Synopsis: Nima Farzan is the CEO of Kinnate Biopharma, a clinical-stage precision oncology company focused on expanding on the promise of targeted therapies for those battling cancer. The company is developing medicines for known oncogenic drivers where there are no approved targeted drugs. Having started his biotech career in the late 90s, Nima discusses how his vision of leadership has evolved over the years. He talks about joining Kinnate in March of 2020 and the challenges of running the company and growing the team during a pandemic. He dives into the evolution of genomics over the last several years and what has surprised and impressed him about the genomic ecosystem as a whole. He discusses where Kinnate is from a development perspective and how he thinks about indication selection. Finally, he talks about how being an immigrant has shaped his approach to team building, culture and leadership style. Biography: Nima brings over two decades of executive leadership experience in the biopharma industry to our team. Prior to joining us, Nima spent seven years at PaxVax, where he was most recently President and CEO. While there, he successfully led a team of 250 people through a number of commercial product launches and negotiated the sale of PaxVax to Emergent BioSolutions in 2018. Prior to PaxVax, he was at Novartis AG in a series of roles of increasing responsibility, most recently as Vice President of US Marketing at Novartis' Vaccines and Diagnostics division. Prior to Novartis, Nima worked at DoubleTwist, a pioneering genomics company, and was a consultant with Boston Consulting Group. He was a member of the founding Board of the Coalition for Epidemic Preparedness Innovations (CEPI). He is on the Executive Committee of the Northern California Chapter of Young President's Organization (YPO) and a Trustee at the Hamlin School. He is also a Director of Keros Therapeutics (NASDAQ: KROS). He holds a BA in Human Biology from Stanford University and an MBA from the Harvard Business School.

Competition makes you better. [The school system would rather educate than inspire kids] because you don’t need to do that if you’re trying to churn out a consistent product. [...] They’re trying to churn out a consistent product so that they don’t lose funding. Schools are playing defense. The fact that you didn’t pay attention in school doesn’t mean you have ADD. It just means that you can’t sit there with everybody else who is literally average, by definition, and memorize stuff. [...] Many people who think they have ADD probably don’t. They were just cooped up in a bullshit environment for a really long time and that’s not how they learn. No job is like school. Zero jobs involve sitting there, being academic, and memorizing things while a teacher lectures. Zero jobs do this. Listening to lectures is one of the worst ways I can think of to deliver information to young people with active minds.

She served on Kaiser Permanente's High Performance Building Committee to develop national sustainable design standards, led a team in the development of a USGBC Volume Build Pilot Program for Wells Fargo Bank in 39 states and was a green consultant with the University of New Mexico's new Children's Hospital and Critical Care Pavilion. She led sustainable research in the textile industry for Kaiser Permanente and co-authored a white paper on the research, titled, Future of Fabric: Healthcare. The Interview: Cyrus Farivar on Xtracycle Touring with Kids Cyrus [“suh-ROOS”] is an investigative tech reporter at NBC News in San Francisco. In addition to being a radio producer and author, Cyrus was most recently a senior tech policy reporter at Ars Technica. He was also previously the sci-tech editor and host of “Spectrum” at Deutsche Welle English, Germany’s international broadcaster, from 2010-2012. Cyrus is the author of multiple books, including Habeas Data (2018) and The Internet of Elsewhere (2011). Praised by The New Yorker, among others, Habeas Data takes a look at legal cases that have had an outsized impact on surveillance law in America. His first book focuses on the history and effects of the Internet on different countries around the world. In 2019, Cyrus and Olivia Solon won the Technology Reporting award from the Society of Professional Journalists, Northern California Chapter for their coverage of Silicon Valley, including reporting on privacy, facial recognition, and more. Two years earlier, Cyrus and Joe Mullin also won the Technology Reporting award from SPJ NorCal for their August 2016 story: “Stealing bitcoins with badges: How Silk Road’s dirty cops got caught.” Cyrus has also reported for the Canadian Broadcasting Corporation, National Public Radio, Public Radio International, The Economist, Wired, The New York Times, Slate, and many others. She previewed her talk with a custom video addressed to the chapter: Talkin' 'bout my generation For those Baby Boomers who didn't die before they got old--and learned the value of trusting anyone over 30 a few decades back--Anna's talk had particular poignancy. She explained how the recent OK Boomer meme is the equivalent of the advice not to trust anyone over 30. What goes around comes around. Here's why... Generational Codes Anna Liotta has studied generational dynamics for over 25 years. Indeed, she wrote the book on generational CODES™. What's more, she’s lived the research. As one of 19 (!) children, her entire life has been a Ph.D. in generational dynamics. Her presentation addressed the question: What makes this age-old conflict of generational collisions and biases so important to us in business today? Her concept of generational codes helps explain: What defines each generation, including pivotal events and experiences that shaped it. The truths and lies behind generational stereotypes. How various generations define their work ethic. How technology can bridge or break down generational communication. The secrets of selling products and services to different generations. What you need to know as a manager to find and retain new talent. Anna demonstrated how, as speakers, we should develop savvy messages that appeal to the different generations. This task is often confusing, as each generation has unique needs and motivators. Each brings its own set of attitudes, values, and beliefs to the workplace, and the way they do business. They make choices of who to buy from and who to work for, based on these values and beliefs. Understanding what shapes and forms each generation is vital. Interestingly, each generation is sure that their values, attitudes, and beliefs are the right ones. Her insight is that each generation is significantly influenced by what was happening in the world around them during their formative years. The ages of eight to 18 are when each generation is making decisions about how the world works and what’s possible. The events, icons, and leaders they see, experience, adore, and dislike are shaping their world. These influences set the paradigm for decision making, purchasing choices, and job selection for years to come. Anyone doubting this can see the trajectory of individual lives play out in director Michael Apted’s films about a cohort of British Boomer children, the most recent of which--63 Up--opened in the US last month. OK, Boomer! My own rather self-satisfied response to the generational divide was to tweak the noses of the younger members of the audience who were strangely absent from the social media channel the chapter had promoted. Dr. Robert Schmieder, KK6EK Bob is the founder and Expedition Leader of Cordell Expeditions, a nonprofit research group begun in 1977. Decision Fatigue and Strengthening your Mental Muscle

This is Scott Selinger and welcome to another fantastic jabbering edition of - ABC’s for ECP’s, the podcast on behalf of the Northern California’s chapter of the American College of Physicians Council of Early Career Physicians. To those most loyal of followers, you’ll notice it’s been a number of months since there’s been any movement with this podcast due to a combination of work, extracurriculars, and a new baby, but I am making an early new years resolution to more regularly get these up, up and away.  Let’s put a pin in the idea of new years resolutions too because I want to come back to that.There was a recent research letter in JAMA that set off a few ideas downstream for me.  The letter was dubbed “Time of Day and the Decision to Prescribe Antibiotics.”  Now just reading that title, my reaction and that of most people I’ve talked to about this was “well of course antibiotic prescribing goes up as the day wears on.”To summarize a little bit, the study looked at outpatient encounters for a number of upper respiratory infections through coding review, some of which are cases where antibiotics are sometimes indicated (i.e. sinusitis, otitis, strep pharyngitis) and others where they are never indicated (i.e. acute bronchitis, non-strep pharyngitis) in patients with NO comorbidities or other indications to receive antibiotics.  There were a number of more subtle findings but the answer did end up looking like yes, the amount of antibiotics prescribed, whether for conditions indicated or not, did increase as both the morning and afternoon clinic shifts wore on.  This sounds like an expected result, but why?  The study talked about the idea of decision fatigue, meaning the more decisions you have to make, the more you start looking for the easiest solutions.  Think about when you get a survey to fill out - do you spend as much time and thoughtfulness on the first answer as you do the tenth?  For most of us, no, we don’t.  And the translation for this is the otherwise healthy, mildly sick patient coming in asking for a z-pack for their cold.  If it’s 9am and you just finished your coffee, you have a lot more energy in you to talk with them about the lack of efficacy and risk of side effects than you do when it’s 5:30, you’re worrying about traffic on the ride home, you have some loose ends to follow up on from the rest of the day, and you’re just physically and mentally tired.  And those are the people more likely to get inappropriate antibiotics.  This letter pointed to an idea from The Psychology Bulletin comparing the mind’s self-regulation and self-control to a muscle - the more you use it, the more fatigued it gets.  This idea that you have a set amount of self-control to use throughout the day is certainly something to think about.  When you’re running a marathon, you make sure to take in some water at set intervals and sometimes slow down and walk so you can make it to the finish line.  But most of us don’t necessarily get to have those breaks when we need them - usually that’s the busiest part of our day.  And I don’t think this is necessarily something limited to the outpatient world.  Admissions also seem to go up as the day wears on in most places along with more decisions to make about workup and treatment (addendum: I was just guessing at this at first but it was hard to find data to back this - a VA study showed about 51% of patients visiting between 5 pm and 1 am were admitted, but also said this was contrary to national findings. The only real data I could find from the CDC (see below) showed an uptick in the afternoons but pretty steady admissions from about 10 am to 10 pm).  At the end of a shift, are you more likely to order a CT scan than at the start of the day?  What about admitting someone who could potentially be sent home?  What about dealing with calls from the wards?  Looping back around to New Years resolutions, is this the same reason why most people fall off their wagon as the days and months drag on?  Do we just get tired of making the decisions we know we should?  This might be a reason why things like meal planning for weight loss are always recommended - it focuses most of the decision making at the beginning of the week so that when you get to Thursday night, you know you’re having brown rice and salmon with broccoli and not a Double-Double with fries from In-N-Out on the drive home.So how is all this helpful in our daily practice?  With all the focus on mindfulness - mindful meditation, mindful eating, etc. - I think it’s a reminder of an area to focus on for mindful practice with the idea of going into your day knowing you’re going to be tired and trying to factor in a mental break towards the end of your shift, be it something like an actual snack break or a short meditative session (for fun tips about how to do this, check out the earlier podcast in the series, “Just Breathe”).  And maybe a way to help us stick to our new years resolutions as well and help our patients with theirs too.As always, I’d love to hear your feedback on this, so if you have any burning questions or comments,  you can post them on the blog at canocecp.blogspot.com, our podcast page on itunes, our facebook page or email them to canocecp@gmail.com.  And if you have time, be sure to head on over and check out our brand new official page for the Northern California Chapter of the ACP Council of Early Career Physicians so you can find out more about the events going on in the chapter.  Stay safe and have a happy 2015!

This is Scott Selinger and welcome to another edge-of-your-seat-amazing talking blogpost - ABC’s for ECP’s, the podcast on behalf of the Northern California’s chapter of the American College of Physicians Council of Early Career Physicians.  Today I’d like to talk a little bit about this new world of patient satisfaction scores.I really like my new car.  I got it a few months ago and it’s been working great.  But then a few weeks ago I folded the back seat down and now it won’t come back up.  So like most men, I hit it a few times and then decided to take it to the dealership.  It was a pretty easy, seamless process of getting in and out but there were a few snags.  For instance, they told me I needed a new part and they’d call when they got it in - but that didn’t happen and when I called 2 weeks later I found out that the part I needed had been there for 10 days.  And then there was the little issue of the fact that when I got the car home, the seat was still broken.  I brought it back, saw a different service advisor, who apologized profusely, and they got it fixed the same day.  The kicker was, every time I was there, I was reminded before I left that “by the way you might be getting a survey …” at which point I cut them off and just gave them a thumbs up and said “all 5’s guys!” because I really wasn’t interested in hearing the spiel – the surveys they send out are based on top box scoring, so if they get anything except a 5 out of 5, they get dinged, even if they had nothing to do with the dissatisfying issue.This is getting to my main point about patient satisfaction surveys and its effect on us as physicians.  This is something I slowly started hearing more about towards the end of residency, but at the time the satisfaction scores at the resident clinic were abysmal and we felt sort of resigned to that because a lot of the dissatisfying factors were systems issues beyond our control.  That’s if you can imagine patients being dissatisfied by frequently seeing random doctors, long wait times, bedside manners that were still under construction, and frequently being told that they didn’t need antibiotics for their cold or opioid pain medications for their headache. But now that I’m out practicing, I’m realizing how huge an issue this is.  I’m still trying to figure out when someone needs steroids and when they need antibiotics or the best way to convince them they need to quit smoking or lose weight and honestly, sometimes this weighs on my mind especially when I have someone demanding something I know is unreasonable.  With all the money that is tied to patient satisfaction scores between Medicare reimbursements based on it as well as organizations and practitioners trying to maintain patient loyalty and the insurance money that comes along with it, it’s no wonder it frequently feels like our profession is starting to more resemble that of the service industry, but saddled with the complexities of human health.  To tie back to the car problem I had, a colleague of a colleague now routinely, in her follow up emails to her patients, has a little tagline at the bottom talking about what they should do if they get a survey!  Patient satisfaction scores are now commonly being tied to physician pay and advancement or retention at their current job, the theory being to incentivize us to make that extra effort to make sure our patients are satisfied by their medical care.And in theory this sounds great because why shouldn’t patients have an exceptional experience every time?  Why should they expect any different of us than they do of their mechanics (and I don’t mean to pick on mechanics - I’m just still a little miffed at mine).  Well, there may be a few reasons, just because our profession is a little different than many others in the service industry.A study[1] just published a few weeks ago in the Journal of Patient Preferences and Adherence (which I, like I’m assuming many of you, didn’t realize existed until today) surveyed about 4000 doctors in a state medical society about exactly this which came back with 155 responses.  Granted, that’s a small number, but there are some disturbing statistics from it.  First, 78% of respondents said that these patient satisfaction surveys affected their job satisfaction moderately to severely, with 28% of people saying they had thought about leaving the medical profession because of them.  Second, just over half of respondents said that they had inappropriately prescribed an antibiotic or narcotic or ordered an unnecessary lab test because of the patient satisfaction surveys.Finally, while there were 6 neutral or positive comments given on patient satisfaction rankings, there were 47 negative comments and I do appreciate the fact that they published them.  They are actually divided up into 5 themes including the two I just mentioned as well as:- that they’re a poor way to evaluate the quality of medical care,- that there’s too much weight on them and not enough administrative effort to improve medical outcomes- that they’re perceived as a conflict of interest        So that’s one side of the coin, and it is a very striking viewpoint from the healthcare providers, but that study of course needs to be taken with a grain of salt as it is a small number and certainly could select out for the most dissatisfied people to respond.        But what about from the patient side?  Is there validity to these viewpoints?  Well there was another larger study about two years ago[2] published in JAMA that looked at patient satisfaction surveys from 52,000 respondents over 7 years.  While high patient satisfaction was associated with less frequent ER visits, it was also associated with greater inpatient use, higher overall healthcare and prescription drug expenditures, and increased mortality.  There was commentary both ways when this article came out and to date as best I could see, there hasn’t been another large study published regarding this, but if you could tie it to the study looking at the physicians perspectives on these surveys, it’s not hard to believe this data might be real.        So what are we supposed to do with all this information?  There’s been so much change in what the role of a physician is supposed to be.  We’ve gone from a paternalistic approach more to one of shared decision making.  But is the heavy weight of patient satisfaction surveys unbalancing the power of shared decision making?  And aren’t we now being asked to be more paternalistic to society by reducing health care expenses, antibiotic and opioid prescriptions, and be more dogged about preventative cancer screenings and weight loss?        I think the missing part of the equation is time and communication and we definitely do need to be able to clearly communicate our thoughts with patients.  So far, I’ve seen great feedback from people when I’ve actually stopped and instead of auto-refilling medications I don’t think are appropriate or ordering easy labs I know aren’t indicated, I explain to them why doing so would give me that gnawing pit in my stomach that knows when I do something wrong.  We’re all so rushed these days, I think this gets lost and I’ve certainly been guilty of it on occasion, but this has to be a point where we hold the line.  This is why most of us went into medicine in the first place - to help people and to be educators, not to be vending machines.          I haven’t gotten a call for a survey for the car dealership yet.  I’m conflicted though and I actually feel differently now than when I started writing this.  If I give my honest opinion, which was my emotional urge and first reaction, it’s going to bring down someone’s score, it might get him disciplined or he might take a salary cut - it could contribute to his getting fired.  And the errors made were system errors too - probably nothing to do with the guy I dealt with.  Giving anything but 5’s would just seem spiteful.  I guess I’ll wait and see …I know I covered a lot of ground here and I’m hoping you have a few opinions to share (and at this point I feel like I know who all of “you” are as I haven’t seen too many downloads yet) so as always, we’d love to hear your feedback on this, so if you have any burning questions or comments,  you can post them on the blog at canocecp.blogspot.com, our podcast page on itunes, our facebook page or email them to canocecp@gmail.com.  And if you have time, be sure to head on over to our facebook page for the Northern California Chapter of the ACP Council of Early Career Physicians so you can find out more about the events going on in the chapter. Just Breathe

This is Scott Selinger and welcome to another scintillating episode (webisode? pod? I’m not totally sure what to call these serial casts) any way, it’s another ABC’s for ECP’s, the podcast on behalf of the Northern California’s chapter of the American College of Physicians Council of Early Career Physicians.  As I’ve got a wedding to head to in beautiful scenic St. Louis this weekend flanked by a trip to the national ACP meeting in Orlando next week, I’ll make this a  brief one.  We’ve all heard about mindfulness and mindful meditation, and it sounds great in theory, but how do we bring it into our daily practice?  Is there actually a way in our hectic shift schedules to find time for a calming experience?  I think so, and I’ve been starting to do it.Deep breathing exercises are a great relaxation technique.  The NIH’s National Center for Complementary and Alternative Medicine has noted research supporting the power of relaxation techniques on improving control of numerous medical and psychiatric conditions, or to put it a way we more commonly hear it, “Side effects of relaxation techniques may include improved control of anxiety, depression, asthma, fibromyalgia, headaches, chronic pain, angina, hypertension, insomnia, IBS, nausea, TMJ syndrome, tinnitus, and overactive bladder.”  Sounds pretty good right?  If only we had a time when we could incorporate it into our daily practice …But fear not!  There is a way!  The next time you’re getting ready to perform an auscultation as part of your respiratory exam, give this a try: put your hand on your patient’s shoulder, your stethoscope on their chest, ask them to take some deep slow breaths through their mouths, and then breathe along with them.  This can help you out in a few ways:1)  Firstly, if you’re breathing along with them, you’ll be able to make sure they aren’t hyperventilating to the point they might pass out by the end of the exam (as I’ve seen a few overeager patients do)2)  Second, as mentioned above, for the patient with any of the conditions mentioned above, the act of deep breathing may help put them more at ease in their interaction with you.  They may feel less anxious, their pain may be a little diminished, their blood pressure may go down a little bit and if they do get any of these benefits, you will have just illustrated a safe, effective, free, non-pharmacologic treatment that they can take home with them3) Finally, looking at your own wellness and all the stress and external stimuli we’re bombarded with in our daily practice, it can help give YOU a moment of calmness and clarity, probably making you spend a few extra seconds on auscultation, and let you both come out on the other side of the exam feeling a little more at ease and connected as you get ready to discuss the assessment and plan.Personally I can say that I’ve been trying this for the last few months and in the midst of seeing 11 or 12 patients every 4 hours, it has been a little oasis of respite so I definitely encourage you to give it a shot with your next patient.            Additionally, I want to give another little plug for mini meditation sessions to make your day easier.  Search for “how to meditate in a minute” and you’ll find a nice, short, animated cartoon that will talk about the benefits of meditation and show you how quickly you can see them if you can find just a minute in your busy day.  It’s a cartoon and it’s about 5 minutes long and I think it’s something that can be beneficial for us and our patients in a ton of different situations.            As always, we’d love to hear your feedback on this, so if you have any burning questions or comments, you can post them on our facebook page or email them to canocecp@gmail.com.  And if you have time, be sure to head on over to our facebook page for the Northern California Chapter of the ACP Council of Early Career Physicians so you can find out more about the events going on in the chapter.

"Introductions" or "Why didn't they teach me how to say hello"

This is Scott Selinger and welcome to the first podcast on behalf of the Northern California’s chapter of the American College of Physicians Council of Early Career Physicians.  Given that this is our inaugural podcast, I thought it’d be a good time to talk about introductions - how you enter into that first meeting with a new patient and how you handle the name game.Before I’ve done pretty much anything new in life, be it starting 6th grade, a new job, interviews, whatever - I’ve gotten a call from my dad where he just says: “remember first impressions.”  While this started out as rhetoric I would roll my eyes at, it’s become very important in my role as a physician, as I’m sure it has for every doctor out there.  That moment where you meet a patient for the first time is paramount in establishing trust and setting the tone of the rest of your relationship with them.One thing that I’ve found is more important now, especially given how busy we all are in cold and flu season, is taking a moment to ready yourself before meeting that new patient.  We have so many other things are mind is on during the day - returning that page or email or message, following up on a lab, getting out on time - that it’s easy to let that ruin your first impression, your tone of voice, and your body language.  It’s crucial that your new patient knows that the only thing you’re thinking about while you’re talking to them is them.  Of course that can’t always be the case, but more and more I’ve found how helpful it can be before opening that door or pulling back that curtain, to stop, take a deep breath, put on a little smile, and focus on forging a great new doctor-patient relationship.But then how do we introduce ourselves?  It’s probably a lot easier in the outpatient setting because by the time someone arrives in your office, they usually know who you are (and in many cases may have read a little blurb about you as well).  In the hospital, it’s a much more difficult setting - the new patient may not know why they are there (or even who they are), they may have already seen numerous other doctors and healthcare workers and have lost track of names, and they usually don’t know what every doctors’ role is (and why should they?).  In both settings, it’s important that you clarify what your role is in their care.  It could be “I’ll be handling your day to day medical care and coordinating with our specialists, if needed” or “I’m here to talk with you a little about what’s been going on and start you on the path to getting better and then one of my colleagues will meet with you in the morning to check in and help guide your care from there” or “I’m here to be your go-to person for any aches, pains, rashes, coughs and colds that pop up and keep you as healthy as I can.”  This is important and often overlooked as the last thing you want is to be finishing up your encounter and hear something like “so when is my doctor getting here?”Now what’s in a name?  How do you introduce yourself to your patients?  Some people introduce themself as Dr. Smith, others as Dr. Adrian Smith, and still others just say Adrian Smith and then clarify their status as a doctor and their role.  I’ve found that people are pretty divided on this and a lot of it seems to come from where they trained as the east coast (and even the south coast where I trained) has a much more formal atmosphere in general than here on the west coast and I think that plays into what style of introduction you use.  What I was taught, and I think most people had this same training, is that your patient should be addressed as Mr. or Ms. and you introduce yourself as doctor so-and-so.  I can’t count how many training videos for patient interactions I’ve seen that start this exact way.  But is this ideal in today’s world or is this part of the outdated paternalistic model of the patient-physician relationship?  Trying to find hard data out there in favor of this approach is difficult, but there are a lot of opinion pieces and blog posts talking about how being addressed as “Doctor” and keeping the relationship more formal helps preserve the sacred contract we have with our patients.  The point is that as much as we in medicine are being pushed towards a standard consumer business model, we’re not Steve from the cell phone store - we need to present ourselves as a steady shoulder for our patients to lean on when they’re hurt or afraid or angry or scared.On the other side, a quick look through pubmed actually showed some evidence that patients prefer a more informal introduction.  A study done on about 250 patients in a military family practice setting last year showed that patients preferred to be greeted by their first name and for the doctors to introduce themselves by their last names.  A study a few years ago in a vascular surgery clinic showed similar results.  There was also an interesting study from the Archives of Internal Medicine back in 2007 where they asked about 450 patients their preferences and then video taped the actual encounters.  Just over 50% of patients wanted to be addressed by their first names only, and almost another 25% wanted to hear their first and last names.  Interestingly, on video about ⅓ of physicians called patients by their last names and over half didn’t call their patient by any name at all and boy in those cases you really just have to hope you’re talking to the right person or that could get awkward pretty quickly.  Finally, the survey showed that just over half of all the patients wanted to hear their doctors first and last name, about ⅓ only needed their last name, and the videos showed that in this case, you can always get what you want as those percentages matched up pretty nicely.As with most things in medicine though, there is no class 1A evidence here, there is no right answer, there is only the art of medicine.  There are broad guidelines with caveats that you should refer to your local susceptibilities and specialists to tailor things to your individual populations and really this does come down to the personalized, individual-focused medicine we’ve heard so much about in the news lately.  I personally walk into the room, call the patient by their full names (which helps to make sure I’m in the right room), tell them my full name and then sit down and start chatting because that’s what I’m comfortable with and what works with my patients.  I don’t care if they call me doctor since generally the vast majority of my patients are older than me and I feel a little awkward asking my elders to show me a sign of respect (which by and large they do anyway because that’s just how they were brought up).  Oddly, for the first time in my life I have a lot of people referring to me by my middle name, which sometimes appears to be my first name due to the layout on my webpage.  That I’m not ok with because while Scott Selinger is a dedicated physician, dog enthusiast, and Dallas Cowboys fan, I haven’t quite figured out who Jeff Selinger is, if not my evil alter-ego. But what do y’all do in your day to day practice?  Have you found this to be something you still haven’t figure out or did you long ago get into your introduction groove to the point where it’s now just automatic?  As always, we’d love to hear your feedback on this, so if you have any burning questions or comments, you can post them on our facebook page or email them to canocecp@gmail.com.  And if you have time, be sure to head on over to our facebook page for the Northern California Chapter of the ACP Council of Early Career Physicians so you can find out more about the events going on in the chapter.REFERENCES http://www.ncbi.nlm.nih.gov/pubmed/24083924 http://www.ncbi.nlm.nih.gov/pubmed/19577761  http://archinte.jamanetwork.com/article.aspx?articleid=412602

NSA/NC Meeting Report: Saturday March 2, 2013 The Saturday meeting of the Northern California Chapter of the National Speakers Association featured two savvy businesswomen who shared ways professional speakers and information entrepreneurs can book more business and own their niche markets. Stephanie Chandler is a Silicon Valley refugee who left a stressful job on the Peninsula for the life of a successful information entrepreneur. Lois Creamer knows the speaking business. She has worked with the superstars of speaking as both a cheerleader and strategist. She is endorsed by none other than Patricia Fripp, CSP, CPAE. Stephanie Chandler: Living the good life after corporate America Stephanie was inspired by the film Sleepless in Seattle to move to Sacramento, open a bookstore and write novels. Neither worked out for her. She quickly moved on to Plan B, launching herself as a speaker and non-fiction author on the urging of superstar literary agent Mike Larsen. She launched BusinessInfoGuide.com, her first website, in 2004. Now, six books and a series of websites later, she is in demand as an author, speaker, Forbes blogger, and expert on content marketing, internet marketing, small business growth strategies, and publishing. 8 keys to owning your niche The importance of speakers owning a niche has been a core lesson taught in NSA/NC Speakers Academy classes for many years. Stephanie walks the walk when it comes to knowing how to do this in the digital age. Heck, she's even written a book on the topic. She shared eight simple strategies to increase website traffic and own a niche: Define your target audience. Decide who you want to reach and what challenges you can solve for them. Tap into the power of community. Figure out where your audience spends their time and engaged them there. Optimize your website. Incorporate keywords and phrases that your audience would use to find you. Update your site frequently, generate incoming links, and continually add new content. Implement a blog. This will help build your audience, increase website traffic, generate new clients, create consulting opportunities, and attract media interviews and speaking engagements. It's also the heart of your social media strategy. Stephanie recommends updating it at least twice a week. Develop a content marketing strategy. Create ebooks, white papers, and special reports that you give away. Distribute articles to websites to reach your target audience or write articles for print publications. Expand with video and podcasts. Even speakers who hate writing can record their presentations to communicate their message. Embrace social media. There are great reasons why executives are signing on with social media. Facebook, Twitter, LinkedIn, Google+, and Pinterest each have their own unique advantages. You can automate much of the distribution of the content between these different sites. Commit a little time each day. just as you need to spend time weeding and planting seeds to enjoy a luxuriant garden, so you need to set aside time each day to develop content, engage in social media, and cultivate community. Stephanie advises that a little effort goes a long way in expanding your brand. As professional communicators, the worst way to introduce ourselves is by NAME and TOPIC. We need to learn to introduce ourselves in a compelling and interesting way. Much better is an introduction by CONCEPT and OUTCOME. This captures your unique differentiation. She calls this a positioning statement and uses these words whenever she meets someone: "I work with professional speakers who want to book more business, make more money and avoid costly mistakes." This is far more effective than using an elevator speech, which is typically too long and boring. An effective positioning statement should go on you...

This article marks the start of a new series in Professionally Speaking. For the past three years I've published a monthly Pro-Track Profile interview featuring members of the Pro-Track class of the Northern California Chapter of the National Speakers Association. I've decided to start a similar series featuring freelance creatives who make their living as communications consultants, writers, editors, and more. This Freelance Focus series kicks off with an interview with the founder of the Agency which has provided me with the majority of my own freelance work for the past three years. Vickie Sullivan is internationally recognized as a top market strategist for experts. Specializing in branding for high-fee professional speaker markets, she has launched thousands of thought leaders since 1987. Vickie’s groundbreaking work has earned her an appointment on the Women’s Leadership Board for the Kennedy School of Government at Harvard. In that capacity, she presented three times to the Harvard student body on personal branding and prominence. On Saturday she presented to a somewhat less august institution than Harvard. She rocked the house at the meeting of the Northern California Chapter of the National Speakers Association. Her presentation, Brand It And They Will Come: Build a High-Fee Brand When You Are Not Famous, was short on fluff and long on street-wise detail to help the average $1,000 - $5,000 speaker in the audience stand out from the crowd. An era of practicality With meeting expenditure down, even as the number of meetings are trending higher, clients are doing more with less. Unlike the go-go days of the boom period, we are now living in an era of practicality. Executives are micromanaging meetings down to the level of choosing the theme and seating arrangements. They are sending interns to do first-round interviews with prospective speakers. Many speakers are not only competing with each other for gigs, they are competing with the authors, bloggers and social media mavens who will speak for free in order to pimp their books and consulting programs. So what's a novice speaker to do? First, lose the obsession with presentation skills. Being able to present well is table stakes. Next, realize meeting organizers divide the world into speakers who are nice to have and those they gotta have. In order to fall into the latter camp the information you offer has to overlap with the buyers perceived needs and beat out the other options being considered for the meeting agenda. You have to stand out from the crowd by uniquely branding yourself. Getting hired requires you create a vivid picture in the client's mind of what they will get when you speak at their event. Craft your brand in a way that appeals to the buyers emotions and creates an image of you in a category of one. Vicki goes into more detail on the importance of differentiation in this Special Report on her website: If branding is the promise you make to the market, then differentiation is the stage you use to deliver on your promise. The Three Keys to a High-Fee Brand One: A Successful Story Your compelling story is only useful to the extent it teaches others. Sports hero's have a problem on the speaking circuit if their motivational stories don't connect with the couch potatoes in the audience. We can connect with the audience by telling stories that break with the expected and bring opposites into play, compare unexpected things or roles or reveal a Eureka moment. Vickie's website has an article on writing Stories That Create The Emotional Drive To Buy. Two: A Point of View Build a point of view around hot issues. Focus on a Big Idea. Make the invisible visible. Tell people things they've not heard before. Three options that help build a unique point of view: Predict the Future - what can we expect that will impact the audience's world in unexpected ways? In my world, I'm seeing a growing backchannel of communication using PDA's and social media to converse about the speaker while they are still onstage. Sound the Alarm - what does the audience need to know that will save their skins? Again, in my own world, too much information is being crammed into presentations by subject matter experts. Someone else in the audience today shared how middle managers in Silicon Valley technology companies are addicted to the corporate crack of repeatedly launching projects to boost their career without realizing they are burning everyone out with their 7x24 demands. His first book, Journal of the Dead: A Story of Friendship and Murder in the New Mexico Desert, was selected as a New York Times Notable Book of 2003. He has appeared as a guest and commentator on CNN, Fox News, TruTv, NPR, and numerous regional TV and radio shows. His works have been optioned for both film and television. Focus on your speech: "No fancy brochure or website can overcome a mediocre speech." Tips for creating a powerful speech include: Use humor - once the endorphins kick in, the audience is more receptive to your message. Be real, open and authentic. Be congruent with your message, both on and off the platform. Deliver a focused message with one main idea and three points. Tell great stories - especially ones that are about the audience, not you. Outline your speech: Pick one theme and a through-line. Would the message of the speech look good printed on a T-shirt? Does it have three main points and a hook to hang it on? Ask, "Who do I need to become to take my speech to new heights?" It's all about positioning: become an expert first, a speaker second. "Clients no longer want to hire speakers. They want to hire smart people who happen to speak." Expertise - Topics - Revenue Jane recommends that we each become an expert in one area we are are passionate about. This is the "lane" we travel in. Don't get stuck doing things we are merely good at. Find an area of expertise people will pay for. Choose this one area and develop a number of topics around it. Ask "What do I need to become, to be the expert?" It might be focusing on a topic. It might be standing tall and stepping fully into the knowledge you already have. In my case, my expertise is helping executives communicate effectively. The topics I've developed center on my services as a speechwriter. These services include writing one-time speeches for CEOs as well as company overview or Vision Pitch presentations delivered multiple times in the Executive Briefing Centers of major Silicon Valley companies. More recently, I've added topics involving Social Media and re-purposing a speech with podcasting. This allows asychronous communication of the speech outside the room in which it is delivered. Jane suggests that we build revenue streams which flow from each of our topics. In my case, I have my consulting fees and more recently the recorded webinars, workbooks and clinics ranging in price from $39 - $499 available on Practical Social Networking. Websites Jane reminds us that a good website is a key requirement for a successful speaker. One-sheets are so last century. A good website clearly positions you and shares a promise (or brand) that will make the prospect curious about what it is that you offer. She recommended Rene Godefroy's and Keith Harrell's as great examples. Ellen Taliaferro, MD (aka Dr T) is a recovering emergency physician now serving as the Medical Director of the San Mateo Medical Center Keller Center for Family Violence Intervention in San Mateo, CA. Using her background in emergency medicine, stress management, and writing, she has created a program called Healing the Wound Within with a personal Writing Practice Prescription. In addition, she continues to speak on Domestic Violence As A Health Issue and The Medical Aspects Of Manual Strangulation As A Form Domestic Violence Assault. Dr T grew up in Will Rogers country and loves country humor. She sees herself as a true Okie: "Sooner born, Sooner bred, and when I die I’m California dead." Of note, she finished her clinical career at UTSW medical school and Parkland Hospital. While there, the Dallas Morning News named her as one of the 100 most influential Texas women — an amazing feat for an Okie. The Northern California Chapter of the National Speakers Association (NSA/NC) hosted our annual Open House this evening. Over 35 people turned up on the night when much of the rest of the nation was tuning in to the Palin/Biden debate. The audience heard from a bevvy of professional speakers about what it takes to make a living in this profession. Among the visitors, I met Nicole Bachelor, who was born in the UK, but now lives in the San Francisco Bay Area. She works for one of the big technology companies in Silicon Valley and has many years experience as a telecommuter. Nicole has parlayed her expertise into a website and e-book which reveals essential advice and tools for negotiating a telecommuting arrangement in your existing or new job. Then helps make sure you telecommute successfully and keep your career at peak performance. She provides tips, tricks, and techniques for making telecommuting as simple, and easy as going into the office, without all the extra hassle. Elfrena Foord owns Foord, Van Bruggen, Ebersole & Pajak, a Sacramento wealth-management firm. She and her 3 partners are in the process of distilling their 20+ years of experience together in a book titled "Wealth & Beyond". Elfrena plans to do speaking engagements after her book comes out. She currently does seminars for non-profits to help donors use their wealth to create their family working together and staying connected and to benefit causes that are important to them. Last summer she attended just one day of the National Speakers Association Convention in San Diego. This convinced that NSA is the organization to help take speakers to the next level. She has now enrolled in the 2008 Pro-Track program offered by the Northern California Chapter of NSA. The program kicks-off on Saturday 26th in San Francisco. It is sold out. Her enthusiasm for the program is contagious. Not only did she persuade her husband Bruce to join her, but her sister Matson also! They'll be carpooling in once a month for a full day of training on the business of professional speaking. I talked with Elfrena on the phone about her business and how she sees speaking fitting in. I spent the weekend at the annual 'Camp NSA' training program held at the headquarters of the National Speakers Association in Tempe, Arizona. This workshop takes future Presidents of the regional Chapters in the USA, Canada, South Africa and Australia through an intensive consideration of what's involved in serving as President of the Board of Directors of a Chapter. I'll be the 2008-09 President of the Northern California Chapter. It's been fascinating to hear discussions on subjects such as financial planning; fundraising; program development; membership building and retention; conflict resolution and the mechanics of holding successful Board meetings. One of the skills a good Chapter President cultivates is communicating the value of joining this organization to potential members interested in the business of speaking. I realized I had a unique opportunity to ask my fellow Chapter Presidents-in-waiting at the event what attracted them to NSA, what's the value in being a member and why, in fact, someone who is involved in the speaking, training, workshop facilitation or speechwriting business should consider joining the National Speakers Association. In this episode my guest, Kevin Hines is sharing his story about jumping off the Golden Gate Bridge and surviving by a miracle, all the details in the episode. Beers Award for his efforts to improve the lives of and attitudes toward people with mental illnesses.
