Notes:  Why is pregnancy considered a “physiologic stress test,” and why does risk extend beyond delivery? Blood volume, heart rate, and cardiac output rise while systemic vascular resistance falls, peaking in the late second/early third trimester; underlying (even undiagnosed) heart disease can be unmasked or worsened. Postpartum (“fourth trimester”) is a high-risk period, not a safe zone – fluid shifts, rising SVR, and bleeding risk can precipitate decompensation in patients with heart failure, pulmonary hypertension, valvular disease, or aortopathy. Adverse pregnancy outcomes (hypertensive disorders, gestational diabetes, preterm birth, fetal growth restriction, peripartum cardiomyopathy) are markers of future cardiovascular risk and warrant long-term preventive follow-up. What is the practical framework for approaching pre-pregnancy cardiovascular risk? Four broad categories: (1) patients who may need cardiac screening before pregnancy, (2) patients needing risk-factor/medication optimization, (3) known cardiovascular disease where pregnancy is reasonable with structured risk stratification, and (4) high-risk disease where pregnancy may need to be delayed, modified by intervention, or discouraged. Testing should be targeted, not blanket – reserved for symptoms, abnormal exam, concerning family history, or reduced functional capacity. How is risk stratified in patients with known cardiovascular disease? Use a combination of tools per 2025 ESC guidelines: mWHO 2.0 (broad maternal risk category), CARPREG II (additional predictors of maternal cardiac events), and ZAHARA (useful in congenital heart disease). Key lesion-specific factors: aortic size/growth, valve severity, ventricular function, symptoms, blood pressure, and family history of dissection. Translate risk into practical terms for patients rather than leading with a numerical score. Which cardiovascular medications require review before conception? ACE inhibitors, ARBs, and ARNIs should be transitioned off before pregnancy; statins, MRAs, and SGLT2 inhibitors also need review. DOACs are contraindicated in pregnancy and lactation; mechanical valve anticoagulation requires individualized shared decision-making, as no strategy is risk-free for mother and fetus. Medication changes are best made proactively, before conception, rather than reactively. This is not an exhaustive list! The medication list needs to be reviewed carefully. Which conditions carry high or prohibitive risk in pregnancy? Pulmonary arterial hypertension, Eisenmenger syndrome, severe ventricular dysfunction, prior peripartum cardiomyopathy with residual LV dysfunction, severe left-sided obstructive valve disease (e.g., severe mitral stenosis), mechanical valves, significant aortopathy, cyanotic congenital heart disease, and Fontan physiology. Common theme: limited cardiovascular reserve and high risk of decompensation, thrombosis, arrhythmia, heart failure, aortic dissection, or death. These patients need expert multidisciplinary evaluation before pregnancy. Severe mitral stenosis is poorly tolerated because tachycardia shortens diastolic filling time and raises left atrial pressure, risking pulmonary edema and decompensation. When should genetic testing or counseling be offered? Consider when a diagnosis may be inherited or affect the patient, pregnancy, or family members: inherited cardiomyopathies, aortopathies, channelopathies, select congenital heart disease, and some pulmonary hypertension syndromes. Recurrence risk of congenital heart disease in offspring is roughly 6-10% when the mother has CHD; fetal echocardiography should be offered. How should contraception be approached in high-risk cardiac patients? Frame contraception as part of the cardiac care and reproductive safety plan to prevent unplanned high-risk pregnancy. Long-acting reversible contraception is often preferred; progestin-only methods are generally safer than estrogen-containing options with thrombosis risk, pulmonary hypertension, or mechanical valves. What are key delivery-planning considerations for cardiac patients? Vaginal delivery is preferred unless there is an obstetric indication for cesarean or a specific cardiac reason (e.g., unstable maternal status, therapeutic INR) to avoid labor. Planning should address delivery location, anesthesia involvement, telemetry needs, fluid management, and postpartum monitoring, clearly communicated across the multidisciplinary team in advance. How should clinicians counsel patients when pregnancy is discouraged but strongly desired? Acknowledge the patient’s goals and the emotional weight of the conversation; separate the goal (family building) from the timeline (safety now vs. after optimization). If pregnancy remains prohibitively risky, discuss alternatives for family building and ensure adequate patient support. What are the key gaps and future directions in cardio-obstetric risk stratification? Current risk tools (mWHO, CARPREG II, ZAHARA) provide common language but do not fully capture functional status, prior pregnancy history, or how risk evolves over time. Future direction: individualized, dynamic risk prediction incorporating imaging, biomarkers, exercise capacity, and social drivers of health, with better long-term links between pregnancy complications and cardiovascular prevention. Friends of the Rosary,Today, August 11, we celebrate the Memorial of St. Clare of Assisi (1194-1253), a virgin and a most perfect follower of St. Francis of Assisi, who practiced a life of entire poverty.At the wish of St Francis, she established a community of nuns in the small convent of San Damiano — the Order of Poor Clares — and acted as their superior for forty-two years — her own life serving as a constant sermon for her sisters to emulate.Their Rule included austerities, walking barefoot, sleeping on the ground, keeping perpetual abstinence, and making poverty the basis of their lives. St. Clare died on August 11, 1253, and was canonized two years later.The Breviary says of her:"When the Saracens were besieging Assisi and were preparing to attack the convent, St. Clare asked to be assisted as far as the entrance, for she was ill. In her hand she carried a vessel containing the blessed Eucharist as she prayed: O Lord, do not deliver over to beasts the souls that praise You! (Ps. 73). Protect Your servants, for You have redeemed them by Your precious Blood. And in the midst of that prayer, a voice was heard, saying: Always will I protect you! The Saracens took to flight."Heroic in suffering (she was sick for twenty-seven years), she was canonized only two years after her death.In our twentieth century, there still remain large areas with millions suffering under extreme poverty. Poverty in itself is no virtue, but it should be made into a virtue. Christ was poor. His entrance into the world and His departure from it took place in circumstances of greatest need. He had no house wherein to be born, no crib; no house wherein to die, no deathbed. Poverty stood watch at birth and remained to see His death. "The foxes have dens, and the birds of the air have nests; but the Son of Man has nowhere to lay His head" (Matt. 3:20).At least, we can be moderate and frugal, and thereby find the way to the spirit of Christian poverty. The murder trial of Lindsay Clancy, the Duxbury, Massachusetts mother accused of strangling her three children — 5-year-old Cora, 3-year-old Dawson, and 7-month-old Callan — with exercise resistance bands inside the family's Summer Street home on January 24, 2023, moved from courtroom procedure to hard evidence in one sitting. Clancy faces three counts of murder and three counts of strangulation, has pleaded not guilty, and is mounting an insanity defense through attorney Kevin Reddington, built on claims of severe postpartum psychosis and psychiatric overmedication.Plymouth Superior Court Judge William F. Sullivan read stipulations already agreed to by both prosecution and defense — meaning no one's contesting them. The exercise bands recovered from the scene carry DNA from Lindsay Clancy, Patrick Clancy, and the children on their ends. Blood recovered from the basement has been confirmed to belong to Cora. Stipulated facts, not theories.Former Massachusetts State Police criminalist Katarina Stashyn followed with a breakdown of how DNA testing works and what it actually revealed from the samples collected in this case — testimony meant to make sure jurors understand the science behind the stipulation, not just the conclusion.The day closed with something less clinical. Elaine Rossi, who worked as the Clancy family's nanny, testified about her time in the household, including conversations where Lindsay told her she was dealing with postpartum depression. It's the kind of detail the defense will lean on hard in the weeks ahead.Prosecutors maintain Clancy understood what she was doing regardless of her mental state. After the killings, she allegedly attempted suicide by jumping from a second-story window and is now paralyzed from the waist down.     What does it take to keep wounded Soldiers alive when the next war means hours — not minutes — to the operating table? In this special collaboration between WarDocs and OP MED TV, recorded at the Defense Strategies Institute OP MED Symposium, COL Shaun Brown, MD, Commander of the U.S. Army Institute of Surgical Research, lays out the research agenda that will decide whether the wounded of the next conflict survive. COL Brown's path into Army medicine began with a rejection. Poor eyesight closed the door on the U.S. Naval Academy, so he pursued pre-med as a civilian undergraduate. When 9/11 happened, it solidified both his commitment to medicine and his decision to serve, and he took an Army HPSP scholarship in medical school. He trained in general surgery at William Beaumont Army Medical Center — a program with a long, quiet relationship with the special operations community — where attendings would vanish overnight for operational requirements. He chose colorectal surgery as a fellowship to add technical range he could use in civilian practice and on the battlefield, then joined Joint Special Operations Command at Fort Bragg after a year on staff in El Paso. Now commanding the Army's premier combat casualty care research enterprise, COL Brown is candid about what excites him and what worries him. He is most energized by the Organ Support and Automated Technology department, and he uses a widely shared video of a Ukrainian amputee evacuated by unmanned ground system to make his point: autonomous evacuation without autonomous medical support only moves the walking wounded. Ventilators that read changing physiology and adjust themselves, autonomous IV pumps, and en-route support are the missing half of the capability. His concern is combat wounds. In large-scale combat operations with prolonged evacuation timelines, Dr. Brown expects most damage control surgery to be done for sepsis rather than hemorrhage — the patients who cannot be evacuated become septic extremities. He also walks through the blood problem: whole blood is the standard, low-titer O supply will not be sufficient at scale, and the answer is shelf-stable components, including freeze-dried and spray-dried plasma, freeze-dried platelets, and freeze-dried red cells. The conversation closes on people. He then names his chief concern: combat wound research funding, and his expectation that in large-scale combat operations most damage control surgery will be done for sepsis rather than hemorrhage. (08:06-11:59) Blood, Plasma, and the Shelf-Stable Future of Resuscitation The discussion turns to the evolution from component therapy to 1:1:1 ratios to whole blood, and Dr. Brown's assessment that low-titer O will not be available in sufficient quantity for large-scale combat operations. He details work on freeze-dried and spray-dried plasma, freeze-dried platelets, and freeze-dried red cells, noting that spray-drying is faster, cheaper, and uses equipment roughly the size of two ATMs — a major advantage for distributed manufacturing. (11:59-18:16) Burn Care, Surgical Readiness, and Partnerships Across Components COL Brown addresses the burn casualty problem in a future fight: forward Class VIII resuscitation supply, scaling the Advanced Burn Life Support course for deploying units, and partnerships with civilian burn centers, including placing Army burn surgeons in MILCIV sites. He then lays out the diversified surgical platform — military treatment facilities, community hospitals, and underused VA partnerships — and how he works with the Reserve consultant to pair complementary skill sets on deploying units. (18:16-22:22) Forward Surgical Capability Gaps and Why He Still Serves Asked what a forward surgical team still needs, COL Brown points to an off-the-shelf, infection-resistant biologic vascular conduit as a potential game changer over shunts — with the training investment that would require. He closes with his why, quoting the Army War College maxim that you train for the known and educate for the unknown, and asking who will be left to educate the next generation if experienced leaders walk away during the interwar period. Take Home Messages Autonomous evacuation without autonomous care only moves the walking wounded: Unmanned ground and air systems can pull a casualty off the battlefield, but a platform alone does not sustain a patient who needs a ventilator, a pump, or a transfusion en route. The medical community must be in the ground-maneuver conversation early, because a small design change can turn a logistics platform into a casualty evacuation platform. Autonomous ventilators that read changing physiology and adjust themselves are the missing half of that capability. In the next war, sepsis may drive damage control surgery more than hemorrhage: Prolonged evacuation timelines change the casualty population that reaches a surgeon. Patients in uncontrolled hemorrhage far from a surgical team frequently do not survive the wait, while patients with survivable wounds that cannot be evacuated arrive septic days later. Combat wound research and combat wound solutions deserve renewed funding priority for large-scale combat operations. Shelf-stable blood components are the answer to a cold chain that will not hold: Warm whole blood remains the standard of care, but low-titer O will not be available in the quantities a large-scale conflict demands, and cold chain storage is a logistical vulnerability. Freeze-dried and spray-dried plasma, freeze-dried platelets, and freeze-dried red cells are all in the research pipeline. Spray-drying offers a particular advantage: it is faster, cheaper, and the equipment footprint is small enough to support distributed manufacturing forward. Burn readiness is a supply problem, a training problem, and a partnership problem: Thermal weapons, lasers, fuel, and explosions could produce burn casualty volumes the system has not seen in decades. Resuscitation depends on adequate crystalloid and plasma forward, on teams trained to calculate burn surface area correctly, and on scaling the Advanced Burn Life Support course to deploying units. Long-term capacity also depends on formal relationships with civilian burn centers and on placing military burn surgeons inside those partnerships. Surgical readiness comes from a diversified platform, not from one hospital: Military treatment facility volume alone will not sustain a surgeon's skills, so readiness now depends on layering community hospital partnerships and Veterans Affairs relationships on top of the military caseload. Functional VA hospitals near large installations without strong academic affiliations are ripe for preferred referral partnerships. What surgeons need most is not trauma volume but complexity, which older patients with more complex medical conditions reliably provide. In this episode, Jim recounts the unlikely story of transforming a flawed cancer test into a new way of measuring lipoproteins, explains what standard cholesterol tests can miss and why LDL-P and apoB can inform treatment decisions beyond LDL cholesterol alone, and dispels the misconception that large, "fluffy" LDL particles are benign.
