O aumento do número de casos de Covid-19 no país já leva a um aumento das hospitalizações, de acordo com o infectologista francês Benjamin Davido, que atua no hospital Raymond Poincaré, em Garches, perto de Paris. Por precaução, o governo francês decidiu antecipar o início da campanha de vacinação, prevista para o dia 17 de outubro, para o próximo dia 2. Taíssa Stivanin, da RFIOs hospitais do país registram há pelo menos duas semanas um aumento de cerca de 30% de pacientes positivos para o vírus Sars-CoV-2 atendidos em pronto-socorro, segundo a agência nacional de saúde pública (Santé Publique France). Segundo a rede francesa SOS Médécins, que atende pacientes em casa, entre 4 e 10 de setembro a alta foi de cerca de 17% de infecções em todas as faixas etárias, e de 58% entre as crianças.No início da campanha de vacinação, o primeiro imunizante disponível para a população será o da BioNTech-Pfizer, a base de RNA mensageiro e eficaz contra a subvariante XBB.1.5, da ômicron. A vacina da Moderna, que utiliza a mesma tecnologia, também já foi aprovada para uso no país, assim como a Novavax, que ainda aguarda uma autorização final para ser comercializada.O infectologista francês Benjamin Davido, que atua no hospital Raymond Poincaré, nos arredores de Paris, vive no dia a dia os efeitos da alta do número de casos de Covid-19. Ele lembra que, atualmente, não há modelização matemática ou monitoramento que indiquem, com precisão, qual é a dimensão da atual onda de casos.Além disso, observa, os franceses simplesmente não se testam mais. “Nas ondas anteriores, as pessoas já chegavam ao hospital com o diagnóstico. Hoje, os pacientes vêm ao pronto-socorro porque passaram mal ou não estão se sentindo bem, têm dor de cabeça ou falta de ar. O diagnóstico é feito no atendimento de emergência.”De acordo com ele, de cada três pacientes que chegam ao pronto-socorro, um será hospitalizado. Para o infectologista, isso é surpreendente, já que a taxa de internação por Covid, desde o início da epidemia, oscila entre 10 e 15%.“A epidemia está acelerando com novas variantes que são mais contagiosas. O risco, se não aceleramos também a campanha de vacinação, é acabar com pacientes em macas nos corredores, e com dificuldade para organizar a hospitalização”, alerta.Medidas de proteção agora são inexistentesSegundo ele, atualmente também há um risco mais elevado de contaminação dentro do hospital, onde as medidas de proteção, como o uso de máscaras, por exemplo, foram deixadas de lado. Com a chegada do inverno no hemisfério norte, outros vírus, como o da gripe e o da bronquiolite, vão se somar ao da Covid-19, o que pode complicar ainda mais a situação nos hospitais.Na França, a cepa predominante é a EG.5.1, que representa pelo menos 35% das contaminações, mas cerca de 1% dos casos envolvem a subvariante BA.2.86, considerada mais "preocupante" pela OMS (Organização Mundial de Saúde). Elas são mais contagiosas e têm capacidade para driblar a imunidade adquirida pela população ao longo do tempo, o que preocupa as autoridades francesas."A Covid-19 não é um resfriado""É necessário parar de banalizar a Covid-19 e de associar casos graves e mortes apenas à população idosa e com comorbidades", frisa o infectologista francês. A ômicron, lembra, foi responsável pela morte de pelo menos 30 mil pessoas no país em 2022.“Infelizmente, todos os dias pessoas morrem depois de terem sido contaminadas. Não é um resfriado”, reitera. “Hoje, as pessoas que chegam ao hospital são idosas e têm outras patologias. Não quero ser dramático, mas a má notícia é que a epidemia sempre começa assim e depois os pacientes são cada vez mais jovens. Quanto mais o vírus circula, terão mais casos graves.”Outro problema é a falta de informação. A população, diz, não sabe da existência de uma vacina atualizada e também não interiorizou o fato de que as doses de reforço, assim como no caso da gripe, devem ser frequentes – entre seis meses e um ano, em função dos riscos."É preciso parar de contar as doses. A vacinação no caso da Covid deve ser feita periodicamente", lembra. Seis meses depois da vacina ou de uma infecção natural, não há mais proteção contra as novas subvariantes. Este é o caso hoje de uma grande parte dos franceses, ressalta, e por isso a dose de reforço é essencial.O desafio agora é lidar com o ceticismo de uma boa parte da população francesa. “De uma certa forma, há urgência. Essas pessoas, nos próximos dias, podem ser contaminadas. Então, neste contexto, avançar a campanha em 15 dias faz muita diferença. Mas esse tempo deve servir para convencer os franceses a se vacinarem.”

În cadrul ediției de pe 21 martie 2023 a emisiunii #știința360 de pe Radio România Cultural, Dr. Marius Geantă, Președintele Centrului pentru Inovație în Medicină, a comentat ultimele noutăți ale domeniului medical precum și topul săptămânal Esențial Covid-19 de pe Raportuldegardă.ro. Pe fondul unei încrederi deja scăzute în vaccinare, apariția pandemiei de COVID-19 și a vaccinurilor contra SARS-CoV-2 a dat apă la moară antivacciniștilor și grupurilor conspiraționiste. Temerile multor oameni au crescut și pe fondul vaccinării cu seruri obținute în timp record prin tehnologii de ultimă oră, explicațiile autorităților pe acest subiect fiind sumare. Vaccinarea anti-COVID-19 a început în România, la 27 decembrie 2020, prima persoană vaccinată fiind asistenta medicală Mihaela Anghel, de la Institutul Național de Boli Infecțioase „Prof.dr. Matei Balș” din Capitală. Acțiunile au continuat apoi la nivel național. A fost constituit un Comitet național de coordonare a activităților privind vaccinarea împotriva COVID-19 – organism interministerial, fără personalitate juridică, aflat în subordinea Secretariatului General al Guvernului și sub cooordonarea primului ministru. Totodată, a fost lansată o platformă informatică care permitea programarea persoanelor la vaccinare. Prioritate, într-o primă fază, a avut personalul medical și cel social, angajați în instituțiile de îngrijire și apoi, în faza a doua, populația încadrată în grad de risc și lucrătorii din domenii considerate cheie. În etapa a treia, s-a putut imuniza populația generală. Prima tranșă de vaccinuri împotriva COVID-19 a fost compusă din două containere, fiecare a câte 5 cutii conținând în total 9.750 de doze de la compania BioNTech/Pfizer. Vaccinul anti-COVID-19 a fost recomandat mai întâi persoanelor peste 18 ani, după care, din 2021 a fost disponibil și pentru persoanele peste 16 ani. Ultima categorie care a avut acces la un ser dedicat au fost copiii între 5 și 11 ani care au putut fi imunizați cu serul produs de Pfizer/BioNTech începând cu ianuarie 2022. Ultimele date al Platformei ROVaccinare arată că, la nivel național, în februarie 2022, proporția vaccinării a fost de 40,7 la nivelul populației generale, mai mare la oraș – 43-45% și mai mică la țară – 30%. Drew has spent his career working on RNA biology and vaccine development. He, along with his colleague Katalin Kariko, have been pivotal in the development of mRNA vaccines most notably the COVID-19 vaccines developed by BioNTech/Pfizer and Moderna. Drew received his MD/PhD from Boston University in 1987, following which he did a residency at Beth Israel and followship at the NIH under Anthony Fauci. He has received numerous awards including the Lasker-DeBakey Clinical Medical Research Award and Rosenstiel Award.Today we discuss Drew's career trajectory, mRNA biology and vaccine development.

According to the report, “Other pharmaceutical executives have addressed the committee, including the CEO of Moderna and senior officials from AstraZeneca and Sanofi.” Bourla? Not so much. The committee's chair, Belgian MEP Kathleen Van Brempt, told POLITICO she “deeply regrets” the decision taken by Pfizer.After a visit to BioNTech's headquarters last week, Van Brempt had said in a written statement that she looked forward to discussions “with other CEOs” including “Mr. Albert Bourla, the CEO of Pfizer” on October 10. -Politico In early September, an audit repirt into the EU's vaccine procurement strategy raised questions over Bourla's relationship with European Commission President Ursula von der Leyen before they struck a multibillion-euro vaccine deal. The report, by the European Court of Auditors, found that von der Leyen had been directly involved in preliminary negotiations for the EU's biggest vaccine contract, for up to 1.8 billion doses of the BioNTech/Pfizer vaccine, which was concluded in May 2021. This was a departure from the negotiating procedure followed with other contracts, where a joint negotiating team made up of officials from the Commission and member countries conducted exploratory talks. -Politico In April, von der Leyen admitted that she had been texting with Bourla for a month straight while they were negotiating the massive contract. Two months later, the texts disappeared, triggering accusations of maladministration by the EU's ombudsman, Emily O'Reilly,” Reuters reported at the time. She investigated RNA-mediated immune activation and together with Drew Weissman discovered that nucleoside modifications suppress the immunogenicity of RNA, which widened the therapeutic potentials of mRNA. Her patent, co-invented with Drew Weissman, was used to create the FDA-approved covid-19 mRNA vaccines by BioNTech/Pfizer and Moderna. Her groundbreaking contribution was to use chemistry to modify the mRNA so that it wouldn't be rejected by the immune system. Seit heute sind in Europa die auf die Omikron Subvariante BA.1 angepassten Impfstoffe von BioNTech/Pfizer und Modern zugelassen. Ab der kommenden Woche können Sie bestellt werden. Eine STIKO Empfehlung gibt es noch nicht (Stand 02.09.22 15:00 Uhr). Vor 2 Tagen wurden noch dazu in den USA  per Notfallzulassung die Impfstoffe gegen die, heute in Deutschland vorherrschende Omikron Subvarianten BA.5  und BA.4 zugelassen. Es wird erwartet, dass auch die EMA gegen Ende des Monats eine Zulassung aussprechen wird. Was ist also zu tun? Impfen, oder warten? Die aktuelle Folge diskutiert einige Antworten.

Grund der Bedenken sind sehr selten vorkommende Blutgerinnsel in Verbindung mit einer verringerten Zahl an Blutplättchen nach einer Impfung mit dem Johnson-&-Johnson-Vakzin. Neuer Ocean-Viking-Rettungseinsatz Das Schiff der Hilfsorganisation SOS Méditerranée hat vor der Küste Libyens 114 Menschen aus dem Mittelmeer geholt. Coronaimpfung für 5- bis 11-jährige Kinder (Stand 15.12.21). Heute sprechen wir über das aktuelle Top-Thema: Seit dem 09.12.2021 empfiehlt die STIKO nun offiziell eine Impfung mit dem Impfstoff von Biontech/Pfizer für 5- bis 11-jährige Kinder. Vor allem sollen solche geimpft werden die entweder selber eine schwere Vorerkrankungen haben und Kontakt zu Menschen haben, die ein erhöhtes Risiko durch Corona haben (z.B. andere vorerkrankte Kinder oder erwachsene Risikogruppen). Gerade der 3. Teil der Empfehlung, dass auch geimpft werden soll wenn die Eltern und das Kind den individuellen Wunsch hat. Viele empfinden dieses Statement als "Zurückgeben" der Verantwortung an die Erziehungsberechtigten. Bei welchen Vorerkrankungen sollten Kinder besonders dringend geimpft werden? Wie gut ist der Impfstoff überhaupt in dieser Altersklasse erforscht? Welche Nebenwirkungen können auftreten? Wie sieht die aktuelle Coronalage bei Kindern aus? Was ist eigentlich PIMS bzw. (NEXT) Foreclosure Looms for Homeowners Who Thought They'd Won, Thanks to Top New York Court Ruling Sam Mellings The City and New York Focus, November 30, 2021 Christine Fife was “speechless with joy” when she won her foreclosure case in January 2020, she recalled, believing her decade under threat of foreclosure in her Upper West Side condo was finally over. Now, though, Fife is once again facing the seizure of the apartment she has owned since 1990. In February 2021, New York's top court issued a decision that eliminated a path that New York homeowners had used for years to fight foreclosure. The decision in Freedom Mortgage Corporation v. Engel allowed Fife's lender to renew its foreclosure suit against her. “They said it was OK. How can they change their mind?” Fife asked during an interview with New York Focus and THE CITY. Across New York State, homeowners who believed that their cases had been settled in their favor are now once again facing foreclosure due to the Engel decision. Many are in danger of losing their homes, even as two bills aimed at protecting owners wend their way through the state Legislature. Case Reopened In New York, if a borrower misses a mortgage payment, the lender is allowed to demand the entire remaining balance immediately and then move to foreclose after 120 days, if the money owed remains unpaid.. But a lender must start the legal proceedings within six years of first demanding full payment, or the suit becomes invalid. Until recently, the clock kept ticking until the lender informed the borrower that they were no longer seeking foreclosure. In Fife's case, the lender had never done so. The bank sued Fife twice: first in 2010, a case the lender claims it later voluntarily withdrew, and again in 2017. Her lawyers, representing Fife pro bono, successfully argued that the bank's second foreclosure suit was barred by the six-year limit and got it dismissed. But the Engel decision changed the rules. The Court of Appeals found that voluntarily ending a foreclosure suit stops the clock on the six-year time limit — even if the homeowner is never notified. The court's ruling applies retroactively to any foreclosure cases ongoing or still open to appeal at the time the decision was issued. Following the ruling, many foreclosures that expired under the six-year limit have been reopened or appealed to higher courts. Holly Meyer, a Suffolk County lawyer who represented one of the defendants in the Engel case, estimated that the number of affected homeowners could be in the tens of thousands. Fife's was one of them. In April 2021, the bank moved to renew its foreclosure suit against her — and the trial judge cited the Engel ruling as a reason to rehear the case. “I was shocked at this decision, because I had put all my faith in [the foreclosure court's] initial decision, which was in my favor,” Fife said. With Fife's best defense gone, her hopes for avoiding foreclosure now appear slim, her legal counsel acknowledged. ‘Incompetently' Managed Loans It's not uncommon for lenders to allow their right to foreclose to expire, according to real estate lawyers. “There are millions of residential loans being serviced somewhat incompetently, so these things do sometimes slip between the cracks,” said Joshua Stein, a commercial real estate lawyer. Real estate industry supporters of the Court of Appeals' decision say it made little sense for a foreclosure case to fail because of what they consider a clerical error — one that basically lets borrowers shirk their debts. “The idea that you should still be at risk because you haven't repaid the money you borrowed doesn't strike me as egregious,” Stein said. Homeowner attorneys say that cases get dropped all the time in the legal system because of technical violations of procedural requirements, and that foreclosure cases should be no different. “If you have somebody on trial for murder, but you find that their constitutional rights were violated, they go free. It's the same thing here,” Meyer said. Chief Judge Conflicted? The day after the Court of Appeals ruled on the Engel case, the law firm Greenberg Traurig, which had represented two of the plaintiffs, cheered what it called a “ground shifting” victory for lenders. “Statewide application will likely protect billions in assets for mortgage holders,” its press release claimed. Chief Judge Janet DiFiore, who wrote the majority opinion in Engel, was a Greenberg Traurig client at the time that she ruled in favor of their bank clients in that and other cases, the New York Law Journal reported in April. The firm defended her in a suit brought by judges forced into retirement as a cost-cutting measure. Defense attorneys said they had not been informed of the potential conflict for the judge who ruled against their clients. “The law's not on our side anymore, and that means that there are a number of people who will be facing foreclosure when they wouldn't have faced it a couple of years ago,” Julie Howe, a senior staff attorney at the New York Legal Assistance Group, who is representing Fife pro bono, told New York Focus. Governor Andrew Cuomo's Office Jacob Inwald, director of foreclosure prevention at Legal Services NYC, said many of the foreclosure cases affected by the Engel decision originally stemmed from the real estate crash of 2008 and freewheeling lending leading up to it. Fife, for instance, had borrowed $731,000 against her apartment in April 2007 to pay living expenses after a disabling injury and divorce. Monthly payments were nearly $5,000, with adjustable rate mortgage that started at 8% annual interest, potentially rising as high as 15%. “I didn't know anything about mortgages,” Fife said. “I was just so happy that I was able to live on another day. I was probably the easiest take on the block.” Within a year, the bank alleged that she had fallen behind on her mortgage payments — kicking off foreclosure proceedings that her loan's holder, Wilmington Trust Association, has resurrected more than a decade later. ‘It's Really Scary' Rockland County resident Susan Azcuy is in a situation similar to Fife's — believing that she'd survived foreclosure only to find the cloud hanging over her once again. For 23 years, Azcuy and her husband kept up with the mortgage payments on their house in Pomona, including a 2005 refinancing, for which she took on a debt of $210,000 at 5.75% interest. But in 2012, after Azcuy's husband was fired from his job, they missed a payment and their lender quickly moved to foreclose. The bank voluntarily withdrew the suit in March 2016 for technical reasons but did not notify Azcuy, refiling the case the next month. It went to trial in 2019, and Azcuy won, after a key prosecution witness failed to show up. ‘We're still struggling. I was very, very hopeful to be able to continue living here.' Before Engel, this would have been the end of the case, since more than six years had elapsed since the 2012 foreclosure suit. But thanks to Engel, the six-year clock restarted in 2016 — giving Azcuy's lender another chance to sue. Due to penalties and foreclosure fees, Azcuy now owes nearly $400,000, just shy of double the amount of the 2005 refinancing. Efforts to reach a settlement or a modification with the bank have been unsuccessful, according to Azcuy's attorney, Derek Tarson of the Legal Aid Society of Rockland County. If the bank brings another foreclosure lawsuit, which Tarson believes is likely, Azcuy will not be able to rely on the defense that more than six years have passed. “It's really scary. We're still struggling,” Azcuy said. “I was very, very hopeful to be able to continue living here.” Lawmakers Respond State lawmakers have introduced two bills seeking to reverse some of Engel's effects. One measure, sponsored by Sen. Kevin Thomas (D-Nassau) and Assemblymember Helene Weinstein (D-Brooklyn), would require lenders to inform borrowers if they withdraw their demands for payment, since that action serves to stop the clock on the six-year countdown. If lenders withdraw the lawsuit but fail to notify the borrowers, the clock would keep ticking — a return to the status quo before Engel. The bill would also forbid lenders from foreclosing if any part of the loan had previously expired — a measure that would bar reviving suits against homeowners like Fife and Azcuy. The legislature is also considering a second bill, sponsored by Sen. James Sanders (D-Queens) and Assemblymember Latrice Walker (D-Brooklyn). This bill would prevent lenders from discontinuing a demand for full payment, stopping the six-year countdown clock, without the consent of the borrower. The measure also would start the countdown from the time that a mortgage holder missed a payment. Though meant to protect homeowners, the Sanders-Walker bill could actually incentivize lenders to begin foreclosure more quickly, one foreclosure defense attorney told New York Focus. “If the clock is ticking, all plaintiffs are going to want to do is get their case started,” the attorney, who asked not to be named, said. Sanders rejected the critique. “I don't think that you can further incentivize the financial institutions” to foreclose after the leeway granted them by Engel, he said. Whether either of the bills would apply retroactively to homeowners like Fife and Azcuy is an open question. “It may not be able to help those, but it is our desire,” Sanders said. “We will get guidance on whether we can do that.” Sanders said that he has spoken to Gov. Kathy Hochul and legislative leaders about his bill, and while they have not endorsed it, he said they are open to supporting it. (A Hochul spokesperson said the governor “will review all legislation that reaches her desk.” “We are making excellent progress with both, and we expect good things in the coming days,” Sanders said. (NEXT) Israeli study says COVID shot efficacy decreases dramatically after 3 months, calls for boosters British Medical Journal, November 24, 2021 A study published by The BMJ today finds a gradual increase in the risk of COVID-19 infection from 90 days after receiving a second dose of the Pfizer-BioNTech vaccine. The study was carried out by the Research Institute of Leumit Health Services in Israel. Israel was one of the first countries to roll out a large scale COVID-19 vaccination campaign in December 2020, but which has seen a resurgence of infections since June 2021. The findings confirm that the Pfizer-BioNTech vaccine provided excellent protection in the initial weeks after vaccination, but suggest that protection wanes for some individuals with time. To do this, the researchers examined electronic health records for 80,057 adults (average age 44 years) who received a PCR test at least three weeks after their second injection, and had no evidence of previous COVID-19 infection. Of these 80,057 participants, 7,973 (9.6%) had a positive test result. These individuals were then matched to negative controls of the same age and ethnic group who were tested in the same week. The rate of positive results increased with time elapsed since a second dose. For example, across all age groups 1.3% of participants tested positive 21-89 days after a second dose, but this increased to 2.4% after 90-119 days; 4.6% after 120-149 days; 10.3% after 150-179 days; and 15.5% after 180 days or more. GET WITH THE PROGRAM. Ivermectin has been affiliated with Nobel-ity. It's not only been awarded a prize for its safe use as an anti-parastic for decades and been doled out to literally billions of people, (including pregnant women and children) with no ill effects, it's has an excessively successful safety profile as an off-label drug in the context of COVID-19.¹ 3. Just like we don't need to seek a ‘cure' for the common cold or the flu. That's one of the things that our bloody immune systems are for and very good it - preventing disease. Viruses are EVERYWHERE. ALL THE TIME. It's not a reason to freak out. Educate yourself and others on this. It about high time people learned that we are constantly engaging and have co-evolved with viruses and bacteria for the entirety of our existences. It's what we are.¹² Our genome is 7% retrovirus. (NEXT) Why aren't healthcare workers speaking out about the catastrophe caused by the vaccines? Steve Kirsch, November 23, 2021 Everyone thinks that if the jabs were really dangerous, doctors and other healthcare workers would be speaking out about it. They are wrong. Here are the four main reasons they do not speak out.Two important things you need to know: 1.All the kids were recently vaccinated. 2. Kids that age NEVER get tachycardia (i.e., the medical experts I've talked to have never seen it before in their careers). Here are some reasons very few people are speaking out: 1. Delegated trust. People trust their doctors, the doctors trust other doctors and ultimately the CDC. Nobody is independently verifying the CDC is telling the truth. Doctors are really really bad in critically reading scientific studies. Mask guidelines are the PERFECT example of this. There isn't any scientific proof (well-done randomized trial) that masks work. Yet very few question the narrative (and those that do are ostracized). So everyone basically goes with the flow and the whole thing is a positive feedback loop despite zero scientific basis. See my article Masks don't work and read the accolades in the Nature article. All these experts who hailed the study never read it with a critical eye and lack the skills to do so. This is how misinformation propagates. 2.Fear of job loss. Nobody wants to lose their job. Look what happened to Deborah Conrad and others who speak out. Fired within hours after speaking out. So the lab technicians who are now seeing kids with tachycardia just keep their mouth shut. They know something is very wrong, but their job is more important. Besides, if they spoke out, it wouldn't make any difference since they are just a lab technician. Doctors have a similar problem. The medical system, despite claims of physician autonomy, actually offers very little, as it takes very little to be thrown out of the system. Medicare, the FDA, a state medical board, a malpractice insurer, the DEA, a hospital medical staff, an employer - you only have to cross one of these to have your career ruined. Combine that with the idea that most physicians wouldn't be willing to stand against a medical establishment agency such as the CDC (the ones who will have long since been ostracized) and that to do so would require a huge amount of energy and time spent on medical paper research to make a case (and most docs don't have time for that) and that most of medicine is necessarily a form of group think anyway. Then add on to it that the policy makers in large medical corporations roles are more immediately to protect the interests of the corporation than to "save the world," and you arrive at our current situation. 3.Belief that COVID is even worse than the vaccine injuries. Many people are deceived by erroneous reports that the number of vaccine cases (e.g., of myocarditis) are occurring far less often now that the vaccines have been rolled out. Dr. John Su is the big culprit here because he's never told the world that VAERS is under-reported. The pediatric cardiologists know what is going on, but they aren't going to say anything due to #1. So I see doctors tweeting the myth that “sure, there is myo after the vaccine, but the rates due to COVID are worse so the vaccine is the better of the two options.” 4.Belief that the injuries are really rare. I know a doctor who treats vaccine injured patients. He has no clue whether these are every single vaccine injured patient in the US or he's only seeing a tiny fraction of the injuries. He believes he's seeing them all so writes it off as just “coincidence” and “bad luck” since if it was the vaccine, the CDC would have spotted it. 5. Cognitive dissonance/trust in authority figures. They are so convinced the vaccines are safe (since nobody else is speaking out), that any adverse events that happen must be due to something else. Positive feedback loop. 6.Belief that they can treat you for your vaccine side effects, but that they can't treat you if you have COVID. So lesser of two evils. And of course, they think no early treatments work, so they think they are doing you a favor by telling you to get the vaccine. 7.Belief that there is no viable alternative for treating COVID and that the vaccines work. So even 100,000 dead or injured people is better than 750,000 dead people from COVID. 8. Trust in the NIH and CDC. If it was a problem, the CDC would tell people. Telling people isn't their job. Their job is to follow the direction set by the experts. 9.Fear of being ostracized. People who do research fear if they speak out they would be labelled as anti-vaxers and their research would thus be discredited. 10.Critical thinkers have been fired. Hospitals and medical facilities have already fired vaccine hesitant employees per vaccine mandates thereby self selecting for vax believers. 11. They think that the side-effects show that the vaccine is “working.” This is more of a patient thing. It's how the patients look at their adverse events… as a positive thing. (You really can't make this stuff up.) 12.They are being paid to look the other way. The federal government gave “grants” (aka BRIBES) to hospitals and physicians to promote the vaccines. If they speak out against them now, the government will demand the grants are repaid. [A physician reported this to me on Telegram. You really can't make this stuff up.] 13. They will lose their research funding if they publish their results.

Ulmer Eltern haben stellvertretend für viele Familien im Land einen Offenen Brief an Gesundheitsminister Lucha geschrieben. Die Ulmer Eltern fordern Lucha auf sofortige Vorbereitungen für ein Kinder-Impfangebot zu treffen. Die europäische Arzneimittelbehörde EMA will voraussichtlich schon nächste Woche über die Zulassung des Corona-Impfstoffes von Biontech-Pfizer für Kinder ab fünf Jahren entscheiden. Die Eltern fordern deshalb ein konkretes Kinder-Impfangebot über die Weihnachtsferien. Und das müsse man sofort vorbereiten. Man dürfe jetzt nicht ungenutzt Zeit verstreichen lassen. Episode 75: Multisystem Inflammatory Syndrome in Children (MIS-C).  Dr Schlaerth explains the signs, symptoms, and basic management of MIS-C. Lam explain the role of anti-obesity medications in weight management. Introduction: The Role of Drugs in Weight Loss Management    By Lam Chau, MS3, Ross University School of Medicine     Today about 70% of adult Americans are overweight or obese. Obesity is associated with increased risk of heart disease, stroke, and diabetes, among many other diseases. Studies have shown losing 5-10% of your body weight can substantially reduce your risk of cardiovascular disease.  Traditional belief is that weight loss can only be attributed to diet and exercise. While there are certainly elements of truth to that statement, medication is a safe and proven method for weight management that is often overlooked. The fact of the matter is that weight loss is an ongoing field of study with constant new research and innovations.  In June of this year, a medication named Wegovy was approved for weight loss management by the FDA. This drug is indicated for chronic weight management in patients with a BMI of 27 or greater with an accompanying weight-related ailment or in a patient with a BMI of 30 or greater. Rachel Batterham, PhD, of the Centre for Obesity Research at University College London, shared: "The findings of this study represent a major breakthrough for improving the health of people with obesity. No other drug has come close to producing this level of weight loss — this really is a game changer.” Despite breakthroughs like these, the use of medication for weight loss is still relatively low. Dr. Erin Bohula, a cardiologist and assistant professor at Harvard Medical School, believes “there are probably a few reasons for this, including cost, if not covered by insurance, and a perception these agents are not safe in light of the history with weight loss agents.” A study from 2019 examined the medical records from eight geographically dispersed healthcare organizations. They found that out of 2.2 million patients who were eligible for weight loss medication, only 1.3% filled at least 1 prescription. Weight loss is a dynamic process with many different variables. However, about 18 months ago, a new pediatric complication of COVID-19, possibly postinfectious, was described.  The eight children who were initially described had a clinical presentation which was similar to either Kawasaki Disease or perhaps toxic shock syndrome, and since these children had signs of a hyperinflammatory state coupled with shock, the new syndrome was named Multisystem Inflammatory Syndrome in Children, or MIS-C for short. By midsummer of 2021, the United States had about two thousand cases and 30 deaths in children under 21.  Other name for this condition is Pediatric Hyperinflammatory Shock. DiagnosisWhat are the criteria for a diagnosis of Multisystem Inflammatory Syndrome? They include:Age below 21Fever above 100.4 degrees Fahrenheit or 38 degrees centigrade for 24 hours (a subjective fever for more than 24 hours counts too). Laboratory evidence of inflammation which should include at least two of the following tests: elevated CRP, elevated ESR, elevated fibrinogen level, procalcitonin, D-dimer, ferritin, lactic acid dehydrogenase (LDH), interleukin-6, and neutrophil counts, low lymphocyte count and low albumin.Severe disease necessitating hospitalization with multisystem organs affected. The systems affected include cardiac, renal, respiratory, hematologic, gastrointestinal, dermatologic, and neurologic (at least three systems need to be involved). No creditable other diagnosis. Other symptoms include:GI complaints (diarrhea, vomiting, abdominal pain)Skin rashConjunctivitisHeadacheLethargyConfusionRespiratory distressSore throatMyalgiasSwollen hands/feetLymphadenopathyCardiac signs and symptoms include troponin/BNP elevation and arrhythmia. Findings on ECHO may include depressed LVEF, coronary artery abnormalities, including dilation or aneurysm, mitral regurgitation, and pericardial effusion. There also must be a positive test for SARS-CoV-2 and this test can be either a reverse transcriptase polymerase chain reaction (RT-PCR), serologic, or antigen testing. Exposure to someone who has had or is suspected of having had COVID-19 within the last 4 weeks also counts.  Patients with MIS-C may have predominately gastrointestinal symptoms, mucocutaneous findings, and may be hypotensive or “shocky” on presentation. Up to 80% require ICU admission. Thrombocytopenia and /or elevated transaminase levels can also be seen.  MIS-C vs Kawasaki DiseaseThe big issue in diagnosing MIS-C is the overlap with Kawasaki's disease and with toxic shock syndrome. Patients with Kawasaki Disease in their second week of illness often will have thrombosis, not thrombocytopenia. Whereas MIS-C usually affects school age children or adolescents, Kawasaki Disease is more commonly a problem in younger children, who have an average age of 2 years.  Kawasaki Disease is also more common in Asian children and MIS-C disproportionately seems to affect Black and Hispanic children.  Obesity seems to be another risk factor for MIS-C.  Kawasaki's Disease also has different cardiac manifestations from MIS-C. Coronary artery dilatation is common in Kawasaki's disease and left ventricular dysfunction in MIS-C, although sometimes coronary artery dilatation and rarely aneurisms can be noted on echocardiogram in putative MIS-C, which is why differentiation from Kawasaki's Disease is an issue.  PathophysiologyThe cause of MIS-C is probably postinfectious immune dysregulation. Only a minority of MIS-C patients are identified as having COVID-19 by RT-PCR, but most have positive tests for immunoglobulin G.  Statistically, there is a lag of 4-6 weeks between peak community cases of COVID-19 and the time at which children present with MIS-C.   Although research is being done on MIS-C, and theories abound about etiology, there is no clear-cut answer to why some children get MIS-C and the vast majority do not. In a review of the literature on MIS-C using literature from December 2019 through May 2020, gastrointestinal symptoms such as diarrhea, and abdominal pain were 4-5 times more common than cough and respiratory distress.  There was a slight preponderance of male patients and mean age was 8 ½ years. ICU admission was common and 2/3 required inotropic support, over ¼ needed respiratory help with extracorporeal membrane oxygenation warranted in 31 children. The death rate was 1.5 % of these very sick children treated in hospital.  In another smaller study, 80% had mild, but 44% had moderate to severe EKG abnormalities including coronary involvement. The good news was that coronary arteries were normal in all children after a month, and at 4-9 months, only 2-4% had mild heart abnormalities. Unfortunately, mechanisms of MIS-C as well as universal treatment is still being worked out. Published articles may be delayed due to time constraints in publishing. Other immunologic interventions do not have sufficient data. TreatmentWhat about the treatment of children diagnosed with MIS-C?Usually, a variety of specialists become involved initially. These can include pediatric rheumatology, infectious disease, cardiology, and hematology. If children with MIS-C meet criteria for complete or incomplete Kawasaki disease as well, regardless of COVID-19 testing results, IVIG and aspirin are reasonable.  Corticosteroid use must be individualized, and if used it may require a taper.  An echocardiogram can be done initially looking for coronary aneurisms and repeated in a week.  In severe cases, shock may be a presenting factor needing urgent attention. Generally, the treatments used are decided by the aforementioned consults and may consist of immunomodulating therapy, including possibly IVIG (2g/kg), and/or corticosteroids methylprednisolone (30mg/kg).  AntiviralsThe role of antiviral therapy is unclear and remdesivir should be reserved for children with acute COVID-19.  COVID-19 vaccination-associated myocarditisAnother entity which needs further evaluation is COVID-19 vaccination-associated myocarditis in adolescents. This problem is more common in young males and may occur after the administration of mRNA based COVID-19 vaccines. The presentation occurs within 2 weeks of COVID-19 vaccination, and clinical presentation can include chest pressure, abnormal biomarkers (elevated troponins), and cardiac imaging findings. It is unknown if subclinical cases occur.  COVID-19 infection in children, while usually benign, has the potential to become serious, and the association between some mRNA vaccines and the occurrence of myocarditis has yet to be thoroughly studied. We look forward to more and better data to guide the care of children and young adults in these spheres. The risk of having myocarditis is still higher with the actual COVID-19 than the COVID-19 vaccine. The incidence of myocarditis after BioNtech/Pfizer vaccine was 2.13 cases per 100,000 persons in a large study done in a large health care organization in Israel where more than 2 million people were vaccinated (that represents 0.00213%). Another US study showed that there were 77 cases per million doses of vaccines in young male, in contrast, there were 450 cases of myocarditis per million COVID-19 cases in the same age group.____________________________Conclusion: Now we conclude our episode number 74 “Multisystem Inflammatory Syndrome in Children.” Dr. Schlaerth explained that MIS-C is a work in progress in terms of pathophysiology, diagnosis, treatment, and prognosis. MIS-C and Kawasaki Disease are very similar, but, for example, GI symptoms, cardiac dysfunction, shock and multisystem dysfunction are more prominent in MIS-C than Kawasaki Disease. An drei von 153 Standorten der Biontech/Pfizer-Zulassungsstudie gab es Unregelmäßigkeiten. Das berichtet eine Whistleblowerin im British Medical Journal. Experten betonen aber: Zweifel an der Sicherheit und Wirksamkeit des Impfstoffes gebe es nicht. Beides sei durch andere Studien sehr gut belegt. Youngkin schlug demnach bei der Wahl am Dienstag mit 2,7 Prozentpunkten Vorsprung knapp seinen Rivalen Terry McAuliffe von der Demokratischen Partei. Die Gouverneurswahl galt als Stimmungstest für US-Präsident Joe Biden und seine Demokraten. Die Niederlage McAuliffes im bislang demokratisch regierten Virginia ist eine schwere Schlappe für die Demokraten. Der 64-jährige McAuliffe hatte in Umfragen lange klar vor Youngkin gelegen, zuletzt hatte Youngkin aufgeholt. USA starten Corona-Impfungen für 5-11-Jährige In den USA starten die Corona-Impfungen für Kinder im Alter zwischen fünf und elf Jahren: Nachdem die Gesundheitsbehörde CDC eine Impfempfehlung für diese Altersgruppe mit dem Vakzin von Biontech/Pfizer ausgesprochen hatte, wurden noch am selben Abend die ersten jüngeren Kinder geimpft. Präsident Joe Biden sprach von einem "Wendepunkt" im Kampf gegen die Pandemie. Die US-Arzneimittelbehörde FDA hatte dem Impfstoff Ende vergangener Woche eine Notfallzulassung für diese Altersgruppe erteilt. Die Regierung begann daraufhin bereits, weitere Impfstoffdosen ins ganze Land auszuliefern. IS bekennt sich zu Anschlag in Kabul Bei einem Angriff auf ein Militärkrankenhaus in der afghanischen Hauptstadt Kabul sind mindestens 19 Menschen getötet und 50 weitere verletzt worden. Dies teilte ein Vertreter des Gesundheitsministeriums der radikalislamischen Taliban mit, die seit August in Afghanistan an der Macht sind. Der afghanische Ableger der Dschihadisten-Miliz Islamischer Staat bekannte sich zu dem Anschlag. Nach der Machtübernahme der Taliban hat die mit ihnen verfeindete IS-Miliz bereits mehrfach Anschläge verübt. Äthiopien ruft landesweiten Ausnahmezustand aus Nach dem Vormarsch der Volksbefreiungsfront von Tigray hat Äthiopiens Zentralregierung für sechs Monate einen landesweiten Ausnahmezustand ausgerufen. Er gibt den Behörden unter anderem die Möglichkeit, Straßensperren zu errichten sowie Verkehrs- und Kommunikationsverbindungen zu unterbrechen. Auch die Übernahme der Verwaltung durch das Militär in bestimmten Bereichen ist damit möglich. Zuvor hatten die Behörden der Hauptstadt Addis Abeba die Bewohner zu den Waffen gerufen. Die Rebellen hatten zuletzt die Einnahme zweier strategisch wichtiger Städte gemeldet und mit einem Vormarsch auf die Hauptstadt gedroht. Facebook gibt Gesichtserkennungs-Funktion auf Facebook schafft die Gesichtserkennungs-Funktion ab, mit der Nutzer automatisch in Fotos markiert werden konnten. Gespeicherte Daten zur Identifizierung der Gesichter von mehr als einer Milliarde Menschen werden gelöscht, wie das Online-Netzwerk ankündigte. In der Gesellschaft gebe es Sorgen um den Einsatz von Gesichtserkenungs-Technologien - und Regulierer seien immer noch dabei, Regeln dafür zu entwickeln, hieß es zur Begründung. Facebook sehe aber weiterhin Einsatzmöglichkeiten für die Technologie etwa für die Anmeldung bei einem Account oder das Entsperren eines Geräts. Niederländer müssen wieder Masken tragen Die Niederlande haben wegen steigender Ansteckungszahlen die Corona-Maßnahmen erneut verschärft. Regierungschef Mark Rutte kündigte in Den Haag unter anderem eine Ausweitung der Maskenpflicht an. Auch der Corona-Pass wird für mehr Orte verpflichtend. Bürger sollen wieder den Sicherheitsabstand von 1,5 Meter wahren. Erst vor gut einem Monat hatte das Land viele Maßnahmen aufgehoben und etwa die 1,5 Meter-Regel abgeschafft. Daraufhin waren die Infektionszahlen schnell gestiegen. Bayern und Juve erfolgreich bei der Champions League Qualifikation Bayern München hat dank eines überragenden Robert Lewandowski frühzeitig und locker das Minimalziel Achtelfinale in der Champions League erreicht. Bei der Rückkehr des zuletzt an Corona erkrankten Trainers Julian Nagelsmann gewann der deutsche Fußball-Rekordmeister gegen Benfica Lissabon klar mit 5 : 2. Auch Juventus Turin konnte sich vorzeitig qualifizieren. Der italienische Fußball-Rekordmeister setzte sich gegen Zenit St. Petersburg mit 4 : 2 durch.
