In 1616, the year that William Shakespeare died, anatomist Helikiah Crooke published a book of medical diagrams that included a surprisingly high level of detail about human anatomy for a society that didn't yet have powerful instruments like a microscope. However, noticeably absent from his medical drawings are any anatomically correct terms for the female body. For example, Crooke's drawings correctly name many parts of the male anatomy, like the epididymis, peritoneum, penis, and the testicles, but when it comes to the female anatomy diagram, even though he has the fallopian tubes correctly drawn in the picture, he labels them as “Spermatical veins.” In another diagram, the uterus is definitely drawn in the shape of a man's primary sexual organ, only it is labeled as a “womb,” with no mention of the cervix or the vagina. Shakespeare's plays give us some examples of how this level of medical knowledge was known by general society because in his works Shakespeare uses anatomical terms like ‘urine” as well as “sperm” and even “Nerves and veins”, so we can see that some knowledge of human anatomy was broadly available in Shakespeare's lifetime, but Helikiah Crooke's diagrams raises some major questions about not only the medical understanding of women's bodies, but exactly what kind of medical care was available for women in Shakespeare's lifetime. As days grew longer and time in school shortened, we suddenly found more time for each other."Have you thought what you'll do after you graduate?" Jerry asked the frightening question. Dr. Maria Uloko is fired up, and we are right there with her. As the first ever Black female Urology student at her university, she has been trailblazing for the entirety of her career. Dr. Uloko specializes in comprehensive sexual health which includes the medical and surgical management of ALL genders. She is considered a world leader in chronic pelvic pain and vulvar conditions, she believes in empowering her patients to take ownership of their sexual health by providing a shame-free space to ask questions and share their stories, and she seeks to close gaps in disparities of care and implement systems that promote diversity and inclusivity. She is also an international award winning researcher with a focus in vulvar health, most notably her research in defining how many nerves are in the human clitoris. (Can you believe that this number was not known until recently, when she and her colleagues embarked on a study?) She told us that the anatomy of the vulva is not taught in medical school, even to gynecologists, and that this lack of knowledge and adequate care is equivalent to malpractice against those with female anatomy. Without her consent or knowledge, her surgeon severed the dorsal nerve of her clitoris, resulting in a loss of clitoral sensation. In the face of clear evidence, her doctor denied wrongdoing. Jessica has since discovered labiaplasty is regularly performed by OBs even though (a) it's not covered in residency, (b) there are no training standards, and (c) there is no procedure code, which means there is no way to know how many labiaplasty surgeries are performed, and (d) adequate vulvar anatomy is not talk to OBGYNs, which makes damages difficult to prove in litigation. Through her online platform, Jessica has attracted countless women who've shared their stories of losing sensation during the same surgery. Newsome relates this to arguments she hears often from the tech industry stating that algorithms and computers cannot be racist. She adds that humans created the programs and wrote the medical journals, which means that biases will be present, and that neither computers nor physicians are entirely objective, and both can indeed be racist. Next, the three discuss the power of language, and the critical relationship of language and the biases we hold. Elaine Martin speaks to her experience as a nurse in San Francisco, and what she notices about the language that is used when interacting with patients. She reflects on how medical providers' choice of language can be dismissive or hurtful to patients. She shares experiences from her family members interactions with medical providers, and how she has learned to communicate with patients differently based on these experiences. They discuss a New York Times article by Rachel Gross about the linguistic origins of the pudendal nerve and its ties to patients with pudendal neuralgia who feel shame due to their condition. Finally, our guests give advice to current medical trainees. They discuss how we can examine our current structures and systems and change them where we identify problems. Sex, Part Deux! For an episode about sex we talk a lot about movies for some reason. There's not much sex in The Avengers, Man Of Steel or Thor but Jon learned everything he knows about women's underwear from the original Superman. Lex gets his information about women's underwear from Shark Tank. Grease is a great movie for learning about sex. Lots of direct references to sex facts that your kid won't understand. "Any questions?" The Godfather is good for teaching your kids about violence. Ace Ventura would be a good movie to teach kids about homophobia. Jacqueline Bissett in The Deep was Intro to Female Anatomy 101 for many a boy during the '70s. Although, Benny Hill is how Moltz learned about female anatomy.
