Our conversation with Nadia Bevan continues our series profiling research about women's football. Angela Christian-Wilkes and Nadia delve into Nadia's ethnographic research on developing a women's football team at a club in Melbourne. We discuss the structures that include and exclude within community sport, building inclusive practices, and the messiness of insider research.   Nadia is an early career researcher specialising in the barriers and enablers of access to sport for a range of groups, including women, girls, youth and the LGBTQIA+ community. She completed a PhD in Education at Monash University, looking at weight stigma as a barrier to physical activity.   The Short Corner is a The Far Post's home for all things interesting, different and fun that don't fit under our usual programming of women's football analysis and news.   Paper discussed: Bevan, N., Jeanes, R., & Truskewycz, H. When we're able to get in touch with a Canadian Baseball Hall of Fame and Houston Astros Hall of Fame member, among others, we roll out the red carpet. Such is the case with Terry Puhl, whose career kind of speaks for itself. A 15-year Major Leaguer and 18-year pro, with all but 15 games in the bigs coming with the Astros. He was an MLB All-Star in 1978 and was outstanding, particularly in the field and on the base paths. Puhl helped the Astros win their first ever NL West Division crown and postseason berth in 1980. He now lives in Texas, and we were fortunate to sit down with him to talk about his career this past week. Special thanks to Scott Crawford at the Canadian Baseball Hall of Fame for helping us connect with Terry to line up this conversation!

Mensch in Deutschland statistisch gesehen einmal in seinem Leben behandlungsbedürftig psychologisch erkranken.

The Harm of Weight Stigma

This is the Weight and Healthcare newsletter! If you like what you are reading, please consider subscribing and/or sharing!I've written before about weight stigma in healthcare practice, and I'm currently co-authoring a study with Dr. Lesleigh Owen about the harm weight stigma does to the highest-weight patients. Today I'm going to dig into the research that already exists that explores the harm done to fat* patients by weight stigma. As we get into the research, a few reminders:First, weight stigma is rooted in and inextricable from racism and anti-Blackness, and continues to do disproportionate harm to those communities today. I urge you to read Sabrina Strings' Fearing the Black Body – the Racial Origins of Fat Phobia and Da'Shaun Harrison's Belly of the Beast: The Politics of Anti-Fatness as Anti-Blackness to understand more about this.Second, typically research over-represents privileged people and under-represents (or fails to represent at all) marginalized people, and thus the harm done to them is also under- or un-represented.Third, I wrote here about how weight stigma research is often still rooted in anti-fatness. I recommend reading this to contextualize a lot of the research and researchers I will talk about here. Many of the studies that will be discussed in this article look at perceived weight bias/weight status. In this way, weight stigma can harm people of many different sizes, so just a reminder that weight stigma, in particular structural weight stigma, always does the most harm to those at the highest weights and/or those with multiple marginalized identities.I also want to point out how often the things that are correlated with both weight stigma and body size are blamed on body size itself without any discussion of weight stigma as a strongly researched possible confounding variable (this is also common with weight cycling and healthcare inequalities.)In this piece I'll be looking at studies that show harm to physical and/or mental health, including iatrogenic harm, which means harm that is done by the healthcare system/healthcare practitioners. Weight stigma does much, much more harm in all aspects of the lives of higher-weight people that won't be examined here. Also, this provides brief overviews of the studies included and is far from an exhaustive list. Please feel free to add other examples in the comments. Finally, just a reminder that my policy is that when I refer to studies that perpetuate anti-fatness and harm, I will provide enough information to Google them, but I won't link to them directly. Last last thing before we dig in: studies refer to this variously as weight stigma, weight bias, sizeism, and other less common (sometimes more stigmatizing!) terms, I'm combining those for the purpose of this article.Content note: This section will discuss physical and psychological harm that can result from weight stigma, including bias and self-harm, and may be harmful/triggering to higher-weight people, so please take care of yourselfI think therefore I am: Perceived ideal weight as a determinant of health, Muennig et al., 2008This study looked at whether stress related to negative body image perception and the desire to lose weight could impact health, and found that the difference between actual and desired body weight was a stronger predictor of physical and mental health than actual body mass index (BMI) in cis women. “[cis] Women who say they feel they are too heavy suffer more mental and physical illness than women who say they feel fine about their size - no matter what they weigh.”The body politic: the relationship between stigma and ob*sity-associated disease, Muenniug, 2008This work examines the relationship between weight stigma and health issues, and finds that weight stigma may drive health issues that are typically blamed on body size.“Stigma and prejudice are intensely stressful. Over time, such chronic stress can lead to high blood pressure and diabetes.”The Weight of Stigma: Cortisol reactivity to manipulated weight stigma, Himmelstein et. al:Young women who were told in an experiment that their weights “weren't ideal” experienced higher levels of the stress hormone cortisol, regardless of their actual size.“Experiencing weight stigma was stressful for participants who perceived themselves as heavy, regardless of their BMI. These results are important because stress and cortisol are linked to deleterious health outcomes...”Associations between perceived weight discrimination and the prevalence of psychiatric disorders in the general population, Hatzenbuehler, 2009* Perceived weight discrimination is associated with substantial psychiatric morbidity and comorbidity.* The results remained significant after adjusting for perceived stress (a possible confounding variable)* Social support did not buffer against the adverse effects of perceived weight discrimination on mental health.* Controlling for BMI did not diminish the associations, indicating that perceived weight discrimination is potentially harmful to mental health regardless of weight. They concluded that “These results highlight the urgent need for a multifaceted approach to address this important public health issue, including interventions to assist [higher-weight] individuals in coping with the mental health sequelae of perceived weight discrimination.”Weight stigma as a risk factor for suicidality, Brochu, 2020This study examined the connection between subjective perceptions of “overw*ight” and risks of suicidality and found that they were “associated with increased risk of suicidal ideation, plans, and attempt in a large sample of US adolescents, even when controlling for participant age, gender, race/ethnicity, body mass index (BMI), and depression.”I think that this is part of the serious danger of the new AAP guidelines which call for an intense focus on the weight of children, starting to label them as “overw*ght” and “ob*se” as young as two years old.Weight Discrimination and Risk of Mortality, Sutin et al, 2015The study found that* Perceived weight discrimination was associated with an increase in mortality risk of nearly 60%* This increased risk was not accounted for by common physical and psychological risk factors* In addition to its association with poor health outcomes, weight discrimination may shorten life expectancyWeight stigma and health behaviors: evidence from the Eating in America Study, Lee et al., 2021This study points out that “weight stigma is pervasive across the U.S. and is associated with poor health outcomes including all-cause mortality” and seeks to find if weight stigma drives poorer health behavior. (As a reminder, health is an amorphous concept and is not an obligation, barometer of worthiness, or entirely within our control.)The study found that weight stigma was significantly associated with greater disordered eating, sleep disturbance, and alcohol use.Overcoming Weight Bias in the Management of Patients With Diabetes and Ob*sity, Puhl, et al., 2016The health impacts of weight stigma can include:* increased blood pressure, blood sugar, and levels of the stress hormone cortisol* reduced engagement with health care services, reduced trust of health care providers* depression, anxiety, substance abuse, and suicidal tendency* unhealthy eating behaviors, binge eating disorder, and lower motivation for exercise* long-term health impacts including more advanced and poorly controlled chronic disease, and low health-related quality of lifeI want to point out here, again, how much overlap there is with health issues that get blamed on body size, and that studies that link being higher-weight to health issues typically don't even mention weight stigma, let alone control for it.Weight Stigma Among Healthcare Practitioners that Can Impact CareStigma in Practice: Barriers to Health for Fat Women, Lee and Pausé, 2016https://www.frontiersin.org/articles/10.3389/fpsyg.2016.02063/fullI recommend reading this in its entirety. Some of the highlights:* Fat individuals are less likely to access healthcare, and are less likely to receive evidence-based and bias-free healthcare when they do engage* Anti-fat attitudes are found in both practitioners and students, from physicians/doctors, researchers, clinicians, physiotherapists, dieticians, and nurses and  students in the medical and dietetic fields* Negative attitudes about fat patients result in doctors choosing to spend less time with fat patients and often resulting in fewer preventive and diagnostic tests for fat patients* Whether due to improper equipment, lack of education, bias, or an interaction between the three, it is clear that providers also present a barrier to the health and well-being of fat individuals.Nurses' Weight Bias in Caring for Ob*se Patients, Tanneberger & Ciupitu-Plath, 2018* Nurses found higher-weight patients lazy, stupid, noncompliant, less healthy, gluttonous* Biases stemmed from belief that weight is under the individual's control and/or that they would require more resourcesImpact on Nurses* Care was given out of disapproval or condemnationImpact on Patients* Canceling or delaying appointments after experiencing negative attitudes, discourteous treatment, and too small equipment being used during careThe Stigma of Ob*sity: A Review and Update, Puhl and Heuer, 2009* Over 50% of doctors found their higher-weight patients awkward, ugly, weak-willed and unlikely to comply with treatment.* 28% of nurses were “repulsed” by higher-weight patients* 12% of nurses said that they did not want to touch higher-weight patientsWeight bias among health care professionals: A systematic review and meta- analysis, Lawrence et al., 2021This is a systematic review and random- effects meta-analysis of 41 studies. It's important to note the overall quality of these studies was very low and the authors point out the need for better-quality research.“Findings show that medical doctors, nurses, dietitians, psychologists, physiotherapists, occupational therapists, speech pathologists, podiatrists, and exercise physiologists hold implicit and/or explicit weight-biased attitudes toward [higher-weight] patients.”I agree with Lawrence et al that we need more and better-quality research. And I think a major aspect of better quality is research that comes from a perspective that affirms the existence of higher-weight people and not from a perspective that is fully invested in the pathologization and eradication of existing fat people and the prevention of any more from existing. Weight stigma research that is invested in the weight-centric paradigm, as so much of the research above is, both displays and perpetuates additional weight stigma. Did you find this post helpful? Our culture worships thinness and equates it to health and moral virtue. People - particularly women, femmes, trans, people in larger bodies, and people with disabilities - believe that if their body doesn't match the thin ‘ideal' that they're broken and fundamentally flawed. They spend massive amounts of time and energy trying to shrink themselves even though research has proven weight loss isn't sustainable or healthy.  Truth bomb: thinness does not equal health. If this has been a long-held belief of yours, you'll want to listen in for a different perspective. What you'll learn in the episode: In this episode, you'll learn:The myths that cause us to believe fat=unhealthyThe social determinants of healthHow the fat=unhealthy myths are informing your relationship to food and your bodyMentioned on the show: K.M. Merriam-Webster's Word of the Day

Merriam-Webster's Word of the Day for August 1, 2021 is: pulchritude • PUHL-kruh-tood • noun : physical comeliness Examples: The magazine features a photo essay of celebrities who are famed for their Hollywood stardom and pulchritude. "Sadly, Renee's judgment on Mrs. Appleyard's baby's pulchritude, or lack of it, turned out to be true—he was an 'ugly little thing.'" — Kate Atkinson, Life After Life: A Novel, 2013 Did you know? Pulchritude is a descendant of the Latin adjective pulcher, which means "beautiful." Pulcher hasn't exactly been a wellspring of English terms, but it did give English both pulchritude and pulchritudinous, an adjective meaning "attractive" or "beautiful." The verb pulchrify (a synonym of beautify), the noun pulchritudeness (same meaning as pulchritude), and the adjective pulchrous (meaning "fair or beautiful") are other pulcher offspring, but those terms have proved that, in at least some linguistic cases, beauty is fleeting.

Abstract: Several forms of endocannabinoid (eCB) signaling have been described in the dorsal lateral striatum (DLS), however most experimental protocols used to induce plasticity do not recapitulate the firing patterns of striatal-projecting pyramidal neurons in the cortex or firing patterns of striatal medium spiny neurons. Therefore, it is unclear if current models of eCB signaling in the DLS provide a reliable description of mechanisms engaged under physiological conditions. To address this uncertainty, we investigated mechanisms of eCB mobilization following brief synaptic stimulation that mimics in vivo patterns of neural activity in the DLS. To monitor eCB mobilization, the novel genetically encoded fluorescent eCB biosensor, GRABeCB2.0, was expressed in corticostriatal afferents of C57BL6J mice and evoked eCB transients were measured in the DLS using a brain slice photometry technique. We found that brief bouts of synaptic stimulation induce long lasting eCB transients. Inhibition of monoacylglycerol lipase, prolonged the duration of the eCB transient, while inhibition of diacylglycerol lipase inhibited the peak amplitude, suggesting that 2-AG is the predominate eCB generated following brief synaptic stimulation. 2-AG transients were robustly inhibited by AMPA and NMDA receptor antagonists, DNQX and DL-AP5 respectively. Additionally, the 2-AG transient was inhibited by the muscarinic M1 receptor (M1R) antagonist, VU 0255035, and augmented by the M1R positive allosteric modulator, VU 0486846, indicating that acetylcholine (ACh) release is required for efficient 2-AG production. The dopamine D2 receptor (D2R) agonist, quinpirole, inhibited the 2-AG transient. However, in slices from mice lacking D2Rs on cholinergic interneurons (CINs), quinpirole did not inhibit the 2-AG transient, demonstrating that D2Rs on CINs can modulate 2-AG production. The AMPA receptor or NMDA receptor antagonists, DNQX or DL-AP5 respectively, occluded 2-AG augmentation by VU 0486846 suggesting that converging glutamatergic and cholinergic signals are required for efficient 2-AG production following brief synaptic stimulation. Collectively, these data uncover unrecognized mechanisms underlying 2-AG mobilization in the DLS. Copy rights belong to original authors. Visit the link for more info

L., Cai, R., Duan, J., Albarran, E., Ding, J., Lovinger, D. M., Li, B., Soltesz, I., Li, Y. Abstract: Endocannabinoids (eCBs) are retrograde lipid neuromodulators involved in many physiologically important processes. However, their release and dynamics in the brain remain largely unknown, in part due to lack of probes capable of reporting real-time eCBs with sufficient spatiotemporal resolution. Here, we developed a new G protein-coupled receptor Activation Based eCB sensor GRABeCB2.0 using the human CB1 cannabinoid receptor and a circular-permutated EGFP. GRABeCB2.0 exhibited proper cell membrane trafficking, ~seconds kinetics, high specificity and robust fluorescence response to eCBs at physiological concentrations. Using GRABeCB2.0, we detected evoked eCB dynamics in both cultured neurons and acute brain slices. Interestingly, we also observed spontaneous compartmental eCB transients that spread ~11 m in cultured neurons, suggesting locally-restricted eCB signaling. By expressing GRABeCB2.0 in vivo, we readily observed foot-shock elicited and running triggered eCB transients in mouse amygdala and hippocampus, respectively. Lastly, using GRABeCB2.0 in an epilepsy model, we observed a spreading eCB wave following a calcium wave in mouse hippocampus. In summary, GRABeCB2.0 is a powerful new probe to resolve eCB release and dynamics under both physiological and pathological conditions. Copy rights belong to original authors. Dr. Puhl is a leading voice in both research and in policy efforts aimed at reducing weight-based discrimination, stigma, and victimization. She has conducted research on weight stigma for over 16 years and has numerous publications on related topics. She has testified in legislative hearings on weight discrimination and routinely provides expertise on strategies to reduce weight bias to national and international health organizations. Dr. Puhl has received awards for her work from national organizations such as the National Eating Disorders Coalition, the Obesity Action Coalition, and most recently The Obesity Society. Interview Summary Rebecca, you have done very impressive work with the media on how people with obesity are portrayed. Can you remind us again how quickly how weight is portrayed in the media and what you have done to address this? The media is a powerful and pervasive source of weight bias in that people who have larger body sizes are really depicted in negative ways in media that reinforce societal stereotypes like lazy or sloppy or a target of ridicule and this happens in many forms of media. Not just entertaining media but also the news media. And one of the things that struck us early on when we began studying this issue is just how many news reports about obesity are accompanied by stigmatizing images and videos of people with obesity. And we did some research where we looked at hundreds of news reports on obesity across lots of different national news outlets. And we found that essentially over two thirds of the images that accompany these reports were very stigmatizing. And so to try to help change this we created a comprehensive gallery of images and videos that essentially portray children and adults who have obesity in ways that are respectful and positive and non-stigmatizing. We hired professional photographers to do this and we essentially created the first image bank of its kind, which provides these images and videos for free to the media, and also to help professionals or researchers or educators. And the idea here is to try to provide an easy source of images that can help challenge weight bias in the media. And we had a lot of success with this. Our images have appeared in national and even international news outlets. And what I have been really delighted to see over the years, especially recently is other organizations are now following suit. So there are now image banks available in Canada and through the World Obesity Federation. And from my perspective, these types of efforts are really important to try to help shift media portrayals of obesity. But it doesn't really stop there. We try to speak out against the weight bias in the media when we see examples of it to you, those as teachable moments of why it's, it's damaging and harmful. And, you know, I'll be honest, it can be challenging to stay on top of this, especially with weight bias that is now emerging on social media. But it's definitely been often that stick to see more people speaking out against stigma and shaming and calling out the harms of weight stigma when this occurs. And I don't think I could have said that five or 10 years ago. But certainly the voices against the weight stigma and the public awareness of it I think are increasing. In an earlier podcast you mentioned the pretty serious impact of lead by us in medical care settings. And I know you when others have worked on addressing this issue, what's been done in this area? So I think that engaging medical professionals in efforts to combat weight bias is really important. And you know, part of the reason for that is that we know that weight bias is a big problem in healthcare and that we need to really educate providers about weight bias and the harmful impact that it has on our patients. You know, when we think about medical training, you know, topics of obesity and nutrition don't really get curriculum coverage in the way that the need to. And essentially there's no education about weight stigma. And so we've developed a number of educational resources for health care providers to try to help them become aware of weight bias and how it impacts their patients and what they can do about this in the clinical practice. And another reason I think we need to really engage healthcare providers in this way is that they can be very important allies and sources of support to patients who have obesity, who may be experiencing stigma. This is especially true for children who are vulnerable to teasing and victimization because of their weight, not only at school from their peers, but at home from family members. And for kids who experienced weight stigma in both of those settings, health providers, pediatric providers, may be one of the few allies who can really identify this issue and support them. So as an example of a way to kind of address this in the medical community, I coauthored a policy statement with Steven Cotton and other colleagues that was published by the American Academy of Pediatrics in 2017. It essentially aims to increase awareness in the medical community about the harmful effects of weight stigma on children. And it provides guidance for ways that healthcare professionals can address this issue. And what I think was important about this is that not only were there clinical practice recommendations in this policy statement for ways that pediatric providers can reduce weight stigma, but we also made a number of broader advocacy related recommendations of ways that providers can try to help reduce a broader societal weight stigma in children. So for example, we recommended that health professionals work with schools to try to ensure that anti-bullying policies protect students when it to weight based bullying. We called upon health professionals to advocate for training for medical trainees and health professionals. That they're educated about weight stigma and its harmful effects. And to really advocate for responsible portrayals of people with obesity in the media. And so I think efforts like this in the medical field are really important because we need to bring attention to the fact that stigma really does pose a barrier to effective clinical care and treatment. And to find ways to better support patients regardless of their body size. Rebecca, you and your colleagues at the Rudd Center developed a series of videotapes to show the impact of weight bias. Could you explain what these videos they just portray? Sure. As part of our efforts at the Rudd Center to reduce weight stigma, we created three educational videos and these have different target audiences that include health care providers, parents and teachers, and students. And we worked on these videos with an Emmy-award-winning writer, Heather Hale. And we also involved Emme in this project, who is considered to be kind of the world's first plus size model. And we created these films as a way to bring attention to the issue of weight stigma and how it impacts people and what can be done to address this problem. And our videos have been very successful. Particularly our video targeting health providers, which has been disseminated across the country and actually outside of the US, and is used frequently in medical facilities as part of a sensitivity training. What I've been delighted to see is that there's also been several studies testing the videos and what they have found is that these are effective interventions in improving attitudes and reducing weight bias. I will also mention that we worked with HBO on their Weight of the Nation Series. We also created a short video with HBO called the human cost of obesity. And that really kind of shows the personal side of weight stigma. And it has also been tested in research and found to improve attitude. So I think that these kinds of tools, these brief videos, which are really less than 20 minutes each, can be really useful ways to increase awareness of weight stigma and offer some strategies to be part of a solution to address this problem. You know the Weight of the Nation was a major enterprise by HBO to address obesity issues in the United States. And it was a sign of changing times to have included weight biases among the issues in that documentary. And I thought some of the personal stories that came out in those documentaries are really very powerful and I give you a lot of the credit for making that happen. So good work. So let me ask you on another level about the policy level about what might be done. And I know there has been some discussion about dealing with weight discrimination through antidiscrimination laws. Where does that stand? So you know, from my perspective, policy is really one of the most important strategies we have to address the problem of weight stigma on a broad scale. The kinds of policies and legislation that are relevant here are different for adults than for children. So for adults, you're right that this involves passing antidiscrimination laws and policies that essentially make it illegal to discriminate against people on the basis of body weight. And currently there are very few options for legal recourse in the United States if someone has faced weight discrimination. Right now Michigan is the only state that has a law prohibiting weight discrimination. This was a law that was passed as part of their state civil rights statute back in the 1970s. There's also now half a dozen cities across the country that have passed local jurisdictions to prohibit discrimination. But essentially there are very few options available. And so we've been conducting national studies really for the past eight years or so, looking at public support for different types of laws that could address this. And what we have found is substantial public support for laws that would make it illegal for employers to discriminate against people because of their weight in the workplace. And we see as much as 80% of people in favor of this. And we also see a similarly high levels of support for adding body weight as a protected characteristic in existing civil rights laws. And essentially this is what Michigan has and we're starting to see that some states want to do the same. For some time Massachusetts has been trying to pass a state law to add weight as a protected category in their state civil rights law. And they are making progress. And I think that we will likely see more states begin these types of efforts as well. So that is somewhat promising. When it comes to children though we were talking about weight-based bullying, which is a huge problem. And it's often something that gets ignored at the policy level, whether it's school-based anti-bullying policies or state anti-bullying laws. So right now almost every school district across the country has to have an anti-bullying policy. But these policies vary considerably in terms of how comprehensive they are. So some policies will list characteristics that place kid that risk for bullying, like sexual orientation or race and ethnicity, whereas others don't. And what we know is that schools that have more comprehensive policies that can enumerate these characteristics tend to have lower rates of bullying and better student safety, and more teacher intervention. So what's really concerning is that body weight is essentially a glaring omission in most of these policies. And so there is a clear need to strengthen these policies. And the same goes for anti-bullying laws. So every state in the country has an anti-bullying law, but they really vary from state to in terms of how strong they are. And right now only a couple of state laws even mentioned body weight. So we've done a lot of research on these issues and looking at things like parental support for strengthening policies and laws to better protect kids from weight-based bullying. And again, we see really high levels of support from parents, often 80% or higher who want to see body weight added into the language of school based policies in anti-bullying laws. So I think what this means is that we need to not only work with policymakers and school administrators to make this happen. But I think we also need to educate parents about this issue because when it comes to policy, parents have an influential voice in motivating political will for action on things that affect children. But many parents don't really know if their child's school has an anti-bullying policy. And if it does, whether it even addresses weight at all. So I think we can encourage parents to really talk to their schools, to learn more and to ask and request that body weight be included. Because we really needed to treat weight-based bullying as a legitimate form of harassment and bullying. And you know, at the Rudd Center we've provided examples of model policies and what they look like, and ways that parents can talk to their child or their child's school about making improvements So how can people learn more about your work and about the issue of weight stigma overall? So I would encourage folks to go to our website, which is www.uconnruddcenter.org. We have a whole section of our website devoted to weight bias and that includes not only our research but lots of resources that we created coming out of our studies that are available for policymakers, health providers, and parents and teens. That's a great place to start to get more information.
