A quick heads up on a couple of things before we dive in: first, both of these interviews were recorded at World Agri-Tech in San Francisco and they were other conversations happening in the media room for part of the time, so I hope you'll forgive a little bit of background noise. Second, similar to Amie Thesingh's episode last month, I originally recorded these interviews to be spotlight episodes featuring the work Headstorm does. Just like in Amie's case I thought this story warranted a full-length episode, so we will focus on the work Microsoft and Bayer are doing together, but I will also include the role Headstorm is playing in all of this as well. Just a heads up on that. You heard from both Claudia and Mark as part of our Generative AI episode which was #409, but the focus today is on this initiative started by Microsoft with their Azure Data Manager for Agriculture, or ADMA. We'll also explore the collaboration with Bayer Cropscience, in particular they're Ag Powered Services Platform that brings together agronomic data for a variety of applications. Because sometimes this data stuff can get a little abstract, I think it's probably helpful to level-set with some basics. Starting with cloud services. I think most of us intuitively know what a massive leap forward cloud computing has been for technology in general. From software applications to file storage to other sources of data - cloud computing is how we are able to power digitization. The cloud is not new obviously. But what has become clear is that just giving people access to the cloud isn't enough to really tap into the power of all of this information - it's just a place to store it. Moving from stored data to actionable data is a very very heavy lift - especially in an industry like agriculture. So, Microsoft started creating industry-specific data management platforms. They describe this as “industry-specific data connectors and capabilities to connect farm data from disparate sources.” They've been successful with similar efforts in other industries like retail, finance and healthcare, and last year they unveiled Azure Data Manager for Agriculture, a continuation of the work they were doing with FarmBeats, which you might remember from episode 266 with Microsoft's Ranveer Chandra. So when it comes to making data more valuable, the cloud is a massive step forward, now we have another massive step forward in ADMA, and we're also going to talk about what could be yet another massive step forward Bayer's Ag Powered Services. Bayer is providing additional data infrastructure that they first developed to use internally, and now are offering to other companies that rely on agronomic data to power their various digital applications. The ultimate goal here though is that data no longer becomes the bottleneck to progress. If a buyer, for example, wants to pay a farmer more for certain agronomic practices, all they need is...

It's not sexy but like AI, it's going to affect your job – and your company. Another salvo in the fast approaching privacy regime set for tabling in parliament in August was fired last week by the ACCC around how personal information is collected and used by data firms – Experian, Nielsen, Publicis-owned Epsilon and Woolworths-owned Quantium were among those flagged by the competition regulator last week in its eighth interim report as part of the multi year Digital Platforms Inquiry. And to be blunt, any professional working in ecom, marketing, customer experience, digital advertising and data and analytics is going to have a rude shock for what they can do now versus what is likely in a year or perhaps a bit longer. But UNSW Business School's Professor of Practice, Peter Leonard, says last week's release by the ACCC of its Data Products and Services interim report makes “every firm in this economy a data firm.” And in the short-term, that's not good news for most companies because their data readiness and maturity is not matched by the “fundamental change” which will force everyone to “rethink their understanding” of what even defines personal information” according to ADMA's Director of Legal and Advocacy, Sarla Fernando. Leonard and Fernando are joined by Capital Brief's Legal and Regulatory Affairs Correspondent, Laurel Henning and Civic Data's founder, Chris Brinkworth. Ravi Prasad is a former ad man turned social entrepreneur. A strategist, who in a career spanning over 20 years, worked for agencies including, EURO RSCG, Leo Burnett, John Singleton Advertising, Ogilvy & Mather, Sapient Nitro and Clemenger BBDO. Over the years his work has won, or been a finalist, in awards, from W3 in New York to the IIB Awards in London, and from ADMA to AIMIA in Australia.  In 2013 Ravi shifted the focus of his life and work to pursue his interest in social justice and civil society, and founded Parliament on King. Parliament on King addresses the barriers to social, cultural and economic participation faced by asylum seekers and refugees.  It's also a social enterprise catering business, that offers training, work experience and paid employment to asylum seekers and refugees – all funded by the proceeds of its commercial catering operations. Summary   Hello and welcome to episode #19!   This is Ralph Sanchez and today I'll be talking the outcomes of two recent studies that investigated the potential use of Viagra and Cialis in the risk reduction for late-onset Alzheimer's disease (LOAD).   I was in part inspired to provide an overview on these two recent studies as they are cautionary tales on how many studies do not include the interrelated factors that are essential in arriving to an integrated assessment and analysis that serves their very premise— which is, does this or that work in a potential solution to something else?   Does Viagra or Cialis offer any proposed solution to the risk for LOAD and dementia?   Well today, I'll be adding a great deal of information—the missing pieces to the puzzle as it were—with regard the pathways by which Viagra and Cialis may or may not work, and many other complimentary or natural alternatives that play a similar role in maintaining and optimizing a healthy cardiovascular and cerebrovascular system.   First, let me provide a little insight as to the molecular pathways in which drugs like Viagra and Cialis function, and why they may be considered as repurposed drug candidates for the treatment or in the risk reduction for LOAD.   Viagra (sildenafil) and Cialis (tadalafil) are Phosphodiesterase-5 inhibitors (PDE5is) which fall into a class of drugs that are normally prescribed to men to treat erectile dysfunction (ED), benign prostatic hyperplasia (BPH) and lower urinary tract symptoms (LUTS).   PDE5is can have a profound effect on cardiovascular health and PDE5is mediate their benefits by inhibiting the breakdown of a molecule, cyclic GMP (cyclic 3′,5′ guanosine monophosphate).   Cyclic GMP (cGMP) is an intracellular and second messenger molecule that modulates many downstream pathways, including significant effects in vasorelaxation—the ability of your blood vessels to dilate and expand as needed.   The vascular effect that is enabled by PDE5i-induced vasodilation is a pivotal pathway in vascular homeostasis and a healthy heart-brain axis.   And that vasodilation effect is how PDE5is improve and treat ED.   There is a lot more to that vasodilation benefit mediated by PDE5i therapy  which I'll get to here soon.   So on to a brief description of the two recent studies on Viagra and Cialis, and so much more that was not included in those studies that will provide a crucial insight into how you can improve your vascular health and reduce your risk for LOAD.   NIH Studies First, a recent (2021)National Institutes of Health (NIH) funded study reported a risk reduction benefit of 69% for Alzheimer's disease (AD) in users of Viagra (Sildenafil).   The analysis simply compared Viagra users to those who did not take it, and the study was focused on a screen of drugs that could potentially be repurposed in the risk reduction for AD in aging individuals.   In a similar and second NIH funded study published this year (2022) titled— Drug Repurposing for Effective Alzheimer's Medicines—(DREAM), the NIH analyzed data from Medicare beneficiaries that were treated with Viagra and Cialis.   The NIH team compared people with pulmonary arterial hypertension (PAH) treated with Viagra and Cialis over those with PAH on another class of drugs (endothelin receptor antagonists) used to treat pulmonary hypertension.   And note that PAH is a term that refers to high blood pressure in the blood vessels leading from the heart to the lungs   Yes, PDE5is are also prescribed to patients to reduce blood pressure in PAH, and off-label use of PDE5 inhibitors (PDE5is) is used to treat cardiovascular diseases, Raynaud's disease and women with female sexual arousal disorder.   The average age of patients included in the DREAM study “was 74 years (range 65–96 years), and 69% were women.”   Notably, studies have shown that in women, "PDE5 inhibitor efficacy is estrogen dependent in female heart disease."   The DREAM PDE5i study rationale for focusing on patients with PAH and treated with PDE5is, Viagra or Cialis, for the new study analysis over those individuals on another class of antihypertensive drugs was attributed to the odds that the two groups were  "more likely to have people with similar characteristics".   In comparing those two pulmonary hypertension groups in the DREAM study the research team concluded that they "observed no evidence for a reduced risk of Alzheimer's disease and related dementia with phosphodiesterase-5 inhibitors"—Viagra or Cialis.   Bottom line, two differences in the design and outcomes as the Viagra study was favorable while the DREAM study was not. Fair enough.   However, there is much more to this story that was not included in the two studies, which includes an important benefit for heart and brain health that is only in part mediated by PDE5i therapy.   And we begin with nitric oxide— a vital molecule produced in your body and brain that not only impacts many potential benefits to your health, it can also be a component in deleterious oxidative stress reactions that are very damaging to your body and brain.   Nitric Oxide Nitric oxide (NO) was first discovered in 1772 but it was not until 1987 that it was identified as an important signaling molecule that played a vital role in endothelium-dependent vasodilation in mammals.   A few years later, in 1992, the journal Science nominated nitric oxide as the “Molecule of the Year” due to its role as a fundamental signaling agent in cardiovascular health and function.   In the body and brain, NO can be synthesized by two distinct pathways.   First, endogenous nitric oxide (NO) can be synthesized from the amino acid L. arginine (L-arginine-NO-synthase pathway) which is the initial upstream driver of cGMP activation in vasodilation.   Remember that we started this overview by emphasizing the role of PDEis in blocking the degradation of cGMP.   However, NO signaling is where cGMP activation begins.   Additionally, another amino acid—L. citrulline—is metabolized from arginine and can be utilized in regeneration of arginine.   Both amino acids are at the center of many studies which showcases their metabolism in vasodilation pathways.   Nitric oxide may be also be derived from the intake and metabolism of foods rich in nitrate (nitrate–nitrite–nitric oxide pathway).   Vegetables such as beets, celery, arugula and spinach, and fruits (e.g., strawberries) supply approximately 80%—85% of dietary nitrates in individuals that consume such foods regularly.   Indeed, those foods are not only a terrific source of nitrate, they also are rich in many types of polyphenols which are key nutrients in protecting against oxidative stress pathways associated with nitric oxide metabolism.   Many of you listening in or reading the transcript have likely become familiar with supplements using beets, or arginine and citrulline and other synergistic ingredients that have been heavily marketed to athletes as performance enhancers.   In a clinical setting, these NO enhancing products are often used for ED and cardiovascular health support.   Drugs (organic nitrates) are also well-known NO donors (e.g., nitroglycerin, amyl nitrite).   Thus, NO can be produced from the precursors L. arginine and L. citrulline, or nitrates and nitrites that are either derived from foods, supplements and drugs.   Another important understanding in all of this is that oral and gut bacteria convert dietary nitrate (NO3) to nitrite (NO2), and in the acidic stomach nitrite is further reduced to nitric oxide.   NB, antiseptic mouthwashes inhibit nitrate to nitrite metabolism by eradicating oral bacteria, and proton pump inhibitors (PPIs) and antacids suppress stomach acid and nitrite to NO metabolism.   To recap NO is an essential signaling and vasodilatory molecule secreted by vascular endothelial cells, which stimulates the production of cGMP (NO/cGMP Pathway) via activation of the receptor for cGMP— soluble guanylate (guanylyl) cyclase (sGC).   Another important point about NO in women is that estrogen (E2) increases NO synthesis.   Thus, the estrogen-NO dynamic is vital in vascular relaxation and endothelial-dependent vasodilation in women and should be included in any risk assessment for cardiometabolic and dementia risk in perimenopause or the earliest stage of menopause.   In contrast, cardiovascular disease (CVD) risk factors such as excess belly fat, high blood pressure, insulin resistance and type 2 diabetes, glycation and chronic inflammation (inflammageing) disrupts endothelial function, promotes arterial stiffness, and blunts the synthesis of NO.   Studies have shown that ED occurs in approximatelym35% to 75% of men with type 2 diabetes, and related studies have concluded that ED predicts future cardiovascular events.   Additionally, CVD risk factors upregulate the formation of an arginine metabolite—ADMA (asymmetric dimethylarginine)—that inhibits vascular NO production (eNOS uncoupling).   ADMA impairs vascular endothelial function and increases vascular oxidative stress (NO-ONOO cycle), and elevated ADMA has been linked to CVD in many studies. See image below.   A useful assessment in analyzing the underlying factors associated with cardiometabolic disease such as atherosclerosis is the serum arginine/ADMA ratio which provides information on arginine bioavailability for production of NO.   So, to summarize, the integrity of NO-sGC-cGMP Pathway is critical to signaling and vasodilation mechanisms that are essential to blood flow and vascular/endothelial homeostasis.   Healthy endothelial function and NO levels is critical in the normal function of many vital organ systems including the cardiovascular and cerebrovascular system (neurovascular system), and the respiratory and renal systems.   Indeed, a healthy endothelium is a fundamental cornerstone to living younger, longer.   For a more thorough overview of the neurovascular system, please listen in to episode #16 titled: “Brain Detoxification-Part 1-The Role of the Blood Brain Barrier and The Glymphatic System” here on this channel.   NO-PDE5i Brain Benefits But what about the role of PDE5is in all of this?   Is there a role for PDE5i therapy in preventing cognitive decline?   Would the PDE5i—sildenafil, or other PDE5 inhibitors, impart the same benefits on brain and cognitive health as healthy levels of NO does?   Previous studies have shown that low dose sildenafil activates signaling pathways which suppresses the processing and generation of beta-amyloid and tau protein aggregates.   Additionally, PDE5 inhibition mediated benefits include: induces cellular antioxidant levels, blunts neuroinflammation, and stimulates the production of new mitochondria (mitochondrial biogenesis) Now, is there a world where the combined therapy of NO and PDE5i therapy exists?   Only a handful of recent studies have shown that supplemental citrulline or arginine therapy in combination with a PDE5 inhibitors could be a synergistic and therapeutic alternative to PDE5i monotherapy for severe ED and pulmonary hypertension.   Overall, combination therapy was superior to monotherapies.   No such combination studies that investigated PDE5 inhibitors with citrulline or arginine therapy in the risk for late-onset Alzheimer's disease (LOAD) have been undertaken.   Bottom line, given the lack of research that includes the multiple pathways of NO metabolism and its role in endothelial function, the investigation into Viagra or any other PDE5i in the risk reduction for LOAD is grossly incomplete.   And that answers the unfinished Viagra and Cialis story as a viable treatment for cognitive impairment or Alzheimer's as the NIH studies on Viagra and Cialis just did not go deep enough into interrelated mechanisms that intersect with the PDE5 enzyme.   However, key nutrients, nutraceuticals and herbs are well-known NO precursors and PDE5 inhibitors, and they also protect against the upregulation of NO and the pro-inflammatory pathways and oxidative stress cascades associated with excess NO production.   Without a doubt, dietary, nutrient and botanical/herbal extracts are vital interventions in an optimal heart-brain health protocol.   More on that below after this brief overview on the glycocalyx.   The Glycocalyx (preview) I must add that any overview on NO in vascular and endothelial health is incomplete without an overview on the glycocalyx—a gel-like thin protective layer covering present on endothelial cells which maintains the endothelial barrier.   In fact, almost every cell in the human body, including bacteria, are covered by a glycocalyx layer.   And yes, the role of a glycocalyx layer in bacteria is another story.   The degradation of the endothelial glycocalyx layer in aging and cardiovascular disease significantly reduces endothelial cell production of NO, and collectively these interactions are a major and underlying factor in cardiovascular AND neurovascular disease.   I have been closely following the emerging research with regard to the glycocalyx over the past 5 years, and the evidence that you cannot have a healthy-heart-brain axis without a vibrant endothelial glycocalyx is compelling.   Case in point, the recent studies that have demonstrated the importance of the glycocalyx in vascular/endothelial health have established a new and critical insight into the treatment and potential reversal of atherosclerosis.   Nutrition-Diet (preview) A host of nutrients, nutraceuticals and herbs are well-known NO precursors and PDE5 inhibitors.   First, apart from arginine and citrulline, vitamin C, E and D are core nutrients in NO-mediated endothelium-dependent relaxation.   Recently, vitamin K2-MK-7 studies have also been linked to enhanced NO-dependent endothelial function.   Glutathione has been shown to be an essential and protective antioxidant in modulating NO reactivity and protecting against the damaging NO-ONOO cycle.   Glutathione and glutathione-based enzymes detoxifies peroxynitrite (ONOO)—a reactive nitrogen species, and improves nitric oxide bioavailability and endothelial function.   And, tetrahydrobiopterin (BH4) is a key enzymatic cofactor required for the synthesis of several neurochemicals—serotonin, dopamine and NO.   Additionally, polyphenols and in particular flavonoids, are the most the most frequently reviewed and studied phenolic NO precursors and PDE5 inhibitors, and they support the integrity of the glycocalyx structure and function.   So many nutrients  to make a case for including the phytoestrogenic benefits of many flavanoids, but here is an abbreviated list of the most cited plant/herbal derivatives and nutrients: Resveratrol Quercetin Catechins, epicatechin (e.g.,green tea) Pine bark (Pycnogenol®) and grape seed extracts (GPSE) Isoflavones (e.g. genistein) Hawthorne (Crataegus species) Icariin (Epimedium brevicornum) Aged garlic extracts Pomegranate extracts Gingko biloba Black ginger (kaempferia parviflora) Xanthones (Anaxagorea luzonensis) And, all the nitrate/flavanoid-rich foods including beets, spinach/leafy greens, and celery, pomegranate, berries, cherries, citrus, garlic, dark chocolate and many others.   Of course, a low-carbohydrate Mediterranean diet or Mind Diet lifestyle is rich in polyphenols and other heart and brain health nutrients.   In addition, exercise training and caloric restriction promotes NO activity and endothelium-dependent vasodilation through activation of eNOS.   Additional points covered in this episode's audio file…   NO Synthesis (preview) Now, I'll briefly describe how NO is synthesized as there are principal pathways that illustrate key and elemental features of NO metabolism—good and bad.   The NO-cGMP pathway is in part regulated by the activity of a family of enzymes—nitric oxide synthases that regulate nitric oxide (NO) synthesis.   There are three NOS enzyme synthases—neuronal (nNOS), endothelial (eNOS), and inducible (iNOS).   iNOS driven NO production is associated with immune system responses to various stimuli such as infections, and NO can either function as a regulator of such responses, or excessive production of NO by iNOS can upregulate those pathways in a destructive manner.   Nitric Oxide-S-nitrosylation (preview) I want to add another important feature of NO metabolism with regard to the risk for LOAD.   And that is the physiological and pathophysiological role of another NO reaction that takes place in the body and brain which is termed S-nitrosation, or S-nitrosylation.   I was not planning on adding this to this episode, but a recent research study just published reported that a S-nitrosylation pathway was a risk factor in Alzheimer's disease in women.   S- nitrosylation is the bonding of NO to sulfur compounds on amino acids such as cysteine.   In a recent study finding reported on just a few days ago—December 14, 2022, the elevated S-nitrosylation modification of an immune system protein known as complement component C3 (SNO C3) was present at much higher (six-fold) levels in the brains of women who had died of Alzheimer's, compared to men who had died with the disease.   The postmortem brain research that was conducted at Scripps Research and Massachusetts Institute of Technology (MIT) also reported that declines in estrogen, which normally serves as a neuroprotective hormone, was likely a strong factor in the generation of the SNO-C3 form of complement C3.   Cured Meats (preview) Another key factor to weigh into this overview with regard to diet, nitrates, nitrites and NO, are cured meats.   In fact, some of you may be anticipating this section of the  overview by now as there is considerable concern and media dissemination of the potentially deleterious role of nitrates and nitrites used in various types of processed meat products such as bacon, sausages and other “deli meats”.   In fact, many studies have explored the risk of nitrate and nitrite added to various foods as their metabolism under certain conditions can potentially convert into a toxic nitrosamines (N-nitroso compounds)   That's it for this summary and as always, thank you for listening in or taking the time to We've always had to embed that type of approach whilst doing our other normal work.  So I think the big tip is to really get in and understand what are the challenges and drivers, the positive and negative consequences for people doing their jobs in the areas you're focusing on? Wow. So you know, is there a risk of embarrassment? Where are the real pain points for them about doing their job? And, with that is looking at what their interaction is with their internal stakeholders or internal customers. Do they understand what their internal customers really want from them? And are they trying their hardest to deliver that? And a lot of that really, you know, involves focusing specifically on what those people do as part of their job. And before you even go about improving it, you have to really have, you know, some aspect of understanding with them about what their job actually is. Yeah, that's neat. So in a way, you're saying, mate, really hone in on that Gemba piece. You might know those discipline areas you want to bring in or that you've got your kit bag to bring in. But go to Gemba. Understand and then go back to the kit bag for what do we need to become more disciplined with to help them help themselves then? In my senior year of high school, I came to the US as a cultural exchange student through a Rotary Club. As a part of this one year exchange program, I stayed with a family in Arizona and attended the local high school. Because I was an exchange student, I was offered a place in the Student Council at my school, a position that other students had to be elected for. But I declined the offer and chose to take a Physics class instead. I hid behind the excuse that I needed the science class credits for college (after three years of high school in India, I probably had enough science credits for a lifetime). But the truth was that I was scared that I would make a fool of myself. So instead of exploring this unique opportunity and living the American high school experience to its fullest, I chose to hide behind books. It was still a fantastic year full of many new experiences - I ate a taco for the first time, I sold Girl Scout cookies, and I spent six weeks traveling around the US by train. But I don’t have many memories from the school. And I still wish I had found the courage to say “yes”. I would have made many more friends and memories. In this episode, Catherine reminisces about an experience from the earlier part of her career when she said no to an opportunity because of self doubt. This experience taught her an important lesson, and even today motivates her to override her self-doubt and say yes to opportunities.Catherine Madden is a co-founder of UXDX, a global conference and community that helps tech team's shift from projects to product teams. Im Tal Siddim stellten sie sich zum Kampf auf gegen Kedor-Laomer, den Großkönig von Elam. Auf dessen Seite waren die Könige Tidal von Gojim, Amrafel von Schinar und Arjoch von Ellasar. So standen also die vier Großkönige den fünf Stadtkönigen gegenüber. Das Tal Siddim aber war voller Asphaltgruben. Die Könige von Sodom und Gomorra ergriffen die Flucht und fielen dort hinein. Die übrigen Stadtkönige flüchteten ins Gebirge. Da plünderten die Großkönigeden gesamten Besitz von Sodom und Gomorra. Sie raubten auch alle Ernteerträge. Dann zogen sie davon. Sie nahmen Abrams Neffen Lot mit und seinen ganzen Besitz. Lot wohnte damals in Sodom. Einer von denen, die fliehen konnten, kam zu dem Hebräer Abram. Er berichtete ihm alles, was geschehen war. Abram wohnte damals bei den Eichen von Mamre. Mamre hieß auch der Bruder von Eschkol und Aner, ein Amoriter. Die drei waren mit Abram verbündet. So erfuhr Abram, dass man seinen Verwandten gefangen genommen hatte. Da rief er seine kampferprobten Leute zusammen. Es waren 318 Männer, die alle aus seinem Lager stammten. Mit ihnen verfolgte er die Großkönige bis Dan. Dort teilte er seine Truppe auf. Abraham und seine Leute überfielen die Großkönige. Mitten in der Nacht schlugen sie sie in die Flucht. Dann verfolgte er sie bis Hoba, das nördlich von Damaskus liegt. Abram nahm ihnen alles wieder ab, auch das, was Lot gehörte. Er befreite seinen Verwandten Lot, die Frauen und die übrigen Gefangenen. Abraham begegnet Melchisedek Nach seinem Sieg über Kedor-Laomer und die Großkönige, die mit diesem verbündet waren, kehrte Abram zurück. Da zog ihm der König von Sodom entgegen ins Tal Schawe, das jetzt Königstal heißt. Auch Melchisedek, der König von Salem, kam dorthin und brachte Brot und Wein. Er war Priester des Höchsten Gottes. Er segnete Abram und sagte: »Gesegnet sei Abram vom Höchsten Gott, der Himmel und Erde erschaffen hat! Gepriesen sei der Höchste Gott, der deine Feinde in deine Hand gegeben hat!« Da gab Abram dem Melchisedek den zehnten Teil der ganzen Beute. Der König von Sodom sagte zu Abram: »Gib mir meine Leute, die Beute kannst du behalten!« Da antwortete Abram dem König von Sodom:» Ich erhebe meine Hand zum Herrn, dem Höchsten Gott, der Himmel und Erde erschaffen hat. Ich werde nichts behalten, was dir gehört –nicht einmal einen Faden oder Schuhriemen! Du sollst nicht behaupten können: ›Ich habe Abram reich gemacht.‹! Für mich selbst will ich nichts, abgesehen von dem, was meine Leute verzehrt haben. Aber Aner, Eschkol und Mamre sollen ihren Anteil an der Beute erhalten. Diese Männer sind mit mir gezogen. Sie sollen das bekommen, was ihnen zusteht.

El profeta Oseas trajo el mensaje de Dios al pueblo del norte durante los años previos a la cautividad asiria, que ocurriría durante el reinado del rey del norte que llevaba el mismo nombre que él, Oseas. El profeta nos muestra la misericordia incomprensible de Dios a través del ejemplo de un hombre que, a pesar de la continua infidelidad de su mujer la sigue amando y perdonando. Nos enseña que Dios tiene esta misma disposición hacia su pueblo rebelde, porque Dios ama la misericordia. Dios usó el matrimonio de Oseas para mostrar a su pueblo el amor fiel de Dios a pesar de la infidelidad de estos. Dios también usó el nombre de los hijos de Oseas para comunicar el mensaje al pueblo. Cuando Oseas y Gomer tuvieron su primer hijo, Dios le pidió: “Ponle por nombre Jezreel; porque de aquí a poco yo castigaré a la casa de Jehú por causa de la sangre de Jezreel.” Cuando su hija nació, Dios le dijo: Ponle por nombre Lo-ruhama, Esto es, No compadecida, porque no me compadeceré más de la casa de Israel, sino que los quitaré del todo. Mas de la casa de Judá tendré misericordia, y los salvaré por Jehová su Dios; y no los salvaré con arco, ni con espada, ni con batalla, ni con caballos ni jinetes.”Cuando Gomer tuvo el tercer hijo, Dios le dijo: “Ponle por nombre Lo-ammi, Esto es, No pueblo mío, porque vosotros no sois mi pueblo, ni yo seré vuestro Dios.”¿Qué podría estar diciendo Dios con estos nombres?Esto describía dónde estaba Israel en ese momento con Dios. Su primer hijo, Jezreel, recordaba al pueblo del lugar donde había habido muerte y guerra, y les advertía que en el día del juicio, ahí se llevaría a cabo la batalla final. En el nombre Lo-ruhama, la partícula Lo- indicaba el negativo. Dios les estaba indicando que si Israel continuaba yendo por el mal camino, Él no podría compadecerse de ellos. Y con el nombre Lo-Ammi, de nuevo indicaba que Dios no los podía ver como su pueblo mientras actuaban tan apartados de Él.Oseas 10:1-2 compara a Israel con una frondosa viña, que da abundante fruto, pero todo para sí misma; ¿Qué pena, verdad? Imagina una vid llena de uvas, y nadie para comerlas; que ni personas, y ni siquiera los pájaros pudieran disfrutar de la dulce fruta. Estaba cargada de fruta para sí misma, y ella misma tampoco las podría disfrutar. Nos da una clara imagen de la vanidad que el pueblo estaba viviendo.Continúa el texto denunciando lo que Israel había hecho: “conforme a la abundancia de su fruto multiplicó también los altares, conforme a la bondad de su tierra aumentaron sus ídolos. Está dividido su corazón. Ahora serán hallados culpables;”Israel había edificado altares y se había creado sus propios ídolos a los que adorar. Su corazón estaba dividido, y elegían agradarse a sí mismos. Así como de absurdo sería que una viña diera fruto para sí misma, es también vano que vivieran ellos para sí mismos. Llegarían al momento de darse cuenta que así no podrían ser vistos como el pueblo de Dios, ni este se podría compadecer de sus desgracias. Sin embargo, ya desde el principio del libro, en Oseas 2:1 Dios nos muestra su deseo de quitar ese prefijo negativo “Lo” de delante de sus nombres para poder verlos como pueblo suyo, compadecido por Dios. Dice así: “Decid a vuestros hermanos: Ammi; Esto es, pueblo mío, en lugar de Lo-Ammi, y a vuestras hermanas: Ruhama, esto es, compadecida, en lugar de Lo-Ruhama. En el 2:23 ya nos decía: “Y la sembraré para mí en la tierra, y tendré misericordia de Lo-ruhama; y diré a Lo-ammi: Tú eres pueblo mío, y él dirá: Dios mío.”Esto claramente les mostraba las buenas intenciones de Dios. Dios sembraría esta viña en buena tierra, y lo haría para él, dice. El fruto de la viña tendría un buen fin, y agradaría al Rey de Reyes. Dice que Dios tendría misericordia de Lo-Ruhama, la “no compadecida”, y que Lo-ammi, recuerdas que significaba “no pueblo mío”, sería su pueblo y dirían: Dios es mi Dios. Ya lo había dicho en Oseas 2:16, donde Dios sigue enseñándonos a través de términos hebreos. Dios dice a Israel “ya no me llamarás “Baali”, lo cual significa “mi señor”. Dice “En aquel tiempo me llamarás Ishi: Mi marido”. Habría una relación amorosa entre Dios y su pueblo. No sería meramente una relación de posesión y pertenencia, sino una de amor y fidelidad. Y todo esto lo ofrecía a personas que le daban la espalda.Oseas 11:1-9 nos describe cómo Dios había escogido y amado a Israel desde el principio y como ellos lo habían rechazado:“Cuando Israel era muchacho, yo lo amé, y de Egipto llamé a mi hijo. Cuanto más yo los llamaba, tanto más se alejaban de mí; a los baales sacrificaban, y a los ídolos ofrecían sahumerios. Yo con todo eso enseñaba a andar al mismo Efraín, tomándole de los brazos; y no conoció que yo le cuidaba. Con cuerdas humanas los atraje, con cuerdas de amor; y fui para ellos como los que alzan el yugo de sobre su cerviz, y puse delante de ellos la comida. No volverá a tierra de Egipto, sino que el asirio mismo será su rey, porque no se quisieron convertir. Caerá espada sobre sus ciudades, y consumirá sus aldeas; las consumirá a causa de sus propios consejos. Entre tanto, mi pueblo está adherido a la rebelión contra mí; aunque me llaman el Altísimo, ninguno absolutamente me quiere enaltecer.”Continúa mostrando que esto no podría seguir así eternamente. Dios ofrecía un cambio basado en confianza y fidelidad:“¿Cómo podré abandonarte, oh Efraín? ¿Te entregaré yo, Israel? ¿Cómo podré yo hacerte como Adma, o ponerte como a Zeboim? Mi corazón se conmueve dentro de mí, se inflama toda mi compasión. No ejecutaré el ardor de mi ira, ni volveré para destruir a Efraín; porque Dios soy, y no hombre, el Santo en medio de ti;”A pesar de la rebeldía de Israel, Dios no podría, por su carácter Santo y compasivo a la vez, abandonarlos para siempre. Conmovido por su misericordia los rescataría y los volvería a hacer suyos. Al final del libro vemos la súplica de Dios a su pueblo y sus promesas.Oseas 12:5-6 “Mas Jehová es Dios de los ejércitos; Jehová es su nombre. Tú, pues, vuélvete a tu Dios; guarda misericordia y juicio, y en tu Dios confía siempre.Oseas 13:9 “Te perdiste, oh Israel, mas en mí está tu ayuda.”Dios ofrecía una relación restaurada. Olvidando la infidelidad reiterada de Israel, Dios ofrecía fidelidad eterna. Una vez más, ¿por qué haría esto Dios? No es por lo que puede sacar de sus transacciones. Tampoco es por debilidad emocional. Lo hace únicamente en concordancia a su carácter. Su santidad no le permite aguantar el pecado humano, mas su compasión le obliga a ofrecer una salida. El rechazo de todo lo que Dios defiende trae dolor y muerte. Mas, como leemos en Romanos 6:23, “el regalo de Dios es vida eterna”, y esa vida está en Jesucristo. Cuando vemos que vamos rumbo a la destrucción, que no somos pueblo de Dios, y que no merecemos su compasión, entonces estamos en la posición ideal para clamar a Él y pedirle la salida de este camino de perdición. Justo en ese punto podemos depositar toda nuestra confianza en aquel que lo ha dado todo por tener una relación con alguien que lo había ignorado, o rechazado. Por medio de la fe en Cristo podemos recibir la compasión amorosa de Dios, podemos llegar a ser su pueblo amado, y disfrutar de la verdadera vida en Cristo. Dr Carolyn Lam:                Welcome to Circulation on the Run, your weekly podcast summary and backstage pass to the journal and its editors. I'm Dr Carolyn Lam, Associate Editor from the National Heart Center and Duke National University of Singapore.                                                 Is there a unique lipoprotein profile for incident peripheral artery disease as opposed to coronary or cerebral vascular disease? Well, you're just gonna have to wait for our feature discussion to find out. That's coming right up after these summaries.                                                 Our first original paper this week tells us that gene variance known to be associated with idiopathic and peripartum cardiomyopathy are also associated with preeclampsia. First and corresponding author Dr Gammill from University of Washington and colleagues studied 181 participants with confirmed preeclampsia from the Preeclampsia Registry in BioBank. Saliva samples were collected for DNA isolation and whole exome sequencing was performed to detect rare variants in 43 genes known to be associated with cardiomyopathy.                                                 Results were compared with data from two controlled groups, unrelated women with a gynecological disorder, sequence using the same methods and instruments, as well as published variant data from 33,000 subjects in the Exome Aggregation Consortium.                                                 The results showed that women who developed preeclampsia are more likely to carry protein altering mutations in genes associated with cardiomyopathy, particularly, the TTN gene which encodes the sarcomeric protein titin. Thus, detecting these gene variants may allow more specific diagnosis, classification, counseling and management of women at risk.                                                 Prior trials have shown that nonsteroidal anti-inflammatory drugs or NSAIDS confer cardiovascular risk. Now this has been postulated to be due to enhanced formation of methyl arginines in the kidney that would limit the action of nitric oxide throughout the vasculature. However, the next original paper in this week's journal suggests that this may not be correct. First author, Dr Ricciotti, corresponding author, Dr FitzGerald from University of Pennsylvania Perelman School of Medicine and colleagues, used multiple genetic and pharmacological approaches to disrupt the COX 2 pathway in mice and analyze plasma from patients taking NSAIDS.                                                 However, they did not observe an increase in methyl arginines. In contrast, they did observe an increase in plasma asymmetric dimethylarginine or EDMA in mice-rendered hypertensive by infusion of angiotensin II at a dose that also caused renal impairment. After a four week washout period following the infusion of angiotensin II, blood pressure, creatinine, and ADMA levels all fell back to normal levels.                                                 Celecoxib-treated mice also exhibited increased ADMA and plasma creatinine in response to infusion of angiotensin II and their levels also returned to normal thereafter. Thus, it seems likely that the previous reported elevations in ADMA reflected renal dysfunction rather than a direct consequence of COX 2 deletion or inhibition. The authors end by suggesting that the most plausible mechanism by which NSAIDS confer a cardiovascular risk, is by suppression of COX 2 derived cardioprotective prostaglandins such as Prostacyclin rather than by enhanced formation of methyl arginines.                                                 The next original paper identifies new targets with the potential to prevent vascular malformations in patients with hereditary hemorrhagic telangiectasia. Co-corresponding authors, Dr Ola and Eichmann from Yale University School of Medicine and colleagues looked at SMAD4, which is a downstream effector of transforming growth factor-beta/bone morphogenetic protein family ligands that signal via activin-like kinase receptors.                                                 The authors generated a tamoxifen inducible postnatal endo-fetal specific SMAD for a mutant mouse and showed that SMAD4 prevented flow-induced arterial venous malformations by inhibiting casein kinase II. The uncovered pathways provided novel targets for the treatment of vascular lesions in hereditary hemorrhagic telangiectasia related juvenile polyposis patients carrying SMAD4 mutations.                                                 The next original paper provides important data for the accurate diagnosis of long QT syndrome. Long QT syndrome can be a challenging diagnosis partly because the optimal method for QT assessment is not unequivocally established. QT experts advocate manual measurements with a tangent or threshold method.                                                 In today's paper, first and corresponding author, Dr Vink from Academic Medical Center University of Amsterdam and colleagues, aimed to assess similarities and differences between these two methods of QT interval analysis among 1,484 patients with a confirmed pathogenic variant in either KCNQ1, KCNH2 or SNC5A genes from 265 families. Both QT measurement methods yielded a high inter and intra reader validity and a high diagnostic accuracy.                                                 Using the same current guideline cutoff of QTC interval 480 milliseconds, both methods had similar specificity but yielded a different sensitivity. QTC interval cutoff values for the QT measured by the tangent method was lower compared to that measured by the threshold method. Plus, values were different depending on the correction for heart rate, age, and sex.                                                 The authors provided an adjusted cutoff values specified for method, correction formula, age, and sex. In addition, a freely accessible online probability calculator for long QT syndrome at www.QTcalculator.org has been made available as an aid in the interpretation of the QT interval.                                                 The next original paper demonstrates for the first time that thrombin mediated signaling may play a role in diet-induced atherogenesis. Co-first authors, Dr Raghavan and Singh, corresponding author Dr Rao from University of Tennessee Health Science Center and colleagues, used a mouse model of diet-induced atherosclerosis and molecular biological approaches and explored the role of thrombin and its G protein coupled receptor signaling in diet-induced atherosclerosis.                                                 They found that thrombin-induced CD36 expression and foam cell formation required protease activated receptor 1, G alpha 12, Pyk2, GAB 1, and protein kinase C theta dependent activating transcription factor 2 activation. Thus, inhibition of thrombin G protein coupled receptor signaling could be a promising target for the development of new drugs in reducing the risk of diet-induced atherogenesis.                                                 The next study provides insights into the long- term association of LDL cholesterol with coronary heart disease mortality in individuals at low tenure risks of atherosclerotic cardiovascular disease. First and corresponding author, Dr Abdullah, from VA North Texas Medical Center and UT Southwestern Medical Center and colleagues studied more than 36,000 subjects in the Cooper Clinic Longitudinal Study cohort who are at low tenure estimated risk of atherosclerotic cardiovascular disease. In other words, a low tenure risk of less than 7.5%. They've followed these patients for more than two decades.                                                 Results showed that LDL cholesterol and non-HDL cholesterol at or above 160 milligrams per deciliter were independently associated with a 50 to 80% increased relative risk of cardiovascular disease mortality. The associations between LDL cholesterol and cardiovascular disease mortality were more robust when follow up was extended beyond the traditional 10 year estimated risk period.                                                 The associations remain significant in those with an estimated tenure atherosclerotic cardiovascular disease risk of less than 5%. These data suggests that LDL cholesterol levels at or above 160 milligrams per deciliter in individuals deemed to be at low tenure atherosclerotic cardiovascular risk are associated with worse long term cardiovascular disease mortality. These findings, along with other observational data and data extrapolated from clinical trials, support further consideration of appropriate LDL cholesterol thresholds for lipid lowering interventions in individuals categorized as low short-term risk.                                                 The final paper this week uncovers a novel therapeutic target for the prevention and treatment of thoracic aortic aneurysms. First author, Dr Nogi, corresponding author Dr Shimokawa from Tohoku University Graduate School of Medicine and colleagues, used genetically modified mice to show a pathogenic role of the small GTP binding protein, GDP dissociation stimulator in the development of angiotensin 2 induced thoracic aortic aneurysms and dissection. Down regulation of this protein contributed to dysfunction of aortic smooth muscle cells and hence oxidative stress, and matrix metalloproteinase activities in the pathogenesis of thoracic aortic aneurysms and dissection.                                                 Local over expression of this small GTB binding protein GDP dissociation stimulator around the thoracic aorta inhibited aortic dilatation and rupture in deficient mice. And that wraps it up for this week's summaries. Now for our feature discussion.                                                 Atherosclerosis has been considered a systemic process, meaning that when we see a disease in one vascular bed, we assume that that's a risk marker for disease in other vascular territories, and that they share pathophysiology, they share risk factors. However, if we think about it, the prior studies have all been sort of focusing on coronary and cerebral vascular disease, but today's feature paper changes that a bit because it addresses a key knowledge gap in peripheral artery disease risk, and interestingly suggests that there may be a unique lipid profile that's related to peripheral artery disease.                                                 This is gonna be an exciting discussion and I have the first author, Dr Aaron Aday from Vanderbilt University Medical Center currently. We have our editorialist, Dr Parag Joshi from UT Southwestern, and our associate editor, Dr Anand Rohatgi from UT Southwestern. Welcome gentlemen and Aaron, could we start with you sharing about your study? Dr Aaron Aday:                 So, as you mentioned, a lot of the previous epidemiologic data on atherosclerosis have been primarily in coronary artery disease and stroke, and when we looked at peripheral artery disease or PAD, there seemed to be some subtle differences. So for instance, total cholesterol on HTL cholesterol seemed to be the strongest risk factors for future peripheral artery disease and in terms of LDL cholesterol, the data are somewhat mixed. Some have found a weak association, some have actually found no association. And so building on that, we wanted to see if using nuclear magnetic resonance spectroscopy, we could elucidate more details about the litho protein pathways associated with peripheral artery disease.                                                 And we did this in the women's health study which is a prospective cohort study of women free of cardiovascular disease, the baseline, they were aged 45 and older. And what we've found in terms of the standards with their profiles, we again found that there was no association between LDL cholesterol and future peripheral artery disease, whereas certain standard lipid measures like HDL cholesterol were strongly associated with PAD, and then using the Endemol spectroscopy tool, we found that actually, small LDL particles and total LDL particles were concentrations of both of those markers, were strong risk factors for future PAD and other measures like total HDL particle concentration were even more strongly associated with future PAD than coronary artery disease.                                                 So essentially the signature associated with future peripheral artery disease, had some important differences than that for a composite of coronary artery disease and stroke. Dr Carolyn Lam:                Aaron thanks for that. That's beautifully described and just so intriguing. Parag, could you tell us how should we be thinking about results like this? Dr Parag Joshi:                   It's a great paper and it really highlights a new and unique approach in that we ... Peripheral artery disease as an isolated incident event is fairly understudied I guess we could say and so, this is a really nice paper to start choosing out some of the risk factors for that. I think overall, when we think of peripheral arterial disease in general, I think historically, we've thought of it as similar pathophysiology, you know LDL particles and perhaps other particles depositing in the arterial space. But this does highlight some important differences that might exist and I think one of those seems to be that maybe this is more a signature of elevated remnant lipoproteins or triglyceride rich remnant lipoproteins, small dent LDL particles, low HDL, that sort of metabolic syndrome type patterns that we look at as a high risk factor that may be more contributory to peripheral artery disease than coronary disease, or at least more specific to peripheral artery disease.                                                 I guess one of my main questions about that from your work Aaron is, how can we be sure this isn't just a pre-clinical marker of diabetic patients which we know have this type of pattern? Dr Aaron Aday:                 Sure, it's certainly a possibility. I think what's notable in the cohort, at least a time enrollment. And there was a very little diabetes and actually there was a much greater prevalent of metabolic syndrome. So in my mind, it may be more of a metabolic syndrome specific marker rather than necessarily down the diabetes pathway, but it's certainly something that needs to be explored further. Dr Parag Joshi:                   I wonder whether women's health studies such a healthy cohort that I wonder if this is picking up some signal before the answer to diabetes or as you said, metabolic syndrome, you know which certainly suggests an insulin resistance pattern and we know the association of diabetes with peripheral artery disease is stronger and so I wonder if this may be a sort of earlier way of picking that up. Dr Aaron Aday:                 It may be. I think one thing to notice is the outcome of peripheral artery disease that we're using. So it is symptomatic disease. So, we're not picking up a lot of ulcers that are developing in the future, it's more the claudication and then people who've undergone revascularization. Certainly diabetics have both of those as well but I think that may suggest it's not fully unexplained by developing diabetes than peripheral artery disease further down the line. Dr Parag Joshi:                   Yeah that's a great point. Dr Carolyn Lam:                Yeah great questions, great thoughts. Anand, what about you? Did you have questions too? Dr Anand Rohatgi:            I think from my perspective and thinking about it for circulation and its readership, we found this really interesting for several reasons. Number one, I think is, as you all have discussed, peripheral arterial disease just is not as well characterized and you can see that here in over 25,000 people, add about a 100 a bed, so I think in younger folk, it takes a lot of people to study, to be able to really understand kind of the pathophysiology of peripheral arterial disease.                                                 The other thing that they think they really shed some light on is how this is happening in women in particular and in women, of course as we know have been understudied in all cardiovascular diseases, but in particular, diseases like this which are less common. It's really insightful to see that these lipid abnormalities in women are contributing to peripheral arterial disease more so than your typical LDL cholesterol management and interestingly enough, most of the women who had PAD events in this study, did not have other cardiovascular events.                                                 They really just had PAD events exclusively and I thought that was really intriguing, and the use of this advanced lipoprotein testing, this NMR modality has been very useful in terms of biology and research, and I think that's the case here where we really go under the hood Carolyn, as you said, and get kind of deep dive, the lipid metalobles on abnormalities. And I think Parag and Aaron hit the nail in the head that this is really capturing an insulin resistance of phenotype and what I really liked about this is, instead of studying people who are 70, 80 years old and a lot of things are sort of clustering, a lot of diseases are clustering and they're manifesting all at the same time, it's very hard to tease apart the effective age.                                                 Here, we captured women in their 50s and middle aged, just as they have kind of gone through menopause and this adverse metabolite's phenotype starts to rise in women. And then we could follow them over time and see what the natural history of that is, and the women who have this phenotype go on to have this devastating consequence, this peripheral arterial disease. One of the questions I had then, Aaron for you is, what do you think the implications are from these findings? Does it mean that in terms of diagnostics, we should be doing more advanced testings looking at LDL and HDL type particles with NMR or some other mortality? Does it change therapies with new therapies beings studies right now? What do you think the implications are from your work? Dr Aaron Aday:                 That's important right. I think you mentioned this and I see the inter marked tool in this study, is really a way to try to dig further into the biology of peripheral artery disease as a form of atherosclerosis. I think that we already know patients who are extremely high risk or PAD, those are patients with diabetes, smoking history, metabolic syndrome et cetera., and as you can see in a patient population in 28,000 middle aged women who are pretty healthy, we only had just over a 100 PAD events.                                                 So, I think even if you were to scale this up in terms of cost, I'm not sure that that would necessarily be a viable option for patients, but I think it does suggest that truly focusing on LDL in a very high-risk patient population, meaning patients with PAD, or we may not be fully addressing their risk. And so I think this is a need to highlight that important gap, think about other therapeutic options and we'll soon have ongoing trials, triglyceride low in therapy that may be particularly beneficial in this patient population and so that's how I see this being used. Dr Anand Rohatgi:            That makes a lot of sense and particular because in middle aged women like this, your standard risk score algorithms will not really capture that they're at increased risk, even if they smoke, just because they're women and they're younger and so, I think this really is a call to arms to more refined risk assessment in these women. Dr Parag Joshi:                   Aaron, do you think there's actually a difference in the biology in the peripheral arteries compared to the coronary and cerebral vascular beds, or is there data to kind of look at that or maybe histopathological data to look at that? Dr Aaron Aday:                 We know there's a lot of overlaps, so I don't wanna suggest that PAD is not a former atherosclerosis. I think one limitation is that the primary animal model for PAD is the hyperCKemia model. That doesn't fully recapitulate what's happening in a limb with PAD and so I think that has been one limitation in understanding the biology. But I think what we're starting to see in some clinical trials that have come out in the last couple of years or starting to see a somewhat different signal for therapies in patients with PAD so for instance, in 48, we actually saw that there was a greater benefit to LDL lower [inaudible 00:21:00] inhibitors than for coronary disease. We now have the compass trial results, again, more events, higher risk among these patients but for their benefit, add on River Oxodine therapy, we've seen lymph events or lymph signals in the SGLP2 inhibitor trials. So, I think we're starting to get a sense that there may be something else on top of the traditional ascariasis biology that may be a potential target on down the road. Dr Parag Joshi:                   I think it's really a fascinating biological question of how these different territories might actually differ in their pathophysiology. I think it's a really a nice time to look at this. Also I think, Anand and Aaron both mentioned ongoing trials. The omega 3 fatty acid trials I think reduce it, will be soon to be presented and hopefully published in the next month or so. It would be nice to see if they evaluate peripheral events in that group, I'm sure they will. Dr Carolyn Lam:                Indeed, these have been just such great thoughts and discussion. Nothing really much to add there. I suppose I could say something cheeky like for the first time, and I never thought I'd say it on the podcast, I feel kind of bad that there are no men included in this trial but anyway, I just learnt so much from this. In over half of all cases of hospitalization for a cardiovascular event, the first symptom is the event itself. So anything we can do to get any early indicator that something is going wrong in the cardiovascular system can have a huge impact. Erectile dysfunction is one such early signal. According to cardiovascular health expert Daniel Chong, ND, identifying sexual dysfunction is essential for improving cardiovascular outcomes.  Approximate listening time: 30 minutes About the Interview It may seem counterintuitive to interview a cardiologist, and not a urologist, on the topic of erectile dysfunction (ED). But we now know that ED is a result of endothelial cell dysfunction and ED can be an early warning sign of systemic atherosclerosis. Looking at ED from a cardiovascular perspective is essential. That’s why we invited cardiovascular expert Daniel Chong, ND, to talk to us about ED’s connection to heart health. In this interview, Natural Medicine Journal’s editor-in-chief, Tina Kaczor, ND, FABNO, asks Chong about the complex interplay between vascular function and sexual function. According to Chong, cardiovascular disease always has some degree of contribution—potentially a major one—in ED. That’s in part because blood flow is the key facet to obtaining a full erection. Cardiovascular dysfunction, including plaque in the arteries that regulate that blood flow, can therefore have an impact on ED. Even before plaque development becomes a problem, endothelial dysfunction in the inside walls of the arteries can play a role in erectile function. In this enlightening interview, Chong explains the different issues that can contribute to ED, including anatomical, physiological, and psychological problems. It’s an important listen for any practitioner who sees men, since beyond being a problem in and of itself ED can be an early signal of other serious health concerns. About the Expert Daniel Chong, ND, has been a licensed naturopathic physician, practicing in Portland, Oregon, since 2000 and focusing on risk assessment, prevention, and drug-free treatment strategies for cardiovascular disease and diabetes, as well as general healthy aging, and acute and chronic musculoskeletal injuries. Chong has also completed certificate training in cardio-metabolic medicine from the American Academy of Anti-Aging Medicine and is an active member of the Society for Heart Attack Prevention and Eradication (SHAPE). In addition to his clinical work, Chong serves as a clinical consultant for Boston Heart Diagnostics Lab. Transcript Tina Kaczor, ND, FABNO: Hello. I’m Tina Kaczor for the Natural Medicine Journal. Today, we’re going to be talking about erectile dysfunction and cardiovascular disease with Dr Daniel Chong. Dr Chong is a naturopathic physician with a private practice in Portland, Oregon for the past 17 years. He specializes in what he likes to call "vascular wellness optimization." He’s also the founder of the web-based consulting company, the Healthy Heart Project which offers a number of educational and direct consulting options for both the general public as well as healthcare practitioners on how best to assess and reduce risk for cardiovascular disease. Dr Chong also lectures and serves as a clinical consultant for Boston Heart Diagnostics Lab. Thanks so much for joining me today, Dr Chong. Daniel Chong, ND: You're welcome, Tina. Good to be here. Kaczor: As I mentioned, our topic today is erectile dysfunction. At first, it may seem odd to our listeners that I’m talking to a cardiology expert and not a urologist or men’s health expert but we now know that erectile dysfunction is a result of dysfunction of endothelial cells and in fact, this can be an early warning sign of systemic atherosclerosis. Dr Chong, can you start us out with a brief overview of how erectile dysfunction and cardiovascular disease are related? Chong: Sure. I can do my best there. There’s definitely going to be different circumstances that can contribute to erectile dysfunction. Some of which may not be actually anatomical, so to speak, or physiological from the cardiovascular perspective but I would say the majority is at least indirectly affected because even if we’re talking, for example, about a psychological contributor which we may touch on later, if somebody has dysfunctional arteries down there in the penis, they’re going to be more vulnerable to effects from psychological aspects than they would be otherwise. In other words, a young teenager may get stressed out in an early sexual experience but that’s not going to affect function as much as it could a 50-year-old man. Anyways, in general, we could just say that cardiovascular disease is going to have some degree of contribution and potentially major. Obviously, blood flow is the key facet to obtaining a full erection and certain arteries are going to be more vulnerable to impacts from the development of cardiovascular disease but even so, the arteries in the penis may or may not actually have plaque in them but they can still dysfunction. Typically, we know, and we’re going to talk about this later, in cardiovascular disease, the preceding step prior to actual anatomical change or plaque development is endothelial dysfunction or dysfunction in the inside wall of the arteries and even that going on without any actual plaque having developed yet can affect erectile function and not to be noticeable by the person. All in all, I guess you could say they’re intimately intertwined because you have to have good blood flow. It may or may not have plaque. Plaque may or may not be actually playing a role yet but it will in some cases and cause really significant dysfunction, but even minor dysfunction is going to be at least the partial result of the arteries starting to misbehave for various reasons that hopefully we’ll touch on. Kaczor: Yeah. I actually came across some mention of erectile dysfunction in that whole idea of plaque formation. One author said that it could signify in some patients, or at least it should be followed up to see if it signifies subclinical atherosclerosis. Chong: Correct. Kaczor: Yeah. Atherosclerosis being pretty much asymptomatic in people until there’s larger consequences. On that note- Chong: Right. Yeah. Sorry to cut you off. Sadly, it’s been shown that in over 50% of cases of hospitalization for a cardiovascular event, the first symptom is the event and that’s over half of all of them, so anything we can do to get any early indicator of something in this, so to speak, before, for example, erectile dysfunction, is hugely important for us because we are not doing a very good job at least conventionally in identifying early on what’s going on with people. Kaczor: Yeah. I look forward later in this discussion to talk to you about how to assess it, to find early markers besides just the symptom of erectile dysfunction but let’s start with the larger picture in conventionally recognized erectile dysfunction and cardiovascular disease risk factors. Can you talk a little bit about like when we’re, as clinicians, who walk into our office, who we should suspect it in or at least engage in the conversation because many patients won’t bring it up themselves unless they're directly asked? Dr Chong: Yeah, absolutely, so, certainly age. The older a man gets, the more potential there's going to be for all kinds of different changes going on physiologically. Some people are well aware of testosterone production, how crucial that is and that certainly begins to change as a man ages. But certainly, very standard, interestingly enough, it’s the same standard risk factors you might consider for cardiovascular disease in general in terms of high blood pressure, diabetes, certainly, smoking. Conventionally, you're going to see high cholesterol as a stated contributor but we can certainly talk in more detail about that because I know that some people out there in the functional medicine world, naturopathic world, et cetera, consider high cholesterol as a past tense risk factor for cardiovascular disease which it really is and it’s just more complicated than that. Obesity, lifestyle factors in terms of exercise and then certainly, psychological factors, depression and anxiety, et cetera are all going to be key things. I also want to make a just brief mention even though this is kind of a topic in and of itself, when we talk about erectile dysfunction, obviously, we’re talking about men but it should be very clearly stated that the same potential processes are going on in women as they age. Women with difficulty with sexual activity or orgasm, et cetera, may in fact be having their own version of “erectile dysfunction” with the clitoris as essentially an analogous structure in a woman and all of these blood flow issues can occur in women as well. It’s important to really kind of make mention to that. I say men, I keep saying men, as men age, blah, blah, blah, but it really should be looked at as both sides of the coin, so to speak. Kaczor: That’s actually an important point. Thank you for mentioning that. Chong: Sure. Kaczor: I want to do a follow-up on that cholesterol thing that you just mentioned because I think that that’s kind of top of mind. I think it’s important to give voice to any new data on looking at cholesterol because I'm with you on it being much more complex and it’s more complex than I understand. I'm happy for you to kind of flesh it out for us. Chong: Yeah. I mean, I guess anybody that says that cholesterol has nothing to do with cardiovascular disease is not really thinking about the fine details of the situation. You can't have a plaque form without cholesterol and lipoprotein particles being involved because they are what are the sort of primary components to the development of the plaque. What I don’t agree with conventionally is the idea that high cholesterol, in and of itself, is just going to definitively contribute to cardiovascular disease because obviously, there are many people out there who have relatively “high cholesterol” who don’t get cardiovascular disease. There's certainly something else going on that’s playing a role as to whether or not high cholesterol is going to lead to that issue in some people versus others. Long story short, I consider cholesterol and related markers to be secondary factors. They are absolutely involved but they are not … There's going to be other things that help sort of determine the likelihood or lack thereof of the high cholesterol sort of turning into cardiovascular disease. That’s a really fun discussion in and of itself. It could be another hour or so by itself but hopefully, that kind of answers your question, at least preliminarily. Kaczor: Well, it brings up another question which is- Chong: Certainly, keep going with that. Yeah. Kaczor: Yeah. If cholesterol is considered a secondary factor, and I see what you're saying, cholesterol is not … needs to be present but can't be causative because there's not a cause and effect 100% of the time. Chong: Correct. Kaczor: If it’s secondary, what are you looking at as primary? Chong: Well, to me, the absolute most important thing that’s going to contribute to the potential or lack thereof of eventual cardiovascular disease development or i.e. plaque, development is the health and vitality of the walls of the artery and how well they're functioning. In other words, the healthier, more nutritionally replete the walls of the arteries are themselves and the better they're being sort of manufactured in the first place by the body, are going to be the primary factor that leads to vulnerability or not. If you imagine like … I would like to use analogies. On a coastline, you may have, let’s say, in Hawaii versus somewhere else on the mainland. Hawaii is made up of volcanic rock which is, tends to be a little bit more brittle and it can sort of erode more easily. If you have waves crashing into the wall, into a wall of rock in Hawaii, it may erode more quickly. Then, an analogous wall somewhere else in the world that’s made up of a different, harder, more resilient material, the waves are still crashing into them with the same potential force but one’s going to erode more quickly than another. If we then relate that to the vascular system, somebody who has poor nutrition and tons of inflammation, oxidative stress, et cetera, and especially long-term poor nutrition, they're not going to be able … especially if we’re talking about collagen production, they're not going to be able to manufacture the sort of strong, resilient vascular walls that they should which will inevitably be, if they are stronger, will inevitably be more resistant and resilient to the impact of the turbulence of the flow of blood. There are certainly other things that are going to impact that as well especially the turbulence itself and the viscosity of the blood. That’s going to make for essentially like stronger waves crashing in which obviously, the stronger the waves is crashing into the area, the more potential there is for erosion as well. To me, long story short, the primary situation that’s going to lead to the potential development of plaque is a combination of two primary factors. That’s the vulnerability of the wall of the artery and the stress that is being placed on the wall of artery. Kaczor: By- Chong: If you look at every single risk factor we know of, they are impacting one or both of those factors. Kaczor: Okay. When you say stress, you mean mechanical forces, as well as chemical? Chong: Chemical. Absolutely. Kaczor: As in oxidative stress? Chong: Correct. That would be one of them. I mean, even environmental toxins, different types of infectious organisms and certainly mechanical stress as well or what we call blood viscosity which is impacted by a variety of factors. Primarily, probably the main ones for blood viscosity would be hydration and like even iron levels or high sort of … basically, concentrated solid substances in the blood and then also, cloudiness of the blood, how high is fibrinogen levels and things like that are going to impact the viscosity of the blood. Then, the classic risk factor of high blood pressure is going to be too, more or less, stress on the wall of artery. Kaczor: Let’s- Chong: Sorry. One other thing. I mean, one of the ways that high cholesterol may be contributing to things is it’s known that the higher the cholesterol is, the stronger the impact on the vascular wall is. It actually causes … High cholesterol itself can contribute to endothelial dysfunction or stress on the function of the wall of the artery. Kaczor: Doing mechanical forces, you're saying, to the viscosity of the blood. Chong: Right, and more technical reasons, like it literally messes with certain aspects of how the wall, the endothelium is supposed to be functioning. It’s not just that it gets into and becomes part of the plaque. The higher your cholesterol goes, the potentially worse the endothelial function initially. Kaczor: Okay. Let’s switch gears a little bit. If we’re talking about endothelial dysfunction as the commonality between erectile dysfunction, atherosclerosis, cardiovascular disease, it’s all about a healthy endothelium. Chong: Right. Kaczor: It’s interesting, in that same paper I mentioned before, I came across a term that I had not seen before. It was the endothelium as a single organ which I thought was a really interesting concept like, “Oh,” thinking, “I'm sure it’s different, in different tissues,” but just the idea of overall health of it being a singular thing was interesting to me. Chong: Right. People look at the blood vessel as like these tubes that are just allowing for the passage of blood flow. There's so much going on at the wall of the artery physiologically. It is absolutely an entire organ. Kaczor: Let me ask you this. As far, for us as clinicians, what are either biomarkers or assessment tools, how do we gauge endothelial function in a patient? Chong: Well, technically, when we’re specifically talking about endothelial function, there's only a few ways to directly assess that. Clinically, they're going to involve some way, shape, or form of actually testing, in-office, the function of the arteries themselves. There's a general … There's a few … There's basically two main machines that I'm aware of. One is called an EndoPAT and one is called the EndoTherm that are designed to directly assess endothelial function. The way they basically work is they … You have your fingers in some type of device that’s monitoring either blood flow or temperature at the fingertips. Then, you basically occlude the artery and the arm like you would with the blood pressure cuff. You have to do that for about 5 minutes which is not enjoyable for the patient because, as you can imagine, it isn’t feel very good to have your blood occluded for 5 minutes. Then, prior to doing that though, you're doing a general assessment on blood flow and temperature of the fingers. Then, you occlude the blood flow and then you let it out all at once. When the blood comes, as you might imagine, rushing back into the extremities in the fingers, you should get some degree of expansion of the arteries. Normal function would lead to the arteries, as the blood really rushes in there, would lead to the arteries expanding to a certain extent. People that have endothelial dysfunction, their blood vessels will not expand appropriately. The machines are designed to sort of read that, sort of the tip, where your tips of your fingers are sitting, the machines is detecting, is there a significant enough change in temperature and or blood flow. There's also something called arterial pulse velocity which basically, there's a smaller device called an iHeart like an iPod but it’s iHeart. I'm not connected to any of these companies or anything like that but that is a newer device that’s being developed that checks sort of indirectly the same thing. It looks kind of like a pulse oximeter but it’s actually detecting arterial pulse wave velocity and literally how quickly a pulse rate is moving down the arterial tree. If you might imagine, the sort of left compliant and arterial, an artery is, the quicker the pulse rate is going to move down it. That’s generated by heart, a heartbeat. Those are the only ways that I'm aware that are … Those are the only things that I'm aware that are being used in-office to directly assess endothelial function. There is a lab test that can be measured with people called ADMA. It stands for asymmetric dimethylarginine. That is considered a surrogate or indirect assessment of endothelial function. The higher the ADMA is, the higher the potential for endothelial dysfunction because it’s a direct sort of inhibitor of nitric oxide production. Kaczor: All right. Well, that leads us into our next little piece, doesn’t it? Nitric oxide production being integral to the whole relaxation of the smooth muscle and the endothelium to allow for blood flow whether we’re talking about the fingertips or the penis. Can you talk a little bit about nitric oxide? Maybe briefly mention how an assessment can be made, the ADMA being one of the means of assessing that as far as the blood test and anything else that might be accessible to a general physician or clinician that might be seeing these patients. Chong: Well, I mean, endothelial function is, to me, the ideal way to get an assessment of that because I'm a big proponent of the idea that we want to check end of point factors as often as we can. Classic example of this is looking at the different impacts of certain dietary changes on cholesterol markers and making conclusions about whether or not that is good for the vascular system or not, certain changes like HDL going up, for example, after the implementation of a certain diet did not guarantee by any stretch of the imagination that you're having a positive effect on the vascular system so I like to use endpoint markers or end, sort of, functional markers as much as possible so far and away still, the best way to me to assess nitric oxide levels is via those endothelial function tests that we mentioned already. Other ways to sort of try to get an assessment of it, the only other way that I’m really aware of is if you've seen … You've been to enough conferences, I know. You’ve probably seen this company that has this little saliva test that you can use to check basically nitrate levels in the saliva. That’s going to be … Nitrate is a crucial factor, nitric oxide production as well, so some people are using these little saliva tests to check what a person’s typical nitrate intake is and then recommending dietary or supplement interventions based on that. Those are really the only ways that I’m aware of to sort of really truly get an assessment on that other than, obviously, history and talking to a person, seeing how well things are working, so to speak. Kaczor: Can I ask you a question? I don’t mean to put you on the spot and I do not know the company that’s offering nitrate levels in saliva but is this something that’s been validated or is it with any rigor or is this one of those early adoption things that happen? Chong: Right. You're asking me if something has been validated with scientific tests or research? Can you restate? Kaczor: Or at least … Yeah. Chong: You do that with everything which is great. That’s why I like you so much but I don't know for sure. This is … In all honesty, I haven’t really looked too deeply into that method of assessment with people, so I wouldn’t be able to say with any certainty at all. I know that they’re quite widely used and it’s not a very complicated, technically complicated test so I think it’s pretty straightforward. I do recall seeing literature being made available by these companies but I have not looked that in-depth at that at this point. Kaczor: Well, I appreciate your honesty. When you're on the cutting edge, early adoption of new technologies is part of our … We get to do that. We get to be right there doing, instituting things but it’s important, I think, for us all to go at a pace that has some, at least reproducibility, if not rigor. Chong: Absolutely. The other thing that I would say to add to that is like using different angles of assessment is also crucial, not just relying on one piece of information whether it be cholesterol. That’s why the classic conventional mistake is like, “Okay, we’re going to check and see if you have a high risk for cardiovascular disease. Let’s check your lipid panel. There’s so much more beyond that that can be done to assess and evaluate people and get a much clearer picture. That’s a classic idea, just sort of not settling on one thing, not just using the newest thing, whatever it is. Use as many tools as you can within reason to get the clearest picture. Kaczor: Yeah. I want to continue on the molecular biology of this and specifically, we have just a few minutes left, really talk about- Chong: Time flies when you're talking about erectile dysfunction. Kaczor: What’s that? Chong: I said time flies when you're talking about erectile dysfunction. Kaczor: Well, oxidative stress, being something that you mentioned and it’s just something that we’re … That inflammation is kind of always at the forefront of anyone who’s doing integrative medicine or optimal wellness or however you want to term it. I guess my thought is this. In a concise way, can you tell me if you use any actual blood markers that are widely available and what are some of your favorite ways of, kind of across the board, addressing oxidative stress issues, which even beyond erectile dysfunction, it becomes part and parcel with that but it’s also just part of life and part of being alive, is creating oxidation? Chong: Right. In the realm of assessment, especially if we were going to so far as to separate out inflammation in oxidative stress because obviously, they aren’t exactly the same thing, when we’re talking inflammation, the primary markers that I’m measuring with people certainly are high sensitivity CRP as our sort of general global marker of inflammation or lack thereof. When we’re talking about the vascular system, I’m also typically going to be checking something called Lp-PLA2 or what’s also known as the PLAC test. That is more specifically an inflammation marker for the vascular system so it’s going to actually reveal immunoactivity and inflammation going on in the wall of the artery whereas a high CRP is not going to be able to definitively determine that or not. MPO or myeloperoxidase is a later stage, nonspecific but frequently correlated marker for late stage vascular inflammation for a vulnerable vascular system. In the realm of oxidative stress, the 2 primary markers that I might look at is actually … number 1 is actually oxidized LDL so it’s pretty hard to have moderately elevated LDL levels and a high amount of oxidative stress and not see a relatively increased level of oxidized LDL in the bloodstream. That is sort of a good, what you'd call extracellular oxidative stress marker, but we can also get intracellular oxidative stress for different reasons. For that, you can also check something called 8-oxoguanine which is an actual, actually a urinary test. Not too many labs run that. I’m not sure if we’re supposed to name names here but that is an … If you just Google 8-oxoguanine test or something like that, you can probably find the labs that run that but that’s going to give you more of an assessment of intracellular oxidative stress. Then, beyond that, you can, in all honesty, get a pretty good idea whether or not somebody is going to be a candidate for high oxidative stress just by talking to them and looking at them and that type of thing as well. Kaczor: Yeah. A lot of those other markers for cardiovascular disease like obesity, even the aging process, certainly smoking, all- Chong: Right. Absolutely. Kaczor: Obviously, we would take into account for oxidation. Can you let me know or let the listeners know your top three? Someone looks at you and they’re like, “Listen. I do everything right. I exercise. I eat well. My BMI is normal. I don’t drink. I don’t smoke. What are the three supplements you …” You only get to see them once. They’re going to leave your office. Chong: These people are eating well, you said, in my opinion? Kaczor: Okay. That brings up the point. What would that look like in your opinion? Chong: No, no. I’m sorry. I’m just- Kaczor: We only have 2 minutes left but what would be an ideal guy in your opinion and then- Chong: No, no, no, no, no. I’m sorry. I was just clarifying the question. If these people are already eating well like they’re eating lots of fruits and vegetables, et cetera and I’m just talking about supplements, the 3 main ones I’m going to recommend are going to be vitamin C, magnesium, and then probably some type of concentrated plant-based antioxidant. As a naturopath, herbal medicine trained, I have an affinity to hawthorn but also, I frequently recommend hibiscus tea to people. Kaczor: Nice. Hibiscus being, you're also from Hawaii so that’s- Chong: Good point. You could certainly go beyond that and complement it with things like arginine, citrulline, and then there are a number of nitric oxide precursor type of products that are high in dietary nitrates. Kaczor: Well, Dan, I really appreciate this. I feel like we could have a whole part 2 where we go into the therapeutics and more details into all of this but I think the listeners have gotten good overview today and I really do appreciate the time you've taken and your expertise, and best of luck with your Healthy Heart Project.
