Join for this information packed Q&A!  Helen Is there any point having my blood test done when every cycle is different to the last? Symptom wise and sometimes length wise. Lesa I have been keto, carnivore and fasting with extended fast periodically for a year and a half with no weight loss. I am post menopausal and started HRT 6 months ago. I feel all of the thyroid symptoms but can't get anyone to listen. Thyroid labs look good except T3 is below optimal and RT3 is 30.1. I can't get into the endocrinologist for 6 months and fear he won't listen either. I feel in my heart pure T3 would get me on the way to healing and finally get my insulin down to be able to lose the 40 pounds that I gained in 4 months. I also have high ferritin that I read can keep insulin high. Gail Dr. Brownstein mentioned a specific type of iodine supplement to take. I'm currently working with my physician on my iodine levels and was wondering what the supplement was called/ what it included. If I remember correctly, he said you needed iodine and iodide in it? Emma I am 50 years old and for the most part feel pretty good despite the fact that I am in perimenopause. I am struggling with perimenopause weight gain, (gained 30 pounds since October) sleep. I am finally getting my hair to grow back in the front and my dry skin is getting better, my nails are healthy again. I eat as clean as possible. At this time, I can't afford to go see a functional doctor. So, I have been thinking of buying over the counter estrogen and progesterone bioidentical products. What would you recommend? Thyroid issues: testing (like TSH, T4, T3, free T3, Free T4, Reverse T3) – Dr. Jason Jones Elizabeth City NC, Chiropractor In this month of MAY, being Women’s Health Month, we are going to discuss one of the common conditions affecting women - thyroid issues. These health conditions can come in form of hypothyroidism, hyperthyroidism, or autoimmune thyroid diseases such as Hashimoto’s thyroiditis or Grave’s disease. We are also going to discuss blood tests for thyroid function – like TSH, total T4, free T4, Free T3, rT3, and others. These tests are parts of important for diagnosing and treating these thyroid issues. Usually, a combination of tests is needed to establish the full nature of your thyroid health, but sometimes, a single test is enough to draw some conclusion. The values of the thyroid tests help doctors determine the kind of thyroid issue a person is having. What is the thyroid gland? The thyroid gland is a butterfly-shaped endocrine gland that is located at the front of your neck. The job of this gland is to make thyroid hormones, which help the body use energy and keep the heart, brain, muscles, and other organs working optimally. The thyroid hormone is secreted into the bloodstream and transported to every tissue in the body. The majority of this hormone is thyroxine, also called T4 (containing four iodine atoms). This T4 is often converted to triiodothyronine (T3), through the removal of a single iodine atom. This effect takes place mostly in the liver and in some tissues where the T3 acts, including the brain. Another gland called the pituitary gland secretes the thyroid-stimulating hormone (TSH) which controls the amount of T4 produced by the thyroid gland. The amount of TSH produced is dependent on the amount of T4 the pituitary sees. More TSH is often produced if the pituitary sees very little T4. But once the T4 goes above a certain level, the production of TSH is automatically shut off. You can liken the work of the thyroid and pituitary gland to the action of a heater and thermostat. When the heater is cold, the thermostat reads and turns on the heater, and when the heat rises to a particular level, the thermostat senses this and turns off the heater. Having a sufficient amount of the T4 AND T3 hormones is important to maintain the work of the thyroid gland in the body. An imbalance of the thyroid hormones can lead to various health issues, hence various testing methods are used to determine the level of these hormones in the body Testing methods Blood tests are usually taken to measure the amount of thyroid hormone in the body. Some of the tests commonly used to evaluate thyroid function include: TSH tests This is the first test to check thyroid function. It is conducted to measure the TSH level in a blood sample. A high TSH level indicates that the thyroid gland is not secreting enough hormone (primary hypothyroidism). A low TSH level usually arises from an abnormality in the pituitary gland, which disrupts the ability to stimulate the thyroid (hyperthyroidism). Usually, a normal TSH value indicates that the thyroid is working properly.     T4 tests T4 is the main thyroid hormone circulating in the blood. A total T4 gives a measure of the free and bound hormone, and this value can change when the binding proteins differ. A free T4 measures the T4 hormones that are not bound and can enter and affect the body tissues. Tests that measure free T4 -either free T4 (FT4) or free T4 index (FT1) - are more accurate in reflecting the thyroid function when checked with a TSH. A Low FT4 OR FTI and an elevated TSH indicate primary hypothyroidism due to disease in the thyroid gland. A low FT4 or FTI and a low TSH shows an individual has hypothyroidism due to an issue with the pituitary gland. An elevated FT4 or FTI and a low TSH are found in people suffering from hyperthyroidism. T3 tests T3 tests are usually conducted to diagnose and determine the severity of hyperthyroidism. An elevated amount of this hormone is seen in hyperthyroid patients. Some individuals having a low TSH shows an elevated amount of the T3 alone with the FT4 or FTI remaining in the normal range. A T3 test is rarely needed in people with hypothyroidism as it is the last test to become abnormal. People with a severe case of hypothyroidism can have a high TSH and low FT4 and FTI but maintain the normal T3 value. A free T3 measurement is often not reliable, so it is not helpful. Reverse T3 Reverse T3 is structurally similar to T3 but the iodine atoms are located differently, which makes it inactive.   In healthy people, measurement of reverse T3 is useful as it helps determine that if there is a high rT3 which can lead to blocking the ability of T3 to be utilized. At the hospital, they gave her morphine. Sadly, Dr. Warren had developed thousands of stones in her gallbladder. After getting the gallbladder removed, Dr. Warren needed to recover from morphine withdrawals. Even at 16, Dr. Warren wondered why she was born with a gallbladder if she didn’t need it. Just because they cut out the gallbladder, that didn’t mean Dr. Warren was cured. At 19, Dr. Warren found out that she had cancer. However, it was the “good” cancer to have, thyroid cancer. Well, the doctors were ready to have her thyroid cut out, and Dr. Warren had the surgery done. Warren became weight loss resistant, and she started to struggle with depression. When you think about depression, you think about suicide. However, depression can mean waking up every single day with a massive burden. Eventually, Dr. Warren found keto and fasting to change the healing process in her body. [10:30] An Overview Of The Thyroid + What To Know About Reverse T3 The hypothalamus is a thyroid-releasing hormone; it goes from the hypothalamus to the pituitary and the pituitary to the thyroid. When we talk about carbs, fat, and ketosis, it all goes back to the hypothalamus. The hypothalamus is the master control center of all the hormones, not just your thyroid hormones, but all hormones. The Reverse T3 (RT3) Test is one that Dr. Warren recommends. Many doctors have an issue with it. RT3 is an inflammatory marker that gives you a good idea of how your body responds to stressors. When we are in survival mode, our thyroid will shut down. Having flex days will tell your body that there is no stressor. Now, your body can utilize your carbs to produce hormones in your thyroid. Q&A with Karen 1. Can low Iron cause secondary thyroid issues? 2. If you do resistance training 2 weeks of the month and something lighter the other 2, will you loose the gains every 2 weeks?  3. Hi I recently found your website via your interview with Dr Paul Saladino and I am fascinated with your approach to balancing hormones. Every month, my 21 year old daughter, on her first day of her period is violently ill with vomiting and/or diarrhea, for about 5-6 hours and then she recovers and everything is back to normal. This has been occurring for about the last two year. She is normal weight, and active. She follows a loose keto diet - whole foods, with limited processed foods and sugar. Her GP simply prescribed anti nausea tablets, which don't help. Do you have any ideas or directions we can take?  4. I have non-Hashimoto hypothyroidism. I'm on NDT. I've also had adrenal issues in the past but haven't tested recently. I started a carnivore diet 2 months ago. I've been horribly weak, light-headed, shaky and even labored breathing at times. I'm 5'3" and ~98-100 lbs. Dr. Saladino recommended I add more carbs immediately so I added olives, avocado, honey the past couple days but still feel very weak and shaky. Currently: I'm eating 1-1.5 lbs of meat, suet, bone broth, 3 egg yolks and a large romaine salad with cucumbers, olives, avocado, EVOO & lemon juice per day. Prior to carnivore I was veggie heavy Paleo with low amount of animal protein. I was weak and light headed then hence the carnivore diet. I started out feeling better the first try but horrible this time though I see improvements to hair growth and less loss (though dry) and other positive changes. Sometimes feels like adrenals are maybe stressed like you talk about. A naturopath monitors my thyroid/adrenals however we have never tested my RT3. Thank you!  4. Hi Karen I love your podcast I have a quick question for you I eat fairly well lower carb paleo type diet with a few cheats here and there but I have noticed in the last few years that I have been getting progressively more constipated. I have always suffered with it but it seems to be getting worse and there are times I won’t go for 4-5 days at a time. Any suggestions? Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I’m looking forward to sharing with you some of our community’s questions that have come in over the past few weeks… Let’s get started!    Ryan: Whats up Dr. Cabral, I really think what you are sharing with the world is great. I have been listening to so many of your podcasts recently, and am so grateful I found them. I will just ask my question outright. Are there, or could there potentially be options for patients who received Radio-Active Iodine therapy to reverse the damage done to the thyroid? Somewhat similar to you, by the time I was in my late teens I was completely burning out and after a long bout of Mono at 18 I was later diagnosed with Graves Disease (not until I was 23, but I am certain this was the case after I was diagnosed with EBV). Anyhow I had done so much guess work and boughten so many supplements to help my heath all through my late teens and early twenties. Unfortunately I did not understand what it meant to truly live a healthy holistic lifestyle till about the age 25 to 26 right after I got the radiation done. Ever since I have been fully entrenched in functional medicine/holistic learning, but also struggling with unlearning all the harmful lifestyle choices that lead to me developing Graves. I am awaiting my Hair and mineral test results, and will be doing some of the other labs in the upcoming month or so, but am waiting to see if any promotions are run in a few weeks for the holidays. Are there supplements I can take for possible scarring and tissue damage done? Could it make any sense to see stem cell availability to rejuvenate the gland? I am on 1 grain of Nature Throid but my Naturopath seems to think it is fine to only test my total T4 and T3, which are low bottom range. My hands are still cold, I have low energy, and I do not feel adequate. I have always done my own testing even before having this Naturopath, because no doctors would test what I found to be beneficial through my own research. Conventional Medicine had me on Levothyroxine early on which was not helpful, and through my own testing I found my RT3 to always be extremely elevated. My Naturopath seems to think that this is because my albumin is elevated, with low globulin, and elevated creatinine. Basically having left kidney atrophy she thinks I just have to manage it through lifestyle forever, and deal with the problem. It is frustrating because I have order the 4 Iron labs, ZRT 4 point saliva testing, both Vitamin D tests, B12, and many other beneficial labs, yet she only looks at lab ranges and no optimality. I have been reading non stop any and all book recommendations I get from people like you or Paul Chek for instance. I am currently reading Dr. Jensons guide to Better Bowel Care. And it is awesome information, I cannot wait to run my organic acids test. I am just sick of every doctor I have not being open or willing to constant learning themselves. Or telling me they wont run this or that, or that you need to come in and pay $100 to get your refill of Nature throid and talk about how its going. Finding the ability to test these root causal problems through functional medicine lab testing will I believe change my life. I think about my health non stop, and have since I was young. Cannot wait to find more out about my health, and I am thankful for all that I have been forced to learn over the years, as I believe it will shape my future. I have learnt so much from your podcast- thank you for your commitment with sharing your knowledge. I have recently completed your IHP lvl 1 & 2 - the course was amazing! I’m using the information to heal those in my community and also my family with amazing results.I have a child on the Autism spectrum we have done so much work with her over the past 3 years to heal her sensitive system and can proudly say she’s developing along side her peers and thriving in life- but still has some healing to do. We have done your parasite protocol which was amazing and saw one of her last autisms - persistent stimming, disappear, almost over night. We then started your Children’s CBO protocol and almost from the beginning saw massive flare ups of autistic behaviours such as OCD, fears, stimming, rigidity, poor communication, toe walking and anxiety - to the level they were 12 months ago, prior to a massive amount of healing. While I know that this type of flair up can happen when healing, and I’m ok with it and will soldier on with the protocol, I’d really like to know why this happens. I love to hear what you have to say on this matter as I plan on specialising in this area of health - helping families and children with Autism. It gets worse after a meal, sometimes when stress or after exercise but mainly after meals. I just can't pin point what it is. Iv tried everything. When im relaxed or only eating shakes for eg i dont get any feeling. For eg when i did the 21 day detox the 2 fasting days i had no upper sensation but the day i introduced food it came back and was worse like a stitch in the upper left stomach. Inflammation is at the core of most chronic illnesses including hypothyroidism and Hashimoto’s disease. But how exactly does inflammation cause hypothyroidism? Nonthyroidal illness syndrome (NTIS) is the state in which inflammation causes negative changes to thyroid hormone including low T3 and increased reverse T3 (rT3). T3 is the most active form of thyroid hormone and rT3 actually blocks T3 receptors so inflammation can really knock out thyroid function. TSH levels however stay relatively “normal” in NTIS thus leaving many patients with the symptoms of hypothyroidism but no diagnosis since no one is checking their T3 or rT3 levels. In this article I’ll be referencing a paper entitled, “IL-6 promotes nonthyroidal illness syndrome by blocking thyroxine activation while promoting thyroid hormone inactivation in human cells.” T4 is normally converted into the more active T3 under normal conditions but when you’re chronically inflammed, the enzymes that perform this conversion may not be functioning properly. The “4” in T4 indicates that it has 4 iodine molecules and the “3” in T3 indicates that it has 3 iodine molecules. There are three enzymes called deiodinases involved in this process: D1 converts T4 into T3 by removal of one iodine from T4 to make T3. D2 does the same thing as D1. D3 inactivates both T4 and T3 but also increases rT3. 80% of the T3 in your body comes from D1 and D2 converting T4 into T3. Inhibition of D1 and D2 due to inflammation is the most common cause of low T3 but D3 is also a contributing factor to low T3 and increased rT3. What is inflammation? Inflammation is basically an overactive immune response that is acute or chronic. This overactive immune response can result in tissue damage if it is localized to a specific area in the body or it can be present throughout the entire body resulting in various symptoms such as fatigue, brain fog, joint pain, digestive problems and much more. There really aren’t any symptoms I could list that aren’t related to inflammation. If you have elevated C-reactive protein levels on your blood tests then this can indicate inflammation. I test all of my patients for this marker to see their level of inflammation. Cytokines are chemical messengers in your body that communicate with your immune system in response to any kind of stress. Interleukin 6, or IL-6, is a well-studied cytokine that is elevated when there is inflammation and it has been specifically studied in relation to thyroid hormone. Increased levels of IL-6 have been shown to lower T3 levels and increase reverse T3 levels. What causes inflammation? Many things can cause inflammation and some of these tend to be overlooked. Here is a list to think about if you have hypothyroidism or Hashimoto’s disease: Stress-both physical and emotional Infections of all types ie. viruses, bacteria, parasites, yeast Food sensitivities ie. gluten, dairy etc. Loneliness Social isolation Lack of meaningful purpose in life Lack of sleep Lack of exercise or excessive exercise Relationship conflicts Gut dysbiosis Dental infections Autoimmunity Lack of physical affection Environmental toxins Some of the above may surprise you but all of these need to be seriously confronted if they are present. What did the above study find on inflammation and hypothyroidism? When the authors subjected cells to the inflammatory cytokine IL-6, both D1 and D2 activity decreased thus resulting in low T3 and increased rT3 levels. Additionally, D3 activity increased thus lowering T3 levels even more and increasing rT3. It was clear that inflammation significantly disrupts thyroid hormone in the peripheral tissues of the body. This means that your thyroid may be producing healthy levels of thyroid hormone but once it leaves the thyroid and is supposed to be activated in other organs such as the gut, liver, kidneys etc., it may not do so optimally.

Getting this lab panel provides you with information to help you determine how your thyroid is functioning. If you are feeling poorly or symptomatic then these patterns may help explain your hypothyroid symptoms. Legend: * means that you should pay the most attention to this thyroid lab test. Check out today's questions:  Jordan: Hi Dr Cabral. I am a collegiate athlete and so typically I train for 3 hours a day (2 hrs of sport, 1 hr of weight room stuff). How can I make sure I am getting enough calories in? I'd even like to build muscle if possible. I know you aren't a fan of eggs and bacon first thing in the morning but taking a protein shake and a bowl of oatmeal only fills me up for around two hours. Should I start taking more protein shakes (although i get gas and bloating with whey)? I know there is some healthy plant based mass gainer shakes that use plant protein, gluten free oats, and organic maltodextrin, maybe i should try those? anyways, I would appreciate any advice. I recently became a nurse practitioner and have started with IFM certification program, yay!! I have a question regarding my own labs. My TSH is 3.1 FT3 is 2.2 FT4 1.1 RT3 is 10 TPO 17 ANTI-TG 928 I restested 4 weeks later and numbers were pretty much the same. I realize my thyroid is sluggish. I however have NO signs of hypothyroid except a little bit of stubborn lower body fat. I am 30 years old, have no medical history, I'm very active and eat a paleo diet 90% of the time. I somewhat acknowledge that I may feel better with a little thyroid hormone but Im not ready to start that at this time. My REAL question is ..... I don't understand why my anti-tg is so elevated but TPO is not.... I am working to lower this to the best of my current knowledge. Can you explain why we seem to focus more on TPO and I don't hear as much about anti-tg and wouldn't both be elevated? Anonymous: Hello Dr Cabral, I have been listening to this podcast for some time now and I appreciate all the information you share. Thank you! I have been suffering with pain, throbbing pain at the rectum that can last for hours, making it difficult to sit; following bowel movements for approximately 5 years now. A bit of background is 5 years ago I had an procedure to correct a anal fistula and I have never been the same since both physically or mentally. The procedure was excruciating and tortuous. I have since seen many doctors to seek help for my consistent pain. I’ve had many treatments to help with pain including Botox, steroid creams, estrogen creams, hemerriod creams, opioids and I have also eliminated certain foods that I noticed caused more pain such as almonds - I’m not sure why consumption of almonds affect me in this way, they don’t change my bowl motion just the pain. I don’t suffer constipation and sometimes have lose stool but the consistency of my stool has no correlation to the pain, it is the passing motion that results in the pain. I am able to manage life since removing almonds which is amazing considering all the other interventions I’ve had that didn’t give me any results, but I would love to be pain free. I will not have surgery again as I was in such a bad place after the initial surgery where I saw no hope for my life due to the immense pain I was in. Do you have any idea what is wrong with me and what I might be able to try? I’ve tried candida cleanses, gut protocols, elimination diets, I do FM detoxes regularly, eat a clean diet, had a stool test that showed strep and low ecoli and low diversity of probiotics and I’m doing protocol to address this now. I take milk thistle and a B complex as well as eat a very clean, whole food vegan diet. After doing research, i am doing everything right, but I am still experiencing terrible cystic acne. What can I do to help balance my hormones naturally? Thank you. Lisa: Hi Dr. Cabral and team! I'm really struggling with inflammation. I have done the 21 day detox, CBO, gut rebuilding protocols and my tongue still has red painful spots and a weird pattern that is white. My OAT did show some yeast overgrowth, but for some reason my body is struggling to rid it. I got desperate and even tried months of a strong antifungal pharmaceutical with the keto diet. My tongue did not improve. My biomedical doctor thinks I need to give LDN a try to help decrease inflammation. I've done multiple labs, including your adrenal/ thyroid, OAT, IgG food sensitivity, stool, HTMA, and I also do a lab called NutEval. Shockingly my labs look good besides a little Estrogen dominance (taking Estrogen Dominance), little yeast overgrowth, aluminium in my HTMA, and low omega 3s. I do notice improvements with taking 2000mg of omega 3s and CBD oil twice a day, so I feel like that is sign I need to focus on anti inflammatories. Today is episode #24 and it's all about bad genes and how they impact your thyroid function. There are certain genetic changes, known as single nucleotide polymorphisms or SNP's for short, which can slightly impact certain enzymes in your body. These genes are very important when it comes to your thyroid because they can impact a set of enzymes known as the deiodinases. These deiodinases are responsible for activating or inactivating thyroid hormone at the cellular level. And they can be impacted by factors such as your genes, medical conditions, medications, and so on. There are 3 major deiodinase enzymes: - D1 - You can consider this an activating enzyme and it is responsible for 30% of T3 in the body. - D2 - You can consider this an activating enzyme and it is responsible for 70% of T3 in the body. - D3 - You can consider this an inactivating enzyme which takes T4 and turns it into reverse T3 or rT3. Certain SNP's impact the function of these enzymes which create conditions in which you may not be able to convert T4 to T3 very well. DIO2 SNP's and DIO2 SNP's can both impact how your body tolerates certain medications. You can test for the presence of these enzymes which can help guide your treatment and help you understand which thyroid medication you should be on. Hey guys! He has personally been through the wringer with the traditional medical system as he shared with me in  But that set him on a quest to discover more about his own health by gathering and analyzing the medical research staying up-to-date on everything he could get his hands on. There are a lot of health myths that unfortunately pervade in the realm of health and Chris regularly addresses these on his highly popular  and wildly-popular  podcast. One area of specific interest to him is the subject of thyroid. Around 20 million Americans deal with some form of thyroid disease and yet it is left undiagnosed due to a variety of reasons. Yet there is a lot of confusion about thyroid conditions that we will hopefully have Chris help bring clarity to in this episode! LOW-CARB, GLUTEN-FREE CONDIMENTS MADE WITH STEVIVANOTICE OF DISCLOSURE:  Here are some of the questions we addressed in this podcast: Are there any dietary changes that will "treat" hypothyroid? There is a meme that has spread quickly to Paleo sites that low carb causes low T3, and is a cause of hypothyroidism. How can this be? My doc who treats hypothyroidism and insulin resistance had me do low carb, and it worked. How can low carb be both the cause and part of the solution? Someone has this wrong. What is going on? CRIS ASKS:I am a 53 year old female who had a Total Thyroidectomy four years ago due to thyroid cancer. My doctor keeps me just a smidgen "hyper" with Synthroid replacement medication to help the cancer not grow back. I have lost 50 pounds. In one year on a low carb diet, I feel great. But I have been stalled for two months now. I can't seem to lose any more weight. I need to lose 50 more. I know I need thyroid replacement meds but could this be the reason I can't seem to lose anymore? JENNIFER ASKS:Which is better--natural or synthetic thyroid hormone (like Synthroid) or a combination? PAUL ASKS:When I went zero carb Paleo, my T3 dropped out of the range and I needed to take antibiotics for a mouth abscess. I resumed carbs and gained back the weight I lost. I am now trying to drive down the carbs again. What can I do to ensure that my T3 remains ok. I am hypothyroid (take 100T4 + 20T3) and I was assessed a few years back by a former family doctor as having low adrenal function. Are adrenal and thyroid metabolism related and can a low adrenal state lead to a low T3? REGINA ASKS:What is your opinion of Jack Kruse's Leptin Reset Rx and Cold Thermogenesis on the thyroid? LAUREN ASKS:I underwent a total thyroidectomy in January of 2006 following a thyroid cancer diagnosis and will be on replacement T4 (Synthroid) for the rest of my life. Are there any modifications that those of us without a thyroid need to keep in mind when following a Paleo diet? JOSH ASKS:My wife had about 90% of her thyroid removed several years ago after undergoing a thyroid storm and goiter. She's on levothyroxin and we've switched over to a Paleo diet about two years ago, and she's pretty good with adherence, especially when it comes to avoiding wheat and grains. One of my real concerns is with her long term bone health due to the thyroid removal. She currently takes anywhere from 2000mg to 3500 mg of calcium each day to combat calcium loss from her bones. She takes 5000 IU of D3 daily, as well as K2 to help with proper absorption. Her blood calcium levels are usually high, but if she doesn't take the oral supplements she gets pain in her hands, wrists, and other joints. What supplements would you recommend for proper calcium absorption? What are some good blood markers you'd like to to indicate proper function? SHARON ASKS:Can you discuss the role of antibodies (AB), both blocking and stimulating, on thyroid health? When the typical thyroid labs appear "normal", could there be an AB issue occurring, causing the autoimmune response? DONNA ASKS:Since giving up soy milk and grains (including daily wheat germ) when I started on a low-carb regimen about a year ago, my arthritis symptoms have virtually disappeared, and my thyroid started working better (I was able to lower my dosage of levothyroxine slightly). During the same period, my osteopenia has progressed to osteoporosis. The latter may just be age, but what I want to know is, could the same root cause underlie co-existing hypothyroidism, osteoarthritis and osteoporosis? PAUL ASKS:My TSH increased from 3.86 a year ago to 5.23 last month. I have no hypothyroid symptoms but my doctor want to put me on levothyroxine. Does this make sense? Other relevant blood valuesT4 free 1.09T4 total 5.9T3 uptake 38TPO antibody: 19Total cholesterol increased from 197 to 215 over last year.T total 417T free 45Free T% - 1.1%SHBG 79.6 LAURA ASKS:I am a 42-year-old female who has been eating Primal/Paleo (low to moderate carb) for over one year now (after being a vegetarian and vegan for almost my ENTIRE life). I had my one and only child almost three years ago and lost all my baby weight (and then some) by changing my eating habits. I looked so much better after changing my lifestyle, but still had low thyroid symptoms (FREEZING cold, tired, digestion issues, etc) AND it was discovered I had near zero sex hormone production (estrogen, progesterone, and testosterone). Also, I had BEYOND low Free T3 and Free T4 (TSH was all over the place, but is now quite low). I was diagnosed with hypothyroid due to a pituitary disorder. I have been put on Estradiol, Prometrium, and a small amount of topical Androgel (testosterone). I have also been taking Armour. I have gained 30 pounds thus far. I am miserable. My questions are: Do you believe in supplementing with sex hormones or should the thyroid medication be sufficient? Would daily intermittent fasting be contraindicated for someone with issues like mine (or does this stress the adrenals too much)? I want to be HEALTHY. But I cannot lie, I am so OVER being overweight. I would love to find a healthy balance. STEVE ASKS:This question is about real thyroid health vs "health on paper" (good test results). I've been treated for hypothyroid, chronic fatigue, and insulin resistance since 2007, with good results. I see a top doctor that deals with all three and they use the so-called bio-identical T3, hormones like testosterone or progesterone cream, and a lot of supplements. I lost 120 pounds quickly and look much different. My blood work went from train wreck to awesome. Weight is normal enough and I feel fine, even though I have low calorie requirements now (about 1,400 a day, and I'm male 51, 5"8, 192 pounds) My question is how do I know if I get to a point where I'm spending a lot of money on meds and supplements and maybe need to back off and let the body take over? My doc wants to keep doing what has worked. Am I broke forever, or should I think about cutting back on supplements and hormones and let the body handle more? TIRIL FROM NORWAY ASKS:I am wondering if thyroid problems is the cause of hair thinning in a woman? Are there any other causes that should be investigated as well? And what can you do as a first intervention? JAN ASKS:What thyroid tests do you recommend when weight loss has stalled? (In my case TSH is elevated but the HMO will not test T3 or rT3--is it worth paying privately for those tests to have a more complete picture of what might be going on?) PAULA ASKS:Do you have any insight on what might cause thyroid nodules? (I have several, and biopsies show them to be non-cancerous at this time. One doctor recommends surgery; another says they are nothing to worry about)
