Abstract: Mice with cardiac-specific overexpression of adenylyl cyclase (AC) type 8 (TGAC8) are under a constant state of severe myocardial stress and have been shown to have a remarkable ability to adapt to this stress. However, they eventually develop accelerated cardiac aging and cardiac fibrosis, and experience reduced longevity. Here we show that young (3-month-old) TGAC8 animals are characterized by a broad and extensive inflammatory state, that precedes the development of cardiac fibrosis. We demonstrate that activation of ACVIII in the cardiomyocytes results in cell-autonomous RelA-mediated NF-{kappa}B signaling. This is associated with non-cell-autonomous activation of proinflammatory and age-associated signaling in myocardial endothelial cells, increases in serum levels of inflammatory cytokines, changes in myocardial immune cells, and changes in the size or composition of lymphoid organs. Finally, we provide evidence suggesting that ACVIII-driven RelA activation in cardiomyocytes might be mediated by calcium-Protein Kinase A (PKA) signaling. Our findings highlight a novel mechanistic connection between cardiomyocyte stress, myocardial para-inflammation, systemic inflammation, and aging, and therefore point to novel potential therapeutic targets to reduce age-associated myocardial deterioration. About 6 months after the twins were born I began working with a functional medicine doctor to reduce systemic inflammation in my body and get my C-reactive protein (CRP) to an "optimal" level.  In today's episode, we're talking about how systemic inflammation can present, what CRP is, what is considered an "optimal" level and how I'm getting there.  At 6 months postpartum, my CRP was about 10 mg/L and 4 months later it's down to 3.6 mg/L. Here in Wisconsin, we had one of the worst air quality index levels ever this past week. Smoke from Canada's wildfires hung in the air, creating a thick haze. The smoke didn't bother me, but I assumed that some people would be extra sensitive to it. As the day progressed, I started thinking of ways people might protect themselves from the effects of wildfire air pollution, including the use of supplements or essential oils and other simple choices. I also wondered, of course, how much of the hysteria about the air quality index was created by simply having an "Air Quality Index" to look at. It would be like having an app that notifies you of each crime as it happens in Minneapolis. Nobody would want to go there. We don't watch any mainstream media or local news, but I can imagine it was one of the top stories in the news cycle, creating unnecessary fear. While Canada's wildfires might be at record levels, it isn't the first time in history that people have breathed in polluted air. But we've been conditioned to fear a lot more about everyday life in recent years. It's possible our adoption of a "safety culture" has made us more likely to freak out from a day of smoky air, whereas in the past we'd carry on with life without giving it much thought. Whichever the case, there are some things you can do to keep your lungs healthy in the midst of higher levels of smoke. Use this as a guide when facing a similar circumstance as we were this past week, and remember that I'm referring to occasional exposure to poor air quality from wildfire smoke far away. I'm not referring to how to approach things if you're right next to the fire. Wildfire Air Pollution and PM2.5 A raging wildfire, fueled by dry conditions and strong winds, sends billowing plumes of smoke into the atmosphere. This smoke isn't just a nuisance; it's a complex mixture of gases and fine particles that can have a significant impact on air quality, both locally and hundreds, even thousands, of miles away.Liu JC, Pereira G, Uhl SA, Bravo MA, Bell ML. A systematic review of the physical health impacts from non-occupational exposure to wildfire smoke. Environ Res. 2015;136:120-132. The primary concern regarding wildfire smoke is something known as PM2.5. This term refers to particulate matter that measures less than 2.5 micrometers in diameter. For context, that's approximately 3% the diameter of a human hair."Particulate Matter (PM2.5) Trends". U.S. Environmental Protection Agency. 2023. Their minuscule size allows these particles to stay aloft for long periods, infiltrating urban and rural areas alike. You might wonder, "These particles are tiny. How much harm can they really do?" Don't let their size fool you. Abstract: The EBI2 receptor is a major modulator of innate immunity and, together with its ligand, oxysterol 7,25OHC, has been implicated in several neuroinflammatory and autoimmune diseases. 7alpha,25OHC is synthesised from cholesterol with CH25H and CYP7B1 enzymes and degraded with HSD3B7. The concentration of 7alpha,25OHC in the brain increases in the early phases of the murine model of multiple sclerosis, leading to an enhanced central nervous system (CNS) infiltration with EBI2-expressing lymphocytes. Here, we aimed to investigate whether the enzymes involved in the synthesis and degradation of 7alpha,25OHC are expressed directly in the mouse brain microvascular cells and whether systemic inflammation modulates their levels in these cells. Normal mouse brain capillaries were isolated and immunostained for EBI2, CH25H, CYP7B1 and HSD3B7. Subsequently, mice were challenged with lipopolysaccharide and the mRNA expression in whole brain homogenates was measured. Changes in the receptor and enzyme levels were quantified directly in endothelial cells (ECs), pericytes and astrocytes. The data indicated high levels of EBI2 in the brain microvascular ECs, pericytes and astrocytes with the highest co-localisation in pericytes. CH25H was detected in ECs and astrocytes with low levels in pericytes. CYP7B1 was moderately expressed in ECs, astrocytes and pericytes. The 7alpha,25OHC degrading enzyme HSD3B7 was the least detected in astrocytes. Moreover, the data indicated that systemic inflammation downregulated the mRNA levels of Ebi2 and upregulated Ch25h expression in the whole brain. Specifically in each cell type, EBI2 was not induced in ECs but increased in astrocytes and pericytes. CH25H levels increased in astrocytes and pericytes and CYP7B1 in ECs and astrocytes. The degrading enzyme, HSD3B7, was least affected by systemic inflammation. Taken together, we here demonstrate that EBI2 and the enzymes regulating its ligand levels are differentially expressed in mouse brain microvessels and are highly modulated by systemic inflammation. Upregulated concentration of 7alpha,25OHC in the brain during inflammation may lead to an increased migration of EBI2-expressing immune cells into the CNS during infection or neuroinflammatory disease. Modulation of the EBI2/oxysterol system in the brain or directly in the brain blood vessels may thus provide a new approach to treating neuroinflammatory diseases including multiple sclerosis. Dr. Pape was evidence based and patient centered before these were buzz words and before the internet made it easy to collaborate. It was like being on a chiropractic island and taking trips with different professions to iron out what he was seeing work. What is the reason that some doctors blame the patient when they don't get the results that the doctor assumes you would get? Should this be taught in schools with diagnosis and student clinic so they can apply these concepts (not cookie cutter programs)? This can help bridge the gap from being excellent at diagnosis and bridging all the techniques you know with practical application based on what the patient presents. Test - Treat - ReTest Are they hyper-mobile?Do they have Systemic Inflammation? (abdominal sub-classification)What's their motor control with dynamic stability like?Regional interdependence also plays a part We really are missing key components to injury and relief if we only focus on the area of chief complaint. When asking about ‘magical' areas that tend to be overlooked... what is his answer? Can we say mechanical dysfunction and drop the ‘subluxation' word? Relationship Advice: When you are having important conversations, slow it down and really listen and actively listen to fully understand what the other person is saying. Abstract: Background: Alzheimer's disease (AD) is the primary cause of cognitive deficit in elderly humans. Late-onset AD (LOAD) is sporadic, multifactorial, non-Mendelian accounting at present for 95% of the cases in contrast to the genetic form. Risk factors for sporadic AD include Gene: Environment interactions. There is increasing evidence that lifestyle and environmental stress such as infection and chronic inflammation are underlying culprits of neurodegenerative dementia. To date, very few mouse models reproduce the pathophysiological progression of sporadic AD, while the majority of studies have employed transgenic animals reproducing the familial form. Methods: We have re-engineered the Polyinosinic:polycytidylic acid (PolyI:C) sterile infection model in wildtype C57Bl6 mice to obtain chronic low-grade systemic inflammation. We have conducted a cross-sectional analysis of aging PolyI:C and Saline control mice (3 months, 6 months, 9 months and 16 months), taking the hippocampus as a reference brain region, based on its vulnerability, and compared the brain aging phenotype to AD progression in humans with mild AD, severe AD and Controls (CTL). Results: We found that PolyI:C mice display both peripheral and central inflammation with a peak at 6 months, associated with memory deficits. The hippocampus is characterized by a pronounced and progressive tauopathy. In PolyI:C brains, microglia undergo aging-dependent morphological rearrangements progressively adopting a phagocytic phenotype. Transcriptomic analysis reveals a profound change in gene expression over the course of aging, with a peak in differential expression at 9 months. We confirm that the proinflammatory marker Lcn2 is one of the genes with the strongest upregulation in PolyI:C mice upon aging. Validation in brains from patients with increasing severity of AD shows a general tendency of the genes to decline except for GFAP. Conclusions: The PolyI:C model of sterile infection demonstrates that peripheral chronic inflammation is sufficient to cause neuropathological processes resembling a mixed-AD, with progressive tau hyperphosphorylation, changes in microglia morphology, and gene reprogramming reflecting the increased neuroinflammation and the loss of neuronal functionality seen to some extent in humans.
