La testostérone symbolise la force et la virilité. Elle est l'hormone masculine par excellence, mais attention les femmes en produisent aussi ! Elle joue un rôle important même si chez elles, cette hormone est 7 à 10 fois moins présente que chez l'homme. On fait le point aujourd'hui avec le Docteur Gérard Bersand, spécialisé en médecine fonctionnelle et en médecine anti-âge. La testostérone : quel est son rôle ? Chez la femme, elle est produite par les ovaires notamment. Elle sert par exemple à développer et maintenir le tissu musculaire. Elle a aussi un rôle dans le renforcement des os. Sans oublier la mémoire. C'est en tout cas ce qu'ont démontré des scientifiques en 2013. La testostérone agirait aussi au niveau de la libido (attention, les scientifiques ne sont pas tous d'accord sur ce point) Les signes d'une baisse de testostérone Au moment de la ménopause, les ovaires cessent de fonctionner. Résultat, le taux de testostérone baisse brutalement puisque seules les glandes surrénales continuent d'en assurer la production. Ainsi, le manque de testostérone, chez la femme peut se constater au niveau de la silhouette. Les muscles deviennent moins toniques. La masse grasse elle se répartit différemment : elle se place plutôt autour du nombril. Cela entraine également une perte de densité osseuse. Par ailleurs, la testostérone imprègne quasiment tous les organes et notamment la peau. Elle agit surtout sur sa tonicité. C'est pourquoi une baisse de testostérone entraine une peau plus flasque. Comment augmenter naturellement son taux de testostérone ? Il existe aujourd'hui quelques astuces naturelles pour booster sa propre sécrétion de testostérone, si le taux n'est pas totalement effondré. Par ailleurs, il faut toujours veiller à être encadré par un professionnel de santé !  Ainsi, on peut citer quelques plantes comme le Tribulus terrestris et le Maca. La testostérone améliorerait la libido et le plan musculaire. Cela est valable aussi à l'inverse. Si vous avez une activité physique régulière ainsi qu'une activité sexuelle, vous augmenterez naturellement son taux. Parmi les recommandations plus basiques : pensez à privilégier une alimentation saine et un sommeil réparateur, essentiel à la construction musculaire, mais aussi pour conserver un bon taux de testostérone.

Once again this week, I want to start with some clinical insights, because my patients have been so fascinating recently! I've been working with a 66 year old man for about 3 years now and when I first saw him, he had recently retired. He loved to run and to golf, but his ability to do those things was deteriorating rapidly due to aging. We've been having an ongoing conversation about how to level up despite aging. Slowly but surely, we started to break some of the paradigms of this retired banker. We started doing one-on-one yoga sessions to improve his range of motion for golf. We started tweaking his diet and nudging him towards a cleanse, which he was not thrilled about as a huge wine connoisseur. At first, he just agreed to 2 days a week of his wine, but it was making a difference! He came in this week and told me he had the best round of golf in his life… at 66!  We just continued to stack good health habits and he continued to improve. This week, we had a conversation about what his next year will look like. Even through all of this, he's still skeptical. Sometimes he asks when he can stop doing all the new health habits he's stacked. But, when you consider that he's only been leveling up for 3 of his 66 years, it makes sense. Since he was a banker, I explained it in terms he could understand… Compound Interest. Health works the same way. As long as you're taking action, you're investing in your future and the longer you continue to invest, even if it's maintaining the same good habits, will pay dividends! I had another patient in his 70s I talked with this week. He's worked his entire life as an investment banker and financial planner and after our conversation about my finances, he stopped me and said it's time for him to start investing in his health. He'll be starting his journey from scratch at 74! But here's the thing… he's starting. You're never too old to begin to invest in your health. We have people who do our New Year's Cleanse every year, because it's become a part of who they are. It's a yearly reset that draws a line in the sand and prevents aging to creep in. The most dangerous part of aging is that our standards for ourselves are lower. Just because you're aging doesn't mean you need to gain weight, be on tons or prescriptions, or give up on your health. While we have some people who look forward to a yearly cleanse, we have others who are hesitant to join us. Maybe they eat good enough or think they're doing enough and don't need it. But, what blows me away is, even though I eat 99% organic, doing the cleanse makes me feel SO MUCH BETTER every year! It's about getting clean, taking out even good foods that are hard to break down, and allowing ourselves to rehab using the supplements. We kick off January 3rd, we spend time together as a tribe, share tips and recipes and level up together! Now let's get to the questions… 1. I'm in my mid-40s and pre-menopausal. I've had my cycle every 10 days. What can I do to ease my transition into menopause? This is about the time of life when perimenopause hits and if you haven't hit it, you need to prepare. Your adrenals need to be strongly supported. During menopause, your ovaries retire; during perimenopause, they go on an extended vacation. One of the absolute best things you can do is cleanse. All your hormones have receptors and they act like a baseball glove to absorb the hormone as needed. If those receptors are full of junk, when that hormone comes, it can't be absorbed and it goes away, often being stored in a fat cell. We typically use a Core Four protocol. We start with an herb called Chaste Tree, which would be helpful for all non-pregnant women, because it helps the body produce its best amount of progesterone. Next, is a supplement called Hypothalmex, which supports the hypothalamus gland. It helps regulate all the hormones in your body. If you're having your cycle that often, your hypothalamus and ovaries are out of sync. Since the ovaries are also likely getting weaker, I would add Ovex. The last supplement is Pituitrophin PMG. The hypothalamus and pituitary gland are connected and this will provide additional support. You should also be having a conversation with me or someone like me about the future of your health. What about a testosterone deficiency? You can easily tell through blood work or a saliva test. Testosterone is a responsive hormone, it's responding to what you're doing. If you're giving yourself a false sense of energy with carbs and sugar, your body doesn't think it needs to make more testosterone. Your adrenal glands signal your testes to make more testosterone, if the message isn't being sent, the hormone isn't being made. Also, in the event of serious stress for a long period of time, the adrenal glands wear out and steal sex hormones to make cortisol because your body believes you need more of it. There's an herb called Tribulus that works phenomenally to help with this. One of the best indicators of testosterone levels in the body is waking up with an erection. It's an indicator that your free testosterone is being activated. Once again, we start by cleaning and then going from there.  3. If your adrenals are overtaxed, can that contribute to your period stopping? Yes, it can dysregulate your system in different ways. You have four primary hormonal participants, your HP, Thyroid, ovaries or testes, and your adrenal glands. They all need to work in harmony for your body to function properly. The body is brilliant in knowing what state it's in to produce what it needs. But when the adrenals are exhausted by constant stress, they begin to overtake the HP and the Thyroid and constantly signal. This can easily throw off your cycle. 4. Are hormones responsible for metabolism? Does metabolism slow as we age? Hormones are responsive. They don't just decide to stop working, they're responding to how we move, how we eat, and how we think. If you want balanced hormones, start with sleep. If you operate in nonstop stress, your hormones respond by being out of balance. Metabolism isn't simply one thing, it's more a set of metabolic activities. It plays a huge role in systems, organs and glands, but it's also dependent on what you put in your body. If you burden your metabolic activities over time, it will have an effect. Medications also speed up and slow down the processes.  It's important to trust our intuition when it comes to these things. We've been trained to not listen to our body, but it's essential to our health. When we're clean, we can better listen to our body and determine what's best for us. Often, we can help ourselves by taking a moment and listening to ourselves. We don't often need to make decisions quickly or in an emotional state. Next time, pump the brakes, pause, and step back. Making a decision in a fear state is like flipping a coin. Ask yourself “Am I calm?”.  5. I'm having post-Covid hair loss, can you help? This can happen, even months after recovery. It goes back to the same things for all the long-haulers, you have to clean out the debris from your body. We have to clean up and clear out and then influx things like B12, B7, and Biotin (we use Cataplex B). We also need to focus on gut health, nutrient deficiencies will affect hair follicles. The good news is most of the Covid hair loss cases I've seen have gotten their hair back.  And last but certainly not least, this week's action to level up is… Be More Intuitive Trust yourself and become a great listener to your own body. You live in it, and the more you become in tune with it, the more you become superhuman. 89. Scott Howell PhD: Do Trans Athletes Have a Competitive Advantage in Women's Sports?

Dr Howell's most recent consulting work was for the US Power Lifting Association in helping them look into the science of trans athletes competing and helping them formulate their position based on science. Dr. Howell is a former Professor and Ph.D. faculty member in the School of Health and Human Services at Trident University. Dr. Howell holds a Ph.D. in Health Sciences from Trident University, a Medical Degree from BMU School of Medicine, a Master of Science in Sport and Health Science from American Military University, a Bachelor of Science in Sport and Health Science from American Military University, and a Mechanical Engineering degree from Forsyth Technical College. He is an epidemiologist, exercise physiologist, and mechanical engineer with current research interests in the long-term effects of therapeutic androgen use and endocrine disrupting chemicals exposure. He is a recipient of the prestigious AMU Academic Scholar Award (2016) and has worked on two major NIH and DoD funded studies at Wake Forest University: Strength Training for Arthritis Trial (START) and The Runners and Injury Longitudinal Study (TRAILS).    Questions discussed: Do men who have transitioned to women have an advantage in sports over biological females. If so can it be quantified? Laurel Hubbard transitioned at age 35 now she is 43 in competing in the Olympics as a trans athlete in the superheavyweight women's division. Prior to that Laurel competed as a man. While being an elite athlete as a man Laurel was never a world-class athlete. Based on your knowledge does Laurell have a significant advantage over her female competitors. Does this advantage cover a whole range of sports? Is the testosterone produced in men and women naturally the same or different? Based on XX or XY chroma zones chromosomes how is testosterone processed in the body, especially at lower levels? What are the effects of giving men estrogen? Does it impede the effects of testosterone? Is there any difference in terms of putting on in maintaining muscle mass? Especially if the testosterone levels are the same.  Can you talk about routine benefits retained benefits of steroids after they have been no longer after they have no longer used? Generational levels of testosterone are going down. The labs have just dropped their reference range from mid-1100 to 964.  This is a drop of around 12%. The lower reference ranges have also been dropped to 264. According to Dr Keith Nichols those ranges were in the 1500s  15 years ago.  He also stated that men in their 20 and 30 are showing up in his clinic with lower baseline levels than those in their 50 and 60s.  What are you seeing developmentally in boys and young men these days? What is their prognosis? Do you see this lower testosterone level as a factor in gender dysphoria? Many people with gender dysphoria report depression. Do you believe this could be related to low T? The two channels that run down the penis, the corpus cavernosum and the corpus spongiosum, those are erectile tissue. Those are the chambers that fill up with blood to give a man an erection. Those chambers are in our clitoris. So, we have a clitoral head, a clitoral shaft, so we actually have a shaft, the glans is like the tip of the penis, the shaft is like the shaft of the penis. So, a woman actually achieves a clitoral erection given enough massaging or oral pleasuring, or lovemaking. And then, the clitoris has two little arms that drape over the entrance to the vagina. That’s called the introitus, the entrance to the vagina. And then it has two plump bags that are the legs of the clitoris, that are called the vestibular bulbs, that are actually under the pubic hair on each side of the opening to the vagina. And that whole structure is the erectile tissue of her clitoral structure that needs to get filled with blood. Then she has a urethral sponge. We have an extruded little tube that goes down the urethral outlet, where your urine comes from your bladder and exits out of your vulva, between your clitoris and the opening to your vagina. That’s an exit. There’s a tube that runs, a fluffy tube of erectile tissue there. That’s what people call the G-spot. You can get to it from the outside, and you can get to it from the inside of the vaginal canal. And then there’s another sponge, called the perineal sponge, that’s located between the rectum and the vagina, those two tubes are parallel up inside her, and there’s a spongy tissue between them. All of that’s erectile tissue. You only see half of a man’s shaft. There’s 50 percent of his penile tissue that’s buried up inside him. For a woman especially, to be able to let go, she needs the conditions to be right outside the bedroom as well as inside the bedroom. Women want to be totally taken care of, but they also want some freedom. Women today are very emancipated. Men don’t understand how much romance is important to women, how much encouraging her, telling her you love her, taking her out for a walk in the countryside, rowing her in a boat on the lake, taking her for a hike. Moving a woman’s body moves her emotions, and she wants to feel an emotional connection to you. Men, often, what they want is recreational companionship. They want touch, affection, passion. La suplementación con Ashwagandha también ha demostrado ser un estimulante eficaz de la testosterona en hombres infértiles. En otro estudio, hombres no entrenados empezaron un programa de entrenamiento de resistencia y, tomando Ashwagandha, vieron aumentos significativamente mayores en el tamaño muscular, los niveles de testosterona y recuperación muscular comparada con placebo. Para empezar hay que tener en cuenta que los sujetos en este estudio tenían niveles muy bajos de testosterona. En otro estudio, los sujetos con estrés crónico que recibieron una dosis diaria de Tongkat Ali durante un mes y tuvieron un aumento del 37% en los niveles de testosterona. Este estudio esta muy bien. Se hizo a 32 hombres y 31 mujeres. Y se encontraron mejoras significativas. El perfil hormonal del estrés (cortisól y testosterona) mejoró significativamente, con una reducción del cortisól (?16%) y un aumento de la testosterona (+ 37%). Aunque estos estudios dicen que la suplementación con Tongkat Ali tiene una influencia positiva en la testosterona, es importante tener en cuenta que los sujetos estaban estresados o tenían niveles muy bajos de testosterona, eso para empezar.
