But remember, your voice is unique and vital to the future of sci-fi. Sci-fi is evolving rapidly, and the genre needs fresh voices, perspectives, and ideas to keep expanding.9. Don't Fear ExperimentationSome of the best science fiction has come from writers willing to push boundaries and take risks. Explore odd structures, unusual timelines, or new perspectives. Try writing a story from an alien's viewpoint, or using second-person narration. Sci-fi readers tend to be open-minded and love seeing something they've never seen before. Experimentation is where the genre thrives, so feel free to test your limits.10. Remember That Sci-Fi Is Ultimately About HumanityAlthough sci-fi often involves aliens, robots, and distant planets, at its heart, it's about humanity—our hopes, fears, and endless drive to understand the universe and ourselves. Friday Reflection 27: The Poor Historian

BW was a 66-year-old woman who presented for an urgent visit to the general medicine clinic. She reported that she had been having dizziness for the last four days. When pressed, she said it occurred intermittently, being present more than absent. She could not identify any palliative or provocative features, and when asked about associated symptoms she said that she felt “bad and scared” when it was present. The doctor encouraged her, many times, to characterize the dizziness, and she could only say that when she had it, she felt dizzy.Sensible Medicine is a reader-supported publication. If you appreciate our work, consider becoming a free or paid subscriber.Many a medical trainee has been humiliated on morning rounds after proclaiming that their presentation was wanting because the “patient was a poor historian.” Any attending worth her white coat will respond in one of the following ways:“There are no poor historians, just poor history-takers.”“You do realize, don't you, that the patient is not the historian? You are the historian.”“Did you consider the differential diagnosis of why you were unable to obtain a useful history?”I admit that, going for pith over constructive criticism, I employ the former two more than the latter one. There is a differential diagnosis for the patient who cannot describe the history of their medical concerns.Often, the inability of a doctor to obtain a history is actually a physical exam finding – an extremely non-specific finding, but a finding nonetheless. Psychiatric disease, dementia, and delirium (whose differential diagnosis itself is practically a textbook of medicine) will render a patient unable to provide an accurate history. I can recall dozens of “poor historians'' who became Robert Caro-esque once their hypercapnea, uremia, or alcohol withdrawal was treated.There are three other reasons that obtaining reliable and informative histories might be a struggle.1.      We think with languageThe first — the saddest, most troublesome, and probably most common – reason that patients are unable to provide a reliable history is because of their impaired language skills. Not only do we use language to communicate, we also use language to think. George Orwell wrote, “…if thought corrupts language, language can also corrupt thought.”[i]Those of us who failed to master language, usually through inadequate education, are unable to express their health concerns clearly. Sometimes, listening to a patient try to describe symptoms, I get the sense that the problem is more than expressive.BW was not an especially striking example of this for me, she was just one of the more recent. She also presented with a problem for which an accurate history is critical. The history of a patient's dizziness radically alters the differential diagnosis. We teach trainees that the first question to pose to a patient with dizziness is, “What do you mean, dizzy?” We tell the trainee to ask the question and then sit back and listen. More often than not, patients will describe their dizziness in a way that can be interpreted as lightheadedness, vertigo, unsteadiness, or a non-specific feeling of being unwell.[ii]As I interviewed BW, my sense was that she not only struggled to articulate what she was feeling but to figure it out herself. Beyond my frustration in having trouble caring for her, I considered the lifelong impact of leaving people educationally impoverished. Sure, we limit people's earning potential, but we also limit their internal life and their healthcare.2.      Anxiety affects how we experience symptomsVG is a patient I have seen for years, from his mid 30's to his early 60's. He has a few very mild chronic medical conditions and very severe anxiety disorder. He is a successful professional but struggles with intermittent episodes of health-related anxiety.Our interactions usually begin with an email or a phone call. VG will have become anxious about a new symptom. The symptom is real – joint pain, a rash, a new floater. The symptom has generated a web of worry about what it could portend. The worries are always baseless and would be amusing if they were not so clearly painful and disturbing to him.I have learned that for VG, as well as for many patients like him, these concerns require a visit. An accurate history cannot be obtained without seeing him. The anxiety clouds VG's experience of the symptom. The knee pain becomes excruciating, the rash ubiquitous, the floater debilitating. The response to simple questions -- Is the knee pain worse coming down the stairs? Where is the rash? Do you see the floater in one or both eyes? -- become unreliable. Seeing VG, providing some reassurance, and obtaining objective physical data is imperative.3.      Secondary gain“Listen to your patient; he is telling you the diagnosis” is is an Osler (or merely Oslerian) quotation. From the earliest days of our training we are taught to listen to our patients and believe what they are telling us. Our greatest sin in history taking is that we interrupt too soon and too often. We ask closed-ended and leading questions. When the answer to a question doesn't align with our hypothesis, we either ignore the response or doubt its veracity.However, patients are people and people lie. Patients exaggerate the symptoms of their sinus infection to get an antibiotic prescription. Patients tell you that their oxycodone or Ambien fell in the sink or toilet so they can get an early refill.[iii] I have had numerous people appear on my schedule with the chief concern of “chest pain” documented by the nurse. When I ask, “Tell me about your chest pain?” the patient responds, sheepishly, “I'm not actually here for chest pain but I really needed to see you and I knew that would get me in.”Some of these untruths are the proverbial chickens coming home to roost. Patients exaggerate symptoms because they fear they will not otherwise be taken seriously. They fear mild but troublesome or worrisome symptoms will be met with:“Let's just watch that.”“That doesn't really sound like it warrants treatment.”“If it hurts to bend your arm, don't bend your arm.”Most everyone learns at some point the factoid that 80% of diagnoses come from the medical history. However, contrary to a standardized test on which the medical history is presented in a paragraph of clean prose, it requires skill to obtain a medical history. A novice will take longer than an expert to extract a less accurate history. From some patients, however, not even an experienced physician can obtain a cogent medical history. This failure may be a clue to an underlying diagnosis. It may also be related to a person's education, their health-related anxiety, or their effort, conscious or unconscious, to influence the doctor. [i] Orwell obviously took this idea to in a frightening direction in 1984 describing a government that uses the control on language to control thought.[ii] We often jokingly describe this last type of dizzy as “dizzy dammit” – as in “I am just dizzy, dammit.”[iii] It is interesting that people only drop opiates, benzodiazepines, and stimulants in the sink. Nobody has every dropped an SSRI, statin, or antibiotic. This is a public episode. How to Tell If A "Weight Bias" Training Is Diet Culture in Disguise

If you appreciate the content here, please consider supporting the newsletter by subscribing and/or sharing!As the diet industry works hard to co-opt the language of weight-neutral health and fat liberation and misuse it to sell weight loss interventions, I'm seeing more and more “anti-weight bias” trainings that are actually just diet industry marketing in disguise.Sometimes the trainers are very aware of what they are doing, sometimes they are actually well-meaning but simply duped by diet culture. Regardless, this is especially dangerous since the attendees leave thinking that they've learned how to reduce or eliminate weight stigma, when in fact what they've learned is how to be uncompensated, unwitting marketers for the diet industry, increasing weight stigma in the process.Here are some common red flagsThe trainer is one or more of the following:* Involved in “ob*sity medicine” * Paid to sell/prescribe/provide weight loss interventions* Taking payments from the weight loss industry* Represents an astroturf organization like the Ob*sity Action Coalition, Ob*sity Society etc.Taking the position that “I don't want to stigmatize fat people, but I want to dedicate my career to eradicating them and making sure that no more ever exist” is not an anti-weight stigma stance.You cannot be invested (ideologically or monetarily) in anti-fatness (aka anti-ob*sity) and be anti-weight stigma, they are mutually exclusive positions.The training uses pathologizing/person first languageThe words “ob*se” and “overw*ight” were literally made up to pathologize and medicalize higher-weight bodies. “Overw*ight” is inherently shaming (as it indicates that a body is “over” whatever is being considered a “correct” weight,) and “ob*se” comes from a Latin word that means “to eat until fat,” so much more stereotype than science there. Person-first language (saying person with ob*sity, person affected by ob*sity, person with overw*ight etc.) does NOT come from weight-neutral health community or fat liberation community. It was co-opted from disability community (where it is controversial) by the weight loss industry in the service of their goal of declaring that simply existing in a higher-weight body (regardless of any measure or concept of health) is a “chronic lifelong health condition” (that requires their profit-driving interventions.) This is not about reducing stigma, it's about increasing the bottom line of the weight loss industry.The training suggests that weight loss is a solution for weight stigmaIf they list bullying, lack of accommodation, or other types of weight stigma as a reason that people need access to weight loss interventions (including and especially drugs and surgeries) then they are inciting bias, not reducing it. While weight stigma is real and does real harm, and fat people are allowed to make whatever choices they want in dealing with it, in an anti-bias training it is wildly inappropriate to teach that oppressed people should have to change themselves (including risking their lives and quality of life with dangerous and/or expensive weight loss interventions,) to escape oppression. Teaching that oppressed people should change themselves to suit their oppressors is not an anti-stigma position. Even if someone believes that fat people are less healthy, healthism does not justify weight stigma.If the curriculum is not focused on creating a world that fully affirms and accommodates fat people, then it's likely diet industry propaganda.They suggest that the “real” injustice is a lack of access to weight loss interventionsI'm seeing this more and more from people who work for/take payments from the weight loss industry. They try to claim that the true injustice and stigma is that some people don't have access to their dangerous and expensive interventions. This has, actually, nothing to do with reducing weight stigma and, instead, is part of the weight loss industry's long game to get their procedures covered by insurance, which will vastly increase profits.Playing the Rename GameThere is definitely a place in anti-bias training for discussing language. However, if, instead of working to dismantle stigmatizing diet culture concepts, they are just renaming them (ie: instead of “willpower” use “commitment,” instead of “ideal weight” use “goal weight” etc.) then they are just repackaging diet culture.Reducing bias isn't about using different words for the same harmful concepts and practices, it's about dismantling the biased paradigm and using words that create a new paradigm (instead of “ob*se” use fat/higher-weight, stop conceptualizing weight as ideal, healthy, a goal etc.)They claim that the problem is that there isn't enough anti-fat educationIf they are claiming that healthcare practitioners and others should receive more education about pathologizing fat people and prescribing/recommending weight loss interventions, this is a weight loss marketing seminar, not an anti-bias training.  NovoNordisk is involvedHaving made (and continuing to make) a literal fortune from price gouging on insulin, NovoNordisk is bringing this same attitude (and the Perdue Pharma Oxycontin marketing playbook) to their weight loss drug Wegovy, which they promised their shareholders would be a massive profit-driver. I wrote more about this here. You can also check out this episode of the excellent podcast Untrapped during which I got to talk about this with the brilliant Louise Adams and Fiona Willer! Their message is “don't blame fat people, but do focus on eradicating them”If they are only admitting the fact that weight isn't a simple matter of calories in/calories out so that they can use that as a reason to promote diet drugs and weight loss surgeries then, again, you are not at an anti-stigma training, you are at a marketing seminar for the diet industry. (Even worse if they are acting like they just discovered this fact, when true anti-weight stigma and fat liberation activists have been screaming it from rooftops for literal decades.) Bottom Line:A true anti-weight bias/anti-weight stigma training de-pathologizes fatness and promotes a world that fully affirms and accommodates fat people. While there may be harm reduction models that fall short of this, if any of the red flags above are present, it tell me that I should do even more digging about who is creating/funding/providing this training, and who is profiting from the ideas presented. For many of us, the end of the year can bring up a lot of emotions and stress. When we're in fight or flight mode, the mind cannot see possibility–it only sees catastrophe. Most people don't give themselves sufficient space for all of the things on their to-do list. Between travel plans, shopping lists, and in my case, setting up our condo in Miami for Thanksgiving, it's safe to say that I was feeling overwhelmed. I noticed that my mind was racing and my body was giving me all sorts of signs to slow down. But instead of leaning into that stress, I found my island of respite. There's real power in cushioning between events and responsibilities. Even a little bit of space provides the opportunity to reset and refocus on what's actually important during the holidays. So, I gave myself permission to rearrange plans and focus on the details that were in my control. And guess what? Things are turning out exactly as they were meant to. The universe has a funny way of doing that. In this episode we check out the Fog of War variant of Classic, talk about the role of Mods and what to bring to their attention and conclude with new development server talk that may get variants up and running again for the Dip community!  Apologies up front on how the ambiance gets louder and louder as more people arrive in the pub! If this pisses you off too much a) go have a drink or two yourself or b) let us know to go somewhere quieter! How do Moderators (Mods) work? Both Kaner and Amby have been Mods in the past for vDip, they discuss their prior experience and how it varies between vDip and webDip:vDiplomacy - link is in the top right. You can only see posts you have raised with the ModsIt's the place to bring up concerns about multi-accounting with fake accounts or suspected meta-gaming. Amby discusses how he suspected this occurred in a game and how he raised this with the ModsKaner discusses contacting the Mods where there are technical problems, where a player feels another player has crossed the line with their languageThe guys also digress blocking players, and why you'd do this - if they were obnoxious, or a player seems to CD too often, doesn't communicate well. You can block players by going into their profile page and clicking the smiley face, turning it into an unhappy face (the same functionality doesn't exist in webDip, only vDip)webDiplomacy - you contact the Mods differently.  Go to Help and then email the mods. With a larger number of members on webDip, the moderators there tend to respond fasterKaner talks about how webDip is starting to add new variants before Amby shoots him down. webDiplomacy focuses far more on Classic games with around 80-90% of all games played on the server being the standard classic game.Variant focus - Fog of War We then turn our attention to the Fog of War variant on vDiplomacy:In this variant you can only see the territories immediately adjacent to your supply centres and your units. The show notes include an animated GIF showing how things would look as a game evolves as TurkeyAs you can't see where all the other players are, and what they're doing (only the ones immediately around you) it presents a significant challenge.This can be complicated further with a Gunboat game (no-press). Kaner talks about the subtle ways you can still send out subtle messages about working together and highlights the importance of checking the large map rather the standard small map. The supports don't render 100% correctly on the small map, but they do on the large map.The guys also talk about how in standard press games how you can lie and manipulate players with information - right or wrong - about areas of the map others can't see.Kaner talks about two recent Fog-of-War games he's recently played in - Cold Blooded Fog and Pompey . Kaner talks about how his loss as Germany was more a result of absolutely terrible game play rather than the map's fog-of-war element. But as Russia he had greater success getting a draw despite challenges of:subtle overtures to Germany being ignored (no checking of the large map again!)"partnering" with Turkey by sacrificing his southern front... whilst Turkey ignored this and kept eating him upefforts to make this message stronger by attacking Italy... only for Turkey to keep ignoring it!eventually getting French support with a Barents fleet support of St Petersburg to create a stalemate line, forcing Turkey to draw despite only being one supply centre short of a win - and Kaner only having 4 SC's himselfIn addition to playing Fog of War in classic, we talk about how it can also be played in the variants 1066 and Rat Wars (and why the technology behind coding Fog maps tends to limit the number of variants that use it)New development server talk Finally we talk about variant development and news from CaptainMeme (vDip mod and regular on webDip) that while the vDip Lab is pretty much dead, a new test server for development is being slowly created in the background. This is some really cool news that could see new variants created again that won't crash vDip. The guys talk about how this could impact on new variants, including suggestions on webDip on air support, Classic with neutral armies, Kaner's Mongolian variant and Amby's ideas on making a massive Cold War variant. Found this interesting montage while searching for something else. It's a show we did from Hong Kong looking ahead to how the Special Administrative Region would change when HK was handed back to the Chinese in July 1997. It's interesting as is currently chairman of the BBC Trust. That quote at the start of the programme is rather appropriate in the UK at the moment. Between now and July 1st 1997, an estimated 8000 journalists will be passing through Hong Kong examining basically the same story. In January 1841, China and Britain signed a Convention which ceded Hong Kong island to Britain, a year later Kowloon was ceded too and in 1898 the land north of the Kowloon peninsula was leased by the British from the Chinese. Now that lease is coming to an end. As sovereignty of the whole of this area changes from British to Chinese, what will happen to life in Hong Kong as it becomes a special administrative territory. Around 2000 Dutch speaking families are part of the international community living here, most of them working in the banking or electronics sector.  We've been talking to them as well as to Chinese and English speakers to find out what they think will happen. The answer is the same. China has pledged to preserve Hong Kong's capitalist economy, currency and freemarket policies until the year 2047. But with economy booming in the Peoples Republic of China, home to 1.2 billion people, its no wonder some doubt whether the government in Beijing really needs or cares about the long term fate of 6.3 million packed into the tiny space called Hong Kong. And since the public media is government controlled, many feel than any changes will first be heard over the airwaves. Hong Kong's governor, Chris Patten, stressed this during the recent meeting of the Asia-Pacific Broadcasting Union. Radio TV Hong Kong puts out a continuous relay of BBC WS in English on 675 kHz.The majority speak of the residents speak Cantonese. Next year, though RTHK will start programmes in a new languageThe name Hong Kong comes from the Cantonese which means "fragrant harbour". Its not so fragrant these days, more rather choppy as hundreds of boats criss-cross across Victoria Harbour between the island of Hong Kong and the mainland tip of Kowloon. The Star Ferry charges 2 Hong Kong Dollars to cross from one side to the other. That means the upperdeck trip costs a mere 28 US cents. It's one of the cheapest rides in the world. It also allows you to escape for a few minutes from the continuous traffic and watch the high-rise skyline. The travel brochures encourage you to shop till you drop and the shops in both Kowloon and Hong Kong island stay open until 10 at night. Shop windows bulge with famous name fashions, electronics and photo goods. Tourists from Europe comment that Hong Kong's relatively expensive. But depends what you're looking for. Shifty looking characters on many street corners near Nathan Road near Tsim Sha Tsui hiss at foreigners in loud whispers. Copy watches, fake clothing? And if you're willing to disappear up three flights of stairs into a dimly-lit backroom, you too can bargain for a fully functional Rolex-looking watch for less than 80 dollars. Its backed by a lifetime money-back guarantee. If you value your life, you won't try and claim your money back. Police in Kowloon at least at currently cracking down on the counterfeits.But if you take a few stops north on the super efficient underground railway, the MTR, get off at Sham Shui Po. The high rise buildings are the same. But the shop and street signs are only in Cantonese and there are fewer Western faces. Passengers splurge from the high-speed escalators onto the pavement. Across the road, the Golden Arcade has a scruffy sign about the Internet which is flapping in the upward breeze generated by the humid air and exhaust fumes. Inside are three vast floors of computer and video hardware. A sign strongly discourages you to take photographs. Once your eyes adjust to the off-white fluorescent bulbs, you find stand after stand of software. It looks genuine. But its a copy. Often the CD-ROM covers are empty. If you decide to purchase just about all the Internet publishing software available on one CD-ROM, expect to pay 7 dollars, to watch the stall holder call someone on a mobile phone and 20 minutes later someone else taps you on the shoulder and shoves a wafer thin plastic bag at you. Inside, there's a shiny CD with the software you wanted. Further inside, someone is selling software manuals. A sign claims they're original. A man standing in a tiny shop front tries to tempt us to step inside and buy a multi-media PC. It looks like it might work. But is what you see anything like what you get? For there's such a fine line between fake and genuine. Even with bargaining, the real hardware is still expensive by most standards. The illegal software of course costs a few cents to mass produce. Most of it is coming from factories across the border in China. Will the tide really stop in 1997? Making money seems to be more important that authors rights. The packaging often turns out to be more impressive than the contents. The video CD of the latest Walt Disney block buster is the result of someone taking a consumer video into a cinema and filming the screen from his backrow seat. That maybe the hunchback of notre dame shuffling across the TV screen. Its difficult to tell after a videos been copied so many time. And the soundtrack is muffled by the sound of someone munching popcorn and giggling in Cantonese. Short-wave portables from Philips, Grundig and Sangean are easy to find in Hong Kong. Shop salesmen know what they're talking about too. Three hours drive from Hong Kong into Ghanghou province of China, you'll find the joint venture factories making many of the budget portables. China may have diplomatic problems with Taiwan. But more than 10,000 Taiwanese are running factories in Southern China and exporting the products out of the peoples republic. They'd like to exploit the vast domestic Chinese market too. But that's more difficult and means finding more influential friends.
