Filomena Trindade, MD, MPH, ABFM, ABOIM, FAARM, IFMCP is a physician, teacher, author, and internationally sought after lecturer in functional medicine. She is faculty at the Institute for Functional Medicine (IFM) and in the fellowship program in Metabolic Medicine at Metabolic Medical Institute (MMI).   After obtaining her BA degree in Biology she went on to finish a master's in Public Health in the area of environmental health and epidemiology before starting medical school. She graduated first in her class in family practice from the University of California Davis School of Medicine and did her residency training in family practice at the U.C. San Francisco/Santa Rosa Program.   She has been in clinical practice for over 24 years. Before starting her own private practice in 2004 in functional medicine she was the medical director of a non-profit organization that catered to the underserved.   She recently authored a chapter on Nutritional Influences on Hormonal Health in the Integrative and Functional Medicine Nutrition Therapy Textbook and another chapter on Diabetes in the Integrative Cardiovascular Medicine textbook. Slowing aging , preserving memory, and preserving energy are big topics of concern in medicine and health care. Over the past 10 years research and development has show a special interest in how to modify Sirtuin-1 as a protein for longevity and slowing the disease process. The connection between iodine and Hashimoto’s disease has been one of the most requested topics that I cover, so I’d like to present the research on this topic, so we can set the record straight. Please be aware that none of this is my opinion but rather a detailed analysis of what the scientific literature currently presents. Your body has about 15 to 20 mg of iodine and 70 to 80% of it resides in the thyroid gland. Iodine is transported into the thyroid gland through the sodium-iodine symporter or NIS. The thyroid peroxidase enzyme oxidizes iodine which is then integrated into thyroglobulin resulting in the production of thyroxine (T4) and triiodothyronine (T3). Thyroid stimulating hormone (TSH) regulates this process. Your thyroid only needs 150 to 250 micrograms a day of iodine to function properly. Once iodine intake exceeds this range, hypothyroidism and autoimmune thyroid disease may occur in some individuals. How Much Iodine do the Japanese Consume? The first issue I want to briefly clear up is the false idea that the Japanese consume 13.8 mg of iodine per day from their diet. This is actually a mischaracterization of a single study by Nagataki et al that I will clarify. Here is what the literature shows: Aceves et al state that, “This element (iodine) has been associated with the low incidence of benign and malignant breast disease in Japanese women (iodine average consumption in the Japanese population is 5,280 μg/day versus 166-209 μg/day in the UK and USA, respectively).” 5,280 ug/day equals 5.28 mg/day. Cann et al agree in their paper stating, “Japan has a comparatively high iodine intake with an average urinary iodine concentration of 3400 μg/L (approximately 5,280 μg/day) as determined in a recent study of 4138 men and women in Sapporo, Japan.” Tajiri et al state that, “...the average daily iodine intake was 1-5 mg in the remaining 10 patients, equivalent to the ordinary intake of normal Japanese.” Kapil et al state, “Iodine Intake in Japan: Average daily intake of iodine in Japan has been reported to be 3000 micrograms, which is 20 times more than the RDA value of 150 mcg in India.” According to a report on iodine from the FAO/WHO Expert Consultation on Human Vitamin and Mineral Requirements, the following is stated about the daily iodine intake in Japan: “The amount of iodine intake by the Japanese is in the range of 2-3 mg/day.” And finally, let’s clarify the Nagataki study. Nagataki states, “...the dry weight of such food as ‘tangle’(Laminaria) contains 0.3% iodine...” “...the average daily intake of seaweed was 4.6 g (wet weight).”  The well-known doctors who make this claim of 13.8 mg made a major mathematical error. 0.3% of 4.6 g does in fact equal 13.8 mg, but they confused dry weight and wet weight. The issue is that the paper is referring to dry weight in the above quote but the second quote refers to wet weight. A follow-up paper in 2008 by Nagataki further clarifies the question of how much iodine is ingested by the Japanese on a daily basis. In this paper, Nagataki states, “The average of dietary iodine intake due to the ingestion of seaweeds is 1.2 mg/day in Japan.” Alan Gaby, MD, also picked up on this in an editorial from The Townsend Letter for Doctors and Patients entitled “Iodine: A lot to swallow,” states the following: “The idea that Japanese people consume 13.8 mg of iodine per day appears to have arisen from a misinterpretation of a 1967 paper. In that paper, the average intake of seaweed in Japan was listed as 4.6 g (4,600 mg) per day, and seaweed was said to contained 0.3% iodine. The figure of 13.8 mg comes from multiplying 4,600 mg by 0.003. However, the 4.6 g of seaweed consumed per day was expressed as wet weight, whereas the 0.3%-iodine figure was based on dry weight. Since many vegetables contain at least 90% water, 13.8 mg per day is a significant overestimate of iodine intake.” Does Iodine Cause Hashimoto’s Disease or Make it Worse?

My returning guest Scott Forsgren had ample experience dealing with Lyme disease naturally and is now helping others do the same. And he’ll teach us how he did all that in this episode. First and foremost, Scott Forsgren says his whole journey in dealing with Lyme disease naturally was influenced by Dr. Dietrich Klinghardt’s energetic testing. Scott Forsgren worked with practitioners for years that use his autonomic response testing and eventually got certified to administer the testing, too. Improving Gut Health Majority of the immune system and neurotransmitters originate in the gut. So, Scott Forsgren says the more we can optimize our G.I. health, the more we can improve our overall health. He advises to first look at diet. He believes it needs to be individualized but at the same time, remove the foods that are known to be more common stressors like gluten, cow dairy, and sugar. Scott Forsgren also recommends working with a nutritionist to find out which diet might make the most sense for someone to explore. “One that I observed helps most people dealing with Lyme disease is the low-histamine diet. It is not an easy diet to do. And it’s easier if people generally prepare their food at home rather than eat out,” said Scott Forsgren. “I see it helps reduce inflammation in people and to help with a lot of their symptoms just by changing the diet.”  He adds, “The more we can focus on reducing inflammation in general, while we are treating the underlying causes, or triggers of our health condition, the easier it is to get through that process. The less inflamed people are and the better they feel while they are being treated.”  Realities of Lyme Disease In dealing with Lyme disease naturally, you have first to understand the components of the disease. Scott Forsgren says if someone is dealing with Lyme, they probably have Bartonella and or babesia. Some other common co-infections or opportunistic infections that need to be explored as well. Again, he advises people to work with a practitioner who can start treating them with antibiotics, herbs, homeopathy or other tools. Food Sensitivity Testing Food sensitivity testing is worth exploring when you are dealing with Lyme disease naturally. This is also needed if people have very specific and unique foods that might be an issue for them. Scott Forsgren says avoiding those foods can be significant. He also says that gluten is non-negotiable on people with Lyme and mold-related issues. Furthermore, Scott Forsgren believes probably at least half, or more people would do better by eliminating cow dairy. “Avoid those things stressful to the system, which creates more inflammation and instead, incorporate foods that are as nutritionally dense as possible,” Scott Forsgren advises. “Not the prepared, processed food but getting real food into the diet. As much organic food as possible, eliminating the GMO foods and restricting the processed foods is important.”  He adds, “And then depending on whether or not people consume meat, organic grass-fed meat, pasture-raised poultry, and wild-caught seafood can be very helpful as well.”  Healthy Fats Scott Forsgren also says consuming healthy fats can help in dealing with Lyme disease naturally.  Healthy fats include butter or ghee, chia seeds, coconut oil, and olive oil. He also likes a lot of the plants and seed-based oils. What Scott Forsgren does daily is making a power shake, with high-quality protein, collagen powder, a blend of different fibers that helps not only with gut but also from a perspective of binding toxicity and supporting cell membranes. Scott Forsgren also likes to mix in these healthy fats in the form of flaxseed oil, sunflower seed oil, a tablespoon of chia seeds and organic almond milk or any nut milk.  It helps build muscle and structural integrity which is useful in overall health maintenance.  Mega SporeBiotic Mega SporeBiotic is a spore-based probiotic developed by Kiran Krishnan and Microbiomelabs. People tolerate it pretty well. Scott Forsgren says many other probiotics are a problem or an additional stressor if you have a histamine issue. “It can also help address leaky gut issues which are often a reason why we are so reactive to foods. And why we have some of the inflammation which is due to the immune system constantly being triggered by these food and other particles that are getting through the gut membrane,” said Scott Forsgren. Restore Another product Scott Forsgren recommends is Dr. Zach Bush’s Restore, which is a dietary supplement for gut health. Depending on each person’s situation, Scott Forsgren says looking at gut health will often go beyond the diet piece and the intestinal hyperpermeability piece but also into some of those microbial overgrowths, SIBO, SIFO, dysbiosis, and parasites. “The majority of our symptoms are the results of the inflammatory event that occurs — microbes for example. It’s not so much that the symptoms are from the bug. But it’s from the immune system’s reaction to those bugs that creates inflammation,” said Scott Forsgren. Mast Cell Activation Syndrome Scott Forsgren says that people may have heard of mast cell activation syndrome which is the overproduction of histamine. He says there are many other mediators besides the histamine. And this is something that has been known in the autism arena for many years. “Most people who are dealing with chronic infection, chronic toxicity, Lyme disease, and mold probably have some degree of mast cell activation and histamine-related issues,” Scott Forsgren said. “One of the primary triggers for mast cell activation is mold. Hence, it is the top thing people should look out.”  He adds, “But there are so many other triggers like parasites, Lyme disease, co-infections of Lyme disease-like Bartonella in particular. Environmental toxins like heavy metals, some medications, food, some supplements or temperature change can trigger mast cell reaction. Physical or emotional stress and electromagnetic field also trigger it as well.” Treatment Options Some practitioners might use some pharmaceutical medications like diamine oxidase. Scott Forsgren says it can be helpful for some people especially around eating time if foods trigger their mast cell issues. One product Scott Forsgren recommends is Neuroprotek from a company called Algonot. Lots of people in the autism area are familiar with that product because NeuroProtek reduces oxidative stress and inflammation. Another product that Scott Forsgren recommends is Dr. Ben Lynch’s probiotic combination. There’s also ProBiota HistaminX from seekinghealth.com, and vitamin c helpful as well. On the other hand, Beyond Balance’s Mast-Ease was formulated to support the body’s natural ability to normalize mast cell and histamine response, as well as aid in reducing inflammation. “People do find that they have an impairment. And they lose hope that they can get well. They feel as though their body is failing them in some way and not supporting them in their recovery,” said Scott Forsgren. “So, I think it’s critical to get into the gene conversation. to look at the epigenetics or what are the influencers of our genes rather than the potential for something to go wrong.” Sauna Therapy While Scott Forsgren recognizes the benefits of doing sauna sessions, he says it is essential to talk to your health practitioner to guide you. This is to make sure the timing is right, make sure they are sweating and that they got the proper support.  You must also make sure you are getting electrolytes and detoxifying well before doing saunas. “What we don’t often think about is the toxins that are getting inside the body after the sauna session is over. So, make sure your liver, kidneys, lymphatic system and extracellular matrix are all optimized,” said Scott Forsgren. EMR And EMF Dr. Dietrich Klinghardt was talking about the adverse effect of EMR and EMFs since way back 2006. However, there were a lot of skeptics then. But as the years went by, more studies show that EMFs are stressors to our cells. Dr. Dietrich Klinghardt also talks about how they make molds and other organisms that are within us more aggressive or produce more biotoxins. Scott Forsgren says to be alert if you have mold from water damage in your home environment. Because the exponential increase in EMF is probably another reason why those molds now produce more toxins that influence our health. “Endogenous retroviruses that we are conceived with are not necessarily viruses that we acquire. Historically, endogenous retroviruses have not caused us problems from a health perspective. Reducing EMF exposure certainly plays a role in some of those viruses that might be stressing our system as well,” said Scott Forsgren.  He adds, “It’s not possible to eliminate EMFs. The focus has to be how to reduce our exposure. Avoid exposure when sleeping because that is when the body is regenerating, restoring and repairing. Nothing should be plugged in. Dr. Klinghardt even recommended turning off the circuit breakers to the house. And minimize use during the day.” As for choosing the right bed to sleep in, Scott Forsgren says you don’t want metal in your bed or box spring. Bottom line, the mattress should have no metal and recommends OrganicPedic.  In dealing with Lyme disease naturally and all other chronic illnesses, even blankets should be safe and free of EMFs. A good brand is Little Tree Group, which according to their website, is a company who makes EMF/EMR Protective Blankets and Bed Canopies, plus other protective products, CES Units, and Earthing Products. Dirty Electricity Scott Forsgren also advises being wary of dirty electricity which runs on electrical outlets because it creates health stress. He also recommends getting meters to measure how much dirty electricity you have in your environment. “Have a building biologist to do some thorough testing to evaluate your home. EMF sensitivity is correlated to the level of heavy metals in the body and metal toxicity,” said Scott Forsgren. “And focusing on detoxification help over time to reduce electromagnetic hypersensitivity.” Scott Forsgren also reveals that you cannot detoxify when you are constantly exposed to EMFs. So, you have to be a little bit careful about how quickly you reduce your exposures. Kryptopyrroluria Kryptopyrroluria or KPU is a condition that Dr. Kinghardt feels that probably 80% or more of people with chronic Lyme disease and similar conditions are dealing with. Scott Forsgren says this is also common in the autism population. It can be something that we inherit or induced in us by different types of traumas or chronic infection. Or it can be something that is epigenetically turned on by influences like intrauterine or birth or childhood or transgenerational traumas, etc. Scott Forsgren says white spots on fingernails is a sign of zinc deficiency. Not recalling your dreams and depression are also symptoms of KPU. KPU is a significant loss of zinc, B6, biotin, manganese, omega 6 and other nutrients. “It can also lead to higher levels of heavy metal toxicities. Addressing KPU can also help from that perspective,” said Scott Forsgren. “Slowly detox because if you come in with huge amounts of zinc, you can start releasing metals and things in the body and create an inflammatory situation.”  Scott Forsgren also recommends the brand BioPure Core-S which contains KPU supporting nutrients. SIBO There are breath tests that you can do for SIBO that look at hydrogen and methane. Scott Forsgren says the GI-MAP (GI Microbial Assay Plus) stool test and MegaSporeBiotic help in this realm. Another worthy product to try is Bio-Botanical Research Biocidin. “Unfortunately, SIBO and SIFO seem to be fairly common issues that people are experiencing. Parasites can also play a role in this GI dysbiosis,” Scott Forsgren said.  He adds, “One of the challenges with parasites is that people often think of them as third world country problems. But it’s not true. Testing for parasites is poor. So do different systems of energetic testing like autonomic response testing or Dr. Simon Yu’s acupuncture meridian assessment.” But Scott Forsgren also says but you still have to know what questions to ask your practitioner to explore the issues for your specific system. “The more broadly we can cast the net, the broader the number of things we look at, the more likely it is that people can make progress with the number of things that we talked about,” said Scott Forsgren. Furthermore, Scott Forsgren says that the broader we cast the net, the less aggressive we have to be with any one of them because we’re just taking a more holistic approach. That’s why he believes Naturopathic Doctors are a phenomenal fit for a condition like Lyme disease since they are used to looking at all of the different factors. Dental Contributors Dental issues can be a significant factor for health. Scott Forsgren believes that dental problems can be part of setting the stage for why we later develop a condition from mold or Lyme disease. He says incorrectly removing mercury amalgams can lead to a more significant health problem. And it’s the same for root canals. “Tonsils can also play a big role. A little bit different from the dental piece. But it’s not uncommon for people to have tonsil issues,” Scott Forsgren said.  He adds, “People dealing with chronic Lyme disease and similar conditions do have some really good oral health program in place. When we reduce the burden of microbes in the oral cavity, we are also reducing stress on the rest of the body.” PANDA Scott Forsgren says PANDA is where Lyme disease might have been 10 to 15 years ago concerning acceptance and awareness. It stands for Pediatric Autoimmune Neuropsychiatric Disorder Associated with Streptococcal Infections. PANDA is a condition affecting a small group of children and results in a sudden and abrupt onset of Obsessive Compulsive Disorder (OCD) symptoms. But Scott Forsgren doesn’t see PANDAS or PANS similar to autism, chronic Lyme disease or Alzheimers. He feels it’s more associated with chronic infections, chronic toxicity and the environment which we live. Because we have so many things that are just not supportive of our health. “Eventually we’ll see that there a lot of the same underlying factors. And the presentation tends to depend a lot on the age in which we encounter these things. It also depends on whether or not we’ve already neurologically developed or not,” said Scott Forsgren.  Lastly, he adds, “I definitely would not trade my experience because it has brought me to my purpose and passion. And the thing that excites me about is waking up every day and be able to help someone else that’s going through a similar journey. But because the symptoms are not initially apparent, it takes time, and a lot of testing before it gets diagnosed. My guest Scott Forsgren is no stranger to this disease, and he’ll tell us how he survived Lyme disease in this episode. Scott Forsgren is a health coach, podcaster, and health advocate who took years to recover from Lyme disease. He says Lyme disease can be a complex and challenging condition for people. And the more broadly that we cast the net, the more potential we have to optimize health and wellness. Early Signs Scott Forsgren shares that his journey started 22 years ago when he had a tick bite in Northern California in 1996. But he didn’t think anything of it at the time because Lyme disease wasn’t something anyone talked about back then. There was very little information available. “So, I did find for several months, around April 1997, that I had the flu on steroids. It was a tremendous sickness over several days. Those symptoms continued for several more months. And I wasn’t sure if I’d even survive that experience,” recalls Scott Forsgren. Every system of Scott Forsgren’s body was affected. He had vision problems, fevers, joint pain, muscle pain, lots of gastrointestinal problems, cognitive issues regarding brain fog, and memory loss. There were also episodes of numbness and tingling, tapping sensation in his left foot, tremors, internal vibration, muscle twitches. A lot of these conditions can also lead to anxiety and depression and obsessive-compulsive behavior. Finding A Cure From 1997 to 2005, Scott Forsgren saw about 45 different practitioners. The majority suggested that it was all psychological and that he should work with a psychologist or psychiatrist. Scott Forsgren did get a diagnosis of chronic fatigue syndrome and fibromyalgia. He even had a doctor who suggested multiple sclerosis. “I did work with a fantastic medical doctor in Arizona. There were a few years was good. Then in 2004 all of the symptoms came back,” said Scott Forsgren. “I do think Lyme is a worldwide problem. When someone has Lyme, there’s more than just Lyme disease that we need to look at to maximize the potential for our recovery.”  Starting Over Scott Forsgren went over to the American College for the Advancement of Medicine and looked for a new functional medicine doctor. He wanted someone that was Holistic and who would look broadly at all the potential issues. He was then referred to an acupuncturist who worked in an outlet mall that was doing computer-based energetic testing. Scott Forsgren recalls that the acupuncturist advised him to have a doctor to test him for borrelia or spirochaete. Furthermore, Scott Forsgren was also encouraged to see if there were some food that might be stressing his body. Turning Into A Believer Scott Forsgren was a skeptic, but when he got himself tested, everything the acupuncturist said was right. That piqued his interest in this realm of energy testing. Over the years, acupuncture became his number one tool to guide my journey back to wellness. “So, one of the important things is to keep an open mind. A lot of things that I would not have been open to 20 years ago when this whole journey started are the things that I ultimately now have found to be the most beneficial,” said Scott Forsgren.  After Scott Forsgren got officially diagnosed with Lyme disease, he also started learning from Dr. Dietrich Klinghardt. Dr. Dietrich Klinghardt has a system of energetic testing that Scott Forsgren learned over the years as well as utilized and benefited from. He also did work with Dr. Klinghardt as a patient. And according to Scott Forsgren, that was the best decision he ever made overall in his health journey. Autonomic Response Testing Autonomic Response Testing (APT) is a system of muscle testing. But Scott Forsgren says there is no perfect system. He says we use these systems to uncover information and get us closer to the truth.  It’s also a way to understand the things that are stressing the body that may need some support. Scott Forsgren believes some of us have less than optimal detoxification capacity to even end up with the condition that is related to Lyme disease and many others. But they may not manifest symptoms at all. “I do think that one’s detoxification capacity plays a significant role concerning symptoms that we develop. Dr. Dietrich Klinghardt either suspects or believes that probably everyone has been exposed to some Lyme related organisms at some point during their lifetime,” Scott Forsgren said.  He adds,” I think genes do not tell the entire story, but I think epigenetics does. It influences our genetic expression. We change how our genes express through stress management, nutrition, detoxification and joy in our life.” Scott Forsgren also reveals that most people who are dealing with Lyme disease very type A+ personalities and perfectionists. So, he thinks there are aspects of certain characters that are always in that sympathetic dominant mode. Key Things To Regain Balance First is removing the impact of negative emotions, thought patterns, emotional traumas, and conflicts. Scott Forsgren also advises looking at your environment. Do mold or biotoxin exposures surround you daily? Because that’s one of the critical causes of harmful exposures. Scott Forsgren also advises to look at nutrition and gut health as well as explore detoxification. He says we need to create an environment that is supportive of our healing. “Look for parasites and other microbial overgrowths whether it’s Lyme, co-infections or fungal issues like yeast. Viruses can play a big role as well,” said Scott Forsgren. “Also explore dental contributors like amalgam fillings, root canals or cavities. Rewire the limbic system which is part of the brain that includes the hypothalamus.” Limbic System The limbic system is called the feeling and reacting brain. Scott Forsgren says it is involved in determining our level of safety regarding the things that we might encounter, smell, see, hear and taste. Scott Forsgren explains that you can think of it as the alarm center and anxiety switch. It influences the functioning of our immune system, hormonal system, endocrine system, autonomic nervous system. Ultimately, it is involved in blood pressure, heart rate, breathing and digesting. Any of these things can trigger a limbic system impairment. “If someone doesn’t resonate with having had emotional trauma or conflict that set the stage for their condition, it creates a trauma which we need to process in some way,” said Scott Forsgren. Using Saunas Saunas are good, but Scott Forsgren says some people may not entirely be ready for it. We can mobilize some toxins internally as well. So we have to make sure the routes of elimination are open. “When you use a sauna, you do sweat things out, but at the same time, you can mobilize some toxins internally. A sauna is a good tool, but you need to listen to your body and not force something that maybe they’re not quite ready for,” said Scott Forsgren. “Detoxification is a life-long focus. I do it every day.”  He adds, “When we’re recovering from a chronic condition like Lyme disease or mold exposure, detoxification is critical. Microbes like candida and parasites may be present in the body because the body is utilizing them or allowing them to be there to concentrate heavy metals, to help us detoxify. So not all of these organisms are necessarily bad or that we need to get rid of.”  Testing For Molds If someone is not recovering from chronic Lyme disease despite doing all the right things, Scott Forsgren advises that they have to explore the mold piece. He urges everyone who is suffering from Lyme disease, making sure that your environment is safe from a mold perspective and an EMF perspective. Because bottom line, you want to create a sanctuary for recovery. “About 25% of the population has a genetic predisposition to illness from water-damaged buildings. And similar to Lyme disease, there is no perfect test for molds, so it’s critical for people to explore,” said Scott Forsgren.  Starting with your home, Scott Forsgren says any place where you spend time on a regular basis should be suspect until you explored it. And then if an issue is identified, you may be able to do some remedies like moving to a new environment. Putting in some good air filtration device in the environment is also very helpful. It’s not a solution to a problem, but it’s something that can fix the situation while you are addressing the core issue. ISEAI International Society for Environmentally Acquired Illness (ISEAI) is an organization that has come together reasonably recently with doctors that deal with biotoxin illnesses. Some environmental professionals are part of this organization as well. According to Scott Forsgren, the organization is an excellent resource if you dive deeper into learning the cause of illnesses. The ISEAI website states that it is a nonprofit professional medical society whose aim is to raise awareness of the environmental causes of inflammatory illnesses.  They also support the recovery of people affected by these illnesses using an integration of clinical practice, education, and research. Better Health Guy Website The website started in 2005 before Scott Forsgren had the Lyme disease diagnosis. Three months after the site launched, Lyme disease became the focus of the site. “The most interesting area for people to check out is the podcast where I interview experts as well as articles he has written over the years. Part of what’s on the blog are notes from different conferences that I have attended over the past ten plus years,” said Scott Forsgren. LymeLight Foundation Another organization that Scott Forsgren wanted to highlight was the LymeLight Foundation. The LymeLight Foundation is an organization that provides treatment grants to children and young adults 25 and under for Lyme disease treatments. “I think it’s critical for people to maintain hope. There are so many things that have improved in our understanding of Lyme disease and chronic conditions over the last several years,” said Scott Forsgren. He adds, “Lyme disease is a messenger for many of us. And the process of going through it is life changing. A review of how the gut and brain communicate through both the nerves and gut microbial metabolites is discussed. They also talk about how intestinal permeability and brain permeability are associated and what you can do about it. As a naturopathic clinician with over 40 years' experience, Sandberg-Lewis shares some clinically useful pearls along the way.  About the Expert Steven Sandberg-Lewis, ND, DHANP, has been practicing since 1978, teaches gastroenterology at National University of Natural Medicine and has a private practice at 8Hearts Health and Wellness in Portland, Oregon. He lives in Portland with his wife, Kayle. His interests include mandolin, guitar, writing, and lecturing. Transcript Tina Kaczor, ND, FABNO: Hello, I'm Tina Kaczor with the Natural Medicine Journal. I'm speaking today with Dr. Steven Sandberg-Lewis and our topic is the gut-brain axis. Dr. Sandberg-Lewis has been a practicing clinician for over 40 years now and he is the author of Functional Gastroenterology: Assessing the Causes of Functional Gastrointestinal Disorders, and that has come out in a second edition as of March 2017. He's also adjunct full professor at the National University of Natural Medicine. Dr. Sandberg-Lewis, thank you so much for joining me today. Steven Sandberg-Lewis, ND, DHANP: You're welcome. Kaczor: Alright, so I think our talk about the gut-brain axis is extremely timely because of the media attention now given to the bacteria and the effect of our microbiome on our physical ailments and I think it's beginning to look at how it effects the brain both cognition and mood as well. And so, what I'd like to do is really start at the beginning and can you just give us a quick overview of what exactly do we mean when we talk about the gut-brain axis? Sandberg-Lewis: Yeah, naturopathic medicine seems to always be at least 30 years ahead of the rest of medicine. We've been talking about this a long time but now we have a lot of research to back up what we talk about. So, the gut-brain axis probably has many more players than we're aware of but the ones we know about are, of course, the microbiota, a lot of people call that the 'microbiome'. But it's the bugs. About 100 trillion of them and they are, of course, not just in the colon but in the small bowel, in the stomach, which is not sterile and the oral and esophageal areas. The true meaning of microbiome is the genome of the gut floor which has way more, at least 100X more genes than the human genome, which is 26,000. And when you put the two together, you call it the 'holobiome', which is the human genes and the microbial gene. But really, you need to do that because they interplay so much and the bugs really control our genome so intensely. Then there's that whole genetic piece then there's all the, what we call the metabolon. What the bugs and the enteric cells make, all their metabolic products and that includes secretory IGA, short chain fatty acids, lipooligosaccharide, bacterial hormones and neurotransmitters and cytocinesis. We can talk about all those kind of details. And then of course there's the enteric nervous system speaking to the vagus and the vagus speaking back. And there's the HPA axis and then there's the immune system and the gut. So, it's huge. It's so much talk it's deafening crosstalk. Kaczor: It is interesting and one of the pleasures, I think, I've been practicing nearly 20 years and I know that you've been practicing over 40 eyras so, it's quite a long time to watch the evolution of thought processes in medicine as well as the population at large. And in some of the folk medicine even, naturopathic medicine, but good old folk medicine, an apple a day and staying regular and keeping the bowels moving, it's amazing how that comes back at us and now we're talking about it in scientific terms which is fascinating to watch the pendulum swing. Okay so, when we talk about how they communicate, you gave a little overview of some of the ways, when you mention the vagus system, what do you mean exactly? Sandberg-Lewis: On every new patient, I like to have them open their mouth, stick out their tongue, take a look at their palatal arch and their uvula and then have them say 'ah'. When they phonate, we've all done this, check the vagus nerve. The place you can check the vagus nerve directly is there in the palate because the levator palatini muscles on either side raise the palate when you say 'ah' and when you phonate. So, I'd like to see both arches go up symmetrically and not an asymmetrical rod. Occasionally you see nothing. The patient says 'ah', nothing happens. There's no palatal rise and you can have them do it over and over and nothing happens. So, that's a sign that the vagus isn't really firing the way it's supposed to and there are lots of ways to try and improve vegul tone. But that's a good thing to know about your patient. The next thing is the vagus nerve gets sensory information from the enteric nervous system and the neuroendocrine cells in the gut as well as the epithelial cells. It gets input in actually 90% they assume, 90% of the crosstalk is from the enteric nervous system to the Brian through the vagus. So, it's mostly the gut talking to the Brian and then the brain through the vagus talks back about 10% of the time. So, there's direct transmission through the nerves and then there's all the cytocinesis and other factors and neuro peptides that also speak through the humoral method. Kaczor: So, what you're saying is the gut itself is sending signals through the vagus nerve to the central nervous system and effecting what exactly? Sandberg-Lewis: So, yeah. It's pretty interesting what the gut is interested in talking about. Yeah, you wonder, what does my gut have to say? What does it care about? So, what the gut is saying, the 90% of impulse is going from the gut to the brain, it's talking about the shape and the consistency of the bowless of food moving through and scraping up against the ... rubbing up against the mucosa. The sheering forces of the bowless against the mucosa. That's stimulating serotonin locally but there's also this ... That's what the gut's interested in. Is, what kind of food is it, what's the shape, what's the consistency and what does it feel like as it rubs against the enteric cells? This seems to stimulate taste receptors on the antero endocrine cells that are scattered throughout the mucosa and give information about the composition of the food, there's, again, there's all these neuro peptides and hormones, GI hormones that are released as well. But directly through the vagus nerve, we think it's mostly the gut talking about it's scratchiness of the food and the size and the consistency and the kind of food. Kaczor: Okay. And I have to clarify for my own self, when you say 'taste receptors', you're going beyond the tongue? Is that what I hear you saying? Is this "taste receptors" that you say along the GI tract, how does that work? Sandberg-Lewis: Air quotes, well, our genius in our midst, Paul [Calmens 00:08:18], he's been telling me for years that there are taste receptors throughout the entire gut. And especially most pronounced is the bitter, bitter receptors. And I've tried to go into the research and find out what exactly they do. We don't know a lot about what they do but we know that bitter receptors are not just in the tongue and the mouth, they're throughout the whole gut and they seem to trigger the release when they're stimulated. They trigger the release of ghrelin and glucagon peptide one which have to do with blood sugar balance and hunger and my guess is, Flip Wilson used to say, "The devil made me do it" when he did something that he thought was nasty. But I really think that in many ways, the GI flora and the food that we eat interact to create cravings so that the body can get what it needs. And if you get more ghrelin, you get hungrier. And certain other, like GLP, maybe you're gonna create more sugar. It's really important, of course, to eat food ... I think this is why Indies think it's so important to eat foods that's close to nature because once you get these synthetically modified foods or their consistency and their flavor and their compositions is altered, it probably throws off these natural mechanisms that tell us when to eat, when not to eat, when our blood sugar is fine, when it needs to go up or down. So, these are really finely tuned things. Kaczor: Yeah, it's interesting in context, again, of traditional medicines because it automatically makes me think of Ayurvedic medicine and other traditional practices that naturally balance the flavors on the plate, that's a big part of making sure each meal is healthy in that tradition. So, let's talk about the microbiotas some more. You mentioned briefly that its metabolites are part of the talk between the gut and the brain, can you elaborate on that? Sandberg-Lewis: Yeah. First of all, I wanted to mention that the metabolic byproducts, the products of the microbiota, it's huge, it's huge. And Emerson Mayor's book, the gut-brain connection, he makes a quote that 40% of the metabolites in human blood are derived from gut flora, which blew my mind completely. And so, I said, "Where is he getting that?" And I looked, I found it in two or three different articles. And so, to me, that means 40% of our blood chemistry is derived from the intestinal flora. And that makes sense since there are at least 10-to-1 more of them than there is of us in terms of cells. But I never really put that together. So, there's these metabolites in our blood derived from the flora that do fine tuning of eating behavior, mod, blood glucose, digestive secretion, absorption, motility, just it's mind blowing. It's so important and it makes sense. You can understand that when you go ahead and even just take a broad spectrum antibiotic, we know that greatly increases the risk of kids and even adults, getting inflammatory bowl disease, especially Crohn's. Just so many effects on immunity and the balance within the body if we knock down the bacteria or alter them or decrease their diversity. So, pretty important, pretty major stuff that everybody's been messing around with since the 1920s and '30s with antibiotics. Kaczor: Mm-hmm (affirmative). Yeah and you mention a metabolite that caught my attention because I think it's at least in our naturopathic circles, we're paying a lot of attention to the role of that lipopolysaccaride and the LPS, for short. Can you talk a little bit about that? Because I feel like that's, as far as I can see, getting a bit of attention these days in how the gut and brain effect one another. Sandberg-Lewis: Yeah well, I'm glad it's getting attention, it deserves it. You know, every physician knows about LPS in one particular way and that is, it is the cause of septic shock. It kills people if the LPS is high enough. What we don't usually hear about and we're starting to get more and more research on is, what about physiological levels of LPS when it's not super high? Sandberg-Lewis: LPS is used in research, they inject it into lab animals to activate the NF kappa B pathway of inflammation and there's a tremendous amount of it. These are from the gram-negatives. The gram-positives also have an inflammatory precursor like this, which is the peptidoglycan. But, mostly we talk about the gram-negative because it's so potent and there were a million copies or so of LPS in each gram-negative microbe and it's not just something that gets emitted when the bug dies, it's also just when it's replicating or if you take an antibiotic and weaken it a little bit, they don't have to die to give off LPS. And it's thought that in the adult human gut, you have up to a gram of LPS, a thousand milligrams at any one time. So, it's a major player. There's a lot of it and it can trigger the Zonulin pathway, which leads to intestinal permeability, hyperpermeability, which we know is related to autoimmunity and allergies. Obese humans have up to a three fold increase in LPS compared to lean and maybe some of that also dove tails with the fact that obese adipose tissue has 10X as many macrofacies. So, you got a lot of esocine activity, a lot of TNF alpha. Kaczor: Mm-hmm (affirmative). Sandberg-Lewis: And different types of enteric flora have different amounts of LPS. Or even different potentiates. So, antero bacter are thought to have some of the most potent LPS that can be up to 1,000X more potent than some of the other gram-negative bacteria. So, this is a major toxin, it's a major provoker of inflammation and pre radical activity in all kinds of changes. Kaczor: So, how does LPS effect the brain directly? Since, I hear what you're saying and I know even in experimental animals LPS is a common way to reliably instill an inflammatory process in a lab animal. So, it's clearly a very potent, inflammatory molecule. How does it effect the brain? Sandberg-Lewis: So, the bacteria, we get some bacteria trans locating into the blood but they usually get called out after they travel through the portal vein to the liver by the cooper cells. At least if the liver's working well. You're not gonna have a lot of bacteria in the blood but the bacteria can still effect the central nervous system, even if they don't cross the blood brain barrier and never even get there. First of all, one mechanism is that LPS and the inflammatory cytocinesis that it induces include interleukin one, interleukin six and I mentioned TNF alpha and they can actually up regulate the transcription of these cytocinesis in certain discrete areas of the brain. And then one of the things that happens with that is, within the brain you get an up regulation of indoleamine dioxygenase, which is that enzyme that converts tryptophan to kynurenine and that can move further to quinolinic acid, which is neurodegenerative. Although, there's quinolinic acid, which is also has a positive effect. So, depending on how it goes through the pathways, you can have neuro degeneration up regulated. And studies show that depression, anxiety and insomnia can issue from high levels of quinolinic acid. So, there's that. There's also cognitive deficits and in my book, very important, is increased visceral sensitivity. All the functional disorders of the GI tract, there is increased visceral sensitivity meaning, people perceive their own motility and movement within the gut as pain or strong discomfort. And man, those patients are strong to treat because if you start to activate their GI tract and get it moving again after it's been atonic for years, then they're complaining that they're up all night with abdominal pain. And that's a tough one, we're trying to learn more about how to deal with visceral hypersensitivity. But, it's thought that LPS is one of the things that triggers that too. Kaczor: So, I have a two part question. I guess, in our clinical assessment of LPS, is there a means, I mean, I know that we could do testing for small intestine bacteria overgrowth through breath testing but, is there any blood test, I guess is what I'm thinking? Can you tack on any blood test to gauge LPS levels and the second part of this question is, what do we do about it? I suppose treating the gut in a totality dysbiosis present, is the short answer. Any clinical pearls are certainly welcome. Sandberg-Lewis: Yeah. So, you can, this is available, you can measure LPS. You can measure LPS binding protein, I believe as well. And you can, in your patients, you can, of course, measure zonulin, which gets up regulated by LPS. So, yeah. By all means, start experimenting with that and then see if you can get the levels down. Now, yeah. I'm known for spearheading along with some other really busy physicians and researchers. The treatment and in our case, more the natural treatment, of small intestine bacterial overgrowth, you just can't get away from it. It's so key to, we used to say, "Death begins in the colon" when I was in school in the 1970s and now, I think it makes sense to say, "Disfunction and autoimmunity begins in the small intestine". It's just associated with so much and that includes neurodegenerative diseases, like Parkinson's as well. A practical thing, yeah. Learn how to test for, interpret and treat ... Use your testing and learn how to treat SIBO. Both the hydrogen, methane and hydrogen sulfide types. And don't throw out the yeast with it either because they often go together and get that metabolite base 40% of the blood. Get it into a functional mode instead of a dysfunctional mode. Kaczor: Mm-hmm (affirmative). Mm-hmm (affirmative). Yeah. And I know, I will say that, you lecture and write a lot on the clinical aspects of this. So, anyone who wants further information can certainly start Googling you and find lots and lots to followup on. I do want to ask another question because this issue that we are in for the Natural Medicine Journal this month is a special on the microbiota and the microbiome. How do probiotics specially effect the gut-brain axis? Sandberg-Lewis: I don't know that we have enough yet to really have a great answer. Although, there are some docs out there that really have a strong handle on the strain specific effects of probiotics and people like Jason Hawrelak who is a ND and teaches at Western States and practices in Australia, he has totally got that covered. So, I would highly recommend looking at his website Jason Hawrelak, Hawrelak. But I mean, we know that there are studies that show that fermented foods significantly reduce anxiety, especially social anxiety. And there's a lot more research going on on strains, specific things that show that there are specific effects on anxiety and depression. But it's still really early so, I can't say I have a really good picture of that. When people ask me about probiotics, I usually say, "I don't know anything about probiotics". They don't believe me but, yeah. Kaczor: So, is it accurate to say that you advocate the whole foods diet, plenty of prebiotics in the form of fibers and resistant starches and things like that and then trust that if that is done well and consistently and then, of course if there is other treatments to kill undesirable bacteria etc. but complimenting ... I mean, the way to encourage the good bacteria is to give the prebiotics more in your view? Sandberg-Lewis: Well, the problem is a lot of my patients, because they have overgrowth, they can't tolerate prebiotics and fiber. So, when I first start working with them, we can't really use those things except very specific types. We know that partially hydrolyzed program seems to be actually beneficial for people who have overgrowth, sometimes used along with rifaximin in treatment of hydrogen SIBO and it increases the effectiveness of that. But, there's some GOS's that that may also be well tolerated. But that's a problem in the beginning because if you have overgrowth and then you feed them with a prebiotic, then it just increases symptoms and problems. So, it's a fine balance, it's a fine balance. But, yeah. I really encourage my patients to eat whatever fermented foods, probiotic foods that they tolerate. So, we use lactose-free, fermented dairy products, we use pickled items like kimchi or sauerkraut, the real stuff not the fake stuff. The refrigerated kind. Or homemade. And things like that. I think all traditional groups of people around the world have their own probiotic foods. Some of them pretty hard to even relate to, like haggis in ... where is that? Scotland. Where they eat the goat stomach that's fermented. Fish and rotten fish in the northern areas of Europe. There are some really interesting things you'd think, "Why the hell would people ever invent that food?" But these things have tremendous fermentative capabilities. And one thing that I'm sure NDs understand this but it comes up a lot with patients asking, they'll say, "So, you don't want me to eat any fermented food, right? Because I already have ... You want me on a low fermentation diet". And I say to them, "Well, no that's not what I want you to do". Unless you have a histamine sensitivity and you can't handle foods that are fermented, I want you to use those things because if you eat a high fermentation diet that has carbohydrates that are easily fermented, that produces gases. Hydrogen, carbon dioxide, maybe ethanol, methane, hydrogen sulfide in the gut and causes distention and pain and changes the stools and can cause bloating. But if you eat a food that is a fermented food, the gases have already come off into the atmosphere in the process of fermentation and now you're just getting all the bacteria and the great metabolites without the gas that causes the symptoms. I think that's an important differentiation. Kaczor: Well, yeah that's a great way to put it. To help them and us understand it a little bit. One last question, we'll end on a fun question, when you hear the term 'gut reaction to something', because we're talking about the gut-brain axis and someone says, "I used my gut" or gut instinct, gut reaction, what do you think? Sandberg-Lewis: Well, I'll bet physicians with different backgrounds have different ways of interpreting that but clearly if 90% of the input in the gut-brain axis is coming from the gut and if you think about it, if virtually every neuro peptide and GI hormone that's produced in the gut effects insulin and blood sugar and we know the brain suffers within minutes from blood sugar that's too low whereas the other organs may not care for quite some time and oxygen as well, of course, to the brain within three minutes. There are major effects on life and death related emotions that take place when the gut is feeling like something is wrong and it's gonna make more jittery molecules instead of more serotonin and gaba. And that's gonna have very rapid effects on the mood and on the functioning of the person's nervous system. Kaczor: Mm-hmm (affirmative). Alright well, I sure do appreciate the time you took to talk to us today about this. It is a huge topic and I am excited, as a naturopath that everything is coming back to a source, the GI tract, I mean and we've always been taught that we have to remedy the gut and get that in order before we can really keep someone in an optimal health state. So, sometimes that's harder than others and I do appreciate the time it took to enlighten us today with the gut-brain axis and it look forward to talking to you probably in the realm of gastroenterology again in the future. In today's episode, Dr Nirala Jacobi is in conversation with renowned SIBO specialist, Naturopathic Gastroenterologist and Professor, Dr Steven Sandberg-Lewis.   Dr Sandberg-Lewis is a highly regarded and well known naturopathic physician. He has been a professor at the National University of Naturopathic Medicine (NUNM) since 1985, in Portland, Oregon.  Dr Sandberg-Lewis has a part-time clinical practice and is the author of several Townsend Letter award-winning articles, and of the medical textbook ‘Functional Gastroenterology: Assessing and Addressing the Causes of Functional Gastrointestinal Disorders'.  He has also been named one of the Top Docs in Portland Oregon in 2014.   Topics discussed include: How are proton pump inhibitors (PPI) related to SIBO? How the 3 main secretions at the upper gate of the small intestine are bacteriostatic, and how hypochlorhydria and PPI use can support SIBO development. Heidelberg testing and Weakly Acid Reflux. Advanced diabetes and issues related to autonomic neuropathy, such as silent reflux. Laryngopharyngeal reflux, the typical symptoms to look out for, and conditions it is linked to. Enlarged tonsils and the connection to laryngopharyngeal reflux. How do we test for low stomach acid? Riddler's reflex point for stomach acid. Pectoralis major test for hypochlorhydria. Heidelberg test. Bile Reflux - what causes it? Dysplasia and cancer of the lower oesophagus and the theory linking these conditions to duodenal gastroesophageal reflux. What treatment can we do for Bile Reflux? The importance of endorphins and enkephalins in the contraction of the sphincters in the body. Chapman's reflex point to improve the tone of the pylorus (taught by Dr SSL in the GI practicum in November) Nutrients to help to heal Barrett's and prevent cancer include selenium, turmeric, green tea catechins, anthocyanidins (found in berries and colourful fruits and vegetables). Top causes of reflux to pay attention to besides SIBO: Hiatal hernia Hiatal hernia syndrome Pancreatic insufficiency Spasmodic diaphragm Anxiety, or conditions that decrease client's saliva or makes saliva more acidic (eg sjogren's, elderly) Digestive Support for lack of saliva production Use zanthoxylum (prickly ash) - 1-2 drops on the tongue, and/or bitters as a digestive stimulant. Pilocarpine lozenges What is Dr SSL's opinion on methane and PPI use - being there there is potential for PPI users to have less methane output, and conversely for Hydrochloric Acid (HCl) users to have increased methane production. What is the appropriate dose of HCl for patients and should we use capsules or tablets? The combination use of bitters plus HCl. Rebound hyperacidity - why can it occur after taking PPIs? RICHARD KITAEFF, M.A., N.D., Dip.Ac., L.Ac., naturopathic physician and acupuncturist, was the first Westerner to graduate from Meiji University of Oriental Medicine in Osaka, Japan. Kitaeff was also licensed as an acupuncturist by the government of Japan in 1975 and was a member of the first graduating class of naturopathic physicians from Bastyr University in 1982. He interned at the Osaka Medical College Pain Clinic and the Kyoto Pain Control Institute. Richard Kitaeff's research on acupuncture and endorphins, carried out at the University of Washington School of Medicine and published in the journal Pain, was the first to verify the analgesic effect of acupuncture through objective (EEG) measurement. As a pioneering North American acupuncturist, he founded and directed acupuncture training programs of the Northwest Institute of Acupuncture and Oriental Medicine and Bastyr University in Seattle and was founding president of the Acupuncture Association of Washington. He has additionally taught acupuncture and pain management courses and seminars at Meiji, Friends World College, University of Washington, City University (Seattle), Pacific Acupuncture College and Southwest Acupuncture College. He has been an invited speaker at conferences of the American Association of Holistic Medicine, the American Association of Naturopathic Physicians, the Washington Association of Naturopathic Physicians, the Washington Association of Physician Assistants, the Northwest Naturopathic Conference, and Northwest Hospital. Kitaeff has also published articles on acupuncture and pain management in Pain, the Townsend Letter, the North American Journal of Oriental Medicine, and book chapters in Optimal Wellness, the Textbook of Natural Medicine, and the Encyclopedia of Natural Medicine. Since 1984, Richard Kitaeff has owned and directed New Health Medical Center in Edmonds, Washington. New Health Medical Center is a multidisciplinary center integrating North American, European and Asian techniques of pain control, systemic detoxification, allergy elimination and stress management. In this interview I discuss the connection between infections and autoimmune disease which I call the Infection Connection. That being said, let's get on with it. Let me introduce our speaker for tonight. Dr. Nikolas Hedberg is a good friend of mine. I've known Nik for, I think somewhere around 10 years now, we both went to one of the same schools in our careers, not at the same time, but we, I guess, met at a, I think, an internal disorder conference in New Orleans, I think was the first time about 10 years ago. I've followed Nik's work and his career and he's done some outstanding work. Nik is a board certified Chiropractic Internist by the American Board of Chiropractic Internists, he received his bachelor training in Exercise Science from the University of Florida, received his Doctor of Chiropractic from the Texas Chiropractic College in Houston. He currently practices in Asheville, North Carolina and focuses on Lyme disease, thyroid disorders and autoimmune disease. He's the author of the book "The Complete Thyroid Health and Diet Guide", which is a comprehensive guide to diagnosing and treating thyroid disorders. He lectures at a lot of integrated medicine conferences, and he's published in a lot of journals. He teaches Functional Medicine concepts through many forms including Functional Medicine Town, Professional Co-op, and with Hawthorn University. His particular interests lately is the role of stealth infections in chronic disease and in that effort to get this message out, he's created a program called "The Infection Connection" and does some online training in these concepts, and he'll tell you more about that as the night goes on. Little known to everyone in the professional world but known to me, Nik is an accomplished opera singer actually, so you'll hear that when you hear him talk, you can hear that come through in his voice. Anyway, Nik thank you for being on. I know you've been on Clinical Rounds before and thanks for coming back and being a guest again. Nikolas: Thanks for having me on David, it's good to be here. David: Great. I mean this is a fascinating topic and a topic that's been really been, sort of, obsessing me for the last couple years. As you can see by the content or the topic of our Core III seminar. I've recently written pretty extensive articles on these concepts in Townsend Letter and Naturopathic Doctor News and Review and in other forms. I'm really interested and jazzed about tonight's call and interviewing you because I hope to learn more about this as well because it's literally emerging science day by day. I think it would be a full time job just to keep adequate surveillance of the literature on just what we're going to talk about tonight and stay current with it. Anyway, I know that I kind of started having to really school myself on this stuff out of sheer necessity by virtue of my patient population and how it's changed over the years. I guess we'll talk about that a little bit more but I wanted to really learn a little bit more how you got started in managing patients with chronic infections or stealth infections and how you found your way into really almost sub-specializing in this area of functional and integrated medicine. Nikolas: Right, it was actually about eight or nine years ago that I was actually listening to you, David talk about the connection between [inaudible 00:08:55] and autoimmune thyroid disease, and ever since then this has really been an interest of mine. I knew it was something that I need to learn a lot about, started seeing more and more patients with Hashimoto's, and chronic fatigue and various autoimmune diseases. You start doing all the things you know in functional medicine, sealing the gut, vitamin D, you know etc. etc., and you can do all that but there's that group of patients, that population that they just don't respond to those things and I found with a lot of those patients when you dig a little bit deeper,
