Dr Van Spall is a Canadian Institutes of Health Research-funded clinical trialist and researcher with a focus on heart failure, health services, and health disparities.The Decipher the Guidelines: 2022 AHA / ACC / HFSA Guideline for The Management of Heart Failure series was developed by the CardioNerds and created in collaboration with the American Heart Association and the Heart Failure Society of America. It was created by 30 trainees spanning college through advanced fellowship under the leadership of CardioNerds Cofounders Dr. Amit Goyal and Dr. Dan Ambinder, with mentorship from Dr. Anu Lala, Dr. Robert Mentz, and Dr. Nancy Sweitzer. Question #32 Palliative and supportive care has a role for patients with heart failure only in the end stages of their disease. TRUE FALSE Answer #32 Explanation The correct answer is False Palliative care is patient- and family-centered care that optimizes health-related quality of life by anticipating, preventing, and treating suffering and should be integrated into the management of all stages of heart failure throughout the course of illness. The wholistic model of palliative care includes high-quality communication, estimation of prognosis, anticipatory guidance, addressing uncertainty, shared decision-making about medically reasonable treatment options, advance care planning; attention to physical, emotional, spiritual, and psychological distress; relief of suffering; and inclusion of family caregivers in patient care and attention to their needs during bereavement. As such, for all patients with HF, palliative and supportive care—including high-quality communication, conveyance of prognosis, clarifying goals of care, shared decision-making, symptom management, and caregiver support—should be provided to improve QOL and relieve suffering (Class 1, LOE C-LD). For conveyance of prognosis, objective risk models can be incorporated along with discussion of uncertainty since patients may overestimate survival and the benefits of specific treatments – “hope for the best, plan for the worst.” For clarifying goals of care, the exploration of each patient's values and concerns through shared decision-making is essential in important management decisions such as when to discontinue treatments, when to initiate palliative treatments that may hasten death but provide symptom management, planning the location of death, and the incorporation of home services or hospice. It is a Class I indication that for patients with HF being considered for, or treated with life-extending therapies, the option for discontinuation should be anticipated and discussed through the continuum of care, including at the time of initiation, and reassessed with changing medical conditions and shifting goals of care (LOE C-LD). Caregiver support should also be offered to family members even beyond death to help them cope with the grieving process. A formal palliative care consult is not needed for each patient, but the primary team should exercise the above domains to improve processes of care and patient outcomes. Thankfully she delivered a healthy baby via C-section. She has no other past medical history and is currently on losartan 25 mg daily and metoprolol succinate 200 mg daily.  She has been asymptomatic. Which of the following statements is recommended to medically optimize Ms. Augustin prior to her 2nd pregnancy? A No medical optimization or preconception planning is needed as her 1st pregnancy resulted in a healthy infant. B Discontinue losartan and metoprolol with no other needed pregnancy planning  C Change her medication regimen, consider repeat TTE, and provide patient-centered counseling regarding risk of a future pregnancy D Continue losartan and metoprolol and advise against repeat pregnancy  Answer #16 Explanation The correct answer is C – change her medication regimen, consider repeat TTE, and provide patient-centered counseling regarding risk of a future pregnancy. Heart failure may complicate pregnancy either secondary to an existing pre-pregnancy cardiomyopathy or as a result of peripartum cardiomyopathy. In women with history of heart failure or cardiomyopathy, including previous peripartum cardiomyopathy, patient-centered counseling regarding contraception and the risks of cardiovascular deterioration during pregnancy should be provided (Class I, LOE C-LD) Peripartum cardiomyopathy (PPCM) is defined as systolic dysfunction, typically LVEF < 45%, often with LV dilation, occurring in late pregnancy or early postpartum with no other identifiable etiology. PPCM occurs worldwide, with the highest incidences in Haiti, Nigeria, and South Africa. Other clinical risk factors include maternal age > 30 years, African ancestry, multiparity, multigestation, preeclampsia/eclampsia, anemia, diabetes, obesity, and prolonged tocolysis.   The pathogenesis of peripartum cardiomyopathy is complex and it is likely a multifactorial process.  The combination of hemodynamic changes of pregnancy, inflammation of the myocardium, hormonal changes, genetic factors, and an autoimmune response have all been proposed as possible mechanisms and these may certainly be interrelated. 60 years ago, 27-year-old Russian cosmonaut Yuri Alexeivich Gagarin was strapped into a Vostok capsule and launched 188 miles out into space. On a spring morning at 06.07 am UT on April 12th 1961, ascending slowly from a Baikonur cosmodrome launch-pad set in the Kazakhstan steppes, he was to make history. Gagarin became the first ever human being to leave the Earth's surface and experience a new environment in the zero-gravity of the cosmos. He completed what was very nearly a full orbit of the Earth and travelled at almost 18,000 mph, moving around the planet at an extraordinary 5 miles a second. Nick Spall tells the intriguing story of the human and technological achievements of Yuri Gagarin’s historic flight. Nick Spall presented his paper to the RAeS Space Group Conference, Yuri Gagarin’s legacy, 50 years on: securing the vision for the next half century. Terry talks about how his presidential year has meant less travelling, but more opportunity for engagement with members and how inspired he has been seeing so many engineers step up to address COVID. Colin chats about the effect working from home has had on the IMechE staff, but how beneficial it has been bringing members from across the world together through virtual services. Terry shares his thoughts on the new Governance Benchmark and how important that process has been for the Institution. Terry recognises the valuable input of all the members who have been committed to bringing about change over the last few years; including Past President Carolyn Griffiths, Implementation Group Chair Kerry Mashford and members of the Trustee Board. In a look back at the 2019 annual report, Colin talks about his hope for 2020 being our ‘Year of Values' and how members have addressed behaviours and culture in this new working paradigm. Colin highlights the importance of diversity and how all engineers need to feel that they can make a contribution to society. Thinking about 2021, Terry looks forward with enthusiasm to the Institution's future, and hopes more members will want to get involved in driving the IMechE's strategies. His hope is to finally shake the hands of all those who have worked to implement change in the last few years. Terry also looks forward to work commencing on the IMechE building next year, which will ensure members have a historical yet contemporary home for the coming decades. We could not end the interview without mentioning Colin's intention to step down as CEO in May 2021. It’s Saturday night at Bissell Mansion, a nearly 200-year-old home overlooking Interstate 70 in north St. Louis. Dozens of people have gathered together to enjoy a meal — and a murder mystery. Over the course of the evening, dinner theater actors Linda Spall and Charles Huevelman put on a raucous show, “Phantom of the Grand Ole Opry,” in which Spall plays two characters, Tammy Whino and Dolly Pardon, and Huevelman portrays a very drunk Kenny Rogeers. The rest of the cast has been drafted from among audience members, who were each assigned a role as they arrived and provided with a partial script outlining that particular part in the story. By the time salads have been served, one of the characters has been murdered, and it’s up to a wild bunch of country stars to apprehend the killer. The whole scene is one that’s been a fixture at Bissell Mansion for more than three decades and continues to draw a crowd. In this conversation, longtime murder mystery actor John Vullo joins host Sarah Fenske to talk about the goings-on at Bissell and provide a closer look at what it is that keeps diners returning there for more comedic mayhem.

Let’s discover ways to use our favorite Google tools to level the playing field and make differentiation a reality in your classroom. This student-centered teaching strategy helps us reach all learners. G Suite tools help teachers and students communicate and manage multiple assignments with ease. Listen closely for step-by-step instructions, implementation ideas, and some Google tips that will save you time and reap many benefits for you and your students. I hope this episode inspires you to dig deeper into differentiated instruction, and how to use the tools you have available to support all of the learners in your classroom. In This Episode: [00:28] - Kasey welcomes listeners to the show and reveals today’s topic: Digital Differentiation — with G Suite Tools. Provide Flexible Learning Paths Develop Future-Ready Skills Provide an Effective Way to Use Digital Tools FOR Learning (not just an end product) [09:31] - Kasey breaks down the different Google Tools that teachers can use to support differentiation, including Google Classroom, Google Forms, and Google Slides. Differentiating Assignments in Google Classroom [10:13] - Google Classroom allows teachers to send assignments to only selected students rather than the whole class. [11:14] - Kasey provides some tips for using Google Classroom in conjunction with differentiated assignments (full blog post). Tips for Naming Differentiated Assignments in Google Classroom Always number and use consistent naming conventions For differentiated assignments, consider a group or individual identifier in the assignment title. (Ex: Group 4, Special Assignment, Extra Credit, EC Sarah Jones, or come up with your own consistent naming convention.) Remember the name of the assignment names Drive files. Tips for Differentiated Assignment Directions in GC Get specific with your directions! The more information that is available in Google Classroom, the fewer excuses students have for not completing the assignment and the fewer interruptions you are likely to have. Attach a rubric if you have one! Resource: How to Package Your Digital Assignments Tips for Group Differentiated Assignments in GC Designate a group leader in the assignment. The group leader is responsible for creating any new files and “turning in.” Consider asking students to submit a reflection at the end of group work to help you get the whole picture of the learning process. Student Choice Assignment Tips Number the assignment and detail the choices in the directions and/or in an attached (view only) document. (Note: This is also great way to use digital learning menus and choice boards.) Attach links to digital tools that may be offered as a choice for completing the assignment. (Example: Using one of the digital tools below, create a digital story retelling, “The Tell-Tale Heart,” in your own words.) Long Term Projects, Modifications, or Enrichment For long-term projects, special projects like Genius Hour, or for students with special modifications, you may consider creating a separate “Class.” (Note: This is really only worth the time if these groups are long-term, or reused.) (For more ideas, listen to episode 5 of The Google Teacher Tribe.) Using G Suite to Create Interactive Choice Boards [15:43] - G Suite is the perfect companion to create digital choice boards. Learn more in Episode 8, and get the FREE eBook! [16:34] - Kasey talks about creating choice boards in G Suite. Choice Board Templates and Resources Novel Study Tic-Tac-Toe Chrome PD Tic-Tac-Toe Tommy’s Spall’s Student Creation Menu Amber Teamann and Melinda Miller’s PD BINGO Interactive Learning Menus Blog Post Laurie Westphal’s books on Learning Menus How to Differentiate Questions in Google Forms [19:18] - We dive into Google Forms and how you can differentiate questions (full blog post) and allow students to review content, go on a Choose Your Own Adventure, or even have the questions get progressively more difficult. [21:16] - Kasey provides an example of how to use Google Forms to create a branching assessment or review. [25:00] - Google Forms can be very complicated. Kasey offers great tips to make your forms successful and effective teaching tools. Google Forms Branching Tips Start small, just a few questions. Map out your branches before you begin. Utilize the page titles and descriptions to help you organize. Test it! Then, test it again! Remember, this type of assessment will not be graded--Assessment FOR Learning! Your spreadsheet will not be pretty! But that’s okay! Kids are smart! You must facilitate this type of assessment.   Other Ways to Use Branching in Google Forms When a student answers correctly, they could be taken to a harder question. When a student answers incorrectly, you could show more specifically where they went wrong with each answer choice (not just review a concept)–like solving a math problem incorrectly. Student choice: Use this feature to allow students to select from a menu of choices to demonstrate their learning, then upload the file to the form to submit. The spreadsheet would allow you to see what they select and should be working on during class. Gamify! What if every correct answer revealed a clue or a puzzle piece? Students could collect and work individually or collaboratively to put together. Put this feature in students’ hands and let them create something for their class, or another class to teach a skill, or play a game. [20:16] - A friend of Kasey’s, Sylvia Duckworth, created a Choose Your Own Adventure form for her French class that allowed students to create their own ending to a story! Differentiate with Google Slides [30:08] - We can’t leave out Google Slides, the Swiss army knife of G Suite! Kasey gives tips on how to use slides to create interactive assignments. Our Halloween special, marking two years of our podcast, looks at Ken Russell's 1986 "Gothic". During 1816, volanic ash created a Year Without A Summer, with many cold storms keeping people indoors. Among those losing their minds in a Swiss villa, were bad-boy Lord Byron and SJW Percy Shelley. A writing contest among the guests led to the inspiration for Mary Shelley's Frankenstein and Polidori's The Vampyre, paving the way for a century of flourishing Gothic horror literature. That is what the opening and conclusion of this movie is about. Everything in-between is a discordant nightmarish phantasmagoria of steampunk porcelain robots, eye-boobs and "nudey Nick-Nacks". Jonathan gets into some context for what actually happened to the people depicted while Richard notes comparisons with Darren Aronofsky's Mother! And the latest Bond news reveals an upcoming biopic of Nick-Nack actor Hervé Villechaize, where Peter Dinklage plays him and it actually exists. Happy Halloween.

Two very different movies - one a historical drama directed by Mick Jackson of the David Irving libel case, and Nacho Vigalondo's curious horror rom-com Colossal - that feel very close to home today.  Rachel Weisz plays Deborah Lipstadt, a Holocaust historian who is sued in London by David Irving (Timothy Spall) for identifying him as a denier of the genocide of Jewish people in German concentration camps.  Denial also stars Andrew Scott and Tom Wilkinson and has a great deal of resonance to current events given its focus on freedom of speech and the abuse of same.    Colossal is an original and inventive picture by Spanish director Nacho Vigalondo that stretches from South Korea to rural America. This international production mashes up Kaiju monsters with the tropes of Hollywood romantic comedy and then serves up something genuinely startling.  While this vehicle of Anne Hathaway has been described as a metaphor for addiction, there is far more at work here. Stevie and Emmet dance around potential spoilers while trying to sketch out exactly why you should see this curious and powerful film.  Also Emmet tries to suggest a tagline for the film 'The personal is Colossal.'

Next, the panel goes from Tatars to Turners, with Mike Leigh's 2014 feature film, MR. TURNER. 2014 saw an onslaught of biographical movies about troubled-geniuses (See: THE THEORY OF EVERYTHING and THE IMITATION GAME--Or better yet, don't). But MR. TURNER somehow manages to stand out above the fray. The panel talks about how Leigh was able to avoid all the tired "based on a true story" tropes, while still conveying what made JMW TURNER's work so extraordinary.
