There is one thing I know after having lived all these years: God wastes nothing. Not our sorrows. Not our wounds. Not even our failures. Hey there, I'm Dennis Jernigan, your host, and I'm excited to unveil the story behind my song, “Breathe” from the recording, Forty Days and Forty Nights - Songs of Contemplation and Intimacy Vol. 1. As I shared last week, in the year 2010,  I felt led to embark on something I had never experienced in my life…a forty day fast. Always marveling at those who had accomplished such a feat…marveling at, while at the same time,  considering those who had completed a forty-day fast somewhere between ‘hero' and ‘eccentric'…I woke one day feeling compelled to begin my own journey of a forty-day fast. Little did I know that God would speak to me in such profound ways and so radically re-order my life in the process. Having just gone through 3 years of grief and sorrow at the loss of relationship with several friends…reeling from the dramatic downturn of our economy…recognizing the lonely place I found myself due to the call to declare to the world my freedom from same sex attraction…major surgery…another daughter marrying and moving literally to the other side of the earth…coming to grips with how different and removed I was from the ‘normal/mainstream' Christian music scene…I woke that day knowing I needed a change…needed to reboot my system in every way – mentally, emotionally, physically, and spiritually. From May 3 until June 11 I consumed only water and fruit and vegetable juice losing 40 pounds in the process. While the weight loss helped get my body back on track after the surgery, the re-focusing of my mind, emotions, and spirit on the things of the Lord began to get my entire being back on track. I began each day meditating on either Psalm 40 or Isaiah 40. With each day's meditation the Lord saw fit to give me a song…which I recorded immediately after receiving it. The forty songs in these four volumes are the culmination of a 40-day journey of self-rediscovery. My vision for these collections is that you would use them as devotional meditations in your personal daily spiritual walk of intimacy with Christ. We have made the lyrics available to you on this web site along with the Scriptural reference on which I meditated. Use this as a guide to help you rediscover a deeper awareness of your identity in Christ or to uncover new hidden treasures concerning your relationship with the King. Today I am sharing the story behind the song, Breathe, which came to me on May 4, 2010 or day 2 of my 40 Day Fast, as I meditated on Psalm 40:1 which says, “I waited patiently for the LORD” along with Matthew 6:17-18 in the NIV, which says, “But when you fast, put oil on your head and wash your face, so that it will not be obvious to others that you are fasting, but only to your Father, who is unseen; and your Father, who sees what is done in secret, will reward you.” At this point in my fasting journey, I felt it was important to take heed of the Matthew 6 instructions as I continued to wait on the Lord for whatever he might have for me. I continued with my daily physical workouts. Continued with my daily focus on my music ministry. I continued with my duties toward my family and toward my marriage. I carried on with my duties at our local church body. In other words, the only ones who knew I was really fasting were my wife, my children, and a few close friends. The fast was not about stroking my pride or inflating my ego. It was about whatever the Lord wanted me to gain from Him concerning not only my life but those to whom I ministered to as well. I woke that day feeling a deep sense of hunger, both physically and spiritually, and with a deep anticipation of what the Lord would speak to me or reveal to me that day. As the first words of the first verse reveal, I did not even know what I needed other than I knew I needed Jesus first and foremost to be the Lord of every area of my life and existence. The more I thought on these things, the more I related my need to physically breath in order to keep existing. This, in turn, had a direct correlation with my need to breathe in all the Holy Spirit had for me. At that point, the song began to flow out of me. I had physical hunger. This reminded me of my spiritual hunger. I was thirsty and this reminded me of my thirst for God's presence in my life. It reminded me of just how bound up in sin I had once been and of how many more lies of the enemy I had still been believing. It became apparent that I had a great void in my heart that only my heavenly Father could fill. It became very apparent that I had been wondering in the desert for quite some time and I needed the refreshing rain of God's presence to drench my soul. Those 40 days proved to be very healing to my body and to my soul and I must add this: since I have been struggling with Parkinson's, the Lord has brought these songs back to my mind and I find myself, once again, having to still and quiet my own soul - especially since my body is in perpetual motion due to the tremors caused by this disease. It is as if I'm learning an even deeper stilling of my soul even as my body ails and refuses to cooperate. Just do what I do. Just take a few minutes. Get alone with the Lord if possible and make this your heart's cry for today: I waited patiently for the LORD… Psalm 40:1a NASB Let's just do that. Let's just sit here a while and patiently wait for the Lord. He is here whether you feel Him or not. He is just here. SONG What I found on day two of my fast was my great need for intimacy with the Father. My soul longs to just feel Him breathe on me; aches for just a drop of the rain of His presence; yearns to have the chains of the enemy's lies broken from my mind; and now that I battle Parkinson's, I recognize how much more I need Him to set me free from the cares and fears  of this world; that in Him I find that even in the midst of a life-altering sickness, in His presence I have room to breathe. Let's be as those who wait on the Lord this week; among those who find room to breathe even in the midst of our circumstances; room to breathe in our our sorrows; room to breathe even in our fears; room to breathe even when bombarded by the enemy and his lies.     Daniel's Prayer Was “seeking him {God} by prayer and pleas for mercy with fasting . . . ‘O Lord, the great and awesome God . . . we have sinned and done wrong'” MESSAGE SUMMARY: There are times when you need clarity, understanding, knowledge and insights from God about the specific issues that you, your family, your church, or your country are facing. In your personal relationship with God as a Jesus Follower, the Lord will speak to you; but sometimes it takes your listening and fasting to hear Him. For example, when Joel received a Word when the land was to be destroyed, he called a fast (i.e. Joel 1:14): “Consecrate a fast; call a solemn assembly. Gather the elders and all the inhabitants of the land to the house of the LORD your God, and cry out to the LORD.”. Some things you are facing in your life must be dealt with by both prayer and fasting. Fasting releases a spiritual power – in your weakness, God is made strong in your life. Daniel's prayer, from Daniel 9:3-5, provides a context for asking direction from God: “Then I turned my face to the Lord God, seeking him by prayer and pleas for mercy with fasting and sackcloth and ashes. Visit your primary care provider for additional medical advice.DefinitionGestational diabetes mellitus (GDM) is a condition that occurs to previously non-diabetic pregnant women, caused by glucose intolerance at around the 24th week of gestation. PathophysiologyGDM arises due to an underlying pancreatic beta cell dysfunction in the mother which leads to a decrease in the amount of insulin produced and thus leads to higher blood sugar levels during pregnancy. The placenta of the fetus will produce hPL (human placental lactogen) to ensure a steady supply of sugars to the fetus, creating an anti-insulin effect. However, hPL readily crosses the placental barrier causing the mothers insulin requirement to increase, when the mother's pancreas cannot increase production of insulin to that level needed to counter the effect of hPL they become diabetic, and this leads to gestational diabetes. So, basically the placenta is asking for more glucose for the baby and the mother's pancreas struggles to keep the glucose level within normal limits in the body of the mother. If left untreated, high levels of glucose in the mother can cause glucotoxicity in the mother.“Glucotoxicity” refers to the toxic effect of glucose. Glucose is the main fuel for cell functions, but when it is high in the bloodstream, it causes toxicity to organs. Prevalence of GDM.The CDC reports mean prevenance of GDM is 6.9%. In U.S. mothers the prevenance increased from 6.0% in 2016 to 8.3% in 2021. Many different factors have played a role in increasing gestational diabetes in American mothers, some of those being the ongoing obesity epidemic with excess body weight being a known risk factor for insulin resistance. Another being advanced maternal age (AMA) as more American women have children later in life their body becomes less sensitive to insulin and requires a higher insulin output on top of the insulin that is required for the fetus. The “American diet” is also something that has a big effect in diabetes development. With the increase of high-carb foods that are readily available, the diet of Americans has declined and is affecting the metabolic health of mothers as they carry and deliver their children. Despite ongoing awareness of GDM, 6% to 9% of pregnant women in the United States are diagnosed with gestational diabetes, and the prevalence continues to increase worldwide. It is estimated that in 2017 18.4 million pregnancies were affected by GDM in the world, which then continued to increase to 1 in 6 births to women with GDM in 2019. It was also found that women living in low-income communities were disproportionately affected due to limited healthcare access. Additionally, women with GDM had a 1.4-fold increase in likelihood of undergoing a c-section, with 15% increase in risk of requiring blood transfusion. Screening for GDMGestational diabetes is screened between the 24th to 28th week of gestation in all women without known pregestational diabetes. In women who have high-risk for GDM the screening occurs during the first trimester, these women usually have at least one of the following: BMI > 30, prior history of GDM, known impaired glucose metabolism, and/or a strong family history of diabetes. The screening during the first trimester is to detect “pregestational diabetes” because we have to keep a good glycemic control to improve outcomes of pregnancy. So, if it's positive, you start treatment immediately. If these women are found to have a normal glucose, they repeat the testing again as done normally, at 24-28 weeks of gestation. How do we screen?The screening itself consists of two types of approaches. The two-step approach includes a 50-gram oral glucose tolerance test (OGTT), where blood glucose is measured in an hour and if it is below 140 they are considered to not have GDM, however if the reading is greater than 140 they must then do a 3-hour, 100g oral glucose tolerance test. The 3-hour OGTT includes measuring the blood sugars at Fasting which should be less than 95, at 1 hour at less than 180, at 2 hours at less than 155, and at 3 hours at less than 140. If 2 or more of these values exceed the threshold the patient is diagnosed with gestational diabetes mellitus. The one-step approach includes 75g after an overnight fast. Blood glucose is measured while fasting which should be less than 92, at 1 hour less than 180 and at 2 hours less than 153. If any one of these values is exceeded, the patient is diagnosed with GDM.If the mother is found to be GDM positive during pregnancy she will also need continued screening post-partum to monitor for any development of overt diabetes. The testing is usually 75g 2-hour OGTT at 6-12 weeks postpartum. If this testing is normal, then they are tested using HbA1c every 3 years. If the post-partum testing shows pre-diabetes, annual testing is recommended using HbA1c measurements. Maternal complications Women with GDM are at an increased risk for future cardiovascular disease, T2DM, and chronic kidney disease. GDM is also associated with increased likelihood of developing pre-eclampsia following delivery. Pre-eclampsia is a complication seen in pregnancy characterized by high blood pressure, proteinuria, vision changes, and liver involvement (high LFTs). Pre-eclampsia can then progress to eclampsia or HELLP syndrome, both of which can include end organ damage. Additionally, she can develop polyhydramnios which leads to overstretching of the uterus and can induce pre-term labor, placental abruption, and or uterine atony, all of which additionally put the mother at increased risk for c-section. All of these maternal complications that stem from GDM lead to complications and extended hospitalization. Child's complications Although there is an increased set of risks for the mother, the neonate can also develop a variety of risks due to the increased glucose while in utero. While the fetus is growing, the placenta is the source of nutrition for the fetus. As the levels of glucose in the mother increase so does the amount of glucose filtered through the placenta and into the fetal circulation. Over time the glucose leads to oxidative stress and inflammation with activation of TGF-b which leads to fibroblast activation and fibrosis of the placenta. This fibrosis decreases the nutrient and oxygen exchange for the fetus. As the fetus attempts to grow in this restrictive environment its development is affected. The fetus can develop IUGR (intrauterine growth restriction) leading to a small for gestation age newborn which can then lead to another set of complications. The low oxygen environment can lead to increased EPO production and polycythemia at birth which can then lead to increased clotting that can travel to the newborn brain. Newborns can also be born with fetal acidosis due to the anerobic metabolism and lactic acid buildup in fetal tissues which can cause fetal encephalopathy leading to cerebral palsy and developmental delay. And the most severe of newborn complications to gestational diabetes can lead to fetal demise. Furthermore, the increase of glucose can also lead to macrosomia in the infant which can often lead to a traumatic delivery and delivery complications such as shoulder dystocia and brachial plexus injury. Brachial plexus injury sometimes resolves without sequela, but other times can lead to permanent weakness or paralysis of the affected arm. The baby can be born too small or too big.Additionally, once the fetus is born the cutting of the umbilical cord leads to a rapid deceleration in blood glucose in the fetal circulation and hypoglycemic episodes can occur, that often lead to NICU admission. The insulin that is created by the fetus in utero to accommodate the large quantities of glucose is known to affect lung maturation as well. The insulin produced inhibits surfactant production in the fetus. Upon birth some of the newborns also have to be placed on PEEP for ventilation and some children require treatment with surfactant to prevent alveolar collapse and/or progression to NRDS created by the low surfactant levels. Additionally, neonates who are macrosomic, which is usually seen in GDM mothers, are larger and stronger and when put on PEEP to help increase ventilation the newborn's stronger respiratory effort can lead to higher pulmonary pressures and barotrauma such as neonatal pneumothorax.Long term complications to the child of a mother with GDM also occur. As the child grows, they are also at an increased risk for developing early onset obesity because of the increased adipose storage triggered by the increase in insulin in response to the high glucose in utero. This then can lead to a higher chance of developing type 2 diabetes mellitus in the child. With diabetes, also comes an increase in cardiovascular risk as the child ages and becomes an adult. The effects of GDM go beyond the fetal life but continue through adulthood.What can be done?Gestational Diabetes Mellitus has many severe and lifelong consequences for both the mother and the child and prevention of GDM would help enhance the quality of life of both. Many of the ways to prevent GDM complications include patient education and dietary modifications with a diet rich in whole grains, fruits, vegetables and lean proteins. Benefits of some vegetables in the Mexican cuisine that may be beneficial: Nopales, Chayote, and Jicama. Those are good alternatives for highly processed carbs.Mothers are usually offered nutritional counseling to help them develop a tailored eating plan. This and 30 minutes of moderate exercise daily is recommended to increase insulin sensitivity and lower the post-prandial glucose levels. If within 2 weeks of implementing lifestyle changes alone the glucose measurements remain high, then medications like insulin can be put onboard to manage the GDM. If they require insulin, I think it is time to refer to a higher level of care, if available, high risk OB clinic.Conclusion: Now we conclude episode number ###, “[TITLE].” [summary here]. _____________________References:Eades CE, Burrows KA, Andreeva R, Stansfield DR, Evans JM.     New Hope Daily SOAP - Daily Devotional Bible Reading

    Daily Dose of Hope June 6, 2025   Scripture - Matthew 9:1-17   Prayer: Abba Father, Prepare our hearts for your Word today.  Help us approach this Scripture with an openness to learning something new from you.  Jesus, thank you for your perfect example of mercy and love.  More of you and less of me.  In Your Name, Amen.   Welcome back to the Daily Dose of Hope, a Deep Dive into the Gospels and Acts.  Today, we start Matthew 9.   We start with Jesus heading back across the lake into Capernaum, as the scripture refers to his own town.  It seems in no time at all, a paralyzed man is placed before him.  While Matthew does not give the details about the man's friends digging a hole in the roof and lowering him down, this is most likely the same story we find in Mark 2.  Notice how he sees the faith of the friends and heals the man.    But what does Jesus say to the man?  Jesus tells the man that his sins are forgiven.  This stirs things up.  Of course, the teachers of the law speak among themselves, thinking this is blasphemy.  But it's Jesus and he is God so he knows exactly what they are thinking and he calls them out.  What the teachers of the law cannot see is that Jesus is authorized to speak and act on God's behalf. He is God's anointed, the Messiah. As a demonstration of his authority to forgive sins, Jesus tells the paralyzed man to stand up, take his mat and return home, which the man promptly does. Jesus boldly demonstrates his power to bring both spiritual and physical healing, and the crowds respond by being amazed and glorifying God.   We go right from this healing to the calling of Matthew.  I love how Matthew writes about his own conversion.  He doesn't give a lot of details.  It is most likely that he has seen and heard Jesus before.  After all, tax collectors were usually set up in visible places in town.   By the way, the phrase “sitting at the tax collector's booth” is filled with meaning.  At the time, tax collectors were hated.  The Roman Empire took bids for the right to collect taxes.  These tax agents paid a set amount to Rome, and could keep all the rest that they collected.  Thus, tax collectors were quite wealthy.  But they enriched themselves by preying on the poor, stifling trade, and operating kind of like a local mafia.  They were also despised for how they collaborated with the oppressive Romans.   One thing that is interesting about this whole situation to me is that Matthew did not choose Jesus, but rather Jesus chose Matthew.  In the Greek, the word “follow” is in the imperative.  It was a command.  Jesus isn't giving Matthew a choice.  And Matthew got up and followed him.    What would you have done?  Jesus' call is always action-oriented.  When we follow Jesus, it is a call to do something for his Kingdom.  It is also a call to leave the past behind.  How many of us try to follow Jesus without fully leaving our past commitments and issues behind?   Later on, at Matthew's house, Jesus is dining with some of the less desirable folks in town.  Scripture says he ate with tax collectors and sinners and those two groups were seen as pretty synonymous.  This very much perturbs the religious people.  How dare he cavort with such scum?  But Jesus says that it is the sick people who need a physician, not the well.  He came to call sinners.    My guess is that part of the Pharisees' frustration is that Jesus seems to be simply be eating and drinking with the sinners, not providing treatment.  But Jesus was all about relationship.  I think we can learn a lot here.  There were times in which Jesus preached and called for people to repent.  There were times when Jesus performed miracles to demonstrate his power and authority.  But much of the time, Jesus simply sat at the table with people and built relationships.  It is through relationship that life change is most likely to happen.    Think about your own life.  How have your grown the most in your spiritual walk?  Was it through a sermon?  Probably not.  Was it through a Bible study?  Again, maybe not.  Was it through relationship with other Godly people who taught you, poured into you, and showed you the life of Christ?  Most likely.    To effectively point people to Jesus, we have to first get to know them.  Especially for those who have been hurt by the church or just by life, we have to earn the right to speak into their lives.  Jesus did just that.  And he was genuine and loving at the same time.  My guess is there was no condemnation at that dinner table with Matthew and his friends.  They were talking and getting to know one another.  Maybe Jesus did a little teaching at times, but it was probably subtle.  He was giving them the opportunity to get to know him, as he listened to them.    Who sits around your table?  When is the last time you invited some people “on the fringe” to dine with you?  How about unbelievers?  How are you intentionally building relationships with those who don't yet know Jesus?    Our text for today ends with a discussion on fasting.  John's disciples ask why Jesus' disciples don't fast like them.  We don't know if this is a dig but it kind of feels like one.  Jesus responds with an analogy to a wedding feast.  Fasting might be appropriate on some occasions, but certainly not at a wedding feast when the groom is right there. Right now, Jesus is with them.  Thus, this isn't the right time for fasting.  It is time for feasting and enjoying his presence. The point is that Jesus is with his disciples just as the wedding guests are with the groom. It is a time for celebration—not deprivation. It is a time to enjoy the relationship and absorb his teaching and example while they can. When the bridegroom is taken from them, there will be a time for fasting.  As we know, that's going to happen pretty soon. Blessings, Pastor Vicki

In the year 2010,  I felt led to embark on something I had never experienced in my life…a forty day fast. Always marveling at those who had accomplished such a feat…marveling at, while at the same time,  considering those who had completed a forty-day fast somewhere between ‘hero' and ‘eccentric'…I woke one day feeling compelled to begin my own journey of a forty-day fast. Little did I know that God would speak to me in such profound ways and so radically re-order my life in the process. Having just gone through 3 years of grief and sorrow at the loss of relationship with several friends…reeling from the dramatic downturn of our economy…recognizing the lonely place I found myself due to the call to declare to the world my freedom from same sex attraction…major surgery…another daughter marrying and moving literally to the other side of the earth…coming to grips with how different and removed I was from the ‘normal/mainstream' Christian music scene…I woke that day knowing I needed a change…needed to reboot my system in every way – mentally, emotionally, physically, and spiritually. From May 3 until June 11 I consumed only water and fruit and vegetable juice losing 40 pounds in the process. While the weight loss helped get my body back on track after the surgery, the re-focusing of my mind, emotions, and spirit on the things of the Lord began to get my entire being back on track. My good friend, Dean Briggs, sent me a new book he had just written as a guide through a forty-day fast called Consumed:40 Days of Fasting, Repentance & Rebirth. Written in the form of daily devotions, I began each day meditating on either Psalm 40 or Isaiah 40. With each day's meditation the Lord saw fit to give me a song…which I recorded immediately after receiving. The forty songs in these four volumes are the culmination of a 40-day journey of self-rediscovery. My vision for these collections is that you would use them as devotional meditations in your personal daily spiritual walk of intimacy with Christ. It is as if I'm learning an even deeper stilling of my soul even as my body ails and refuses to cooperate. Just do what I do. Just take a few minutes. Get alone with the Lord if possible and make this your heart's cry for today: I waited patiently for the LORD… Psalm 40:1a NASB Let's just do that. Let's just sit here a while and patiently wait for the Lord. He is here whether you feel Him or not. He is just here. SONG Isaiah 40:31 says , “But those who wait on the LORD shall renew their strength; they shall mount up with wings like eagles; they shall run and not be weary; they shall walk and not faint.” Let's be as those who soar this week. Soar above our circumstances; Above our sorrows; above our fears; above the enemy and his lies. Let's just wait on the Lord. Let us learn the art of lingering with Him. It's time well spent. Mine have all come from some acute trauma or injury. I knew it and the answer was obvious, maybe for you too — to save your knees and shoulders without surgery!   My Guest: Dr. David Middaugh is a specialist physical therapist who helps people avoid unnecessary surgery while getting back to being healthy, active, and mobile. He coaches people online and has a clinic where people are treated in person. His contrarian treatment approaches are focused on addressing the root cause of problems like arthritis and tendon tears so that people have the most control over their health. Hands-on body work to manipulate the soft tissues like your muscles, tendons, ligaments tend to be pretty good at moving the joints. Fixing the root problem of knee and shoulder pain issues through movement. Focused on fascia techniques and fascia related treatments.   What Really Works to Save Your Knees and Shoulders?   Knees What to work on? Glutes. If you have weakness in the glutes, that's when the quads get over dominant. The Science: Quads have been overused causing knee arthritis or meniscus tear. What to do? Do your exercises primarily with your glutes. Your hamstrings and quads will still work – they just need to work secondary. Can you figure out how to make your glutes contract apart from your thigh muscles? It's more gaining mental control over your muscles. Stop walking. Turn on your glutes first.   Shoulders  What to work on? Upper traps (trapezius). If you have weak traps, there is more stress and tension holding the weight of the arm, shoulders and maybe chest.. The Science: Rotator Cuff tears are one of the biggest problems because of weak upper traps. What to do? Shrug every time you reach up to use your traps. This preserves your ball and socket joint.
