While almost all students declined academically, a report from The Center for Learner Equity found that students with disabilities were especially impacted. Students with disabilities often face wide and persistent achievement gaps. With the more than 7 million young people with disabilities in the United States, it begs the question of what more we can do to support them. What challenges do students with disabilities face? How can we create more inclusive spaces and services for these students to thrive? And how can we help our schools be more equitable and supportive when it comes to special education? "Is artificial intelligence advancing too quickly?" 60 Minutes warns. "BuzzFeed CEO says AI may revolutionize media, fears possible 'dystopian' path," CBS News tells us. "TV and film writers are fighting to save their jobs from AI. They won't be the last," CNN reports. Over and over, especially in recent months, we hear this line: AI is advancing so fast, growing so sophisticated, and becoming so transformative as to completely reshape the entire economy to say nothing of our shaky media landscape. In some cases, those in the press deem this a good thing; in others, a bad thing but in terms that get the problem all wrong. But virtually all media buy the basic line that something big and transformative isn't just coming, but is in fact already here. Obviously, we can't predict the future, but we can comment on the present. Yes, AI platforms can generate low-level marketing copy, pro forma emails, and shitty corporate art. But progress in these capacities does not, as such, portend a radical advancement into actual human intelligence and creativity. Meanwhile, there's little to no evidence to support the claim that AI, namely large language models like ChatGPT, actually can perform – or even intervene to save time performing – any type of high-level writing craft, journalism, fiction, screenwriting, and a host of “creative” production. So why do we keep hearing otherwise? What purpose does this type of religious-like providential thinking serve? And who stands to benefit from the vague sense of a future of AI-written essays, articles, and scripts, no matter how terrible they may be? In this episode, we explore media's current Inevitability Narrative, namely its credulous warning that ChatGPT is about to do the work of media and entertainment professionals, examining the ways in which this narrative, despite the evidence to the contrary, serves as a constant, implicit threat to workers and a convenient pretext for labor abuses like wage reduction, layoffs, and union-busting. We also review how this media hype works to obscure the very real, banal harms of AI, such as racism, surveillance, over policing and lack of accountability for the powerful. Our guest is Rutgers professor Dr. Lauren M.E. Goodlad.

The number of ships waiting to enter the biggest U.S. gateway for trade with Asia reached the highest since the pandemic began, exacerbating delays for companies trying to replenish inventories during one of the busiest times of the year for seaborne freight. The port problem first became noticeable in the Port of Los Angeles and the Port of Long Beach, the ports where the majority of shipments from Asia are received. Other ports around the USA have also seen delays and disruptions. The port problem was caused by the following factors: Increased demand by US consumers who went on a buying spree. Because restaurants were closed, vacation travel restricted, and many big purchases delayed because of pandemic-driven shortages – bored, house-bound consumers bought a lot of stuff that had to be made in Asia and shipped to America's West coast ports (Los Angeles and Long Beach). Consumer spending also received a bump from government money that was generously distributed during the first year of the pandemic. While American consumers went shopping, particularly online, the Asian factories and supply chains that produce the goods were broken due to labor and materials shortages. Access to vaccines, medical care, and family issues contributed to the shortages. Labor problems in the USA exacerbated the problem as people left the workforce in large numbers due to COVID infection, money from the government, homeschooling of children, and caring for family members. The port congestion may have also been caused by a lack of digitization and technology at the ports. The National Shipper Advisory Committee (NSAC) is a team made up of both public and private sectors organizations tasked with solving the port problems. The NSAC is led by the Federal Maritime Commission (FMC), which is the independent federal agency responsible for regulating the U.S. international ocean transportation system for the benefit of U.S. exporters, importers, and the U.S. consumer. The NSAC is actively working to identify and eliminate bottlenecks. The NSAC is also working on a number of initiatives to improve the ports including: demurrage/detention standards, data standards, transparency, and digitization. Episode 26: Eye KnowThe sun rises over the San Joaquin Valley California. Today is September 4, 2020.It should be not surprise to us that evidence shows the use of marijuana during pregnancy affects the development of the nervous system of the fetus. More than 500,000 live births were analyzed retrospectively from the Canadian birth registry and it showed incidence of autism spectrum disorder (ASD) was higher in children born from mothers who used marijuana during pregnancy compared with non-exposed children (4 versus 2.4 diagnoses per 1000 person-years). Incidence of intellectual disability and learning disorders was also higher in marijuana-exposed children. So, remember to counsel your pregnant patients to avoid marijuana[1].Do you think that patients with obesity have a higher prevalence of musculoskeletal pain? You think? And what’s a common prescription for chronic pain? Yes, you guessed it, it’s opioids. So, you think obesity and opioids are linked? Articles published in the American Journal of Preventive Medicine (AJPM) [2] and Journal of American Medical Association (JAMA)[3] showed a clear link between obesity and opioid use. Patients who are overweight have 24% incidence of long term opioid use, while the incidence in patients with severe obesity was 158%. Again, incidence is 24% in overweight vs 158% in severe obesity. That’s crazy, the most common chronic pain associated with obesity and opioid use was back pain and joint pain. My name is Fermin Garmendia, I am a third-year resident of the Rio Bravo Family Medicine Residency Program. I was born and raised in Cuba where I went to medical school. I came to the US in 2010 and, after several years and some sacrifice, my dream came true. For me, being a family medicine doctor is a privilege. It is diverse and challenging. I have some hobbies, I like watch movies, eat in a good restaurant, passing time with friends but what I enjoys the most it is travel by car, so far yet a short family, my wife, our dog and me. We like to explore and be in several places and California is the opportunity, it is beautiful. I still have a big list of places to visit. Question number 2: What did you learn this week? This week I saw a patient with a subconjunctival hemorrhage. I can picture the face of some colleagues... it is nothing weird or maybe not the most interesting topic, but for some patients (and even for many doctors) this could be frightening. Patients get desperate when they realized the problem, and often those who see someone with subconjunctival hemorrhage may think this is caused by physical trauma. It is common, I have seen many patients with subconjunctival hemorrhages and almost always the treatment is reassuring him or her that it will gradually resolve on its own in few weeks, no need for any treatment, except for some artificial tears for symptomatic relief. We should explain our patients why this event could have happened. This is the interesting topic that I would like to talk about.Subconjunctival hemorrhage Patients are generally asymptomatic. Typically, the patient is unaware of the problem until they look in the mirror or someone else lets them know.A red, bloody eye can look scary, but it is usually harmless and often heal on its own.Causes of subconjunctival hemorrhageThe eye’s conjunctiva contains a lot of tiny blood vessels that can break easily. Rupture of capillaries may happen spontaneously or with Valsalva effect caused by coughing, sneezing, straining, or vomiting (this is because they briefly raise blood pressure in veins and can cause capillaries to break) and trauma, even rubbing your eyes too hard can cause capillaries to break. Less common causes include: Diabetes, HTN, COPD that makes patient cough often; medications that can make you bleed easily such as aspirin or blood thinners like coumadin. Diagnosis of subconjunctival hemorrhageThe diagnosis is confirmed by having a normal vision and the absence of discharge, photophobia, or foreign body sensation.The blood is typically reabsorbed over one to two weeks, depending on the amount of blood. Treatment of subconjunctival hemorrhageNo specific therapy is indicated, but If subconjunctival hemorrhage is recurrent or if the patient has a history of bleeding disorder or blood dyscrasia, or if the patient is on anticoagulant therapy, then an underlying hematologic or coagulation abnormality must be considered.I recommend that we should examine the patient always with a slit lamp or magnifying glass, and stain the eye with fluorescein if you suspect trauma to see any associated corneal injury, such as a corneal laceration or abrasion, or other structures of the eye, especially in patients who wear contact lens.Referral to OphthalmologyAn ophthalmologist should be consulted for the following patients: Suspicion of a leaking eye or intraorbital penetrationSigns of traumatic hyphemaConjunctival lacerations >1 cm in length that will require suturingForeign bodies that are deeply embedded, subconjunctival, or associated with a conjunctival laceration____________________________Speaking Medical: Amaurosis fugax by Xeng Xai Xiong, MS3The medical term for the day is amaurosis fugax. I don’t know about you, but for me this word sounds like a mystifying spell that can wake up the dead. Amaurosis fugax [Suspense sound effect] comes from the Greek "amaurosis," which means dark, and the Latin "fugax," which means transient. Therefore, it refers to a transient loss of vision in one or both eyes. Aww, I thought this word would have a deeper meaning. I first encounter this word, during my internal medicine rotation when a patient presented with weakness on one side of the body, facial drooping, and a transient loss of vision, aka amaurosis fugax. One of the causes of amaurosis fugax is a thromboembolism that blocks the ophthalmic artery and retinal arterioles which results in transient loss of vision. This symptom can be seen in a patient with stroke. Until next time, remember the word amaurosis fugax [Suspense sound effect]. _______________________________________Speaking Medical: Anisocoriaby Li Liang, MS3Hello, I’m Li, a 3rd-year medical student, and I want to introduce another medical word of the week, Anisocoria. I will give you a brief introduction of anisocoria and soon I will expand on this topic. We have three levels of definition depending on how hungry your brain is. First level, anisocoria is when a patient has unequal pupils. The second level of defining anisocoria is by explaining the etymology of the word. Anisocoria comes from the Greek “An”: Not, “iso”: equal, “cor”: Pupil of the eye, and the Latin: “ia”: Disease, pathology or abnormal condition. Our last level as well as the medical definition of anisocoria is defined as an impaired pupillary dilation (parasympathetic nervous system) or constriction (sympathetic nervous system). There is more than meets the eye, and finding which pupil is the abnormal one may be challenging. What can give you clues is history of ocular trauma, old photos (ptosis, ocular deviation, chronic anisocoria), topical medications, drug/toxin exposures, associated ocular and neurological signs and symptoms. I will teach you how to tell which eye and pupil is abnormal. There are some physical evaluations to differentiate which eye is considered the bad eye because it’s not always the “small eye”. You will most likely use this term when you want to ask if the patient has their uterus vs if they’ve had a hysterectomy. You can say “Señora, ¿todavía tiene su matriz?” which means: “ma’am, do you still have your uterus?”. Another instance in which you may hear the word matriz is when asking about uterine cancer, you can ask “¿Ha tenido cáncer de matriz?” “Have you hade uterine cancer?” Instead of matriz, you can also use the more formal term útero but most people will understand you better when you say matriz.Now you know the Espanish word of the week, matriz.____________________________For your Sanity: What is civilization?by Lisa ManzanaresSomeone once asked anthropologist Margaret Mead what she considered to be the first evidence of civilization. She answered: a human thigh bone with a healed fracture found in an archaeological site 15,000 years old. Why not tools for hunting? or religious artifacts? or primitive forms of communal self-governance? Mead points out that, for a person to survive a broken femur, the individual had to have been cared for long enough for that bone to heal. Others must have provided shelter, protection, food and drink over an extended period of time for this kind of healing to be possible. "The fact of tears not being shed at a very early age from pain or any mental emotion is remarkable, as, later in life, no expression is more general or more strongly marked than weeping." pg 154 EEMA Charles Darwin. In this episode we explore Chapter VI: Suffering and Weeping where Darwin describes in excruciating detail the muscles involved in the crying face of infants and how tears are formed in the lacrimal glands. This chapter is the first one in the book to use photographic images to represent the expression of the emotions Darwin is describing. We discussed Plate 1 in detail and it can be seen here:Sarah discussed Duchenne's original photograph that Darwin showed people to see if they could recognize the emotion based upon the stimulation of specific facial muscles. In the 3rd Edition of the book, Ekman includes the original photograph that was not actually in the earlier editions of the book.James discussed his amazement on where in his body tears are made, and where they go ultimately. The lacrimal glands reside above the eye and the tear duct, where James erroneously thought tears came from, is the location where tears drain into the back of your throat. Ever read through a study and wondered how to apply the hazard ratio, or if you should change your practice because of a secondary endpoint finding? In this episode, Lauren M. Catalano, MD, of the University of Pennsylvania, Philadelphia, explains all the common terms and why they matter in the context of the KEYNOTE-024 trial. In Clinical Correlation, Ilana Yurkiewicz, MD, of Stanford (Calif.) University, talks about how to prepare for an unexpected bad outcome. Practice points: Don’t skip over the statistical analysis portion of a paper. Use Google to find simple definitions for unfamiliar biostatistics terms. Understanding the statistical elements is essential to determining the quality of the research. * * * Understanding statistics in the context of KEYNOTE-024 Article discussed: Updated analysis of KEYNOTE-024: Pembrolizumab versus platinum-based chemotherapy for advanced non–small cell lung cancer with PD-L1 tumor proportion score of 50% or greater. J Clin Oncol. 2019 Mar 1;37(7):537-46. Primary endpoint: The outcome that is necessary to ensure the efficacy of the trial. What is the study’s objective? The primary endpoint is defined prior to starting the study, which influences how many patients need to be enrolled to ensure statistical significance. This paper’s primary endpoint: Time since random assignment to disease progression or death. Secondary endpoint: These are interesting trends or observations that the investigators were able to determine, but for which the original study may not have been powered, meaning that they may not have enough data to determine the statistical significance. This paper’s secondary endpoints: Objective response rate (confirmed complete and partial responses) and safety. Hazard ratio: “Ratio” suggests that this is a comparison between the intervention and control arm. HR is a measure of an effect or intervention on the outcome of interest over a period of time (risk per unit of time). The outcome can be positive or negative. Confidence interval: Since it is not possible to survey the entire population, a confidence interval provides a range of values where the true value most likely falls. If the confidence interval crosses “1” then there is no difference between the arms of the study. Kaplan-Meier curve: Often used to illustrate survival. This is a graphical representation of hazard ratio, usually drawn as a step function. P value: The degree of error that we are willing to accept. Often P = .05, which means we are willing to accept a 5% risk that the hypothesis is incorrect. Crossover: Patients assigned in one arm of the study (usually the control arm) can be reassigned to the other arm (usually the intervention group). Intention to treat: A technique used in randomized, controlled trials in which patient outcomes are compared within the group the patient was originally assigned to. This may not reflect the treatment that the patient actually received. If the patient is in a group that is treated but then leaves that group, they are still counted in the original group. She holds an academic appointment of assistant professor in the Department of Psychiatry at the University of Colorado School of Medicine. Dr. Borges’ primary line of research concerns the use of contextual behavioral interventions to help Veterans approach emotions like guilt and shame more flexibly. She is a principal investigator on a federally funded study focused on investigating the acceptability and feasibility of Acceptance and Commitment Therapy for Moral Injury (ACT-MI). She is also interested in using skills from Dialectical Behavior Therapy (e.g., the chain analysis skill) to help Veterans identify and intervene on their suicidal behavior. Beyond her focus on interventional research, Dr. Borges is developing and validating novel approaches to measuring facets of responding to guilt and shame. Dr. Jacob Farnsworth is currently a staff psychologist at the VA Eastern Colorado Health Care System. Jacob's primary research interest is in the area of military-related moral injury. His most recent efforts have focused on the further development of the construct’s definition, assessment approaches and comparing existing and novel intervention approaches for moral injury. He is a co-author of the Expressions of Moral Injury Scale-Military Version.

Lucy Mailing is an MD/PhD student at the University of Illinois. She recently completed her PhD in Nutritional Sciences and continues to perform research on the impact of diet and exercise on the gut microbiome in states of health and disease. She has authored several peer-reviewed journal articles related to the microbiome and health and was recently named an Emerging Leader in Nutritional Sciences by the American Society for Nutrition. Lucy has also been a staff research associate for the Kresser Institute for four years and writes about evidence-based gut health on her blog. She plans to begin medical school at the University of Illinois in 2020 after a year dedicated to writing and the launch of a gut-related startup. In this podcast, Lucy discusses the most promising trends and research in gut health. She talks about the best and worst ways to test for GI problems and the effects of exercise intensity and diet change on the gut microbiota. She also challenges the notion that ketogenic and high-fat diets are bad for the gut, and explains why your SIBO breath test results might be inaccurate. Educator, coach, and exercise physiologist Dr. Mike T. Nelson is back on the podcast today.
