GLP-1 medications like Ozempic, Wegovy, and Mounjaro have become household names, celebrated as breakthrough treatments for weight loss and obesity. But while headlines showcase dramatic before-and-after photos and promises of rapid transformation, when we step beyond the marketing, a more nuanced reality comes to light. In this candid conversation, Dr. Mark Cucuzzella, a family medicine physician, and Dr. Tro Kalayjian, board-certified in internal and obesity medicine, draw on decades of clinical experience to examine where GLP-1s truly shine, where they fall short, and why the foundation for lasting health can't come from a prescription alone. They make the case for integrating GLP-1s into a comprehensive care plan, one that addresses nutrition, movement, sleep, and stress. Without these pillars, patients face higher risks of side effects, reduced long-term benefits, and the all-too-common rebound weight gain.In this interview, you'll learn:Which patients benefit most from GLP-1 therapy (and which may not)How “medication stewardship” ensures safe, responsible useWhy starting at the lowest effective dose and tapering mattersHow lifestyle-first programs can match or exceed drug results at a fraction of the costDrs. Cucuzzella and Kalayjian remind us that true success lies in restoring metabolic health and building habits that last long after the injection ends. GLP-1 medications can be powerful allies, but they're not a silver bullet. Without a foundation of nutrition, movement, sleep, and stress management, they risk becoming just another short-term fix.Expert Featured:Dr. Mark CucuzzellaWebsite: https://www.drmarksdesk.com/Dr. Tro KalayjianWebsite: https://toward.health/IG: https://www.instagram.com/doctortro/X: https://x.com/DoctorTroResources Mentioned:TOWARD: a metabolic intervention that improves addiction and binge eating symptomshttps://doi.org/10.3389/fpsyt.2025.1612551CMEs Mentioned:Managing Major Mental Illness with Dietary Change: The New Science of Hopehttps://www.mycme.com/courses/managing-major-mental-illness-with-dietary-change-9616Brain Energy: The Metabolic Theory of Mental Illnesshttps://www.mycme.com/courses/brain-energy-the-metabolic-theory-of-mental-illness-9615Follow our channel for more information and education from Bret Scher, MD, FACC, including interviews with leading experts in Metabolic Psychiatry.Learn more about metabolic psychiatry and find helpful resources at https://metabolicmind.org/About us:Metabolic Mind is a non-profit initiative of Baszucki Group working to transform the study and treatment of mental disorders by exploring the connection between metabolism and brain health. If you've ever wondered why you keep waking up at 2am, 3am, or right before your alarm goes off, this episode is for you. Middle-of-the-night wake-ups aren't random; they're your body's way of sending signals that something deeper might be out of balance.  Instead of brushing it off as “just bad sleep,” we'll break down what your wake-up times might be revealing about your hormones, gut, blood sugar, and more. Think of your sleep like a night shift at a factory. Different departments run on different schedules. If one department has a problem, the alarm goes off at that time. By tracking your wake-up patterns, you can start to see the clues your body is leaving. We'll dive into the most common causes, from blood sugar dips and cortisol spikes, to low progesterone, high histamine, and even liver overload. You'll hear simple analogies like your brain's “glass of wine” being empty without progesterone, or your liver as the night shift cleaner making noise when it's overloaded that make these complex concepts easy to understand. We'll also cover lifestyle factors like blue light, meal timing, hydration, and circadian rhythm, because sometimes the simplest tweaks make the biggest difference.  By the end, you'll know how to decode your body's signals, track what's really happening, and take small, sustainable steps toward better sleep. Your body isn't broken. It's communicating. This episode will help you finally understand the messages and reclaim deep, restorative rest.  Time Stamps:  (1:25) Upcoming Retreat (3:22) Why Do I Wake Up In The Middle Of The Night? (5:50) Timing Of When You're Waking Up At Night (8:47) 1 to 3 AM and The Liver (9:32) Dryer Lint Talk (11:09) Common Causes of Waking Up (15:17) Cortisol and Blood Sugar (18:05) Low Progesterone (23:54) Dehydration (26:50) Gut Imbalances (29:39) Lifestyle Factors (32:41) First Steps---------------------Find Out More Information on Vital Spark Coaching---------------------Follow @vanessagfitness on Instagram for daily fitness tips & motivation. ---------------------Download Our FREE Metabolism-Boosting Workout Program---------------------Join the Women's Metabolism Secrets Facebook Community for 25+ videos teaching you how to start losing fat without hating your life!---------------------Click here to send me a message on Facebook and we'll see how I can help or what best free resources I can share!---------------------Interested in 1-on-1 Coaching with my team of Metabolism & Hormone Experts? In this encore episode, I'm joined by Christy Harrison, MPH, RD—author of The Wellness Trap and Anti-Diet—to unpack how diet culture infiltrates the wellness world and impacts women's health in ways that are anything but healthy.Christy explains how the multi-billion-dollar wellness industry often sells us pseudoscience, dubious diagnoses, and rigid food rules that can lead to disordered eating and body-image struggles. We have the first episode in a series of thyroid-focused podcasts today. In this episode, Dr. Anshul Gupta, Dr. Carrie Jones, and McCall McPherson join me to explore why Hashimoto's tends to become problematic for women during perimenopause and menopause, and the root causes that drive it. We explore molecular mimicry, the effects of heavy metals, mycotoxins, mold, and other toxins on the thyroid, and explain how perimenopause impacts thyroid gland and immune system function. We cover non-thyroidal illness syndrome and cellular hypothyroidism, also focusing on Graves' disease, which is due to a hyperactive thyroid gland, highlighting the standard of care it receives, preferred medications, and low-dose naltrexone. Dr. Anshul Gupta explains why so many women with thyroid issues are unknowingly living with Hashimoto's. He outlines the five root causes of Hashimoto's, especially during perimenopause and menopause, highlighting the importance of addressing the underlying autoimmune process rather than simply replacing hormones.  Dr. Carrie Jones explains why thyroid dysfunction in women often emerges during major hormonal transitions like perimenopause. She also points out that many cases of hypothyroidism involve issues that we cannot resolve with medication alone. McCall McPherson discusses the challenges of diagnosing and treating Graves' disease, which is often overlooked or mistaken for psychiatric issues. She explains the importance of proper testing, sharing some therapeutic approaches that can help stabilize thyroid function and calm the immune system. Christine Trimpe is a transformative wellness leader dedicated to empowering professional Christian women to reclaim their health and vitality through her faith-based, holistic approach to weight loss. As the founder of The SugarFreed Me Method and a certified SUGAR® Licensed Practitioner, Christine helps clients achieve lifelong healing from sugar addiction and food-related struggles. Her clients experience dramatic transformations, including sustained weight loss and breakthroughs in physical, emotional, and spiritual health, with over 80% retention in her programs.   After a personal 100-pound weight loss journey, Christine created a method that goes beyond conventional dieting, integrating faith, emotional regulation, and metabolic health principles. Christine's own story—marked by a significant health journey, an embrace of ketogenic eating, and a breakthrough lifestyle change—has inspired thousands since she first shared it on her blog and across social media. Her journey has been highlighted in leading wellness magazines, including Woman's World and First for Women, and featured on ⁦DietDoctor.com⁩, where she was named a top low-carb success story.   Key Takeaways Turning Points Matter – A single moment of frustration or physical limitation can become the catalyst for lasting change when paired with ownership and action. Small Steps First – Christine didn't overhaul her diet overnight. She began by removing obvious sources of sugar, then gradually tackled carbs like bread, pasta, and potatoes. Medical Clues Can Guide Recovery – A sleep apnea diagnosis, fatty liver disease, and hormonal concerns pushed her to seek deeper answers. Education Is Empowerment – Learning about insulin resistance through Dr. Jason Fung and others helped her connect the dots between her symptoms, cravings, and food choices. Freedom from Food Obsession – Letting go of “everything in moderation” and embracing abstinence from trigger foods brought lasting relief from cravings and secret eating. Faith as a Foundation – Daily Bible reading and spiritual reflection became Christine's source of true satisfaction and stability, replacing food as a coping mechanism. Social Strategies Work – She uses simple, non-judgmental responses when offered foods she avoids, focuses on connection over food, and prepares mentally before events. Recovery Evolves – What worked at one stage may shift with life changes (e.g., menopause), and flexibility paired with self-awareness supports long-term success. Planting Seeds – Helping others often means sharing your story without expectation, trusting that the right moment for change will come in their own timing.
