Dr. Olivardia is a clinical psychologist, lecturer in the Department of Psychiatry at Harvard Medical School and Clinical Associate at McLean Hospital. He maintains a private practice in Lexington, MA, where he specializes in the treatment of ADHD, executive functioning issues, and issues that face students with learning differences. Trump He was in the private practice of psychiatry and psychoanalysis in Newton, MA for over forty years. A popular teacher and lecturer at Harvard Medical School, particularly on therapeutic process and dreams, Dr. Roth has published widely on clinical topics as well as the Holocaust, romantic love, American short stories, and cinema. He is the author of Psychotherapy: The Art of Wooing Natu Dr. Roth received his B.A. Magna Cum Laude and Phi Beta Kappa from Brooklyn College (1960) and received his M.D. Alpha Omega Alpha from New York University School of Medicine (1965). He was a resident and Chief Resident in Psychiatry at The Massachusetts Mental Health Center, Boston, MA. As a Clinical Associate at NIMH Clinical Center, Bethesda, MD, he was a member of the Family Studies Divisions and chief administrator of the NIMH Adolescent Residential Treatment Center. From 1971–1974 he was Assistant Clinical Director of The Massachusetts Mental Health Center, Harvard Medical School. re. September 28th is International Safe Abortion Day, a global day of action dedicated to raising awareness about the importance of safe and legal abortion care and access to it. In Canada, one in three women will have an abortion in their lifetime, and abortion is one of the most common healthcare needs in our country. While most women in the country support the right to choose and access abortion, there still exist knowledge gaps on the subject. For example, six in every ten Canadian women can't differentiate between medication abortion and the Plan B pill. This data was obtained through a national online Ipsos survey commissioned by Linepharma International to understand the current knowledge of abortion methods, preferences, and availabilities of options in Canada. What is Obsessive Compulsive Disorder (OCD)? Is OCD common with ADHD? What are the types of OCD? Roughly 80% of people with ADHD will be diagnosed with a psychiatric disorder, a comorbidity, at some point in their lifetime. This episode is the first of 2 parts with an expert on both ADHD and OCD, Dr. Roberto Olivardia. Before we go deep and discuss the relationship between OCD and ADHD, let's first know more about OCD and the interplay of symptoms between both. Part 2 will follow next week. Today, I am blessed to have here with me Dr. Steven Gundry. He is a cum laude graduate of Yale University with special honors in Human Biological and Social Evolution. After graduating Alpha Omega Alpha from the Medical College of Georgia School of Medicine, Dr. Steven Gundry completed residencies in General Surgery and Thoracic Surgery at the University of Michigan and served as a Clinical Associate at the National Institutes of Health for years. [00:50] Dr. Gundry Gives His Definition of Ketosis Generating ketone bodies is done by free fatty acids entering the liver from circulation. The liver can convert free fatty acids into ketone bodies. Ketone bodies are short-chain fats that are water-soluble. Ketones have a distinct benefit in getting to the brain, which free fatty acids can't because they're too big to get through the blood-brain barrier. Ketosis should happen every night after about eight hours of not eating if you are metabolically flexible. 50% of ordinary people are not metabolically flexible. [03:50] Uncoupling To Survive: How The Mitochondria Release Pressure Dr. Gundry recommends reading Uncoupling To Survive: https://pubmed.ncbi.nlm.nih.gov/11053672/ When you're starving to death, mitochondria have to save themselves at all costs. If mitochondria die, then it's all over. Making energy is hard work; it's very damaging to mitochondria. Mitochondria have to have ways of protecting themselves, and one of the ways they protect themselves is to release pressure. There are five pressure release valves for the mitochondria. We couple oxygen and protons to make ATP. If we uncouple the burning of oxygen from making ATP, that's how we release pressure. [21:05] The Reason, Most People, Cannot Get Into Ketosis Right Away Most people can not get into ketosis by following a high-fat diet because they lack metabolic flexibility. Many people are insulin resistant. Their cells no longer listen to insulin, so insulin keeps rising and rising. It can take three to four weeks to liberate fat from fat cells on a ketogenic diet because insulin levels stay high for so long. The ketogenic diet came about to treat childhood epilepsy. If you put kids on an MCT oil-based diet, with only about 50% of their calories coming from fat, they would get the same benefit as the full-blown ketogenic diet. [32:25] The “Mito Club” - How Your Body Is Just Like A Nightclub The mito club has one entrance, and there is only one exit. If everything works out okay, a proton and electron will couple and exit the mito club. However, there are a lot more electrons than there are protons. Electrons will also couple with oxygen; it's a bad match. The mito club needs to be cleaned up every night after closing. [38:40] Calorie Restriction Is A Non-Starter, and It Doesn't Work The best way to expand lifespan and healthspan is calorie restriction. However, calorie restriction is a non-starter, and it doesn't work. Calorie-restricted animals are profoundly hungry. A study found that the animals with time-restricted eating lived 11% longer than the animals who ate the same amount of food but ate throughout the day. This concept has been proven in humans as well. [47:05] The Major Health Benefit of Taking MCT Oil or Powder 30% of sheep and goat dairy is MCT. MCTs are an excellent fat that goes directly to our liver, where they are automatically converted into ketones. You can have insulin resistance, take MCTs, and generate ketones. A tablespoon of MCT oil will have you generating adequate amounts of ketones to begin uncoupling mitochondria. MCTs are easy to take; you can mix them in salad dressing. Many females have issues with MCT oils and their stomachs, so start slow. Powered MCTs work well for women. [53:35] You Have Melatonin All Wrong: It May Have Nothing To Do With Sleep Pistachios actually have the highest melatonin content of any food. The Mediterranean diet gets melatonin from olive oil, red wine, and mushrooms. Melatonin isn't there to put you to sleep. Instead, it's there to repair mitochondria. Also, melatonin may be one of the secrets of cancer therapy. Dr. Gundry has cancer patients on up to 100 milligrams a day of melatonin. So many of us know how important foreplay is but yet it doesn't get a lot of attention. Without adequate foreplay, partners can have feelings of disappointment, feel less than, confused, disconnected if sex did not live up to their expectations. Often, partners carry misinterpretations from porn that sex does not require foreplay, clitoral stimulation or consists of quick penetration, and still have the expectation for ecstatic orgasms. When this doesn't happen in our reality, partners often feel deflated. Foreplay is that important. Some therapists say just 15 minutes of foreplay can increase chances of orgasm. Dr. Kate and Raquel talk about specifics to up the exploration of one another's body parts and bring awareness of foreplay to both partners. Today, I am blessed to have here with me Dr. Steven Gundry. He is a cum laude graduate of Yale University with special honors in Human Biological and Social Evolution. After graduating Alpha Omega Alpha from the Medical College of Georgia School of Medicine, Dr. Steven Gundry completed residencies in General Surgery and Thoracic Surgery at the University of Michigan and served as a Clinical Associate at the National Institutes of Health for years. There, he invented devices that reverse the cell death seen in acute heart attacks; variations of these devices subsequently became the Gundry™ Retrograde Cardioplegia Cannula. [00:50] Dr. Gundry Gives His Definition of Ketosis Generating ketone bodies is done by free fatty acids entering the liver from circulation. The liver can convert free fatty acids into ketone bodies. Ketone bodies are short-chain fats that are water-soluble. Ketones have a distinct benefit in getting to the brain, which free fatty acids can't because they're too big to get through the blood-brain barrier. Ketosis should happen every night after about eight hours of not eating if you are metabolically flexible. 50% of ordinary people are not metabolically flexible. [03:50] Uncoupling To Survive: How The Mitochondria Release Pressure Dr. Gundry recommends reading Uncoupling To Survive: https://pubmed.ncbi.nlm.nih.gov/11053672/ When you're starving to death, mitochondria have to save themselves at all costs. If mitochondria die, then it's all over. Making energy is hard work; it's very damaging to mitochondria. Mitochondria have to have ways of protecting themselves, and one of the ways they protect themselves is to release pressure. There are five pressure release valves for the mitochondria. We couple oxygen and protons to make ATP. If we uncouple the burning of oxygen from making ATP, that's how we release pressure. [21:05] The Reason, Most People, Cannot Get Into Ketosis Right Away Most people can not get into ketosis by following a high-fat diet because they lack metabolic flexibility. Many people are insulin resistant. Their cells no longer listen to insulin, so insulin keeps rising and rising. It can take three to four weeks to liberate fat from fat cells on a ketogenic diet because insulin levels stay high for so long. The ketogenic diet came about to treat childhood epilepsy. If you put kids on an MCT oil-based diet, with only about 50% of their calories coming from fat, they would get the same benefit as the full-blown ketogenic diet. [32:25] The “Mito Club” - How Your Body Is Just Like A Nightclub The mito club has one entrance, and there is only one exit. If everything works out okay, a proton and electron will couple and exit the mito club. However, there are a lot more electrons than there are protons. Electrons will also couple with oxygen; it's a bad match. The mito club needs to be cleaned up every night after closing. [38:40] Calorie Restriction Is A Non-Starter, and It Doesn't Work The best way to expand lifespan and healthspan is calorie restriction. However, calorie restriction is a non-starter, and it doesn't work. Calorie-restricted animals are profoundly hungry. A study found that the animals with time-restricted eating lived 11% longer than the animals who ate the same amount of food but ate throughout the day. This concept has been proven in humans as well. [47:05] The Major Health Benefit of Taking MCT Oil or Powder 30% of sheep and goat dairy is MCT. MCTs are an excellent fat that goes directly to our liver, where they are automatically converted into ketones. You can have insulin resistance, take MCTs, and generate ketones. A tablespoon of MCT oil will have you generating adequate amounts of ketones to begin uncoupling mitochondria. MCTs are easy to take; you can mix them in salad dressing. Many females have issues with MCT oils and their stomachs, so start slow. Powered MCTs work well for women. [53:35] You Have Melatonin All Wrong: It May Have Nothing To Do With Sleep Pistachios actually have the highest melatonin content of any food. The Mediterranean diet gets melatonin from olive oil, red wine, and mushrooms. Melatonin isn't there to put you to sleep. Instead, it's there to repair mitochondria. Also, melatonin may be one of the secrets of cancer therapy. Dr. Gundry has cancer patients on up to 100 milligrams a day of melatonin. Concussion is a confusing, somewhat misunderstood diagnosis that impacts many individuals every day. We have an in-depth conversation about Concussion, to discuss what it is, what happens to individuals who have one and what kinds of things help to cure concussion. One intervention that can really make a difference for those who suffer concussion is Vision Therapy, and the growing field of Behavioural Optometry. We discuss how athletes who play sports with high risk of concussion can be assessed by a Behavioural Optometrist to see how well their visual systems and movement systems are coordinated, before an injury happens. This may indicate concussion suseptibility and may help to decrease the impact of a concussion on this individual. BIO: Dr. Meuller is an Optometrist in Vision Development and Rehabilitation. She is the founder of NeuroVision Clinic, which uses neuro-optometric assessment, optometric vision therapy, and NeuroDevelopmental Movement. What you will learn: This week's episode is one you won't want to miss. I have asked Courtney Pullen to join us and work with me and my parents, Marty and Maureen Palumbos – they hesitantly agreed. My parents and I have massaged and updated their estate plan for several years. At a high level the plan is for a portion of their estate to be divided among their four kids, a portion to fund a family foundation and then there is a life insurance trust designed to cover the expenses for Palumbos Family Vacation (PFV) for many years after my parents pass. They have personally funded PFV for over 20 years as a way for the family to be all together for at least one week a year. During this year's family meeting, it became evident that potentially not everyone wanted to continue PFV when my parents are gone – there were over 25 family members in attendance this year. My parents are curious as to how to figure out what they should do and how do they have the conversation with the family. They know that without a conversation PFV won't last. So, what's next? The idea here is to pull back the curtain and give you a glimpse at what good family coaching looks like. I am removing myself from the advisory role and Courtney will be facilitating the entire conversation…we will see what happens. Since I just learned in our 30-minute pre-show call that my mother isn't totally happy with their current estate plan, I'm a bit nervous and excited to see where this goes. The technique starts with focusing on temperature, texture and pressure and uses a tactile sensation to move away from distraction. Linda uses the technique for almost everything ranging from low/ no desire, sexual dysfunction, trauma, body image issues to rekindling connection between couples. One Size Doesn't Fit All With each individual experiencing sensation differently, Linda highlights building up sexual tension with touch. From Linda's experience, there is often a need to manage each level as it is approached. She removes conflict and pressure from couples relationships, by taking on the instruction/ control role. Her sessions help couples identify the difference between vulnerability and rejection. How Do You Start With Sensate Focus? With no hard and fast rule, couples can choose certain factors in their environment to ensure that they are at ease eg clothing (or no clothing). Linda's rules include no kissing and no talking. She points out that talking uses the front of the brain and therefore brings individuals back to logic. Linda mentions that the toucher is supposed to touch for their pleasure while the person being touched, needs to experience the touch and provide feedback if something is not comfortable. Obstacles Linda shares that avoidance is one of the main issues that couples encounter. She reveals how she handles this delay tactic fairly. For clients that don't like the technique, she reminds them about the basic three areas of focus temperature, texture and pressure. Dr. Shalin Shah discusses his perspectives on the patient experience, orthodontic evolution, education, and life philosophies. Dr. Shah maintains two active orthodontic practices in Princeton Junction, NJ and Lawrenceville, PA (Center for Orthodontic Excellence) and was previously serving as a Clinical Associate in the Department of Orthodontics at the University of Pennsylvania for nine years. He received his BA, DMD, Certificate in Orthodontics, and Masters of Science in Oral Biology from the University of Pennsylvania. Dr. Shah is a Diplomate of the American Board of Orthodontics and member of the AAO, ADA, American Cleft Palate and Craniofacial Association, and SCADA among other notable professional organizations. At the University of Pennsylvania, he has lectured on orthodontic records, space analyses, cephalometrics, career development, growth & development, biology of the temporomandibular joint (TMJ), occlusal appliances, practice management, and preventive & interceptive orthodontics as well as various other topics. He also provides didactic and clinical instruction for pre-doctoral students and orthodontic residents. Dr. Shah has received awards for Excellence Clinical Education (2014) and Excellence in Didactic Education (2016) from the University of Pennsylvania. Dr. Shah's research interests primarily include the biological sensitivity of the TMJ. He has conducted novel research on Type III collagen-deficient mice looking at their skeletal development and maintenance, including effects on the TMJ. He is published in various dental and orthodontic peer-reviewed journals. Additionally, he has received several research awards, most notably the 2004 Vernon J. Brightman Research Society Award from the University of Pennsylvania, the 2004 ADA/Dentsply Student Clinician Research Award, the 2006 Quintessence Award for Research Achievement from the University of Pennsylvania, the 2006 Drabkin Memorial Prize for Research in Biochemistry from the University of Pennsylvania, the 2009 Musculoskeletal Disorders Annual Scientific Symposium Poster Award from the University of Pennsylvania, the 2010 Charley Schultz Resident Scholars Award from the American Association of Orthodontists, and the 2010 William S. Kress Award from the Mid-Atlantic Society of Orthodontists. Dr. Shah also maintains a keen interest in the digital workflow, which includes digital scanners, aligner therapy, and 3-D printing. He has comprehensively evaluated orthodontic products and services within these spaces with an intention of improving the user experience while also bringing together current-day CBCT diagnosis to today's aligner treatment. Dr. Wayne Tuckson is a native of Washington, D.C. & a graduate of both Howard University and the Howard University College of Medicine in Washington, D.C. He completed an internship in Obstetrics & Gynecology at the St. Louis University Hospital in St Louis, Missouri, and a residency in General Surgery at Howard University Hospital. He then completed both research and clinical fellowship in colon & rectal surgery in the Department of Colon & Rectal Surgery at The Cleveland Clinic Foundation in Cleveland, Ohio. After a year as a Clinical Associate at The Clinic, he returned to Howard as an assistant professor in the Department of Surgery and as chief of the Division of Colon & Rectal Surgery. In 1994 he left Howard and joined the faculty at the University of Louisville, College of Medicine in the Department of Surgery as an associate professor. He remained at the University for 7 years until July of 2001 when he left and started a private practice. KentuckyOne has employed him as a colon and rectal surgeon since 2008. His clinical interests are in the treatment of fecal incontinence, the prevention & treatment of cancer of the rectum, the management of anal diseases, & the management of postoperative anal pain. To address the apparent disparity in health status in the minority & disadvantaged Kentucky populations, IN 1995, Dr. Tuckson organized The African-American Health Initiative, Inc, (TAAHI). The goal of TAAHI was to improve the “health IQ” or “health literacy” and encourage and foster more responsible participation, by patients, in their own healthcare. TAAHI sponsored conferences on topics such as cancer and healthy eating. To reach a broader audience TAAHI developed the TV show, Kentucky Health (nee Louisville Health). Today, Kentucky Health still airs on the KET network & is hosted & produced by Dr. Tuckson. He has been involved in many community activities including the Air Pollution Control board., Park Duvall Family Health Center board, & the board of the Louisville Metro Department of Health & Wellness. He has given many community presentations to churches, community groups, & local radio & TV news shows. He has received 2 Mayor’s Citations for community service in Louisville & The Thomas S Wallace Jr. Award for “Leadership Role in the Promotion of Health Awareness and the Well-Being of all Citizens of Jefferson County.” Recent recognition includes the “Lyman T. Johnson Distinguished Leadership Award in 2015” from the Louisville Central Community Centers, Inc & a “Real Black Men” certificate of recognition from the Louisville Defender newspaper. My guest on today's podcast and former guest on my show "", Dr. Steven Gundry proposes in his new book "", that the “diseases of aging” we most fear are not simply a function of age; but rather, they are a byproduct of the way we have lived over the decades. In The Longevity Paradox, he maps out a new approach to aging well—one that is based on supporting the health of the “oldest” parts of us: the microorganisms that live within our bodies. He believes that - from diseases like cancer and Alzheimer’s to common ailments like arthritis to our weight and the appearance of our skin, these bugs are in the driver’s seat, controlling our quality of life as we age. Dr. Gundry is a cum laude graduate of Yale University with special honors in Human Biological and Social Evolution. After graduating Alpha Omega Alpha from the Medical College of Georgia School of Medicine, Dr. Gundry completed residencies in General Surgery and Thoracic Surgery at the University of Michigan and served as a Clinical Associate at the National Institutes of Health. There, he invented devices that reverse the cell death seen in acute heart attacks; variations of these devices subsequently became the Gundry™ Retrograde Cardioplegia Cannula. It has become the world’s most widely used device of its kind to protect the heart from damage during open-heart surgery. After completing a fellowship in congenital heart surgery at The Hospital for Sick Children, Great Ormond Street, in London, Dr. Gundry was recruited as Professor and Chairman of Cardiothoracic Surgery at Loma Linda University Medical Center. There, he and his partner, Leonard Bailey, pioneered infant and pediatric heart transplantation. Together, they have performed more such transplants than any other surgeons in the world. During his tenure at Loma Linda, Dr. Gundry pioneered the field of xenotransplantation, the study of how the genes of one species react to the transplanted heart of a foreign species. He was one of the original twenty investigators of the first FDA-approved implantable left ventricular assist device (a kind of artificial heart). Dr. Gundry is also the inventor of the Gundry Ministernomy, the widely used minimally invasive approach to aortic- or mitral-valve repair, the Gundry Lateral Tunnel, a “living” tissue that can rebuild parts of the heart in children with severe congenital heart malformations; and the Skoosh™ venous cannula, the most widely used cannula in minimally invasive heart operations. One of the fathers of robotic surgery, as a consultant to Computer Motion (now ), Dr. Gundry received early FDA approval to use robotic-assisted minimally invasive surgery for coronary artery-bypass and mitral-valve operations. He holds patents on devices for connecting blood vessels and coronary artery bypasses without sutures, as well as for repairing the mitral valve without the need for sutures or a heart-lung machine. He has served on the Board of Directors of the American Society of Artificial Internal Organs (ASIAO), and was a founding board member and treasurer of the International Society of Minimally Invasive Cardiothoracic Surgery (ISMICS). He recently completed two successive elected terms as President of the Board of Directors of the American Heart Association, Desert Division. Dr. Gundry has been elected a Fellow of the American College of Surgeons, the American College of Cardiology, the American Surgical Association, the American Academy of Pediatrics, and the College of Chest Physicians. He is a member of numerous other surgical and medical societies. He is also the author of more than three hundred articles, chapters, and abstracts in peer-reviewed journals on surgical, immunology, genetic, nutrition, and lipid investigations. He has operated in more than thirty countries, including charitable missions to China, India, and Zimbabwe. Inspired by the stunning reversal of coronary artery disease in an “inoperable” patient, using a combination of dietary changes and nutriceutical supplements, in 2001, Dr. Gundry changed the path of his career. An obese, chronic “diet” failure himself, he adapted his undergraduate Yale University thesis to design a diet based on evolutionary genetic coding, which enabled him to reverse his own medical problems. In the process, he effortlessly lost 70 pounds. The equally astonishing results from following what he came to call Diet Evolution in several of his staff led Dr. Gundry to accept a position in Palm Springs where he could devote his efforts to disease reversal. Dr Schimpff is the retired Chief Executive Officer of the University of Maryland Medical Center. Dr. Schimpff, an Eagle Scout, is a 1963 graduate of Rutgers University where he was a Henry Rutgers Scholar and recipient of the Van Der Poole Award for Excellence in Medicinal Chemistry. He obtained his M.D. degree in 1967 at Yale University Medical School and was inducted into Alpha Omega Alpha, the national medical honor society. His internal medicine internship and residency was at Yale-New Haven Hospital. He was selected as a Clinical Associate in the Baltimore Cancer Research Center (BCRC) of the National Cancer Institute from 1969-1972. He was a Fellow in the National Institute of Allergy and Infectious Diseases, NIH, from 1972-1973. He is Board certified in Internal Medicine, Medical Oncology and Infectious Diseases.
