In this episode, we detail the bidirectional relationship between the HPA axis and the intestinal barrier, illustrating how HPA axis dysfunction and cortisol dysregulation can impact intestinal permeability. We further discuss how intestinal dysbiosis can contribute to HPA axis overstimulation. Lastly, we detail some of the roles of short-chain fatty acids (SCFAs) and secretory IgA (sIgA) in this bidirectional relationship.Topics: 1. Overview of the HPA Axis and Gut Bidirectional Relationship- Chronic HPA activation, intestinal permeability, mucosalimmunity, and microbiome composition.   2. Components and Function of the HPA Axis- Hypothalamus, pituitary gland, and adrenal glands.  - Stress signals from the amygdala and prefrontal cortex, CRH release from the hypothalamus.  - CRH stimulates ACTH release from the pituitary.- Cortisol production and release. 3. Structure of the Intestinal Epithelial Barrier- The gut barrier consists of the intestinal lumen, microbiome, mucus layer, epithelial cells, and lamina propria.  - Specialized epithelial cells: goblet cells, enterocytes, enteroendocrine cells, and more.- The lamina propria contains immune cells, blood vessels, and lymphatics, supported by a smooth muscle layer.   4. Intestinal Permeability and Tight Junction Regulation- Transport across the intestinal epithelium: transcellular and paracellular pathways.  - Nutrient absorption and selective permeability.  - Tight junction proteins, such as occludin and claudins.   5. Cortisol's Impact on Gut Barrier Integrity- Cortisol can cross the intestinal microvascular endothelium and enter the lamina propria. - Chronic cortisol exposure and intestinal barrier function.- Increased permeability allows luminal antigens and bacterial endotoxins (e.g., LPS) to infiltrate the lamina propria.   6. Secretory IgA (sIgA) and Gut Immune Function  - Chronic cortisol exposure can reduce sIgA levels, weakening mucosal immunity.  - sIgA neutralizes pathogens, prevents microbial adhesion, and more.  - Lower sIgA levels increase susceptibility to dysbiosis and infections.   7. CRH and Its Role in Intestinal Permeability- CRH is produced in the hypothalamus and also peripherally.- CRH can stimulate mast cells, triggering histamine and inflammatory mediator release.  - Mast cell activation can increase gut permeability. 8. Microbiome's Influence on HPA Axis Regulation- Dysbiosis can disrupt HPA axis function.- IBS and sustained HPA activation.- Dysbiosis reduces SCFA production.  - SCFAs, particularly butyrate, support colonocyte health, tight junction integrity, and anti-inflammatory pathways.  - Inflammation and HPA axis dysfunction. 9. Check out today's questions:    Christine: Dear Dr. Cabral, Thank you! As my interest in holistic health has grown, I have listened to many podcasts and yours has moved into my favorite spot. You give valuable information and I trust your intentions and integrity. Very inspiring! I did your stress and hormones test, and was able to talk to an IHP. One thing that stood out is that my cortisol levels are high and I should likely avoid intermittent fasting as it is a stress on my body. I am almost 54 years old and still in perimenopause. However, there is a lot of family history of cancer and other disease, I don't want to miss out on the benefits of autophagy. Is it possible to fast and counteract that stress with supplements such as ashwaganda or anything else?   Christine: Dr. Cabral, this is my second question submitted today so I understand if you can't answer it. I wondered if you've ever been able to help people with a phobia to swallowing pills. I am embarrassed to say that I am 54 years old and still need to have food to swallow my vitamins. Imagine my horror when I went to the oral surgeon iand before the procedure could be done, they handed me a huge pill to swallow. They all searched in their lunch.bags for crackers or something, and in the end, I just chewed the pill (yuck) and swallowed it. So many people have tried to help and love to show off when they can swallow a handful of pills at once. I just can't get over this phobia.   Diane: My husband and I are 47 years old. We workout 7 days a week. Everyday we run 2-4 miles, then do a 15 minute rowing interval workout, then do strength training, and then go in the sauna for 20 minutes. Although at times we feel like this intense exercise may be doing more harm than good, we also don't want to short change ourselves and not do everything possible to stay fit and in shape. Are we being less effective by doing as much as we are doing?   Simon: Hi Dr Cabral, I heard recently on a bodybuilding podcast that heavy weightlifting sessions can result in systemic inflammation throughout the body and increased intestinal permeability, and therefore anything you ingest at this time might make its way into the bloodstream. The conclusion was that anything you regularly consume during or immediately after a heavy workout might result in the body forming intolerances to that substance. Is there any validity in these claims? I'm conscious that the things I ingest around workouts (glutamine, fruit, WPI) are staple supplements and foods for me, and I don't want to form intolerance! PS I suffer from intestinal permeability and IBS so this is a particular concern for me. Allmennlege Sjur Even Aunmo fikk sparken som fastlegevikar fordi han fortalte pasienter at COVID-19-vaksinen kan gi bivirkninger. Dette er noe han plikter å gjøre ifølge Pasient- og brukerrettighetsloven § 4. Aunmo har satt seg grundig inn i forskningen på disse vaksinene, og påpeker at de for det første er eksperimentelle på dette stadiet. Videre er det Pfizer selv som har finansiert sin studie, en ansatt fikk sparken fordi hun varslet om at blindingen (anonymiseringen) var brutt, personer har blitt tatt ut av studien uten at det er oppgitt noen god grunn og rådataene for studien er ikke offentliggjort. Selv har han sett flere bivirkninger, som kraftige menstruasjonsblødninger, hjerteproblemer og alvorlige nevrologiske bivirkninger. Han frykter at underrapportering kan medføre at bivirkningstallene fra Statens legemiddelverk er for lave. I bring this up because I recently had Eczema pop-up on my middle finger of all places so at least it was fun showing my family and friends, and neighbors and passerby's. Skin problems are not just about your skin. Skin products work but you don't want to become dependent on them in order to keep your appearance. So, where should you start?The first thing to know is that your body uses the skin to excrete toxins or waste. That simple fundamental thing is overlooked by nearly everyone out there, they aren't wrong or bad people but that is just the way they solve problems, by looking at the symptoms & not why they're presenting themselves The Second thing is look into environmental triggers, epigenetics. After all something changed recently otherwise you would always have the exact same skin issue in the exact same place from the moment you were born. And even then you should take the next step and that is to take a look at your gut… not literally but with testing. Most skin issues are linked to some kind of gut issue like an overgrowth of candida or some form of leaky gut or Intestinal Permeability. When it comes to human health, the intestinal barrier is of critical importance. An intact intestinal barrier facilitates absorption of nutrients and prevents the entry of toxins and pathogens into the human body, whereas an impaired intestinal barrier allows their entry, which may trigger local or systemic inflammation and disease.Increased intestinal permeability is a new target for disease prevention and therapy. Intestinal permeability involves the loss of integrity between the epithelial cells of the small intestine. It has been linked to a wide range of chronic inflammatory and autoimmune diseases, including inflammatory bowel disease (IBD), irritable bowel syndrome (IBS), non-alcoholic fatty liver disease, type 1 and type 2 diabetes, and depression.Currently, there are no pharmacological treatments for intestinal permeability; however, accumulating evidence supports are a range of dietary and non-pharmacological interventions, including fibre, vitamin D, zinc, and probiotics. In this episode, Naturopath and Nutritionist Brooke Schiller provides an update on intestinal permeability, including underlying causes, clinical testing methods, and therapeutic interventions for improving and maintaining intestinal barrier integrity.

Of all the health issues our bodies go through every day, the strangest sounding one has to be this condition called leaky gut. If you’ve been in the health space for a while, you may have heard or read about it, but never truly had a deep-dive on it. Then there’s also this other term called Intestinal Permeability, which at least sounds more professional than leaky gut. However, are they the same? What’s the difference between Intestinal Permeability and leaky gut syndrome? Today, we discuss the condition known as leaky gut. We explain what it means to have a leaky gut, what causes it, and some of its immediate symptoms. We share recent findings on leaky gut syndrome and describe why having intestinal permeability is not the same as having a leaky gut. Re-address Blog Comment from Jon from Podcast 57: I think a discussion on what the re-introduction of foods and the subsequent reactions to them means and why would be very interesting as a podcast topic. The paleo concept has expanded a lot from the original ‘cavemen did this so you should to’ logic of guys like cordain (another thing mentioned recently on the show, how robb has become more science orientated because of matt lalonde), but I still think at times robb reverts back to the ‘re- introduce it and see how you feel’ logical fallacy too often, because I bet almost any food completely eliminated for 30 days and then re-introduced would have negative effects and may take a few months to re-sensitive yourself to, so a talk on why this matters (scientifically preferable rather than anecdotally) would be important. It needs to be more than just ‘gluten makes my joints achy’. I’m very surprised that some of your clients eat gluten, robb seems to think 100% of people should avoid it 100% of the time. Another thing is the constant reference to autoimmunity. I get that anecdotally people with these problems see benefits from paleo, but what relevance is this to people without autoimmune conditions? If dairy aggravates autoimmune stuff, what does this mean? Sometimes it seems like robb is implying that because people with (chrohns, rheumatoid arthritis, coeliac, hashimoto etc) get messed up eating a certain food that the food is bad for everyone, but they are a special population, and that’s like comparing the carb needs of a type 2 diabetic to that of an athlete. Again, i recognise this is a problem of the very broad audience of the podcast, but I think some more clarity on the real underlying WHYs need to be answered. For example the ‘these foods have only been around for 10000 years’ line, this is assuming a linear evolution, which many experts suggest is not the case, so maybe Robb could discuss why we did or did not evolve more quickly after the invention of agriculture (http://the10000yearexplosion.com/). 2. Could you please explain the difference between physiological insulin resistance and pathological insulin resistance. I think this is something that gets very mixed up not only in the mainstream, but in the low-carb/paleo community. 2. Cod Liver Oil- WAPF says yes, Cordain says no, who do the lay people believe? Both have very good researcher on both sides. Could it be the problem lies with Cod Liver Oils that have been striped of natural A & D and replaces with the synthetic variety? 3. PWO carbohydrate- I know you wrote a stellar piece sometime ago on Low-carb and CrossFit. How has your viewpoint evolved from that experiment, and over time? 3. I have really been working at being a Paleo health person. My only problem is I have Hashimoto disease. I work out five days a week. I watch what I eat. Perhaps more of a 85% paleo. What can I do to speed things along. I have been doing Paleo since May 2010 I have lost about 15 pounds, very, very slowing. Lots of tweaking with my thyroid meds and I continually tweak my food. Seriously. So that 85% paleo… it needs to be 100% gluten free at a minimum! Datis Kharrazian talks a lot about this topic (the gluten-autoimmune thyroid connection) and I’ll be hearing him speak more about it next month here in the Bay Area. I’ll report back if there’s more.
 julianne says: December 19, 2010 at 9:07 pm Totally agree with Diane – I have Hashimotos. Get Dr K’s book and read it – it is useful, be super strict gluten free. Go 100% paleo. My anti-bodies are dropping since gluten free. Don’t go too low carb though, I’ve found so keep in a little good fruit like berries and good starches like sweet potato. Do the anti-autoimmune protocol in Robbs book, see if that helps. (Dairy and nightshade free). Make sure your vit D is around 45 – 50. Take omega 3. Be careful with iodine. It can cause a flare up.

Ben says: December 17, 2010 at 5:11 pm Short and sweet: what type of eating would he recommend to a lean 31 year old, strict paleo for 1.5 years, 164 lbs at 5 ft 11 inches who is looking to get body weight up to aprox 180-190 pounds on Ripp’s Starting Strength program. I am only 1.5 months in and slowly gaining weight, and progressing on Ripp’s linear path, while maintain strict paleo; i only ask if Lalonde thinks there is another way of eating that would be more beneficial given my goals. If not, cool ill keep up strict paleo – it is after all very tasty and effective. I just want to know if he thinks this is the best path. Thanks.
