an Bedeutung; nicht zu Unrecht, da schließlich die meisten kariösen und parodontalen Läsionen durch rechtzeitige, fachgerecht und regelmäßig durchgeführte Prophylaxemaßnahmen in ihrer Entstehung vermieden werden könnten, so auch gesundheitliches Wohlbefinden fördernd. Das nach wie vor verbreitetste und am häufigsten angewandte „Prophylaxe-Werkzeug“ ist zweifellos die Handzahnbürste. Die vorliegende Untersuchung setzte sich zum Ziel, Unterschiede bezüglich der Dentinabrasivität zwischen verschiedenen aktuellen Handzahnbürsten mit hohem Marktdurchsatz zu ermitteln. Wie sich herausstellte, erwiesen sich die ausgewählten modernen Handzahnbürsten mit ebensolchem Bürstenkopfdesign ausnahmslos als signifikant weniger abrasiv im Vergleich zu der Referenzzahnbürste, der „ADA Control“. Die ADA Control - Zahnbürste verfügt dabei über ein einfaches Design mit geraden Borsten und planem Borstenfeld. Bereits dieses Ergebnis zeigt, eine wie rasante Entwicklung die Handzahnbürsten in den letzten Jahren genommen haben. Nicht nur das Aussehen hat dabei eine spürbare Verbesserung erfahren und sich dem Zeitgeschmack angepaßt, sondern auch die Eigenschaften wurden offensichtlich (hier die Abrasivität) deutlich verbessert. Im gesamten Testfeld zeigten drei Zahnbürsten auffallend günstige Eigenschaften in Bezug auf den Dentinverschleiß. Die „Dr. Best Brillant sensitive“, die „Oral B Advantage Plus“ und die „Dr. Best X-Sensorkopf sensitive“ zeigten die geringsten Dentinabrasionstiefen. Das Ergebnis läßt den Schluß zu, daß diese Bürsten Patienten mit parodontal vorgeschädigtem Gebiß besonders zu empfehlen sind. Gegen diese Empfehlung ist aus Sicht der Abrasionswirkung auch gewiß nichts einzuwenden, Aussagen über die Reinigungswirkung lassen sich daraus jedoch nicht ableiten. Es ist durchaus denkbar (aber keinesfalls sicher, bewegen wir uns mit folgender Schlußfolgerung doch im spekulativen Raum), daß die Bürsten mit der geringsten abrasiven Wirkung nur eine unterdurchschnittliche Reinigungswirkung zeigen. Hier liegt ein Schwachpunkt der Ergebnisse. Um zuverlässige Empfehlungen aussprechen zu können, sollte zukünftig parallel zum Verschleiß an der Zahnoberfläche auch die Reinigungswirkung getestet werden. Schließlich ist in der Reinigung der eigentliche Zweck der Handzahnbürste zu sehen! Ein weiteres Anliegen dieser Untersuchung war es, eine validierbare und zuverlässige Methode zur Testung von Zahnbürsten zu entwickeln. Um die Leistungsfähigkeit des Versuchsaufbaues zu verbessern, mußten exemplarisch zahlreiche Modifikationen an der „Zahnputz-Maschine“ vorgenommen werden. Es wurden beispielsweise neue Halterungen für die Handzahnbürsten entworfen, da die neuen Bürsten über teilweise sehr dicke, ergonomisch gestaltete Handgriffe verfügen. Die alten Halter waren für obige Studie wertlos. Ebenso mußten die Eichgewichte am Putzarm neu hergestellt und in ihrer Bauform geändert werden, um unseren Ansprüchen gerecht zu werden. Die Abrasiv-Slurry-Bäder wurden modifiziert, um einen reibungslosen Versuchsablauf zu gewährleisten. Auch Adapter, die sicherstellen, daß die Zahnbürsten während des gesamten Versuches in der vollen Länge des Bürstenkopfes aufliegen, wurden erdacht und hergestellt. Selbst die Matrize zur Probeneinbettung war zu ändern, die Schlitzfräsung wurde von 14mm auf Maximallänge von ca. 20mm vergrößert, um den Einbau größerer Proben zu ermöglichen, anderenfalls hätten zur Auswertung keine ungeputzten Dentinareale als Referenzflächen mehr genutzt werden können. Da sich die Matrize als tauglich erwiesen hat, sollte eine weitere Matrizenvariante aus dauerhaftem Stahl hergestellt werden und die mechanisch anfällige Variante aus Aluminium ersetzen. Im Rückblick sei auch kritisch vermerkt, daß sich der Nachschub an extrahierten menschlichen Zähnen zur Dentinprobengewinnung problematisch darstellt, herrscht doch oft Mangel an geeigneten Zähnen. Zudem lassen sich Störfaktoren von außen oder Lagerungsfehler nur schwer beeinflussen oder erkennen, da man notwendigerweise auf viele verschiedene Quellen zurückgreifen muß. Beispielsweise wurden stark gebleicht aussehende Zähne nicht zur Probengewinnung herangezogen, um Fehler zu vermeiden. Vermutlich wurden diese Zähne zu irgendeinem Zeitpunkt nicht in physiologischer Kochsalzlösung, sondern in Wasserstoffperoxid gelagert. Trotz Rückfragen war nicht verifizierbar, ob es tatsächlich zu solchen Lagerungsfehlern gekommen war. Obgleich die Verwendung von humanem Dentin sehr realistische Ergebnisse erwarten läßt, stellt sich jedoch die Frage, ob interindividuelle Unterschiede in der Dentinfestigkeit nicht zufällige Fehler der Ergebnisse bedingen. Bei bovinem Dentin hingegen bestehen kaum Nachschubprobleme. Außerdem lassen sich Proben in nahezu beliebiger Größe wählen und verändern. Nicht zuletzt ist es möglich, aus einem Rinderzahn, sogar aus ein und derselben Dentinschicht mehrere Dentinproben zu gewinnen, die in ihren Eigenschaften geradezu identisch sind. Ein weiterer Vorteil bovinen Dentins ist die wesentlich geringere Infektionsgefahr für den Experimentator im Gegensatz zu menschlichem Dentin, sieht man von der Bovinen Spongoencephalopathie (BSE) ab. Die Gleichwertigkeit bovinen Dentins im Vergleich zu humanem Dentin beim Einsatz in Abrasionsversuchen ist inzwischen wissenschaftlich gesichert (Imfeld, 2001). Daher sollte in zukünftigen Untersuchungen vorrangig bovines Dentin als Probenmaterial eingesetzt werden. Die vorliegenden Ergebnisse eröffnen insgesamt die Möglichkeit zu weite-ren gezielten Untersuchungen, von Zahnbürsten-Vergleichen bezüglich Abrasivität und Reinigungswirkung sowie zu Studien mit Blick auf weitere Wirkmechanismen und Interdependenzen von Abrasivität und Reinigungswirkung.

Ziel dieser auf sechs Monate terminierten Studie war die Ermittlung und der Vergleich der Veränderungen an Zahnhart- und Weichgewebe an ausgewählten Zähnen durch den Gebrauch der elektrischen Zahnbürste Sonicare Advance (® Fa. Philips) und einer durch die ADA empfohlenen Referenzzahnbürste. Beide Zahnbürsten wurden bezüglich ihrer Effizienz innerhalb von Sitzungen (kurzfristig) als auch über den Testzeitraum von drei Monaten pro Zahnbürste im Cross-over-Design untersucht. Die Evaluierung der Indices und der daraus gewonnenen Daten ergab, dass beide Zahnbürsten jeweils zu einer Reduktion des Weichgewebevolumens als auch der Plaque - sowohl innerhalb der Sitzungen als auch innerhalb der drei Monate - führten. Die Wash-out-Phase von sieben Tagen sollte mundhygienebedingte Auswirkungen auf die zweite Testphase vermeiden und Carry-over-Effekte minimieren. Mit Hilfe der oralen Metrologie wurden anhand der gewonnenen Abdrücke der untersuchten Zähne, die daraus resultierenden Daten mit den Ergebnissen der Plaque- und Gingivitisindices sowie die Abrasion am Hartgewebe verglichen und objektiv verifiziert. Durch die laseroptische Abtastung der Modelle wurden Ergebnisse erzielt, die aufzeigten, dass beide Zahnbürsten zu einer Reduktion des Zahnhart- und Weichgewebevolumens führten, hierbei war die verursachte Abrasion und Weichgewebereduktion durch die ADA-Zahnbürste größer. Weiteres Ziel dieser Studie war die Bewertung einer neu eingeführten Methode der Abdrucknahme zur Erstellung von hochpräzisen Replikamodellen für das darauffolgende laseroptische 3D-Messverfahren.

In einer In-vitro-Untersuchung sollten Erkenntnisse über den Verschleißvorgang kommerzieller und experimenteller Füllungsmaterialien gewonnen werden. Schwerpunkte der Studie waren die Verschleißquantifizierung, der Einfluss elastischer Eigenschaften auf die Verschleißfestigkeit und die zugrunde liegenden mikromorphologischen Verschleißmuster. Zur Erstellung einer Datenbasis wurden kommerzielle Komposite mit unterschiedlichen rheologischen Eigenschaften verwendet. Von der Firma Degussa/Dentsply wurden Blindproben experimenteller Komposite mit Füllkörpervariationen des Hydroxylapatit- und Präpolymerisatgehaltes sowie experimentelle Komposite mit differierendem Anteil an porösen Füllkörpern und unterschiedlichem Silanisierungsgrad zur Verfügung gestellt. Die Kompositmatrix war für beide experimentellen Gruppen unterschiedlich. Die Verschleißprüfung der Materialproben wurde in der ACTA-Maschine (Fa. Willytec, München) als Dreikörperverschleiß durchgeführt, die als internationaler Standard gilt [De Gee, 1994]. Die Oberflächenermüdung der Materialien wurde von den Verschleißmechanismen Abrasion und Erosion in der ACTA-Maschine überlagert, daher erfolgte zusätzlich ein Zweikörperverschleiß zur Oberflächenermüdung im Fatigue-Tester (Fa. Willytec, München). Alle Proben wurden nach der Verschleißprüfung mit einem computergesteuerten Perthometer vermessen. Daneben wurden 60 ausgewählte Proben vergleichend mit einem neu entwickelten Probeneinsatz (Fa. Willytec, München) im Laserscanner dreidimensional erfasst und, gleich den Perthometerdaten, mit dem Programm Match 3D ausgewertet. Von allen Materialien wurde mit Hilfe des Fischerscopes (Fa. Fischer, Sindelfingen) der Elastizitätsmodul ermittelt. Zur Ergänzung der Verschleißdaten wurden Aufnahmen der Probenoberflächen mit dem Rasterelektronenmikroskop erstellt, um einen Einblick in mikromorphologische Verschleißmechanismen zu erhalten. Das Verschleißverhalten der Komposite wurde von den Füllkörpern dominiert, mit vernachlässigbarem Einfluss der Matrix. Bei den kommerziellen Kompositen war aufgrund der hohen E-Modul-Varianz eine Korrelation der rheologischen Eigenschaften und des Verschleißverhaltens gegeben. Die Verschleißdaten der experimentellen Materialien korrelierten nicht mit dem E-Modul der Materialien. Der prozentual geringe Hydroxylapatit- und Präpolymerisatgehalt führte bei den experimentellen Kompositen gegenüber der Kontrollgruppe ohne ihren Zusatz nicht zu einer höheren Verschleißfestigkeit, was auf einen frühzeitigen Verlust des unsilanisierten Hydroxylapatites und den überlagernden Einfluss der Glasfüllkörper zurückzuführen ist. Die porösen Füller der experimentellen Komposite beeinflussten das Verschleißverhalten in beiden Testreihen unterschiedlich. Während in der ACTA-Maschine kein oder ein geringer Anteil an porösen Füllkörpern zu einem signifikant schlechtem Verschleißverhalten führte, ist diese Korrelation nach der Oberflächenermüdung im Fatigue-Tester nicht gegeben. Kein direkter Einfluss auf den Verschleiß der Komposite konnte der Silankonzentration der porösen Füllkörper zugeschrieben werden. Das Komposit mit 12 Gew.% porösen Füllern und zehnprozentiger Silanisierung schien die optimale Zusammensetzung in Bezug auf die Verschleißresistenz zu sein. Die rasterelektronenmikroskopischen Aufnahmen der experimentellen Materialien zeigten füllkörperdominierte Oberflächendefekte. Neben Füllkörperfehlstellungen, Matrixauswaschung an Korngrenzen oder dem Lösen von Glasfüllkörpern, wurde bei den experimentellen Kompositen der Verlust von Hydroxylapatitpartikeln beobachtet. Bei den experimentellen Kompositen mit porösen Füllkörpern war die Degradation der großen porösen Gläser in verschiedenen Stufen zu erkennen. Angefangen mit Rissen auf der Oberfläche und angelösten Randbereichen, traten Füllkörperfrakturen und der Verlust ganzer Füllkörper auf. In der vorliegenden Arbeit konnten interessante Wege der Verbesserung der Verschleißfestigkeit von Kompositen gewonnen werden und Verschleißmuster nach Zweikörper- und Dreikörperbelastung belegt werden.

In der vorliegenden Dissertation wurde der Einfluss von harten und krossen Nahrungsmitteln wie z. B. Müsli oder Brötchen auf die Lebensdauer von Fül-lungskompositen untersucht und eine Versuchsanordnung zur Erzeugung von In-vitro-Verschleiß an Kompositen erarbeitet. In Vorversuchen wurden in einer Universalprüfmaschine verschiedene harte Nahrungsmittel Bruchtests unterzogen und die dabei auftretenden Kräfte gemes-sen. Die Morphologie der verwendeten Kauflächen zeigte einen signifikanten Einfluss auf die Höhe der Kraft (p=0,007), nicht jedoch die Geschwindigkeit, mit der die Belastung erfolgte (p=0,494). Beim Zerbeißen auf einer flachen Plattform trat mit 355,5 N (± 200,5) bei Bonbons die höchste Bruchkraft aller getesteten Nahrungsmittel auf, mit einer Keramikkrone als Unterkieferzahn bei Popcornmais (209,4 N ± 120,8), der durch die Kaufläche abgestützt wurde. Bei Verwendung der Keramikkrone zerbrachen die Bonbons aufgrund punktförmi-ger Kraftinduktion bereits bei 138,2 N (± 38,9). In den Hauptversuchen wurden Kompositproben in einem Kausimulator jeweils 50.000 Kauzyklen unterzogen. Dies geschah bei der einen Hälfte der Proben unter Verwendung des von der ACTA-Maschine bekannten Abrasivmediums aus Hirse, bei der anderen Hälfte wurde Wasser zugegeben. Bei allen Komposi-ten wurde der Substanzverlust durch Zugabe der Hirse gesteigert, es änderte sich jedoch die Rangfolge bezüglich des Verschleißes. Bei den Versuchen mit Was-ser zeigten Solitaire (286,8 ± 70,3 mm³E-3) und Tetric Ceram (286,5 ± 198,6 mm³E-3) den größten Substanzverlust, bei Hirseverwendung Solitaire (843,3 ± 435,7 mm³E-3) und Heliomolar RO (788,1 ± 164,4 mm³E-3). Mit Wasser trat bei Heliomolar RO der geringste Verschleiß auf (29,7 ± 6,7 mm³E-3), mit Hirse bei Definite (547,0 ± 187,8 mm³E-3). Die Situation mit Wasser als Medium entspricht einer reinen Knirschbelastung, die vor allem bei pathologischem Bruxismus Verschleiß in okklusalen Kontakt-bereichen (OCA) verursacht. Die Versuchsanordnung mit der ACTA-Suspension entspricht der Belastung beim Kauen von Nahrung. Dabei treten in vivo, vor allem beim Zerkleinern harter und krosser Nahrung, sowohl im Kon-taktbereich als auch in kontaktfreien Bereichen (CFA) erhebliche Belastungen auf, die über Fatigue, Abrasion und überkritische Belastung zu Mikro- und Ma-krofrakturen an Zahnhartsubstanz und Füllungskompositen führen können. Bei In-vivo-Untersuchungen zur Lebensdauer von Kompositfüllungen treten interin-dividuelle Streuungen auf, die auch durch diätetische Einflüsse krosser und har-ter Nahrungsmittel zu erklären sind. Die Versuche mit der Hirsesuspension ent-sprechen der Durchschnittsbelastung dieser Untersuchungen. In rasterelektronenmikroskopischen Aufnahmen der Kompositproben nach Kau-belastung sowie zweier Kompositbiopsien, die Verschleiß in vivo unterworfen waren, zeigte sich, dass die Kausimulationsanordnung mit Hirse in der Lage ist, die beim Zwei-Körper-Verschleiß entstehenden scholligen Auflagerungen aus herausgelösten Füllkörperpartikeln von der Probenoberfläche zu entfernen. So-mit kann eine weitere Annäherung der Verschleißsimulation im Kausimulator München III an die Situation in vivo erfolgen und so die Belastung für Patienten in klinischen Untersuchungen reduziert werden. Bei der Betrachtung der klini-schen Eignung von neuen Kompositen sind jedoch auch weitere Faktoren wie Brucheigenschaften und Leistungsfähigkeit des verwendeten Adhäsivsystems zu beachten, was klinische Studien weiterhin unumgänglich macht. Zusammenfassend lässt sich ein Einfluss der individuellen Nahrungsauswahl auf die Haltbarkeit von Füllungskompositen feststellen, besonders bei hoher Bela-stung durch das Zerkleinern harter Nahrung. Für die Zukunft erscheint das Vor-gehen sinnvoll, Versuche mit und ohne Abrasivsuspension durchzuführen, au-ßerdem sollten verschiedene Nahrungsmedien auf ihr Verschleißverhalten unter-sucht werden.
