Atrial fibrillation is the most common chronic arrhythmia worldwide and is associated with increasingly prevalent comorbidities, including advanced age, obesity, and hypertension. Atrial fibrillation is a frequent indication for hospitalization and a complicating factor during hospital stays for other conditions. Here, we discuss considerations for the acute management of atrial fibrillation, including indications for rate versus rhythm control strategies, treatment targets for these approaches, considerations including pharmacologic versus electrical cardioversion, and management in the post-operative setting. Pearls A key component to the management of acute atrial fibrillation involves addressing the underlying cause of the acute presentation. For example, if a patient presents with rapid atrial fibrillation and signs of infection, treatment of the underlying infection will help improve the elevated heart rate. Selecting a rate control versus rhythm control strategy in the acute setting involves considerations of comorbid conditions such as heart failure and competing risk factors such as critical illness that may favor one strategy over another. Recent data strongly supports the use of rhythm control in heart failure patients. Patients should be initiated on anticoagulation prior to pursuing a rhythm control strategy. There are several strategies for rate control medications with therapies including beta-blockers, non-dihydropyridine calcium channel blockers, and digoxin. The selection of which agent to use depends on additional comorbidities and the overall clinical assessment. For example, a patient with severely decompensated low-output heart failure may not tolerate a beta-blocker or calcium channel blocker in the acute phase due to hypotension risks but may benefit from the use of digoxin to provide rate control and some inotropic support. Thromboembolic prevention remains a cornerstone of atrial fibrillation management, and considerations must always be made in terms of the duration of atrial fibrillation, thromboembolic risk, and risks of anticoagulation. While postoperative atrial fibrillation is more common after cardiac surgeries, there is no major difference in management between patients who undergo cardiac versus non-cardiac procedures. Considerations involve whether the patient has a prior history of atrial fibrillation, surgery-specific bleeding risks related to anticoagulation, and monitoring in the post-operative period to assess for recurrence. Notes 1. Our first patient is a 65-year-old man with obesity, hypertension, obstructive sleep apnea, and pre-diabetes presenting for evaluation of worsening shortness of breath and palpitations. The patient has no known history of heart disease. Telemetry shows atrial fibrillation with ventricular rates elevated to 130-140 bpm. What would be the initial approach to addressing the acute management of atrial fibrillation in this patient? What are some of the primary considerations in the initial history and chart review? An important first step involves taking a careful history to understand the timing of symptom onset and potential underlying causes contributing to a patient's acute presentation with rapid atrial fibrillation.     Here is a real-world clinical case with a tricky differential: Our team recently readmitted a patient 6 days postpartum/post C-section (which was done for ICP and fetal macrosomia at close to 4500 grams, with A2GDM). She had elevated blood pressures, a frontal headache, some midepigastric pain/RUQ discomfort. Pretty clear picture right: sounds like preeclampsia (PreE) with severe features based on BP elevation and symptoms. So, we started her on mag-sulfate per protocol. Well, her transaminases were in the 400-600s, which was significantly higher than they were at delivery. They then peaked the next day at 900! OK, it still meets criteria for PreE with severe features. But could this also be postpartum Acute fatty Liver of Pregnancy (AFLP)? The clinical picture of these 2 conditions may overlap but there are distinct differences here. AFLP is potentially fatal, so we have to get that diagnosis correct. So being able to access that choice point is really critical in beginning to shift it.”“If our parents had narrowed windows, if they were coping with a lot of stress and trauma, or if they were absent, if they had mental illness or they were incarcerated, they aren't able to help us wire those things. "The team says, oh, we want to try to do things this way, and the org keeps coming back and saying stuff like, no, no, no, you can't do that, because in this org, we don't allow that." - Alex Sloley Alex shares his current challenge working with a 10-person pilot Scrum team within a 1,500-person organization that has never done Agile before. While the team appears open-minded and eager to embrace agile ways of working, the organization continuously creates impediments by dictating how the team must estimate, break down work, and operate. Management tells them "the right way" to do everything, from estimation techniques to role-based work assignments, even implementing RACI matrices that restrict who can do what type of work. Half the team has been with the organization for six months or less, making it comfortable to simply defer to authority and follow organizational rules. Through coaching conversation, Alex explores whether the team might be falling into learned helplessness or simply finding comfort in being told what to do—both positions that avoid accountability. His experimental approach includes designing retrospective questions to help the team reflect on what they believe they're empowered to do versus what management dictates, and potentially using delegation cards to facilitate conversations about decision-making authority. Alex's key insight is recognizing that teams may step back from empowerment either out of fear or comfort, and identifying which dynamic is at play requires careful, small experiments that create safe spaces for honest dialogue. Self-reflection Question: When your team defers to organizational authority, are they operating from learned helplessness, comfort in avoiding accountability, or genuine respect for hierarchy?     Tim Robinson of Blue Yonder talks about returns: the challenges & potential, role of AI, inventory management, tariffs, & the importance of customer experience.    IN THIS EPISODE WE DISCUSS:   [03.42] Tim's career; how he founded his business, Doddle, and the story behind the name; and how that journey led him to Blue Yonder. “It's quite rare that you isolate and identify supply chain as your chosen goal in life, but I fell into it and have loved it ever since.” [06.34] The current returns landscape, and what Blue Yonder's recent survey revealed about consumer-brand relationships, and how retailers approach returns. “Free and easy access returns, or buy as many as you like and return as many as you like – that philosophy has been one of the major drivers behind the success of ecommerce. How do you create the convenience and the relevance of physical shopping, where I can see a product, touch it, try it on?” [12.10] How tariffs and market turbulence are putting pressure on businesses, squeezing margins; and the importance of customer experience. “This is ultimately an age of uncertainty, if you're in a business that involves crossing borders for any reason. Whilst the challenge around tariffs may have emanated in one part of the world, the reality is that the ripple effect means the whole landscape is re-setting itself – it's a global phenomena now.” “The majority of consumers consider a poor return policy as a bad experience, whether they want to return or not. They may love the product, but now they have a poor sentiment of you and your brand... And if a bad policy is a bad experience, you've lost a customer. ” [20.46] Changing customer expectations and shopping habits, and the challenge of inventory management. “The pace of ever-changing demands is at the heart of everything.” [25.59] The role AI and machine learning can have in overcoming returns and inventory management challenges; and the importance of transparency in consumer communication. “You have complex inventory allocation decisions being made at the time a consumer hits the return button... We're able to orchestrate, influence consumer behavior, and make allocation and replenishment decisions in real-time.” “If we want to change consumer behavior, want them to think more carefully about shopping habits and the choices they make, we should be more transparent with them.” [29.52] The problem and opportunity of returns, and the impact businesses could make by considering returns as a source of extra inventory.
